Biology has had millions of years to find elegant solutions under severe resource constraints, so it is prudent to examine natural systems to look for concepts to adapt for human purposes for resiliency. The common thread for both bio-inspired design and social enterprises is frugality—maximizing the use of available capital resources while delivering effective outcomes. For engineers at work in the 21st century, an emphasis on frugal engineering could provide multiple advantages.

Abstract:Medical technology plays a significant role in the reduction of disability and mortality due to the global burden of disease. The lack of diagnostic technology has been identified as the largest gap in the global health care pathway, and the cost of this technology is a driving factor for its lack of proliferation. Technology developed in high-income countries is often focused on producing high-quality, patient-specific data at a cost high-income markets can pay. While machine learning plays an important role in this process, great care must be taken to ensure appropriate translation to clinical practice.

Abstract:Our planet is experiencing an accelerated process of change associated to a variety of anthropogenic phenomena. The future of this transformation is uncertain, but there is general agreement about its negative unfolding that might threaten our own survival. Furthermore, the pace of the expected changes is likely to be abrupt: catastrophic shifts might be the most likely outcome of this ongoing, apparently slow process. Although different strategies for geo-engineering the planet have been advanced, none seem likely to safely revert the large-scale problems associated to carbon dioxide accumulation or ecosystem degradation. An alternative possibility considered here is inspired in the rapidly growing potential for engineering living systems. It would involve designing synthetic organisms capable of reproducing and expanding to large geographic scales with the goal of achieving a long-term or a transient restoration of ecosystem-level homeostasis. Such a regional or even planetary-scale engineering would have to deal with the complexity of our biosphere. It will require not only a proper design of organisms but also understanding their place within ecological networks and their evolvability. This is a likely future scenario that will require integration of ideas coming from currently weakly connected domains, including synthetic biology, ecological and genome engineering, evolutionary theory, climate science, biogeography and invasion ecology, among others.

Engineering innovation in healthcare: technology, ethics and persons

Abstract:Engineering makes profound contributions to our health. Many of these contributions benefit whole populations, such as clean water and sewage treatment, buildings, dependable sources of energy, efficient harvesting and storage of food, and pharmaceutical manufacture. Thus, ethical assessment of these and other engineering activities has often emphasized benefits to communities. This is in contrast to medical ethics, which has tended to emphasize the individual patient affected by a doctor's actions. However technological innovation is leading to an entanglement of the activities, and hence ethical responsibilities, of healthcare professionals and engineering professionals. The article outlines three categories of innovation: assistive technologies, telehealthcare and quasi-autonomous systems. Approaches to engineering ethics are described and applied to these innovations. Such innovations raise a number of ethical opportunities and challenges, especially as the complexity of the technology increases. In particular the design and operation of the technologies require engineers to seek closer involvement with the persons benefiting from their work. Future innovation will require engineers to have a good knowledge of human biology and psychology. More particularly, healthcare engineers will need to prioritize each person's wellbeing, agency, human relationships and ecological self rather than technology, in the same way that doctors prioritize the treatment of persons rather than their diseases.

Abstract:In the dynamic realm of healthcare, the convergence of engineering and biomedical sciences has emerged as a pivotal frontier. In this review we go into specific areas of innovation, including medical imaging and diagnosis, developments in biomedical sensors, and drug delivery systems. Wearable biosensors, non-wearable biosensors, and biochips, which include gene chips, protein chips, and cell chips, are all included in the scope of the topic that pertains to biomedical sensors. Extensive research is conducted on drug delivery systems, spanning topics such as the integration of computer modeling, the optimization of drug formulations, and the design of delivery devices. Furthermore, the paper investigates intelligent drug delivery methods, which encompass stimuli-responsive systems such as temperature, redox, pH, light, enzyme, and magnetic responsive systems. In addition to that, the review goes into topics such as tissue engineering, regenerative medicine, biomedical robotics, automation, biomechanics, and the utilization of green biomaterials. The purpose of this analysis is to provide insights that will enhance continuing research and development efforts in engineering-driven biomedical breakthroughs, ultimately contributing to the improvement of healthcare. These insights will be provided by addressing difficulties and highlighting future prospects.

