Abstract:ObjectiveTo discuss the application of SUS2 appliance in early Angle classⅡmalocclusion.MethodsA total of 23 cases of orthodontic patients, aged 12 to 15 years old, were selected. All patients were treated with straight wire appliance, and then the upper and lower teeth were well stand. After that, apply to the SUS2 appliance 6-9 months. Lateral cephalogram measurement was carried out before and after treatment and the results were statistically analyzed. Results 23 patients achieved improvement in the proifles, normal overbite, overjet and molar relationship after treatment. The cephalometric analysis showed that there were signiifcant decreased in ANB, UL-E, LL-E, OB, OJ, PP-FH and Y axis after treatment (P<0.05). SNB, Go-Gn, Ar-Go and L1-MP increase signiifcantly after treatment (P<0.05). OP-FH, SNA, SN-MP and U1-SN had no obvious change after treatment (P>0.05).Conclusion SUS2 appliance and MBT can be used efifciently in the orthodontic treatment of Angle II malocclusion at an early stage of permanent dentition. Becase it can effectively use the growing potential of mandibular and stimulate its growing.
Myobrace versus twin block in the treatment of class II malocclusion in Children: A systematic review

Abstract:Background: One of today's largest global problems is malocclusion. We must prevent this through the screening and early treatment of young children, because malocclusion treatment conducted during a child's growth and development stage either the primary or mixed dentition era yields the best outcomes. Functional appliances are usually used during initial orthodontic treatment, such as myobrace and twin block appliances. Myobraces come in various sizes. The size chosen depends on the treatment objectives, which may include correcting class II malocclusions. The twin block appliance is a functional device commonly employed to treat class II malocclusions. Purpose: This investigation's main goal was to compare the efficacy of the myobrace and twin block appliances in class II malocclusion treatment to select a more appropriate pediatric dentistry device. Results: A total of 5 articles were selected from 306 articles based on relevant keywords. All selected studies were conducted within the last 10 years. Discussion: Myobrace and twin block appliances can address overjet issues and achieve significant overjet measurement reductions. This appliance promotes mandibular growth and enhances the facial profiles of individuals with class II malocclusions. Conclusion: In order to treat individuals with class II malocclusions, the myobrace and the twin block both address skeletal and dentoalveolar discrepancies. But compared to the myobrace, the twin block appliance had more noteworthy outcomes.
Comparison between clear aligners and twin-block in treating class II malocclusion in children: a retrospective study

Abstract:The study herein evaluated and compared the efficacy of Clear Aligners (CA) and Twin-Block (TB) appliances as the early orthodontic treatments of developing class II division 1 malocclusion. Twenty-four patients each for CA (11.73 ± 0.33 y) and TB (11.87 ± 0.34 y) groups were selected according to the inclusion and exclusion criteria. The cephalometric X-rays and intraoral photos were taken for the patients after nearly 12 months of treatment. Treatment impacts were evaluated by the molar correction and overjet reduction. The vertical and sagittal changes were analyzed through cephalometric measurements. Sella-nasion-point B angle (SNB), point A-nasion-point B angle (ANB), Wits Appraisal (AO-BO) and overjet were statistically significant regarding the sagittal changes analyzed before and after the treatments in both groups, respectively. So, no significant difference was noted in the sagittal changes between CA and TB groups. However, for the vertical changes, OP (occlusal plane) angle of CA group and OP angle, AFH (anterior facial height) and PFH (posterior facial height) of TB group were statistically significant. Moreover, the Z angle and cranial facial difficulty (C.F. difficulty) were also statistically significant in both groups. Class II children with retrognathic mandible are effectively treated by employing the CA, which has almost the same impact as of TB in sagittal and vertical changes. Resultantly, the patient profile is improved. The CA and TB treatments thus minimize the subsequent treatment difficulty by reducing the C.F. difficulty.
Comparative Evaluation of Twin Block Appliance and Fixed Orthodontic Appliance in Early Class II Malocclusion Treatment: A Randomized Controlled Trial

