Abstract:This paper describes an electrical model of cardiac ventricles incorporating real geometry and motion. The heart anatomy and its motion through the cardiac cycle are obtained from segmentations of multiple-slice MRI time sequences; the special conduction system is constructed using an automated mapping procedure from an existing static heart model. The heart model is mounted in an anatomically realistic voxelmodel of the human body. The cardiac electrical source and surface potentials are determined numerically using both a finitedifference scheme and a boundary-element method with the incorporation of the motion of the heart. The electrocardiograms (ECG) and body surface potential maps are calculated and compared to the static simulation in the resting heart. The simulations demonstrate that introducing motion into the cardiac model modifies the ECG signals, with the most obvious change occurring during the T-wave at peak contraction of the ventricles. Body surface potential maps differ in some local positions during the T-wave, which may be of importance to a number of cardiac models, including those incorporating inverse methods.
Influence of magnetically-induced E-fields on cardiac electric activity during MRI: A modeling study.

Abstract:In modern magnetic resonance imaging (MRI), patients are exposed to strong, time-varying gradient magnetic fields that may be able to induce electric fields (E-fields)/currents in tissues approaching the level of physiological significance. In this work we present theoretical investigations into induced E-fields in the thorax, and evaluate their potential influence on cardiac electric activity under the assumption that the sites of maximum E-field correspond to the myocardial stimulation threshold (an abnormal circumstance). Whole-body cylindrical and planar gradient coils were included in the model. The calculations of the induced fields are based on an efficient, quasi-static, finite-difference scheme and an anatomically realistic, whole-body model. The potential for cardiac stimulation was evaluated using an electrical model of the heart. Twelve-lead electrocardiogram (ECG) signals were simulated and inspected for arrhythmias caused by the applied fields for both healthy and diseased hearts. The simulations show that the shape of the thorax and the conductive paths significantly influence induced E-fields. In healthy patients, these fields are not sufficient to elicit serious arrhythmias with the use of contemporary gradient sets. However, raising the strength and number of repeated switching episodes of gradients, as is certainly possible in local chest gradient sets, could expose patients to increased risk. For patients with cardiac disease, the risk factors are elevated. By the use of this model, the sensitivity of cardiac pathologies, such as abnormal conductive pathways, to the induced fields generated by an MRI sequence can be investigated. Towards the Cardiac Equivalent Source Models in Electrocardiography and Magnetocardiography: A Simulation Study

Abstract:The cardiac equivalent source model is very crucial in the electrocardiogram (ECG) and magnetocardiography (MCG) problems. In this study, we presented a thoroughly investigation of the three models: the dipole, equivalent double layer (EDL) and epicardial potential (EP) models in terms of the ECG/MCG forward problem within a unified framework. The cardiac electromagnetic field is calculated using boundary element method (BEM). The numerical performance and the properties of the three source models are studied in detail. Three different resolutions of dipole and two of EP and EDL models are studied. Besides, the effect of the volume conductor model is studied. The simulation results demonstrated that the resolution of the source models shouldn't be too small, otherwise some information about the cardiac activities will be lost as shown in the single dipole model situation. The effect of the resolution on the MCG is larger than that on BSP. The higher resolutions of the source, the larger effect of the volume conductor happened for each source. While for the EDL source, the lung has little effect on the MCG. The EDL source can generate the similar performance compared to the dipole source, and the largest difference happened at the time when the ventricle start or finish the activation. The presented study suggests that the EDL source is a good choice as source model for ECG/MCG problem

