Abstract:The delivery of high dose radiotherapy to tumors is often limited by the proximity of the surrounding radiosensitive normal tissues, even using modern techniques such as intensity modulated radiation therapy (IMRT). Previous studies have reported that placement of a spacer can effectively displace normal tissues. So that they are some distance away from the lesion, thus allowing for the safe delivery of high-dose radiation. The application of radioprotective spacers was first reported 30 years ago regarding radiotherapy of tongue and abdominal cancers; more recently, they are increasingly being used in prostate cancer. This review focuses on the published data concerning the features of different types of spacers and their application in various tumor sites. Placement-related complications and the cost-effectiveness of the spacers are also discussed. With the increasing use of high-precision radiotherapy in clinical practice, especially the paradigm-changing stereotactic body radiotherapy (SBRT), more robust studies are warranted to further establish the role of radioprotective spacers through materials development and novel placement techniques.
A novel hydrogel orthotopic injection model in moderately hypofractionated radiation therapy for prostate cancer: Adaptive degradation and durable imaging

Abstract:Purpose Moderately hypofractionated radiotherapy (MHRT) holds an important position in prostate cancer management. Existing hydrogel spacers can protect the rectum from radiation damage, but need improvement. We explored the application of a novel hydrogel in MHRT with adaptive degradation and durable imaging functions. Methods and materials The hydrogels were irradiated with 6MV x-ray to detect the radio-resistance property. Male SD rats (n=45) underwent hydrogel injection between the prostate and rectum. CT was used for investigating the novel spacer’s degradation and imaging functions over three months. The hydrogel’s radiation-attenuation properties and biocompatibility were further assessed. Results Hydrogel weight and volume remained stable for six weeks post-injection. After MHRT ended, the hydrogel showed accelerated degradation characteristics and remained in the body for at most three months. CT values of hydrogels exceeded 300 Hounsfield units (HU) throughout treatment, significantly higher than in surrounding normal tissues. A significant dose drop behind the hydrogel was observed post-implantation. Biocompatibility tests of hydrogel found it safe enough for living organisms. Conclusions The novel hydrogel application was fully adaptable to prostate cancer MHRT modalities, largely stable during treatment, rapidly degraded after radiotherapy ended, and consistently maintained superior imaging performance and biocompatibility. This novel spacer will be an effective tool in the era of hypofractionated radiotherapy.

Abstract:Radionuclide therapy (RNT) stands out as a highly effective method for treating solid tumors. However, its therapeutic efficiency faces challenges due to the radioresistance of tumors, the limited penetration depth and intracellular deposition of rays in tumor tissue, which causes residue of living cancer cells. Herein, we report a novel approach by utilizing radionuclide 131 I-labelled polydopamine encapsulated gold nanoparticle co-loaded with the classical anticancer drug gemcitabine within a hydrogel formed from oxidized glucan and chitosan hydrochloride, combining RNT with chemotherapy for cancer treatment. Au, as a high Z element, is able to interact with shortrange /3-rays to emit bremsstrahlung and secondary charged particles which in turn increase the dose deposited in tumor cells. Simultaneously, gemcitabine is able to affect cell cycle redistribution, resulting in an increase in the radiosensitive cellular component of the cycle, and gemcitabine also inhibits the repair of radioactive damage to cellular DNA, which has a radiosensitizing effect. In both in vivo and vitro experiments, the injectable hydrogel demonstrates excellent biosecurity, stability in radionuclide labeling, and capabilities for single-photon emission computed tomography (SPECT) imaging. Compared analysis with single RNT revealed that combination therapy markedly inhibits the growth of triple-negative breast cancer. This integrated therapeutic strategy establishes an efficient tumor synergistic treatment platform, offering new avenues for advancing radionuclide therapy in the clinical treatment of cancer.

