Abstract:The impact of vitamin B2 (riboflavin) intake on cognitive performance among older adults in the United States (US) remains inadequately understood. This study aimed to explore the association between vitamin B2 intake and cognitive performance among non-institutionalized elderly people in the US. Weighted logistic regression was used to evaluate the association between vitamin B2 intake and cognitive performance. Vitamin B2 intake was determined from the mean of two 24-hour dietary recall interviews. Three cognitive ability assessment tests, namely the Immediate Recall Test (IRT), Animal Fluency Test (AFT), and Digit Symbol Substitution Test (DSST), were performed. Participants included all older adults over 60 who underwent cognitive scoring, with cut-offs defined based on the lowest quartile (25th percentile) for each test (the cut-offs for the three scores were 15.625, 12, and 33, respectively). Sensitivity analysis, including dose-response curves, subgroup analyses, interaction effects, per 1 standard deviation (SD), recommended dietary allowance (RDA), and residual energy model analysis, were performed to solidify the solid association between vitamin B2 and cognitive performance. A total of 2893 individuals aged over 60 were included, with a mean age of 69 (7) years, and 46% were men. There was a significant association between vitamin B2 intake and all three cognitive scores (IRT, Odds Ratio = 0.77, 95% confidence interval: [0.65,0.92]; AFT, 0.75, [0.64,0.88]; DSST, 0.72, [0.59,0.88]). Moreover, vitamin B2 intake above the RDA reduced the risk of low cognitive performance (IRT, 0.66, [0.46,0.93]; AFT, 0.83, [0.62,1.11]; DSST, 0.65, [0.45,0.92]) compared to intake below the RDA. Dose-response curves indicated that higher vitamin B2 intake was negatively associated with the risk of low cognitive performance. Physical activity may modify the association between vitamin B2 and cognitive performance. Vitamin B2 intake was positively associated with cognitive performance among older adults. Adequate vitamin B2 intake could help protect cognitive function.

Associations of Vegetable and Fruit Intake with Cognitive Function and Its Decline: Two Longitudinal Studies

Abstract:Objectives: Previous studies suggested protective associations of vegetables and fruits (VF) intake with cognitive function, but evidence on specific types of VF was insufficient. Methods: The current study included 4066 participants from 1997 to 2006 in the China Health and Nutrition Survey (CHNS) and 6170 participants from 2013 to 2020 in the Health and Retirement Study (HRS). Dietary intake (using 3day 24-h dietary recalls in CHNS and food frequency questionnaire in HRS) and cognitive function (using the Telephone Interview for Cognitive Status-Modified, TICS-m) were measured. Linear mixed-effects models were used to estimate the beta coefficients ((3) and the 95% confidence intervals (CI) to evaluate the association of VF with cognitive function (z-score) and its decline. Results: Highest intake of total VF was associated with better cognitive function and slower cognitive decline. Differences in cognitive function z-score between the highest and lowest tertiles of VF consumption were 0.039 (95% CI: 0.002, 0.076) for CHNS and 0.063 (95% CI: 0.026, 0.100) for HRS. The corresponding differences in annual cognitive decline were 0.011 (95% CI: 0.002, 0.021) and 0.012 (95% CI: 0.003, 0.020) units respectively. Vegetables and fruits showed independent associations with cognitive function and its decline. In specific VF subgroups, when comparing the highest to the lowest tertile intake, cruciferous vegetables ((3 = 0.058, 95% CI: 0.017, 0.100 in CHNS and (3 = 0.067, 95% CI: 0.032, 0.101 in HRS) and green leafy vegetables ((3 = 0.036, 95% CI: -0.001, 0.073 in CHNS and (3 = 0.082, 95% CI: 0.046, 0.117 in HRS) was associated with better cognitive function in both cohorts. Similarly, higher intake of dark-colored vegetables ((3 = 0.019, 95% CI: 0.008, 0.030 for red/yellow vegetables in CHNS and (3 = 0.004, 95% CI: 0.001, 0.007 for green leafy vegetables in HRS) were associated with slower cognitive decline in subsequent years. Moreover, rigorous sensitivity analyses confirmed the stability of the results. Conclusions: Our findings support the potential beneficial roles of VF, especially cruciferous vegetables, green leafy vegetables, and red/yellow vegetables, in maintaining cognitive function and slowing cognitive decline in middleaged and older adults. Vitamin B12, B6, or Folate and Cognitive Function in Community-Dwelling Older Adults: A Systematic Review and Meta-Analysis.

