Abstract:The relationship between autophagy and immunity has been well studied. However, little is known about the role of autophagy in the immune microenvironment during the progression of dilated cardiomyopathy (DCM). Therefore, this study aims to uncover the effect of autophagy on the immune microenvironment in the context of DCM. By investigating the autophagy gene expression differences between healthy donors and DCM samples, 23 dysregulated autophagy genes were identified. Using a series of bioinformatics methods, 13 DCM-related autophagy genes were screened and used to construct a risk prediction model, which can well distinguish DCM and healthy samples. Then, the connections between autophagy and immune responses including infiltrated immunocytes, immune reaction gene-sets and human leukocyte antigen (HLA) genes were systematically evaluated. In addition, two autophagy-mediated expression patterns in DCM were determined via the unsupervised consensus clustering analysis, and the immune characteristics of different patterns were revealed. In conclusion, our study revealed the strong effect of autophagy on the DCM immune microenvironment and provided new insights to understand the pathogenesis and treatment of DCM.
Identification and Functional Prediction of Long Non-Coding RNAs in Dilated Cardiomyopathy by Bioinformatics Analysis

Abstract:Dilated cardiomyopathy (DCM) is a relatively common cause of heart failure and the leading cause of heart transplantation. Aberrant changes in long non-coding RNAs (lncRNAs) are involved in DCM disorder; however, the detailed mechanisms underlying DCM initiation and progression require further investigation, and new molecular targets are needed. Here, we obtained lncRNA-expression profiles associated with DCM and non-failing hearts through microarray probe-sequence re-annotation. Weighted gene co-expression network analysis revealed a module highly associated with DCM status. Then eight hub lncRNAs in this module (FGD5-AS1, AC009113.1, WDFY3-AS2, NIFK-AS1, ZNF571-AS1, MIR100HG, AC079089.1, and EIF3J-AS1) were identified. All hub lncRNAs except ZNF571-AS1 were predicted as localizing to the cytoplasm. As a possible mechanism of DCM pathogenesis, we predicted that these hub lncRNAs might exert functions by acting as competing endogenous RNAs (ceRNAs). Furthermore, we found that the above results can be essentially reproduced in an independent external dataset. We observed the localization of hub lncRNAs by RNA-FISH in human aortic smooth muscle cells and confirmed the upregulation of the hub lncRNAs in DCM patients through quantitative RT-PCR. In conclusion, these findings identified eight candidate lncRNAs associated with DCM disease and revealed their potential involvement in DCM partly through ceRNA crosstalk. Our results facilitate the discovery of therapeutic targets and enhance the understanding of DCM pathogenesis.
Early Detection of Coronary Microvascular Dysfunction Using Machine Learning Algorithm Based on Vectorcardiography and Cardiodynamicsgram Features

Abstract:Purpose: As a main etiology of myocardial ischemia, coronary microvascular dysfunction (CMD) can occur in patients with or without obstructive coronary artery disease. Currently, there is a lack of a non-invasive approach for early detection of CMD.Aim: We aim to develop a multilayer perceptron (MLP) algorithm to achieve non-invasive early detection of CMD based on vectorcardiography (VCG) and cardiodynamicsgram (CDG) features. Methods: Electrocardiograms of 82 CMD patients and 107 healthy controls were collected and synthesized into VCGs. The VCGs' ST-T segments were extracted and fed into a deterministic learning algorithm to develop CDGs. Temporal heterogeneity index, spatial heterogeneity index, sample entropy, approximate entropy, and complexity index were extracted from VCGs' ST-T segments and CDGs, entitled as STT- and CDG-based features, respectively. The most effective feature subsets were determined from CDG-based, STT-based, and the combined features (i.e., all features) via the sequential backward selection algorithm as inputs for CDG-, STT-, and CDG-STT-based MLP models optimized with an improved sparrow search algorithm, respectively. Finally, the classification capacity of the corresponding models was evaluated via five-fold cross-validations and tested on a testing dataset to verify the optimal one.Results: The CDG-STT-based MLP model had significantly higher evaluated metrics than CDG-and STT-based ones on the validation dataset, with the accuracy, sensitivity, specificity, F1 score, and AUC of 0.904, 0.925, 0.870, 0.870, and 0.897 on the testing dataset respectively.Conclusions: The MLP model based on VCG and CDG features showed high efficiency in identifying CMD. The CDG-STT-based MLP model may afford a potential computer-aided tool for non-invasive detection of CMD.(c) 2023 AGBM. Published by Elsevier Masson SAS. Novel Algorithm for Diagnosis of Arrhythmogenic Cardiomyopathy and Dilated Cardiomyopathy: Key Gene Expression Profiling Using Machine Learning