Abstract:Aims: To compare skeletal, dentoalveolar, and soft tissue changes between Twin block and early fixed orthodontic appliance for class II division 1 malocclusion treatment through a randomized controlled trial. Materials and methods: Sample and randomization: This study was a randomized controlled trial with a 1:1 allocation ratio in which 40 patients were divided equally into two groups: control and experimental; each group had an equal number of boys and girls. Randomization was achieved using random blocks of 20 patients with allocation concealed in sequentially numbered, opaque, and sealed envelopes. Blinding was only applicable for data analysis of radiographic measurements. Intervention: Twin block appliance was used in the experimental group for 1 year. However, control group was treated with fixed appliance. Inclusion criteria: Skeletal class II division 1 malocclusion with mandibular retrognathism; cephalometric angular measurements: SNA ≥ 82, SNB ≤ 78, ANB ≥ 4; overjet ≥6 mm; and patient in circumpubertal stage cervical vertebral maturation (CVM2 and CVM3). Parameters for evaluation: Cephalometric skeletal, dental, and soft tissue angular and linear measurements were used for evaluation. Results: SNB increased remarkably by 4° in the Twin block group, but only by 0.68 in the control group. There was a significant decrease in vertical dimensions (SN-GoGn) in the Twin block group compared to control group (p = 0.002). Significant enhancement in the facial profile of the patients was observed. Conclusions: The Twin block appliance induced significant skeletal and dental changes. These changes were more obvious relative to the slight changes induced by natural growth. Clinical significance: Early treatment of Class II due to mandibular retrusion with Twin block functional appliance is recommended due to its favourable skeletal effect. Early treatment with fixed appliance affects mainly the dentoalveolar component. Long term follow-up is needed for further insights.

Abstract:Background: Skeletal Class II malocclusion is a common malocclusion that seriously affects patients' profile and occlusal function. The key to treatment is to use functional appliances guide the mandible forward. This study aimed to evaluate the clinical efficacy of traditional functional appliance Twin Block (TB) and invisible functional appliance (A6). Methods: In the retrospective cohort study, 46 patients with Class II Division 1 mandibular retrognathia (23 females, 23 males; mean age 13.66±4.25 years) from the Third Affiliated Hospital of Sun Yat-sen University were selected. They were divided into A6 group and TB group according to the type of appliance guided mandibular forward used in orthodontic treatment (n=23 each; average treatment time 9.82±3.52 months). Lateral cephalometric radiographs were taken before and at the end of each treatment, and paired t-test or paired rank-sum tests were performed when appropriate to detect any statistical significance at the level of α=0.05. Results: The baseline characteristics of the two groups of patients were similar. Treatment with both appliances helped correct Class II malocclusion, improve the discrepancy between the maxilla and mandible, reduce the labial inclination of the maxillary anterior teeth, and relieve the deep overbite. A comparison of the treatment effects of the TB and A6 groups showed that the A6 had a better effect when moving Point A backward, and performed better in the abduction of the anterior teeth. TB group has more advantages than A6 group in moving forward point B and improving the nasolabial angle. Conclusions: Both the A6 and TB can significantly improve Class II malocclusion. A6 showed an obvious advantage in moving Point A backward and adducting the anterior teeth, which better corrects a skeletal Class II malocclusion.
Comparison of 2 modifications of the twin-block appliance in matched Class II samples

Abstract:The purpose of this study was to compare the skeletal and dental changes contributing to Class II correction with 2 modifications of the Twin-block appliance: Twin-block appliances that use a labial bow (TB1) and Twin-block appliances that incorporate high-pull headgear and torquing spurs on the maxillary central incisors (TB2). After pretreatment equivalence was established, a total of 36 consecutively treated patients with the TB1 modification were compared with 27 patients treated with the TB2 modification. Both samples were treated in the same hospital department and the same technician made all the appliances. The cephalostat, digitizing package, and statistical methods were common to both groups. The results demonstrated that the addition of headgear to the appliance resulted in effective vertical and sagittal control of the maxillary complex and thus maximized the Class II skeletal correction in the TB2 sample. Use of the torquing springs resulted in less retroclination of the maxillary incisors in the TB2 sample when compared with the TB1 sample; however, this difference did not reach the level of statistical significance.
META ANALYSIS OF TWIN-BLOCK REMOVABLE APPLIANCE IN TREATMENT OF MANDIBULAR RETROGNATHISM AMONG TEENAGERS

