Abstract:Background: Although compassion is a crucial element of physicians' professional performance and high-quality care, research shows it often remains an unmet need of patients. Understanding patients' and physicians' perspectives on compassionate care may provide insights that can be used to foster physicians' ability to respond to patients' compassion needs. Therefore, this study aims to understand how both patients and physicians experience the concept and practice of compassionate care. Methods: We conducted semi-structured interviews with eight patients and ten resident physicians at a University Medical Center in the Netherlands. Using thematic analysis, we separately coded patient and resident transcripts to identify themes capturing their experiences of compassionate care. This study was part of a larger project to develop an educational intervention to improve compassion in residents. Results: For both patients and residents, we identified four themes encompassing compassionate care: being there, empathizing, actions to relieve patients' suffering, and connection. For residents, a fifth theme was professional fulfillment (resulting from compassionate care). Although patients and residents both emphasized the importance of compassionate care, patients did not always perceive the physician-patient encounter as compassionate. According to residents, high workloads and time pressures hindered their ability to provide compassionate care. Discussion and conclusion: Patients and residents have similar and varying understandings of compassionate care at the same time. Understanding these differences can aid compassion in medical practice. Based on the findings, three topics are suggested to improve compassion in residents: (1) train residents how to ask for patients' compassion needs, (2) address residents' limiting beliefs about the concept and practice of compassion, and (3) acknowledge the art and science of medicine cannot be separated.

Abstract:In recent years, Dutch authorities have attempted to shift from formal to informal care responsibilities to meet an increasing demand for care. There is a growing expectation that social and healthcare professionals will actively involve, support, and collaborate with partners, family members, and others who provide informal care. However, the practical implementation of such collaborations is not yet evident. In this qualitative study, 37 individual interviews and eight focus groups were conducted to explore the preferences of informal caregivers in the Netherlands regarding their collaboration with social and healthcare professionals. In recognition of the growing consensus that professionals should tailor their approach to caregivers' individual circumstances, we adopted an intersectional approach to examine the relationship between caregivers' social positions and preferences as well as the impact of systemic and power dynamics on these collaborations. The findings indicate that most of caregivers' worries concerning collaborations are rooted in the organisational context of their interactions with professionals rather than the interactions themselves. Caregivers' social positions do not necessarily lead to varying opinions regarding collaboration with professionals; however, caregivers do expect professionals to be aware of their social positions, as these positions shape the context within which informal care is provided and determine how easily caregivers can integrate care responsibilities into their lives. To enhance collaboration between caregivers and professionals in the short term, care organisations and social and healthcare educational programmes should encourage a professional mindset that recognises caregivers as equal partners in care. It is essential for professionals to acknowledge the caregiver's role and devise strategies for arranging care together that build on the existing relationship between the caregiver and care recipient.

Abstract:Public services are stressed by soaring demand, inadequate capacity, staff burnout and low morale with strikes reflecting profound unhappiness. With so many difficulties, the fundamental need for compassion in public services has been eroded, with adverse consequences for patients and their families or carers. The need for compassion is not only an individual’s humanitarian and professional responsibility but one that should be given high priority by organisations, educational institutions, and professional bodies. The care of patients with dementia and support for families or carers provides a perfect example of such disparities and their devastating impact. The implications for families affected by dementia in minority ethnic or marginalised communities are considered alongside the need for a ‘paradigm shift’ to patient-centred, ‘needs-based’ services to improve outcomes. This article explores and attempts to extrapolate the relevance of personal experience to those in minority ethnic or marginalised communities on whom there is limited information and those who have little voice or advocacy.1 There is a distinct role for charitable and third-sector organisations with a deeper understanding of the cultural context to demonstrate leadership in moving policy and resources to address such societal and service disparities.
Narrative exploration of empathy acceptance on emotional communication changes between mother and child relationships

