Abstract:Social media platforms actively moderate content glorifying harmful behaviors like eating disorders, which include anorexia and bulimia. However, users have adapted to evade moderation by using coded hashtags. Our study investigates the prevalence of moderation evaders on the popular social media platform TikTok and contrasts their use and emotional valence with mainstream hashtags. We notice that moderation evaders and mainstream hashtags appear together, indicating that vulnerable users might inadvertently encounter harmful content even when searching for mainstream terms. Additionally, through an analysis of emotional expressions in video descriptions and comments, we find that mainstream hashtags generally promote positive engagement, while moderation evaders evoke a wider range of emotions, including heightened negativity. These findings provide valuable insights for content creators, platform moderation efforts, and interventions aimed at cultivating a supportive online environment for discussions on mental health and eating disorders.

The Paradox of Tik Tok Anti-Pro-Anorexia Videos: How Social Media Can Promote Non-Suicidal Self-Injury and Anorexia

Abstract:The literature shows that social pressure promotes non-suicidal self-injury (NSSI) Eating disorders, along with self-injury, are also favored by underregulated social media. Tik Tok is one of the most used social media platforms among adolescents. It has been shown that the time young children spend on this platform doubled during the lockdown. The theme of anorexia is very common on this platform. While most “pro-ana” (pro-anorexia) videos, where users exchanged advice on how to pathologically lose weight, have been censored by the application, other “anti-pro-ana” (anti-pro-anorexia) videos, officially aimed at raising awareness of the consequences of anorexia, have become increasingly popular. However, our case shows how even these safer videos paradoxically lead the users to emulate these “guilty” behaviors.

Abstract:Background: Social media platforms that are based on the creation of visual media, such as TikTok, are increasingly popular with adolescents. Online social media networks provide valuable opportunities to connect with each other to share experiences and strategies for health and wellness. Objective: The aim of this study was to describe the content of the hashtag #mentalhealth on TikTok. Methods: This cross-sectional, descriptive content analysis study included 100 videos with the hashtag #mentalhealth on TikTok. All videos that included the hashtag #mentalhealth were analyzed and coded for the presence of content categories. Additionally, the comments to each video were viewed and coded for content in the following themes: offering support or validation; mentioning experience with suicide or suicidal ideation; mentioning experience with self-harm; describing an experience with hospitalization for mental health issues; describing other mental health issues; and sharing coping strategies, experiences of healing, or ways to feel better. Results: Collectively, the 100 videos studied received 1,354,100,000 views; 266,900,000 likes; and 2,515,954 comments. On average, each video received 13,406,930.69 (SD 8,728,095.52) views; 2,657,425.74 (SD 1,449,920.45) likes; and 24,910.44 (SD 21,035.06) comments. The only content category observed in most (51/100, 51%) of the videos included in the sample was "general mental health." The remaining content categories appeared in less than 50% of the sample. In total, 32% (32/100) of the videos sampled received more than the overall average number of likes (ie, more that 2.67 million likes). Among these 32 videos, 23 (72%) included comments offering support or validation and 20 (62%) included comments that described other mental health issues or struggles. Conclusions: With over 1 billion cumulative views, almost half of the assessed TikTok videos included in this study reported or expressed symptoms of mental distress. Future research should focus on the potential role of intervention by health care professionals on social media.
A Functional Approach to the Assessment of Non-Suicidal Self-Injury on YouTube

Abstract:This research explores representations of non-suicidal self-injury (NSSI) on social media. The current study aims to examine the applicability of a functional model of NSSI to videos posted on YouTube by analyzing the 100 most viewed YouTube videos relating to NSSI at a point in time in 2018. Motivations for continued engagement in NSSI were examined for each content creator who posted one of the top 100 most viewed videos relating to NSSI. Categorizations of a content creator's motivation were based on Nock and Prinstein (2004) four-function model (FFM) of NSSI, in which the motivations, or functions, of NSSI are categorized by two dichotomous factors: (a) positive (i.e., involves the addition of a favorable stimulus) or negative (i.e., involves the removal of an aversive stimulus) and (b) automatic (i.e., intrapersonal) or social (i.e., interpersonal). Uploaders most endorsed automatic negative reinforcement (50%), followed by automatic positive (26%), social positive (3%), and social negative (3%). Considering the growing number of children and adolescents with near constant access to internet and social media, there may be implications for further development of safety measures for social media platforms.ï»¿ The results of this study may be valuable for parents, health care professionals, and educators, who should be aware of available social media content relating to NSSI and consider implementing additional media-literacy and media-safety education in school curriculums and programming for children and adolescents. Furthermore, health care professional should be aware that misconceptions of NSSI may have an impact on patients' willingness to seek treatment, or the attitude with which treatment is sought.
Applying the uses and gratifications theory to identify motivational factors behind young adult's participation in viral social media challenges on TikTok

