Effect of immunosuppressant coadministration with mepolizumab for moderate to severe eosinophilic granulomatosis with polyangiitis(EGPA)

Abstract:Introduction: Mepolizumab (Mepo), an anti-interleukin-5 monoclonal antibody, induces remission and decreases the daily corticosteroid (CS) dose required; however, it is not known whether the coadministration of other immunosuppressants (IS) with Mepo will increase remission rates in patients with EGPA. Aims: To evaluate the effect of IS coadministration with Mepo for moderate to severe EGPA. Methods: Sixty EGPA patients were treated at the NHO Yokohama Medical Center between April 2022 and January 2024. We allocated 43 EGPA patients in whom remission could not be induced by conventional therapy to a group of 29 in whom IS was used at the same time as mepo and systemic CS (CS+IS+Mepo group) and a group of 14 in whom IS was not used concurrently (CS+Mepo group). We investigated relapse-free survival and background factors in the two groups. Results: Among those in CS+Mepo group, 85.7% had received IS as an initial treatment (before mepo administration). Relapse-free survival was significantly higher in CS+Mepo group than in CS+IS+mepo group (P < 0.05). In CS+Mepo group, 51.8% had relapsed after the start of mepo, whereas 93% of CS+IS+mepo group had relapsed by 30 months after mepo initiation (P < 0.05). In CS+Mepo group, the percentage of patients with cardiac involvement was significantly smaller, the BVAS after conventional treatment was significantly lower, and the lowest serum IgG level after conventional treatment was significantly higher than in CS+IS+mepo group (all P < 0.05). Conclusion: EGPA patients receiving CS+Mepo in whom the BVAS decreases and serum IgG levels do not decrease after conventional therapy may achieve long-term remission.
