Abstract:Resident physician labor unions have existed in a limited capacity in the US for decades, but this landscape has been rapidly changing for the past 5 years. Resident labor unions first appeared in 1934 in New York to advocate for compensation (there was none), ultimately forming the Committee of Interns and Residents (CIR) in 1957. 1 ,2 Amid a more intense period of activism in the 1970s, the National Labor Relations Board (NLRB) ruled in 1976 that resident physicians are students, not employees, and therefore are not protected by federal law. 3 This ruling stifled any substantial change until 1999, when the NLRB reversed this decision and declared resident physicians as employees entitled to the right to organize and collectively bargain. 4 As of May 2024, the CIR represents 32 000 trainees, about 20% of the resident workforce. 5 Although not the majority, this number was only 16 000 (10%) in 2019. 6 In fact, since late 2021, trainees from at least 14 institutions have voted to unionize, reflecting an accelerating pace of change. 7 ,8 It has been suggested that changing work attitudes shaped by the COVID-19 pandemic catalyzed this movement, but little research has explored trainee perceptions. Foote et al 9 present a thought-provoking qualitative analysis assessing general surgery residents and program faculty and leadership perspectives on trainee unionization. Amid 15 site visits that occurred in 2019 to 2020 to study resident well-being, they identified and reviewed 22 interviews from 2 programs (one with a union in place for over 20 years and one program that has recently unionized), including 13 resident-focused interviews (with representation from each year of training), in which unionization was discussed. Three major themes emerged from this work: (1) surgical residents unionize for voice and agency, (2) union-negotiated benefits have varying levels of efficacy and relevance to surgical residents, and (3) unionization affects the educational environment. It is important to remember that the data analyzed were not derived from a study that focused on unionization but rather from further analysis of the unionization theme in the context of a more extensive study. This design serves as a strength by eliminating any bias influenced by the targeted questions about unionization. Still, it may have also thwarted other themes from being uncovered and limited the generalizability of the findings (as compared with those from a study that intentionally included trainees from more specialties and leadership teams from more institutions). With that being said, the themes generated from the exploratory analysis by Foote et al 9 take a crucial step forward by helping the graduate medical education community begin to close the substantial gaps in knowledge of unionization's effects on various learning environments. With surgical residents giving the majority of the interviews (13 of 22) in the study by Foote et al, 9 themes 1 and 2, which reflect surgical resident perspectives, are sound given that the number of interviews is in a range to achieve saturation. Surgical residents unionizing for voice and agency (theme 1), which provides a more straightforward path to enact change, is entirely consistent with why groups of laborers have unionized throughout history. In fact, the CIR itself advertises, "Through our union, we have a powerful voice in the fight to improve our healthcare system ... and solve problems we see in our hospital every day." 2 The corporatization of medicine, which inevitably creates systems focused on throughput and the bottom line, places trainees in systems that can overemphasize work and deemphasize education. This environment in the broader context of stark generational differences and changing societal attitudes toward work likely explains the impetus behind this theme. 6 Further data are needed to confirm that it extends beyond surgical trainees, but it is not difficult to imagine that it does. Union-negotiated benefits have varying levels of efficacy and relevance to surgical residents (theme 2), and most gains were felt to be primarily financial, from direct salary increases to other nonsalary benefits (meals, educational stipends, parking, etc). Other union-negotiated benefits were less consistently appreciated or accessible. For instance, time-off benefits had several reported barriers, including potentially extending training time, inadequate coverage of required patient care duties, and possibly perceived judgment from colleagues. Depending on specialty-specific cultural and educational norms, these barriers will be present in varying degrees in different training programs. Contextualizing the Resurgence of U.S. Housestaff Union Activity

