Abstract:Background: Depression is a common mental illness that is widely recognized by its lack of pleasure, fatigue, low mood, and, in severe cases, even suicidal tendencies. Photobiomodulation (PBM) is a non-invasive neuromodulation technique that could treat patients with mood disorders such as depression. Methods: A systematic search of ten databases, including randomized controlled trials (RCTs) for depression, was conducted from the time of library construction to September 25, 2023. The primary outcome was depression. The secondary outcome was sleep. Meta-analysis was performed using RevMan (version 5.4) and Stata (version 14.0). Subgroup analyses were performed to identify sources of heterogeneity. The certainty of the evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE). Results: Three thousand two hundred and sixty-five studies were retrieved from the database and screened for inclusion in eleven trials. The forest plot results demonstrated that PBM alleviated depression (SMD = −0.55, 95% CI [−0.75, −0.35], I 2 = 46%). But it is not statistically significant for patients' sleep outcomes (SMD = −0.82, 95% CI [−2.41, 0.77], I 2 = 0%, p > 0.05). Subgroup analysis showed that s-PBM was superior to t-PBM in relieving symptoms of depression. The best improvement for t-PBM was achieved using a wavelength of 823 nm, fluence of 10–100 J/cm 2 , irradiance of 50–100 mW/cm 2 , irradiance time of 30 min, treatment frequency 15 times. The best improvement for s-PBM was achieved using a wavelength of 808 nm, fluence ≤1 J/cm 2 , irradiance of 50–100 mW/cm 2 , irradiance time ≤ 5 min, treatment frequency ≥ 3/week, number of treatments >15 times. All results had evidence quality that was either moderate or very low, and there was no bias in publication. Conclusion: We conclude that PBM is effective in reducing depression symptoms in patients. However, the current number of studies is small, and further studies are needed to extend the current analysis results. [Repetitive Transcranial Magnetic Stimulation Combined with Antidepressant Medication in Treatment of First-Episode Patients with Major Depression].

Abstract:OBJECTIVE:To assess the early therapeutic and cognitive effect of repetitive transcranial magnetic stimulation (rTMS) combined with antidepressant medication in treatment of first-episode patients with major depression.METHODS:Sixty first-episode depressed inpatients aged 18-45 y, who met the DSM-IV clinical criteria for major depressive episode were randomly assigned to citalopram treatment (20 mg/d) in combination with a two-week period of either rTMS (study group)or sham procedure (control group) on left dorsal-lateral prefrontal cortex (10 Hz, 90% motor threshold). The Hamilton Depression Rating Scale (HAMD) was used to assess the severity of depression. The Wisconsin Card Sorting Test (WCST) and Continuous Performance Test (CPT) were used to assess cognitive function of depression.RESULT:The response rate was significantly greater in the study group compared to the control group after treatment (57% compared with 29%,P<0.05). The HAMD scores significantly declined after treatment in two groups, and the study group showed lower scores compared to the control group after 2 weeks (P<0.01). Neuropsychological assessments showed that there was no significant difference between the two groups except for the significant improvement in the categories on WCST in study group compared to the baseline (P<0.05) and the control group (P<0.05)after 2 weeks treatment. No serious event occurred in the patients during the rTMS study.CONCLUSION:10 Hz rTMS enhances early effects of citalopram and improves cognitive function in first-episode major depressive patients.

