Abstract:Iron deficiency in pregnancy is a major public health problem that causes maternal complications. The objective of this randomized, controlled trial was to examine the bioavailability, efficacy, and safety of oral ferrous bisglycinate plus folinic acid supplementation in pregnant women with iron deficiency. Subjects (12–16 weeks of gestation, n = 120) were randomly allocated to receive oral iron as ferrous bisglycinate (equiv. iron 24 mg) in supplement form with folinic acid and multivitamins (test group, n = 60) or as ferrous fumarate (equiv. iron 66 mg iron, control group, n = 60) after breakfast daily. Iron absorption was assessed by measuring fasted serum iron levels at 1 and 2 h immediately after supplementation. Hematological biomarkers and iron status were assessed before intervention, and at 3 and 6 months. Side effects were monitored throughout the intervention. A significant increase in serum iron was seen in both groups (p < 0.001) during the bioavailability assessment; however, the test group increases were comparatively higher than the control values at each timepoint (p < 0.001). Similarly, both test and control groups demonstrated a statistically significant increases in hemoglobin (Hb) (p < 0.001), erythrocytes (p < 0.001), reticulocytes (p < 0.001), mean corpuscular volume (MCV) (p < 0.001), mean corpuscular hemoglobin (MCH) (p < 0.001), mean corpuscular hemoglobin concentration (MCHC) (p < 0.001), % transferrin saturation (p < 0.001), and ferritin (p < 0.001) at 3 and 6 months after supplementation. However, in all cases, the test group increases were numerically larger than the control group increases at each timepoint. The test intervention was also associated with significantly fewer reports of nausea, abdominal pain, bloating, constipation, or metallic taste (p < 0.001). In conclusion, ferrous bisglycinate with folinic acid as a multivitamin nutraceutical format is comparable to standard ferrous fumarate for the clinical management of iron deficiency during pregnancy, with comparatively better absorption, tolerability, and efficacy and with a lower elemental iron dosage.

Study on the changes of RBC, Hct, and Ret levels in patients with anemia during pregnancy and clinical treatment

Abstract:Objective To explore the changes of red blood cells (RBC),hematocrit (Hct),and reticulocyte percentage (Ret) levels in patients with anemia during pregnancy and coping strategies.Methods A total of 808 patients with anemia during pregnancy were selected and randomly divided into control group and observation group,404 patients in each group.The patients in control group were treated by polysaccharide iron complex capsules,and the patients in control group were treated by sucrose iron injection.The levels of RBC,Hct,Ret,serum hemoglobin (Hb),serum ferritin (SF),and serum iron (SI),clinical effects,and incidence rates of adverse delivery outcomes in the two groups were compared and analyzed.Results After treatment,the levels of RBC,Hct,and Ret in the two groups were significantly better than those before treatment,and the levels of RBC,Hct,and Ret in observation group were significantly better than those in control group (P＜0.05).After treatment,the levels of Hb,SF,and SI in the two groups were significantly better than those before treatment,and the levels of Hb,SF,and SI in observation group were significantly better than those in control group (P＜0.05).The effective rate of treatment in observation group was significantly higher than that in control group (P＜0.05),and the incidence rate of adverse delivery outcomes in observation group was significantly lower than that in control group (P＜0.05).Conclusion Sucrose iron injection can significantly improve the levels of RBC,Hct,Ret,Hb,SF,and SI,enhance the clinical effect,and reduce the incidence rate of adverse delivery outcomes in treatment of patients with anemia during pregnancy.

Abstract:Pregnancy-related anemia, primarily due to iron deficiency, poses significant risks to both maternal and fetal health. Objective: The primary objective of this study is to compare the biotechnological efficacy of intravenous iron sucrose, oral ferrous fumarate, and oral ferrous sulfate in treating pregnancy-related anemia. This condition poses significant risks to maternal and fetal health. Methods: This cross-sectional study at Shalamar Hospital, Lahore (January-December 2023), involved 270 pregnant women with iron deficiency anemia. These women were randomized into three groups (90 each) receiving intravenous iron sucrose, oral ferrous fumarate, or oral ferrous sulfate. Data collected through interviews, records, and lab tests assessed hemoglobin, ferritin, and CBC at baseline, 4, 8, and 12 weeks, along with adverse effects, treatment adherence, quality of life, and biomarkers. Statistical analysis was performed using SPSS with ANOVA and Chi-square tests (p < 0.05). Results: The study included 270 pregnant women with iron deficiency anemia, divided into three groups of 90 receiving intravenous iron sucrose, oral ferrous fumarate, or oral ferrous sulfate. After 12 weeks, intravenous iron sucrose had the highest hemoglobin (13.90 g/dL) and serum ferritin (55.23 ng/mL) levels. It also showed superior iron bioavailability (92.04%), absorption rate (85.18 μg/min), and biochemical response (89.23). Additionally, it had the lowest oxidative stress markers and fewer side effects, with higher treatment satisfaction than the other groups. Conclusion: Intravenous iron sucrose demonstrated the highest effectiveness in treating pregnancy-related anemia, significantly improving hemoglobin levels and serum ferritin compared to oral ferrous fumarate and ferrous sulfate.

