A retrospective real-world study on the safety and efficacy of budesonide orodispersible tablets for the induction therapy of eosinophilic oesophagitis

Abstract:Background: An orodispersible form of budesonide has recently been approved for the targeted treatment of eosinophilic oesophagitis in the United Kingdom, Europe, Australia, Canada and the United States, following favourable results from a randomised controlled trial. This is the first dedicated real-world study exploring the safety and efficacy of budesonide orodispersible tablets for induction therapy in the treatment of eosinophilic oesophagitis while providing insights into its management. Objectives: The primary objective was histologic remission, defined as less than 5 eosinophils per high-powered field. The secondary objectives included histologic response (>50% reduction in peak eosinophil count), clinical remission (complete resolution of symptoms documented on clinic letters), clinical response (improvements in symptoms as reported on clinical letters), endoscopic remission (Endoscopic Reference Score (EREFS) score = 0), and endoscopic response (improvement in EREFS score). The EREFS scores were calculated based on the severity and presence of rings, longitudinal furrows, strictures, oedema and exudates on endoscopic images. Adverse events and safety profiles were also recorded. Design: A multicentre cohort study examining the effectiveness of 1 mg, twice daily, budesonide orodispersible tablet induction therapy for the treatment of eosinophilic oesophagitis. Methods: Ethics approval was obtained through the Western Australia Health: Governance, Evidence, Knowledge, Outcomes system for assessment of Audit and Quality Activities. The study adhered to the Strengthening the Reporting of Observational Studies in Epidemiology guidelines. Results: A total of 43 patients (29 males, 14 females; median age 39) were recruited. Forty-one patients were included in the analysis. After induction therapy, 30 patients (73%) achieved histologic remission, and 35 patients (85%) demonstrated histologic response. Thirty-nine patients (95%) achieved clinical response, and 28 patients (68%) achieved clinical remission. An endoscopic response was seen in 37 patients (90%), and 16 patients (39%) achieved endoscopic remission. No significant adverse events were identified. Conclusion: Budesonide orodispersible tablet is an effective induction therapy for eosinophilic oesophagitis, as evidenced by its high histologic remission rate and favourable safety profile.

Abstract:Eosinophilic Esophagitis is an antigen-mediated inflammatory disease characterized by thickening of the esophageal wall, leading to dysphagia, vomiting, reflux, and abdominal pain. This disease can be treated with a therapeutic approach ranging from diet to pharmacological therapy. Jorveza® (budesonide) and Dupixent® (dupilumab) are treatments for Eosinophilic Esophagitis approved by the European Medicines Agency in adults but not in children. Budesonide-based extemporaneous oral liquid suspensions could be prepared for pediatric use. The main limit of this formulation is that budesonide needs a longer residence time on the esophageal mucosa to solubilize and diffuse in it to exert its local anti-inflammatory effect. Herein, we propose the development of an extemporaneous mucoadhesive oral budesonide solution for the pediatric population. A liquid vehicle containing hydroxypropyl-beta-cyclodextrin as a complexing agent and carboxymethylcellulose sodium as a mucoadhesive excipient was used to prepare budesonide-based formulations. A stable solution at a concentration of 0.7 mg/mL was successfully prepared and characterized. The formulation showed rheological and mucoadhesive properties suitable for an Eosinophilic Esophagitis local prolonged treatment. In this way, pharmacists can prepare stable budesonide-based mucoadhesive solutions, providing both patients and physicians with a new therapeutic option for Eosinophilic Esophagitis pediatric treatment.

Abstract:Eosinophilic esophagitis (EoE) is a chronic inflammatory disorder of the esophagus characterized by symptoms of esophageal dysfunction and eosinophil-predominant inflammation. The incidence of EoE has increased substantially over the past two decades in Europe and North America. The natural course of EoE appears to be progressive with a high risk of stricture formation. The current European guideline recommend swallowed topical corticosteroids, proton-pump inhibitors or dietary intervention for initial and long-term treatment of EoE. Swallowed topical corticosteroids can be considered to be the best studied drug class in EoE, with more than 1000 patients enrolled in randomized clinical trials worldwide. In most of them, fluticasone or budesonide formulations have been used that were originally designed for asthma therapy, thus presumably suboptimal for EoE treatment. The new orodispersible budesonide tablet with effervescent properties is the first approved esophageal-targeted formulation specifically developed for the treatment of EoE, which has become available in many European countries. This article gives an overview of the evolution of topical corticosteroids in EoE and provides an update on recent data from large-scale multicenter trials exploring the efficacy and safety of the orodispersible budesonide tablet with effervescent properties in adult EoE patients.