Abstract:This study aimed to evaluate the dose-dependent brain temperature effects of transcranial photobiomodulation (t-PBM). Thirty adult subjects with major depressive disorder were randomized to three t-PBM sessions with different doses (low: 50 mW/cm 2 , medium: 300 mW/cm 2 , high: 850 mW/cm 2 ) and a sham treatment. The low and medium doses were administered in continuous wave mode, while the high dose was administered in pulsed wave mode. A 3T MRI scanner was used to perform proton magnetic resonance spectroscopy ( 1 H-MRS). A voxel with a volume of 30 × 30 × 15 mm 3 was placed on the left prefrontal region. Brain temperature (°C) was derived by analyzing 1 H-MRS spectrum chemical shift differences between the water (~ 4.7 ppm) and N-acetyl aspartate (NAA) (~ 2.01 ppm) peaks. After quality control of the data, the following group numbers were available for both pre- and post-temperature estimations: sham ( n = 10), low ( n = 11), medium ( n = 10), and high ( n = 8). We did not detect significant temperature differences for any t-PBM-active or sham groups post-irradiation (p-value range = 0.105 and 0.781). We also tested for potential differences in the pre-post variability of brain temperature in each group. As for t-PBM active groups, the lowest fluctuation (variance) was observed for the medium dose (σ 2 = 0.29), followed by the low dose (σ 2 = 0.47), and the highest fluctuation was for the high dose (σ 2 = 0.67). t-PBM sham condition showed the overall lowest fluctuation (σ 2 = 0.11). Our 1 H-MRS thermometry results showed no significant brain temperature elevations during t-PBM administration.

Abstract:Depression, a multifactorial mental disorder, characterized by cognitive slowing, anxiety, and impaired cognitive function, imposes a significant burden on public health. Photobiomodulation (PBM), involving exposure to sunlight or artificial light at a specific intensity and wavelength for a determined duration, influences brain activity, functional connectivity, and plasticity. It is recognized for its therapeutic efficacy in treating depression, yet its molecular and cellular underpinnings remain obscure. Here, we investigated the impact of PBM with 468 nm light on depression-like behavior and neuronal damage in the chronic unpredictable mild stress (CUMS) murine model, a commonly employed animal model for studying depression. Our results demonstrate that PBM treatment ameliorated behavioral deficits, inhibited neuroinflammation and apoptosis, and notably rejuvenates the hippocampal synaptic function in depressed mice, which may be mainly attributed to the up-regulation of brain-derived neurotrophic factor signaling pathways. In addition, in vitro experiments with a corticosterone-induced hippocampal neuron injury model demonstrate reduced oxidative stress and improved mitochondrial function, further validating the therapeutic potential of PBM. In summary, these findings suggest PBM as a promising, non-invasive treatment for depression, offering insights into its biological mechanisms and potential for clinical application.

Abstract:Transcranial photobiomodulation refers to irradiation of the brain through the skull using low-intensity red or near-infrared light, which is the most commonly studied method of light energy biotherapy for central nervous system disorders. The absorption of photons by specific chromophores within the cell elevates ATP synthesis, reduces oxidative stress damage, alleviates inflammation or mediates the activation of transcription factors and signaling mediators through secondary mediators, which in turn trigger downstream signaling pathways to cause a series of photobiological effects including upregulation of neurotrophic factors. Multiple mechanisms are simultaneously involved in the pathological process of central nervous system disorders. The pleiotropic treatment of transcranial photobiomodulation towards multiple targets plays a beneficial role in improving hemodynamics, neural repair and improving behaviors in central nervous system disorders such as ischemic stroke, traumatic brain injury, neurodegenerative diseases, epilepsy and depression. This review mainly introduces the mechanism and recent preclinical and clinical advances of transcranial photobiomodulation for central nervous system disorders, which will provide a reference for clinicians to understand and engage in related studies, and calls for more and larger studies to validate and develop a wider application of transcranial photobiomodulation in central nervous system.

