Abstract:The macular area, a unique tissue structure, has the most keen vision, color vision and perception in shape.The study of human macular pigment and its distribution in vivo is now possible through determination of macular pigment optical density (MPOD).Previous studies have shown that MPOD decreasing is related to the development of diabetes and age-related macular degeneration.Now, more and more studies show that MPOD measurement can also be used to evaluate the prognosis of some eye diseases and related systemic diseases such as glaucoma, Stargardt disease, macular telangiectasia type 2 and Sj(o)gren-Larsson syndrome.

Abstract:Background: Carotenoids are present throughout retina and body its dense deposition leads to an identifiable yellow spot in the macula. Macular pigment optical density (MPOD) measured in the macula is vital to macular well-being and high-resolution visual acuity. MPOD has also been associated with various health and disease states. We sought to review the literature on this topic and summarize MPODs role as a measurable modifiable clinical biomarker, particularly as a measure of the eye's antioxidant capacity in the context of oxidative damage and retinal ischemia. Methods: A literature review collated the articles relevant to MPOD, carotenoid intake or supplementation, and their influence on various health and disease states. Results: Literature reveals that MPOD can serve as a reliable biomarker for assessing the retinal defense mechanisms against oxidative stress and the deleterious effects of excessive light exposure. Elevated MPOD levels offer robust protection against the onset and progression of age-related macular degeneration (AMD), a prevalent cause of vision impairment among the elderly population. MPOD's implications in diverse ocular conditions, including diabetic retinopathy and glaucoma, have been explored, underscoring the real need for clinical measurement of MPOD. The integration of MPOD measurement into routine eye examinations presents an unparalleled opportunity for early disease detection, precise treatment planning, and longitudinal disease monitoring. Conclusions: Longitudinal investigations underscore the significance of MPOD in the context of age-related ocular diseases. These studies show promise and elucidate the dynamic nuances of MPOD's status and importance as a measurable, modifiable clinical biomarker.

Association of Optical Coherence Tomography Angiography Metrics with Detection of Impaired Macular Microvasculature and Decreased Vision in Amblyopic Eyes: the Hong Kong Children Eye Study.

Abstract:Importance:Microvascular abnormalities in amblyopia are becoming evident with high-resolution imaging, such as optical coherence tomography angiography (OCT-A); however, to our knowledge, the clinical significance and use of these findings are unknown.Objective:To assess changes in quantitative OCT-A metrics in amblyopic eyes and explore their association with visual acuity in children.Design, Setting, and Participants:This population-based nested case-control study included children aged 6 to 8 years who were consecutively recruited between January 2016 and July 2017 from the population-based Hong Kong Children Eye Study (HKCES) at the Chinese University of Hong Kong Eye Centre. All participants underwent OCT-A with a swept-source OCT and detailed ophthalmic investigations. Macular microvasculature of the superficial capillary plexus was quantified by a customized automated image analysis program. A multivariable linear regression was conducted to evaluate the differences in OCT-A metrics between amblyopic and nonamblyopic eyes after adjustment for all known confounders. Data analysis was conducted from September to November 2018.Main Outcomes and Measures:Differences in OCT-A metric (foveal avascular zone [FAZ]) area, FAZ circularity, vessel density, vessel diameter index, and fractal dimension between amblyopic and nonamblyopic eyes.Results:There were 30 participants with amblyopia (mean [SD] age, 7.57 [1.2] years; 16 girls [53.3%]) and 1045 controls (mean [SD] age, 7.65 [1.0] years; 580 girls [55.5%]) in this cohort. Compared with control eyes, amblyopic eyes had decreased FAZ circularity (-0.058; 95% CI, -0.096 to -0.021, P = .002), decreased fractal dimension (-0.014; 95% CI, -0.024 to -0.003; P = .01), and increased vessel diameter index (0.002; 95% CI, 0.002 to 0.003; P < .001). A difference was not identified between FAZ area and vessel density. LogMAR visual acuity was associated with FAZ circularity (sβ, -0.133; P < .001) and vessel diameter index (sβ, 0.097; P = .001) but not with vessel density nor FAZ area.Conclusions and Relevance:The results of this population-based study in children supports the presence of macular microvascular abnormalities in amblyopic eyes. Such changes as measured by OCT-A metrics are associated with visual acuity, inferring retinal involvement in the development of amblyopia and suggesting a potential role of quantitative OCT-A metrics in the diagnosis and recognition of amblyopia.
Macular pigment optical density and its relationship with refractive status and foveal thickness in Chinese school-aged children