Abstract:BACKGROUND:It is difficult to distinguish between arrhythmogenic cardiomyopathy (ACM) and dilated cardiomyopathy (DCM) because of their similar clinical manifestations. This study aimed to develop a novel diagnostic algorithm for distinguishing ACM from DCM.METHODS:Two public datasets containing human ACM and DCM myocardial samples were used. Consensus clustering, non-negative matrix factorization and principal component analysis were applied. Weighted gene co-expression network analysis and machine learning methods, including random forest and the least absolute shrinkage and selection operator, were used to identify candidate genes. Receiver operating characteristic curves and nomograms were performed to estimate diagnostic efficacy, and Spearman's correlation analysis was used to assess the correlation between candidate genes and cardiac function indices.RESULTS:Both ACM and DCM showed highly similar gene expression patterns in the clustering analyses. Hub gene modules associated with cardiomyopathy were obtained using weighted gene co-expression network analysis. Thirteen candidate genes were selected using machine learning algorithms, and their combination showed a high diagnostic value (area under the ROC curve = 0.86) for distinguishing ACM from DCM. In addition, TATA-box binding protein associated factor 15 showed a negative correlation with cardiac index (R = -0.54, p = 0.0054) and left ventricular ejection fraction (R = -0.48, p = 0.0015).CONCLUSIONS:Our study revealed an effective diagnostic model with key gene signatures, which indicates a potential tool to differentiate between ACM and DCM in clinical practice. In addition, we identified several genes that are highly related to cardiac function, which may contribute to our understanding of ACM and DCM.

Abstract:Canine Dilated Cardiomyopathy is a common disease characterized by progressive loss of myocardial systolic function and serious dilation of atrium and ventricle.Despite its relatively high prevalence,the molecular mechanisms of this disease are poorly understood.In this study,the DCM expression chip data in the NCBI′s GEO database were analyzed by gene set enrichment analysis(GSEA).A total of 4 gene pathways and the 15 significantly up-regulated GO were found.These significant gene sets can be used as candidate pathways of canine DCM,which established a basis for further early diagnosis,prevention and personalized treatment.

Abstract:Objective To identify the characteristic genes and immune infiltration in dilated cardiomyopathy (DCM) by bioinformatic analysis. Methods We identified differentially expressed genes (DEG) on two DCM gene expression data sets, and performed gene ontology (GO), disease ontology (DO), and gene set enrichment analysis (GSEA) functional enrichment to obtain potential pathways. Two machine learning algorithms including support vector machine recursive feature elimination (SVM-RFE) algorithm and Least Absolute Shrinkage and Selection Operator (LASSO) algorithm were used to determine diagnostic markers. Finally, we used the cell type analysis tool CIBERSORT for immune cell infiltration analysis. Results A total of 51 DEGs were finally identified. Thioredoxin interacting protein (TXNIP), crystallin Mu (CRYM), heat shock 70kDa protein 1-like (HSPA1L), and eukaryotic elongation factor 1A-1 (EEF1A1) were considered candidate diagnostic markers. Enrichment analysis focused on features including cardiac processes, outer membranes of mitochondria and organelles, ubiquitin-like protein ligase, natural killer cell-mediated cytotoxicity, Th1, and Th2 cell differentiation, T cell receptor signaling pathways, and Th17 cell differentiation. Immune cell infiltration found naive B cells, neutrophils, and γT cells may be involved in the pathogenesis of DCM. Besides, neutrophils, T follicular helper cells, and M1 macrophages were highly correlated with four characteristic genes. Conclusion The four characteristic genes identified by machine learning, TXNIP, CRYM, HSPA1L, and EEF1A1, show potentially close relation to DCM. At the same time, immune cell infiltration analysis can better showcase the pathophysiological process of DCM.