Abstract:Background: Iron deficiency anemia is the most common and important public health problem all over the world in the risk group of antenatal women. Research is going on to improve the iron status of the pregnant women with different forms of iron available. In this regard Carbonyl Iron is showing promising result in improving the red cell mass with better compliance. Objective: The purpose of present study was to compare the efficacy of Ferrous Sulfate and Carbonyl Iron in the blood film of iron deficiency anemia in pregnant women. Methodology: An observational prospective study in obstetric and gynae out – patient department of Sir Salimullah Medical College and Mitford Hospital. Women between 16 – 28 weeks of pregnancy with haemoglobin level 7 – 10 gm/dl and with microcytic hypochromic blood film were included in this study. Pregnant women in 1st and 3rd trimester of pregnancy were excluded from the study. Among the study population one group was given tablet Ferrous Sulfate and another group was given capsule Carbonyl Iron. Results: At the end of study (after 12 weeks) MCV, MCH, MCHC level were more increased incase of Carbonyl Iron group than Ferrous Sulfate group which is statistically significant. A comparative study of intravenous ferricarboxymaltose and double dose oral iron therapy in treatment of anemia in pregnancy

Abstract:Background: During pregnancy, iron deficiency is due to an imbalance between demand and supply, this worsens as pregnancy advances, according to CDC (centres for disease control and prevention) anemia is defined as Hb concentration lesser than 11 gm/dl in the first and third trimesters, and/or lesser than 10.5 gm/dl in the second trimester.Methods: The prospective comparative study was planned with the objective to assess the effectiveness of intravenous ferricarboxymaltose and oral double dose iron in treating anaemia in pregnant women. This study was conducted from January 2019 to June 2020 among 100 (50 in each group) pregnant anemic women attending a rural tertiary care centre.Results: This comparative study between double dose oral iron and i.v. iron treatment showed, hemoglobin levels improved at comparable rate across both treatments, however significantly more women achieved anemia correction with ferric carboxymaltose than oral iron More women achieved significant improvement in Hb with single dose of ferric carboxymaltose, whereas with oral iron additional dose of iron supplementation was required. Treatment related adverse reactions were seen more which oral iron treatment than with FCM. With markedly higher rates of gastrointestinal disorders.Conclusions: FCM is comparable more effective and better tolerated than oral iron treatment in pregnant women. Rapid correction of anemia was seen with i.v. FCM, thus in late stage pregnancy, when time to delivery is a limiting step, administration of FCM may be a more appropriate option than oral iron for rapid and effective anemia correction.

Abstract:Primary cutaneous meningioma (PCM) is a rare tumor whose pathogenesis is quite obscure. We reported PCMs occurring on almost the same occipital region of two siblings studied by histology, immunohistochemistry, and electron microscopy. Both lesions were attached to duras, but extracranial. One lesion was histologically diagnosed as meningothelial meningioma; its tumor cells showed electron microscopically interdigitating cytoplasmic processes with junctional complexes. The other was interpreted as fibroblastic meningioma; its tumor cells were arranged linearly in a stepping‐stone arrangement and had small dense bodies in the cytoplasm. The tumor cells stained positively with anti‐vimentin antibody. Both lesions had adenomatous hyperplasia of the eccrine glands. Although the histologic and electron microscopic features of these two lesions slightly differed from each other, their pathogenesis was essentially considered to be acoelic or rudimentary meningocele.