Abstract:Background: Previous studies have indicated that B vitamin deficiencies are an essential cause of neurological pathology. There isa need to provide evidence of the benefit of B vitamins for the prevention of cognitive decline in community-dwelling older adults. Objective: To examine the association between intake and plasma levels of vitamins B12, B6, and folate and cognitive function in older populations through a systematic review and meta-analysis. Methods: Medline (PubMed), EMBASE, and Cochrane databases were used to search the literature though August 8, 2019. We included observational population-based studies evaluating the association between concentrations or intake levels of vitamins B6, B12, or folate and cognition in older adults aged >= 45 years. The quality of all studies was assessed by the modified Newcastle-Ottawa Scale. Odds ratios (ORs) and hazard ratios (HRs) were analyzed by the random-effects model. Sensitivity analyses were conducted by excluding the studies with significant heterogeneity. Results: Twenty-one observational studies with sample sizes ranging from 155-7030 were included in the meta-analysis. Higher levels of vitamin B12 (OR = 0.77, 95% CI = 0.61-0.97) and folate concentration (OR = 0.68, 95% CI = 0.51-0.90) were associated with better cognition in cross-sectional studies, but not in sensitivity analyses or prospective studies. High vitamin B6 concentrations showed no significant benefit on cognition and dementia risk. Prospective studies did not provide substantial evidence for the relationship. Conclusion: The results from our meta-analysis suggest that vitamins B12, B6, and folate may not be modifiable risk factors for slowing cognitive decline among community-dwelling older individuals.

Abstract:We derived a negative association between dietary niacin and low cognition. Our study may be able to provide some reference for dietary prevention and treatment strategies for people with cognitive hypoacusis, which has significant implications for the management of patients with impaired cognitive function. Recent studies have shown an inconsistent association between dietary niacin and cognitive function. And this remains unclear in the American outpatient population. The aim of this study was to assess whether there is an association between dietary niacin and cognitive performance in an older American population aged ≥60 years. A total of 2523 participants from the National Health and Nutrition Examination Survey (NHANES) 2011–2014 were enrolled. Cognitive function was assessed by the CERAD Word Learning (CERAD‐WL) test, the CERAD Delayed Recall (CERAD‐DR) test, the Animal Fluency test (AFT), and the Digit Symbol Substitution test (DSST). Cognitive impairment that meets one of the four scoring conditions listed above is defined as low cognitive function. Dietary niacin intake was obtained from 2 days of a 24‐h recall questionnaire. Based on the quartiles of dietary niacin intake, they were divided into four groups: Q1 ( 26.91 mg). The stability of the results was assessed using multifactorial logistic regression, restricted cubic spline (RCS) models, and sensitivity stratified analysis. More than half of the participants had cognitive impairment (52.52%). In the fully adjusted model, niacin was associated with a significantly reduced risk of cognitive impairment in Q3 and Q4 compared with the Q1 group (OR: 0.610, 95% CI: 0.403, 0.921, p = .022; OR: 0.592, 95% CI: 0.367, 0.954, p = .034). Meanwhile, niacin was negatively associated with poor cognition as assessed by the CERAD‐WL test, CERAD test, AFT, and DSST. Abstract:Background and objectives The presence of substantial evidence regarding the association between dietary niacin intake and cognitive impairment among the elderly remains limited, with inconsistent findings. Thus, the objective of this study was to assess the aforementioned relationship, utilizing data obtained from the National Health and Nutrition Examination Survey (NHANES). Methods This cross-sectional study analyzed 2255 participants aged ≥60 years from NHANES 2011–2014.The assessment of dietary niacin intake was conducted through two 24-hour dietary recalls, while cognitive function was evaluated using a battery of five tests. Multivariable logistic regression models and generalized additive model (GAM) was utilized to investigate the association between dietary niacin intake and cognitive impairment. Furthermore, subgroup analysis was conducted to assess the robustness of the primary findings. Results A total of 2255 old adults were included in this study, of whom 47.9% were male. In the fully adjusted model, we observed a significant inverse association between dietary niacin intake and cognitive decline [as a quartile variable, Q4 vs. Q1, odds ratio (OR):0.5 and 95% confidence interval (CI): (0.35∼0.72), p < 0.001; as a continuous variable, per 1 mg/day increment, OR (95%CI):0.97(0.95∼0.98), p < 0.001].The smooth curve fitting results revealed that A linear relationship was found between niacin intake and cognitive impairment in elderly people. The results of the sensitivity analysis remained stable. Conclusions Dietary niacin intakes might be inversely associated with the prevalence of cognitive impairment. Further research is required to confirm this association.