Abstract:Social media usage can impact its users’ mental and physical health. In particular, social media challenges, activities performed by an individual or a group that are uploaded to a social networking site to achieve a specific goal, have the potential to encourage and spread positive and negative behavior. We conducted interviews with thirty-two college students (ages 18-23) and using qualitative analysis grounded in the Uses and Gratifications Theory determined the motivations underlying participation in these challenges on the rapidly growing and unique social media platform TikTok. Six categories consistent with the Uses and Gratifications Theory emerged from this analysis, indicating that individuals use TikTok for 1) entertainment, 2) convenience and utility for widespread communication, 3) increasing social interaction, 4) finding social support, 5) seeking and sharing information, and 6) escaping from everyday life. We discuss how these motivations can be used to minimize the spread of viral self-harm challenges through TikTok.
Self-Expression and Sharing around Chronic Illness on TikTok

Abstract:While prior work has investigated the benefits of online health communities and general purpose social media used for health-related purposes, little work examines the use of TikTok, an emerging social media platform with a substantial user base. The platform's multimodal capabilities foster creative self-expression, while the content-driven network allows users to reach new audiences beyond their personal connections. To investigate users' challenges and motivations, we analyzed 160 TikTok videos that center on users' first hand experiences living with chronic illness. We found that users struggled with a loss of normalcy and stigmatization in daily life. To contend with these challenges, they publicly shared their experiences to raise awareness, seek support from peers, and normalize chronic illness experiences. Based on our findings, we discuss the modalities of TikTok that facilitate self-expression around stigmatized topics and provide implications for the design of online health communities that better support adolescents and young adults.

Abstract:Non-suicidal self-injury (NSSI) is a common phenomenon among adolescents, but is often not disclosed due to fear of stigmatization. Social media is frequently used to publish photos of NSSI and share experiences with NSSI. Objectives of this study were to find out more about the motivation for publishing NSSI content and to investigate the effect that sharing this content on social media has on young people. In the current study, we interviewed N=59 participants (mean age = 16.7 years [SD = 1.2 years]; 72.9% female), who had all posted NSSI content within the past month on the social media platform Instagram. Semi-structured interviews were conducted via the Instagram messaging app. Interviews were analyzed qualitatively, assisted by the Software Atlas.ti 7. Participants were asked about their motivation for and their experiences with posting NSSI content online. Motivations for posting pictures online were mainly social (connecting, disclosure, communicating), while self-focused reasons like documenting NSSI or recovery were also mentioned. All participants reported having received positive reactions (being offered help, connecting, receiving empathy), as well as negative comments (harassment, being misunderstood) to their own NSSI content by other Instagram users. Participants' reactions to other users' NSSI content on Instagram was often identification with the content or being triggered, but also wanting to offer help or sometimes even being deterred from NSSI. None of the participants mentioned successful referral to professional help through their online NSSI activity. One target for future interventions could therefore be social media, or other online platforms, where adolescents might be more easily reached. Mental health practitioners should be aware of their clients' online activity and encourage reflection upon positive and negative effects of viewing or sharing NSSI content online.