Abstract:Therapeutic Advances in Drug Safety, Volume 14, Issue , January-December 2023. Aims:To investigate the safety of oral iron therapy in pregnant women with iron-deficiency anemia (IDA) in the real world.Methods:A retrospective analysis was performed on 1792 pregnant patients with IDA who received oral iron supplements from 12 hospitals in Shandong Province from 1 April to 31 June 2021; follow-up and adverse reactions were recorded. They were divided into six groups according to the treatment drugs.Results:The overall adverse reaction rate was 15.4%, and the main adverse reaction site was the digestive system. The incidence of all kinds of oral iron adverse reactions from high to low in order: compound ferrous sulfate and folic acid tablets (21.88%); iron proteinsuccinylate oral solution (20.90%); ferrous succinate tablets (19.76%); ferrous succinate sustained-release tablets (18.00%); iron polysaccharide complex capsule (12.06%); and iron dextran oral solution (6.94%). It was found that there was a significant difference in the incidence of adverse reactions among the six drugs (p < 0.05). Pairwise comparison showed that the incidence of adverse reactions was higher in the iron proteinsuccinylate oral solution than that in the iron polysaccharide complex capsule (p 0.05), but there was a significant difference in the incidence of adverse reactions in different gestational ages (p < 0.05). In Adverse Drug Reaction (ADR) patients, the adverse reaction result of most patients is recovery or improvement, and there was no serious adverse reaction outcome such as sequela and death.Conclusion:All the adverse reactions of oral iron were mainly gastrointestinal adverse reactions, and no heavy adverse reactions were found. Iron proteinsuccinylate oral solution has a higher incidence of adverse reactions than iron polysaccharide complex capsule. The results showed that oral iron was safer for anemia patients during pregnancy.Plain language summarySafety of oral iron in the treatment of iron-deficiency anemia during pregnancyIntroduction: The safety of different oral iron agents varies. This study compared the safety of six commonly used oral iron supplements in the treatment of iron-deficiency anemia during pregnancy, aiming to provide a reference for clinical medication.Methods: We conducted a study involving 1792 patients in 12 hospitals in Shandong Province from 1 April to 31 June 2021.Results: Among the six groups, 276 ADR patients reported 302 adverse reactions. There were significant differences in the rates of adverse reactions among the six oral iron agents, and the incidence of adverse reactions in the iron proteinsuccinylate oral solution was significantly higher than that of iron polysaccharide complex capsules. The main incidence of adverse reactions was constipation (6.96%), and most of the outcomes were cured or improved.Conclusion: In this study, there were no heavy adverse reactions. The incidence of adverse reactions of iron proteinsuccinylate oral was higher than that of iron polysaccharide compound capsule. The results showed that oral iron had a good safety in patients with anemia during pregnancy.

Treatment of anemia using parenteral iron supplements

Abstract:Background. Anemia occurs in 30 % of women of childbearing age worldwide. The main causes of iron deficiency and iron deficiency anemia (IDA) are the decrease in iron intake from food (vegetarian diet, unbalanced diet, anorexia, eating disorders), impaired iron absorption (celiac disease, malabsorption, concomitant digestive diseases, drug use), blood losses (menstrual bleeding, childbirth, surgery, gastrointestinal bleeding, blood donation), and the increased need for iron (children and adolescents, pregnancy and lactation, endurance exercise). In the 3rd trimester of pregnancy, the need for iron is almost 10 times higher than in the 1st trimester (7.5 vs. 0.8 mg per day). Objective. To describe the treatment of anemia with parenteral iron supplements. Materials and methods. Analysis of literature sources on this topic. Results and discussion. Hemoglobin content <90 g/l increases the risk of miscarriage, low birth weight and gestational age, premature birth, preeclampsia and eclampsia. Low hematocrit (<29 %) is accompanied by an increase in the probability of growth retardation and fetal death. On the part of the pregnant woman, tolerance to physical activity worsens, the probability of heart failure increases (in severe anemia), the condition of the thyroid gland and wound healing deteriorates. Among children born to mothers with IDA, the probability of IDA is higher than among children born to mothers with normal hemoglobin levels (47.2 % vs. 6.5 %; Colomer I. et al., 1990). Childbirth deepens IDA due to blood loss. During vaginal childbirth or cesarean section 300-500 ml of blood is lost, in case of profuse bleeding – 500-1000 ml, and in case of profuse bleeding, uterus atony, and in case of the disseminated intravascular coagulation – 2-3 L or even more. Iron supplements are prescribed for the treatment of IDA. They can be divided into oral (iron salts, iron complexes, elemental iron) and parenteral. Pregnant women are recommended to take 60 mg of iron per day from the beginning of gestation (as early as possible) until the end of pregnancy, as well as during the first 3 months of lactation. Oral iron preparations have a number of side effects: constipation, diarrhea, heartburn, nausea, and epigastric pain. This causes unsatisfactory adherence: one in five women stops taking iron supplements. Polymaltose iron complex is tolerated slightly better than iron sulfate. In case of intolerance to oral drugs, intravenous iron may be prescribed. Its advantages include fast action and good efficiency. Drugs for intravenous administration are divided into iron preparations and iron complexes with carbohydrates (carboxymaltose, sucrose, dextran). The sucrose complex of iron allows to reach the maximum level of hemoglobin much faster than iron sulfate (6.6 against 9.4 weeks). According to German and French guidelines for the treatment of anemia in pregnant women, if the hemoglobin is reduced to <90 g/l, IDA should be treated with intravenous iron. In the recommendations of the Asia-Pacific region, the threshold value is 100 g/l. Conclusions. 1. Insufficient iron levels before pregnancy and the increased need in iron during pregnancy justify the proactive assessment and correction of IDA in pregnant women. 2. In case of moderate anemia and early stages of pregnancy, normal iron levels can be restored before delivery with the help of oral medications. 3. Intravenous iron supplements normalize iron levels faster than oral ones.
Effectiveness of treatment of iron-deficiency anemia in infants and young children with ferrous bis-glycinate chelate