Abstract:Eosinophilic esophagitis (EoE) is a chronic, local immune-mediated inflammatory oesophageal disease. Although Budesonide is recommended as one of the first-line drugs for EoE treatment, its efficacy is still controversial in multiple studies. Due to the continuous emergence of new and reliable research evidence in recent years, we updated the meta-analysis using RCT trial results to evaluate the efficacy and safety of budesonide. Retrieve the data of the randomised controlled trial literature from 2000 to June 20, 2021, on using Budesonide in the treatment of eosinophilic esophagitis from the three major databases. Based on the results achieved with the Cochrane risk assessment tool, evaluate the quality of the included literature to extract the data, and perform the Meta-analysis with RevMan5.4 and Stata15.0. A total of 958 articles were retrieved, with 10 articles finally included, thus forming a sample size of 712 cases. The main outcome indicators of the meta-analysis are as follows: (1) Histological remission: the Budesonide group performs better than the placebo control group when it comes to histological remission of injuries [RR = 23.82, 95%CI = (13.46, 42.21), p < .001]; (2) Eosinophil count: the Budesonide group is superior to the control group in terms of reduced eosinophil count [SMD = −1.34, 95%CI = (−1.52, −1.15), p < .001]. More and more high-quality randomised controlled trials show that oral budesonide in the treatment of eosinophils esophagitis was better than the placebo group. Mounting high-quality RCTs have confirmed the efficacy of oral budesonide in the treatment of eosinophilic esophagitis and that the effects of this drug may not be so dose-dependent. It is safe to take budesonide for a long time, and this drug is a relatively ideal option for drug treatment of eosinophilic esophagitis at present, so it is worthy of clinical application. We used high-quality randomised controlled trials to meta-update the previous results to further confirm the clinical efficacy and safety of budesonide. Oral budesonide in the treatment of eosinophilic esophagitis is significantly better than the placebo control group. We have confirmed the value of its clinical application and promotion by including more high-quality randomised controlled trials. We also found that the efficacy of budesonide in patients is not dose-dependent, and more research is needed to confirm this.

Abstract:INTRODUCTION: The effect of esophageal dilation history on efficacy outcomes in patients with eosinophilic esophagitis (EoE) receiving swallowed corticosteroids is not well established. METHODS: This post hoc analysis assessed data from a 12-week, randomized, double-blind, placebo-controlled phase 3 study (NCT02605837) of budesonide oral suspension (BOS) 2.0 mg twice daily in patients aged 11–55 years with EoE and dysphagia. Co-primary efficacy outcomes were histologic (≤6 eosinophils per high-power field [eos/hpf]) and dysphagia symptom (≥30% reduction in Dysphagia Symptom Questionnaire [DSQ] scores from baseline) responses at week 12. Secondary efficacy outcomes included histologic response (<15 eos/hpf) and change from baseline to week 12 in DSQ scores and EoE Endoscopic Reference Scores (EREFS). Data were analyzed post hoc by esophageal dilation history (dilation history vs no dilation history). RESULTS: Of 318 patients who received ≥1 dose of study drug, 42.8% had a history of esophageal dilation (dilation history: BOS, n=91; placebo, n=45; no dilation history: BOS, n=122; placebo, n=60). Histologic responses (≤6 and <15 eos/hpf) were similar regardless of dilation history. Fewer BOS-treated patients with dilation history than no dilation history achieved a dysphagia symptom response (44.0% vs 59.0%); conversely a slightly greater improvement from baseline in total EREFS was observed for BOS-treated patients with dilation history than no dilation history (LS mean [standard error of the mean]: −4.1 [0.3] vs −3.4 [0.4]). CONCLUSIONS: Esophageal dilation history may confound the association between histologic outcomes and dysphagia symptom or endoscopic efficacy outcomes in patients with EoE receiving swallowed corticosteroids.