Abstract:Dilated cardiomyopathy (DCM) is a condition of impaired ventricular remodeling and systolic diastole that is often complicated by arrhythmias and heart failure with a poor prognosis. This study attempted to identify autophagy-related genes (ARGs) with diagnostic biomarkers of DCM using machine learning and bioinformatics approaches. Differential analysis of whole gene microarray data of DCM from the Gene Expression Omnibus (GEO) database was performed using the NetworkAnalyst 3.0 platform. Differentially expressed genes (DEGs) matching (|log2FoldChange ≥ 0.8, p value < 0.05|) were obtained in the GSE4172 dataset by merging ARGs from the autophagy gene libraries, HADb and HAMdb, to obtain autophagy-related differentially expressed genes (AR-DEGs) in DCM. The correlation analysis of AR-DEGs and their visualization were performed using R language. Gene Ontology (GO) enrichment analysis and combined multi-database pathway analysis were served by the Enrichr online enrichment analysis platform. We used machine learning to screen the diagnostic biomarkers of DCM. The transcription factors gene regulatory network was constructed by the JASPAR database of the NetworkAnalyst 3.0 platform. We also used the drug Signatures database (DSigDB) drug database of the Enrichr platform to screen the gene target drugs for DCM. Finally, we used the DisGeNET database to analyze the comorbidities associated with DCM. In the present study, we identified 23 AR-DEGs of DCM. Eight ( PLEKHF1 , HSPG2 , HSF1 , TRIM65 , DICER1 , VDAC1 , BAD , TFEB ) molecular markers of DCM were obtained by two machine learning algorithms. Transcription factors gene regulatory network was established. Finally, 10 gene-targeted drugs and complications for DCM were identified.

Accurate detection of dilated cardiomyopathy onset through machine learning predictions from ECG data

Abstract:Background Dilated cardiomyopathy (DCM) is a primary heart muscle disease characterised by left-ventricular dilatation and reduced contractility, often associated with arrhythmia and conduction disease. DCM often has a genetic component, putting family members of diagnosed patients at risk for the disease. Stratification tools to provide personalized screening advice are currently lacking. Artificial intelligence (AI) based analysis of the electrocardiogram (ECG) offers a potential solution to this challenge. Purpose Our study aims to develop and validate a novel AI-ECG model capable of prediction and early detection of DCM. Methods An AI-ECG model was developed on a United States (US) based secondary care centre dataset including 337,463 ECGs from 50,932 patients within 4 months of diagnostic echocardiography and 265,576 ECGs from 51,086 patients prior to the first diagnostic echo. The data was split 50/10/40% for training, validation, and testing, with each subset containing 9-12% DCM positive labels. The model architecture was based on a convolutional neural network with residual blocks with a modified final layer for a discrete-time survival model. The AI-ECG score was linked to changes in the median ECG within the test set to provide explainability. External validation was performed on 68,885 diagnostic ECGs (0.08% DCM positive) from the UK biobank (UKB) and a Chinese general hospital dataset with 317,854 diagnostic ECGs (2.2% DCM positive) and 76,936 predictive ECGs (1.7% DCM positive). We performed common and rare variant association studies for the AI ECG scores on quality-controlled UKB data. Results The AI-ECG score for predicted future DCM was associated with diverse ECG features (Fig. 1). In the US cohort test set, the model achieved high diagnostic accuracy with an AUC of 0.9 (95% CI 0.89-0.90) overall, and 0.89 (0.85-0.92) in a subset excluding ischemic or valvular heart disease. External datasets showed AUCs of 0.86 (0.81-0.91) in the UKB and 0.97 (96-0.97) in the Chinese dataset. The predictive performance of the AI-ECG score, adjusted for age and sex, closely approximated echocardiographic traits (LVEF and LVEDD, Fig. 2) with a concordance index (CI) of 0.79 (0.76-0.83) and 0.81 (0.77-0.85), respectively, at 10 years of follow up (FU). Combining AI-ECG scores with echocardiographic traits enhanced the CI to 0.84 (0.8-0.87). Similar findings were observed in the Chinese dataset, with a CI of 0.87 (0.85-0.89) for the AI-ECG score and 0.9 (0.88-0.92) for echocardiographic traits. Genetic analyses in UKB identified associations between the AI-ECG score and rare TTN variants as well as 25 common variant loci. Conclusion Our explainable AI ECG model for future DCM prediction demonstrates robust performance across datasets, providing comparable predictive accuracy to echocardiographic traits up to 10 years pre-diagnosis. Genetic associations support the model's validity and provide new tools for DCM genetic discovery.

