Abstract:The recurrence of outbreaks of vaccine-preventable diseases in close-knit, underimmunized communities in the US underscores the importance of effective and adaptable vaccine promotion efforts. In their article, Martoma and colleagues 1 elegantly describe the transmission details of the most recent measles outbreak to occur in the US. This outbreak affected a close-knit, underimmunized community in central Ohio between 2022 and 2023. Following international importation, transmission occurred exclusively among individuals of Somali descent—noting that Columbus, Ohio is home to the second largest Somali community in the US—but then spread to underimmunized individuals outside of this community via community and nosocomial transmission. Among the cases identified, 100% were either underimmunized or completely unimmunized against measles. Lower immunization coverage is associated with vaccine-preventable disease outbreaks and threatens the elimination status of measles and other vaccine-preventable diseases in the US. Martoma and colleagues 2 estimated 53% measles immunization coverage (well below the needed 95% coverage) among the affected population in Ohio, setting the stage for the central Ohio outbreak. In measles outbreaks among similar close-knit, underimmunized communities, measles immunization rates were also low compared with national coverage rates. For example, first-dose measles immunization coverage was 36% among Somali children in Hennepin County, Minnesota, leading to a 2017 measles outbreak following international importation. 3 Similarly, there was 77% estimated measles immunization coverage among an Orthodox Jewish community during a 2018 outbreak involving 242 cases in Rockland County, New York. 4 In identifying the potential underpinnings of lower vaccine coverage among the affected Somali community, Martoma and colleagues 1 highlight the role of vaccine misinformation, which has gained new ground since the pandemic and perpetuates myths about vaccination, ultimately leading to underimmunization. They note parents' still pervasive concern about a potential association between measles vaccination and autism although this myth has been rigorously debunked in multiple studies. This idea—that the measles, mumps, and rubella (MMR) vaccine is associated with autism—has been promoted among several underimmunized communities in which measles outbreaks have occurred, 5 suggesting a need for trusted evidence-based information tailored to these communities. To prevent the next outbreak of vaccine-preventable disease, public health and medical communities must leverage and nimbly adapt evidence-based strategies to address historical and current misinformation and other barriers to immunization uptake. As Martoma and colleagues 1 state, the successful containment of disease transmission occurred thanks to a rapid public health response involving efforts and collaboration among federal, state, and local entities. We believe that an equally important rapid response requires the bolstering of vaccine promotion efforts to prevent outbreaks before they occur. We propose that immunization could be promoted through a multipronged approach that acknowledges the diverse experiences influencing vaccine decision-making, particularly among close-knit communities, such as the Somali community in central Ohio. This multipronged approach would include (1) acknowledging the diverse barriers to vaccination among distinct communities, (2) training health care professionals (HCPs) in evidence-based strategies for vaccine communication and their use in culturally competent ways, (3) promoting diversity in the health care sector, (4) strategically involving community stakeholders in traditionally underimmunized communities, and (5) strengthening state and federal policies to promote immunization. Acknowledging and addressing the diverse causes of vaccine hesitancy should direct our vaccine promotion efforts. Defined as delay or refusal of a vaccine despite its availability, vaccine hesitancy is a fluid concept, and patients' concerns may differ for each vaccine and reflect the sociodemographic, geographic, historical, political, or social experiences of an individual or population. The concern about autism associated with MMR despite evidence contrary to this claim is pervasive and has been identified as a prevalent concern in communities where measles outbreaks have occurred, including the Somali community in central Ohio. 5 However, other communities identify other barriers to immunization, such as concerns about adverse reactions, safety, the total number of vaccines administered, and uncertainty about the immunization schedule.
