Abstract:Objective: To evaluate the efficacy of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in the diagnosis of thoracic sarcoidosis. Methods: A retrospective dataset of 35 patients who received EBUS-TBNA was analysed. These patients underwent EBUS-TBNA due to suspected sarcoidosis (radiographic stages I and II) with enlarged hilar or mediastinal lymph nodes (>= 1.0 cm) revealed by computed tomography. When EBUS-TBNA was considered to be nondiagnostic, surgical biopsy and clinical and radiological follow-up was performed. Results: Twenty-eight of 35 (80%) patients had a final diagnosis of sarcoidosis. Among them, 25 (89.3%) were diagnosed by EBUS-TBNA. Conclusion: Endobronchial ultrasound-guided transbronchial needle aspiration is a safe and effective procedure for the diagnosis of stages I and II of thoracic sarcoidosis.
[The value of bronchoscopy in the diagnosis of sarcoidosis]

Abstract:Objective: To evaluate the role of bronchoscopy in the diagnosis of sarcoidosis. Methods: A retrospective analysis was conducted for 200 patients who were diagnosed to have sarcoidosis and underwent bronchoscopy from June 2009 to June 2014 in Peking Union Medical College Hospital. The diagnostic value of different bronchoscopic procedures was analyzed. Results: Of the 200 patients, 145 were finally confirmed to have sarcoidosis by pathology through bronchoscopic sampling techniques. The diagnostic yields of endobronchial biopsy (EBB), transbronchial lung biopsy (TBLB), and EBUS-TBNA were 71.14%, 46.77%, 46.80%, respectively. The yields among the 3 techniques were statistically different. In those with mucosal lesions, the yield of EBB was 80.80%, while that of the combination of EBB, TBLB and EBUS-TBNA was 83.3%, the difference being not significant. In those without mucosal lesions, the yields of EBB, and the combination of EBB, TBLB and EBUS-TBNA were 20.8% and 48.0% respectively, with significant difference(χ(2)=4.463, P=0.035). Conclusions: EBB is a preferred approach for the diagnosis of sarcoidosis with endobronchial abnormalities, while for cases without mucosal lesions, combined TBLB, EBUS-TBNA and bronchoalveolar lavage can improve the diagnostic yield.
Role of Bronchoscopy in Diagnosis of Sarcoidosis

Abstract:Sarcoidosis is a multisystem inflammatory disorder with unclear etiology and can often pose a diagnostic challenge. A tissue diagnosis is often necessary to illustrate the non-caseating granulomas on histopathology. This review aims to synthesize current evidence related to tissue diagnosis of sarcoidosis using various bronchoscopic techniques. We start by discussing standard bronchoscopic techniques which have remained the cornerstone of diagnostic workup such as bronchoalveolar lavage (BAL), endobronchial biopsy (EBB), conventional transbronchial needle aspiration (cTBNA) and transbronchial lung biopsy (TBLB) followed by newer modalities that incorporate real-time image guidance using endobronchial and endoscopic ultrasound. Although BAL, EBB, and TBLB have been employed as a diagnostic tool for several decades, their sensitivity and diagnostic yield is inferior to ultrasound-based endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) and endoscopic ultrasound-guided fine needle aspiration (EUS-FNA). More recently, convincing evidence has also emerged to support the diagnostic accuracy and tissue yield of transbronchial lung cryobiopsy which will also be discussed in this review. These advances in bronchoscopic equipment and techniques over the last 2 decades have made it possible to obtain tissue samples using minimally invasive techniques thus avoiding invasive open lung biopsy and the risks that inherently follow. Up-to-date knowledge of these modalities is imperative for ensuring evidence-based medicine and improving patient-centric outcomes.
Increased Serum Sedimentation and Positive Tuberculosis Antibody Combined Multiple Pulmonary Nodules in Chest CT in a Middle-Aged Patient Firstly Misdiagnosed As Tuberculosis Proved As Sarcoidosis by CT Guided Percutaneous Lung Puncture Biopsy: a Case Report and Literature Review