Abstract:There is a growing trend of resident and fellow physician unionization in the United States, with 14 new housestaff unions formed at private employers since 2022. This resurgence of housestaff union organizing parallels the last era of housestaff activism in the 1960s. Today's housestaff organizing takes place within the context of longstanding challenges in medicine, including the burnout and systemic inequities highlighted by the COVID-19 pandemic, and an increase in national activism and labor organizing. Housestaff unions offer opportunities for residents and fellows to negotiate for improvements across multiple issues.In this Commentary, the authors focus on common bargaining topics: poor working conditions, undercompensation, and inadequate representation in an increasingly corporatized health care landscape. The authors also discuss the role of collective bargaining for improving the housestaff experience and address common concerns about unionization. Finally, the authors explore the limited evidence of the impact of unions in health care settings and outline key considerations for future scholarship.The current generation of housestaff started their medical careers with an awareness of systemic challenges to the profession and have responded through collective organizing. While the short- and long-term ramifications of housestaff organizing need further study, the authors express optimism that unionization will lead to improved working conditions and thus improved health care delivery.
Experiences With Unionization Among Surgical Resident Physicians, Faculty, and Staff

Abstract:Importance Labor unions are a mechanism for employee advocacy, but their role in surgery resident wellness is poorly characterized. Objective To understand experiences with unionization among general surgery residents and residency program faculty and staff. Design, Setting, and Participants This exploratory qualitative study included data from the Surgical Education Culture Optimization Through Targeted Interventions Based on National Comparative Data (SECOND) trial. In the exploratory phase of the SECOND trial (from March 6, 2019, to March 12, 2020), semistructured interviews about wellness were conducted with residents, faculty (attending physicians), and staff (program administrators) at 15 general surgery residency programs. Unionization was identified as an emergent theme in the interviews. Data analysis was performed from March 2019 to May 2023. Main Outcomes and Measures The main outcome was resident and faculty experience with resident labor unions. In the qualitative analysis, lexical searches of interview transcripts identified content regarding resident labor unions. A codebook was developed inductively. Transcripts were coded by dyads, using a constant comparative approach, with differences reconciled by consensus. Results A total of 22 interview transcripts were identified with relevant content. Of these, 19 were individual interviews conducted with residents (n = 10), faculty (n = 4), administrative staff (n = 1), a program director (n = 1), a department chair (n = 1), and designated institutional officials (n = 2), and 3 were from resident focus groups. Residents from all postgraduate year levels, including professional development (ie, research) years, were represented. Interviewees discussed resident unions at 2 programs (1 recently unionized and 1 with a decades-long history). Interviewees described the lack of voice and the lack of agency as drivers of unionization ("Residents...are trying to take control of their well-being"). Increased salary stipends and/or housing stipends were the most concretely identified union benefits. Unanticipated consequences of unionization were described by both residents and faculty, including (1) irrelevance of union-negotiated benefits to surgical residents, (2) paradoxical losses of surgery department–provided benefits, and (3) framing of resident-faculty relationships as adversarial. Union executives were noted to be nonphysician administrators whose participation in discussions about clinical education progression may increase the time and effort to remediate a resident and/or reduce educators' will to meaningfully intervene. Active surgical resident participation within the union allows for an understanding of surgical trainees' unique needs and reduced conflict. Conclusions and Relevance In this qualitative study, unionization was a mechanism for resident voice and agency; the desire to unionize likely highlighted the lack of other such mechanisms in the training environment. However, these findings suggest that unionization may have had unintended consequences on benefits, flexibility, and teaching. Effective advocacy, whether within or outside the context of a union, was facilitated by participation from surgical residents. Future research should expand on this exploratory study by including a greater number of institutions and investigating the evolution of themes over time.

Resident Unions: Why Now and Will They Change Medical Education and Health Care?

Abstract:During the COVID-19 pandemic, resident unions proliferated. While unionization resulted in increased compensation and benefits, the process of union negotiations may have created adversarial relationships between residents and their institutions' leadership, who residents depend on for supervision and the development of clinical expertise. Such adversarial relationships could affect the learning environment, which is critical to the delivery of high-quality care. In this commentary, the authors suggest that academic medical centers should offer residents an authentic seat at the institutional care delivery leadership table, ensuring residents' full participation in key organizational decisions. Doing so represents an alternative to unionization, with its potentially adversarial relationships, while still achieving a key goal of residents-to be included in the decisions that affect them and the care they provide. In this way, residents can use their unique understanding of the institutions' strengths and weaknesses to improve the quality of patient care and the learning environment. Such engagement can also help residents achieve competence in systems-based practice and provide a vital link between institutions and the patients and community they serve through health policy and advocacy activities.