TikTok and teen mental health: an analysis of user-generated content and engagement

Abstract:Background: TikTok is a social media mobile application that is widely used by adolescents, and has the potential to serve as a revolutionary platform for public and mental health discourse, education, and intervention. Objective: Our study aimed to describe the content and engagement metrics of the hashtag #teenmentalhealth on TikTok. Methods: In this study, we: (a) conducted a directed content analysis of the Top 100 TikTok videos tagged with #teenmentalhealth, and (b) collected data on video engagements (views, likes, saves, and shares) and computed view-based engagement rates. Results: The videos collectively garnered 144,320,591 views; 28,289,655 likes; 219,780 comments; 1,971,492 saves; and 478,696 shares. Most of the generated content were from teens and therapists. Engagement metrics revealed strong user engagement rates across user types. The most prevalent content categories represented across videos were personal experience, coping techniques or treatment, humor, interpersonal relationships, and health campaign. The content categories with the highest engagement rates were relatable media representation, health campaign, social isolation, and humor. Only a single video incorporated evidence-based treatment content. Conclusion: TikTok facilitates communication and information dissemination on teen mental health. Future research should focus on improving the quality and credibility of digital content while maintaining engagement through creativity, self-expression, and relatability. Use of popular social media platforms and community-engaged research to disseminate evidence-based content may help bridge the translational research gap.
TikTok use and psychosocial factors among adolescents: comparisons of non-users, moderate users, and addictive users

Abstract:Short-video applications like TikTok are increasingly popular. This study examines the association between short-video application use (SVU) and psychosocial factors in 1,346 adolescents (M age = 14.97, 51.8% female). 199 non-users and 1147 users (686 moderate users, 461 addictive users) were identified. Results revealed a high prevalence of addictive SVU in the sample. Addictive users exhibited worse mental health conditions than non-users and moderate users, including higher levels of depression, anxiety, stress, loneliness, social anxiety, attention problems, and lower life satisfaction and sleep quality. Addictive users also faced higher academic stress, poorer academic performance, more bullying victimization, worse parental relationships, more negative parenting styles, and lower parental education levels. Moderate users had different family environments than non-users, but no differences in mental health or school performance. Together, these findings suggest that addictive users experience a more disadvantageous situation across mental health, family, and school conditions, while non-users have advantageous family environments. Moderate SVU may not be associated with negative mental health condition or poor school performance. Moderate and addictive SVU should be considered distinct phenomena. Given the psychiatric symptoms present in addictive users of TikTok and similar apps, targeted interventions and treatments are urgently needed.

Helpful or Not? A Qualitative Study on Female Adolescents' Experience of TikTok When Recovering From Anorexia Nervosa

Abstract:Objective: The scientific literature reports on how social media potentially influences eating disorders, although there is a large gap in the specific case of TikToks influence of adolescent's recovery from anorexia nervosa (AN). Our study uses in-depth interviews with female adolescents primarily suffering from AN to explore how they perceive the social media platform TikTok in relation to their recovery. Method: A total of 14 interviews with female adolescents recovering from AN were conducted and analyzed with reflexive thematic analysis. Results: We developed four distinct main themes: social connectivity, algorithmic engagement, regulation and adaptation, and personal agency and recovery pathways. Our findings indicate both potential benefits and harm by TikTok use when in recovery from AN, depending on a complex interplay of individual and contextual factors. Discussion: The study adds nuance to the on-going scientific debate on the role that TikTok plays in recovery from AN in general from the perspective provided by female adolescents. Suggestions are made for clinical implications at adolescent AN outpatient care including parental or professional support in TikTok adaptations and advice on how to discern when use may be triggering or supportive. Future research would benefit from longitudinal designs and inclusion of how individual differences, such as gender and personality, influence the effects on recovery and TikTok use.
Safe Spaces or Toxic Places? Content Moderation and Social Dynamics of Online Eating Disorder Communities

Abstract:Social media platforms have become critical spaces for discussing mental health concerns, including eating disorders. While these platforms can provide valuable support networks, they may also amplify harmful content that glorifies disordered cognition and self-destructive behaviors. While social media platforms have implemented various content moderation strategies, from stringent to laissez-faire approaches, we lack a comprehensive understanding of how these different moderation practices interact with user engagement in online communities around these sensitive mental health topics. This study addresses this knowledge gap through a comparative analysis of eating disorder discussions across Twitter/X, Reddit, and TikTok. Our findings reveal that while users across all platforms engage similarly in expressing concerns and seeking support, platforms with weaker moderation (like Twitter/X) enable the formation of toxic echo chambers that amplify pro-anorexia rhetoric. These results demonstrate how moderation strategies significantly influence the development and impact of online communities, particularly in contexts involving mental health and self-harm.