Abstract:Objectives In this study, it is a field study that explores the process of change in maternal and child emotional communication through the acceptance of empathy by counselors. Methods This study conducted a narrative inquiry to understand in-depth the life experience of empathy acceptance on mother-child relationship emotional communication. Eight and 10 sessions of counseling and mock counseling were conducted for youth siblings in Chungcheongnam-do, respectively, and additional counseling was conducted to verify their effectiveness in the future. Focusing on the experience of maternal and child relations, the experience of the research participants formed in the continuity of time, interaction, and experience in the social context were analyzed and interpreted. Results Studies have shown that ‘kindness and warmth found in the sparkling eyes’, ‘recognition of stability and self-value through mirroring’, ‘promoting stability and internalization of self-regulation through empathy’, ‘expanding self-understanding through empathy’, ‘understanding and acceptance of new family concepts through empathy’, ‘recovery of family’ and ‘restoration’. In addition, the family relationship of the mother-child relationship, which changed positively in future counseling, continued to be maintained, and positive emotional communication was being carried out with a hopeful plan for the future. Conclusions In this study, positive emotional changes and healing processes were found in changes in emotional communication between mother and child relations by the counselor's acceptance of empathy. Based on the relationship between the counselor's acceptance of empathy and the participant's emotions, the need for relationship-based counseling and programs was presented.

Emotional communication in home care: A comparison between Norway and Sweden

Abstract:Objective: Given the free movement of workers across countries, knowledge regarding communication differences between countries is imperative. In this study, we explored and compared the supportive responses of nursing staff to older persons' emotions in home care in Norway and Sweden. Methods: The study had an observational, cross-sectional, comparative design, which included 383 audio-recorded home-care visits. Communication was coded using Verona Coding Definitions of Emotional Sequences. Worries and responses were categorised with regard to reference, communicative function and level of person-centredness. Standard statistical tests were used to analyse the data. Results: The Swedish nursing staff provided space for further disclosure of worry more frequently than the Norwegian nursing staff (75.0 % versus 60.2 %, χ2 = 20.758, p < 0.01). In all, 65 % of the responses were supportive. Multiple logistic regression analyses showed that highly person-centred responses were independently associated with worries phrasing an emotion, OR (95 % CI) 3.282 (1.524-7.067). Conclusion: The level of person-centredness was associated with the way in which older persons expressed their distress. The Swedish nursing staff provided opportunities for further disclosure of worries more frequently than the Norwegian nursing staff. Practice implications: Findings of intercultural differences should be incorporated into the training of nursing staff.

Abstract:ConclusionsA group of physicians of different ages and background gathered together to express their hope for a person-based medicine. The inescapable conclusion of their encounter is that care begets more care, whereas lack of care is contrary to one’s sense of mission and self-worth and generates the need for escapes from oneself. The translation of this concept into clinical practice lies in our ability to communicate our experience in our most common seat of encounter: when we are with another human being in need.

Abstract:Abstract Providing healthcare services that respect and meet patients’ and caregivers’ needs are essential in promoting positive care outcomes and perceptions of quality of care, thereby fulfilling a significant aspect of patient-centered care requirement. Effective communication between patients and healthcare providers is crucial for the provision of patient care and recovery. Hence, patient-centered communication is fundamental to ensuring optimal health outcomes, reflecting long-held nursing values that care must be individualized and responsive to patient health concerns, beliefs, and contextual variables. Achieving patient-centered care and communication in nurse-patient clinical interactions is complex as there are always institutional, communication, environmental, and personal/behavioural related barriers. To promote patient-centered care, healthcare professionals must identify these barriers and facitators of both patient-centered care and communication, given their interconnections in clinical interactions. A person-centered care and communication continuum (PC4 Model) is thus proposed to orient healthcare professionals to care practices, discourse contexts, and communication contents and forms that can enhance or impede the acheivement of patient-centered care in clinical practice.

Abstract:The ‘Cultivating Compassion’ project was developed in response to a research and innovation call relating to compassion training for National Health Service staff in the South East of England. The project aims included: the use of Appreciative Inquiry to develop, implement and evaluate a sustainable and evidence based programme of compassion awareness training through engaging with a diverse group of health professionals and support staff; an evaluation of a ‘train the trainers’ approach; and an evaluation of ‘compassion lead’ roles and a multi-modal compassion toolkit. The project team included academics from two universities and one medical school, NHS staff from three separate organisations, and service users. The participants recruited to the study included doctors, nurses, receptionists, chaplains and others working in close contact with service users from within four NHS organisations in the South East of England. The main findings from the project using thematic analysis from participant focus groups and interviews identified: project enablers and inhibitors; the value of project resources; and shifts in perspectives. Project conclusions highlighted the importance of effective senior level support and organisational leadership in cultivating compassion within a healthcare organisation and the importance of the integration of compassion-promoting resources within existing staff development initiatives.