Abstract:Purpose: To investigate macular pigment optical density (MPOD) and its relationship with refractive status and foveal thickness in Chinese school-aged children. Methods: Ninety-four healthy Chinese children, 6 to 12 years old, were recruited to the study. MPOD was measured with a heterochromatic flicker photometer (HFP), and foveal thickness, including both minimum and central foveal thicknesses (MFT and CFT, respectively), were measured by optical coherence tomography (OCT) with fast macular map scan. A noncontact tonometer was used to measure intraocular pressure (IOP) followed by determination of the refraction using an autorefractor after cycloplegia. Information on body mass index (BMI) was obtained. The correlation between MPOD values and foveal thickness, spherical equivalent (SE) refraction, IOP, BMI, sex, and age was statistically analyzed using SAS 8.2 statistical software. Results: The MPOD in examined school-aged children was 0.56 ± 0.25, without any significant difference between boys and girls (p = 0.12). MPOD showed no significant association with age, BMI, IOP, SE, MFT, or CFT. In the myopia group, however, there was an inverse relationship between MPOD and MFT (R =-0.66, p = 0.028) and a positive relationship between MPOD and CFT (R = 0.67, p = 0.025). Conclusions: MPOD was inversely related to MFT and positively related to CFT in Chinese school-age children with low-to-moderate myopia. MPOD showed no significant association with age, BMI, IOP, SE or foveal thickness.
Quantitative retinal microvasculature in children using swept-source optical coherence tomography: the Hong Kong Children Eye Study.

Abstract:Aims To evaluate the distributions of quantitative optical coherence tomography angiography (OCT-A) metrics and its associated factors in children. Methods 1059 children aged 6-8 years were recruited from the Hong Kong Children Eye Study. All the participants underwent OCT-A with a swept-source OCT. Retinal microvasculature on superficial capillary plexus was assessed and quantified by a customised automated image analysis programme. Univariable and multiple linear regression analyses were performed to determine systemic (body mass index (BMI), waist circumference, head circumference and blood pressure), demographic and ocular (axial length (AL), macular thickness and volume, retinal nerve fibre layer (RNFL) thickness and visual acuity) variables (independent variables) associated with OCT-A metrics (dependent variables). Results In multiple linear regression analyses, enlarged foveal avascular zone area was associated with female gender (beta=0.110, p< 0.001), decreased AL (beta=-0.097, p< 0.001) and decreased central macular thickness (beta=-0.008, p< 0.001). Decreased vessel density was associated with increased BMI (beta=-4.12x10-4, p=0.006), decreased AL (beta=0.003, p< 0.001) and decreased central macular thickness (beta=7.87x10-5, p=0.001). Increased vessel diameter index was associated with female gender (beta=0.020, p=0.007) and decreased AL (beta=-0.020, p< 0.001). Decreased fractal dimension was associated with older age (beta=-0.001, p=0.008). There were no significant correlations between OCT-A metrics with visual acuity, RNFL thickness, central corneal thickness, waist circumference, head circumference and blood pressure (all p> 0.05). Conclusion Our findings provides new information on baseline morphology of retinal microvasculature and its associated factors in school children, which will be useful for interpreting OCT-A metrics and for identifying and characterising pathological changes in retinal microvasculature.
Macular Pigment Optical Density in a Healthy Chinese Population

