Abstract:Brain injury in preterm infants is a leading cause of mortality and disability in children, bringing a heavy economic burden to families and society [ 1 ]. Nonetheless, we still lack a deep understanding of its pathogenesis and effective treatment. Recent evidence found that brain injury in preterm infants was a chronic disorder of aberrant brain development. In 2005, Volpe et al . first proposed the concept of 'encephalopathy of prematurity', which included the development of abnormalities of white matter and neuron [ 2 ]. Then, in 2009, Volpe et al . reported that the abnormalities of brain development and maturation were secondary to primary brain injury, including both white matter (axons and subplate neurons) and gray matter (cerebral cortex, thalamus, and basal ganglia) [ 3–5 ]. More recently, in 2021, they reported that neuronal maturation abnormalities were primary, but not secondary to white matter damage [ 6 ]. Therefore, aberrant neuronal differentiation and maturation may be the leading cause of brain injury in preterm infants. However, the molecular mechanisms for this remain unclear.