Abstract:Abstract Patients with advanced colorectal cancer (CRC) and diffuse peritoneal metastasis are not eligible for surgical intervention, and calling for more efficient and precise treatment strategies to complement the traditional systemic chemotherapy and intraperitoneal chemotherapy. Herein, an injectable two-dimensional molybdenum disulfide (2D-MoS2)-integrated adhesive N-(2-aminoethyl)-4-(4-(hydroxymethyl)-2-methoxy-5-nitro-sophenoxy) butanamide-linked sodium alginate-MoS2-5-fluorouracil (AlgNB/MoS2/5-Fu) hydrogel, which can function as an near-infrared light (NIR)-triggered photothermal and drug-delivery implant for CRC treatment is introduced. Ultraviolet (UV) light-activated aldehyde groups in AlgNB bound to the surface-modified MoS2 nanosheets via a Schiff base reaction. The MoS2 nanosheets maintained superior dispersibility in the hydrogel and exhibited a highly efficient NIR-triggered photothermal effect. More importantly, the aldehyde group in AlgNB also imparted tissue adhesion to the hydrogel, the adhesive hydrogel was used to infiltrate and fix in tumor tissue. Combining applications as a 5-Fu drug delivering implant, the injectable adhesive AlgNB/MoS2/5-Fu hydrogel shows remarkable capability in the inhibition of SW480 cells and colorectal tumour regression by triggering photothermal therapy (PTT) and delivering the 5-Fu drug in both in vitro and in vivo studies. Therefore, this research provides distinct guidance strategies for the synergistic tumor therapy of localized CRC and shows enormous potential for cancer treatment in the clinical setting.

The role of a radiopaque peri-rectal hydrogel spacer in aiding accurate daily image-guidance for prostate stereotactic radiotherapy

Abstract:Introduction: Precise patient positioning with image guidance (IGRT) is essential for safe prostate radiotherapy. We present the first report of utilizing a CT-visible hydrogel spacer, used to decrease rectal radiation dose, as a surrogate fiducial marker to aid in daily IGRT with cone-beam CT (CBCT) in stereotactic radiotherapy (SABR) for prostate cancer. Materials and methods: Prior to CT simulation, patients underwent placement of three intraprostatic gold fiducial markers and radiopaque hydrogel spacer per standard practice. At treatment, after initial setup, a CBCT was acquired and fused to the planning CT based on 3-dimensional matching of the spacer. A second alignment was then performed based on the fiducial markers. The six directional shifts (three linear and three rotational) were recorded, and the differences compared. Results: 140 individual fractions across 41 consecutive patients were evaluated. Mean/median differences between hydrogel spacer-based and fiducial-based alignment in linear (vertical, longitudinal, lateral) and rotational (rotation, pitch, roll) shifts were 0.9/0.6mm, 0.8/0.5mm, and 0.6/0.4mm, and 0.38/0, 0.62/0, and 0.35/0 degrees, respectively. No difference was observed in 9.9%, 22.9%, and 22.14% of linear shifts, and 65.7%, 65%, and 66.4% rotational shifts, respectively. Significantly smaller differences were observed in the latter 70 fractions vs. the former, and results were consistent across evaluators. Conclusions: For precise daily IGRT with CBCT for prostate SABR, alignment using a radiopaque hydrogel spacer was highly comparable to intraprostatic fiducial markers. This represents the first report supporting an additional indication of IGRT for a CT-visible hydrogel spacer, to further enhance treatment accuracy and potentially obviate the need for the additional fiducial marker procedure.

Abstract:Brachytherapy stands as an essential clinical approach for combating locally advanced tumors. Here, an injectable brachytherapy hydrogel is developed for the treatment of both local and metastatic tumor. Fe‐tannins nanoparticles are efficiently and stably radiolabeled with clinical used therapeutic radionuclides (such as 131I, 90Y, 177Lu and 225Ac) without a chelator, and then chemically cross‐linked with 4‐ArmPEG‐SH to form brachytherapy hydrogel. Upon intratumoral administration, magnetic resonance imaging (MRI) signal from ferric ions embedded within the hydrogel directly correlates with the retention dosage of radionuclides, which can real‐time monitor radionuclides emitting short‐range rays in vivo without penetration limitation during brachytherapy. The hydrogel's design ensures the long‐term tumor retention of therapeutic radionuclides, leading to the effective eradication of local tumor. Furthermore, the radiolabeled hydrogel is integrated with an adjuvant to synergize with immune checkpoint blocking therapy, thereby activating potent anti‐tumor immune responses and inhibiting metastatic tumor growth. Therefore, this work presents an imageable brachytherapy hydrogel for real‐time monitoring therapeutic process, and expands the indications of brachytherapy from treatment of localized tumors to metastatic tumors. Supramolecular hydrogel-loaded Prussian blue nanoparticles with photothermal and ROS scavenging ability for tumor postoperative treatments

