Abstract:
Coronavirus disease 2019 (COVID‐19) has now spread all over the world. The National Health Commission of the People's Republic of China reported 78,439 cured and discharged cases, 4634 deaths, 83,462 confirmed cases and 760,818 close contacts as of June 25, 2020. Joint detection of nucleic acids and antibodies has become an important laboratory diagnostic for COVID‐19 patients. Disease progression and infection stage can be established based on the biological characteristics of these tests. However, there have been few studies of the different infection stages of COVID‐19. We conducted a retrospective analysis to explore the clinical characteristics of COVID‐19 patients at different infection stages and to characterize the characteristics of specific serum antibodies at each stage. These data will provide a theoretical basis for clinical diagnosis and treatment.

This article is protected by copyright. All rights reserved.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is to explore the application value of serum antibody detection in different stages of COVID - 19 infection. Specifically, by retrospectively analyzing the clinical characteristics of COVID - 19 patients, the author studied the characteristics of specific serum antibodies in different infection stages, aiming to provide a theoretical basis for clinical diagnosis and treatment. ### Background of the Paper - **Problem Background**: The COVID - 19 epidemic has spread rapidly around the world, and nucleic acid testing has become an important means of diagnosis. However, nucleic acid testing has the problem of false - negative results, which poses challenges to clinical diagnosis and treatment. Combining nucleic acid testing and serum antibody testing can improve the accuracy of diagnosis and help identify the specific stage of infection. - **Research Purpose**: To explore the application value in clinical diagnosis and treatment by analyzing the serum antibody levels in different infection stages. ### Research Methods - **Research Subjects**: 723 adult COVID - 19 patients diagnosed in Wuhan Taikang Tongji Hospital from February 13, 2020 to March 30, 2020. - **Detection Methods**: - **Nucleic Acid Detection**: Use real - time PCR technology to detect SARS - CoV - 2 nucleic acid in nasopharyngeal swab samples. - **Serum Antibody Detection**: Use a magnetic particle chemiluminescence immunoassay analyzer to quantitatively detect IgM and IgG antibodies in serum. - **Statistical Methods**: Use SPSS 25.0 for statistical analysis, including chi - square test, t - test, analysis of variance and Mann - Whitney U test, etc. ### Main Results - **Basic Patient Information**: A total of 723 patients were included, with an average age of 61.30 ± 14.55 years, and the majority were female. - **Classification of Infection Stages**: According to the results of nucleic acid testing and serum antibody testing, patients were divided into early stage, active stage, middle - late stage and recovery stage. - **Clinical Characteristics in Different Infection Stages**: - **Early Stage**: Mainly manifested as multiple small shadows and interstitial changes. - **Active Stage**: Multiple ground - glass opacities in the lungs with unclear boundaries, and some patients had different degrees of pulmonary fibrosis. - **Middle - Late Stage**: Exudation in both lungs or a single lung decreased, and the changes in the absorption stage were obvious. - **Recovery Stage**: The performance in the lung inflammation absorption stage was obvious, and the CT results of some patients were normal or had only a small number of scattered shadows. - **Serum Antibody Levels**: - **IgM Antibody**: The level was higher in the active stage and further increased in the recovery stage. - **IgG Antibody**: The level was low in the early stage and remained high after the active stage. - **Relationship with Disease Severity**: The IgG antibody levels in severe and critically ill patients were significantly higher than those in non - severe patients. ### Discussion - **Diagnostic Significance**: Combining nucleic acid testing and serum antibody testing can effectively improve the accuracy of diagnosis and reduce the impact of false - negative results. - **Antibody - Dependent Enhancement (ADE)**: The increase in IgM antibody levels in the active stage may be related to the ADE effect, that is, non - neutralizing antibodies promote the entry of viruses into cells, leading to aggravated conditions. - **Clinical Significance of IgG Antibody**: The high levels of IgG antibody in the active stage and middle - late stage indicate disease progression and are related to the severity of the disease. ### Conclusion This study provides an important reference basis for the clinical diagnosis and treatment of COVID - 19 by analyzing the serum antibody levels in different infection stages. Combining nucleic acid testing and serum antibody testing can more accurately judge the infection stage, so as to take effective treatment measures.
