The Effects of Ultrasound-Guided Fascia Iliaca Compartment Block for Perioperative Analgesia and Rehabilitation in Elderly Patients Undergoing Total Hip Arthroplasty

Abstract:Objective: To investigate the effect of ultrasound-guided fascia iliaca compartment block in the perioperative period of total hip arthroplasty in elderly patients before surgery.Methods: The relevant information of 98 elderly patients, undergoing elective general anesthesia for total hip arthroplasty were retrospectively analyzed after filtering, among those, 43 patients with ultrasound-guided fascia iliaca compartment block (fascia iliaca block group, group F) before surgery and 55 patients without fascia iliaca compartment block (control group, group C). A propensity score matching analysis was performed according to gender, age, BMI, ASA classification, surgical method and underlying disease, after that, Group F and Group C included 43 cases respectively. We compared opioid dosage, analgesic effect, numerical rating scales(NRS) at rest and activity (2, 6, 12, 24, 48 hours after surgery); We also compared the incidence of nausea and vomiting, urinary retention, pruritus, respiratory depression and sleepiness between the two groups.Results: After matching, there was no difference in preoperative baseline data between the two groups. The analgesic effect of group F was significantly higher than that of group C (P < 0.05). The intraoperative opioid dosage, activity NRS score, first postoperative activity time, hospital stay, postoperative nausea, vomiting and urinary retention in group F were lower than those in group C (P < 0.05). There was no significant difference in pruritus, respiratory depression and lethargy after the operation.Conclusion: Ultrasonic-guided fascia iliaca compartment block before surgery can provide good perioperative analgesia for elderly patients undergoing total hip arthroplasty, reduce the use of analgesics, improve the satisfaction of analgesia, and promote postoperative recovery of patients.Funding Information: This project was sponsored by the National Natural Science Foundation of China (No.82071196).Declaration of Interests: The authors declare no conflict of interest.Ethics Approval Statement: This study has been approved by the ethics committee of Zhongda hospital, Southeast university (2020zdsyll066-p01).