Abstract:BACKGROUND Tuberculosis is a common infectious disease in developing countries. Tuberculosis and sarcoidosis are difficult to differentiate. We presented an adult case with increased serum sedimentation and positive tuberculosis antibody combined with multiple pulmonary nodules in chest CT in a middle-aged patient firstly misdiagnosed as tuberculosis proved as sarcoidosis by CT guided percutaneous lung puncture biopsy. METHODS Appropriate laboratory tests are carried out. The chest CT scan, bronchoscopy CT guided percutaneous lung puncture biopsy were performed for diagnosis. RESULTS Serum sedimentation was increased and tuberculosis antibody was positive. The chest CT scan showed multiple pulmonary nodules in both lungs and multiple lymphadenopathy. The bronchoscopy demonstrated no abnormality. Pathology of CT guided percutaneous lung puncture biopsy showed non-caseous multiple granulomatous lesions and acid-fast staining was negative. CONCLUSIONS When a patient has multiple pulmonary nodules and lymphadenopathy without obvious tuberculosis poisoning symptoms, physicians should pay attention to tuberculosis, sarcoidosis, and lung cancer. Pathology is crucial for the ultimate diagnosis.
Analysis of the Bronchospopic Diagnosis of Sarcoidosis

Abstract:Objective To evaluate the role of fiberotic bronchopsy findings in the diagnosis of thoracic sarcoidosis. Methods A retrospective analysis was performed in 55 sarcoidosis patients from October of 1999 to June of 2005. Results The airway with endobronchial military lesions was found in 71.43% patients,while airway without endobronchial military lesions was 21.74% (P =0.002). The diagnosis was comfirmed in 73.33% patients of combination transbronchial mucosa biopsy (TBB) and the transbronchial lung biopsy (TBLB) through fiberotic bronchospy. The level of angiotensin-converting enzyme (ACE) is greatly related with the percent of lympphocytes in broncho-alveolar lavage fluid (BALF). Conclusions The fiberotic bronchospy is clinically valuable for the diagnosis and the analysis activity of sarcoidosis.
Subcutaneous Nodules, Mediastinal and Hilar Lymphadenopathy

Abstract:A 52-year old man was admitted to our hospital because of dyspnea on exertion for 2 months and subcutaneous nodules for 1 month. Chest enhanced CT showed bilateral hilar and mediastinal lymphadenopathy. Bronchial alveolar lavage fluid revealed a CD 4+/CD 8+ T cell subsets ratio of 4.3 and culture for acid-fast bacillus (AFB) was negative. The pathology of skin nodules and transbronchial needle aspiration biopsy guided by endoscopic ultrasound (EBUS-TBNA) revealed non-caseating necrotizing epithelioid granulomas with negative acid-fast bacilli staining and periodic acid-Schiff staining, which was compatible with sarcoidosis. The patient was diagnosed as sarcoidosis and glucocorticoid was administrated. The subcutaneous nodules were improved. However, the lymph nodes were enlarged instead of shrinking after 6-month therapy. The failure to respond to glucocorticoids raised the possibility of sarcoidosis complicated with tuberculosis infection. The patient received anti-tuberculosis therapy. Lymphadenopathy responded within 4 months, and there was complete regression after 18-month treatment. The patient was followed up for 5 years and repeated chest CT scan showed that the sizes of bilateral hilar and mediastinal lymph nodes were normal.
Endobronchial Ultrasound-guided Transbronchial Needle Aspiration for the Diagnosis of StageⅠandⅡThoracic Sarcoidosis

Abstract:Objective To evaluate the usefulness of endobronchial ultrasound-guided transbronchial needle aspiration ( EBUS-TBNA) in the diagnosis of stage Ⅰand Ⅱthoracic sarcoidosis . Methods A total of 92 patients with suspected stage ⅠandⅡsarcoidosis in our hospital between September 2009 and September 2013 were studied.All the patients underwent EBUS-TBNA. Those with undetermined diagnosis then underwent surgical biopsies or were followed up for at least 6 months. Results EBUS-TBNA was performed in 232 lymph node stations of the 92 patients, 161 (69.4%) of which were located at mediastinum and 71 (30.6%) of which at hilar and interlobar region .Of the 92 patients, 71 were diagnosed as having sarcoidosis , 64 (90.1%) of which were diagnosed with EBUS-TBNA.The sensitivity, specificity, accuracy, positive and negative predictive value of EBUS-TBNA in the diagnosis of stage Ⅰand Ⅱthoracic sarcoidosis were 90.1% (64/71), 100% (21/21), 92.4% (85/92), 100% (64/64) and 75%(21/28), respectively.The procedure was well tolerated by all the patients with no complications observed . Conclusion EBUS-TBNA is an accurate and safe method in the diagnosis of stage ⅠandⅡthoracic sarcoidosis .