Abstract:Background and aim: Despite their flaws, the low-cost but powerful economical solutions can ensure everyone has access to health. The main aim of this study is to extract characteristics of frugal innovation (FI) and social innovation (SI) for Primary Health Centers (PHCs) in low resource settings (LRS) for sustainable development. We will use the gained insights to design the mobile primary healthcare infrastructure using FI and SI strategies. There is a lack of methodology to design sustainable healthcare infrastructure for LRS. There is a gap in the literature about building sustainable infrastructure to provide basic healthcare facilities essential to the community. This article studies several factors necessary for designing sustainable infrastructure from the lens of FI, SI, and sustainability to develop a mobile healthcare infrastructure for last-mile people. Methods: Started with purposive sampled case studies to find out factors and criteria that most affect the success for an innovation to be frugal, social, and sustainable. The established criteria were used to design, develop, and deploy the mobile Primary Health Center (mPHC). Moving forward, we tested the system designed with stakeholders to gather insights. At this stage we found the feedback loop from the stakeholders and the role of interdisciplinary discussions between experts, medical officers, nurses, patient, and other staff of PHCs during the design, development, deployment, and test stage to be useful in taking design decisions efficiently. Results: The designed healthcare infrastructure of mPHC through the aspects of FI and SI proves to be efficient in providing key healthcare services to LRS. Conclusion: Focusing on essential capabilities and optimizing performance with technology, methodologies, and processes reduces costs in an innovation. Focus on socially inclusive and rebalancing power disparities, overcome societal challenges and improve human capabilities will create a sustainable and novel solution.
Bioengineering technologies for the protection and development of coastal lands of small water bodies of the agro-industrial complex

Abstract:The rationale for the effectiveness of the use of bioengineering technologies in the protection and development of small water bodies and the reclamation of coastal lands of the agro- industrial complex is given. For this purpose, full-scale and theoretical research methods were used for known and proprietary technologies for the construction of bank protection structures using biopositive elements from local and environmentally friendly artificial materials. The development of innovative designs for bank protection and protection of problem areas of the agro-industrial complex, including eroding coastal areas of small ponds, rivers and canals of small irrigation systems, is presented. A concept of environmentally friendly development of the coastal territories of a water body, including the water body itself, its floodplain areas and the adjacent catchment area, has been proposed and tested. Studies of various sections of small rivers conducted in recent years have shown that the use of biopositive and flexible designs of protective and regulatory structures and their elements provide not only reliable engineering protection of coastal zones, but also create favorable conditions for the restoration of green water protection zones. Over time, the constructed structures, overgrown with grass and shrubs, merge with the natural environment, turning into bioengineered structures.

Abstract:Universities throughout the world are realigning engineering education by integrating the impact and importance the profession has on humanity, society and the natural environment. Amongst western universities it is common practice for the engineering programs to have ethics and sustainability embedded within most professional practice courses. Western universities are now understanding the important role cultural, social and emotional intelligence play in producing holistic 21st century engineers. Universities are supporting and fostering fieldtrips to resource-constrained communities, exposing students to real-world problems and finding solutions not necessarily typical in western environments. This, to a certain degree, has helped promote the concept of the Humanitarian Engineer. However, many activities run the risk of disempowering communities and incorrectly promote neo-colonialism as a positive concept to students: the exact opposite of what universities should be striving to achieve. This paper explores the concept of voluntourism and the need for holistic and sustainable solutions for universities to produce culturally, socially and emotionally intelligent engineers with a strong depth of technical knowledge. The paper provides case studies of different programs, highlighting different methods of service-learning and community empowerment. The paper provides a framework to assess program suitability to produce holistic 21st century engineering graduates.

Abstract:The panoramic characteristics of human-machine interfaces (HMIs) have prompted the needs to update the biotechnology community with the recent trends, developments, and future research direction toward next-generation bioelectronics. Bioinspired materials are promising for integrating various bioelectronic devices to realize HMIs. With the advancement of scientific biotechnology, state-of-the-art bioelectronic applications have been extensively investigated to improve the quality of life by developing and integrating bioinspired nanoplatforms in HMIs. This review highlights recent trends and developments in the field of biotechnology based on bioinspired nanoplatforms by demonstrating recently explored materials and cutting-edge device applications. Section 1 introduces the recent trends and developments of bioinspired nanomaterials for HMIs. Section 2 reviews various flexible, wearable, biocompatible, and biodegradable nanoplatforms for bioinspired applications. Section 3 furnishes recently explored substrates as carriers for advanced nanomaterials in developing HMIs. Section 4 addresses recently invented biomimetic neuroelectronic, nanointerfaces, biointerfaces, and nano/microfluidic wearable bioelectronic devices for various HMI applications, such as healthcare, biopotential monitoring, and body fluid monitoring. Section 5 outlines designing and engineering of bioinspired sensors for HMIs. Finally, the challenges and opportunities for next-generation bioinspired nanoplatforms in extending the potential on HMIs are discussed for a near-future scenario. We believe this review can stimulate the integration of bioinspired nanoplatforms into the HMIs in addition to wearable electronic skin and health-monitoring devices while addressing prevailing and future healthcare and material-related problems in biotechnologies.