Abstract:Eosinophilic esophagitis (EoE) is a chronic Th2-mediated inflammatory disease of the esophagus driven by dietary or inhalant allergens which if left untreated, leads to fibrosis and poor esophageal function. Although the inflammation in the esophagus is dominated by eosinophils, there are also elevated levels of T and B cells. Blood samples from ten patients with EoE before and after treatment with orodispersible budesonide and 10 healthy controls were compared using cytometry by time-of-flight. An antibody panel was designed that covers the major immunological cell populations with a particular focus on eosinophils. The data was analyzed with multivariate methods and cluster analysis. Correlation analysis was done between immune markers and endoscopic, histological, and symptomatologic assessments. Our analysis revealed that patients with EoE had lower levels of effector memory T cells after treatment with orodispersible budesonide to the same level as healthy subjects. In addition, more suppressive eosinophils were present in the circulation of EoE patients before treatment and more immature eosinophils were present after treatment. Furthermore, levels of galectin-10+ eosinophils correlated with histological findings in esophageal tissue from EoE patients. In all patients, the peak eosinophils were decreased after treatment with orodispersible budesonide. Intriguingly, 90% of the patients had remission in the histological assessment and 50% improved in the endoscopic assessment. This study reports a detailed immune profile in patients with EoE before and after treatment with orodispersible budesonide and it is a step toward finding blood-based immune parameters that could be useful to monitor response to treatment.

Abstract:Eosinophilic esophagitis (EoE) is a chronic, progressive disease, in which type 2 inflammation drives pathologic changes in esophageal tissue. Pathologic disease activity and severity can be assessed using peak intraepithelial eosinophil count (PEC). Additionally, the EoE Histologic Scoring System (EoE-HSS) is a validated measure scoring severity of changes (grade) and extent of pathology (stage) of 8 components. Dupilumab, a fully human monoclonal antibody that blocks key drivers of type 2 inflammation, interleukin (IL)-4 and IL-13, is approved in the USA for the treatment of patients with EoE aged ≥1 year, weighing ≥15 kg. This analysis assessed the impact of dupilumab vs placebo on PEC and EoE-HSS grade/stage scores, in pediatric patients aged 1 to < 12 years with active EoE in the phase 3 EoE KIDS trial (NCT04394351).

Abstract:Background/Objectives: Paediatric eosinophilic oesophagitis (EoE) treatment is challenging due to the limited number of age-appropriate formulations. This study aims to develop and evaluate oral viscous suspensions and solid formulations of budesonide (BUD), focusing on their in vitro mucoadhesive properties, to enhance drug delivery and therapeutic outcomes in paediatric EoE. Methods: This study encompasses the development of oral viscous suspensions and orodispersible solid formulations (moulded tablets and 3D-printed dosage forms) containing BUD. The formulations underwent quality control tests as per the European Pharmacopoeia, chemical stability assessments, and an in vitro evaluation of their mucoadhesiveness properties. Results: A validated analytical method enabled accurate BUD quantification and efficient extraction, and all developed formulations demonstrated chemical stability for 30 days, meeting Ph. Eur. quality standards. Three-dimensional printing using SSE successfully produced 1 mg and 0.5 mg BUD printlets, complying with quality tests for conventional tablets. Formulations containing xanthan gum (L2-XG and P1-0.5-XG) exhibited superior mucoadhesive properties. L2-XG showed significantly higher mucoadhesion than L1-MC. Among the solid formulations, P1-0.5-XG demonstrated the highest mucoadhesive properties. Conclusions: This is the first study to develop solid oral dosage forms of BUD at a very low dose, specifically for paediatric use. The results highlight the potential of 3D printing for developing individualised orodispersible BUD formulations with improved bioadhesion for paediatric EoE treatment. The L2-XG formulation and the XG-containing printlets are the most promising formulations in terms of increasing contact time with the oesophageal mucosa, which could translate into improved therapeutic efficacy in this patient population.