Abstract:Background The value of lymphocyte profiling (LP) in mediastinal lymph nodes for the differential diagnosis of sarcoidosis has not been extensively studied, and existing literature presents mixed results. Methods This was a prospective study of patients with intrathoracic lymphadenopathy who underwent endobronchial ultrasound‐guided transbronchial needle aspiration (EBUS‐TBNA). LP in lymph node puncture fluid (LNPF) was evaluated using flow cytometry. The results of LP in sarcoidosis patients were compared with tuberculous lymphadenitis (TBLA) patients. Receiver operating characteristic (ROC) analysis was performed to determine the optimal cut‐offs of the statistically significant parameters for screening for sarcoidosis. Based on the optimal cut‐offs and the final diagnosis of sarcoidosis and TBLA, the sensitivity, specificity, and accuracy of every statistically significant parameter and different combinations of the above three parameters were calculated for the diagnosis of sarcoidosis. Results Forty‐five cases of sarcoidosis and 33 cases of TBLA were enrolled in this study. Compared with the LP in TBLA patients, in sarcoidosis patients, the proportion of CD4 T cells and CD4/CD8 ratio increased, and the proportion of CD8 T cells and natural killer (NK) cells decreased. Among all single parameters, the CD4/CD8 ratio had high diagnostic sensitivity (84.4%), specificity (81.8%), and accuracy (83.3%) for sarcoidosis. Among all the combinations of three parameters, the combination of CD4, CD8, and NKT/NK ratio had high diagnostic sensitivity (91.1%), specificity (84.8%), and accuracy (87.2%) for sarcoidosis. Conclusions Assessment of LP in LNPF may improve the differential diagnostic accuracy of sarcoidosis from TBLA and further strengthen the importance of LP in LNPF in the diagnostic workup of sarcoidosis.

Abstract:OBJECTIVE:To evaluate the role of mediastinoscopy in the diagnosis of thoracic sarcoidosis.METHODS:This study was retrospective. Mediastinoscopy was performed in a total of 30 patients who had enlarged mediastinal lymph nodes on chest CT and were clinically suspected of having sarcoidosis from November 1999 to June 2004. Biopsy tissues from peritracheal, retrosternal, subcarinal, or bilateral hilar regions were obtained for pathological study.RESULTS:All the patients had a definitive pathologic diagnosis, consisting of 24 cases of stage I and 6 stage II thoracic sarcoidosis. The diagnostic consistency between preoperation and postoperation was 40% (12/30), and the misdiagnosis rate preoperation was 60% (18/30). In this series, the initial diagnosis had been malignancy in 17 cases (57%), of whom 3 had received chemotherapy at local hospitals. The overall incidences of postoperative complication and mortality were 0.CONCLUSION:Our experience showed that mediastinoscopy is a valuable procedure which helps confirm the diagnosis of mediastinal disease. Given its safety and efficacy, it should be considered in the diagnosis and differential diagnosis of thoracic sarcoidosis of stage I or II.
A case of sarcoidosis associated with lung cancer

Abstract:A 65-year-old woman presented at the First Affiliated Hospital of Zhejiang University with chest pain and cough. She had been diagnosed with sarcoidosis 4 years ago. Two years after the initial diagnosis and treatment, she presented with multiple facial subcutaneous nodules, which were pathologically diagnosed as sarcoidosis after biopsy at another hospital. The aforementioned symptom recurred on September 22, 2009, and computed tomography of the chest performed at the time revealed lymphadenectasis at both the hila of the lungs and at the mediastinum, in addition to the appearance of nodular lesions at the apex of upper lobe of the left lung, which was indicative of lung cancer. Radical resection of the cancer was performed on October 26, 2009. The results of postoperative pathological examinations showed evidence of posterior segment adenocarcinoma at the apex of the upper lobe of the left lung as well as granulomatous inflammation (on inspection of the lymph nodes), thereby confirming the diagnosis of sarcoidosis. The patient underwent postoperative chemotherapy and has since been maintained in a stable condition.

