Abstract:Despite numerous advances in the recognition, assessment, and management of pain in neonates over the past two decades, there has been limited improvement in the knowledge base regarding parental responses to their infant's pain. This study examined parents' views of their experiences observing and coping with their infant's pain in the neonatal intensive care unit (NICU). Twelve participants were recruited using purposive sampling from two groups: (a) parents who had infants currently receiving care in the NICU (n=6); and (b) parents whose infants had been discharged from the NICU and were enrolled in the outpatient follow-up clinic at each hospital (n=6). An exploratory, semi-structured format was used to interview parents individually (n=5) or in focus groups (n=7) regarding their infant's clinical course, infant pain experiences, and the parenting experience during and after the NICU stay. Thematic content analysis was used to develop conceptual categories. Two broad themes were identified: (a) infant pain as a source of parental distress and (b) relief of parental distress due to infant's pain.

Abstract:Background: There is a knowledge-to-practice gap regarding parent-delivered pain management, and few studies have investigated parents' and nurses' participation in and acceptance of combined parent-delivered pain-alleviating interventions such as skin-to-skin contact (SSC), breastfeeding, and parental musical presence. This study investigated parents' and nurses' perceptions of and reflections on experiencing combined parent-delivered pain management. Methods: This qualitative study applies a collaborative participatory action research design using ethnographic data collection methods such as focus groups, video observations, and video-stimulated recall interviews with parents and nurses. Results: The results concern three main categories, i.e., preparation, participation, and closeness, as well as various sub-categories. Preparations were central to enabling combined parent-delivered pain management. Participation was facilitated by parental musical presence, in which parents shifted their attention toward their infant. Closeness and presence during neonatal care helped parents become active during their infant's painful procedures. Parental lullaby singing created a calm and trusting atmosphere and after the procedure, both parents and nurses felt that they had successfully supported the infant through a potentially painful procedure. Conclusions: Mental and practical preparation is central to implementing combined parent-delivered pain management. When parents and nurses explored the interventions, they found the methods feasible, promoting self-efficacy and confidence in both parents and nurses.

Abstract:In daily practice in the neonatal intensive care unit (NICU), newborns are subjected to a series of procedures and therapeutic interventions that cause pain; Pain management in the newborn is important within specialized care in neonatology (Gallegos-Martínez & Salazar-Juárez, 2010). Patricia Benner in her theory points out that, as the professional acquires this experience, clinical knowledge becomes a mixture of practical and theoretical knowledge. It is important to mention that nursing sees the human being as a holistic being in each of the stages of his life. Objective: Identify the level of knowledge about pain management in the Neonatal Intensive Care area by the nursing professional Methodology: A quantitative, descriptive study was carried out, its type of research is prospective, longitudinal. The subjects were nursing staff. With a universe of 35 nurses from all shifts and a sample of 20 participants at the convenience of the NICU area from different shifts (morning, afternoon, night and cumulative shift). An instrument was used to measure knowledge of neonatal pain by nursing professionals in neonatal intensive care units carried out by Edwin Millán Flores and validated by the Ana G. Méndez University System (SUAGM) in Puerto, Rico. Results: The level of knowledge about neonatal pain management education was 35.0% of the nursing staff had a good level of knowledge and 45.0% had an excellent level. Conclusions: Although we have the knowledge, the nursing professional Benner tells us that we must reach the experts, therefore, it is necessary to design tools or strategies that take us to the experts, to apply the knowledge and share it with the other personnel in the area.
Effectiveness of Non-Pharmacological Methods, Such as Breastfeeding, to Mitigate Pain in NICU Infants

Abstract:Neonates do experience pain and its management is necessary in order to prevent long-term, as well as, short-term effects. The most common source of pain in the neonatal intensive care unit (NICU) is caused by medically invasive procedures. NICU patients have to endure trauma, medical adhesive related skin injuries, heel lance, venipuncture and intramuscular injection as well as nasogastric catheterization besides surgery. A cornerstone in pain assessment is the use of scales such as COMFORT, PIPP-R, NIPS and N-PASS. This narrative review provides an up to date account of neonate pain management used in NICUs worldwide focusing on non-pharmacological methods. Non-steroidal anti-inflammatory drugs have well established adverse side effects and opioids are addictive thus pharmacological methods should be avoided if possible at least for mild pain management. Non-pharmacological interventions, particularly breastfeeding and non-nutritive sucking as primary strategies for pain management in neonates are useful strategies to consider. The best non-pharmacological methods are breastfeeding followed by non-nutritive sucking coupled with sucrose sucking. Regrettably most parents used only physical methods and should be trained and involved for best results. Further research in NICU is essential as the developmental knowledge changes and neonate physiology is further uncovered together with its connection to pain.