Abstract:Humanitarian emergencies, catalyzed in part by increased armed conflicts in many parts of the world, have resulted in the precious loss of life and strained or destroyed local health systems that are no longer capable of providing care to those who desperately need it [1]. At the same time, millions of people remain in refugee camps, slums, and informal settlements for decades with limited provisions for adequate and quality care. In addition to existing health challenges, these complex emergencies provide a fertile environment for the emergence of new pathogens that can impact communities and countries all around the world. For humanitarian emergencies and protracted crises, the need for terrain ready and robust diagnostics cannot be underestimated. At the same time, context-appropriate tools to diagnose and manage chronic diseases are also badly needed. Humanitarian agencies, working in these at-risk communities, are often in need of not just cash and medical supplies, but also require trained professionals [2] who are comfortable with technology, can innovate, and guide and train others in appropriate use of technology.

Abstract:This article explores the relevance of universal design and empathic design in education. The universal design focuses on creating accessible and usable products, environments, and systems for individuals with diverse abilities. Empathy, on the other hand, involves understanding and sharing the feelings of others, encompassing cognitive, emotional, and compassionate empathy. The article investigates the application of empathy in design, particularly its importance for engineers. Teaching empathy to engineers is emphasized as a crucial aspect. By developing empathic skills, engineers gain a deeper understanding of user needs and perspectives, leading to more inclusive and user-centered design solutions. Effective communication techniques such as asking open-ended questions, active listening, observation, and perspective-taking are explored to enhance empathy in communication. The article also explores methods for measuring empathy, thus enabling engineers to assess the effectiveness of their empathic design approaches. Potentials and challenges of using co-design in health services research in low- and middle-income countries

Abstract:Co-design with people having poor access to health services and fragile health systems in low- and middle-income countries can be momentous in bringing service users and other stakeholders together to improve the delivery and utilisation of health services. There is ample of evidence from high-income countries regarding how co-design can translate available evidence into developing acceptable, feasible, and adaptable health solutions in different settings. However, there is limited literature on co-design in health research in the context of low- and middle-income countries. Therefore, it is crucial to understand how knowledge about collaborative working can be translated into policy and practice in the context of low- and middle-income countries. Thus, this paper discusses the concept of co-design, co-production, and co-creation in health and the potentiality and challenges of using co-design in health services research in low- and middle-income countries. Despite the challenges, the co-design research has considerable potential to encourage the meaningful engagement of service users and other stakeholders in developing, implementing, and evaluating real-world solutions in low- and middle-income countries. It is essential to balance power dynamics in a co-design process through mutual recognition and respect, participant diversity, and reciprocity and flexibility in sharing. The inclusive and collaborative approach to working is complex due to existing rigid hierarchical structures, socio-cultural beliefs, political interference and working practices. However, this could be minimised by developing transparent terms of reference that reflect the value and benefits of equal partnership in particular co-design work.
Machines matter too: including biomedical engineering partnerships in global health initiatives

Abstract:As the global burden of disease shifts from communicable to non-communicable diseases and trauma-related debility, the global health-care community has increasingly advocated for equitable access to surgical services. Much of this attention has focused on bolstering clinical and research expertise through the expansion of clinical training programmes and research resources. However, despite the crucial role of equipment, including medical devices, in safe clinical care, comparatively little attention has been paid to sustainably bridging the biomedical and technical gaps that exist in global health. Although closing these gaps with locally driven solutions is the goal, the reality in many settings is that partnerships between institutions in high-income countries and low-income and middle-income countries currently remain necessary. Thus, this Viewpoint provides guidelines conscious to biomedical engineering for institutions looking to expand or begin global health initiatives with the Partners In Health Five S's-staff, stuff, space, systems, and social support-framework for health systems strengthening as a guide. We focus first on initiatives involving staff, stuff, and space, encouraging institutions to ask, listen, and enable, through a set of questions and actionable recommendations. Then, we urge institutions to strengthen systems and social support, underscoring the need for wider societal scaffolding to support and sustain initiatives beyond initial efforts.