Abstract:Forty infants, 6 to 36 mo old, with iron-deficiency anemia (hemoglobin < 11 g/dL) were matched and assigned to two groups. One group received FeS0(4) and the other received ferrous bis-glycinate chelate at a dose of 5 mg of Fe daily per kilogram of body weight for 28 d. Both groups had significant hemoglobin increases (P < 0.001), but only the group treated with ferrous bis-glycinate chelate had significant increases (P < 0.005) in plasma ferritin. Apparent iron bioavailabilities were calculated at 26.7% for FeS0(4) and 90.9% for ferrous bis-glycinate chelate. Regression analysis indicated that absorption of both sources of iron were similarly regulated by the body according to changes in hemoglobin. We concluded that ferrous bis-glycinate chelate is the iron of choice for the treatment of infants with iron-deficiency anemia because of its high bioavailability and good regulation.

Abstract:Objective: Iron deficiency anemia (IDA) is a significant health problem in pregnant women. We aimed to evaluate the effect of gestational IDA and to compare the results of iron therapy in pregnant women. Methods: This retrospective cohort study of pregnant women in the third trimester analyzed the records of 63 women not taking any iron supplements, 70 women receiving intravenous ferric carboxymaltose therapy (FCM), and 73 pregnant women receiving oral iron therapy. It compared maternal and neonatal outcomes of the three groups. Results: The mean age of the 206 pregnant women included in the study was 29.19±4.34 (19-44; min–max). Only 1% of anemic women received a blood transfusion in the postpartum period. There was no difference between the groups' mean age and gestational week. The median values of gravida and parity were the same in all 3 groups (Gravida=2, Parity=1). Maternal hemoglobin (Hb) value of the non-iron group was lower than that of the other groups (p<0.001). Although the mothers' Hb levels at birth were different, the newborns' Hb values were not affected (p=0.547). Neonatal height and weight were lower in the non-iron group than in the FCM and oral iron groups (p=0.048 and p<0.001, respectively). Neonatal head circumference, 1-minute Apgar, and 5-minute Apgar scores were similar in all three groups. Conclusion: FCM is effective in pregnant women with IDA in late pregnancy and women undergoing this treatment deliver healthier newborns.
Intermittent Versus Regular Daily Regimen of Antenatal Oral Iron Supplementation for Preventing Iron Deficiency Anemia During Pregnancy

