Abstract:Generative pretrained transformer models have been popular recently due to their enhanced capabilities and performance. In contrast to many existing artificial intelligence models, generative pretrained transformer models can perform with very limited training data. Generative pretrained transformer 3 (GPT-3) is one of the latest releases in this pipeline, demonstrating human-like logical and intellectual responses to prompts. Some examples include writing essays, answering complex questions, matching pronouns to their nouns, and conducting sentiment analyses. However, questions remain with regard to its implementation in health care, specifically in terms of operationalization and its use in clinical practice and research. In this viewpoint paper, we briefly introduce GPT-3 and its capabilities and outline considerations for its implementation and operationalization in clinical practice through a use case. The implementation considerations include (1) processing needs and information systems infrastructure, (2) operating costs, (3) model biases, and (4) evaluation metrics. In addition, we outline the following three major operational factors that drive the adoption of GPT-3 in the US health care system: (1) ensuring Health Insurance Portability and Accountability Act compliance, (2) building trust with health care providers, and (3) establishing broader access to the GPT-3 tools. This viewpoint can inform health care practitioners, developers, clinicians, and decision makers toward understanding the use of the powerful artificial intelligence tools integrated into hospital systems and health care.

Abstract:Pre-trained language models have attracted increasing attention in the biomedical domain, inspired by their great success in the general natural language domain. Among the two main branches of pre-trained language models in the general language domain, i.e., BERT (and its variants) and GPT (and its variants), the first one has been extensively studied in the biomedical domain, such as BioBERT and PubMedBERT. While they have achieved great success on a variety of discriminative downstream biomedical tasks, the lack of generation ability constrains their application scope. In this paper, we propose BioGPT, a domain-specific generative Transformer language model pre-trained on large scale biomedical literature. We evaluate BioGPT on six biomedical NLP tasks and demonstrate that our model outperforms previous models on most tasks. Especially, we get 44.98%, 38.42% and 40.76% F1 score on BC5CDR, KD-DTI and DDI end-to-end relation extraction tasks respectively, and 78.2% accuracy on PubMedQA, creating a new record. Our case study on text generation further demonstrates the advantage of BioGPT on biomedical literature to generate fluent descriptions for biomedical terms. Abstract:The advent of artificial intelligence (AI) and machine learning (ML) has revolutionized the field of medicine. Although highly effective, the rapid expansion of this technology has created some anticipated and unanticipated bioethical considerations. With these powerful applications, there is a necessity for framework regulations to ensure equitable and safe deployment of technology. Generative Adversarial Networks (GANs) are emerging ML techniques that have immense applications in medical imaging due to their ability to produce synthetic medical images and aid in medical AI training. Producing accurate synthetic images with GANs can address current limitations in AI development for medical imaging and overcome current dataset type and size constraints. Offsetting these constraints can dramatically improve the development and implementation of AI medical imaging and restructure the practice of medicine. As observed with its other AI predecessors, considerations must be taken into place to help regulate its development for clinical use. In this paper, we discuss the legal, ethical, and technical challenges for future safe integration of this technology in the healthcare sector.

Abstract:The advent of artificial intelligence (AI) has catalyzed a revolutionary transformation across various industries, including healthcare. Medical applications of ChatGPT, a powerful language model based on the generative pre-trained transformer (GPT) architecture, encompass the creation of conversational agents capable of accessing and generating medical information from multiple sources and formats. This study investigates the research trends of large language models such as ChatGPT, GPT 4, and Google Bard, comparing their publication trends with early COVID-19 research. The findings underscore the current prominence of AI research and its potential implications in biomedical engineering. A search of the Scopus database on July 23, 2023, yielded 1,096 articles related to ChatGPT, with approximately 26% being medical science-related. Keywords related to artificial intelligence, natural language processing (NLP), LLM, and generative AI dominate ChatGPT research, while a focused representation of medical science research emerges, with emphasis on biomedical research and engineering. This analysis serves as a call to action for researchers, healthcare professionals, and policymakers to recognize and harness AI's potential in healthcare, particularly in the realm of biomedical research.