Clinical Efficacy of Spleen Aminopeptide Combined with Budesonide and Levosalbutamol Hydrochloride, and Its Effects on Inflammatory Markers and Immune Function in Pediatric Bronchial Asthma

Abstract:Background: Bronchial asthma (BA) is a heterogeneous disease characterized primarily by chronic airway inflammation and airway hyperresponsiveness (AHR). Objectives: This was a retrospective study aimed at exploring the clinical efficacy of spleen aminopeptide (SA) combined with budesonide (BUD) and levosalbutamol hydrochloride (LEV) as a therapy for pediatric BA, and to observe its effects on inflammatory markers and immune function. Methods: From June 2022 to June 2023, eighty individuals diagnosed with BA were divided into two groups: A control group and an observation group, based on different therapeutic methods. The control group received inhalation therapy with nebulized BUD and LEV, while the observation group received the same inhalation therapy as the control group, along with SA oral lyophilized powder (SAOLP) treatment. A comparative analysis was conducted between the two groups, evaluating clinical efficacy, changes in inflammatory markers and immunological parameters, and adverse drug reactions (ADRs). Results: Upon treatment completion, the observation group exhibited an overall response rate (ORR) of 95.00%, whereas the control group showed an ORR of 80.00% (P < 0.05). Additionally, the observation group had a significantly shorter length of hospital stay (LoHS) and a more rapid improvement in symptoms compared to the control group (both P < 0.05). Following treatment, both groups demonstrated significant decreases in interleukin‐2 (IL‐2), interleukin‐4 (IL‐4), interleukin‐6 (IL‐6), interleukin‐8 (IL‐8), and tumor necrosis factor‐α (TNF‐α) levels (all P < 0.05). However, these reductions were more pronounced in the observation group than in the control group (all P < 0.05). Similarly, after treatment, both groups showed significant increases in T cell subsets CD3+ and CD4+, the CD4+/CD8+ ratio, and levels of immunoglobulin M (IgM), immunoglobulin A (IgA), and immunoglobulin G (IgG) (all P < 0.05), with these changes being more evident in the observation group than in the control group (all P < 0.05). Both groups experienced adverse reactions during the treatment, with an overall adverse reaction rate (OADRR) of 7.50% in the observation group and 10.00% in the control group (χ2 = 0.157, P = 0.692). Conclusions: The combined therapy using SA, BUD, and LEV can significantly improve immune function and reduce inflammatory markers, thereby achieving satisfactory efficacy in treating pediatric BA with a clinically safe profile.

Abstract:Eosinophilic esophagitis (EoE) is a chronic inflammatory disease of the esophagus that is immune/antigen-mediated and often requires targeted treatment. In clinical practice, an oral viscous budesonide suspension prepared by adding sucralose to a budesonide suspension for inhalation (Pulmicort®) is used to treat adult EoE and enhance retention in the esophageal mucosa. Inspired by this off-label drug use, oral viscous budesonide solutions (OVBSs) were developed in this study, and their capacities for adhesion, permeation, and stability were explored. Given the insolubility of budesonide as a BCS II drug, we first evaluated its equilibrium solubility and found that Transcutol® HP was an excellent choice for creating an OVBS at a concentration of 0.2 mg/g. The rheological properties of the OVBSs were evaluated with a rheometer, and shear-thinning, which aids in swallowing, was observed. The addition of hydroxyethyl cellulose (HEC) increased the adhesion strength of the preparation, which was associated with the hydration and thickening mechanism. This result was confirmed in a dynamic gelation study and in vitro elution experiment conducted with porcine esophagus tissue. Furthermore, the permeabilities of the OVBSs in the porcine esophagus were evaluated with a Franz diffusion cell device. More than 80% of the budesonide was released after 24 hours, and the release profile was similar to that of the solution. To explore the storage conditions of OVBSs, critical factors such as pH, content, and impurities were determined. It was found that OVBSs exhibited different behaviors at different pH values and temperatures. Notably, the OVBSs containing 1.7% HEC could be stored for more than 6 months at a temperature of 5°C ± 3°C and a pH of 4.5 without significant degradation. Overall, this study demonstrated that OVBSs have the potential to adhere to the esophageal mucosa, permeate the tissue, and remain stable during storage. Moreover, OVBSs exhibit a distinct advantage over traditional converted inhalation-to-oral budesonide therapies by enabling flexible dose adjustment in clinical applications, thereby potentially minimizing systemic side effects commonly associated with oral glucocorticoid administration.