Abstract:PurposeTo measure the macular pigment optical density (MPOD) values in a healthy Chinese population using the one-wavelength reflectometry method and to investigate the relationships of MPOD with age, sex, body mass index (BMI), smoking and lens opacities.MethodsA total of 441 healthy participants, aged 3-81years old (242 male and 199 female subjects), were enrolled in this study. Demographic and lifestyle data were recorded based on physical examinations and questionnaires. Lens opacities were measured according to the Lens Opacities Classification System III (LOCS III). MPOD values were measured at 7 degrees of eccentricity, using the one-wavelength reflectometry method (Visucam 200; Carl Zeiss Meditec). MPOD values were reported in parameters including max' and mean' optical density (OD). The original MPOD values without automated correction were used for analysis.ResultsThe average values were 0.3030.097d.u. (initials of density units) for the max OD and 0.109 +/- 0.031d.u. for the mean OD. A significant inverse relationship was found between age and MPOD (for max OD, =-0.716, p<0.001; for mean OD, =-0.669, p<0.001). Participants with no lens opacities had higher MPOD values than those with moderate lens opacities (p<0.001). The MPOD values were not associated with sex, BMI or smoking status.ConclusionMPOD within 7 degrees of eccentricity, as measured by one-wavelength reflectometry, was found to decrease with increasing age in a healthy Chinese population, and lens opacities had an impact on these measurements. These results provide a reference value for future studies in the Chinese population.
Analysis of correlative factors of macular pigment optical density in 104 young people

Abstract:Objective To survey macular pigment optical density of young people in China,as well as the relationship between MPOD and correlative factors,such as refractive diopter,gender,smoking,alcohol consumption,and family history of age-related macular degeneration.Methods 48 community youth volunteers and 56 students of 2007 or 2008,studied at department of preventive medicine,college of health science,Peking University,were enrolled in the study.All participants completed a simple questionnaire,including smoking history,drinking history,family history of macular degeneration,and a unified examination of refraction process.All the subjects used heterochromatic flicker photometry method for measuring macular pigment optical density.Results The mean of MPOD value of 208 eyes was(0.625 ± 0.191).There was no significant difference between male researchers and female researchers,non-dominant eye and dominant eye,subjects without smoking and with smoking,subjects without drinking and with drinking(P> 0.05).The difference of MPOD values between subjects having AMD family history and subjects having no family history was significant.MPOD did not correlated with the diopter value(r=-0.107,P=0.124,P> 0.05).Conclusion MPOD value of young people with family history of macular degeneration decreased.

Abstract:Purpose: The objective of this study was to observe the macular pigment optical density (MPOD) and the relationship between MPOD and retinal thickness in Chinese primary angle-closure glaucoma (PACG) patients by the one-wavelength reflectometry method. Methods: This study was a prospective comparative observational study, including 39 eyes from 39 PACG patients (15 men and 24 women, mean age 61.89 ± 12.30) and 41 eyes from 41 controls (20 men and 21 women, mean age 63.24 ± 14.02). We measured the MPOD 7-degree area by the one-wavelength reflectometry method and analyzed both the max and mean optical density (OD). The central retinal thickness (CRT) and the total thickness of the macular ganglion cell layer (GCL), and inner plexiform layer (IPL)were measured by spectral-domain-optical coherence tomography (SD-OCT). Statistical methods such as Shapiro-Wilk test, Fisher's exact test, chi-square test, two independent samples test and Spearman's correlation coefficient were used to observe the differences in the MPOD between normal subjects and PACG patients and the correlation between the MPOD and retinal thickness. Results: The max optical density (Max OD) (PACG group: 0.302 ± 0.067d.u, control group: 0.372 ± 0.059d.u., p < .001) and mean optical density (Mean OD) (PACG group: 0.124 ± 0.035d.u., control group: 0.141 ± 0.028d.u., p < 0.05) were significantly reduced in PACG patients compared with control subjects. Significant decreases in GCL + IPL thickness (PACG group: 74.71 ± 39.56 μm, control group:113.61 ± 8.14 μm, p < 0.001) and CRT (PACG group: 254.49 ± 41.47 μm, control group:329.10 ± 18.57 μm, p < 0.001) were also observed in PACG eyes. There was no statistically significant correlation between the MPOD and GCL + IPL thickness (p = .639, p = .828). Conclusions: MPOD was significantly lower in Chinese PACG patients than in the control group, potentially due to thinning of the GCL + IPL thickness. This study provides insights for the pathophysiology, assessment of PACG and potential guidance for lifestyle modifications.