Lasso Algorithm and Support Vector Machine Strategy to Screen Pulmonary Arterial Hypertension Gene Diagnostic Markers

Abstract:BACKGROUND:This study employs machine learning strategy algorithms to screen the optimal gene signature of pulmonary arterial hypertension (PAH) under big data in the medical field.METHODS:The public database Gene Expression Omnibus (GEO) was used to analyze datasets of 32 normal controls and 37 PAH disease samples. The enrichment analysis was performed after selecting the differentially expressed genes. Two machine learning methods, the least absolute shrinkage and selection operator (LASSO) and support vector machine (SVM), were used to identify the candidate genes. The external validation data set further tests the expression level and diagnostic value of candidate diagnostic genes. The diagnostic effectiveness was evaluated by obtaining the receiver operating characteristic curve (ROC). The convolution tool CIBERSORT was used to estimate the composition pattern of the immune cell subtypes and to perform correlation analysis based on the combined training dataset.RESULTS:A total of 564 differentially expressed genes (DEGs) were screened in normal control and pulmonary hypertension samples. The enrichment analysis results were found to be closely related to cardiovascular diseases, inflammatory diseases, and immune-related pathways. The LASSO and SVM algorithms in machine learning used 5 × cross-validation to identify 9 and 7 characteristic genes. The two machine learning algorithms shared Caldesmon 1 (CALD1) and Solute Carrier Family 7 Member 11 (SLC7A11) as genetic signals highly correlated with PAH. The results showed that the area under ROC (AUC) of the specific characteristic diagnostic genes were CALD1 (AUC = 0.924) and SLC7A11 (AUC = 0.962), indicating that the two diagnostic genes have high diagnostic value.CONCLUSION:CALD1 and SLC7A11 can be used as diagnostic markers of PAH to obtain new insights for the further study of the immune mechanism involved in PAH.