Abstract:Background: Anaemia is a medical condition that is quite frequent during pregnancy and the postpartum period globally. It poses several health hazards to both the mother and the child.Objectives: This study aimed to evaluate the efficacy and adverse effects of intermittent versus regular daily regimen of antenatal oral iron supplementation for preventing iron deficiency anemia (IDA) during pregnancy. Patients and methods: A prospective randomized controlled study was conducted at Departments of Obstetrics and Gynecology, Menoufia University Hospital and El Sahel Teaching Hospital through the period from February 2020 to December 2022. During this study, two hundred sixteen non-anemic pregnant women were enrolled and divided into 3 equal groups (each group included 72 patients): Group A (daily) received 60 mg of elemental iron + 1000 ug folic acid daily, group B (every 3 days) received 60 mg of elemental iron + 1000 ug folic acid twice weekly and group C (once weekly) received 120 mg of elemental iron + 2000 ug folic acid once weekly. Complete blood picture and serum ferritin were done at the start of the study and compared with follow up levels at 28 and 36 weeks.Result: Demographic data were matched between groups. hemoglobin, hematocrit and ferritin levels before and after iron supplementation were comparable in the three groups regardless iron supplementation regimen. Iron side effects were more frequently encountered in group A than in groups B and C (27.8% vs 15.3% and 9.7% respectively). Regression analysis revealed that regimen of iron supplementation was not predictive of final hemoglobin concentration and the only significant predictor for final hemoglobin concentration was the initial hemoglobin level.Conclusion: Intermittent (once or twice weekly) iron supplementation is advisable to non-anemic pregnant women as it is not associated with increased risk for developing anemia or ID when compared to daily regimen with fewer side effects.

Abstract:Endometriosis is a gynaecological disorder that is characterized by the growth of endometrial tissue outside the uterine cavity. In developed countries, it occurs in up to 20% of women of reproductive age and is a common cause of pelvic pain and infertility. In sub-Saharan Africa, epidemiological data on the prevalence of endometriosis among African indigenous women are meagre. In some of the few published studies, endometriosis constituted the third most common finding at laparoscopies and was reported in 15.7% of laparoscopies performed for infertility assessment. In South Africa, Wiswedel et al (1989) reported a prevalence of 2% among African indigenous women presenting to an infertility clinic compared to a prevalence of 4-6% among South Africans of mixed and white race. In Nigeria, a prevalence of 4-8% has been reported among women also presenting for assisted reproductive programmes.