Abstract:INTRODUCTION: The slow adoption of innovation into healthcare calls into question the manner of evidence generation for medical technology. This paper identifies potential reasons for this including a lack of attention to human factors, poor evaluation of economic benefits, lack of understanding of the existing healthcare system and a failure to recognise the need to generate resilient products. Areas covered: Recognising a cross-disciplinary need to enhance evidence generation early in a technology's life cycle, the present paper proposes a new approach that integrates human factors and health economic evaluation as part of a wider systems approach to the design of technology. This approach (Human and Economic Resilience Design for Medical Technology or HERD MedTech) supports early stages of product development and is based on the recent experiences of the National Institute for Health Research London Diagnostic Evidence Co-operative in the UK. Expert commentary: HERD MedTech i) proposes a shift from design for usability to design for resilience, ii) aspires to reduce the need for service adaptation to technological constraints iii) ensures value of innovation at the time of product development, and iv) aims to stimulate discussion around the integration of pre- and post-market methods of assessment of medical technology.

Adapting engineering design thinking for sustainability

Abstract:The nature of engineering design thinking needs to change drastically to address the climate crisis, as the technologies and activities that damage the environment are generated by engineering design. Current engineering design practices, as well as engineering design education (EE) pedagogies, are driven by formal knowledge structures, such as mathematical models and optimization techniques, that are far removed from concerns about the environment. To adapt engineering design thinking for sustainability, a clear characterization of the thinking roles played by various design components, including the role of formal structures in sustainable engineering design, is needed. As a first step towards developing this characterization, we investigated the way sustainable micro-hydro power systems were designed by two grassroots technology designers. Our analysis indicated that artifact-based imagination (mental simulation of material structure and dynamics), including artifact-based reasoning (manipulation of imagined artifacts), and synthesis (of requirements and constraints) were key to integrating the eco-social context into the design. Formal structures played a supportive role to these core design thinking processes. To examine the validity of this analysis, we studied two engineering design cases from other areas, exploring whether the patterns we identified using the micro-hydro turbine cases extended to these cases. Results suggested that artifact-based imagination and reasoning and synthesis are common threads across different kinds of engineering design. Based on this finding, we suggest that the ongoing redesign of EE—to support ecological sustainability—needs to focus on artifact-based imagination and synthesis of eco-social factors, in addition to formal structures.

Making gene editing accessible in resource limited environments: recommendations to guide a first-time user

Abstract:The designer nuclease, CRISPR-Cas9 system has advanced the field of genome engineering owing to its programmability and ease of use. The application of these molecular scissors for genome engineering earned the developing researchers the Nobel prize in Chemistry in the year 2020. At present, the potential of this technology to improve global challenges continues to grow exponentially. CRISPR-Cas9 shows promise in the recent advances made in the Global North such as the FDA-approved gene therapy for the treatment of sickle cell anaemia and β-thalassemia and the gene editing of porcine kidney for xenotransplantation into humans affected by end-stage kidney failure. Limited resources, low government investment with an allocation of 1% of gross domestic production to research and development including a shortage of skilled professionals and lack of knowledge may preclude the use of this revolutionary technology in the Global South where the countries involved have reduced science and technology budgets. Focusing on the practical application of genome engineering, successful genetic manipulation is not easily accomplishable and is influenced by the chromatin landscape of the target locus, guide RNA selection, the experimental design including the profiling of the gene edited cells, which impacts the overall outcome achieved. Our assessment primarily delves into economical approaches of performing efficient genome engineering to support the first-time user restricted by limited resources with the aim of democratizing the use of the technology across low- and middle-income countries. Here we provide a comprehensive overview on existing experimental techniques, the significance for target locus analysis and current pitfalls such as the underrepresentation of global genetic diversity. Several perspectives of genome engineering approaches are outlined, which can be adopted in a resource limited setting to enable a higher success rate of genome editing-based innovations in low- and middle-income countries.