Abstract:OBJECTIVES:In this paper, we present a unified electrodynamic heart model that permits simulations of the body surface potentials generated by the heart in motion. The inclusion of motion in the heart model significantly improves the accuracy of the simulated body surface potentials and therefore also the 12-lead ECG.METHODS:The key step is to construct an electromechanical heart model. The cardiac excitation propagation is simulated by an electrical heart model, and the resulting cardiac active forces are used to calculate the ventricular wall motion based on a mechanical model. The source-field point relative position changes during heart systole and diastole. These can be obtained, and then used to calculate body surface ECG based on the electrical heart-torso model.RESULTS:An electromechanical biventricular heart model is constructed and a standard 12-lead ECG is simulated. Compared with a simulated ECG based on the static electrical heart model, the simulated ECG based on the dynamic heart model is more accordant with a clinically recorded ECG, especially for the ST segment and T wave of a V1-V6 lead ECG. For slight-degree myocardial ischemia ECG simulation, the ST segment and T wave changes can be observed from the simulated ECG based on a dynamic heart model, while the ST segment and T wave of simulated ECG based on a static heart model is almost unchanged when compared with a normal ECG.CONCLUSIONS:This study confirms the importance of the mechanical factor in the ECG simulation. The dynamic heart model could provide more accurate ECG simulation, especially for myocardial ischemia or infarction simulation, since the main ECG changes occur at the ST segment and T wave, which correspond with cardiac systole and diastole phases.
Modeling and Simulation of Transmural Cellular Electromechanical Properties in Heart Failure

Abstract:In this paper, we present a simulation study of transmural cellular electromechanical properties in heart failure based on a integrated and refined heart cell model of Luo-Rudy II model and modified Hunter-McCullochter Keurs mechanical model. The results show that prolonged action potential duration (APD) and APD rate-adaptation are more obviously transmurally heterogeneous in heart failure than those in control hearts. The results suggest that the differences of the electrical responses between failing cells and normal cells can cause slowing relaxation of the Ca2+ transient, and the difference of the Ca2+-TnC concentrations between fast and slow myocytes in failing hearts is smaller than in nonfailing hearts. It results in a decrease of force, which might diminish the role of mechanoelectric feedback and then induce an increase of transmural APD gradient that might cause arrhythmia in heart failure. These results are in good accordance with experimental findings reported in the literatures and this electromechanical cardiac cell model might be very useful for further modeling and simulation of heart failure at both the tissue and the whole organ levels.

Abstract:We have developed a canine cardiac cellular electromechanics model to simulate electrophysiological remodeling of heart failure (HF) and predicted cardiomyocyte contractility after HF. I(Na,L) is integrated into this model to study its role to the prolongation of action potential (AP) in control and HF conditions, which was not established well in the past. It may have a great contribution to prolongation of AP in control and even greater contribution to that of HF. Ionic remolding after HF is modeled by downregulation of I(tol), I(Kl), I(Ks), SR pump function and upregulation of Na(+)-Ca(2+) exchange (NCX) and I(Na,L). The HF model could successfully simulate the prolonged AP, reduced I(CaL), enhanced I(NaCa) and blunted Ca(2+) transient. With computed Ca(2+) being the input to myofilament model, myofilament forces are determined. Compared with control, reduced amplitude, increased latency to onset of contraction, increased time to peak (TTP) and attenuated cell shortening are found in HF model. The model could also be embedded into tissue electromechanics model to simulate the altered activation sequence and mechanical function.
Effects of the Fibroblast-Myocyte in Cardiac Electromechanical Coupling: A Preliminary Simulation Study

Abstract:The heart consists of myocytes, vasculature cells and connective tissue cells. In this study, two ventricular electromechanical models were coupled with the fibroblast model. At the cellular level, Niederer-Smith (NS) model of rat ventricular myocyte and ten Tusscher model of human ventricular myocyte and the passive fibroblast model are combined with Rice model of contraction and cooperativity mechanisms. At the tissue level, excited conduction is integrated with elastic mechanics. Numerically, the finite difference method solves the excitation equations, and the finite element method settles the equations governing tissue mechanics. The results showed that fibroblasts slow down wave propagation and increase mesh contraction. The influence of fibroblasts on cardiac excitation and contraction should be pursued in future heart modeling studies

