Abstract:The manuscript by Reshetnikov et al. provides an opportunity to highlight some aspects of childhood cancer that seem to get limited attention, either in the literature or media. First, that the number of children affected by cancer is many more times higher in developing countries, like Russia in several ways, than in so called 'well-developed economic countries'. 1 Second, the overall 5-year survival rates of ~ 80%, 2 or even higher for certain pediatric cancers, taken for granted in North America and Europe, are but a dream in developing countries. Taken together, the burden and societal "experience" of childhood cancer is very different for most children and their families globally, while only a minority of children with cancer contribute to most information found in peer reviewed manuscripts. In other words, the literature provides limited insights of what it means for the vast majority of children worldwide to have cancer. But regardless, there is limited data on society's perception of childhood cancer, whether in developing or developed countries. In general, it is recognized that socioeconomic status, culture, and religion have a considerable impact on how cancer is viewed and experienced, 3 and that " cancer-related stigma and myths about cancer are important problems that must be addressed, although different from a country to another ", 4 but data for childhood cancer is limited. The manuscript by Reshetnikov et al., contributes to that serious knowledge gap and provides starting points for further research and exploration. So how do people in one of Russia's eight districts, with a population of just over 10 million people living in 170,400 km 2 and a relatively high Human Development Index of 0.793, react to children with cancer? In summary: it depends....It seems that about half of parents who participated in the descriptive research study felt that the people around them did not change their attitude towards the affected child or their family, about a fifth weren't able to tell whether anything had changed, while over a third felt that attitudes had changed. Of those that indicated that attitudes had changed, half indicated an increase in sympathy and support and about 40% experienced a more negative, occasionally even hostile change (" friends were avoiding him/her [cancer-affected child] as they thought it [cancer] might be contagious " and " three words: pity, fear and detachment "). Interestingly, only 3% of participating parents responded to have experienced both increased sympathy and alienation. One would have expected a larger portion of participants to have experienced both positive and negative reaction from their environment. A note of caution; by far most participants (>90%) were parents of a child with leukemia, with limited outward signs of disease: alopecia, fatigue, and one suspects regularly wearing a mask. This is in contrast to children with tumors of the extremities or those with brain tumors, who usually exhibit greater visible morbidities. It is possible that the latter group of children, with more visible effects of their cancer and/or treatments, would have had a more pronounced reaction from family, friends, and neighbors.