Abstract:Several previous studies discussed the association between vitamin K (VK) status and cognition. But the association between dietary VK consumption and cognitive performance in the elderly was not well understood. Therefore, we investigated the correlation between dietary VK intake and the cognition of the elderly. Our research used the data of the National Health and Nutrition Examination Survey (NHANES) from 2011 to 2014. The dietary intake of VK was assessed by two 24-h dietary recalls. The cognitive function was measured in the survey of NHANES, including the Consortium to Establish a Registry for Alzheimer’s disease Word Learning subtest (CERAD W-L), Animal Fluency Test (AFT), and Digit Symbol Substitution Test (DSST). Logistic regression and restricted cubic spline models were applied to assess the relationship between dietary VK intake and cognition. Compared with the lowest dietary VK intake group, the multivariate-adjusted odds ratio (OR) [95% confidence interval (95% CI)] of low CERAD W-L score for the highest intake group was 0.39 (0.26–0.60), the multivariate-adjusted OR (95% CI) of low AFT score was 0.59 (0.38–0.92), and the multivariate-adjusted OR (95% CI) of low DSST score was 0.44 (0.29–0.65), respectively. There was an L-shaped dose–response relationship between dietary VK intake and low CERAD W-L score. There was a linear dose–response relationship between dietary VK intake and low AFT score, and there was also a linear dose–response relationship for the low DSST score. In addition, we also found a negative association between VK from vegetables and the risk of low CERAD W-L scores. Dietary VK intake and VK intake from vegetables were inversely related to the risk of low cognitive performance of the elderly.

Abstract:Background: The current study aims to examine association of dietary live microbes and nondietary prebiotic/probiotic intake with cognitive function among older U.S. adults, examining heterogeneity across demographic characteristics and diseases. Methods: Participants from the National Health and Nutrition Examination Survey 2011-2014 cycles were selected and administered 3 cognitive function tests: the Consortium to Establish a Registry for Alzheimer's Disease Word Learning subtest (CERAD W-L, including immediate [CERAD-IRT] and delayed [CERAD-DRT] memory), the Animal Fluency Test (AFT), and the Digit Symbol Substitution Test (DSST). Test-specific and global cognition z-score was created. Based on their estimated dietary live microbes intake, participants were categorized into three groups: low, medium, and high. Text mining was employed to identify nondietary prebiotic/probiotic usage by examining the names and ingredients of dietary supplements or drugs. Results: Participants in the medium (including AFT) and high (including global cognition, AFT, DSST, and CERAD-IRT) dietary live microbes intake group had significantly higher z-score of cognitive function compared to those in the low intake group. Among participants with cardiovascular disease history, nondietary prebiotic intake was associated with higher z-score in global cognition and CERAD-DRT compared to those who did not consume prebiotic. Additionally, probiotic intake was linked to higher z-score in global cognition, AFT, and DSST, particularly in participants with diabetes mellitus or hypertension. Conclusions: Our study suggests that the intake of dietary live microbes and nondietary probiotic/prebiotic was associated with better cognitive function in older adults, particularly in specific disease states.