Abstract:Background: Until recently, WHO recommended daily iron supplementation for all pregnant women (60 mg/d iron combined with 400ug/d folic acid) where anaemia rates exceeded 40 %. Recent studies indicate that this may pose a risk to pregnant women. Therefore, there is a need to explore screen-and-treat options to minimise iron exposure during pregnancy using an overall lower dosage of iron that would achieve equivalent results as being currently recommended by the WHO. However, there is a lack of agreement on how to best assess iron deficiency when infections are prevalent. Here, we test the use of hepcidin a peptide hormone and key regulator of iron metabolism, as a potential index for 'safe and ready to receive' iron. Design/methods: This is a 3-arm randomised-controlled proof-of-concept trial. We will test the hypothesis that a screen-and-treat approach to iron supplementation using a pre-determined hepcidin cut-off value of <2.5 ng/ml will achieve similar efficacy in preventing iron deficiency and anaemia at a lower iron dose and hence will improve safety. A sample of 462 pregnant women in rural Gambia will be randomly assigned to receive: a) UNU/UNICEF/WHO international multiple micronutrient preparation (UNIMMAP) containing 60 mg/d iron (reference arm); b) UNIMMAP containing 60 mg/d iron but based on a weekly hepcidin screening indicating if iron can be given for the next 7 days or not; c) or UNIMMAP containing 30 mg/d iron as in (b) for 12 weeks in rural Gambia. The study will test if the screen-and-treat approach is non-inferior to the reference arm using the primary endpoint of haemoglobin levels at a non-inferiority margin of 0.5 g/dl. Secondary outcomes of adverse effects, compliance and the impact of iron supplementation on susceptibility to infections will also be assessed. Discussion: This trial is expected to contribute towards minimising the exposure of pregnant women to iron that may not be needed and therefore potentially harmful. If the evidence in this study shows that the overall lower dosage of iron is non-inferior to 60 mg/day iron, this may help decrease side-effects, improve compliance and increase safety. The potential for the use of hepcidin for a simple point-of-care (PoC) diagnostic for when it is most safe and effective to give iron may improve maternal health outcomes. Abstract:Background: Iron and folic acid supplementation have been recommended in pregnancy for anaemia prevention, and may improve other maternal, pregnancy, and infant outcomes. Objectives: To examine the effects of daily oral iron supplementation during pregnancy, either alone or in combination with folic acid or with other vitamins and minerals, as an intervention in antenatal care. Search methods: We searched the Cochrane Pregnancy and Childbirth Trials Registry on 18 January 2024 (including CENTRAL, MEDLINE, Embase, CINAHL, ClinicalTrials.gov, WHO's International Clinical Trials Registry Platform, conference proceedings), and searched reference lists of retrieved studies. Selection criteria: Randomised or quasi-randomised trials that evaluated the effects of oral supplementation with daily iron, iron + folic acid, or iron + other vitamins and minerals during pregnancy were included. Data collection and analysis: Review authors independently assessed trial eligibility, ascertained trustworthiness based on pre-defined criteria, assessed risk of bias, extracted data, and conducted checks for accuracy. We used the GRADE approach to assess the certainty of the evidence for primary outcomes. We anticipated high heterogeneity amongst trials; we pooled trial results using a random-effects model (average treatment effect). Main results: We included 57 trials involving 48,971 women. A total of 40 trials compared the effects of daily oral supplements with iron to placebo or no iron; eight trials evaluated the effects of iron + folic acid compared to placebo or no iron + folic acid. Iron supplementation compared to placebo or no iron Maternal outcomes: Iron supplementation during pregnancy may reduce maternal anaemia (4.0% versus 7.4%; risk ratio (RR) 0.30, 95% confidence interval (CI) 0.20 to 0.47; 14 trials, 13,543 women; low-certainty evidence) and iron deficiency at term (44.0% versus 66.0%; RR 0.51, 95% CI 0.38 to 0.68; 8 trials, 2873 women; low-certainty evidence), and probably reduces maternal iron-deficiency anaemia at term (5.0% versus 18.4%; RR 0.41, 95% CI 0.26 to 0.63; 7 trials, 2704 women; moderate-certainty evidence), compared to placebo or no iron supplementation. There is probably little to no difference in maternal death (2 versus 4 events, RR 0.57, 95% CI 0.12 to 2.69; 3 trials, 14,060 women; moderate-certainty evidence). The evidence is very uncertain for adverse effects (21.6% versus 18.0%; RR 1.29, 95% CI 0.83 to 2.02; 12 trials, 2423 women; very low-certainty evidence) and severe anaemia (Hb < 70 g/L) in the second/third trimester (< 1% versus 3.6%; RR 0.22, 95% CI 0.01 to 3.20; 8 trials, 1398 women; very low-certainty evidence). No trials reported clinical malaria or infection during pregnancy. Infant outcomes: Women taking iron supplements are probably less likely to have infants with low birthweight (5.2% versus 6.1%; RR 0.84, 95% CI 0.72 to 0.99; 12 trials, 18,290 infants; moderate-certainty evidence), compared to placebo or no iron supplementation. However, the evidence is very uncertain for infant birthweight (MD 24.9 g, 95% CI -125.81 to 175.60; 16 trials, 18,554 infants; very low-certainty evidence). There is probably little to no difference in preterm birth (7.6% versus 8.2%; RR 0.93, 95% CI 0.84 to 1.02; 11 trials, 18,827 infants; moderate-certainty evidence) and there may be little to no difference in neonatal death (1.4% versus 1.5%, RR 0.98, 95% CI 0.77 to 1.24; 4 trials, 17,243 infants; low-certainty evidence) or congenital anomalies, including neural tube defects (41 versus 48 events; RR 0.88, 95% CI 0.58 to 1.33; 4 trials, 14,377 infants; low-certainty evidence). Iron + folic supplementation compared to placebo or no iron + folic acid Maternal outcomes: Daily oral supplementation with iron + folic acid probably reduces maternal anaemia at term (12.1% versus 25.5%; RR 0.44, 95% CI 0.30 to 0.64; 4 trials, 1962 women; moderate-certainty evidence), and may reduce maternal iron deficiency at term (3.6% versus 15%; RR 0.24, 95% CI 0.06 to 0.99; 1 trial, 131 women; low-certainty evidence), compared to placebo or no iron + folic acid. The evidence is very uncertain about the effects of iron + folic acid on maternal iron-deficiency anaemia (10.8% versus 25%; RR 0.43, 95% CI 0.17 to 1.09; 1 trial, 131 women; very low-certainty evidence), or maternal deaths (no events; 1 trial; very low-certainty evidence). The evidence is uncertain for adverse effects (21.0% versus 0.0%; RR 44.32, 95% CI 2.77 to 709.09; 1 trial, 456 women; low-certainty evidence), and the evidence is very uncertain for severe anaemia in the second or third trimester (< 1% versus 5.6%; RR 0.12, 95% CI 0.02 to 0.63; 4 trials, 506 women; very low-certainty evidence), compared to placebo or no iron + folic acid. Infant outcomes: There may be little to no difference in infant low birthweight (33.4% versus 40.2%; RR 1.07, 95% CI 0.31 to 3.74; 2 trials, 1311 infants; low-certainty evidence), comparing iron + folic acid supplementation to placebo or no iron + folic acid. Infants born to women who received iron + folic acid during pregnancy probably had higher birthweight (MD 57.73 g, 95% CI 7.66 to 107.79; 2 trials, 1365 infants; moderate-certainty evidence), compared to placebo or no iron + folic acid. There may be little to no difference in other infant outcomes, including preterm birth (19.4% versus 19.2%; RR 1.55, 95% CI 0.40 to 6.00; 3 trials, 1497 infants; low-certainty evidence), neonatal death (3.4% versus 4.2%; RR 0.81, 95% CI 0.51 to 1.30; 1 trial, 1793 infants; low-certainty evidence), or congenital anomalies (1.7% versus 2.4; RR 0.70, 95% CI 0.35 to 1.40; 1 trial, 1652 infants; low-certainty evidence), comparing iron + folic acid supplementation to placebo or no iron + folic acid. A total of 19 trials were conducted in malaria-endemic countries, or in settings with some malaria risk. No studies reported maternal clinical malaria; one study reported data on placental malaria. Authors' conclusions: Daily oral iron supplementation during pregnancy may reduce maternal anaemia and iron deficiency at term. For other maternal and infant outcomes, there was little to no difference between groups or the evidence was uncertain. Future research is needed to examine the effects of iron supplementation on other maternal and infant health outcomes, including infant iron status, growth, and development.