Abstract:The need for tumor postoperative treatments aimed at recurrence prevention and tissue regeneration have raised wide considerations in the context of the design and functionalization of implants. Supramolecular hydrogel is an ideal choice due to its compatibility, injectability, and ease of functionalization. Herein, we developed supramolecular hydrogel-loaded Prussian blue nanoparticles (PB NPs) for postoperative treatments. Biocompatible gellan gum was modified with host (cyclodextrin) or guest (adamantane) molecules to form supramolecular hydrogels (GEL) for local therapy. Hollow mesoporous Prussian blue nanoparticles (HMPB NPs) were prepared for use as photothermal agents and reactive oxygen species (ROS) scavengers. Further supramolecular hydrogels containing HMPB NPs loading doxorubicin (HMPB-DOX@GEL) were designed as multifunctional postoperative implants to ensure effective residual tumor elimination and oxidative relief. In vitro and in vivo experiments were carried out to verify the elimination of cancer cells and tumor recurrence prevention through combinational chemotherapy/photothermal therapy with HMPB-DOX@GEL. The tissue regeneration acceleration and in vivo ROS scavenging of HMPB@GEL were verified using diabetic mouse models of wound healing. The findings thus show that, because of the effective tumor recurrence prevention and wound healing acceleration of the multifunctional hydrogels developed here, they are promising for application in tumor postoperative treatments.
Tumor‐Targeted Injectable Double‐Network Hydrogel for Prevention of Breast Cancer Recurrence and Wound Infection via Synergistic Photothermal and Brachytherapy

Abstract:An injectable double‐network nanocomposite hydrogel is engineered by near‐infrared (NIR)‐induced polymerization of polyethylene glycol acrylate and ionic crosslink between alginate and endogenous Ca2+ after injection of precursor solutions into the cavity of resected breasts. The double‐network hydrogel efficiently prevents postoperative tumor recurrence and wound infection via synergistic NIR‐stimuli photothermal and brachytherapy. The high locoregional recurrence rate and potential wound infection in breast cancer after surgery pose enormous risks to patient survival. In this study, a polyethylene glycol acrylate (PEGDA)‐alginate double‐network nanocomposite hydrogel (GPA) embedded with 125I‐labeled RGDY peptide‐modified gold nanorods (125I‐GNR‐RGDY) is fabricated. The double‐network hydrogel is formed by injection of GPA precursor solutions into the cavity of resected cancerous breasts of mice where gelation occurred rapidly. The enhanced temperature‐induced PEGDA polymerization driven by near‐infrared light irradiation, and then, the second polymer network is crosslinked between alginate and endogenous Ca2+ around the tumor. The double‐network hydrogel possesses a dense polymer network and tightly fixes 125I‐GNR‐RGDY, which exhibit superior persistent photothermal and radioactive effects. Hyperthermia induced by photothermal therapy can inhibit self‐repair of damaged DNA and promote blood circulation to improve the hypoxic microenvironment, which can synergistically enhance the therapeutic efficacy of brachytherapy and simultaneously eliminate pathogenic bacteria. Notably, this nanocomposite hydrogel facilitates antibacterial activity to prevent potential wound infection and is tracked by single‐photon emission computerized tomography imaging owing to isotope labeling of loaded 125I‐GNR‐RGDY. The combination of photothermal therapy and brachytherapy has enabled the possibility of proposing a novel postoperative adjuvant strategy for preventing tumor recurrence and wound infection.

