Abstract:The incidence of the inferior vena cava (IVC) tumor thrombus is reported to be 4%-10% in patients with renal cell carcinoma (RCC). Tumor thrombus may extend through to the right atrium. Management of patients with level III/IV tumor thrombus is usually difficult. We report two cases of level IV thrombus in our hospital in 2002 and 2004. CASE REPORT Case 1 A 79-year-old man with edema of bilateral lower extremity for more than 20 days was admitted to our hospital on February 25, 2002. He was diagnosed as right RCC with IVC thrombus after ultrasonography and CT scan. MRI examination showed some occupying lesions in the liver. The renal tumor was about 4 cm in diameter and the upper limit of IVC thrombus reached the right atrium (Fig. 1A). Preoperative evaluation showed anemia (Hb 74.8 g/L) and hypoalbuminemia (29 g/L). AKP was high (312 U/L). Additional staging was made by chest X-ray examination and bone scan with no abnormalities. There was no other suspected distant metastasis. The patient underwent radical nephrectomy including lymphadenectomy and the tumor thrombus extirpation with extracorporeal bypass under the condition of hypothermia. The core temperature was 34°C. The abdomen was opened using a midline incision. After exploration of the abdomen to rule out metastatic disease, a median sternotomy was done and the pericardium was opened. Then mobilized the renal pedicle and ligated the renal artery. The kidney was mobilized out of Gerota's fascia. The thrombus with a solid cephalic head was difficult to be taken out completely from IVC (the vena caval wall was not involved). The right atrium was opened and the upper part of thrombus was taken out from the atrium and the lower part from the incision of the vena cava. Cardiovascular reconstruction was accomplished by direct suture. The duration was 330 minutes with 1500 ml blood loss. The patient had no perioperative complications. Pathological examination for the surgical specimens confirmed RCC along with the inferior vena caval thrombus. It also did not show any metastase in lymph nodes and adrenal gland. TNM staging was T3cN0M0. The patient has no recurrence or metastasis at the 49-month follow-up.Fig. 1.: MRI scan shows the right (A, case 1) and left (B, case 2) renal cell carcinoma with the V4 inferior vena cava thrombus.Case 2 A 40-year-old man with gross hematuria for 4 months was admitted to our hospital on July 7, 2004, without flank discomfort, fever or edema in lower extremity. The patient had a history of diabetes mellitus. Ultrasonography showed left renal tumor with invasion into IVC. Abdominal CT and MRI scans showed no abdominal visceral metastasis and the upper limit of IVC thrombus reached the right atrium (Fig. 1B). Chest X-ray film and AKP were normal. The patient also underwent radical nephrectomy including lymphadenectomy and the tumor thrombus extirpation with extracorporeal bypass. The abdomen was opened using a roof-shaped incision. The vena caval wall was not involved by the tumor thrombus. But the thrombus had a solid cephalic head (Fig. 2) which was difficult to be taken out completely from IVC. The right atrium was opened and the upper part of thrombus was taken out from the atrium and lower part from the incision of the vena cava. Cardiovascular reconstruction was accomplished by direct suture. The duration was 300 minutes with blood loss 2000 ml. This patient also had not perioperative complications. The pathology of surgical specimen showed RCC along with the inferior vena caval thrombus (Fig. 3). TNM staging was T3cN0M0. The patient recurred after 4 months under the diaphragm and underwent a second resection. Now he lives cancer-free for about 19 months.Fig. 2.: The thrombus was 20 cm long with a solid cephalic head (Case 2).Fig. 3.: The histological section of the inferior vena caval tumor thrombus (Case 2, HE staining, original magnification×400).DISCUSSION Vena caval tumor thrombus extension is a relatively uncommon event occurring in 4% to 10% of all patients with renal cell carcinoma.1–4 Among them, 2% to 27% have tumor thrombus extending into the right atrium.1,2,4,5 Historically, RCC with level III and IV thrombus (above the level of the liver) was deemed incurable. Considerable advances in the identification and management of thrombus including the use of new diagnostic imaging modalities to define the level of thrombus and the use of extracorporeal bypass, have permitted surgical resection of these complex thrombus possible. Symptoms of these patients are usually related to concomitant pulmonary emboli, renal or hepatic dysfunction or subtle physical signs including varicocele, dilated superficial abdominal veins or lower extremity edema. If there is suspicion of thrombus extension to the inferior vena cava, we proceed to MRI scan, which can reliably demonstrate a tumor thrombus and its extension, and also can rule out IVC wall invasion facilitating the exact surgical decision-making. Resection of RCC with tumor thrombus in IVC needs some technical considerations. ① To achieve optimal exposure, we use midline or chevron incision with a median sternotomy for level IV thrombus. ② Controlling the inferior vena cava above the thrombus to prevent cephalic mobilization of tumor fragment. This necessitates mobilization and isolation of the tributaries of the vena cava. ③ For atrial thrombus, the cardiopulmonary bypass allows the majority of these cases to be managed successfully. It should be kept in mind that digital manipulation of the tumor thrombus in an attempt to push it out from the IVC without the protection of cardiopulmonary bypass may be hazardous and may result in tumor embolism. There was few intraoperative complications occurred in our patients. This is in accordance with other reports. If treated without cardiopulmonary bypass, there would be a higher incidence of severe and unexpected bleeding or tumor embolization. Welz et al6 reported that no tumor embolization occurred in cardiopulmonary bypass group, whereas four (16%) occurred in the conventional technique group (P<0.05). Severe bleeding occurred in 11% of the cardiopulmonary bypass group and 16% in the conventional group. Perioperative mortality is high in patients with IVC thrombus. Sweeney et al7 reported 96 patients with complex IVC thrombi and the overall perioperative mortality was 5%. Among patients with level III/IV thrombi, they noted increased perioperative mortality (19%). Therefore there is little doubt that these patients present some of the most challenging oncological considerations. If these patients are not treated they have unfortunately poor survival rates.8 Some reports have shown that total resection of this tumor affords the best chance of cure and long-term survival when no distant metastases are present, the 5-year survival rates range between 40% and 69% after complete surgical resection.1–3,7 Both our patients have been alive for 19 and 49 months. It is in accordance with these reports. The prognostic factors of RCC with vena caval tumor thrombus extension are perinephric fat involvement, lymph node involvement, or distant metastases. If these findings are present, survival will be dismal. The prognostic significance of the tumor thrombus level in the IVC is still in controversy. Sosa et al9 reported a 2-year survival rate of 80% in patients with infrahepatic IVC tumor thrombi, while only 21% in those with suprahepatic thrombi. They found that patients with suprahepatic caval extension were more likely to have perinephric fat and regional lymph node involvement. Others found that in the absence of other adverse prognostic factors, the cephalic extension of IVC involvement did not appear to be prognostically important.7,10