Abstract:Background: This study was carried out to compare the outcome of oral iron Ferrous sulphate tablets v/s parenteral Iron sucrose injection. Parenteral iron was more benecial in quickly restoring iron deciency in antenatal . Aim: To study outcome of comparison between oral and parenteral iron supplements in improving iron deciency anemia in pregnant women . Materials and Methods: This was a comparative and prospective study, conducted in Department of obstetrics and gynecology Dhiraj General hospital,Vadodara. Total 40 pregnant women with mild and moderate iron deciency anemia were taken as cases and followed up and study was conducted for 6 months. Results: Total 40 patients , 20 taking ferrous sulphate tablets and 20 given iv iron sucrose to avoid discrepancy , were studied for prevalence of iron deciency anemia . The anemic patients ranged from 20-35 years, of low socioeconomic status ,with prevalance in both the primigravida and multigravida, majorly in multigravida patients . Majority of patients were seen in 14-28 weeks gestation, which was timely managed in our setup. Majority of patients with iron deciency anemia had <50 kg weight , 13 of which had oral treatment, while 10 patients were given intravenous iron . Majority of women in this study had microcytic hypochromic iron deciency anemia with the nal mean hemoglobin after giving the treatment in oral group being less than in intravenous group . 15 patients (75%) in intravenous group and 13 patients ( 65%) in oral group did not have any side effects. Side effects like Gastrointestinal disturbance were seen more in oral group ( 5 patients i.e. 25%) while sweating and burning were seen in patients where intravenous treatment was given ( 2 patients i.e. 10%) and the nal mean hemoglobin in intravenous group is greater than the oral group. Thus, greater level of hemoglobin was achieved with intravenous mode in the same stipulated 6 months duration . Conclusion: The study depicts that intravenous iron is safe, convenient, and effective, without any serious side-effects , better tolerated by the patients with higher increase in mean hemoglobin. So, it's a good substitute to oral iron therapy in pregnant females with moderate anemia roughly at 14-28 weeks of gestation
Comparison of the Anaemia and Transfusion Rates of Pregnant Women Treated with Intravenous <em>versus</em> Oral Iron in the Third Trimester