Multifunctional Glycopeptide-Based Hydrogel via Dual-Modulation for the Prevention and Repair of Radiation-Induced Skin Injury

Abstract:The radiation-induced skin injury (RISI) remains a great challenge for clinical wound management and care after radiotherapy, as patients will suffer from the acute radiation injury and long-term chronic inflammatory damage during the treatment. The excessive ROS in the early acute stage and prolonged inflammatory response in the late healing process always hinder therapeutic efficiency. Herein, we developed an extracellular matrix (ECM)-mimetic multifunctional glycopeptide hydrogel (oCP@As) to promote and accelerate RISI repair via a dual-modulation strategy in different healing stages. The oCP@As hydrogel not only can form an ECM-like nanofiber structure through the Schiff base reaction but also exhibits ROS scavenging and DNA double-strand break repair abilities, which can effectively reduce the acute radiation damage. Meanwhile, the introduction of oxidized chondroitin sulfate, which is the ECM polysaccharide-like component, enables regulation of the inflammatory response by adsorption of inflammatory factors, accelerating the repair of chronic inflammatory injury. The animal experiments demonstrated that oCP@As can significantly weaken RISI symptoms, promote epidermal tissue regeneration and angiogenesis, and reduce pro-inflammatory cytokine expression. Therefore, this multifunctional glycopeptide hydrogel dressing can effectively attenuate RISI symptoms and promote RISI healing, showing great potential for clinical applications in radiotherapy protection and repair.
Balloon Inspired Conductive Hydrogel Strain Sensor for Reducing Radiation Damage in Peritumoral Organs During Brachytherapy

Abstract:Conductive hydrogels have recently gained impressive attention in biological medicine and intelligent electronics. Despite the multifunctions demonstrated by existing conductive hydrogels, it is still a challenge to introduce a hydrogel to adjust the distance between the peritumoral organs and the tumor, reducing the radiation damage to the peritumoral organs. Here, a hydrophobic associating hydrogel with multiple desirable features is fabricated based on diverse supramolecular assembly. The introduction of MXene and hydrolyzed keratin (HK) imparts the hydrogel with excellent conductivity, ultra‐stretchability (>2000%), and good self‐adhesion. Moreover, the hydrogel is utilized as an intelligent sensor intended for monitoring various human movements and physiological signals, demonstrating a wide strain window, the rapid response time (130 ms), and outstanding strain sensitivity (GF = 10.22). Inspired by balloon inflation, the hydrogel is designed to separate the tumor from the peritumoral organs in brachytherapy. It plays a role in reducing the radiation dose and damage to the peritumoral organs. The authors also simulate the attenuation process of the radiation signal according to the change of the hydrogel size and develop a smartphone application (app) to monitor the safety range of the different radiation risks, manifesting its great potential in soft intelligent sensors. A hydrogel‐based sensor with outstanding stretchability, excellent conductivity, and remodeling performance has been successfully designed. Inspired by balloon inflation, the hydrogel is designed to separate the tumor from the peritumoral organs in brachytherapy. The authors also simulate the attenuation process of radiation signals and monitor the safety range of different risks through an App.