Abstract:Artificial intelligence (AI) has witnessed significant advancements, reshaping various industries, including healthcare. The introduction of ChatGPT by OpenAI in November 2022 marked a pivotal moment, showcasing the potential of generative AI in revolutionising patient care, diagnosis and treatment. Generative AI, unlike traditional AI systems, possesses the ability to generate new content by understanding patterns within datasets. This article explores the evolution of AI in healthcare, tracing its roots to the term coined by John McCarthy in 1955 and the contributions of pioneers like John Von Neumann and Alan Turing. Currently, generative AI, particularly Large Language Models, holds promise across three broad categories in healthcare: patient care, education and research. In patient care, it offers solutions in clinical document management, diagnostic support and operative planning. Notable advancements include Microsoft's collaboration with Epic for integrating AI into electronic medical records (EMRs), enhancing clinical data management and patient care. Furthermore, generative AI aids in surgical decision-making, as demonstrated in plastic, orthopaedic and hepatobiliary surgeries. However, challenges such as bias, hallucination and integration with EMR systems necessitate caution and ongoing evaluation. The article also presents insights from the implementation of NUHS Russell-GPT, a generative AI chatbot, in a hand surgery department, showcasing its utility in administrative tasks but highlighting challenges in surgical planning and EMR integration. The survey showed unanimous support for incorporating AI into clinical settings, with all respondents being open to its use. In conclusion, generative AI is poised to enhance patient care and ease physician workloads, starting with automating administrative tasks and evolving to inform diagnoses, tailored treatment plans, as well as aid in surgical planning. As healthcare systems navigate the complexities of integrating AI, the potential benefits for both physicians and patients remain significant, offering a glimpse into a future where AI transforms healthcare delivery. Abstract:The integration of artificial intelligence technologies, such as large language models (LLMs), in health care holds potential for improved efficiency and decision support. However, ethical concerns must be addressed before widespread adoption. This article focuses on the ethical principles surrounding the use of Generative Pretrained Transformer-4 and its conversational model, ChatGPT, in healthcare settings. One concern is potential inaccuracies in generated content. LLMs can produce believable yet incorrect information, risking errors in medical records. Opacity of training data exacerbates this, hindering accuracy assessment. To mitigate, LLMs should train on precise, validated medical data sets. Model bias is another critical concern because LLMs may perpetuate biases from their training, leading to medically inaccurate and discriminatory responses. Sampling, programming, and compliance biases contribute necessitating careful consideration to avoid perpetuating harmful stereotypes. Privacy is paramount in health care, using public LLMs raises risks. Strict data-sharing agreements and Health Insurance Portability and Accountability Act (HIPAA)-compliant training protocols are necessary to protect patient privacy. Although artificial intelligence technologies offer promising opportunities in health care, careful consideration of ethical principles is crucial. Addressing concerns of inaccuracy, bias, and privacy will ensure responsible and patient-centered implementation, benefiting both healthcare professionals and patients.

Abstract:Generative artificial intelligence (AI), a subset of machine learning that creates new content based on training data, has witnessed tremendous advances in recent years. Practical applications have been identified in health care in general, and there is significant opportunity in transplant medicine for generative AI to simplify tasks in research, medical education, and clinical practice. In addition, patients stand to benefit from patient education that is more readily provided by generative AI applications. This review aims to catalyze the development and adoption of generative AI in transplantation by introducing basic AI and generative AI concepts to the transplant clinician and summarizing its current and potential applications within the field. We provide an overview of applications to the clinician, researcher, educator, and patient. We also highlight the challenges involved in bringing these applications to the bedside and need for ongoing refinement of generative AI applications to sustainably augment the transplantation field.
GPT4MIA: Utilizing Generative Pre-trained Transformer (GPT-3) as A Plug-and-Play Transductive Model for Medical Image Analysis

Abstract:In this paper, we propose a novel approach (called GPT4MIA) that utilizes Generative Pre-trained Transformer (GPT) as a plug-and-play transductive inference tool for medical image analysis (MIA). We provide theoretical analysis on why a large pre-trained language model such as GPT-3 can be used as a plug-and-play transductive inference model for MIA. At the methodological level, we develop several technical treatments to improve the efficiency and effectiveness of GPT4MIA, including better prompt structure design, sample selection, and prompt ordering of representative samples/features. We present two concrete use cases (with workflow) of GPT4MIA: (1) detecting prediction errors and (2) improving prediction accuracy, working in conjecture with well-established vision-based models for image classification (e.g., ResNet). Experiments validate that our proposed method is effective for these two tasks. We further discuss the opportunities and challenges in utilizing Transformer-based large language models for broader MIA applications.