We assessed cognitive function using five tests and analyzed the association between these variables using a multivariate logistic regression model.ResultsA total of 2255 participants aged >= 60 years from the National Health and Nutrition Examination Survey (NHANES) for the 2011-2014 cycle were included in the analysis. Vitamin E intake was negatively associated with cognitive function. In the Z test, with each 1 mg/day increase in dietary intake of vitamin E, there was a 6% decrease in the correlation with cognitive impairment (OR = 0.94, 95% CI 0.91-0.97) in the fully fitted model (OR = 0.94, 95% CI 0.91-0.97). These findings remained consistent when analyzing the exposure as a categorical variable. In comparison to Q1, Q4 showed a 53% reduction in the incidence of cognitive impairment in the Z test (OR = 0.47, 95% CI 0.33-0.67).No significant statistical interaction between the variables was found.ConclusionAn increased dietary intake of vitamin E has been associated with decreased cognitive impairment in individuals over 60 years old. Abstract:Diet is an important modifiable factor for brain health and aging. Present study aimed to explore association of dietary quality with cognitive function and poor cognition in middle-aged and older adults participating in the China Health and Nutrition Survey (CHNS). A longitudinal study with a twenty-year follow-up. Data were drawn from the CHNS 1997, 2000, 2004, 2006, 2015 and 2018. Subjects aged 55 years and more who participated in at least two waves and had completed data on socio-demographics, lifestyle, disease history, anthropometrics, dietary measure and cognitive assessment were eligible in present study. Baseline diet were assessed by 3-day 24-hour dietary recalls and used to evaluate diet quality via China Elderly Dietary Guidelines Index 2022 (CDGI 2022-E). Cognitive function was examined using part items of the Telephone Interview for Cognitive Status-modified. Three-level linear mixed effects models and three-level mixed effects logistic regression models were performed to estimate the association between diet quality and cognitive function and odds of poor cognition, respectively. At baseline, 4173 subjects with median age of 63.7 years were recruited. Median of CDGI 2022-E total score was 44.7. Median score of global cognition was 16.0, and the proportion of people with poor cognitive function was 13.9
Dietary Intake Levels of Iron, Copper, Zinc, and Manganese in Relation to Cognitive Function: A Cross-Sectional Study

Abstract:Background: Previous studies have related circulating levels of trace metal elements, of which dietary intake is the major source, to cognitive outcomes. However, there are still relatively few studies evaluating the associations of dietary intake levels of iron, copper, zinc, and manganese with cognitive function (CF). Methods: We leveraged the data of 6863 participants (mean [standard deviation] age = 66.7 [10.5] years) in the Health and Retirement Study (2013/2014). Dietary intake levels of iron, copper, zinc, and manganese were calculated from a semi-quantitative food frequency questionnaire. CF was assessed using the 27-point modified Telephone Interview for Cognitive Status (TICS). We used linear regression models to calculate the mean differences in global CF scores by quintiles of dietary intake levels of trace metal elements. Results: Among the study participants, the mean (SD) values of daily dietary intake were 13.3 (6.3) mg for iron, 1.4 (0.7) mg for copper, 10.7 (4.6) mg for zinc, and 3.3 (1.6) mg for manganese. Compared with the lowest quintile of dietary iron intake (<8.1 mg), the highest quintile (≥17.7 mg) was associated with a lower cognitive score (−0.50, −0.94 to −0.06, P-trend = 0.007). Higher dietary copper was significantly associated with poorer CF (P-trend = 0.002), and the mean difference in cognitive score between extreme quintiles (≥1.8 vs. <0.8 mg) was −0.52 (95% confidence interval: −0.94 to −0.10) points. We did not observe significant associations for dietary intake of zinc (P-trend = 0.785) and manganese (P-trend = 0.368). Conclusion: In this cross-sectional study, higher dietary intake of iron and copper was related to worse CF, but zinc and manganese intake levels were not significantly associated with CF.
Long-term Intake of Vegetables and Fruits and Subjective Cognitive Function in US Men.