Impacts of resident physician unionization on house staff compensation

Abstract:Background: Physicians-in-training in the United States work long hours for relatively low wages. In response to increased economic burden, the popularity of unionization in residency training programs has increased dramatically. In this study, we conducted a cross-sectional investigation of the association between unionization status and Internal Medicine PGY-1 compensation and benefits. Methods and findings: We compiled residency salary and benefits data from all Internal Medicine residency training programs in the United States. Using a mixed effects modeling approach, we evaluated the differences in salary and total compensation while adjusting for regional factors and cost-of-living differences. In aggregate, PGY-1 salary was higher for unionized vs. non-unionized programs ($69648 vs. $62214; [95% CI 670.7-3563.7]). However, there was no difference after adjusting for cost-of-living ($62515 vs $62475; [95% CI. -1317.5, 1299.7]). Unionized programs do however offer greater monetary benefits in the form of stipend disbursements, and total compensation is higher in unionized vs. non-unionized residency programs ($65887 vs $63515; [95% CI 607.6, 3551.5]). Conclusions: Unionized residency programs offer higher total compensation packages than their non-unionized counterparts. This increase in compensation is driven in large part by an increased variety and amount of stipend disbursement.
National Evaluation of the Association Between Resident Labor Union Participation and Surgical Resident Well-being

Abstract:Importance: Labor unions are purported to improve working conditions; however, little evidence exists regarding the effect of resident physician unions. Objective: To evaluate the association of resident unions with well-being, educational environment, salary, and benefits among surgical residents in the US. Design, setting, and participants: This national cross-sectional survey study was based on a survey administered in January 2019 after the American Board of Surgery In-Training Examination (ABSITE). Clinically active residents at all nonmilitary US general surgery residency programs accredited by the American Council of Graduate Medical Education who completed the 2019 ABSITE were eligible for participation. Data were analyzed from December 5, 2020, to March 16, 2021. Exposures: Presence of a general surgery resident labor union. Rates of labor union coverage among non-health care employees within a region were used as an instrumental variable (IV) for the presence of a labor union at a residency program. Main outcomes and measures: The primary outcome was burnout, which was assessed using a modified version of the abbreviated Maslach Burnout Inventory and was defined as experiencing any symptom of depersonalization or emotional exhaustion at least weekly. Secondary outcomes included suicidality, measures of job satisfaction, duty hour violations, mistreatment, educational environment, salary, and benefits. Results: A total of 5701 residents at 285 programs completed the pertinent survey questions (response rate, 85.6%), of whom 3219 (56.5%) were male, 3779 (66.3%) were White individuals, 449 (7.9%) were of Hispanic ethnicity, 4239 (74.4%) were married or in a relationship, and 1304 (22.9%) had or were expecting children. Among respondents, 690 residents were from 30 unionized programs (10.5% of programs). There was no difference in burnout for residents at unionized vs nonunionized programs (297 [43.0%] vs 2175 [43.4%]; odds ratio [OR], 0.92 [95% CI, 0.75-1.13]; IV difference in probability, 0.15 [95% CI, -0.11 to 0.42]). There were no significant differences in suicidality, job satisfaction, duty hour violations, mistreatment, educational environment, salary, or benefits except that unionized programs more frequently offered 4 weeks instead of 2 to 3 weeks of vacation (27 [93.1%] vs 52 [30.6%]; OR, 19.18 [95% CI, 3.92-93.81]; IV difference in probability, 0.77 [95% CI, 0.09-1.45]) and more frequently offered housing stipends (10 [38.5%] vs 9 [16.1%]; OR, 2.15 [95% CI, 0.58-7.95]; IV difference in probability, 0.62 [95% CI 0.04-1.20]). Conclusions and relevance: In this evaluation of surgical residency programs in the US, unionized programs offered improved vacation and housing stipend benefits, but resident unions were not associated with improved burnout, suicidality, job satisfaction, duty hour violations, mistreatment, educational environment, or salary.