In Vivo Correlation Between Macular Pigment Optical Volume and Retinal Layers Thickness

Abstract:Purpose: This study aims to investigate the potential in vivo relationship between macular pigment (MP) and retinal layers thickness in healthy subjects and dry, non-advanced age-related macular degeneration (AMD). Methods: An observational, cross-sectional study was conducted. Healthy subjects >40 years and patients with early or intermediate AMD were recruited. Structural OCT and macular pigment optical volume (MPOV) were collected for each subject. Retinal layers parameters were calculated based on the standard early treatment diabetic retinopathy study (ETDRS) map. Additionally, MPOV within 1°, 2°, and 9° of eccentricity was assessed and associated with retinal layers thickness and volume. Linear mixed-effects models were used to test the relationship between MP and structural OCT parameters, while adjusting for known possible confounding factors. Results: A total of 144 eyes of 91 subjects (60.4% females) were evaluated, comprising 43% normal eyes and 57% with early/intermediate AMD. Among the retinal layers, only the outer nuclear layer (ONL) thickness and volume appeared to be associated to higher MP levels. Specifically, the central ONL thickness was identified as a significant predictor of the MPOV 1°(P = 0.04), while the parafoveal ONL thickness (inner ETDRS subfield) was identified as a significant fixed effect on the MPOV 9° (P = 0.037). Age and the presence of drusen or subretinal drusenoid deposits were also tested without showing significant correlations. Conclusions: Among the retinal layers examined, only the ONL thickness demonstrated a significant association with MPOV. Consequently, ONL thickness might serve as a potential biomarker related to MP levels.
Macular Pigment Optical Density Measured by a Single Wavelength Reflection Photometry with and without Mydriasis

Abstract:PURPOSE: 1. To evaluate the repeatability of macular pigment optical density (MPOD) measurements in non-mydriatic and mydriatic conditions with a single wavelength reflection photometry. 2. To compare the non-mydriatic measurement with the mydriatic measurement of MPOD values.METHODS: Fifty-one healthy young subjects were recruited in this prospective study. MPOD parameters including maximum optical intensity (OD), mean OD, volume and area, were measured using a single 460-nm wavelength reflection method (Visucam 200, Carl Zeiss Meditec) twice both before and after pupil dilation. Paired t test, Pearson's correlation, intraclass correlation coefficient (ICC) and Bland-Altman plots were used to evaluate the agreement and correlation of MPOD parameters before and after mydriasis.RESULTS: The differences of maximum OD, mean OD and volume between the two measurements without mydriasis was statistically significant for all parameters, but not significant between the two measurements with mydriasis or between non-mydriatic and mydriatic measures. The ICCs were higher for the two measurements with mydriasis (range: 0.687-0.840) than those without mydriasis (range: 0.448-0.695) or non-mydriatic and mydriatic measures (range: 0.473-0.769). The 95% limit of agreement (LOA) of measurements between the two measurements after mydriasis was the smallest among all the other comparisons. The coefficient of repeatability was lower for the measurements with mydriasis than those without mydriasis.CONCLUSIONS: MPOD measurement by Visucam 200 with mydriasis has higher repeatability than the measurement without mydriasis. The non-mydriatic and mydriatic measures had less agreement compared to the two measure after mydriasis. Therefore, mydriasis is recommended.

Effect of macular pigment optical density on Yellow‐Blue and Red‐Green colour discrimination