Manual and software-driven processes guided coding and analysis of data. Caring theory guided the analysis of themes; while COREQ criteria guided research conduct and reporting.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Four key themes emerged; (a) Challenges in communication, (b) The science versus art of nursing; (c) The impact of ineffective communication and (d) Strategies for coping. Eleven sub-themes are reported within these themes. Overall, nurses felt they had sufficient language and nursing skills to undertake the technical or scientific aspects of their work; however, they reported experiencing restricted ability to participate in complex cross-cultural conversations, such as providing explanations and reassurance about treatment options or discussing end of life and treatment decisions. This limitation diminished the nurses' ability to engage in the art of nursing and left them unable to employ themselves therapeutically to attain a sense of true presence with patients and their families.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>This article highlights the need for language and communication support, and Arabic-speaking advocates as partners in care for expatriate nurses.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Relevance to practice</h3><p>Internationally qualified nurses in this Middle Eastern setting lack cultural orientation and language skills to fully enact the art and true presence of nursing. [An adult case of asymptomatic congenital tracheal stenosis].

Abstract:A 42-year-old woman was admitted with abnormal chest radiographs. Though interstitial pneumonia associated with dermatomyositis was diagnosed, her chest radiograph also revealed a narrowed trachea about 6 mm in diameter. Bronchoscopy showed that her trachea lacked a membranous posterior segment and O-shaped complete tracheal rings were present throughout the trachea, indicating congenital tracheal stenosis. Congenital tracheal stenosis is a rare disorder and is usually recognized in the first few weeks of life, but the patient had no history of dyspnea or recurrent pneumonia. This case suggests that among healthy people there are a very few who have asymptomatic congenital tracheal stenosis.

Complementary Therapies in Clinical Practice

Abstract:Background and Purpose: Complementary medicine such as yoga, massage and art therapy has become increasingly popular among patients with cancer. However, the topic remains under-discussed during oncology consultations: patients seem hesitant to disclose complementary medicine use, and healthcare providers lack resources to discuss complementary medicine. This study aims to gain an understanding of how to improve communication and information provision in oncological settings about complementary medicine by assessing the experiences and needs of patients and healthcare providers. Materials and Methods Semi-structured interviews were conducted with 17 patients with cancer and 13 oncology healthcare providers recruited from two general hospitals and one breast cancer center in the Netherlands. Nine (former) patients with breast cancer collaborated with the research team as 'co-researchers'. Reflexive thematic analysis was used. Results The main themes identified were barriers to patient-provider communication about complementary medicine (e.g. lack of time and knowledge among healthcare providers, negative attitudes toward complementary medicine), facilitators of communication (e.g. openness of healthcare providers, complementary medicine as a routine topic) and information provision needs (e.g. easy access to information, the hospital being involved in providing information). Conclusion Patients with cancer and healthcare providers report issues with the current approach to discussing complementary medicine and are of the opinion that complementary medicine should be a routine topic in oncology consultations. Future studies should focus on effective methods for standardizing complementary medicine discussions into oncology care and making reliable information available for patients and healthcare providers.

Abstract:<p>In this call to action, a coalition of Indigenous and non-Indigenous researchers from Australia, Aotearoa New Zealand, United States and Canada argue for the urgent need for adequately funded Indigenous-led solutions to perinatal health inequities for Indigenous families in well-resourced settler-colonial countries. Authors describe examples of successful community-driven programs making a difference and call on all peoples to support and resource Indigenous-led perinatal health services by providing practical actions for individuals and different groups.</p>