Abstract:OBJECTIVE:To evaluate the prospective association of long-term intake of vegetables and fruits with late-life subjective cognitive function (SCF).METHODS:Among 27,842 men with a mean age of 51 years in 1986, we used multinomial logistic regression to examine the relation of vegetable and fruit consumption to future SCF. Average dietary intake was calculated from 5 repeated food frequency questionnaires collected every 4 years until 2002. SCF score was assessed twice (2008 and 2012) using a 6-item questionnaire; validity was supported by strong associations with APO ε4 genotype. We categorized the average of the 2 scores as good, moderate, and poor SCF.RESULTS:Higher intakes of total vegetables, total fruits, and fruit juice were each significantly associated with lower odds of moderate or poor SCF after controlling for major nondietary factors and total energy intake. The association with total fruit intake was weaker after further adjusting for major dietary factors. In this model, the multivariate odds ratios (95% confidence intervals) for vegetable intake (top vs bottom quintile) were 0.83 (0.76-0.92), p trend <0.001 for moderate SCF and 0.66 (0.55-0.80), p trend <0.001 for poor SCF. For orange juice, compared to <1 serving/mo of intake, daily consumption was associated with a substantially lower odds of poor SCF (0.53 [0.43-0.67], p trend <0.001). Higher consumption of vegetables and fruits 18 to 22 years before SCF assessment was associated with lower odds of poor SCF independent of more proximal intake.CONCLUSION:Our findings support a long-term beneficial role of vegetable, fruit, and orange juice consumption on SCF.
Association Between Composite Dietary Antioxidant Index and Cognitive Function Among Aging Americans from NHANES 2011–2014

Abstract:Background: Antioxidant diets are considered to be protective factors for cognitive function. However, comprehensive measures of antioxidant diets are lacking. Objective: To examine the association between the Composite Dietary Antioxidant Index (CDAI) and cognitive function in the elderly. Methods: This cross-sectional study included a total of 2,456 participants (≥60 years old) from NHANES 2011–2014. Calculation of CDAI based on 6 minerals and vitamins (manganese, selenium, zinc, vitamins A, C, and E). Cognitive function was measured by the Consortium to Establish a Registry for Alzheimer's Disease (CERAD) Word Learning sub-test, Animal Fluency Test (AFT), and Digit Symbol Substitution Test (DSST). We also created a composite cognitive z-score to represent global cognition. The statistical analyses we used included multiple linear regression analyses, subgroup analyses, curve-fitting analyses, and threshold effects analyses. Results: After controlling for demographic characteristics, lifestyle factors, and disease history, multivariate linear regression analyses showed that increased CDAI was positively associated with scores on global cognitive function and each cognitive domain (p < 0.05), with subgroup analyses suggesting that this association was more pronounced in stroke patients (p for interaction < 0.05). Curve-fitting analyses and threshold effect analyses showed saturation effects between CDAI and CREAD Test, AFT, and composite Z-score, and an inverted U-shaped relationship with DSST, with inflection points of –1.89, 0.79, 1.13, and 1.77, respectively. Conclusions: Our findings support that higher levels of CDAI are correlated with significantly elevated cognitive function. Maintaining CDAI in an appropriate range may contribute to cognitive health in elderly.

Abstract:Abstract Background Research investigating the correlation between dietary riboflavin intake and cognitive function in older adults is scarce and constrained. Our objective was to investigate the relationship between riboflavin intake levels and cognitive performance in individuals aged 60 years and above in the United States. Methods We analyzed data from 2,255 adults aged 60 years and older who participated in the 2011–2014 National Health and Nutrition Examination Survey (NHANES). The Consortium to Establish a Registry for Alzheimer's Disease test, Digit Symbol Substitution Test, and Animal Fluency Test were used to evaluate cognitive performance. Dietary intake was assessed using the 24-hour recall method, which reflects recent dietary habits of individuals and groups. Logistic regression was used to examine the association between riboflavin intake and cognitive impairment, and restricted cubic spline analysis was also conducted. Results The study included a total of 2,255 patients, with 47.9% being male. The incidence of low cognitive function was 23.8%. The findings showed that high riboflavin intake was associated with a lower risk of cognitive impairment in adults in the United States. Compared to those with the lowest intake, individuals with the highest riboflavin intake had a 32% reduction in the correlation between AFT, DSST, and Z-test low cognitive function (OR: 0.68, 95% CI: 0.49–0.96, P = 0.028), a 47% reduction in the correlation between DSST low cognitive function (OR: 0.53, 95% CI: 0.37–0.77, P = 0.001), and a 44% reduction in the correlation between Z-test low cognitive function (OR: 0.56, 95% CI: 0.39–0.8, P = 0.002). The study also found an L-shaped association between low cognition and riboflavin intake, with an inflection point at approximately 2.984 mg/d. Conclusions Our cross-sectional study supported the negative associations of dietary riboflavin intake with low cognitive function, also showed a L-shaped dose–response relationship with the point of 2.984mg/day.