Abstract:Background: This study addresses the urgent need for effective alternatives to treat major depressive disorder (MDD) in patients who do not respond to conventional therapies Transcranial photobiomodulation therapy (tPBM) shows promise by enhancing mitochondrial function and reducing oxidative stress, as demonstrated in the chronic mild stress (CMS) rat model. To analyze the impact of tPBM with two wavelengths (red and infrared) on behavioral and biological parameters related to MDD in a CMS model. Methods: Male Wistar rats were subjected to CMS for five weeks and categorized into resilient (CMS-R) and susceptible (CMS-S) groups using the sucrose consumption test (SCT). The CMS-S group received tPBM treatment (600nm and 840nm) for five weeks. Biological measures included lipid damage (TBARS), antioxidant defense (TEAC), mitochondrial complex IV activity (CCO), and nitric oxide (NO) concentration in the prefrontal cortex and blood were measured. Results: As expected, post-tPBM treatment (both red and infrared groups) exhibited increased sucrose consumption compared to the sham (Kruskal Wallis chi-squared=26.131; p<0.001). The red and infrared presented higher serum TEAC levels than the sham and control groups, but these effects did not reach statistical significance (p=0.306). In contrast, the red group showed lower peripheral TBARS levels (M = 9.50, SD = 2.87) than the sham group (M = 13.66, SD = 2.20) (p=0.0048); such effect was similar to the control non-stress group. The infrared group showed higher NO levels within the hippocampus than the sham group (Mean = 107.83; SD = 6.48, Dunn Test p = 0.0134) and higher prefrontal CCO activity levels than the red group (p=0.012), which was similar to the control non-stress group. Conclusions: Our study demonstrated that tPBM using both red and infrared wavelengths significantly improved behavioral and biological parameters in the chronic mild stress (CMS) rat model. In particular, tPBM may offer therapeutic benefits by ameliorating oxidative stress and enhancing mitochondrial function, thereby presenting a promising alternative for the management of MDD.

Abstract:Background: Transcranial alternating current stimulation (tACS) offers a new approach for adult patients with major depressive disorder (MDD). The study is to evaluate the efficacy and safety of tACS treating MDD. Methods: This is an 8-week, double-blind, randomized, placebo-controlled study. Ninety-two drug-naive patients with MDD aged 18 to 65 years will receive 20 daily 40-min, 77.5-Hz, 15-mA sessions of active or sham tACS targeting the forehead and both mastoid areas on weekdays for 4 consecutive weeks (week 4), following a 4-week observation period (week 8). The primary outcome is the remission rate defined as the 17-item Hamilton depression rating scale (HDRS-17) score ≤7 at week 8. Secondary outcomes are the rates of response at weeks 4 and 8 and rate of remission at week 4 based on HDRS-17, the proportion of participants having improvement in the clinical global impression-improvement, the change in HDRS-17 score (range, 0-52, with higher scores indicating more depression) over the study, and variations of brain imaging and neurocognition from baseline to week 4. Safety will be assessed by vital signs at weeks 4 and 8, and adverse events will be collected during the entire study. Discussion: The tACS applied in this trial may have treatment effects on MDD with minimal side effects. Abstract:Abstract Treatment of depression with antidepressants is partly effective. Transcranial alternating current stimulation can provide a non-pharmacological alternative for adult patients with major depressive disorder. However, no study has used the stimulation to treat first-episode and drug-naïve patients with major depressive disorder. We used a randomized, double-blind, sham-controlled design to examine the clinical efficacy and safety of the stimulation in treating first-episode drug-naïve patients in a Chinese Han population. From 4 June 2018 to 30 December 2019, 100 patients were recruited and randomly assigned to receive 20 daily 40-min, 77.5 Hz, 15 mA, one forehead and two mastoid sessions of active or sham stimulation (n = 50 for each group) in four consecutive weeks (Week 4), and were followed for additional 4-week efficacy/safety assessment without stimulation (Week 8). The primary outcome was a remission rate defined as the 17-item Hamilton Depression Rating Scale (HDRS-17) score ≤ 7 at Week 8. Secondary analyses were response rates (defined as a reduction of ≥ 50% in the HDRS-17), changes in depressive symptoms and severity from baseline to Week 4 and Week 8, and rates of adverse events. Data were analysed in an intention-to-treat sample. Forty-nine in the active and 46 in the sham completed the study. Twenty-seven of 50 (54%) in the active treatment group and 9 of 50 (18%) in the sham group achieved remission at the end of Week 8. The remission rate was significantly higher in the active group compared to that in the sham group with a risk ratio of 1.78 (95% confidence interval, 1.29, 2.47). Compared with the sham, the active group had a significantly higher remission rate at Week 4, response rates at Weeks 4 and 8, and a larger reduction in depressive symptoms from baseline to Weeks 4 and 8. Adverse events were similar between the groups. In conclusion, the stimulation on the frontal cortex and two mastoids significantly improved symptoms in first-episode drug-naïve patients with major depressive disorder and may be considered as a non-pharmacological intervention for them in an outpatient setting.