Abstract:In recent years, there has been a significant surge in non-suicidal self-injury (NSSI)-related content on the Internet. In fact, the technology landscape rapidly evolved, with social media now playing a pivotal role in the lifestyle of the general population, particularly among youths and adolescents. The current review aims at overviewing all studies so far published investigating the relationship between social media and NSSI among adolescents, focusing on the motivations behind posting NSSI-related contents and the role that social media may play in the promotion and/or maintenance of NSSI behaviors. A comprehensive systematic literature review was carried out by using MEDLINE/PubMed. Following screening through established inclusion criteria, eleven studies were finally selected and here retrieved. Due to the high methodological heterogeneity, all studies were assessed and categorized, based on the type of data collection, in three groups: a) studies using direct data (e.g., interviews/surveys); b) indirect data (e.g., content analysis on social media); and, c) mixed data. Major findings reported that social media may significantly influence the development and maintenance of NSSI behaviors. Those victims of cyberbullying often seek out pro-self-harm contents, to both normalize the behavior and/or seek peer-to-peer emotional support. Social media platforms facilitate the dissemination of self-harm techniques and encourage competition among users, exacerbating the severity of self-injury. Interviews highlighted motivations such as attention-seeking, expressing emotions, and feeling part of a group. Other studies showed that social media contents often contain both supportive and harmful advice regarding self-harm. Social media could cover a dual role in both providing support and triggering self-harm through exposure to harmful content(s). Furthermore, social media could also offer opportunities for positive interventions (i.e., awareness campaigns, mental health resources, etc.). Mental health professionals should be aware about the impact of social media on youngsters, as well as providing a routine assessment on social media use in their clinical practice. Future research should explore the (bi)directional causal role of social media on NSSI and viceversa, as well as developing preventive and treatment strategies to support young people to disincentive NSSI behaviors via social media.

Abstract:Introduction: Nonsuicidal-self-injury (NSSI)-related content recently emerged on social networking sites (SNS), despite its relationship with NSSI conducts is still unclear. Methods: Hence, the current population-based cross-sectional study investigated the interplay between SNS use, NSSI content engagement, risky social media challenges (RSMCs), in a sample of 404 young adults (aged 18–24), focusing on the influence of problematic social media use (PSMU) and fear of missing out (FoMO). Results: Around 51.5% of the samplewas engaged inNSSI-related contents on SNS, being mostly females (p<0.001), younger (p=0.005), transgender people and nonbinary people (p=0.030) and those who displayed higher PSMU (p<0.001) or FoMO (p=0.031). Around 66.2% of the sample currently practice NSSIs, predominantly among females (p<0.001), those using BeReal (p=0.012), actively looking for NSSIrelated contents on SNS (p<0.001) to be part of a group (p=0.0025) or learn how to practice NSSI (p=0.025). PSMU (p<0.001) and FoMO (p<0.001) emerged as significant predictors of NSSI content engagement, particularly among active seekers. NSSI conducts were significantly predicted by FoMO (p<0.001) and Snapchat (p=0.044), while negatively predicted by male sex (p<0.001), higher educational level (p=0.019) and age at which NSSI-related contents were firstly looked for (p=0.028). Discussion: These findings underline the need to implement preventivepolicies and targeted interventions to monitor NSSI-related contents on SNS, the impact of PSMU and FoMO on NSSI, particularly among youngsters.