Abstract:BACKGROUND: Caring for cancer patients can take a toll on the emotional health of oncology nurses, which may lead to compassion fatigue, resulting in decreased quality of nursing care, absenteeism and decreased retention of staff.AIM: The aim of this study was to describe compassion fatigue from the perspective of oncology nurses. This study is part of a larger mixed-methods action research study to develop an in-facility intervention to manage compassion fatigue in oncology nurses.SETTING: This study was conducted at Durban, KwaZulu-Natal, South Africa.METHODS: The research setting comprised one state hospital (with oncology clinics and wards), a private hospital (with oncology wards) and a hospice in Durban, KwaZulu-Natal, South Africa. Semi-structured individual interviews (guided by Figley's Compassion Fatigue Process, 2005) were conducted with eight participants. Data were analysed using manifest content analysis.RESULTS: Five categories emerged from the data, namely, emotional connection, emotional fatigue, emotional loss, blurring boundaries and acceptance.CONCLUSION: The findings revealed that oncology nurses are affected emotionally in caring for their patients, thus making them prone to compassion fatigue. Oncology nurses need to acknowledge compassion fatigue and be able to self-reflect on how they are managing (both positively and negatively) with the stressors encountered in the oncology wards or units.

Communication strategies for expressing empathy during family-clinician conversations in the intensive care unit: A mixed methods study

Abstract:Objectives: To explore communication strategies intensive care clinicians and patients' family members prefer for expressing empathy during family-clinician conversations. Research methodology/design: Mixed-methods survey study. Setting: Two medical ICUs of a German academic tertiary care hospital. Main outcome measures: Using a self-developed online survey with closed and open-ended questions with free-text options, the participants' preferences of communication strategies for expressing empathy were investigated. Quantifiable similarities and differences were determined by statistical analysis. Qualitative themes were derived at by directed content analysis. Findings: The responses of 94 family members, 42 nurses, and 28 physicians were analysed (response rate: 45.3 %). Four communication strategies were deduced: (1) reassuring the families that the intensive care unit team will not abandon neither them nor the patient, (2) acknowledging emotions and offering support, (3) saying that the families are welcome and cared for in the intensive care unit, (4) providing understandable information. In comparison, the families considered an expression of nonabandonment as more empathic than the physicians did (p =.031,r = 0.240), and those expressions focussing solely on the family members' well-being (p =.012,r = 0.228) or comprising evaluative wording ("good", "normal") (p =.017,r = 0.242) as less empathic than the nurses did. Unanimously advocated nonverbal communication strategies included to listen attentively and to avoid interrupting as well as being approachable and honest. Conclusion: The participants' preferences supported expert recommendations and highlighted that it is not only important what the clinicians say but also how they say it. Further research is needed to elucidate ways of successfully expressing empathy during family-clinician conversations in the intensive care unit. Implications for clinical practice: Intensive care unit clinicians are encouraged to practice active listening and to express their caring and nonabandonment. It is further suggested to reflect on and adjust pertinent nonverbal behaviours and relational aspects of their communication, as applicable.

Abstract:Background Extensive nursing literature exists on the term person-centred care. Nevertheless, there is scant literature defining or explicating the concept. Rather, necessary contextual pre-conditions are often presented under the guise of explicit definitions. Person-centredness appears to be explained as a culture that could allow person-centred care to exist. This raises problems knowing what is being provided and what is being received in a person-centred care experience. Contemporary issue Within a caring space, a nurse providing person-centred care assumes an ontological perspective to provide such care. Specifically, it is typically assumed that the care giver is oriented to a single objective reality and the needs and desires of the care receiver can and should be judged from that perspective. However, when perceived reality noticeably differs between people, this ontological position may not hold. This is particularly apparent when a care receiver is experiencing dementia or other conditions which appear to alter conscious experience. This fundamental problem raises the contemporary issue of the nature of consciousness, as reality is perceived in the context of consciousness. Discussion Some recent evidence such as that presented by Hoffman, suggests that consciousness may be more fundamental than physiology. This may form a basis for connecting with and understanding the experience of another. Consistent with this perspective, we introduce the concept of a Multiple Ego State System (MESSy). This concept may be a starting point for a theory-based person-centred approach to nursing which truly honours the perspective of the care receiver. Conclusions An ontological position inclusive of all individual realities as parts of one consciousness is an attractive position for the theoretical placement of person-centred care. Foundational work is therefore needed to explicate this concept.