Abstract:Background: Repetitive transcranial magnetic stimulation (rTMS) has been proven to be safe and effective in treating major depressive disorder (MDD). However, the treatment parameters of rTMS are still divergent and need to be optimized further. The aim of this study was to compare the efficacy of rTMS in treating MDD with different parameters of stimulating frequency and location, and course of treatment. Methods: A total of 221 patients with MDD were recruited in the randomized, double-blind, controlled trial. All eligible patients were randomly assigned into four treatment groups: (1) 10 Hz in left dorsolateral pre-frontal cortex (DLPFC) (n = 55), (2) 5 Hz in left DLPFC (n = 53), (3) 10 Hz in bilateral DLPFC (n = 57), and (4) 5 Hz in bilateral DLPFC (n = 56). The patients received treatment for 6 weeks and an additional 6-week optional treatment. The efficacies were evaluated by Hamilton Depression Rating Scale-24 items (HDRS) and Clinical Global Impressions Scale (CGI). The trial is registered at the Chinese Clinical Trial Registry as ChiCTR-TRC-12002248. Results: The ANOVAs of HDRS scores up to 6 weeks and 12 weeks with repeated measure of time showed a significant effect of duration without statistical difference among four treatment groups and no significance when time was interacted with inter-group as well. The response rates up until the 5th week were significantly different with the previous week. Conclusions: It concludes that there were no statistical differences in the efficacy of rTMS between unilateral left and bilateral DLPFC, and between 5 and 10 Hz for treating MDD.

The intensity of electricity is 2 mA, lasting for 30 minutes over two consecutive working days (10 sessions in total). The medication lasts for 6 weeks. The primary outcome measure was the response rates within 6 weeks (week 6 is also the endpoint of the study), and secondary outcome measures included changes in other clinical measurements. Safety and acceptability are measured by adverse event rates and dropout rates. Exploring outcome consist of the performance of cognitive battery as well as neurophysiology results. Conclusion: To the best of our knowledge, the present study is the first double-blind controlled study comparing tDCS, tACS, and clinically used antidepressants, which will provide further evidence for their efficacy and safety in possible clinical applications. Currently, the predominant treatment for MDD is antidepressant drugs, which only brings about 1/3 clinical remission rate. 2 , 3 Meanwhile, antidepressant drugs usually result in undesirable side effects and undergo 2–4 weeks to achieve clinical efficacy. Therefore, it is necessary to explore other alternative efficient approaches. Non-invasive physical therapy interventions, including transcranial direct current stimulation (tDCS) and transcranial alternating current stimulation (tACS), have attracted the researchers' attention. Both tDCS and tACS stimulate the brain through scalp electrodes to modulate cortical activity, whose benefits include painlessness, causing few adverse events, portable, easy to operate, and economic. TDCS delivers direct currents (0.5–2 mA) to the scalp to modulate the neural activity of the cerebrum. 4 , 5 Its exact mechanism has yet to be defined. 5 The stimulatory effect of tDCS is characterized by polarity regulation: the anode current promotes depolarization of neuronal membrane potentials to enhance cortical excitability, and the cathode current induces hyperpolarization changes to inhibit cortical excitability. 6 TDCS potentially improves cognitive performances, such as reduction in response time 7 and vigilance to threat, 8 and improved emotion recognition. 