Abstract: A case of systemic lupus erythematosus complicated by pemphigus vulgaris is reported. A 42-year-old female patient presented facial red macules with the joints pain on the limbs for 4 years. She was photosensitive with hematologic abnormal, positive antinuclear antibody, positive antibody to ds-DNA. She got relapsing erosions of mucous membranes in mouth for 2 years. Then extensive, generalized bullae developed, and the bullae soon ruptured to form erosions. The diagnosis of pemphigus vulgaris was confirmed by clinical, histological and immunofluorescent evidences.

Abstract:Background Mal de Meleda (MDM, OMIM 248300) is an autosomal recessive disease characterized by symmetrical and progressive palmoplantar hyperkeratosis soon after birth. Mutations in SLURP1 gene could lead to MDM. Clinically, MDM is easily misdiagnosed as other types of keratoderma due to phenotypic variation and overlap. Objective and Methods A patient with suspected MDM was confirmed by the combination of next-generation sequencing and Exomiser, and the patient was attempted with the treatment of Ixekizumab and Adalimumab. Results A homozygous mutation c.256G>A (p.Gly86Arg) in the SLURP1 gene was identified in the patient. The inflammatory erythemas on his hands, feet and buttocks were mildly relieved after the treatment of high dose of Ixekizumab. Conclusions Our findings helps to enhance the understanding of MDM. Ixekizumab may be a potential strategy to treat MDM.

Abstract:Wooly hair is a rare hair shaft abnormality characterized by extremely curly hair. It can occur in isolation or associated with palmoplantar keratoderma (PPK) and cardiac defects. We present the case of a 56-year-old female patient who initially presented with non-itchy, painful skin lesions over palms and soles since 5years and had abnormal scalp hair texture since childhood. She responded positively to treatment with topical application of 6%salicylic acid twice a day for 3 months on which the thickness and size of the plaques gradually reduced.

Abstract:A case of hyperimmunoglobulinemia E syndrome with subcutaneous chronic mycosis is reported.A 24-year-old male patient presented with erythema,paplues and patches on his extremities and trunk for 23 years,subcutaneous nodule and inguinal swollen lymph nodes for 4 years.His peripheral blood eosinophils and serum IgE levels were high.Skin biopsy revealed that chronic dermatitis changes and eosinophils could be seen in the dermis.Silver staining of inguinal swollen lymph node in the revealed hyphae,pseudohyphae and spores.

Abstract:Objective To report a group of cases characterized by pigmented symmetrical acrokeratoderma.Methods Sixteen patients characterized by pigmented syrnmetrical acrokeratoderma were collected from outpatients from May 1998 to September 2007 in the Department of Dermatology,Second Affiliated Hospital,Sun Yat-Sen University.General information on the sex,age,profession,family history of patients were documented.All patients underwent clinical and histopathological examinations.and were followed up for the assessment of treatment efficiency.Results Among these patients,14 were males and 2 were females,ranging in age from 9 to 54 years,with duration of disease varying from 2 months to 8 years(mean course 2.0±1.5 years).Characteristic manifestations were rough,scaling,brownish aubum or black maculopapular eruptions with mild pruritus.The lesions were symmetrically distributed on palms,dorsal digits and wrists of 16(100%)patients,elbows and knees of 2(12.5%)patients,and ankles of 3(18.8%)patients.Histopathologic examination revealed epidermal hyperkeratosis,acanthosis and papillomatous hyperplasia as well as dermal infiltration with a few lymphocytes.No heredity tendency was observed in any of the patients.The patients showed a poor response to,or experienced a relapse after treatment with oral tretinoins,nicotinamide,vitamin B6 as well as topical tretinoin ointment,glucocorticoid ointment,and so on.Condusion These cases may represent a new entity clinically characterized by pigmented symmetrical acrokeratoderma,and pathologically characterized by epidermal hyperkeratosis,acanthosis and papillomatous hyperplasia with unknown etiology.

Abstract:An otherwise healthy man in his 20s presented to the dermatology department with thickening and swelling of hands after immersion in water, accompanied by burning pruritus for 3 years. The lesions occurred as symmetrical white and transparent papules, white keratosis, excessive wrinkling on the dorsum of hands after water immersion and worsened in summer, and remitted in winter. He was treated with topical tretinoin ointment intermittently per a diagnosis of chronic eczema at the local clinic with little improvement. The lesions gradually aggravated, and the affected area dispersed to wrists and elbows during past the 1.5 years. He attributed these changes to the need for washing hands frequently in the period of the COVID-19 epidemic. He had no family history of similar skin problems and did not have a history of hyperhidrosis, cystic fibrosis, atopy, or trauma to the hands.

Abstract:This woman in her late 40s presented with widespread papules and tightening of the skin that had progressed over the past seven years (fig 1). Physical examination showed numerous 1-3 mm asymptomatic waxy, skin coloured papules on the face and neck, dorsal aspect of the hands, forearms, trunk, and legs, along with hyperpigmentation and reduced joint mobility in the hands and larger extremities. She had no history of Raynaud's phenomenon or dysphagia, and results for thyroid function and autoantibody screening were normal, excluding myxoedema and systemic sclerosis as likely diagnoses. Abstract:A 25-year-old male patient presented erythema with desquamation on the scalp for 1 year.He had a 1-month history of pruritic skin,lesion on trunk and limbs.Skin examination showed widespread annular lesions with polycyclic arrangement on the trunk and limbs with development of peripheral vesicular lesions.Nikosky′s sigh was negative.Skin biopsy showed intraepidermal vesicules containing eosinophils,no acantholytic cells,papillary oedema and dermal inflammatory infiltrate containing eosinophils,lymphocytes and histiocytes.Directimmunofluorescence staining showed the cell surface C3 deposits throughout the epidermis.