Abstract:Rationale and Objectives: Chronic obstructive pulmonary disease (COPD) is highly heterogeneous and not well understood. Hyperpolarized xenon-129 (Xe129) magnetic resonance imaging (MRI) provides a unique way to assess important lung functions such as gas uptake. In this pilot study, we exploited multiple imaging modalities, including computed tomography (CT), gadolinium-enhanced perfusion MRI, and Xe129 MRI, to perform a detailed investigation of changes in lung morphology and functions in COPD. Utility and strengths of Xe129 MRI in assessing COPD were also evaluated against the other imaging modalities. Materials and Methods: Four COPD patients and four age-matched normal subjects participated in this study. Lung tissue density measured by CT, perfusion measures from gadolinium-enhanced MRI, and ventilation and gas uptake measures from Xe129 MRI were calculated for individual lung lobes to assess regional changes in lung morphology and function, and to investigate correlations among the different imaging modalities. Results: No significant differences were found for all measures among the five lobes in either the COPD or age-matched normal group. Strong correlations (F? > 0.5 or < -0.5, p < 0.001) were found between ventilation and perfusion measures. Also gas uptake by blood as measured by Xe129 MRI showed strong correlations with CT tissue density and ventilation measures (R > 0.5 or < -0.5, p < 0.001) and moderate to strong correlations with perfusion measures (R > 0.4 or < -0.5, p < 0.01). Four distinctive patterns of functional abnormalities were found in patients with COPD. Conclusion: Xe129 MRI has high potential to uniquely identify multiple changes in lung physiology in COPD using a single breath-hold acquisition.
Computational modelling of blood-flow-induced changes in blood electrical conductivity and its contribution to the impedance cardiogram

Abstract:Studies have shown that blood-flow-induced change in electrical conductivity is of equal importance in assessment of the impedance cardiogram (ICG) as are volumetric changes attributed to the motion of heart, lungs and blood vessels. To better understand the sole effect of time-varying blood conductivity on the spatiotemporal distribution of trans-thoracic electric fields (i.e. ICG), this paper presents a segmented high-resolution (1 mm(3)) thoracic cardiovascular system, in which the time-varying pressures, flows and electrical conductivities of blood in different vessels are evaluated using a set of coupled nonlinear differential equations, red blood cell orientation and cardiac cycle functions. Electric field and voltage simulations are performed using two and four electrode configurations delivering a small alternating electric current to an anatomically realistic and electrically accurate model of modelled human torso. The simulations provide a three-dimensional electric field distribution and show that the time-varying blood conductivity alters the voltage potential difference between the electrodes by a maximum of 0.28% for a cardiac output of about 5 L min(-1). As part of a larger study, it is hoped that this initial model will be useful in providing improved insights into blood-flow-related spatiotemporal electric field variations and assist in the optimal placement of electrodes in impedance cardiography experiments.

Abstract:Classical homogenization theory based on the Hashin-Shtrikman coated ellipsoids is used to model the changes in the complex valued conductivity (or admittivity) of a lung during tidal breathing. Here, the lung is modeled as a two-phase composite material where the alveolar air-filling corresponds to the inclusion phase. The theory predicts a linear relationship between the real and the imaginary parts of the change in the complex valued conductivity of a lung during tidal breathing, and where the loss cotangent of the change is approximately the same as of the effective background conductivity and hence easy to estimate. The theory is illustrated with numerical examples, as well as by using reconstructed Electrical Impedance Tomography (EIT) images based on clinical data from an ongoing study within the EU-funded CRADL project. The theory may be potentially useful for improving the imaging algorithms and clinical evaluations in connection with lung EIT for respiratory management and monitoring in neonatal intensive care units.

Abstract:Computational imaging of personalized cardiac electrophysiology has attracted increasing research interest because of its clinical relevance in aiding in the diagnosis and prediction of cardiac electrical malfunctions of individual subjects. We have developed a statistical physiological-model-constrained framework that, rather than delivering a personalized cardiac electrophysiological model with customized parameters, uses simple standard electrophysiological models as constraints and produces maximum a posteriori estimation of three-dimensionally distributed transmembrane potential (TMP) dynamics inside the ventricular myocardium of individual subjects [1]. Taking part in 2010 Cardiac Electrophysiological Simulation Challenge (CESC'10), we modify this framework to use epicardial optical mapping data to estimate subject-specific TMP dynamics inside the 3D myocardium. Results of estimated dynamics are compared to the simulations by the same electrophysiological model with standard or adjusted parameters. As shown, while it is rather challenging to personalize the parameters of a cardiac electrophysiological model for the entire 3D myocardium, because of the drastically simplified model structure and limited subject's data, the presented approach of TMP estimation is able to computationally reproduce subject-specific electrical functions inside the 3D myocardium with simple standard model as constraints.
Modeling ecological drivers in marine viral communities using comparative metagenomics and network analyses