Abstract:Unionization is a leading trend impacting higher education setting the stage for the struggle between faculty and the administration over the future direction of higher education. Higher education in the U.S. is under pressure to make some profound changes to increase its productivity, to lower costs, and to globalize. Recent trends show movement towards centralized management, revenue-based education models, focus on short-term input/output student outcomes, revised curricula driven by digital media, modified learning environments, and MOOCs – massive open online courses. Trends also show that faculty is losing its ability to influence curricula as colleges change their fundamental operating model to increase efficiency. In the U.S., faculty responded to this trend through unionization with 38% of U.S. faculty unionized by 2013, and faculty unionization is increasing while private-sector unions are in decline. In 1986, Louis L. Jones made the claim that “unions are a permanent fixture in higher education.” Few, if any, faculty or administrators would deny that claim today. However, the question remains: “Is the unionization trend good for higher education in the U.S.?” A second and closely related question is: “Will unionization enable faculty to influence the future of higher education?” The answer to these two questions must begin with an understanding of the forces that push-and-pull faculty to seek unionization.

Abstract:Medical trainees (applicants, students, and house officers) often engage in global health initiatives to enhance their own education through research and patient care. These endeavors may concomitantly prove of value to host nations in filling unmet clinical needs. At present, healthcare institutions generally focus on the safety of the trainee and the welfare of potential patients and research subjects when sanctioning such programs. The American medical community has historically afforded less consideration to the ethics of engagement by trainees from the United States in nations known for serious human rights transgressions. This essay examines the ethics of such endeavors and argues for increased consideration of these broader considerations when trainees engage in global health work abroad.

Managing Workplace Job Attitudes and Performance in Organizations with Labor Unions

Abstract:While union membership has been in decline in recent decades, still 11% of the US workforce belong to unions. The job attitudes of both union and non-union employees are important to organizational success. Various studies suggest that union employees are as satisfied as non-union employees with benefits, wages, and job security. Non-union employees have more favorable attitudes toward the type of work they’re asked to do, autonomy, opportunities for advancement, and supervision. But job attitudes for both union and non-union employees vary widely by the team they are on, according to a summary of Gallup’s employee engagement global database. In this chapter, the authors summarize opinions from within highly unionized organizations, including 239,459 union-member and 42,053 non-union-member employees. They outline key challenges in engaging unionized employees and practical advice compiled from the study of successful organizations across industries.
Recruitment of House Staff into Anesthesiology: Factors Responsible for House Staff Selecting Anesthesiology as a Career and Individual Training Program

Abstract:Due to a variety of factors, there was a recent redistribution of graduating American medical students toward primary care specialties and a decline in the number of applicants seeking training positions in anesthesiology. In the present study, factors responsible for Mayo house staff (i.e., residents and clinical fellows) selecting anesthesiology as a career, and Mayo Clinic as their training program, were identified. We also evaluated their level of satisfaction with their choice of career and training program, and their perceptions of the future for anesthesiology trainees. A cross sectional analysis was conducted using a questionnaire to survey 67 house staff enrolled in the anesthesiology training program at Mayo Clinic, Rochester, MN during the 1995-1996 academic year. All responses were anonymous. Data were analyzed using the chi-square and Mann-Whitney rank sum tests. P ? 0.05 was considered statistically significant. Forty-eight (72%) of those surveyed responded to the questionnaire. The most frequently cited reasons for selecting anesthesiology as a career included it: is a "hands-on" specialty, involves clinical application of physiology and pharmacology, and provides immediate gratification in one's work. The most frequently cited reasons for selecting our training program were the diversity of training experience, prestige associated with Mayo Clinic, and employment opportunities following training. Forty-four (92%) felt downsizing of anesthesiology training programs was a national trend, 26 (54%) anticipated difficulty obtaining a job following training, and 16 (33%) felt they had future job security. Overall, 47 (98%) were happy with their career choice, and 40 (83%) would choose anesthesiology as a career if they were now graduating from medical school. Our data indicate that selection of a career in anesthesiology and training program are strongly associated with concerns regarding educational experiences and postgraduate employment opportunities. More than half anticipated difficulty obtaining postgraduate employment; however, this skepticism was overshadowed by two findings: the trainees reported a very high level of satisfaction in their choice of career and training program, and the vast majority reported they would seek a career in anesthesiology if they were now selecting a medical specialty. Additionally, all 1996 graduates found suitable employment without difficulty.
Characteristics of the informal curriculum and trainees' ethical choices