Neonatal Nurses' Perceptions of Pain Management: Survey of the United States and China.

Abstract:Despite growing knowledge, neonatal pain remains unrecognized, undertreated, and generally challenging. A cross-sectional survey study was conducted to investigate neonatal nurses' perceptions, knowledge, and practice of infant pain in the United States and China, including 343 neonatal nurses (American nurses [n = 237]; Chinese nurses [n = 106]). Nurses' responses regarding neonatal pain reflected adequate knowledge in general pain concepts, but knowledge deficits related to several topics were found (e.g., preterm infants are more sensitive to pain and long-term consequences of pain). Most reported regular use of pain assessment tools, but fewer agreed that the tool used was appropriate and accurate. More American nurses (83%) than Chinese nurses (58%) felt confident in the use of pain medications, while more Chinese nurses (78%) than American nurses (61%) acknowledged the effectiveness of nonpharmacologic interventions. About half reported that pain in their units was well managed (American: 44.3%; Chinese: 55.7%), and less than half felt that pain guidelines/protocols were research-based (American: 42.6%; Chinese: 34.9%). Nurses' perceptions of well-managed pain in their units were significantly correlated with adequate education/training, use of accurate tools, and use of research-based protocols. Barriers to effective pain management included resistance to change, lack of knowledge, lack of time, fear of side effects of pain medication, and lack of trust in the tools. The survey reflects concerns that pain has not been well managed in many neonatal intensive care units in the United States and China. Further actions are needed to solve the issues of inadequate training, lack of clinically feasible pain tools, and absence of evidence-based guidelines/protocols.

Abstract:When an infant is admitted to the neonatal intensive care unit (NICU), the ability of parents to spend time with and provide meaningful care for their infant is often restricted. Added to this is the distress parents may be feeling as a result of the premature birth of their child, as well as the intimidating and technological environment of the NICU. These factors strain the development of an optimal parent-infant relationship, which can have effects that last beyond discharge. A solution to this problem is to offer parents the opportunity to be the primary caregivers for their infant in the NICU. This article reviews the development and theory of the care-by-parent model, including examples of successful programs, discusses the benefits and challenges of the model, and looks to the future of care in the NICU.

Abstract:Objective: To better understand the experience of parents with neonates with congenital heart diseases (CHD) admitted to a neonatal intensive care unit (NICU) in order to identify challenges faced by parents and discover support strategies helpful in positive coping. Study design: Prospective cohort study of parents of neonates with CHD. Parents completed a questionnaire with open ended questions regarding their experience and feeling during the hospitalization within one week of the child discharge from the NICU. Krippendorff's content analysis was used to examine data. Results: Sixty-four parents participated. Three themes were highlighted - Dialectical parental experiences, Suboptimal Parental Experiences and Positive Parental Experiences - describing the state of being and feelings that these parents face. Through this analysis, we were able to develop clinical considerations and identify coping strategies. Conclusion: The understanding of parental experience and challenges when dealing with their child admitted in the NICU is crucial to identify coping strategies to promote adaptation and enhance the development of positive coping mechanisms.

Abstract:Purpose: Children with medical complexity experience frequent pain. But it can be challenging to recognize unexpected behaviors as pain related, especially in the absence of self-report. Often these children undergo extensive workups aimed at diagnosing the sources of pain. This study had three objectives: to describe the signs and symptoms parents of nonverbal children with medical complexity found worrisome, to describe the sources of pain in these children, and to describe nursing pain assessment practices in this population. Design and methods: Retrospective chart review was used to identify the initial presenting symptoms, sources of pain, and nursing documentation for 46 children with medical complexity who were admitted with a chief concern of pain to a 395-bed pediatric teaching hospital in the northeastern United States. Results: Irritability, pain, feeding intolerance, and “not acting like herself [or himself]” were common parent-reported symptoms that prompted further evaluation. On average, five diagnostic studies were taken to identify a source of pain, and four specialty services were consulted during the admission. Nursing assessments of pain were documented approximately every three hours; the mean pain intensity score documented was 1.1 out of 10. The discharge diagnoses included infection (including urinary tract infection), seizures, constipation, chronic pain, failure to thrive, dehydration, and subdural hematoma. Conclusions: The discharge diagnoses covered a wide range. A systematic approach to pain evaluation could help to ensure that the diagnostic process is both thorough and efficient. Common childhood ailments such as constipation or urinary tract infection, as well as other causes, must be considered when diagnosing pain in this population. Practice implications include consulting parents regarding changes in a child's behavior.