Abstract:The macular pigment (MP) absorbs preferentially short‐wavelength light and its narrow spectral absorption curve overlaps mostly the spectral responsivity function of S‐cones, although the M‐ and L‐cones are also affected. The MP is restricted to the centre ~14° disc region of the retina with a peak optical density (OD) at the centre in the range 0 to 1 log units. Although the macular pigment optical density (MPOD) decreases rapidly with increasing eccentricity, carotenoid supplementation studies reveal a large uptake of lutein and zeaxanthin, particularly in the parafoveal region and covering the centre ~6° to 8° of the fovea (doi: 10.1111/j.1475‐1313.2006.00386.x ). Since the MP contributes to the reduced excitation of S‐cones, it is of interest to examine the effects MP may have on colour vision, particularly when ‘blue‐blocking’ lenses are also involved. We studied the relationship between MPOD and red/green (RG) and yellow/blue (YB) colour thresholds by measuring the spatial distribution of the MP in normal trichromats using the Macular Assessment Profile (MAP) test (doi: 10.1111/j.1475‐1313.2010.00748.x ) and the corresponding RG and YB colour thresholds using the CAD test (doi: 10.1093/bmb/ldx007 ). The results show negligible correlation between peak MPOD and YB thresholds and a small, but significant negative correlation with RG thresholds. This observation is of interest since increased MPOD values result in lower RG thresholds. The same experiment was also carried out in subjects with significantly larger amounts of MP following 3 months of supplementation with carotenoids. When averaged over the centre disc region of 2.5° and then over a parafoveal annulus between 2.5° and 4°, the corresponding, mean MP densities show only negligible correlations with RG and YB colour thresholds. To explain these findings, we calculated the expected cone‐contrasts for S‐, M‐ and L‐cones for chromatic displacements defined by the threshold ellipse. The results show that MP absorption of short‐wavelength light has a negligible effect on S‐cone contrast over the whole range of displacement directions associated with the colour threshold ellipse. Repeated calculations with simulated MPOD values up to 1 log unit, produced the same result. More recently, we modelled the effects ‘blue‐blocking’ lenses and pre‐receptor filters in the eye to explore changes to the orientation and the size of the colour threshold ellipse. The model predicts well the measured RG and YB thresholds and confirms the absence of significant threshold changes for MPOD values in the range 0 to 1 log unit. The model does, however, predict systematic changes in the orientation of the major axis (i.e., the tritan axis) and MPOD. These observations are of interest since the orientation of the major axis of the best‐fit ellipse appears to decrease monotonically from ~75° to 55°, as MPOD increases from very small values up to a peak OD of 1 log unit. More work is needed to investigate how YB and RG chromatic sensitivity is affected by combined absorption of short‐wavelength light by MPOD, the lens of the eye and ‘blue‐blocking’ spectacle lenses.

Association of macular pigment optical density with plasma macular carotenoids levels and serum lipids in South Indian healthy volunteers and patients with early age-related macular degeneration

Abstract:European Journal of Ophthalmology, Ahead of Print. Purpose: The objective of the present study was to investigate the relationship between macular pigment optical density (MPOD) and plasma carotenoids [(L) and (Z)] and serum lipids in South Indian young healthy volunteers and patients with early age-related macular degeneration (AMD). Methods: Two hundred and fourteen (N = 214) study participants (Healthy control group (N) = 178; Early AMD group (N) = 36) were enrolled after getting their written informed consent. The MPOD of the study participants was assessed using MPS II (Electron Technology, UK) after completing their routine ocular examination. Serum lipids were measured by the standard technique. Plasma levels of L, Z, lycopene and beta-carotene were estimated by high performance liquid chromatography with photo diode array detector. Statistical analysis used: Correlations among variables in serum, plasma and the MPOD were established using Spearman's rho correlation coefficient. Results: The overall mean MPOD in healthy control group and early AMD group were found to be 0.47 ± 0.16 (N = 178; 317 eyes) and 0.35 ± 0.22 (N = 36; 38 eyes) at 1° eccentricity respectively and were found to be statistically significant (p < 0.001). A strong positive association was found between plasma L, Z and L + Z and MPOD. Serum HDL showed a strong negative association with MPOD and other lipids showed a very weak association. MPOD was unaffected by body mass index. Conclusions: MPOD is positively associated with plasma L,Z and L + Z, adding further evidence that additional intake of L/Z may be beneficial in delaying the risk of AMD in our population