Abstract:Background: Fibrosing mediastinitis (FM) secondary to atypical sarcoidosis (atypical presentation of sarcoidosis) is rarely reported at home and abroad. Its clinical manifestations represent a lack of specificity, and the initial diagnosis is frequently difficult. In particular, this case has multiple pulmonary nodules with mediastinal lymph node enlargement and bilateral pleural effusion, and pulmonary fibrosis still exists after treatment, which is inconsistent with any clinical stage of pulmonary sarcoidosis, further increasing the diagnostic difficulty. We retrospectively analyzed the clinical data of a case of FM secondary to atypical sarcoidosis diagnosed by endobronchial ultrasound-guided cautery-assisted transbronchial mediastinal cryobiopsy (EBUS-CA-TBMCB) in Chongqing University Fuling Hospital, to improve clinicians' attention to FM and understand that EBUS-CA-TBMCB remains an effective way of etiological diagnosis. Case description: A 70-year-old man was hospitalized with cough and dyspnea for two months. After admission, through chest computed tomography (CT), ultrasound guided bilateral lung biopsy, left parietal pleural biopsy, and EBUS-CA-TBMCB, the final diagnosis was atypical sarcoidosis secondary FM. After taking glucocorticoid orally, the patient's condition improved significantly, and was discharged from the hospital. We continued following up outside the hospital, and the patient's condition was further improved. Conclusions: The diagnosis of FM is mainly based on typical imaging manifestations. When the contrast-enhanced chest CT finds localized or diffuse soft tissue density shadows around the mediastinum and pulmonary hilum with an irregular shape, with or without calcification, particular attention should be paid to exclude FM. EBUS-CA-TBMCB, as an improved minimally invasive method, can obtain enough tissue samples for pathological diagnosis, which may be the effective biopsy method for the etiology of FM to avoid missed diagnosis and misdiagnosis in the future.

Abstract:Objective To determine the current role of mediastinoscopy in the diagnosis and diflferential diagnosis of stage Ⅰ thoracic sarcoidosis.Methods The clinical data of 60 patients with a presumptive diagnosis of stage Ⅰ thoracic sarcoidosis underwent mediastinoscopy from November 1999 to June 2007 were analyzed retrospectively.All the patients had hilum of lung and/or mediastinal lymphadenopathy with normal lung parenchyma on thoracic CT scan.Typical stage Ⅰ sarcoidosis was defined as presence of bilateral hilum of lung lymphadenopathy with/without mediastinal lymphadenopathy.Results All the patients had definitive pathologic diagnosis.Among the 33 patients with typical presentation of stage I sarcoidosis,32 patients were confirmed by pathology.One patient was reactivelymph node.Among the 27 patients with atypical patterns on CT,17 patients were confirmed by pathology.No postoperative complication and mortality occurred.Conclusion For the patient with a presumptive diagnosis of typical stage Ⅰ thoracic sareoidosis after clinical and radiological evaluation,confirmation of the diagnosis by mediastinoscopy and lymph node biopsy is unwarranted.

Abstract:Background Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is considered to have high value in the staging of mediastinal lymph nodes in lung cancer. The current study was conducted to investigate the diagnostic value of EBUS-TBNA in intrapulmonary lesions located near the central airway. Methods From September 2009 to March 2013, 66 patients with pulmonary masses located close to the central airways suspected to be lung cancer were accessed by EBUS-TBNA. Conventional bronchoscopic biopsy before EBUS-TBNA was nondiagnostic in all cases. If EBUS-TBNA did not result in a formal pathological diagnosis of malignancy, patients were subsequently referred for a surgical procedure. Results Among the 66 cases, 59 were confirmed as pulmonary malignancies by EBUS-TBNA, of which 48 cases were non-small cell lung cancer, nine were small cell lung cancer, and two were metastatic lung tumors. No evidence of malignancy was found by biopsy and histopathological examination in the other seven cases. Thoracoscopy or thoracotomy was subsequently undergone for them. Postoperative pathological examinations confirmed three cases of squamous cell carcinoma of the lung, one case of lymphoma, two cases of sclerosing hemangioma, and one case of pulmonary tuberculoma. The definitive diagnosis rate of EBUS-TBNA for intrapulmonary lesions near the central airway was 89.4%. The sensitivity, specificity, and accuracy of EBUS-TBNA in distinguishing benign from malignant intrapulmonary lesions were 93.7%, 100.0%, and 93.9%, respectively. The positive and negative predictive values were 100.0% and 42.9%, respectively. The EBUS-TBNA procedures were well-tolerated by all patients. No associated complications were observed. Conclusions For intrapulmonary lesions near the central airway highly suspected of cancer, EBUS-TBNA has satisfactory diagnostic value. However, the negative predictive value of this technique is low, so negative results obtained by EBUS-TBNA should be confirmed by other methods.