Abstract:Introduction Non-suicidal self-injury (NSSI) is defined as any deliberate destruction of one's body tissue, engaged in for reasons that are non-suicidal. Online platforms, notably social media, witness a surge in NSSI-related content, amplified by the COVID-19 pandemic. Young individuals increase video and post uploads, prompting scholarly inquiry into the impact on vulnerable demographics in the online environment. Despite potential benefits, concerns surface regarding content reinforcing self-injurious behavior. The Blue Whale phenomenon exemplifies serious consequences in this digital landscape. Objectives The present study aims at screening the prevalence of NSSIs on SNS among Italian young people. Methods An observational cross-sectional study was conducted by recruiting 373 Italian young people (aged 18-25). Bergen Social Media Addiction Scale (BSMAS), Fear Of Missing Out Scale (FOMO), Inventory of Statements About Self-Injury (ISAS) were administered to investigate the relationship between NSSIs, social media use and frequency and underpinned motivations. Results Overall, 99.7 % (n=372) of participants declared to have used at least one social network. Around 92.5 % (n=345) declared to know Blue Whale Challenge and more than half of the sample (51.5%) referred to have looked for NSSI contents on SNS, mostly (28.7 % (n=107)) have sought for curiosity, 17.7 % (n=66) have sought for help/support. 53.4 % (n=199) of the sample was found to have problematic social media use (PSMU) according to BSMAS. 85 % (n=317) have committed self-injurious gestures in the past, 66.2 % (n=247) practice NSSI currently, most subjects practice them to vent 51.7% (n=193), calm themselves 41.6% (n=155), and punish themselves 30% (n=112). The mean age of transgender and nonbinary subjects (30 % n=112)) who sought/saw content pertaining to NSSIs appears to be lower (p=0.033) than cisgender subjects. Those who searched for content inherent to NSSIs scored higher mean scores on the FOMO (p=0.022) and BSMAS (p=0.013) scales. Those who follow social pages inherent to NSSIs scored higher on the FOMO scale (p=0.035). Subjects who practice NSSIs at their present state, on average, have higher scores on the FOMO and BSMAS scales (p=<.001). Linear regression analysis was conducted showing an association between BSMAS and FOMO (R2=0.199, B=0.260; F(1.371)=92.334; p=<.001). Logistic regression analyses were conducted to define the effects of FOMO, PMSU, sex, and NSSI search on the development of self-injurious conduct. The logistic regression model was statistically significant, χ2 (1)=3.909; p=0.048. Conclusions The study examines NSSI behaviors among young Italian college students on digital platforms, particularly social networks. It stresses the critical need for targeted interventions, addressing concerns like social media addiction, to provide essential mental health support and foster a safer online environment for this population. A Critical Discourse Analysis of Sexual Violence Survivors and Censorship on the Social Media Platform TikTok

Abstract:Sexual violence is a public health threat in the USA and past research has found that survivors who seek support and disclose their experiences tend to have greater positive outcomes, such as positive life changes, reduced post-traumatic stress disorder, and fewer depressive symptoms. While social media have become an important site of disclosure, over-moderation of sex and sexuality on social media platforms due to the passing of the Fight Online Sex Trafficking Act and the Stop Enabling Sex Traffickers Act may inadvertently lead to the removal of survivors' posts and hinder their ability to build communities of support. In line with critical discourse analysis, this study examined the discursive strategies utilized in online disclosures made by sexual violence survivors on the social media platform TikTok to determine if they make use of algospeak, a neologism referring to the strategy of misspelling words or using coded language to avoid detection and deletion by site moderators. Analysis of the data indicated that sexual violence survivors do make use of algospeak, as well as direct and indirect language, to share their disclosures, and that, while TikTok claims to provide space for survivors' stories, its guidelines and approach to language about sex and sexuality may create barriers to building supportive communities.

Abstract:Amidst rising concerns about mental health in adolescents, the role of social media (SM), particularly highly visual platforms such as TikTok, has become a growing focus of investigation. In the extant literature, conclusive evidence is limited by the aggregate analysis of very heterogeneous SM platforms. This systematic scoping review examines the relationship between TikTok and mental health in adolescents. Additionally, it aims to highlight potential interactions between typical developmental processes and the unique aspects of TikTok, exploring their relevance to mental health within this demographic. Following PRISMA guidelines, a systematic search across PubMed, Scopus, Web of Knowledge, and PsychINFO yielded 20 studies that met the inclusion criteria, encompassing a total of 17,336 subjects from 10 countries. The retrieved studies analyzed four main topics related to TikTok use, i.e., the overall impact on adolescents' mental health, risk of problematic use and behavioral addiction, consequences on body image and self-esteem, and possible spreading of mental illness behaviors. While TikTok offers creative opportunities for self-expression and peer connection, the literature raises concerns about its potential negative effects on teens, such as lower life satisfaction, increased risk of "contagion" of certain psychiatric symptoms, and problematic usage patterns. Significant variations in study methodologies and outcome measures were observed. In the current era of evolving SM landscapes, a comprehensive approach informed by developmentally grounded research is critically needed to further understand the interplay between SM and adolescent mental health, providing insights for health authorities and policymakers.