Abstract:Objective: To evaluate and compare the blood transfusion requirements during delivery in third-trimester pregnant women with iron deficiency anaemia (IDA) who were treated with intravenous (IV) ferric carboxymaltose (FCM) versus those treated with oral iron supplementation. Study design: Comparative study. Place and Duration of the Study: Bakirkoy Dr. Sadi Konuk Training and Research Hospital, Istanbul, Turkiye, from January 2017 to December 2022. Methodology: Pregnant women with haemoglobin (Hb) levels <10 g/dL in their third trimester were included. One group (n = 50) received IV FCM, while the other group (n = 96) received oral iron therapy. Key outcome measures included Hb levels at delivery and the need for a postpartum blood transfusion. Inclusion criteria were third-trimester pregnancy with IDA, and exclusion criteria included haematological or chronic systemic diseases and high-risk pregnancies. Results: The mean initial Hb levels in the third trimester of pregnancy in the FCM group and oral iron group were 8.31 ± 0.96 g/dL and 9.29 ± 1.20 g/dL, respectively (p <0.001). The mean Hb levels in the delivery room were 11.09 ± 1.38 and 9.44 ± 1.16 g/dL, respectively (p <0.001). The rates of postpartum erythrocyte transfusion requirement were 6% (n = 3) and 18.75% (n = 18), respectively (p = 0.037). Conclusion: IV FCM administration to pregnant patients with IDA during the third trimester was found to be more effective than oral iron treatment in reducing blood transfusion rates. Abstract:Aim: There are opinions about the negative effects of the increase in iron storages or plasma iron levels during pregnancy on glucose metabolism. We aimed to investigate the effects of oral iron supplementation on glucose metabolism in non-anemic pregnant women who are in the risk group for gestational diabetes mellitus. Material and methods: While oral iron supplementation of 40 mg/day was givento 41 pregnant women in the study group (Group I) for eight weeks, oral iron supplementation was not givento 35 pregnant women in the control group (Group II). 100 g oral glucose tolerance test (OGTT) was applied for the diagnosis of GDM in pregnant women whose 50 g OGTT results were positive. Analyzing the results between groups. Results:There was no significant difference between the two groups in terms of mean fasting plasma glucose (FPG) value (p: 0.185), GDM (p:0.292) and mean results of 50gr OGLT (p: 0.109).A total of 26 pregnant women with 50 gr OGLT positive were applied 100 gr OGTT for diagnostic purposes. Conclusion:It was concluded that 40 mg/day oral iron supplement used from the first trimester in pregnant women in the risk group for GDM did not have a significant negative effect on glucose metabolism and did not cause an increase in the risk of GDM.

Abstract:In India, the national nutritional anaemia control programme (NNACP) was started in 1970 and is being implemented through primary health centers and sub centers. It recommended pregnant women to take one iron tablet per day for at least 100 days (each tablet containing 100 mg of natural iron and 500 mcg of folic acid) after first trimester of pregnancy; a similar dose applies to lactating women. Study Design: Randomized clinical trial. 160 pregnant ladies going to antenatal clinic at 14-36 weeks of gestation with Hb 7- 9.9g%. Who met the inclusion criteria were arbitrarily distributed into two groups. One with odd registration number was given iron sucrose and one with even was given FCM. Hematological profile includes Haemoglobin, Reticulocyte count, Blood indices (MCH, MCHC, MCV) Fringe smear, Serum iron and Serum TIBC. 52% cases from FCM group observed in the age gathering of 21-24 years and 39% cases from Iron Sucrose group were observed in the age gathering of 25-28 years. 54% cases from FCM and 46% cases from Iron sucrose were observed in the gestational Age of 29 to 36 weeks. 43% cases from FCM and 46% cases from Iron sucrose were primigravida. Patients from FCM group and 4 patients from Iron sucrose group did not follow up. 2 pts. From FCM group and 1 pt. from Iron sucrose group refused treatment. The rise in mean Hb level after 1 week and 4 weeks of treatment with FCM is more than Iron sucrose. The rise in mean Sr. Iron level after 1 week and after 4 weeks of treatment with FCM is more than Iron sucrose. The reduction in mean Sr. TIBC level after 4 weeks of treatment with Iron sucrose is more than FCM. The rise in mean MCH, MCHC and MCV levels after 1 week and 4 weeks of treatment with FCM is more than Iron sucrose. There was no serious adverse effect noted in both groups.