Abstract:[Objective] To evaluate the effect of twin-block removable appliance in treatment of mandibular retrognathism among teenagers excluding the influence of natural growth, so that to objectively, actually and accurately reflect the treatment effect. [Methods] Several electronic databases were searched, including CNKI (1996-2010.3), ChinInfo database (1996-2010.3), VIP (1994-2010.3), CBM (1996-2010.3), PubMed (1996-2010.3) and ScienceDirect (1996-2010.3). Four Chinese journals were hand searched for possible missing articles. Also, we used literature retrospective searching to collect 120 literatures published within 15 years regarding effect of Twin-block treatment. Randomized controlled trials (RCTs) on the efficacy of Twin-block appliance in mandibular retrousion adolescents without any surgical intervention or syndromic characteristics were considered. Meta-analysis was used to evaluate the effect of functional appliance used in the late mixed, or preadolescent dentition with cephalometric analysis such as SNB, Overjet, Co-Gn, Ar-Go and Pog/OLp According to Homogeneity test. Random effect model was used. Random effect model should be selected to calculate the total pooling WMD and its 95% confidence interval (95%CI). [Results] ① Ten RCTs including 289 patients were identified. ② Homogeneity test showed that all factors (P≤0.05) were significant heterogeneity. ③Result of meta-analysis: the total pooled WMD value (95%CI) of SNB, Overjet, Co-Gn, Ar-Go and Pog/OLp were above 0, and were 1.43 (1.23, 1.64), -2.83 (-2.90, -2.76), 3.41 (2.93, 3.89) , 2.29 (1.77, 2.81) and 0.85 (0.69, 1.00) , respectively. All data showed significant statistical difference. [Conclusion] For mandibular retrousion adolescents, Twin-block appliance can effectively treat mandibular retrognathism among teenagers, and decrease overlap, improve growth of mandible, and increase the length and ascending limb of mandible.

Abstract:Background D And Objectives: Treatment of class II malocclusion has been a prime focus of orthodontic investigators for decades. Objectives: To evaluate the effects of Herbst fixed functional appliance in the treatment of class II malocclusion, to evaluate the effects of Advansync 2 fixed functional appliance in the treatment of class II malocclusion, to compare the effects of Herbst and Advansync 2 fixed functional appliances in the treatment of class II malocclusion Method: The sample size consisted of 14 patients who reported to department of orthodontics and dentofacial orthopedics, Coorg institute of dental sciences, Virajpet seeking fixed orthodontic treatment. The patients were divided into two groups: Group A: 7 patients who were treated with Herbst fixed functional appliance. Group B: 7 patients who were treated with Advansync 2 fixed functional appliance The effects of appliances were measured at two intervals T0 Â– At the beginning of the treatment T1 - Nine months after appliance delivery Results: Patients who were treated with the Advansync 2 fixed functional appliances had better C0- GN, C0- GO, and UI A after the nine months of appliance delivery Patients who were treated with the Herbst fixed functional appliances had better SNA, SNB, WITZ, CO- A, ANB, UI- A, LI-B, LI B, UL- EPL, and FMA after the nine months of appliance delivery. INTERPRETATION AND CONCLUSION: Advansync 2 and Herbst appliance was effective in normalizing Class II malocclusions.
Retrospective comparison of dental and skeletal effects in the treatment of Class II malocclusion between Herbst and Xbow appliances

Abstract:Introduction: The purpose of this research was to compare dentoskeletal changes produced by Herbst and Xbow appliances in late mixed/early permanent dentition patients with Class II Division 1 malocclusion to an untreated control group. Methods: The retrospective cohort consisted of 41 patients treated with the Herbst appliance on average for 14 months (mean age of 11.3 years), 41 patients treated with Xbow appliance on average for 14 months (mean age of 11.11 years), and an untreated control sample of 25 patients followed on average for 21 months (mean age of 11.9 years). All patients had Class II Division 1 malocclusion characteristics. Lateral cephalometric radiographs were taken before and after phase 1 treatment/follow-up. Data were analyzed by an analysis of variance followed by Tukey post-hoc tests. Results: Although there was a high equivalence among the groups in the pretreatment cephalometric values, 4 variables showed differences (U6-FHp, L6-FHp, LAFH, and PP-U1). When comparing the mean changes (before and after phase 1 treatment/follow-up), incisor mandibular plane angle (IMPA), Wits appraisal, L6-FHp, Co-Pog, and PP-U1 measurements showed statistically significant differences. In addition, more relative mesial movement of the mandibular molars (an additional 2.4 mm) and a larger increase in mandibular length (an additional 3.2 mm) was noted for the Herbst group. Conclusions: Class II correction using Herbst and Xbow occurred in both groups through improvement in the maxillomandibular relationship and labial inclination of the mandibular incisors, as well as a relatively increased mesialization of the mandibular molars. Although both appliances improve occlusal features, the portrayed changes were not always similar. Herbst seems to produce more mandibular size increase over a similar treatment period.