Abstract:Dilated cardiomyopathy (DCM) is characterized by immune cell infiltration and can readily progress to heart failure (HF). In the study, differential expression analysis, enrichment analysis, and protein-protein interaction (PPI) network analysis were performed on DCM with HF-related datasets. The CytoHubba was used to identify hub genes. Diagnostic biomarkers were obtained by validating their expression and diagnostic value in another external dataset, and a diagnostic model was constructed. Finally, single-sample gene set enrichment analysis (ssGSEA) was used to predict immune cell infiltration in cardiac samples. The associations between diagnostic biomarkers and immune cells were investigated. The NetworkAnalyst and miRDB databases were used to predict transcription factors and microRNAs, followed by establishing regulatory networks. The DSigDB database was used to predict drug candidates. Subsequently, a mouse model of DCM with HF was used to validate the expression levels of these genes. The present study revealed that differentially expressed genes were enriched in the extracellular matrix organization, cardiac muscle hypertrophy, and other immune-related biological processes. OMD and THBS4 were finally identified, and the nomogram has satisfactory prediction and strong calibration ability. In addition, the two diagnostic biomarkers exhibited significant associations with multiple immune infiltrating cells. Finally, two TFs, 65 microRNAs, and 10 drug candidates were obtained. In animal experiments, two diagnostic biomarkers showed expression trends consistent with the results of bioinformatic analysis. OMD and THBS4 have been identified as hub immune-related diagnostic biomarkers for DCM with HF. Our research provides novel insights into the diagnosis and treatment of the disease.
Identification of the Key Genes of Immune Infiltration in Dilated Cardiomyopathy.

Abstract:Dilated cardiomyopathy (DCM) is a common cause of heart failure. In this study, we screened the immune infiltration-related genes associated with DCM to explore the potential molecular mechanisms and provide a basis for the early diagnosis and development of new immunotherapeutic targets. A dataset related to DCM was downloaded from the Gene Expression Omnibus (GEO) database. R software was applied to the genetic differential analysis of patients with DCM and healthy individuals, and the obtained differential expressed genes (DEGs) were screened for differentially expressed immune-related genes (DEIRGs) after comparison with the immune microsatellite database. Gene functional analysis established a protein interaction network (PPI). The immune infiltration in patients with DCM versus normal controls was assessed using the CIBERSORT algorithm, the hub genes were screened using the MOCDE app, and the hubs were validated in multiple datasets. A total of 246 DEGs were screened (adj. P < 0.05 and |logFC| > 0.3), and a total of 170 DEIRGs were compared. Gene Ontology analysis showed significant (adj. P < 0.05) Biological Process entries of 591, Cellular Component of 10, and Molecular Function of 39; Kyoto Encyclopedia of Genes and Genomes showed 20 significant entries, mainly focused on cytokines involved in immune-related response, etc. A protein interaction network comprising 28 hub DEGs was constructed in combination with the PPI network interactions. DEIRG was mainly distributed in the T-cell receptor pathway by immune infiltration detection analysis, and significant changes in central memory T-cells were found by analyzing T-cell-related subpathways, where INSR, HLA-B, IFITM1, and HBEGF were significantly differentially expressed. We selected 632 hospitalized patients for validation and found that INSR and HLA-B expression were associated with DCM development by Nomogram. The expression of HLA-B in peripheral blood T-cells was higher in DCM patients than in the normal group, as verified by qRT-PCR. However, the detailed mechanism needs to be further explored.

Abstract:Heart failure (HF) threatens tens of millions of people's health worldwide, which is the terminal stage in the development of majority cardiovascular diseases. Recently, an increasing number of studies have demonstrated that bioinformatics and machine learning (ML) algorithms can offer new insights into the diagnosing and treating HF. To further discover HF diagnostic genes, we utilized least absolute shrinkage and selection operator (LASSO) and Support Vector Machine (SVM) to identify novel immune-related genes. The HF dataset was obtained from the gene expression omnibus (GEO) database and three candidate genes (LCN6, MUC4, and TNFRSF13C) were finally screened. In addition, the myocardial infarction (MI) modeling experiments on mice were performed to validate the expression of LCN6, MUC4, and TNFRSF13C on experimental HF mice. Altogether, these three candidate genes are promising targets for the prediction of HF with immunological perspective.
Machine Learning in Identifying Marker Genes for Congenital Heart Diseases of Different Cardiac Cell Types