Abstract:In October 1995, the Association of American Medical Colleges held its first Conference on Students' and Residents' Ethical and Professional Development. In a plenary session and break-out sessions, the 150 participants, representing a wide variety of medical and professional specialties and roles, discussed the factors and programs that affect medical trainees' development of ethical and professional standards of behavior. The main challenge of addressing students' professional development is the enormous range of influences on that development, many of which, such as the declines in civic responsibility and good manners throughout the United States, fall outside the scope of academic medicine. Nonetheless, many influences fall within reach of medical educators. In a pre-conference survey, participants ranked eight issues related to graduating ethical physicians. The respondents ranked highest the inadequacy of the understanding of how best to influence students' ethical development, followed by faculty use of dehumanizing coping mechanisms, and the "business" of medicine's taking precedence over academic goals. The plenary speakers discussed the "informal curriculum" and the "hidden curriculum" and the need for medical faculty to take seriously the great influence they have on students' and residents' moral and professional development as they become physicians. Whether consciously or not, medical education programs are producing physicians who do not meet the ethical standards the profession has traditionally expected its members to meet. In three series of break-out sessions, the participants analyzed the nature of the ethical dilemmas that medical students and residents face from virtually the first day of their training, the use of role playing in promoting ethical development, and ways to improve policies and overcome barriers to change.
The relationship between union membership and self-rated health among racially diverse U.S. healthcare workers: The role of control over work schedules and locations

Abstract:Background Labor unions are associated with better wages, improved working conditions, and greater worker empowerment, which may result in better health. However, less is known about the relationship between unionization and health among U.S. healthcare workers, whether the relationship differs among racially diverse workers, and how much control over workplace schedules and location mediates the relationship. Methods We analyzed a cross-sectional survey of a nationally representative sample of 3000 U.S. healthcare workers collected from March 14 through April 5, 2023. Using ordinal logistic regression, we evaluated whether unionized healthcare workers had better self-rated health (SRH) than their nonunionized counterparts and examined potential differences between White and racially minoritized respondents. We quantified the mediation percentage explained by control over one's schedule and workplace location in the total and stratified samples using Karlson, Holm, and Breen decomposition analysis. Results Over a third (36.1%) of racially minoritized respondents were unionized, compared to 22.3% of White respondents. Among racially minoritized workers, a greater share of unionized workers reported excellent health (40.6% vs. 21.8%) than their nonunionized counterparts. In confounder-adjusted ordinal logistic regression analyses, labor union membership was associated with better SRH overall, with a stronger association for racially minoritized workers. Among White healthcare workers, control over workplace arrangements explained 68.1% of the union membership and SRH relationship. For racially minoritized workers, control over workplace arrangements partially mediated the relationship, explaining 17.4% of the variation, suggesting that labor unions may impact health through additional pathways for these workers. Conclusions This study provides empirical evidence of the relationship between labor union membership and health among U.S. healthcare workers. We demonstrate that control over schedules and location is an important mechanism by which unionization may protect healthcare workers' health. Among racially diverse healthcare workers, labor unions may play an important role in health through various pathways beyond workplace control.