Abstract:Significance Microorganisms and their viruses are increasingly recognized as drivers of myriad ecosystem processes. However, our knowledge of their roles is limited by the inability of culture-dependent and culture-independent (e.g., metagenomics) methods to be fully implemented at scales relevant to the diversity found in nature. Here we combine advances in bioinformatics (shared k-mer analyses) and social networking (regression modeling) to develop an annotation- and assembly-free visualization and analytical strategy for comparative metagenomics that uses all the data in a unified statistical framework. Application to 32 Pacific Ocean viromes, the first large-scale quantitative viral metagenomic dataset, tested existing and generated further hypotheses about ecological drivers of viral community structure. Highly computationally scalable, this new approach enables diverse sequence-based large-scale comparative studies. Long-standing questions in marine viral ecology are centered on understanding how viral assemblages change along gradients in space and time. However, investigating these fundamental ecological questions has been challenging due to incomplete representation of naturally occurring viral diversity in single gene- or morphology-based studies and an inability to identify up to 90% of reads in viral metagenomes (viromes). Although protein clustering techniques provide a significant advance by helping organize this unknown metagenomic sequence space, they typically use only ∼75% of the data and rely on assembly methods not yet tuned for naturally occurring sequence variation. Here, we introduce an annotation- and assembly-free strategy for comparative metagenomics that combines shared k-mer and social network analyses (regression modeling). This robust statistical framework enables visualization of complex sample networks and determination of ecological factors driving community structure. Application to 32 viromes from the Pacific Ocean Virome dataset identified clusters of samples broadly delineated by photic zone and revealed that geographic region, depth, and proximity to shore were significant predictors of community structure. Within subsets of this dataset, depth, season, and oxygen concentration were significant drivers of viral community structure at a single open ocean station, whereas variability along onshore–offshore transects was driven by oxygen concentration in an area with an oxygen minimum zone and not depth or proximity to shore, as might be expected. Together these results demonstrate that this highly scalable approach using complete metagenomic network-based comparisons can both test and generate hypotheses for ecological investigation of viral and microbial communities in nature.

Abstract:This paper describes a method for estimating lung density, air volume and changes in fluid content from a non-invasive measurement of the electrical resistivity of the lungs. Resistivity in Ω m was found by fitting measured electrical impedance tomography (EIT) data to a finite difference model of the thorax. Lung density was determined by comparing the resistivity of the lungs, measured at a relatively high frequency, with values predicted from a published model of lung structure. Lung air volume can then be calculated if total lung weight is also known. Temporal changes in lung fluid content will produce proportional changes in lung density. The method was implemented on EIT data, collected using eight electrodes placed in a single plane around the thorax, from 46 adult male subjects and 36 adult female subjects. Mean lung densities (±SD) of 246 ± 67 and 239 ± 64 kg m(-3), respectively, were obtained. In seven adult male subjects estimates of 1.68 ± 0.30, 3.42 ± 0.49 and 4.40 ± 0.53 l in residual volume, functional residual capacity and vital capacity, respectively, were obtained. Sources of error are discussed. It is concluded that absolute differences in lung density of about 30% and changes over time of less than 30% should be detected using the current technology in normal subjects. These changes would result from approximately 300 ml increase in lung fluid. The method proposed could be used for non-invasive monitoring of total lung air and fluid content in normal subjects but needs to be assessed in patients with lung disease.