Abstract:Congenital heart disease (CHD) represents a spectrum of inborn heart defects influenced by genetic and environmental factors. This study advances the field by analyzing gene expression profiles in 21,034 cardiac fibroblasts, 73,296 cardiomyocytes, and 35,673 endothelial cells, utilizing single-cell level analysis and machine learning techniques. Six CHD conditions: dilated cardiomyopathy (DCM), donor hearts (used as healthy controls), hypertrophic cardiomyopathy (HCM), heart failure with hypoplastic left heart syndrome (HF_HLHS), Neonatal Hypoplastic Left Heart Syndrome (Neo_HLHS), and Tetralogy of Fallot (TOF), were investigated for each cardiac cell type. Each cell sample was represented by 29,266 gene features. These features were first analyzed by six feature-ranking algorithms, resulting in several feature lists. Then, these lists were fed into incremental feature selection, containing two classification algorithms, to extract essential gene features and classification rules and build efficient classifiers. The identified essential genes can be potential CHD markers in different cardiac cell types. For instance, the LASSO identified key genes specific to various heart cell types in CHD subtypes. FOXO3 was found to be up-regulated in cardiac fibroblasts for both Dilated and hypertrophic cardiomyopathy. In cardiomyocytes, distinct genes such as TMTC1, ART3, ARHGAP24, SHROOM3, and XIST were linked to dilated cardiomyopathy, Neo-Hypoplastic Left Heart Syndrome, hypertrophic cardiomyopathy, HF-Hypoplastic Left Heart Syndrome, and Tetralogy of Fallot, respectively. Endothelial cell analysis further revealed COL25A1, NFIB, and KLF7 as significant genes for dilated cardiomyopathy, hypertrophic cardiomyopathy, and Tetralogy of Fallot. LightGBM, Catboost, MCFS, RF, and XGBoost further delineated key genes for specific CHD subtypes, demonstrating the efficacy of machine learning in identifying CHD-specific genes. Additionally, this study developed quantitative rules for representing the gene expression patterns related to CHDs. This research underscores the potential of machine learning in unraveling the molecular complexities of CHD and establishes a foundation for future mechanism-based studies.

Integration of Cine-cardiac Magnetic Resonance Radiomics and Machine Learning for Differentiating Ischemic and Dilated Cardiomyopathy

Abstract:Rationale and Objectives This study aims to evaluate the capability of machine learning algorithms in utilizing radiomic features extracted from cine-cardiac magnetic resonance (CMR) sequences for differentiating between ischemic cardiomyopathy (ICM) and dilated cardiomyopathy (DCM). Materials and Methods This retrospective study included 115 cardiomyopathy patients subdivided into ICM ( n = 64) and DCM cohorts ( n = 51). We collected invasive clinical (IC), noninvasive clinical (NIC), and combined clinical (CC) feature subsets. Radiomic features were extracted from regions of interest (ROIs) in the left ventricle (LV), LV cavity (LVC), and myocardium (MYO). We tested 10 classical machine learning classifiers and validated them through fivefold cross-validation. We compared the efficacy of clinical feature-based models and radiomics-based models to identify the superior diagnostic approach. Results In the validation set, the Gaussian naive Bayes (GNB) model outperformed the other models in all categories, with areas under the curve (AUCs) of 0.879 for IC_GNB, 0.906 for NIC_GNB, and 0.906 for CC_GNB. Among the radiomics models, the MYO_LASSOCV_MLP model demonstrated the highest AUC (0.919). In the test set, the MYO_RFECV_GNB radiomics model achieved the highest AUC (0.857), surpassing the performance of the three clinical feature models (IC_GNB: 0.732; NIC_GNB: 0.75; CC_GNB: 0.786). Conclusion Radiomics models leveraging MYO images from cine-CMR exhibit promising potential for differentiating ICM from DCM, indicating the significant clinical application scope of such models. Clinical Relevance Statement The integration of radiomics models and machine learning methods utilizing cine-CMR sequences enhances the diagnostic capability to distinguish between ICM and DCM, minimizes examination risks for patients, and potentially reduces the duration of medical imaging procedures.