Abstract:Purpose: To examine the quantitative measurements of OCTA in children with amblyopia using the meta-analysis methodology. Methods: PubMed, Embase, and Cochrane library were searched for available papers up to March 2021. Weighted mean differences (WMD) were used to compare the retina parameters between the eyes with amblyopia and the contralateral eyes or healthy control eyes. Results: Twelve studies were included. When considering the parafovea, the microvessel density was reduced in amblyopic eyes compared with healthy control eyes in the superficial capillary plexus (SCP) in 6 × 6 volume scan (WMD = −2.12, 95%CI: −3.24, −0.99) but not SCP in 3 × 3 volume scan (WMD = −1.43, 95%CI: −2.96, 0.11). In the deep capillary plexus (DCP), amblyopia did not decrease vessel density in the 6 × 6 volume scan (WMD = −2.22, 95%CI: −5.86, 1.42; I 2 = 79.6%, P = 0.008), but a difference was observed in the whole eye 3 × 3 (WMD = −1.95, 95%CI: −3.23, −0.67; I 2 = 27.5%, P = 0.252). There were no significant differences in the foveal avascular zone area and foveal thickness between amblyopic eyes and healthy control eyes. There were no significant differences in microvessel density, foveal avascular zone area, and foveal thickness between amblyopic eyes and fellow eyes. Conclusion: According to OCTA, amblyopic eyes had lower vessel density in parafoveal SCP and DCP compared with healthy control eyes, but not compared with fellow eyes. There were no significant differences regarding the foveal avascular zone area and foveal thickness between amblyopic and non-amblyopic eyes.

Macular vascular density alteration patterns in paediatric optic neuritis patients with serum MOG antibody positivity detected by optic coherence tomography angiography

Abstract:Purpose The retinal microvascular network plays a crucial role in inflammatory injury in paediatric optic neuritis (PON) with serum MOG antibody positivity (MOG + PON). This study compared retinal microvascular densities and structural alterations in MOG + PON eyes with paediatric isolated optic neuritis (PION) eyes and followed up with the final best-corrected visual acuity (BCVA) after 6 months. Methods A total of 29 children (52 eyes) with PON, including 15 MOG + PON cases (28 eyes), 6 PION cases (10 eyes), 2 neuromyelitis optica spectrum disorders associated PON(NMOSD-PON) cases (4 eyes), 6 MOG-associated disease (MOGAD) patients without ON-affected eyes (MOG + NPON) cases (10 eyes) and age- and gender-matched healthy controls (HCs) underwent superficial/deep retinal angiography density (SAD/DAD) by optical coherence tomography angiography (OCTA). Their BCVAs were followed up until 6 months after PON onsets. Results MOG + PON cases had better final BCVAs than PION and NMOSD-ON. MOG + PON (35.7 ± 10.3 %) and PION (40.1 ± 10.3 %) eyes experienced severe SAD reductions in contrast to MOGAD+NPON (48.7 ± 5.2 %) and HCs eyes (55.6 ± 8.2 %). However, DAD in MOG + PON eyes (48.5 ± 9.2 %) and MOG + NPON eyes (53.1 ± 3.3 %) increased compared to HC eyes (45.7 ± 9.6 %; p = 0.028 and 0.009, respectively). SAD reduction occurred in acute PON and was detected as early as 2 weeks after PON onset. Conclusions MOG + PON eyes had better final BCVAs than PION eyes, which displayed superficial retinal microvascular perfusion reductions and deep microvascular perfusion increases. SAD could be a sensitive surrogate for PON attacks in children with MOGAD.

Abstract:Psychophysical discrimination of structured light (SL) stimuli may be useful in screening for various macular disorders, including degenerative macular diseases. The circularly-oriented macular pigment optical density (coMPOD), calculated from the discrimination performance of SL-induced entoptic phenomena, may reveal a novel functional biomarker of macular health. In this study, we investigated the potential influence of eye dominance and testing order effects on SL-based stimulus perception, factors that potentially influence the sensitivity of screening tests based on SL technology. A total of 28 participants (aged 18-38 years) were selected for the study after undergoing a comprehensive eye examination. A psychophysical task was performed where various SL-based entoptic images with multiple azimuthal fringes rotating with a specific temporal frequency were projected onto the participants' retinas. By occluding the central areas of entoptic images, we measured the retinal eccentricity ($R$) of the perceivable area of the stimuli. The slope of the coMPOD profile ($a$-value) was calculated for each participant using a spatiotemporal sensitivity model that takes into account the perceptual threshold measurements of structured light stimuli with varying spatial densities and temporal frequencies. The Pearson correlation coefficient between eye dominance and testing order effects was $r=0.8$ ($p<0.01$). The Bland-Altman plots for both factors indicated zero bias. The results indicate repeatable measurements for both eyes, implying minimal impact from eye dominance and testing order on SL-based stimulus perception. The results provide a foundation for future studies exploring the clinical utility of SL tools in eye health.

