Abstract:Mechanical ventilation is an essential life-support treatment for patients who cannot breathe independently. Patient–ventilator asynchrony (PVA) occurs when ventilatory support does not match the needs of the patient and is associated with a series of adverse clinical outcomes. Deep learning methods have shown a strong discriminative ability for PVA detection, but they require a large number of annotated data for model training, which hampers their application to this task. We developed a transfer learning architecture based on pretrained convolutional neural networks (CNN) and used it for PVA recognition based on small datasets. The one-dimensional signal was converted to a two-dimensional image, and features were extracted by the CNN using pretrained weights for classification. A partial dropping cross-validation technique was developed to evaluate model performance on small datasets. When using large datasets, the performance of the proposed method was similar to that of non-transfer learning methods. However, when the amount of data was reduced to 1%, the accuracy of transfer learning was approximately 90%, whereas the accuracy of the non-transfer learning was less than 80%. The findings suggest that the proposed transfer learning method can obtain satisfactory accuracies for PVA detection when using small datasets. Such a method can promote the application of deep learning to detect more types of PVA under various ventilation modes.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background and objective</h3><p>Mismatch between invasive mechanical ventilation and the requirements of patients results in patient-ventilator asynchrony (PVA), which is associated with a series of adverse clinical outcomes. Although the efficiency of the available approaches for automatically detecting various types of PVA from the ventilator waveforms is unsatisfactory, the feasibility of powerful deep learning approaches in addressing this problem has not been investigated.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We propose a 2-layer long short-term memory (LSTM) network to detect two most frequently encountered types of PVA, namely, double triggering (DT) and ineffective inspiratory effort during expiration (IEE), on two datasets. The performance of the networks is evaluated first using cross-validation on the combined dataset, and then using a cross testing scheme, in which the LSTM networks are established on one dataset and tested on the other.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Compared with the reported rule-based algorithms and the machine learning models, the proposed 2-layer LSTM network exhibits the best overall performance, with the F1 scores of 0.983 and 0.979 for DT and IEE detection, respectively, on the combined dataset. Furthermore, it outperforms the other approaches in cross testing.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>The findings suggest that LSTM is an excellent technique for accurate recognition of PVA in clinics. Such a technique can help detect and correct PVA for a better patient ventilator interaction.</p>

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background and Objective</h3><p>: Patient-ventilator asynchrony (PVA) is the result of a mismatch between the need of patients and the assistance provided by the ventilator during mechanical ventilation. Because the poor interaction between the patient and the ventilator is associated with inferior clinical outcomes, effort should be made to identify and correct their occurrence. Deep learning has shown promising ability in PVA detection; however, lack of network interpretability hampers its application in clinic.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>: We proposed an interpretable one-dimensional convolutional neural network (1DCNN) to detect four most manifestation types of PVA (double triggering, ineffective efforts during expiration, premature cycling and delayed cycling) under pressure control ventilation mode and pressure support ventilation mode. A global average pooling (GAP) layer was incorporated with the 1DCNN model to highlight the sections of the respiratory waveform the model focused on when making a classification. Dilation convolution and batch normalization were introduced to the 1DCNN model for compensating the reduction of performance caused by the GAP layer.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>: The proposed interpretable 1DCNN exhibited comparable performance with the state-of-the-art deep learning model in PVA detection. The F1 scores for the detection of four types of PVA under pressure control ventilation and pressure support ventilation modes were greater than 0.96. The critical sections of the waveform used to detect PVA were highlighted, and found to be well consistent with the understanding of the respective type of PVA by experts.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>: The findings suggest that the proposed 1DCNN can help detect PVA, and enhance the interpretability of the classification process to help clinicians better understand the results obtained from deep learning technology.</p>

Early Detection and Classification of Patient-Ventilator Asynchrony Using Machine Learning.