Abstract:We designed this study to identify potential key protein interaction networks, genes, and correlated pathways in dilated cardiomyopathy (DCM) via bioinformatics methods. We selected the GSE3586 microarray dataset, consisting of 15 dilated cardiomyopathic heart biopsy samples and 13 nonfailing heart biopsy samples. Initially, the GSE3586 dataset was downloaded and was analyzed with the limma package to identify differentially expressed genes (DEGs). A total of 172 DEGs consisting of 162 upregulated genes and ten downregulated genes in DCM were selected by the criterion of adjusted Pvalues less than 0.01 and the log2-fold change of 0.6 or greater. Gene Ontology functional enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis were performed to view the biological processes, cellular components, molecular function, and KEGG pathways of the DEGs. Next, protein-protein interactions were constructed, and the hub protein modules were identified. Then we selected the key genes DLD , UQCRC2 , DLAT , SUCLA2 , ATP5A1 , PRDX3 , FH , SDHD , and NDUFV1 , which are involved in a wide range of biological activities, such as the citrate cycle, oxidation-reduction processes and cellular respiration, and energy derivation by oxidation of organic compounds in mitochondria. Finally, we found that currently there are no related gene-targeting drugs after exploring the predicted interactions between key genes and drugs, and transcription factors. In conclusion, our study provides greater understanding of the pathogenesis and underlying molecular mechanisms in DCM. This contributes to the exploration of potential gene therapy targets.
Integrated machine learning and multimodal data fusion for patho-phenotypic feature recognition in iPSC models of dilated cardiomyopathy

Abstract:Integration of multiple data sources presents a challenge for accurate prediction of molecular patho-phenotypic features in automated analysis of data from human model systems. Here, we applied a machine learning-based data integration to distinguish patho-phenotypic features at the subcellular level for dilated cardiomyopathy (DCM). We employed a human induced pluripotent stem cell-derived cardiomyocyte (iPSC-CM) model of a DCM mutation in the sarcomere protein troponin T (TnT), TnT-R141W, compared to isogenic healthy (WT) control iPSC-CMs. We established a multimodal data fusion (MDF)-based analysis to integrate source datasets for Ca 2+ transients, force measurements, and contractility recordings. Data were acquired for three additional layer types, single cells, cell monolayers, and 3D spheroid iPSC-CM models. For data analysis, numerical conversion as well as fusion of data from Ca 2+ transients, force measurements, and contractility recordings, a non-negative blind deconvolution (NNBD)-based method was applied. Using an XGBoost algorithm, we found a high prediction accuracy for fused single cell, monolayer, and 3D spheroid iPSC-CM models (≥92 ± 0.08 %), as well as for fused Ca 2+ transient, beating force, and contractility models (>96 ± 0.04 %). Integrating MDF and XGBoost provides a highly effective analysis tool for prediction of patho-phenotypic features in complex human disease models such as DCM iPSC-CMs.

Abstract:Previous studies have revealed that patients with hypertrophic cardiomyopathy (HCM) exhibit differences in symptom severity and prognosis, indicating potential HCM subtypes among these patients. Here, 793 patients with HCM were recruited at an average follow-up of 32.78 ± 27.58 months to identify potential HCM subtypes by performing consensus clustering on the basis of their echocardiography features. Furthermore, we proposed a systematic method for illustrating the relationship between the phenotype and genotype of each HCM subtype by using machine learning modeling and interactome network detection techniques based on whole-exome sequencing data. Another independent cohort that consisted of 414 patients with HCM was recruited to replicate the findings. Consequently, two subtypes characterized by different clinical outcomes were identified in HCM. Patients with subtype 2 presented asymmetric septal hypertrophy associated with a stable course, while those with subtype 1 displayed left ventricular systolic dysfunction and aggressive progression. Machine learning modeling based on personal whole-exome data identified 46 genes with mutation burden that could accurately predict subtype propensities. Furthermore, the patients in another cohort predicted as subtype 1 by the 46-gene model presented increased left ventricular end-diastolic diameter and reduced left ventricular ejection fraction. By employing echocardiography and genetic screening for the 46 genes, HCM can be classified into two subtypes with distinct clinical outcomes.