Abstract:Abstract Background Transbronchial lung biopsy (TBLB) in the diagnosis of lymphangioleiomyomatosis (LAM) is not a common approach, although TBLB is often performed in diffuse lung diseases. We aimed to examine the diagnostic value and safety of TBLB in LAM patients based on the data collected in our center. Methods We reviewed LAM patients registered in our LAM Clinic from December 8, 2006, to December 31, 2019. All patients with definite or probable diagnosis of LAM who had been examined using TBLB were included. All available pathology slides were reviewed by an experienced LAM pathologist. All complications were reviewed by the medical records and confirmed using telephone interviews. Results The pathology results of 86 patients (including 74 definite LAM and 12 probable LAM) were available. The positive rate of TBLB in LAM patients was 49/86 (57.0%). The positive rates of SMA, HMB-45, ER, and PR in LAM patients were 97.6%, 93%, 84.6%, and 78.4% respectively. The positive rate of TBLB was 40%, 60% and 60.8% in patients with CT Grade I, Grade II, and Grade III respectively, and the difference was not significant. Patients who had 3–4 or 5–6 biopsied specimens had a higher rate of diagnosis than those with 1–2 biopsied specimens. Four patients (5.6%) reported pneumothorax. No major hemoptysis was reported. Conclusions TBLB is a feasible and safe procedure for obtaining a pathological diagnosis of LAM. Taking more than 2 samples during the biopsy procedure increased the rate of diagnosis.

Abstract:Objective To improve the recognition of the parenchymal lesions in sarcoidosis and to emphasize its significance in the pathological diagnostic process.Methods Retrospective analysis was performed.Pathologically confirmed cases of sarcoidosis with lung parenchymal lesions were enrolled.Clinical features,radiologic manifestations and pathologic diagnostic procedures were analyzed.Results Twenty-six cases of sarcoidosis confirmed by pathologic study were enrolled.Chest X ray showed parenchymal lung lesions of different degree in all cases,of which 9(9/26,34.6%) were categorized as stage Ⅲ and the remaining as stage Ⅱ.Lung CT was performed in 24 cases,in which mediastinal and hilar lymphadenopathy were found in 22 cases(22/24,(91.7%)).Diffuse micro-nodular changes (19/24,79.2%) and mutifocal patchy infiltration(13/24,54.2%) were the most common parenchymal CT patterns,while the majority of the cases(20/24,83.3%) showed at least two different patterns.Lung malignancy,tuberculosis and interstitial fibrosis were the most commonly misdiagnosed entities by radiologists.For methods used to obtain pathological specimens,bronchoscopic examination with mucosal biopsy and/or transbronchial lung biopsy(TBLB) were performed in 21 cases,of which 18((18/21,)(85.7%)) achieved positive yield.3 cases with a negative yield via bronchoscopic biopsy and the 5 cases without bronchoscopic biopsy received other biopsy procedures including video-assisted thoracoscopic surgery(VATS) and mediastinal lymph node biopsy.Conclusions The concurrence of different patterns of parenchymal disease is a feature of lung involvement in sarcoidosis which is prone to be misinterpreted as malignancy and tuberculosis.Bronchoscopic examination and biopsy with the best efficiency to risk ratio is the optimal choice for a pathologic diagnosis.
Clinical and Histopathologic Characteristics of Recurrent Sarcoidosis in Posttransplant Lungs: 25 Years of Experience