Abstract:Doxorubicin (DOX) is the classic soft tissue sarcomas (STS) first‐line treatment drug, while dose‐dependent myelosuppression and cardiotoxicity limit its application in clinic. This research intends to apply DOX, which is also an inducer of immunogenic cell death (ICD) as a part for "in situ vaccination" and conjointly use PD‐1 inhibitors to enhance antitumor efficacy. In order to achieve the sustained vaccination effect and real‐time monitoring of distribution in vivo, the in situ forming and injectable hydrogel platform with the function of visualization is established for local delivery. The hydrogel platform is synthesized by hyaluronic acid‐dopamine (HA‐DOPA) coordinated with gadolinium Ions (Gd2+). Gd2+ provides the ability of magnetic resonance imaging (MRI), meanwhile further cross‐linking the hydrogel network. Experiments show excellent ability of sustained release and imaging tracking for the hydrogel platform. In mouse STS models, the "in situ vaccination" hydrogels show the best effect of inhibiting tumor growth. Further analysis of tumor tissues show that "in situ vaccination" group can increase T cell infiltration, promote M1‐type macrophage polarization and block elevated PD‐1/PD‐L1 pathway caused by DOX. These results are expected to prove the potential for synthesized hydrogels to achieve a universal platform for "in situ vaccination" strategies on STS treatments. Abstract:Radiotherapy is a pivotal means of cancer treatment, but it often leads to radiation dermatitis, a skin injury caused by radiation-induced excess reactive oxygen species (ROS). Scavenging free radicals in the course of radiation therapy will be an effective means to prevent radiation dermatitis. This study demonstrates a novel double network hydrogel doped with MoS2 nanosheets for the prevention of radiation-induced dermatitis. The resultant SPM hydrogel constructed from polyacrylamide (PAM) and sodium alginate (SA) nanofiber presented favorable mechanical and adhesion properties. It could conform well to the human body's irregular contours without secondary dressing fixation, making it suitable for skin protection applications. The in vitro and in vivo experiments showed that the antioxidant properties conferred by MoS2 nanosheets enable SPM to effectively mitigate excessive ROS and reduce oxidative stress, thereby preventing radiation dermatitis caused by oxidative damage. Biosafety assessments indicated good biocompatibility of the composite hydrogel, suggesting SPM's practicality and potential as an external dressing for skin radiation protection.

Abstract:The widespread use of video-assisted thoracoscopic surgery (VATS) has triggered the rapid expansion in the field of computed tomography (CT)-guided preoperative localization and near-infrared (NIR) fluorescence image-guided surgery. However, its broader application has been hindered by the absence of ideal imaging contrasts that are biocompatible, minimally invasive, highly resolvable, and perfectly localized within the diseased tissue. To achieve this goal, we synthesize a dextran-based fluorescent and iodinated hydrogel, which can be injected into the tissue and imaged with both CT and NIR fluorescence modalities. By finely tuning the physical parameters such as gelation time and composition of iodinated oil (X-ray contrast agent) and indocyanine green (ICG, NIR fluorescence dye), we optimize the hydrogel for prolonged localization at the injected site without losing the dual-imaging capability. We validate the effectiveness of the developed injectable dual-imaging platform by performing image-guided resection of pulmonary nodules on tumor-bearing rabbits, which are preoperatively localized with the hydrogel. The injectable dual-imaging marker, therefore, can emerge as a powerful tool for surgical guidance.

Abstract:The convenience of injectable hydrogels that can provide high loading of diverse phototherapy agents and further long-time retention at the tumor site has attracted tremendous interest in simultaneous photothermal and photodynamic cancer therapies. However, to incorporate the phototherapy agents into hydrogels, complex modifications are generally unavoidable. Moreover, these phototherapy agents usually suffer from low efficiency and work at different irradiation wavelengths outside the near infrared windows. Hence, a method for the fabrication of an injectable hydrogel for simultaneous photothermal therapy and photodynamic therapy, through the Schiff-base reaction between amido modified carbon dots (NCDs) and aldehyde modified cellulose nanocrystals is proposed. The NCDs act as both phototherapy agents and crosslinkers to form hydrogels. Significantly, the NCDs demonstrate an extremely high photothermal conversion efficiency of 77.6% which is among the highest levels for photothermal agents and a high singlet quantum yield of 0.37 under a single 660 nm light-emitting diode irradiation. The hydrogels are examined through in vitro and in vivo animal experiments which show nontoxic and effectively tumor inhibition. Thus, the strategy of direct reaction of phototherapy agents and the matrix not only provides new strategies for injectable hydrogel fabrication but paves a new road for advanced tumor treatment.