Abstract:During mechanical ventilation, a common problem known as patient-ventilator asynchrony (PVA) occurs when there is a mismatch between the needs of the patient's breathing and the breath cycle delivered by the ventilator. PVA is problematic because it can be associated with adverse effects such as discomfort for the patient, increased work of breathing, longer mechanical ventilation duration and ventilator-induced lung injury. An automated means of early PVA detection and classification could lead to improved health outcomes and help reduce the impact of PVA on hospital resources. This paper presents a machine learning framework to detect PVA events using only the first half second of data after the start of a PVA event. When trained on more than 5000 PVA events sampled from 25 subjects, our logistic classifier achieves a sensitivity (specificity) of 99.81% (99.72%) for detecting PVA events. We then present a system capable of early classification of Ineffective Effort (IE) and Double Trigger (DT) events, which achieves a sensitivity (specificity) of 63.73% (92.88%). By demonstrating the feasibility of early PVA event detection and classification, our findings suggest that more effective intervention processes could be possible, including automated interventions with different response strategies for different PVA event types.

Abstract:This study summarizes the application of automatic recognition technologies for patient-ventilator asynchrony (PVA) during mechanical ventilation. In the early stages, the method of setting rules and thresholds relied on manual interpretation of ventilator parameters and waveforms. While these methods were intuitive and easy to operate, they were relatively sensitive in threshold setting and rule selection and could not adapt well to minor changes in patient status. Subsequently, machine learning and deep learning technologies began to emerge and develop. These technologies automatically extract and learn data characteristics through algorithms, making PVA detection more robust and universal. Among them, logistic regression, support vector machines, random forest, hidden Markov models, convolutional autoencoders, long short-term memory networks, one-dimensional convolutional neural networks, etc., have all been successfully used for PVA recognition. Despite the significant advancements in feature extraction through deep learning methods, their demand for labelled data is high, potentially consuming significant medical resources. Therefore, the combination of reinforcement learning and self-supervised learning may be a viable solution. In addition, most algorithm validations are based on a single dataset, so the need for cross-dataset validation in the future will be an important and challenging direction for development.
Automatic classification of patient-ventilator asynchronies with a 1D convolutional neural network: a pilot study

Abstract:Critically ill patients often experience a mismatch between breathing efforts and mechanical ventilation, leading to adverse outcomes. However, patient-ventilator asynchronies (PVAs) go underdiagnosed because traditional bedside visual evaluation of respiratory waveforms is subjective and cannot be consistently performed. This study aims to develop a robust, automatic deep-learning method for continuous monitoring of respiratory waveforms and PVA classification. Retrospective analysis was conducted on 1-hour intervals of ventilation waveforms (pressure, flow, and volume curves) from 20 intensive care unit (ICU) patients at Humanitas Research Hospital (median age: 61.5 years; 7 women). Events (n=4402) were manually categorized as normal breaths (1008) or one of six PVA classes (flow starvation, delayed cycling, leak, auto-trigger, ineffective effort, double trigger). Classification models, including Random Forest, XGBoost, and a 1D deep convolutional neural network (CNN), were trained using Python. The 1D CNN emerged as the best-performing model in testing, with a weighted average precision, sensitivity, and F1 score of 0.885, 0.880, and 0.880, respectively. Class-specific performance varied, with the highest sensitivity in normal breath (99.0%) and auto trigger (91.3%) classification. Receiver operating characteristic (ROC) curves yielded an average area under the curve (AUC) of 0.98 (max 1.00, min 0.96). These preliminary results highlight the effectiveness of our model in classifying PVAs, showing significant potential for real-time and automated PVA detection in the ICU.