Abstract:Objective Numerous clinical and pathological studies have confirmed that lung injury can cause cardiovascular disease, but there is no explanation for the mechanism by which the degree of lung injury affects cardiac function. We attempt to reveal this mechanism of influence by simulating a cyclic model. Method This study established a closed-loop cardiovascular model with a series of electrical parameters. Including the heart, lungs, arteries, veins, etc., each part of the cardiovascular system is modeled using centralized parameters. Adjusting these lung resistances to alter the degree of lung injury is aimed at reflecting the impact of different degrees of lung injury on cardiac function. Finally, analyze and compare the changes in blood pressure, aortic flow, atrioventricular volume, and atrioventricular pressure among different lung injuries to obtain the changes in cardiac function. Results In this model, the peak aortic flow decreased, the earlier the trough appeared, and the total aortic flow decreased. Left atrial blood pressure decreased from 6.5 mmHg to around 5.5 mmHg, left ventricular blood pressure decreased from 100 mmHg to around 50 mmHg, and aortic blood pressure also decreased from 100 mmHg to around 50 mmHg. The blood pressure in the pulmonary artery, right atrium, and right ventricle increases. The right ventricular blood pressure decreased from 20 mmHg to around 40 mmHg, while the right atrial blood pressure slightly increased. It can be seen that the increase in impedance has a greater impact on ventricular blood pressure than on atrium. Pulmonary arterial pressure significantly increases, rising from 20 mmHg to around 50 mmHg, forming pulmonary hypertension. The left ventricular end-systolic potential energy, filling energy, stroke work, stroke output, left ventricular filling period, maximum blood pressure during ventricular ejection period, and stroke energy efficiency decrease. Conclusion We established a closed-loop cardiovascular model that reveals that the more severe lung injury, the higher blood pressure in the pulmonary artery, right atrium, and right ventricle, while the lower blood pressure in the left atrium, left ventricle, and aorta. The increase in pulmonary impedance leads to abnormalities in myocardial contraction, diastolic function, and cardiac reserve capacity, leading to a decrease in cardiac function. This closed-loop model provides a method for pre assessment of cardiovascular disease after lung injury.

In this study, we investigate the feasibility to use an alternative geometrical model that excludes local details but captures subject-specific global geometrical parameters that have been regarded essential for reliable IECG solutions. This is done by using limited images and image metadata to customize a pre-defined, generic ventricle and electrode-array representation to subject-specific ventricle size, position, orientation and electrode position on the body surface. We apply this simplified geometrical modeling in IECG studies of post myocardial infarction patients; the results of transmembrane potential imaging and infarct quantitation are compared with the gold standard and results from the same IECG approach using traditional, detailed heart-torso model. This study shows that local geometrical details do not have significant impact on IECG solutions and excluding them from geometrical modeling might be of potential to drive cardiac electrophysiological imaging closer towards clinical practicability.

Abstract:Clinical translation of computational models of the heart has been hampered by the absence of complete and rigorous technical and clinical validation, as well as benchmarking of the developed tools. To address this issue, a dataset containing the cardiac anatomy and fibre orientations from magnetic resonance images (MRI), as well as epicardial transmembrane potentials from optical mapping acquired on ex-vivo porcine hearts, have previously been made available to the community. Image processing techniques were developed to integrate MRI images with electrical information. Different models were tested and compared with the integrated data1, including: i) a new methodology to customize and regularize heart shape and myocardial fibre orientation to predict activation waves in these personalized meshes with generic conduction parameters; ii) a statistical model-constrained framework to produce maximum a posteriori (MAP) estimation of the 3D distribution of transmembrane potential; iii) a personalized simplified reaction-diffusion 3D electrophysiological model; iv) a personalized fast conduction Purkinje system with a simple Eikonal-based electrophysiological model.

Abstract:Electrical Impedance Tomography (EIT) is a functional imaging method that is being developed for bedside use in critical care medicine. Aiming at improving the chest anatomical resolution of EIT images we developed a fuzzy model based on EIT high temporal resolution and the functional information contained in the pulmonary perfusion and ventilation signals. EIT data from an experimental animal model were collected during normal ventilation and apnea while an injection of hypertonic saline was used as a reference . The fuzzy model was elaborated in three parts: a modeling of the heart, a pulmonary map from ventilation images and, a pulmonary map from perfusion images. Image segmentation was performed using a threshold method and a ventilation/perfusion map was generated. EIT images treated by the fuzzy model were compared with the hypertonic saline injection method and CT-scan images, presenting good results in both qualitative (the image obtained by the model was very similar to that of the CT-scan) and quantitative (the ROC curve provided an area equal to 0.93) point of view. Undoubtedly, these results represent an important step in the EIT images area, since they open the possibility of developing EIT-based bedside clinical methods, which are not available nowadays. These achievements could serve as the base to develop EIT diagnosis system for some life-threatening diseases commonly found in critical care medicine.