Abstract:Background: The differential diagnosis of atypical melanocytic palmoplantar skin lesions (aMPLs) represents a diagnostic challenge, including atypical nevi (AN) and early melanomas (MMs) that display overlapping clinical and dermoscopic features. We aimed to set up a multicentric dataset of aMPL dermoscopic cases paired with multiple anamnestic risk factors and demographic and morphologic data. Methods: Each aMPL case was paired with a dermoscopic and clinical picture and a series of lesion-related data (maximum diameter value; location on the palm/sole in 17 areas; histologic diagnosis; and patient-related data (age, sex, family history of melanoma/sunburns, phototype, pheomelanin, eye/hair color, multiple/dysplastic body nevi, and traumatism on palms/soles). Results: A total of 542 aMPL cases—113 MM and 429 AN—were collected from 195 males and 347 females. No sex prevalence was found for melanomas, while women were found to have relatively more nevi. Melanomas were prevalent on the heel, plantar arch, and fingers in patients aged 65.3 on average, with an average diameter of 17 mm. Atypical nevi were prevalent on the plantar arch and palmar area of patients aged 41.33 on average, with an average diameter of 7 mm. Conclusions: Keeping in mind the risk profile of an aMPL patient can help obtain a timely differentiation between malignant/benign cases, thus avoiding delayed and inappropriate excision, respectively, with the latter often causing discomfort/dysfunctional scarring, especially at acral sites.

Abstract:T the editor: A 28-year-old Chinese woman was referred for the evaluation of whitish macules. The macules firstly appeared in left upper back three years ago and then spread to left limb slowly but steadily in size. After the first gravidity at age of 26 years, the patient observed a marked acceleration of this process. She was previously diagnosed with pityriasis versicolor and was treated with ciclopirox cream and zinc pyrithione shampoo without improvement. There was no history of itching or dysaesthesia. The patient denied recreational sun exposure in past five years. When exposed to sunlight, the whitish macules did not change in the color whereas the non-affected skin became more tanned, thus the contrast against the whitish macules skin was remarkably intensified. There is no similar individual in her family. Skin findings of this patient consisted of scattered, partial confluent, non-scaly, well-defined, round or oval hypopigmented macules which asymmetrically distributed on left upper back and left limb (Figure 1A and 1C), and no similar lesions on the other side of the body (Figure 1B). No signs of inflammation were present in these macules.

Abstract:Background: The differential diagnosis of atypical melanocytic skin lesions localized on palms and soles represents a diagnostic challenge: indeed, this spectrum encompasses atypical nevi (AN) and early-stage melanomas (EN) displaying overlapping clinical and dermoscopic features. This often generates unnecessary excisions or delayed diagnosis. Investigations to date were mostly carried out in specific populations, focusing either on acrolentiginous melanomas or morphologically typical acquired nevi. Aims: To investigate the dermoscopic features of atypical melanocytic palmoplantar skin lesions (aMPPLs) as evaluated by variously skilled dermatologists and assess their concordance; to investigate the variations in dermoscopic appearance according to precise location on palms and soles; to detect the features with the strongest association with malignancy/benignity in each specific site. Methods: A dataset of 471 aMPPLs—excised in the suspect of malignancy—was collected from 10 European Centers, including a standardized dermoscopic picture (17×) and lesion/patient metadata. An anatomical classification into 17 subareas was considered, along with an anatomo-functional classification considering pressure/friction, (4 macroareas). A total of 156 participants (95 with less than 5 years of experience in dermoscopy and 61 with ≥than 5 years) from 17 countries performed a blinded tele-dermoscopic pattern analysis over 20 cases through a specifically realized web platform. Results: A total of 37,440 dermoscopic evaluations were obtained over 94 (20%) EM and 377 (80%) AN. The areas with the highest density of EM compared to AN were the heel (40.3% EM/aMPPLs) of the sole and the "fingers area" (33%EM/aMPPLs) of the palm, both characterized by intense/chronic traumatism/friction. Globally, the recognition rates of 12 dermoscopic patterns were non statistically different between 95 dermatology residents and 61 specialists: aMPPLs in the plantar arch appeared to be the most "difficult" to diagnose, the parallel ridge pattern was poorly recognized and irregular/regular fibrillar patterns often misinterpreted. Regarding the aMPPL of the "heel area", the parallel furrow pattern (p = 0.014) and lattice-like pattern (p = 0.001) significantly discriminated benign cases, while asymmetry of colors (p = 0.002) and regression structures (p = 0.025) malignant ones. In aMPPLs of the "plantar arch", the lattice-like pattern (p = 0.012) was significant for benignity and asymmetry of structures, asymmetry of colors, regression structures, or blue-white veil for malignancy. In palmar lesions, no data were significant in the discrimination between malignant and benign aMPPLs. Conclusions: This study highlights that (i) the pattern analysis of aMPPLs is challenging for both experienced and novice dermoscopists; (ii) the histological distribution varies according to the anatomo-functional classification; and (iii) different dermoscopic patterns are able to discriminate malignant from benign aMPPLs within specific plantar and palmar areas.