Using TikTok in recovery from substance use disorder

Abstract:Background: There are many effective treatment options for substance use disorder (SUD), yet most individuals with SUD do not seek formal treatment services. Given the rising popularity of TikTok and need to foster innovative means through which to attract and engage individuals with SUD with treatment, we sought to characterize how TikTok users in SUD recovery are using this platform to bolster their recovery support and/or give hope to others who are struggling with substance use. Methods: Our sample consisted of 82 of the most liked TikTok videos related to attempts to cut down on or abstain from substances and/or strengthen SUD recovery. We employed an iterative process to codebook development resulting in codes for demographics, user-sentiment, video type, and mechanisms of recovery-related behavior change. Videos were independently double-coded and evaluated for inter-rater reliability. Results: Video in this sample were heavily viewed, accounting for over 2 million views per video and 325,000 likes on average. Most common video themes were sharing a journey from active SUD to recovery (40.2%) and sharing/celebrating a recovery milestone (37.8%), followed by recurrence of substance use (12.2%). Commonly exemplified mechanisms of recovery-related behavior change included embracing a strong social identity as a person in recovery (81.7%), social support (45.1%), and participation in rewarding alternative activities (39.0%). Conclusion: TikTok SUD recovery-focused videos can potentially reach millions with portrayed themes similar to established therapeutic mobilizers and mechanisms. More research is needed to better understand whether digital recovery narratives can effectively normalize experiences of addiction and help-seeking behaviors.

Abstract:The Human-Computer Interaction (HCI) community has consistently focused on the experiences of users moderated by social media platforms. Recently, scholars have noticed that moderation practices could perpetuate biases, resulting in the marginalization of user groups undergoing moderation. However, most studies have primarily addressed marginalization related to issues such as racism or sexism, with little attention given to the experiences of people with disabilities. In this paper, we present a study on the moderation experiences of blind users on TikTok, also known as "BlindToker," to address this gap. We conducted semi-structured interviews with 20 BlindTokers and used thematic analysis to analyze the data. Two main themes emerged: BlindTokers' situated content moderation experiences and their reactions to content moderation. We reported on the lack of accessibility on TikTok's platform, contributing to the moderation and marginalization of BlindTokers. Additionally, we discovered instances of harassment from trolls that prompted BlindTokers to respond with harsh language, triggering further moderation. We discussed these findings in the context of the literature on moderation, marginalization, and transformative justice, seeking solutions to address such issues.

An Analysis of Depression, Self-Harm, and Suicidal Ideation Content on Tumblr

Abstract:Abstract. Background: Social networking about depression can be indicative of self-reported depression and/or can normalize risk behaviors such as self-harm and suicidal ideation. Aim: To gain a better understanding of the depression, self-harm, and suicidal content that is being shared on Tumblr. Method: From April 16 to May 10, 2014, 17 popular depression-related Tumblr accounts were monitored for new posts and engagement with other Tumblr users. A total of 3,360 posts were randomly selected from all historical posts from these accounts and coded based on themes ascertained by the research team. Results: The 17 Tumblr accounts posted a median number of 185 posts (range = 0–2,954). Content was engaged with (i.e., re-blogged or liked) a median number of 1,677,362 times (range = 0–122,186,504). Of the 3,360 randomly selected posts, 2,739 (82%) were related to depression, suicide, or self-harm. Common themes were self-loathing (412, 15%), loneliness/feeling unloved (405, 15%), self-harm (407, 15%), and suicide (372, 14%). Conclusion: This study takes an important first step at better understanding the displayed depression-related references on Tumblr. The findings signal a need for suicide prevention efforts to intervene on Tumblr and use this platform in a strategic way, given the depression and suicidal content that was readily observed on Tumblr.

Abstract:TikTok is the second most popular social media platform among American teenagers, suggesting it may be a way to reach young people with sexual violence-related messages. This qualitative content analysis focused on what TikTok users share about sexual violence. The content of the videos fell into two primary types of information, people sharing experiences of sexual violence (i.e., what happened, how they responded, impacts of experiences) and people sharing other information related to sexual violence (i.e., education, raising awareness). The study results demonstrate that people use TikTok as a platform to share their messages about sexual violence-related topics, underscoring the potential of this platform for prevention and recovery messages targeted toward individuals who directly or indirectly experience sexual violence.