Abstract:The purpose of this study was to explore whether dietary live microbe intake is associated with various cognitive domains using data from the National Health and Nutrition Examination Survey (NHANES) from 2011 to 2014. And the specific relationship between low, medium and high dietary live microbe intake groups and cognitive ability of the elderly. Dietary live microbe intake was calculated from 24-h diet recall interviews. Cognitive function was assessed using the number symbol substitution test (DSST, which measures processing speed), the animal fluency test (AFT, which measures executive function), the Alzheimer's Registry sub-test (CERAD, which measures memory), and the Composite Z-score, which adds the Z-values of individual tests. Multiple linear regression models and restricted cubic bar graphs were used to investigate the relationship between live microbe intake and cognitive performance. A total of 2,450 participants aged 60 or older were included. Live microbe intake was positively correlated with cognitive ability on the whole. Specifically, when the intake of low, medium and high live microbe was > 2640 g, > 39 g and > 0 g respectively, the CERAD, DSST, AFT and compositive-Z score of the subjects increased with the increase of microbial intake (P < 0.05). In American adults age 60 or older, higher intakes of live microbes were associated with better cognitive performance, especially after a certain amount was reached.

Abstract:Background: Nutritional support has been identified as a potential intervention for cognitive frailty; however, the association between 25-hydroxyvitamin D [25-(OH)D], vitamin B12, and cognitive frailty remains ambiguous. Methods: This study utilized data from two cycles (2011-2012, 2013-2014) of the National Health and Nutrition Examination Survey (NHANES) to investigate this relationship. The researchers constructed a 41-item frailty index encompassing diverse aspects of physical functioning, psychological evaluation, and medical conditions, and evaluated each participant individually. The study utilized Spearman's rank correlation coefficient and univariate ordered logistic regression to assess the relationships between variables and cognitive frailty. Recursive feature elimination and cross-validation methods were employed to identify the most influential variables for building and optimizing multivariate ordered logistic regression models. Subgroup analyses and interaction tests were further conducted to validate the identified correlations. Results: The findings of this study confirm a negative linear correlation between 25-(OH)D levels and cognitive frailty in older adults. Specifically, a one-unit increase in 25-(OH)D levels was associated with a 12% reduction in the risk of cognitive frailty. The result was further supported by subgroup analyses and interaction tests. Conclusion: The existence of a negatively correlated linear association between 25-(OH)D levels and cognitive frailty in older adults is plausible, but further rigorously designed longitudinal studies are necessary to validate this relationship.
Long-term Dietary Flavonoid Intake and Subjective Cognitive Decline in US Men and Women