9 However, there is insufficient evidence to state that tDCS has a positive or negative effect as a treatment for depression cognitive dysfunction 10 and should therefore be further investigated. Abstract:Abstract Background In recent years, accelerated transcranial magnetic stimulation (aTMS) has been developed, which has a shortened treatment period. The aim of this study was to evaluate the efficacy and long-term maintenance effects of aTMS in patients with major depressive disorder (MDD). Methods We systematically searched online databases for aTMS studies in patients with MDD published before February 2023 and performed a meta-analysis on the extracted data. Results Four randomized controlled trials (RCTs) and 10 before-and-after controlled studies were included. The findings showed that depression scores significantly decreased following the intervention (SMD = 1.80, 95% CI (1.31, 2.30), p < 0.00001). There was no significant difference in antidepressant effectiveness between aTMS and standard TMS (SMD = -0.67, 95% CI (-1.62, 0.27), p = 0.16). Depression scores at follow-up were lower than those directly after the intervention based on the depression rating scale (SMD = 0.22, 95% CI (0.06, 0.37), p = 0.006), suggesting a potential long-term maintenance effect of aTMS. Subgroup meta-analysis results indicated that different modes of aTMS may have diverse long-term effects. At the end of treatment with the accelerated repetitive transcranial magnetic stimulation (arTMS) mode, depressive symptoms may continue to improve (SMD = 0.29, 95% CI (0.10, 0.49), I 2 = 22%, p = 0.003), while the accelerated intermittent theta burst stimulation (aiTBS) mode only maintains posttreatment effects (SMD = 0.01, 95% CI (-0.45, 0.47), I 2 = 66%, p = 0.98). Conclusions Compared with standard TMS, aTMS can rapidly improve depressive symptoms, but there is no significant difference in efficacy. aTMS may also have long-term maintenance effects, but longer follow-up periods are needed to assess this possibility. Abstract:Non-pharmacological treatment is essential for patients with major depressive disorder (MDD) that is medication resistant or who are unable to take medications. Transcranial alternating current stimulation (tACS) is a non-invasive brain stimulation method that manipulates neural oscillations. In recent years, tACS has attracted substantial attention for its potential as an MDD treatment. This review summarizes the latest advances in tACS treatment for MDD and outlines future directions for promoting its clinical application. We first introduce the neurophysiological mechanism of tACS and its novel developments. In particular, two well-validated tACS techniques have high application potential: high-definition tACS targeting local brain oscillations and bifocal tACS modulating interarea functional connectivity. Accordingly, we summarize the underlying mechanisms of tACS modulation for MDD. We sort out the local oscillation abnormalities within the reward network and the interarea oscillatory synchronizations among multiple MDD-related networks in MDD patients, which provide potential modulation targets of tACS interventions. Furthermore, we review the latest clinical studies on tACS treatment for MDD, which were based on different modulation mechanisms and reported alleviations in MDD symptoms. Finally, we discuss the main challenges of current tACS treatments for MDD and outline future directions to improve intervention target selection, tACS implementation, and clinical validations.

Abstract:Introduction Depression is characterised by easy recurrence, high disability and high burden, and antidepressant therapy is the standard treatment. However, its treatment effect on patients with severe depressive disorder has been unsatisfactory. Previous studies have shown that repetitive transcranial magnetic stimulation (rTMS), as a neurotherapy, can effectively mitigate the severity of depressive symptoms. Yet, more evidence is still required for TMS to treat severe depression. This study will be the first systematic review of the efficacy and tolerability of TMS for treating severe depression. We expect it to guide future clinical practice of TMS for the treatment of psychiatric disorders. Methods and analysis We will search for the randomised controlled trial (RCT) involving rTMS for treating depression in eight electronic databases, including PubMed, Web of Science, EMBASE, the Cochrane Library and Wanfang Database, from publication