Abstract:ObjectiveThe objective of the study is to compare skeletal and dental changes in class II patients treated with fixed functional appliances (FFA) that pursue different biomechanical concepts: (1) FMA (Functional Mandibular Advancer) from first maxillary molar to first mandibular molar through inclined planes and (2) Herbst appliance from first maxillary molar to lower first bicuspid through a rod-and-tube mechanism.Materials and methodsForty-two equally distributed patients were treated with FMA (21) and Herbst appliance (21), following a single-step advancement protocol. Lateral cephalograms were available before treatment and immediately after removal of the FFA. The lateral cephalograms were analyzed with customized linear measurements. The actual therapeutic effect was then calculated through comparison with data from a growth survey. Additionally, the ratio of skeletal and dental contributions to molar and overjet correction for both FFA was calculated. Data was analyzed by means of one-sample Student’s t tests and independent Student’s t tests. Statistical significance was set at p < 0.05.ResultsAlthough differences between FMA and Herbst appliance were found, intergroup comparisons showed no statistically significant differences. Almost all measurements resulted in comparable changes for both appliances. Statistically significant dental changes occurred with both appliances. Dentoalveolar contribution to the treatment effect was ≥70%, thus always resulting in ≤30% for skeletal alterations.ConclusionFMA and Herbst appliance usage results in comparable skeletal and dental treatment effects despite different biomechanical approaches.Clinical relevanceTreatment leads to overjet and molar relationship correction that is mainly caused by significant dentoalveolar changes.

Abstract:Background: Malocclusion is dentofacial growth deviation from its normal size and shape, thus causing abnormal occlusion. One of its etiology is heredity. Removable twin block, a functional appliance that can be used in interceptive orthodontic treatment, can be used to treat Class II malocclusion with large overjet in children during skeletal growth and development. Case: An 11-year-old boy came to pediatric dental specialist clinic with Class II division 1 Angle malocclusion (SNA 82°; SNB 74°; ANB 8°, CVM CS 4, overjet 14 mm, overbite 7 mm, and convex profile). Removable twin block for Class II malocclusion was chosen as the therapy. Case management: Removable twin block appliance with acrylic occlusal bite block was used on maxilla and mandible separately. An angle of 70° was made in the occlusal bite block, which was made based on the bite registration wax during centric occlusion. This device is used 24-hours a day. Monthly dental visit was done to reduce the maxillary and mandibular occlusal bite blocks. After 9 months, an overjet of 5 mm was obtained. Cephalometry calculations became SNA 82°; SNB 7 9 °; ANB 3 °. The patient was given further phase 2 treatment with fixed orthodontic appliance. Conclusion: With proper case selection and good patient cooperation, removable twin block appliance can correct skeletal malocclusion, improve facial profile, reduce overjet and overbite, and correct molar relations.

[Treatment of Class II malocclusion using Herbst appliance with headgear and rapid palatal expansion and straight wire appliance with implant anchorage]

Abstract:Purpose: To investigate the effects of early treatment of Class II malocclusion with Herbst appliance with headgear and rapid palatal expansion and straight wire appliance with implant anchorage. Methods: Twenty-five Chinese children (14 males, 11 females, aged from 10-12 years with an average age of 10.5 years) were treated with Herbst appliance with headgear and rapid palatal expansion for 6 months. Then they were further treated with straight wire appliance with implant anchorage. Cephalometric data pre- and post-treatment were measured and analysed. Multiple correlation analysis was performed with SPSS13.0 software package. Results: Compared with pretreatment, as angle SNB (4.2800±1.4000)° and as angle NP-FH (3.5600±1.0440)° increased posttreatment. as angle SNA (0.5600±0.8206)°, as angle ANB (4.7600±1.2000)°, as angle U1-NA (11.9200±1.4411)° and as angle U1-SN (13.1600±1.7720)° decreased posttreatment (P<0.05). Conclusions: Maxillary growth is successfully inhibited and mandibular growth is significantly promoted by Herbst appliance with headgear and rapid palatal expansion. The maxillary dentition is significantly retracted by fixed appliance with implant anchorage and the skeletal pattern of patients is significantly improved.
Effects of Twin-block vs sagittal-guidance Twin-block appliance on alveolar bone around mandibular incisors in growing patients with Class II Division 1 malocclusion