Abstract:Lung transplantation is the definitive therapy for end-stage pulmonary sarcoidosis. While recurrent sarcoidosis in allografts has been described in several case reports, the incidence and clinicopathologic characteristics remain unclear. In this study, we characterize the clinical and histopathologic features of recurrent sarcoidosis diagnosed in posttransplant lung surveillance transbronchial biopsies (TBBx). We identified 35 patients who underwent lung transplant for pulmonary sarcoidosis during the study period. Among them, 18 patients (51%) experienced recurrent sarcoidosis posttransplant. These included 7 females and 11 males with mean age at recurrence of 51.6 years. The average time interval from transplant to recurrence was 252 days (22 to 984 d). All TBBx contained >4 pieces of alveolated lung tissue with no evidence of International Society for Heart and Lung Transplantation (ISHLT) grade A2, A3, or A4 acute cellular rejection; chronic rejection; or antibody-mediated rejection. There were 33 surveillance TBBx that contained granulomatous inflammation with a mean of 3.6 well-formed granulomas per TBBx (range: 1 to >20). Multinucleated giant cells were identified in 11 TBBx (33.3%), with 1 case containing asteroid bodies. While most of the granulomas were "naked granulomas," 5 cases (15.2%) showed prominent lymphoid cuffing. Two cases showed evidence of fibrosis. One of the granulomas had focal necrosis; however, no infectious organisms were identified by special stains and clinical correlation suggested this case represented recurrent sarcoidosis. Biopsies of recurrent sarcoidosis usually show multiple well-formed granulomas with giant cells in more than half of the cases, while lymphoid cuffing, fibrosis, asteroid bodies, and necrotizing granulomas are uncommon findings. Pathologists should be aware of these features, as recurrence of sarcoidosis following lung transplant occurs in more than half of patients.
Diagnostic value of endobronchial ultrasound-guided transbronchial needle aspiration in mediastinal and hilar lesions

Abstract:Objective To investigate the application, diagnostic value and safety of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in hilar and mediastinal lesions. Methods A total of 246 patients with thoracic computed tomography (CT) examinations showed mediastinal, hilar lymphadenopathy and/or pleural masses, which were examined for EBUS-TBNA in the department of respiratory medicine, Second Affiliated Hospital of Chongqing Medical University from October 2014 to September 2016.The results of cytology or pathology, the positive rate of diagnosis and the incidence of complications were analyzed. Results In the 246 patients, there were 130 cases of malignant tumors, 72 cases of benigh diseases and 26 cases of suspicious cancer, but 18 cases′ pathological diagnosis couldn′t be definite.In malignant tumors cases, 45 cases were small cell lung cancer and 39 non-small cell lung cancer, 42 cases of non-specific type, 1 case of metastatic neuroendocrine carcinoma, 2 cases of type B lymphoma and 1 case of primary mediastinal sarcoma.In benign disease, there were 22 cases of non-specific inflammation, 2 cases of mediastatic cyst, 1 case of pulmonary abscess, 1 case of pulmonary cyst with actinomycete infection, 8 cases of pneumoconiosis, 27 cases of mediastinal lymph node tuberculosis, and 11 cases of sarcoidosis. Conclusions EBUS-TBNA is of great value in the diagnosis of unexplained hilar, mediastinal lymph node enlargement or hilar and mediastinal epiphyseal lesions, especially for the diagnosis of sarcoidosis.It is also an important auxiliary examination to diagnosis of the rare hilar and mediastinal dieases. Abstract:Transition metal clusters, by virtue of their well-defined structures and unique properties, present themselves as an attractive class of structural and functional building blocks for molecular and supramolecular construction. Summarized in this Account are highlights of our efforts utilizing face-capped octahedral [Re(6)(mu(3)-Se)(8)](2+) clusters as the fundamental building units to create a wide variety of preprogrammed architectures. These include molecular "Tinkertoys", featuring stereospecific cluster units bridged by multitopic ligands and extended arrays of clusters engineered via hydrogen bonding and secondary metal-ligand coordination.

Abstract:Advances have been made in minimally invasive diagnostic procedures in sarcoidosis, including bronchoalveolar lavage (BAL), endobronchial ultrasonography-guided transbronchial needle aspiration (EBUS-TBNA), and positron emission tomography (PET). Several independent groups found almost identical predictive values of the CD4:CD8 ratio in BAL for the diagnosis of sarcoidosis. A CD4:CD8 ratio greater than 3.5 shows a high specificity of 93 to 96% for sarcoidosis, but the sensitivity is low (53 to 59%). EBUS-TBNA is a safe and useful tool for diagnosing sarcoidosis stage I and II with a sensitivity of 83 to 93% and a specificity of 100%. Novel imaging techniques have been explored, such as PET using L-[3- (18)F] fluoro-alpha-methyltyrosine ( (18)F-F MT), which is more specific for malignancy than (18)F-fluorodeoxyglucose ( (18)F-FDG)-PET. The combined modality of FMT-PET with FDG-PET could successfully discriminate sarcoidosis from malignancy. These recent developments including novel biopsy procedures and novel imaging techniques could be of value to diagnosing sarcoidosis.