Abstract:Purpose: To determine macular pigment (MP) density scores in healthy Indians and examine correlations with demographic and lifestyle variables.Methods: We observed 484 Indians without an ocular pathology. Body mass index (BMI) and self-reported lifestyle factors (sunglasses usage, physical activity, and smoking) were noted. MP density was assessed as the threshold of perception of the shadow of their macular pigments on their retina using a new MP assessment tool (MP-eye). Lutein and zeaxanthin intake was assessed using a prevalidated questionnaire regionally designed for the Indian diet. Clusters of participants were created for statistical analysis based on MP-eye scores secondarily to detect any relevant effects in very low, low, medium, and high ranges of MPs.Results: Data analyzed included 235 males and 249 females with mean age of 36.1 ± 12.9 years (range, 14-72). The median MP-eye score was 6 (range, 0-10, with 10 being high). Most were non-smokers (413, 85.3%) and did not use sunglasses (438, 90.5%), and 314 (64.9%) had low physical activity. Diabetes was present in 62 participants (12.8%) and hypertension in 53 (10.9%). Advancing age (r = -0.209; P < 0.000) and BMI (r = -0.094; P = 0.038) had weak negative correlation with MP-eye scores. Hypertension was less prevalent (7/88) in the cluster with the highest median MP-eye score (P = 0.033). Dietary intake of MPs and other lifestyle factors did not correlate significantly with MP-eye score overall or when analyzed in clusters.Conclusions: MP-eye scores of an Indian population were normally distributed. Higher age, high BMI, and presence of hypertension were weakly associated with lower MP-eye scores. The impact of diet on MPs requires further evaluation.Translational Relevance: This normative regional database enables risk stratification of macular degeneration.

Abstract:Background: Multiple sclerosis (MS) can cause retinal thinning among persons with MS with optic neuritis (MS-ON). Macular xanthophylls are carotenoids that comprise the macular pigment, filtering blue light and countering photo-oxidation. However, macular xanthophyll status and its implications for markers of neuroaxonal degeneration have not been examined in MS. Objectives: This study characterized differences in macular and serum xanthophylls, and retinal morphometry [retinal nerve fiber layer thickness at the macular (mRNFL) and optic disc (odRNFL) and total macular volume (TMV)] in individuals with MS and healthy controls (HC). Associations between macular pigment optical density (MPOD) and retinal morphometry were also examined. Methods: Adults aged 45-64 y (HC, n = 42; MS, n = 40) participated in a cross-sectional study. MPOD was measured via heterochromatic flicker photometry. Retinal morphometry was measured via optical coherence tomography (OCT). Serum carotenoids were quantified using HPLC. Dietary carotenoids were collected using 7-d records. One-factor ANOVA was conducted to determine group effects on macular, serum, and dietary carotenoids. Partial correlations examined the relations between MPOD, retinal morphometry, diet, and serum carotenoids. Results: Relative to HC, persons with MS-ON had lower MPOD (Cohen's d = 0.84, P = 0.014), lower odRNFL (Cohen's d = 2.16, P <0.001), lower mRNFL (Cohen's d = 0.57, P = 0.028), and lower TMV (Cohen's d = 0.95, P = 0.011). MS without ON (MS) had lower odRNFL (Cohen's d = 0.93, P = 0.001) than HC and lower serum lutein than MS-ON subjects (Cohen's d = 0.65, P = 0.014). Among MS, MPOD was positively correlated with odRNFL thickness (ρ = 0.43, P = 0.049) and TMV (ρ = 0.45, P = 0.039), whereas odRNFL was negatively correlated with serum lutein (ρ = -0.68, P = 0.016) and zeaxanthin (ρ = -0.62, P = 0.028). Conclusions: Persons with MS-ON exhibited poorer xanthophyll status in the macula and serum. MPOD was associated with beneficial anatomical features in the MS group. These findings warrant confirmation with larger cohorts and prospective trials to evaluate xanthophyll effects on the anterior visual pathway in MS.
Macular carotenoid supplementation improves disability glare performance and dynamics of photostress recovery