up to September 2021. We will define Improvement in depressive symptoms, the difference between pretreatment (baseline) and post-treatment as the primary outcomes. The difference between pretreatment and post-treatment changes in resting state fMRI will be regarded as the secondary outcomes. Quality assessment of the included articles will be independently performed according to the Cochrane Risk of Bias tool. Ethics and dissemination Ethical approval is not essential because there is no need to collect individual patient data. And this study will be published in a peer-reviewed journal. Abstract:Background: Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique, which has yielded promising results in treating major depressive disorder. However, its effect on treatment-resistant depression remains to be determined. Meanwhile, as an emerging treatment option, patients' acceptability of tDCS is worthy of attention. Methods: This pilot study enrolled 18 patients (women = 13) with treatment-resistant unipolar (n = 13) or bipolar (n = 5) depression. Twelve sessions of tDCS were administered with anode over F3 and cathode over F4. Each session delivered a current of 2 mA for 30 min per ten working days, and at the 4th and 6th week. Severity of depression was determined by Montgomery-Åsberg Depression Rating Scale (MADRS); cognitive performance was assessed by a computerized battery. Results: Scores of MADRS at baseline (29.6, SD = 9.7) decreased significantly to 22.9 (11.7) (p = 0.03) at 6 weeks and 21.5 (10.3) (p = 0.01) at 8 weeks. Six (33.3%) participants were therapeutically responsive to tDCS. MADRS scores of responders were significantly lower than those of non-responders at the 6th and 8th week. Regarding change of cognitive performance, improved accuracy of paired association (p = 0.017) and social cognition (p = 0.047) was observed at the 8th week. Overall, tDCS was perceived as safe and tolerable. For the majority of patients, it is preferred than pharmacotherapy and psychotherapy. Conclusions: TDCS can be a desirable option for treatment-resistant depression, however, its efficacy may be delayed; identifying predictors of therapeutic response may achieve a more targeted application. Larger controlled studies with optimized montages and sufficient periods of observation are warranted. Efficacy and Safety of Transcranial Temporal Interference Stimulation for Treating Bipolar Disorder with Depressive Episodes

Abstract:Background: Bipolar disorder (BD) presents significant challenges in clinical management, often characterized by persistent depressive symptoms and cognitive deficits. Transcranial temporal interference stimulation (tTIS) has shown promise in targeting deep brain structures with minimal invasiveness. This study aimed to assess the efficacy and safety of tTIS in newly diagnosed or medication-washout BD patients. Methods: We conducted a single-arm clinical trial with 36 BD patients who underwent 10 sessions of tTIS targeting the left nucleus accumbens over one week, with two sessions per day. Each tTIS session lasted 20 minutes, utilizing a maximum current intensity of 2 mA and an envelope stimulation frequency of 40 Hz. Depressive symptoms were assessed pre- and post-intervention using validated scales, including HAMD, QIDS, MADRS, and HAMA, while cognitive functions were evaluated using standardized neuropsychological tests. Safety was monitored through adverse event reporting. Results: Among 36 participants, 25 completed the full intervention protocol. Significant reductions were observed in depressive symptom scores: HAMD decreased from 23.36 to 16.16 (P < 0.0001), QIDS from 13.52 to 9.68 (P < 0.001), MADRS from 39.12 to 31.28 (P < 0.01), and HAMA from 19.68 to 15.44 (P < 0.05). Cognitive evaluations demonstrated notable improvements in memory and executive function. Adverse events were mild, primarily limited to transient scalp discomfort. Conclusions: This study provides preliminary evidence supporting the efficacy and safety of tTIS in alleviating depressive symptoms and improving cognitive function in BD patients.