Abstract:Background: Early precise identification of high-risk dilated cardiomyopathy (DCM) phenotype is essential for clinical decision-making and patient surveillance. The aim of the study was to assess the prognostic value of enhanced cine cardiac magnetic resonance (CMR)-based radiomics in DCM. Methods: We prospectively enrolled 401 (training set: 281; test set: 120) DCM patients. Radiomic features were extracted from enhanced cine images of entire left ventricular wall and selected by the least absolute shrinkage and selection operator. Different predictive models were built using logistic regression classifier to predict all-cause mortality and heart transplantation. Model performances were compared with the area under the receiver operating characteristic curves (AUCs). Kaplan-Meier curves, log-rank test, and Cox regression were used for survival analysis. Results: Endpoint events occurred in 65 patients over a median follow-up period of 25.4 months. 13 radiomic features were finally selected. The Rad_Combined model integrating clinical characteristics, CMR parameters and radiomics features achieved the best performance with an AUC of 0.836 and 0.835 in the training and test sets, respectively. High-risk groups with endpoint events defined by the Rad_Combined model had significantly shorter survival time than low-risk group in both the training [Hazard Ratio (HR) = 7.74, P < 0.001] and test sets (HR = 4.84, P < 0.001). Conclusion: The Rad_Combined model might serve as an effective tool to help risk stratification and clinical decision-making for patients with DCM. Abstract:Background: Ischemic cardiomyopathy (ICM) induced heart failure (HF) is one of the most common causes of death worldwide. This study aimed to find candidate genes for ICM-HF and to identify relevant biomarkers by machine learning (ML). Methods: The expression data of ICM-HF and normal samples were downloaded from Gene Expression Omnibus (GEO) database. Differentially expressed genes (DEGs) between ICM-HF and normal group were identified. Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment and gene ontology (GO) annotation analysis, protein-protein interaction (PPI) network, gene pathway enrichment analysis (GSEA), and single-sample gene set enrichment analysis (ssGSEA) were performed. Weighted gene co-expression network analysis (WGCNA) was applied to screen for disease-associated modules, and relevant genes were derived using four ML algorithms. The diagnostic values of candidate genes were assessed using receiver operating characteristic (ROC) curves. The immune cell infiltration analysis was performed between the ICM-HF and normal group. Validation was performed using another gene set. Results: A total of 313 DEGs were identified between ICM-HF and normal group of GSE57345, which were mainly enriched in biological processes and pathways related to cell cycle regulation, lipid metabolism pathways, immune response pathways, and intrinsic organelle damage regulation. GSEA results showed positive correlations with pathways such as cholesterol metabolism in the ICM-HF group compared to normal group and lipid metabolism in adipocytes. GSEA results also showed a positive correlation with pathways such as cholesterol metabolism and a negative correlation with pathways such as lipolytic presentation in adipocytes compared to normal group. Combining multiple ML and cytohubba algorithms yielded 11 relevant genes. After validation using the GSE42955 validation sets, the 7 genes obtained by the machine learning algorithm were well verified. The immune cell infiltration analysis showed significant differences in mast cells, plasma cells, naive B cells, and NK cells. Conclusion: Combined analysis using WGCNA and ML identified coiled-coil-helix-coiled-coil-helix domain containing 4 (CHCHD4), transmembrane protein 53 (TMEM53), acid phosphatase 3 (ACPP), aminoadipate-semialdehyde dehydrogenase (AASDH), purinergic receptor P2Y1 (P2RY1), caspase 3 (CASP3) and aquaporin 7 (AQP7) as potential biomarkers of ICM-HF. ICM-HF may be closely related to pathways such as mitochondrial damage and disorders of lipid metabolism, while the infiltration of multiple immune cells was identified to play a critical role in the progression of the disease.