Abstract:Background and objective: Patient-ventilator asynchrony (PVA) is associated with poor clinical outcomes and remains under-monitored. Automated PVA detection would enable complete monitoring standard observational methods do not allow. While model-based and machine learning PVA approaches exist, they have variable performance and can miss specific PVA events. This study compares a model and rule-based algorithm with a machine learning PVA method by retrospectively validating both methods using an independent patient cohort. Methods: Hysteresis loop analysis (HLA) which is a rule-based method (RBM) and a tri-input convolutional neural network (TCNN) machine learning model are used to classify 7 different types of PVA, including: 1) flow asynchrony; 2) reverse triggering; 3) premature cycling; 4) double triggering; 5) delayed cycling; 6) ineffective efforts; and 7) auto triggering. Class activation mapping (CAM) heatmaps visualise sections of respiratory waveforms the TCNN model uses for decision making, improving result interpretability. Both PVA classification methods were used to classify incidence in an independent retrospective clinical cohort of 11 mechanically ventilated patients for validation and performance comparison. Results: Self-validation with the training dataset shows overall better HLA performance (accuracy, sensitivity, specificity: 97.5 %, 96.6 %, 98.1 %) compared to the TCNN model (accuracy, sensitivity, specificity: 89.5 %, 98.3 %, 83.9 %). In this study, the TCNN model demonstrates higher sensitivity in detecting PVA, but HLA was better at identifying non-PVA breathing cycles due to its rule-based nature. While the overall AI identified by both classification methods are very similar, the intra-patient distribution of each PVA type varies between HLA and TCNN. Conclusion: The collective findings underscore the efficacy of both HLA and TCNN in PVA detection, indicating the potential for real-time continuous monitoring of PVA. While ML methods such as TCNN demonstrate good PVA identification performance, it is essential to ensure optimal model architecture and diversity in training data before widespread uptake as standard care. Moving forward, further validation and adoption of RBM methods, such as HLA, offers an effective approach to PVA detection while providing clear distinction into the underlying patterns of PVA, better aligning with clinical needs for transparency, explicability, adaptability and reliability of these emerging tools for clinical care.
Early prediction of need for invasive mechanical ventilation in the neonatal intensive care unit using artificial intelligence and electronic health records: a clinical study

Abstract:Background: Respiratory support is crucial for newborns with underdeveloped lung. The clinical outcomes of patients depend on the clinician's ability to recognize the status underlying the presented symptoms and signs. With the increasing number of high-risk infants, artificial intelligence (AI) should be considered as a tool for personalized neonatal care. Continuous monitoring of vital signs is essential in cardiorespiratory care. In this study, we developed deep learning (DL) prediction models for rapid and accurate detection of mechanical ventilation requirements in neonates using electronic health records (EHR). Methods: We utilized data from the neonatal intensive care unit in a single center, collected between March 3, 2012, and March 4, 2022, including 1,394 patient records used for model development, consisting of 505 and 889 patients with and without invasive mechanical ventilation (IMV) support, respectively. The proposed model architecture includes feature embedding using feature-wise fully connected (FC) layers, followed by three bidirectional long short-term memory (LSTM) layers. Results: A mean gestational age (GA) was 36.61 ± 3.25 weeks, and the mean birth weight was 2,734.01 ± 784.98 g. The IMV group had lower GA, birth weight, and longer hospitalization duration than the non-IMV group (P < 0.05). Our proposed model, tested on a dataset from March 4, 2019, to March 4, 2022. The mean AUROC of our proposed model for IMV support prediction performance demonstrated 0.861 (95%CI, 0.853-0.869). It is superior to conventional approaches, such as newborn early warning score systems (NEWS), Random Forest, and eXtreme gradient boosting (XGBoost) with 0.611 (95%CI, 0.600-0.622), 0.837 (95%CI, 0.828-0.845), and 0.0.831 (95%CI, 0.821-0.845), respectively. The highest AUPRC value is shown in the proposed model at 0.327 (95%CI, 0.308-0.347). The proposed model performed more accurate predictions as gestational age decreased. Additionally, the model exhibited the lowest alarm rate while maintaining the same sensitivity level. Conclusion: Deep learning approaches can help accurately standardize the prediction of invasive mechanical ventilation for neonatal patients and facilitate advanced neonatal care. The results of predictive, recall, and alarm performances of the proposed model outperformed the other models.