Abstract:Objective To prospectively examine the associations between long-term dietary flavonoids and subjective cognitive decline (SCD). Methods We followed 49,493 women from the Nurses' Health Study (NHS) (1984-2006) and 27,842 men from the Health Professionals Follow-Up Study (HPFS) (1986-2002). Poisson regression was used to evaluate the associations between dietary flavonoids (flavonols, flavones, flavanones, flavan-3-ols, anthocyanins, polymeric flavonoids, and proanthocyanidins) and subsequent SCD. For the NHS, long-term average dietary intake was calculated from 7 repeated semiquantitative food frequency questionnaires (SFFQs), and SCD was assessed in 2012 and 2014. For the HPFS, average dietary intake was calculated from 5 repeated SFFQs, and SCD was assessed in 2008 and 2012. Results Higher intake of total flavonoids was associated with lower odds of SCD after adjustment for age, total energy intake, major nondietary factors, and specific dietary factors. In a comparison of the highest vs the lowest quintiles of total flavonoid intake, the pooled multivariable-adjusted odds ratio (OR) of 3-unit increments in SCD was 0.81 (95% confidence interval [CI] 0.76, 0.89). In the pooled results, the strongest associations were observed for flavones (OR 0.62 [95% CI 0.57, 0.68]), flavanones (0.64 [0.58, 0.68)]), and anthocyanins (0.76 [0.72, 0.84]) (p trend <0.001 for all groups). The dose-response curve was steepest for flavones, followed by anthocyanins. Many flavonoid-rich foods such as strawberries, oranges, grapefruits, citrus juices, apples/pears, celery, peppers, and bananas, were significantly associated with lower odds of SCD. Conclusion Our findings support a benefit of higher flavonoid intakes for maintaining cognitive function in US men and women.

Abstract:Background Neuroinflammation has been linked to the development of cognitive performance. Epidemiological evidence on dietary inflammatory potential and cognitive performance is scarce. We evaluated the association between dietary inflammatory index (DII) and cognitive performance in older adults. Methods This study included adults aged 60 years or older from the 2011–2014 National Health and Nutrition Examination Survey. The DII scores were calculated based on 27 nutritional parameters. Cognitive performance was assessed with four cognitive tests: the Digit Symbol Substitution Test (DSST, n = 2,780), the Consortium to Establish a Registry for Alzheimer’s Disease Word Learning (CERAD-WL, n = 2,859) and Delayed Recall (CERAD-DR, n = 2,857), and the Animal Fluency (AF, n = 2,844) tests. Restricted cubic splines and logistic regression were adopted to assess the associations. Results Comparing the highest to lowest tertile of DII scores, the odds ratio (95% CI) of lower cognitive functioning was 1.97 (1.08–3.58) [ P -trend = 0.02, per 1 unit increment: 1.17 (1.01–1.38)] on DSST, 1.24 (0.87–1.76) [ P -trend = 0.24, per 1 unit increment: 1.09 (0.96–1.23)] on CERAD-WL, 0.93 (0.57–1.51) [ P -trend = 0.74, per 1 unit increment: 1.02 (0.87–1.20)] on CERAD-DR, and 1.76 (1.30–2.37) [ P -trend < 0.01, per 1 unit increment: 1.17 (1.05–1.29)] on AF. The above-mentioned associations were observed in both men and women. In non-linear dose–response analysis, the association between DII and lower cognitive functioning was not significant at lower DII scores up to 3.0, after which the association was significant and the curve rose steeply. Conclusion Higher DII is associated with lower scores on DSST and AF tests in older adults.

Abstract:Abstract BackgroundDiet quality plays an important role in the development of age-related chronic diseases. However, the association between dietary quality assessed by Healthy Eating Index (HEI)-2015 and cognitive function among the United States (US) elderly adults remains unclear. The present study aims to explore the association between HEI-2015 and cognitive function in elderly adults using data from National Health and Nutrition Examination Survey (NHANES) 2011-2012. MethodsHEI-2015 scores were calculated from two days 24-hour recall interviews. The cognitive function was evaluated by Digit Symbol Substitution Test (DSST), Animal Fluency Test (AFT) and a global cognition level derived by summing the z scores of individual tests. The associations between HEI-2015 and cognitive function were explored using multiple linear regression and binary logistic regression models. ResultsA total of 1278 participants aged 60 years or older were included. Compared to the lowest HEI-2015 tertile, the elderly adults in highest tertile had a higher global cognition, DSST and AFT scores (P＜0.05); with the lowest quartile of global cognition as cognitive impairment, those who were in the highest HEI-2015 tertile had 38% lower odds of cognitive impairment (OR: 0.62; 95%CI: 0.42-0.91). Among HEI-2015 components, the elderly adults adhering to recommended intake of whole grains and whole fruits components were more likely to have better performance on global cognition (P＜0.05). ConclusionThe higher HEI-2015 was positively associated the better cognitive performance on the global cognition, DSST, and AFT in the US elderly adults.