See No Evil, Hear No Evil, Stop No Evil: Institutional-Level Tracking to Combat Mistreatment of Residents and Fellows

Abstract:Background: Mistreatment of trainees, including discrimination and harassment, is a problem in graduate medical education. Current tools to assess the prevalence of mistreatment often are not administered institutionally and may not account for multiple sources of mistreatment, limiting an institution's ability to respond and intervene. Objective: We describe the utility of a brief questionnaire, embedded within longer institutional program evaluations, measuring the prevalence of different types of trainee mistreatment from multiple sources, including supervisors, team members, colleagues, and patients. Methods: In 2018, we administered a modified version of the mistreatment questions in the Association of American Medical Colleges Graduation Questionnaire to investigate the prevalence and sources of mistreatment in graduating residents and fellows. We conducted analyses to determine the prevalence, types, and sources of mistreatment of trainees at the institutional level across graduate medical education programs. Results: A total of 234 graduating trainees (77%) from the University of Minnesota-Twin Cities completed the questions. Patients were cited as the primary source of mistreatment in 5 of 6 categories, including both direct and indirect offensive remarks, microaggressions, sexual harassment, and physical threats (paired t test comparisons from t = 3.92 to t = 9.71, all P < .001). The only category of mistreatment in which patients were not the most significant source was humiliation and shaming. Conclusions: Six questions concerning types and sources of trainee mistreatment, embedded within an institutional survey, generated new information for institutional-, departmental- and program-based future interventions. Patients were the greatest source for all types of mistreatment except humiliation and shaming.

Abstract:What roles should women’s labor education play in the twenty-first-century labor movement? This question sparked a series of research, collaboration, and long-range planning activities undertaken by the Union Women’s Labor Education Project starting in 2013. This article builds on work undertaken to date by the Union Women’s Labor Education Project (in collaboration with the Women’s Caucus of the United Association of Labor Educator [UALE] and the Berger-Marks Foundation), presenting a new analysis of relationships among women’s labor education, leadership development, and movement building, with a particular focus on regional UALE women’s summer schools as a case study.

The Impact of Unions on US Direct Care Workers in Long-Term Care Settings: A Systematic Review

Abstract:Objectives: To meet a growing demand for direct care workers (DCWs) in the United States, structural, organizational, and policy-related solutions are needed. Unionization of the workforce may be one such mechanism; however, its impact on outcomes remains poorly understood. To examine the impact of unionization on DCWs' financial well-being and employment attitudes, as well as patient outcomes. Design: A systematic search of AgeLine, CINAHL, PubMed, Scopus, and Web of Science from database inception through June 20, 2024. We included peer-reviewed empirical studies that used observational, quasi-experimental, and experimental designs. Setting and participants: Studies pertained to DCWs who provided care in the home and long-term care settings. We focused on studies that illustrated the financial outcomes of DCWs (wages, compensation, benefits), employment outcomes (job satisfaction, turnover), and health-related outcomes of DCWs and their patients. Methods: Covidence was used to screen studies for inclusion criteria. Study characteristics were abstracted manually by prespecified domains. The Downs and Black tool was used for quality assessment. The Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guideline was followed. Results: A total of 19 studies met inclusion criteria; they were predominantly observational (94%), with local (50%) and national (50%) samples. Three studies focused on compensation and all found that unionization was associated with higher wages and benefits among DCWs. Seven studies focused on employment, finding that unionization was associated with greater job satisfaction, quality, and retention, particularly among nursing home staff and home health aides. Unionized workplaces reported better care quality and safety, including fewer injuries and better equipment provision. Unionization's impact on patient outcomes showed mixed results, particularly among nursing home residents. Overall, the quality of the research studies varied, with limitations in methodology and sampling affecting reliability. Conclusions and implications: Unionization among DCWs was generally associated with higher wages, benefits, and job satisfaction, as well as reduced turnover; however, its impact on worker and patient outcomes varied across studies. The overall quality of the studies was fair to poor, highlighting the need for more rigorous research in this area.