Abstract:Introduction: The purpose of this study was to comparatively evaluate the effects of Twin-block (TB) appliance and sagittal-guidance Twin-block (SGTB) appliance on alveolar bone around mandibular incisors in growing patients with Class II Division 1 malocclusion, using cone-beam computed tomography. Methods: The sample consisted of 25 growing patients with Class II Division 1 malocclusion (14 boys and 11 girls, mean age 11.92 ± 1.62 years) and was randomly distributed into the TB group (n = 13) and the SGTB group (n = 12). The treatment duration was 11.56 ± 1.73 months. Pretreatment (T1) and posttreatment (T2) cone-beam computed tomography scans were taken in both groups. Height, thickness at apex level, and volume of the alveolar bone around mandibular left central incisors were measured respectively on labial and lingual side, using Mimics software (version 19.0; Materialise, Leuven, Belgium). Based on the stable structures, 3-dimensional (3D) registrations of T1 and T2 models were taken to measure the sagittal displacement of incisors. Intragroup comparisons were evaluated by paired-samples t tests and Wilcoxon tests. Independent-samples t tests and Mann-Whitney U tests were used for intergroup comparisons. Results: In both groups, alveolar bone height and volume on the labial side of the incisors significantly decreased after treatment (P <0.05). Lingual alveolar bone height, lingual and total alveolar bone volume, labial, lingual and total alveolar bone thickness showed no significant difference between T1 and T2 (P >0.05). In both groups the incisors tipped labially and drifted to the labial side. Compared with the TB group, less labial alveolar bone loss, less incisor proclination and crown edge drift were found in the SGTB group (P <0.05). Conclusions: Labial alveolar bone loss around mandibular incisors was observed after both types of appliances treatment in growing patients with Class II Division 1 malocclusion. Less labial alveolar bone loss, less incisor proclination, and crown edge drift were found in the SGTB group than in the TB group during treatment.

Abstract:Introduction The purpose of this study was to evaluate the therapeutic effect of modified clear Twin Block (CTB) aligner and traditional twin block (TB) appliance from skeletal, dentoalveolar and soft tissue changes in adolescents with skeletal class II malocclusion. Methods A total of 80 adolescents, included in this study from two medical centres, were distributed into CTB group, TB group and control group based on the treatment they received. Lateral cephalograms at pre‐treatment (T1) and post‐treatment (T2) were measured by modified Pancherz's cephalometric analysis, and dentoskeletal and soft tissue changes were analysed by independent‐sample t‐test, paired‐sample t‐test, ANOVA test and Scheffe's Post Hoc test. Results Seventy‐five adolescents completed the study, including 32 in the CTB group, 32 in the TB group and 11 in the control group. Both CTB and TB treatment showed significant differences in most dentoskeletal and soft tissue measurements. Abstract:Our study aimed to use three-dimensional (3D) spatial morphological measurement methods to compare the influence of Twin-Block and clear functional aligners on the temporomandibular joint (TMJ) of adolescent Class II division 1 malocclusion mandibular retraction patients. We also aimed to explore the similarities and differences in the effects on the TMJ upon using Twin-Block and clear functional aligner.

Clinical finite element analysis of mandibular displacement model treated with Twin-block appliance

Abstract:Introduction: The mechanical distribution of the mandible is an important factor that affects functional orthosis during Twin-block (TB) appliance correction. Changes in the mandible before and after TB appliance correction are also key factors in maintaining the therapeutic effect. Finite element analysis, a powerful numerical, analytical tool, is widely used to predict the stress and strain distribution of the craniofacial bone that orthodontics generates. Methods: The sample was a 14-year-old male patient with Class II malocclusion during growth. A cone-beam computed tomography scan was undertaken at pretreatment and posttreatment. In the Finite element analysis of the pretreatment model, the remote displacement model of the mandible was established with the sella point as the center. A mandibular model under TB appliance loading was established. Its mandibular displacement and von Mises stress were compared before and after loading. Three-dimensional registration was conducted on the pretreatment and posttreatment models to measure the sagittal displacement of the centrosome. Results: The force on the mandible occurred mainly in the condyle neck and medial mandible after the TB appliance moved the mandible. After displacement, the posterior upper margin of the condyle was farther away from the articular fossa. Three-dimensional registration results showed that new bone had formed behind and above the condyle after TB appliance treatment. Conclusion: The TB appliance provides additional advantages in treating skeletal Class II malocclusions by helping to reduce the burden on the temporomandibular joint and promoting the adaptive reconstruction of the mandible.
Cephalometric Evaluation of the Pre- and Posttreatment Changes after the Correction of Class II Division 1 Malocclusion with Twin Block Appliance in Mixed Dentition