Abstract:Pain assessment is an essential foundation to mitigate pain and its consequences in the developing child. However, pain assessment in neonates and infants is challenging and, to date, there is no “gold standard” infant pain indicator, measure, or approach. This chapter encompasses (1) a comprehensive evaluation of the most current and well validated neonatal/infant pain assessment measures; (2) an overview on biomarkers and cortical indicators on neonatal/infant pain; (3) the integration of recommendations on pain-assessment measures and practices within clinical practice guidelines, policies, and procedures; and (4) challenges associated with neonatal and infant pain assessment in terms of research, clinical, and knowledge translation issues.
Implementing family-integrated care in the NICU: a parent education and support program

Abstract:The purpose of this study was to develop, implement, and evaluate a parent education and support program that enhances family-integrated care in a Canadian neonatal intensive care unit (NICU). A total of 39 mothers of infants born at 35 or fewer weeks' gestation were enrolled in the pilot program. We examined the development, implementation, and qualitative assessment of the education component of a family-integrated care program. We enrolled in groups of 4 or 5, the study mothers agreed to attend daily educational sessions, provide care for their infants for at least 8 hours daily, and participate in medical rounds. The educational sessions were provided by staff and veteran parents to assist parents' development of confidence in providing caregiving skills and assuming the role of a primary caregiver for their infants as they moved closer to discharge. Effectiveness of the program was evaluated through anecdotal feedback and a formal evaluation process at discharge. The results indicated that the mothers were provided with the tools to parent their infants in the NICU, recognize their own strengths, increase their problem-solving strategies, and emotionally prepare them to take their infant home. Feedback from the participants provided direction to adapt the program to provide optimal parent support and education. Parental education is a valued and vital component of family-integrated care in the NICU.

Abstract:Aim To evaluate the impact of an eHealth educational resource about infant procedural pain management, given during the prenatal period, on feasibility, acceptability, knowledge, self‐efficacy, and involvement. Background Routine health care requires newborns to have painful procedures (e.g., intramuscular injection). The impacts of untreated pain in neonates are widely recognised but adoption of effective procedural pain management strategies in clinical practice varies. There is clear evidence supporting the effectiveness of parent‐led pain management during procedures (e.g., skin‐to‐skin care) and reputable resources to raise awareness among parents are warranted. Our team co‐created Parenting Pain Away, a website to equip parents with evidence to assist with managing the pain of procedures and empower them to be involved. Methods A quasi‐experimental evaluation using a pre/post intervention design with low‐risk expectant parents. Results Of the 41 participants, before intervention exposure participants were familiar or had used skin‐to‐skin care (n = 33), breastfeeding (n = 30) and sucrose (n = 13) as pain management. Most participants (n = 38) desired more information on how to be involved. Providing access to Parenting Pain Away during pregnancy was supported and participants ranked the website above average using the System Usability Scale. Parenting Pain Away did not have a statistically significant influence on outcomes. Participants reported variation in clinical support with parent‐led pain management. Conclusion A multifaceted approach is recommended to maintain infant procedural pain management. Implications for the Profession and Patient Care Equipping parents with knowledge related to infant pain management using an eHealth approach satisfied their information desires. The study findings are important considerations for perinatal care providers, policy makers, and families to finally achieve adequate procedural pain management. Reporting Method This study used the STROBE checklist, adhering to EQUATOR guidelines. Patient and Public Contribution A stakeholder group (expectant parents, parents, perinatal researchers, clinicians, and administrators) was created to inform the study design and intervention.