Abstract:Background: The so-called macular carotenoids (MC) lutein (L), zeaxanthin (Z), and meso-zeaxanthin (MZ) comprise the diet-derived macular pigment (MP). The purpose of this study was to determine effects of MC supplementation on the optical density of MP (MPOD), repeated-exposure photostress recovery (PSR), and disability glare (DG) thresholds. Methods: This was a double-blind, placebo-controlled trial. Fifty-nine young (mean age = 21.7), healthy volunteers participated in this study. Subjects supplemented their daily diet with either 10 mg L + 2 mg total Z (1 mg Z + 1 mg MZ; n = 24), 20 mg L + 4 mg total Z (2 mg Z + 2 mg MZ; n = 25), or placebo (n = 10) for 12 months. The primary outcome was a composite measure of visual performance in glare, defined by change in DG and PSR. Secondary outcomes included MPOD and visual fatigue. The primary endpoint for outcomes was 12 months. MPOD was assessed with customized heterochromatic flicker photometry. PSR times for an 8 cycle /degree, 15 % contrast Gabor patch target were determined after each of five successive exposures to intense LED lights. DG threshold was defined as the intensity of a ring of lights through which subjects were able to maintain visibility of the aforementioned target. Measures of all parameters were conducted at baseline, 6 months, and 12 months. Repeated-measures ANOVA, and Pearson product-moment correlations were used to determine statistically significant correlations, and changes within and between groups. Results: MPOD for subjects in both supplementation groups increased significantly versus placebo at both 6- and 12-month visits (p < 0.001 for all). Additionally, PSR times and DG thresholds improved significantly from baseline compared to placebo at 6- and 12-month visits (p < 0.001 for all). At baseline, MPOD was significantly related to both DG thresholds (r = 0.444; p = 0.0021) and PSR times (r = -0.56; p < 0.001). As a function of MPOD, the repeated-exposure PSR curves became more asymptotic, as opposed to linear. The change in subjects' DG thresholds were significantly related to changes in PSR times across the study period (r = -0.534; p < 0.001). Conclusions: Increases in MPOD lead to significant improvements in PSR times and DG thresholds. The asymptotic shape of the repeated-exposure PSR curves suggests that increases in MPOD produce more consistent steady-state visual performance in bright light conditions. The mechanism for this effect may involve both the optical filtering and biochemical (antioxidant) properties of MP. Trial registration: ISRCTN trial registration number: ISRCTN54990825. Modeling the effect of macular pigment enhancement on vision in degraded visual environments (DVE)

Abstract:The macular pigment (MP) is an accumulation of lutein and zeaxanthin, carotenoids derived from dietary sources, which is primarily in the central 15° of the human visual field. MP absorbs light in the 400 to 520 nm range. Consequently the MP is a spectral filter over the photoreceptors, reducing the effects of internally scattered light and attenuating the short wavelength component of natural sunlight. The between-subject average MP optical density (OD) is about 0.2 to 0.6 log units depending on the sample population, while the range of MPOD is reportedly 0 to 1.5 log units. Some people can increase their MPOD by increasing their consumption of lutein and zeaxanthin, and this may be important for vision in DVE. Specifically, nutritional interventions and dietary supplements have produced statistically significant enhancements under laboratory conditions in visual tasks such as visibility through haze, low contrast target detection, contrast sensitivity, glare resistance and recovery, photostress recovery, dark adaptation, mesopic sensitivity, and enhanced reaction times. The question is whether these enhancements are operationally meaningful or not. The present paper begins to address the question by modeling MPOD effects on the visibility to low contrast targets seen under a range of DVE over realistic distances that incorporate atmospheric filtering. Specific model parameters include luminance, target contrast, spectral content, and distance. The model can be extended to estimate the efficacy of MPOD effects on target detection, discrimination, and standoff distances.