Abstract:Generative AI is an expanding domain that employs machine learning models to generate novel data that closely mimic pre existing data. ChatGPT and DALL-E can be customized for specific applications and are expected to transform healthcare, education, and communication. Generative Adversarial Networks (GANs) that can generate synthetic medical images closely mimicking actual patient data may substantially enhance machine learning model training datasets. They also translate medical images from one modality to another, improve medical imaging resolution, reduce radiation exposure, and boost image quality and detail. Despite their challenges, GANs have great potential in the field of medical imaging. The key obstacles are the need for Graphic Processing Units (GPUs) and computing resources to train GANs and the lack of established standards for generating synthetic images. Incorrectly labeled data for training other machine learning models can reduce performance, making ground-truth data labeling for healthcare AI more difficult. Generative AI is revolutionizing healthcare imaging, simplifying diagnosis, and propelling healthcare research and practice to new frontiers. Ensuring the reliability and safety of generated images in medical applications requires addressing ethical considerations and validating data.

Abstract:It has been a year since the launch of Chat Generator Pre-Trained Transformer (ChatGPT), a generative artificial intelligence (AI) program. The introduction of this cross-generational product initially brought a huge shock to people with its incredible potential, and then aroused increasing concerns among people. In the field of medicine, researchers have extensively explored the possible applications of ChatGPT and achieved numerous satisfactory results. However, opportunities and issues always come together. Problems have also been exposed during the applications of ChatGPT, requiring cautious handling, thorough consideration and further guidelines for safe use. Here, we summarized the potential applications of ChatGPT in the medical field, including revolutionizing healthcare consultation, assisting patient management and treatment, transforming medical education and facilitating clinical research. Meanwhile, we also enumerated researchers’ concerns arising along with its broad and satisfactory applications. As it is irreversible that AI will gradually permeate every aspect of modern life, we hope that this review can not only promote people’s understanding of the potential applications of ChatGPT in the future, but also remind them to be more cautious about this “Pandora’s Box” in the medical field. It is necessary to establish normative guidelines for its safe use in the medical field as soon as possible.

Abstract:Foundation models (FMs) have exhibited remarkable performance across a wide range of downstream tasks in many domains. Nevertheless, general-purpose FMs often face challenges when confronted with domain-specific problems, due to their limited access to the proprietary training data in a particular domain. In biomedicine, there are various biological modalities, such as molecules, proteins, and cells, which are encoded by the language of life and exhibit significant modality gaps with human natural language. In this paper, we introduce BioMedGPT, an open multimodal generative pre-trained transformer (GPT) for biomedicine, to bridge the gap between the language of life and human natural language. BioMedGPT allows users to easily ``communicate'' with diverse biological modalities through free text, which is the first of its kind. BioMedGPT aligns different biological modalities with natural language via a large generative language model, namely, BioMedGPT-LM. We publish BioMedGPT-10B, which unifies the feature spaces of molecules, proteins, and natural language via encoding and alignment. Through fine-tuning, BioMedGPT-10B outperforms or is on par with human and significantly larger general-purpose foundation models on the biomedical QA task. It also demonstrates promising performance in the molecule QA and protein QA tasks, which could greatly accelerate the discovery of new drugs and therapeutic targets. In addition, BioMedGPT-LM-7B is the first large generative language model based on Llama2 in the biomedical domain, therefore is commercial friendly. Both BioMedGPT-10B and BioMedGPT-LM-7B are open-sourced to the research community. In addition, we publish the datasets that are meticulously curated for the alignment of multi-modalities, i.e., PubChemQA and UniProtQA. Abstract:Large language models (LLM) have been promising recently in the medical field, with numerous applications in clinical neuroscience. OpenAI's launch of Generative Pre-trained Transformer 3.5 (GPT-3.5) in November 2022 and its successor, Generative Pre-trained Transformer 4 (GPT 4) in March 2023 have garnered widespread attention and debate surrounding natural language processing (NLP) and LLM advancements. Transformer models are trained on natural language datasets to predict and generate sequences of characters. Using internal weights from training, they produce tokens that align with their understanding of the initial input. This paper delves into ChatGPT's potential as a learning tool in neurosurgery while contextualizing its abilities for passing medical licensing exams and neurosurgery written boards. Additionally, possibilities for creating personalized case presentations and study material are discussed alongside ChatGPT's capacity to optimize the research workflow and perform a concise literature review. However, such tools need to be used with caution, given the possibility of artificial intelligence hallucinations and other concerns such as user overreliance, and complacency. Overall, this opinion paper raises key points surrounding ChatGPT's role in neurosurgical education.