Abstract:Injectable hydrogels attract a great deal of attention due to their in situ formation in vivo, which minimizes the risk of major trauma during the surgical procedure as compared to traditional hydrogel scaffolds. Click chemistry is widely used in the preparation of injectable hydrogels, although some of them require photoinitiators or catalysts that may be cytotoxic and impair the proliferation of encapsulated cells. In the present study, an injectable dual crosslinking hydrogel was designed by a thiol-ene click reaction and conversion between sulfhydryl and disulfide bonds at a neutral pH. The sulfhydryl group and acrylate group were successfully grafted onto HA and characterized using FT-IR and H-1-NMR. The gelation time, morphology, injection force, swelling, degradation, mechanical properties and drug release behavior of the hydrogel varied with the molecular weight of the hydrogel (M-w = 0.1, 1.0, 2.0 MDa). MTT, FDA/PI staining and scanning electron microscopy (SEM) showed that the encapsulated L929 proliferated well in different molecular weight hydrogels. It was also observed that in a low molecular weight (0.1 MDa) hydrogel system, the cells were better distributed and secreted more extracellular matrix. Subcutaneous injection in mice also demonstrated that hydrogels could support the proliferation of encapsulated L929 cells in vivo, and no significant infiltration of peripheral cells into the interior of the material was observed. These results indicate that this injectable hyaluronan-based hydrogel is an excellent cell protectant and exhibits good biocompatibility, with potential for biomedical applications such as cell delivery and postoperative anti-adhesion.
Hydrogel spacer shrinkage during external-beam radiation therapy following low-dose-rate brachytherapy for high-risk prostate cancer: a case series

Abstract:Abstract Background Few studies have assessed hydrogel spacer shrinkage during external-beam radiation therapy following brachytherapy for localized high-risk prostate cancer. This case presentation evaluated the changes in hydrogel spacer appearance by magnetic resonance imaging during external-beam radiation therapy after brachytherapy for prostate cancer and analyzed the effect of this shrinkage on the dose distribution in four cases. Case presentation In all cases, we implanted 125 I sources using a modified peripheral loading pattern for seed placement. The prescribed dose for each implant was 110 Gy. After delivering the sources, a hydrogel spacer was injected. All cases underwent external-beam radiation therapy approximately 1–2 months after brachytherapy. The prescribed dose of external-beam radiation therapy was 45 Gy in 1.8-Gy fractions. Magnetic resonance imaging was performed for evaluation on the day following seed implantation (baseline), at external-beam radiation therapy planning, and during external-beam radiation therapy. The median hydrogel spacer volume was 16.2 (range 10.9–17.7) cc at baseline, 14.4 (range, 9.4–16.1) cc at external-beam radiation therapy planning, and 7.1 (range, 2.0–11.4) cc during external-beam radiation therapy. The hydrogel spacer volume during external-beam radiation therapy was significantly lower than that at external-beam radiation therapy planning. The rectum V60–80 (rectal volume receiving at least 60–80% of the prescribed dose of external-beam radiation therapy) during external-beam radiation therapy was significantly higher than that at external-beam radiation therapy planning. Conclusions The potential reduction in hydrogel spacer size during external-beam radiation therapy following brachytherapy can lead to unexpected irradiation to the rectum. This case presentation would be helpful for similar cases.

Abstract:Intratumoral injections have the potential for enhanced cancer treatment efficacy while reducing costs and systemic exposure. However, intratumoral drug injections can result in substantial off-target leakage and are invisible under standard imaging modalities like ultrasound (US) and x-ray. A thermosensitive poloxamer-based gel for drug delivery was developed that is visible using x-ray imaging (computed tomography (CT), cone beam CT, fluoroscopy), as well as using US by means of integrating perfluorobutane-filled microbubbles (MBs). MBs content was optimized using tissue mimicking phantoms and ex vivo bovine livers. Gel formulations less than 1% MBs provided gel depositions that were clearly identifiable on US and distinguishable from tissue background and with minimal acoustic artifacts. The cross-sectional areas of gel depositions obtained with US and CT imaging were similar in studies using ex vivo bovine liver and postmortem in situ swine liver. The gel formulation enhanced multimodal image-guided navigation, enabling fusion of ultrasound and x-ray/CT imaging, which may enhance targeting, definition of spatial delivery, and overlap of tumor and gel. Although speculative, such a paradigm for intratumoral drug delivery might streamline clinical workflows, reduce radiation exposure by reliance on US, and boost the precision and accuracy of drug delivery targeting during procedures. Imageable gels may also provide enhanced temporal and spatial control of intratumoral conformal drug delivery.