Abstract:Eosinophilic gastrointestinal disorders (EGIDs) are characterized by GI tract dysfunction (dysphagia, vomiting, gastro-esophageal reflux, abdominal pain, diarrhea, etc.) and findings of increased tissue eosinophils on biopsy. These disorders can affect any segment of the GI tract, and the best characterized is eosinophilic esophagitis (EoE). Originally described as a distinct entity in the 1990s, EoE is now recognized worldwide with defined clinical, endoscopic, and histologic diagnostic criteria as well as widely accepted guidelines for management [ 1 ]. Treatment options include food elimination diets, which focus on removal of the triggering food(s), or use of medications, to induce disease remission. There are generally three categories of medications utilized: proton-pump inhibitors (PPIs), swallowed, topical corticosteroids (TCS), and biologics. In EoE, PPIs function as anti-inflammatory agents by decreasing the expression of Th2 (allergic) cytokines and improving the disrupted barrier function of the esophagus. Swallowed, topical corticosteroids and many biologics function in a similar way: downregulation of Th2 cytokines, repair of esophageal barrier function, and prevention of tissue remodeling. In trials of PPIs, between 20% and 50% of patients demonstrate clinical and histologic improvement with high-dose monotherapy and the majority maintain this with long-term use [ 2 ]. PPIs are positioned among the first-line treatments for EoE, although they are not approved by the FDA in the United States for this indication. TCS have undergone the most extensive evaluation of any therapeutic for EoE. There are currently two formulations utilized in the United States: budesonide, which is mixed into a 'slurry,' and fluticasone propionate, which is administered as an aerosol through an MDI device. Both formulations are swallowed, providing a topical effect in the esophagus and each is effective for both induction of clinical and histological remission of EoE as well as long-term maintenance therapy [ 3 ]. Biologics have garnered much recent attention, likely due to their success in a variety of other disorders including atopic diseases such as asthma and atopic dermatitis.

Budesonide Suspension for Inhalation Ease Analysis of Patients With Acute Attack of Bronchial Asthma in Adult

Abstract:Objective To observe the Budesonide Suspension for Inhalation acute clinical efficacy of adult bronchial asthma remission.Methods 60 cases of patients with acute phase diagnosed as bronchial asthma from February 2013 to February 2015 in our hospital,randomly divided into two groups.The control group with normal oral prednisone with salbutamol therapy,observation group with inhalation with cloth to Ned suspension liquid atomization inhalation combined albuterol.Compare two groups after treatment in patients with clinical efficacy.Results Observation curative effect is better than that of control group(χ2=6.51,P0.05).Conclusion Budesonide oral prednisone dragon better treatment effect of adult acute bronchial asthma.

Abstract:Objective To evaluate the efficacy of Budesonide/formoterol to control asthma under real-life conditions.Methods A multi-center,open label,non-interventional study was conducted.Asthma control after 12 week therapy with Budesonide/formoterol was assessed by Asthma Control Questionnaire(ACQ) and modified Asthma Control Questionnaire(ACQ5).Results A total of 360 asthma patients were recruited,including 228 adult patients and 132 child patients.After 12 weeks' therapy,all the patients' medium value of ACQ was decreased significantly from 2.03(adults 2.20,children 1.74) at baseline to 0.60(adults 0.78,children 0.29)(P<0.0001),and the medium value of ACQ5 was also decreased significantly from 2.4(adults 2.24,children 1.76) at baseline to 0.47(adults 0.62,children 0.20)(P<0.0001).Conclusion Budesonide/formoterol is effective in asthma treatment,by which most asthma patients obtain and maintain clincal control.
Salbutamol combined with budesonide in treatment of pediatric bronchial asthma and its effect on eosinophils

