Abstract:Résumé Introduction Le pneumothorax (PNO) de l'enfant et de l'adolescent reste souvent difficile à appréhender par le clinicien car aucune définition ni prise en charge n'est consensuelle en pédiatrie. États des connaissances La suspicion diagnostique est clinique mais la confirmation est faite par l'imagerie thoracique avec une place de plus en plus importante pour l'échographie pulmonaire par rapport à la radiographie thoracique. Le but du traitement est double : obtenir une ré-expansion pulmonaire complète et prévenir les récidives. Selon l'importance du PNO et la tolérance clinique, peuvent se discuter une attitude dite conservatrice, l'ajout d'oxygène, une exsufflation à l'aiguille ou la mise en place d'un drain thoracique. Perspectives Des recommandations de définition et de prise en charge graduée du PNO de l'enfant et de l'adolescent sont nécessaires afin d'harmoniser les pratiques. Conclusions La fréquence des PNO spontanés idiopathiques est importante chez l'adolescent, 16 à 52 personnes sur 100 000, avec une récidive fréquente. La prise en charge n'est pas consensuelle. La récidive n'est pas prédite par les résultats de la tomodensitométrie thoracique. L'information des patients sur le risque de récidive est primordiale. Summary Introduction Due to the absence of consensual definition and agreed-upon pediatric treatment, pneumothorax (PNO) in children and adolescents often remains difficult to properly apprehend. State of the art While initial diagnostic suspicion is clinical, confirmation necessitates chest imaging, and lung ultrasound has become increasingly prevalent, often at the expense of chest radiography. The goal of treatment is twofold, on the one hand to a fully re-expand the lungs, and on the other hand to forestall PNO recurrence. Depending on PNO severity and clinical tolerance, it may be advisable to envision conservative management, oxygen supplementation, needle exsufflation, or chest tube drainage. Perspectives In order to harmonize clinical practices, guidelines for the precise definition and graduated management of PNO in children and adolescents are highly advisable. Conclusions Idiopathic spontaneous PNO frequently occurs in teenage populations, and its likewise frequent recurrence is not satisfactorily predicted by chest CT findings. It is of paramount importance that patients be fully informed of the risk of recurrence.