Abstract:Objective: To explore the efficacy, adverse reactions, feasibility, and acceptability of transcranial alternating current stimulation (tACS) treating drug-naive adult patients with major depressive disorder (MDD), and provide basis for further study with a large sample. Methods: The study was performed in the Neuromodulation laboratory, Department of Neurology of Xuanwu Hospital, Capital Medical University (Beijing, China) from July, 2017 to June, 2018. Thirty Eligible first-episode MDD outpatients were randomized 1∶1 to receive active tACS or sham intervention. The tACS was administered in a 40 minute, 77.5 Hz frequency, 15 mA session with one forehead (Fp1, Fpz, and Fp2, in the 10/20 international placement system, 4.45 cm×9.53 cm) and two mastoid (3.18 cm×3.81 cm) stimulation for 20 times in 4 consecutive weeks at fixed day time frame once daily from Monday through Friday, with weekends off (week 4), followed by 4 weeks with no tACS treatment (week 8). By utilizing the Hamilton rating scale for depression-17 item (HRSD-17) to assess the depressive severity of MDD patients, adverse events were administered by the treatment-emergent adverse events, the Young mania rating scale, and the self-made common questionnaire on cranial electrical stimulation. The primary efficacy outcome was the remission rate defined as HRSD-17 score ≤7 at week 8. Secondary outcomes included the rates of remission at week 4 and response at weeks 4 and 8. Safety was assessed by evaluation of adverse events. Also the proportions of participants accepting the intervention and this study procedure were evaluated at weeks 4 and 8. Results: Thirty MDD patients completed the study, and both groups had no statistical differences on their demographic characteristics (P>0.05). At week 8, the active group had a remission rate of 10/15, which was higher than 3/15 in the sham group (P<0.05). Also, the remission rate (14/15) in the active group was higher than 5/15 of the sham group at week 4 (P<0.05). For the response rates, significant differences were found between groups at week 8. For safety, both groups showed no severe adverse events and no mania/hypomania. One participant per group had 2 times of tinnitus cerebri during the intervention days. All patients accepted the intervention and the study procedure. Conclusions: The pilot study indicated that tACS with 77.5 Hz and 15 mA may have a therapeutic effect on depressive symptoms. It is well-tolerated and safe, as well as feasible and acceptable for adults with MDD.
Cognitive potency and safety of tDCS treatment for major depressive disorder: a systematic review and meta-analysis

Abstract:Background: The benefits of transcranial direct current stimulation (tDCS) for patients with major depression disorders are well-established, however, there is a notable research gap concerning its comprehensive effects on both depressive symptoms and cognitive functions. Existing research is inconclusive regarding the cognitive enhancement effects of tDCS specifically in MDD patients. The present study aims to fill this knowledge gap by scrutinizing the most updated evidence on the effectiveness of tDCS in anti-depressive treatment and its influence on cognitive function. Methods: A systematic review was performed from the first date available in PubMed, EMBASE, Cochrane Library, and additional sources published in English from 1 January 2001 to 31 May 2023. We examined cognitive outcomes from randomized, sham-controlled trials of tDCS treatment for major depression. The evaluation process strictly followed the Cochrane bias risk assessment tool into the literature, and meta-analysis was performed according to the Cochrane System Reviewer's Manual. Results: In this quantitative synthesis, we incorporated data from a total of 371 patients across 12 studies. Results showed significant benefits following active tDCS compared to sham for the antidepressant effect [SMD: −0.77 (−1.44, −0.11)]. Furthermore, active relative to sham tDCS treatment was associated with increased performance gains on a measure of verbal memory [SMD: 0.30 (−0.02, 0.62)]. These results did not indicate any cognitive enhancement after active tDCS relative to sham for global cognitive function, whereas there was a noticeable trend toward statistical significance specifically in the effect of verbal memory. Conclusions: Our study offers crucial evidence-based medical support for tDCS in antidepressant and dimension-specific cognitive benefits. Further well-designed, large-scale randomized sham-controlled trials are warranted to further validate these findings. Abstract:Transcranial direct current stimulation (tDCS) is a potential home-based treatment for major depressive disorder (MDD). In our randomised controlled trial, a fully remote, 10-week course of home-based active tDCS showed greater clinical efficacy as compared to sham tDCS in a double-blind randomised controlled trial (RCT). tDCS was administered at 2mA for 30-minute sessions in a bifrontal montage with anode over the left dorsolateral prefrontal cortex (DLPFC) and cathode over the right DLPFC. Following the 10-week blinded trial, all participants could receive active tDCS in a 10-week open label phase. Participants who had completed the open-label treatment were invited for additional long term follow-up visits over 6 months. Participants were allowed to keep their tDCS device and could choose to continue stimulation during the follow-up period. In the follow-up phase, 42 participants (27 women) attended at least one visit. Ongoing use of the tDCS device was 59% at 3 months and 55% at 6 months during the follow-up period. The clinical response rate was 64% at 3 months and 76% at 6 months. Participants who had shown a clinical response after open label treatment maintained a response rate of 90% at 6 months. In summary, high rates of clinical response were maintained in 6-month follow-up whether or not participants continued to use the tDCS device.