Enhancing NICU Care and Communication: Perspectives of Moderately Preterm Infant Parents

Abstract:Background: Moderately preterm infants (MPTI) comprise a large proportion of NICU admissions and are an understudied population. The unique experience of families with MPTIs has yet to be examined in the literature. Describing MPTI parent needs and preferences may inform interventions to improve care and outcomes for this population. Methods: Semistructured qualitative interviews were performed with English-speaking birth parents of infants born between 32 and 34 weeks gestation to describe their NICU experience and identify areas for improvement specifically surrounding care team inclusion, education, discharge, and communication. Interviews were recorded, transcribed, and analyzed using directed content analysis. Enrollment ceased when the data reached thematic saturation. Results: Sixteen birth parents participated. Four themes emerged around parent-medical team connectedness, parental confusion, discharge readiness, and the desire for a use of a mix of in-person and electronic communication methods (e-mail, texting, apps, etc) for communication. MPTI parents valued a strong connection with the medical team; however, they described a lack of knowledge regarding the reasons for admission and ongoing management. Near discharge, parents desired more information regarding feeding, reflux, and breathing patterns. Parents preferred in-person discussions but described a role for electronic methods to improve their understanding of their infant and discharge readiness. Conclusions: From the MPTI parent perspective, clinicians can focus improvement efforts on communication, specifically around reasons for admission, discharge planning, and anticipatory guidance. These results may serve as a foundation for initiatives to improve the MPTI parent experience and potentially parent and MPTI outcomes.

Abstract:With the frequency of infant deaths in the United States, many attributed to congenital malformations and prematurity, the neonatal intensive care unit (NICU) nurse must be adept at planning and providing perinatal palliative care. The NICU nurse requires education and training to proficiently contribute to the care planning and delivery of care and facilitate memory-making activities. The provision of perinatal palliative care may precipitate moral distress and needs to be addressed with education and resilience-fostering activities. To further perinatal palliative care effectiveness, research needs to be conducted.

Abstract:Moderate-to-severe pain arising from injuries, diseases, and medical procedures is inevitable during infancy and childhood, yet pain is often unrecognized, ignored, poorly assessed, underestimated, untreated, or poorly treated. Ineffective care for clinically significant acute and chronic pain typically reflects a lack of scientific understanding, inadequate treatments, and insufficient education of healthcare professionals in evidence-based care. Infants and older children warrant special care. Despite a moral responsibility to prevent and ameliorate pain and suffering in children, there is substantial evidence of systemic and individual failures to prevent and control their pain, despite known, evidence-supported, and cost-effective interventions in both the developing and developed world. Instructive perspectives from the philosophy of ethics focus on the consequences of pain, delineating intrinsically bad and destructive features, the intentions of parties responsible for inflicting and relieving pain, specific imperatives addressing human rights concerning inflicting and relieving pain, and characteristics of virtuous people committed to ethical caring. We address ethical implications of the inadequacies of our understanding and control of pediatric pain, as well as barriers preventing important research in the field. Problematic issues concern dominance of biomedical care, rather than care based on a biopsychosocial model of pain, and the human capacity for cognitive biases in interactions with other people. Ethical obligations of healthcare professionals are addressed. Attention is directed to an online training resource for ethical practice in caring for children’s pain. Specific issues are also addressed, including clinician competence, free and informed consent and assent confidentiality, and assessment and treatment choice.
The emotional experiences and supports for parents with babies in a neonatal nursery

Abstract:Purpose: The experience of the neonatal intensive care unit (NICU) for parents can be anxiety-provoking, fearful, and distressing. To help parents cope with these overwhelming feelings, a number of interventions, including parent support groups, are typically offered. It is hoped that the provision of these groups and other forms of emotional support lessen the distressing experience for parents and lessen the anxiety of hospital discharge. This study focuses on the emotional reactions during the transition to home from the NICU for parents who participated in one such support group. Methods: Parents were interviewed 4 to 6 months after discharge of their baby from hospital. Results: Themes from these interviews included anxiety and concern about the baby's readiness for discharge, concerns about the risks of further illness and rehospitalization, and whether the parent felt prepared sufficiently to care for his or her baby at home. Recalling their time in the nursery was distressing for parents, but despite this, they identified that positive staff interactions helped them through the hardest times in the NICU. Conclusions: The support group was reported to be effective and helpful for parents. Parents often maintained social contact with other parents and recalled advice and supportive information from the group as needed. Participants recommended that the support group continue to assist other parents.
Nonpharmacological Interventions to Mitigate Procedural Pain in the NICU: An Integrative Review