Abstract:Background: The aim of this study was to investigate and discuss the salbutamol combined with budesonide in treatment of pediatric bronchial asthma (BA) and its effect on eosinophils (EOS). Methods: Ninety-eight BA children admitted and treated in our hospital from July 2016 to June 2017 were collected and divided into control group (N.=49) and observation group (N.=49) according to random number table. The children in control group were treated with budesonide and those in observation group were treated with salbutamol combined with budesonide. The clinical efficacy, pulmonary functions and levels of T-lymphocyte subsets (including cluster of differentiation 3 (CD3)+, CD4+, CD8+ and CD4+/CD8+) in the immune system between two groups were compared after the treatment; the levels of eosinophil cationic protein (ECP) and eotaxin in the children were compared before the treatment and at 1, 4 and 8 weeks after the treatment; the changes in EOS counts in blood and induced sputum of the children before and after the treatment were compared, and the EOS apoptosis rate was compared at 1, 4 and 8 weeks after the treatment. Results: The effective rate of treatment in observation group was significantly higher than that in control group (P<0.05). After the treatment, the indexes of pulmonary function in observation group were obviously better than those in control group (P<0.05). Compared with those in control group, the levels of CD3+, CD4+ and CD4+/CD8+ of the children in observation group were elevated remarkably, while the CD8+ level was lowered (P<0.05). The levels of ECP and eotaxin in the two groups were decreased after the treatment compared with those before the treatment, and the levels in observation group were superior to those in control group (P<0.05). After the treatment, the EOS counts of both groups of children were lower than those before the treatment, and the decrease in observation group was more notable than that in control group. At 1, 4 and 8 weeks after the treatment, the EOS apoptosis rate in observation group was obviously higher than that in control group (P<0.05). Conclusions: The treatment of salbutamol combined with budesonide for pediatric BA has significant therapeutic effects; it can restore the pulmonary functions rapidly and improve the immunity of the lung, reduce the levels of eotaxin, ECP and EOS of the child patients and promote EOS apoptosis.

Efficacy of nebulized budesonide compared to oral prednisolone in acute bronchial asthma

Abstract:To evaluate the efficacy of nebulized budesonide compared to oral prednisolone early in the emergency room management of acute asthma, we conducted a double-blind, placebo-controlled trial. Eighty children, 2 years to 12 years of age, with acute moderate attacks of asthma, were randomized into two groups. One group received nebulized salbutamol (0.15 mg/kg) and placebo at half-hourly intervals for three doses, and a single dose of oral prednisolone (2 mg/kg) (prednisolone group) and other group received three doses of nebulized salbutamol and budesonide (800 microg) at half-hourly intervals and a single dose of placebo tablets (budesonide group). The baseline characteristics of the two groups were similar, but after three doses of nebulization oxygen saturation, respiratory rate, pulmonary index and respiratory distress score were significantly improved in the budesonide group compared to prednisolone group (p < 0.01). The proportion of patients who were fit for discharge at the end of 2 h after the third dose of nebulization was significantly higher in the budesonide group than in the prednisolone group (22/ 41, 54% vs 7/39, 18%, p < 0.001). The data suggest that a combination of nebulized salbutamol and budesonide should be preferred in the emergency room management of children with acute moderate to severe exacerbation of asthma and who are not on prior oral or inhaled steroid therapy.

Preliminary evidence in treatment of eosinophilic gastroenteritis in children: A case series

Abstract:BACKGROUND: Eosinophilic gastroenteritis is a rare inflammatory disorder in children. However, there is still no standard guideline in the treatment of pediatric eosinophilic gastroenteritis.AIM: To report our experience with the diagnosis and treatment of children with eosinophilic gastroenteritis.METHODS: From January 2017 to December 2019, a total of 22 children were diagnosed with eosinophilic gastroenteritis.RESULTS: Endoscopic examination showed eosinophil infiltration in the duodenum [mean number of eosinophils/high-power field (HPF) = 53.1 ± 81.5], stomach (mean number of eosinophils/HPF = 36.8 ± 50.5), and terminal ileum (mean number of eosinophils/HPF = 49.0 ± 24.0). All 18 children with low eosinophil infiltration (< 14%) responded well to the initial drug treatment without relapse, while two of four children with high eosinophil infiltration (> 14%) relapsed after initial methylprednisolone/montelukast treatment. In addition, children with high eosinophil infiltration (> 14%) showed symptomatic relief and histological remission without further relapse after receiving budesonide/methylprednisolone as initial or relapse treatment.CONCLUSION: Methylprednisolone/montelukast is still the best treatment for children with low eosinophil infiltration (< 14%). Budesonide can be considered as the initial or relapse treatment for children with high eosinophil infiltration (> 14%).