Abstract:Background: The learning environment within a school of medicine influences medical students' values and their professional development. Despite national requirements to monitor the learning environment, mistreatment of medical students persists. Methods: We designed a program called WE SMILE: We can Eradicate Student Mistreatment In the Learning Environment with a vision to enhance trainee and faculty awareness and ultimately eliminate medical student mistreatment. We provide a description of our program and early outcomes. Results: The program has enhanced student awareness of what constitutes mistreatment and how to report it. Faculty members are also aware of the formal processes and procedures for review of such incidents. Our proposed model of influences on the learning environment and the clinical workforce informs the quality of trainee education and safety of patient care. Institutional leadership and culture play a prominent role in this model. Our integrated institutional response to learning environment concerns is offered as a strategy to improve policy awareness, reporting and management of student mistreatment concerns. Conclusions: Our WE SMILE program was developed to enhance education and awareness of what constitutes mistreatment and to provide multiple pathways for student reporting, with clear responsibilities for review, adjudication and enforcement. The program is demonstrating several signs of early success and is offered as a strategy for other schools to adopt or adapt. We have recognized a delicate balance between preserving student anonymity and informing them of specific actions taken. Providing students and other stakeholders with clear evidence of institutional response and accountability remains a key challenge. Multiple methods of reporting have been advantageous in eliciting information on learning environment infringements. These routes and types of reporting have enhanced our understanding of student perceptions and the specific contexts in which mistreatment occurs, allowing for targeted interventions. A common platform across the healthcare professions to report and review concerns has afforded us opportunities to deal with interprofessional issues in a respectful and trustworthy manner. We offer a model of learning environment influences with leadership and institutional culture at the helm, as a way to frame a comprehensive perspective on this challenging and complex concern.
The More Things Change, the More They Stay the Same: Residents Perspectives on Learning to Teach Over 7 Years

Abstract:Despite increasing emphasis on the development and implementation of Residents-as-Teachers programs, resident perspectives on their roles as teachers have rarely been explored. This paper explores trends across 7 years of data collected from resident leaders across North American orthopaedic residency programs. Methods: Data were collected during the American Orthopaedic Association's resident-only engagement activity known as the C. McCollister Evarts Resident Leadership Forum (RLF). A total of 997 of 1,050 RLF participants responded from 2015 to 2021. Results: Residents perceived themselves as having a strong influence on medical students' education more so than any other teacher. However, less than one third are provided with any formal instruction from their institutions to support their teaching, and nearly 3 quarters of participants felt there was room for improvement in their teaching effectiveness. For the most part, resident perspectives have stayed relatively consistent over time. Discussion: Residents are keen and willing to invest time into developing their teaching effectiveness. There may be a need for improved advocacy to program directors to increase the presence and availability of formal Residents-as-Teachers programs to support residents in their roles as teachers. We offer suggestions for the composition of such programs for consideration.

Abstract:Objective: Teaching by trainee doctors is also established practice in general practice in English-speaking countries. This study examines the involvement of trainee doctors in the supervision of students in German general practices and the acceptance of trainee doctors as teachers from the perspective of physicians with a license for post-graduate training (PLT) and the trainee doctors themselves. Methodology: Semi-structured qualitative interviews were conducted with 9 PLTs and 9 trainees. The interview guide was developed based on the Theoretical Framework of Acceptance. Interviews were recorded, transcribed and evaluated using Kuckartz's qualitative analysis. Results: Trainee doctors are involved in student supervision in GP teaching practices to varying degrees and often in unstructured ways. Supervision by trainees is considered advantageous as they are closer in terms of hierarchy, possess more up-to-date knowledge and are less far ahead in terms of knowledge and function as role models. However, professional uncertainty or revealing knowledge gaps to patients and students is experienced as difficult by some trainees. Competing for time with patient care is seen as a challenge. Better time planning and didactic preparation could avoid pressure in this area. Teaching is seen as part of the GP profession, especially by trainee doctors. However, a potential obligation to teach is seen as more of a hindrance to encouraging the next generation of doctors by both trainee doctors and PLTs. Conclusion: The inclusion of trainee doctors in student teaching is frequently practiced by those surveyed, which suggests a high level of acceptance but is not consistently implemented. Structured organization of teaching in real life, didactic qualifications and offering credits for teaching activities might further improve inclusion and acceptance.