Abstract:Aim and objective: To evaluate the pretreatment cephalometric records on the dental, skeletal, soft tissue, and incisor relationship with a twin block functional appliance in class II malocclusion patients in and around Mahe; evaluate the posttreatment cephalometric records on the dental, skeletal, soft tissue, and incisor relationship with a twin block functional appliance in class II malocclusion patients in these children; and to compare cephalometrically certain dental, skeletal, and soft tissue points in pretreatment and posttreatment cephalograms in them. Materials and methods: This study was conducted on 20 class II patients in the mixed dentition period, who were treated with twin block therapy. Each had to meet the following criteria-(1) skeletal class II malocclusion with retrognathic mandible; (2) full cusp class II molar relationship; and (3) an angle of ANB of 50 or greater at the start of treatment. All patients wore the appliance 24 hours/day. The pretreatment cephalometric head films for the group were taken using standard cephalometric X-ray equipment. The length of time required to achieve a class I molar relationship was assessed. Appointments during the twin block phase were scheduled at intervals of 8 weeks. Lateral head films were obtained again at the posttreatment follow-up stage. Results: There was a significant increase in effective mandibular length, ramus height, SNB, ANB, overjet, overbite, and I to NA (mm and degrees) after twin block therapy. The maxillary incisor position showed a decrease in its value by 4° in five cases. Conclusion: Thus, in the present study, evidence of skeletal and dentoalveolar changes leading to the correction of class II division 1 malocclusion with the twin block functional appliance has been established. However, further studies with a longer period of follow-up and a larger sample are required to substantiate the results of the present investigation. A comparison of treatment effectiveness between clear aligner and fixed appliance therapies

Abstract:Background: Align technology has developed greatly over past few years. Patients tended to prefer clear aligners over conventional brackets because of the superior comfort and esthetics, while the effectiveness of clear aligners was still controversial. The aim of this systematic review was to verify whether the treatment effectiveness of clear aligners was similar to the conventional fixed appliances. Methods: A comprehensive search of the Pubmed, Web of Science, Embase, Scopus, and Cochrane Central Register of Controlled Clinical Trials Register databases for studies published through to August 20, 2018 was conducted. Comparative clinical studies assessing the effectiveness of clear aligners compared with braces were included. Results: Eight papers were included in this study. Two of the included papers were randomized controlled trials and six were cohort studies. Clear aligners might not be as effective as braces in producing adequate occlusal contacts, controlling teeth torque, increasing transverse width and retention. While no statistically significant difference was found between two groups in Objective Grading System score (WMD = 8.38, 95% CI [- 0.17, 16.93]; P = 0.05). On the other hand, patients treated with clear aligners had a statistically significant shorter treatment duration than with braces (WMD = - 6.31, 95% CI [- 8.37, - 4.24]; P < 0.001). Conclusion: Both clear aligners and braces were effective in treating malocclusion. Clear aligners had advantage in segmented movement of teeth and shortened treatment duration, but were not as effective as braces in producing adequate occlusal contacts, controlling teeth torque, and retention.
Evaluating the Effects of Carriere Motion Appliance and Twin Block Appliances in Class II Correction-A Retrospective Study

Abstract:This retrospective study compared Class II orthodontic non-extraction treatment using Carriere Motion Appliance (CMA) and Twin Block (TB) appliances. Methods: The treatment of 38 patients was assessed. Pre- and post-treatment cephalometric radiographs were analyzed to evaluate skeletal, dental, and soft tissue treatment outcomes and efficacy. Results: Both appliances effectively corrected the Class II molar relationship. When measured at the distal aspect of the first molar, TB achieved 4.22 mm, while CMA had a 2.55 mm correction. When measured in the mesial aspect, the CMA achieved a 3.9 mm correction. The changes in SNB and ANB were statistically significant only in the TB group. The CMA appliance demonstrated statistically significantly less protrusion of the mandibular incisors and less upper incisor retrusion without vertical changes compared to the TB appliance. The TB demonstrated statistically significant lower lip protrusion compared to the CMA. Conclusion: The CMA corrects Class II malocclusions only by exerting a dentoalveolar influence and does not demonstrate the added effects associated with TB, such as elongation of lower facial height (LFH) and less loss of lower anchorage. Nonetheless, the correction in the TB group comprised both dentoalveolar and skeletal components. The CMA promotes a multidirectional upper and lower molar movement, and despite our 2D cephalometric analysis, we were able to estimate the extent of upper molar derotation.
