Intestine‐targeted Explosive Hydrogel Microsphere Promotes Uric Acid Excretion for Gout Therapy

Abstract:Purines could be converted into uric acid in the gut microenvironment, triggering metabolic diseases, especially gout, while the uric acid excretion is impaired. However, increasing uric acid excretion remains a challenge. In this study, we prepared a uricase‐containing intestine‐targeted explosive hydrogel microspheres via gas‐shearing microfluidics for mucosal immobilization of uricase, to realize gout inhibition through a novel transporter protein regulation pathway to accelerate the uric acid excretion for the first time. After oral administration of hydrogel microspheres, uricase could be controllably released in the intestine to hydrolyze uric acid preliminarily for hydrogen peroxide production. Once exposed in the intestinal specific catalase, hydrogen peroxide would further generate a local hyperoxic microenvironment beneficial for dopamine polymerization, thereby immobilizing uricase in intestinal mucosa finally. Through this cascade reaction, only a small amount of uricase is required for local immobilization to in‐situ up‐regulate uric acid transporter proteins of intestinal epithelial cells, excreting uric acid from the epithelial cells into the intestines to be excreted, lowering the systemic uric acid level. In vitro and in vivo studies confirmed the precisely intestinal delivery of dopamine and uricase through intestine‐targeted explosive microspheres. In vitro evaluation demonstrated that the uric acid level in human fecal samples from gout patients could be significantly reduced by up to 37% by the mimic mucosa‐immobilized uricase on the isolated porcine tissues. In vivo experiments in mouse hyperuricemia model and acute gouty arthritis model confirmed that microspheres could effectively accelerate intestinal uric acid excretion, effectively increasing fecal uric acid excretion by up to 27.6% and reducing blood uric acid by up to 71%. In addition, 16S ribosomal RNA sequencing showed that the microspheres optimized intestinal flora composition, with a significant increase in probiotic bacteria such as Akkermansia muciniphila and Verrucomicrobiales, and inhibited some common intestinal bacteria in gout patients such as Bacteroidetes, thereby improving intestinal homeostasis. In conclusion, we developed a unique inducing pathway via the small intestine mucosal layer in‐situ regulation to firstly realize an efficient uric acid intestinal excretion for gout therapy.