Abstract:Background: Small infants experience a myriad of stimuli while in the Neonatal Intensive Care Unit (NICU), with many being painful or stressful experiences, although medically necessary. Purpose: To determine what is known about nonpharmacological developmental care interventions used in the NICU to mitigate procedural pain of infants born under 32 weeks gestation. Search/strategy: Five electronic databases were searched: Medline, CINAHL, Scopus, Embase and the Cochrane Library. The inclusion criteria were as follows: experimental and nonexperimental studies from all publication years with infants born at less than 32 weeks gestational age; peer-reviewed research articles studying nonpharmacological interventions such as skin-to-skin care, facilitated tucking, nonnutritive sucking, hand hugs, and swaddling; and English language articles. Our search yielded 1435 articles. After the elimination of 736 duplicates, a further 570 were deemed irrelevant based on their abstract/titles. Then, 124 full-text articles were analyzed with our inclusion and exclusion criteria. Findings: Twenty-seven studies were reviewed. Sucrose, facilitated tucking, pacifier, skin-to-skin care, and human milk appeared to lessen pain experienced during heel sticks, suctioning, nasogastric tube insertions, and echocardiograms. All nonpharmacological interventions failed to prove efficacious to adequately manage pain during retinopathy of prematurity (ROP) examinations. Implications for practice: Evidence review demonstrates that healthcare practitioners should use nonpharmacological measures to help prevent pain from day-to-day procedures in the NICU including heel sticks, nasogastric tube insertions, suctioning, echocardiograms, and subcutaneous injections. Implications for research: Future research is necessary to better understand and measure how pain is manifested by very small premature infants. Specific research on mitigating the pain of examinations for retinopathy of prematurity is also needed.

Abstract:BACKGROUND: Parents have reported that they want to learn how to reduce pain in infants during vaccinations. Our objective was to compare different levels of intensity of postnatal education about pain mitigation on parental self-reported use of interventions at future infant vaccinations.METHODS: We conducted a longitudinal, 3-group parallel, add-on, randomized controlled trial on the postnatal ward of a hospital. New mothers, unaware of the hypothesis, were randomly assigned to 1 of 3 intervention groups and 3 follow-up groups (i.e., 9 groups, 3 × 3). The 3 intervention groups were control (general immunization information), pain pamphlet (pain mitigation information), and pain pamphlet and pain video (pain mitigation information). Both pain mitigation education groups also received general immunization information. The 3 follow-up groups were 2-, 4- and 6-month infant vaccinations. Mothers reported use of breastfeeding, sucrose and topical anesthetics during infant vaccinations in a telephone survey.RESULTS: Of 3420 participants, follow-up was available for 2549 (75%): 36.1%, 34.2% and 29.7% reported on pain mitigation practices at 2-, 4- and 6-month vaccinations, respectively (p = 0.9). Maternal characteristics did not differ (p > 0.05): mean age, 33.6 years; 58% were primipara. Utilization of any intervention (breastfeeding, sucrose or topical anesthetics) was 53.2%, 61.4% and 63.0% for control, pain pamphlet, and pain pamphlet and pain video groups, respectively (p < 0.001); both pain education groups had higher utilization than the control group, but did not differ from one another. Uptake differed among intervention groups at 2 and 4 months but not at 6 months.INTERPRETATION: Hospital-based postnatal education increased parental use of pain interventions at infant vaccinations and can be added to existing education.TRIAL REGISTRATION: ClinicalTrials.gov, no. Abstract:Care and outcomes of infants admitted to neonatal intensive care vary and differences in family-centered care may contribute. The objective of this study was to understand families’ experiences of neonatal care within a framework of family-centered care. We conducted focus groups and interviews with 18 family members whose infants were cared for in California neonatal intensive care units (NICUs) using a grounded theory approach and centering the accounts of families of color and/or of low socioeconomic status. Families identified the following challenges that indicated a gap in mutual trust and power sharing: conflict with or lack of knowledge about social work; staff judgment of, or unwillingness to address barriers to family presence at bedside; need for nurse continuity and meaningful relationship with nurses and inconsistent access to translation services. These unmet needs for partnership in care or support were particularly experienced by parents of color or of low socioeconomic status.