Abstract:Patient–ventilator asynchrony (PVA) refers to instances where a mechanical ventilator’s cycles are desynchronised from the patient’s breathing efforts, and may result in patient discomfort and potential ineffective ventilation. Typically, they are identified with constant monitoring by trained clinicians. Such expertise is often limited; therefore, it is desirable to automate PVA detection with machine learning methods. However, there are three major challenges to applying machine learning to the problem: data collected from non-invasive ventilation are often noisy, there exists high variability between patients or between setting changes, and manual annotations of PVA events are not always consistent. To produce meaningful inference from such noisy data, a model needs to not only provide a measure of uncertainty, but also take into account potential inconsistencies in the training signal it is based on. In this work, we propose a conditional latent Gaussian mixture generative classifier with noisy label correction, which is capable of capturing variations within and between classes, providing well-calibrated class probabilities, detecting unlikely input instances that deviates from training data, while also taking into account possible mislabelling of event classes. We show that our model is able to match the performance of a well-tuned gradient boosting classifier, but also produce better calibrated predictions and smaller performance variability between patients.
Application of a Cloud Platform That Identifies Patient-Ventilator Asynchrony and Enables Continuous Monitoring of Mechanical Ventilation in Intensive Care Unit

Abstract:Background: Patient-ventilator asynchrony (PVA) frequently occurs in mechanically ventilated patients within the ICU and has the potential for harm. Depending solely on the health care team cannot accurately and promptly identify PVA. To address this issue, our team has developed a cloud-based platform for monitoring mechanical ventilation (MV), comprising the PVARemoteMonitor system and the 24-h MV analysis report. We conducted a survey to evaluate physicians' satisfaction and acceptance of the platform in 14 ICUs. Methods: Data from medical records, clinical information systems, and ventilators were uploaded to the cloud platform and underwent data processing. The data were analyzed to monitor PVA and displayed in the front-end. The 24-h analysis report for MV was generated for clinical reference. Critical care physicians in 14 hospitals' ICUs that involved in the platform participated in a questionnaire survey, among whom 10 physicians were interviewed to investigate physicians' acceptance and opinions of this system. Results: The PVA-RemoteMonitor system exhibited a high level of specificity in detecting flow insufficiency, premature cycle, delayed cycle, reverse trigger, auto trigger, and overshoot, with sensitivities of 90.31 %, 98.76 %, 99.75 %, 99.97 %, 100 %, and 99.69 %, respectively. The 24-h analysis report supplied essential data about PVA and respiratory mechanics. 86.2 % (75/87) of physicians supported the application of this platform. Conclusions: The PVA-RemoteMonitor system accurately identified PVA, and the MV analysis report provided guidance in controlling PVA. Our platform can effectively assist ICU physicians in the management of ventilated patients.
Large Neural Network Based Detection of Apnea, Bradycardia and Desaturation Events

Abstract:Apnea, bradycardia and desaturation (ABD) events often precede life-threatening events including sepsis in newborn babies. Here, we explore machine learning for detection of ABD events as a binary classification problem. We investigate the use of a large neural network to achieve a good detection performance. To be user friendly, the chosen neural network does not require a high level of parameter tuning. Furthermore, a limited amount of training data is available and the training dataset is unbalanced. Comparing with two widely used state-of-the-art machine learning algorithms, the large neural network is found to be efficient. Even with a limited and unbalanced training data, the large neural network provides a detection performance level that is feasible to use in clinical care.

Quantifiable identification of flow-limited ventilator dyssynchrony with the deformed lung ventilator model

Abstract:Background: Ventilator dyssynchrony (VD) can worsen lung injury and is challenging to detect and quantify due to the complex variability in the dyssynchronous breaths. While machine learning (ML) approaches are useful for automating VD detection from the ventilator waveform data, scalable severity quantification and its association with pathogenesis and ventilator mechanics remain challenging. Objective: We develop a systematic framework to quantify pathophysiological features observed in ventilator waveform signals such that they can be used to create feature-based severity stratification of VD breaths. Methods: A mathematical model was developed to represent the pressure and volume waveforms of individual breaths in a feature-based parametric form. Model estimates of respiratory effort strength were used to assess the severity of flow-limited (FL)-VD breaths compared to normal breaths. A total of 93,007 breath waveforms from 13 patients were analyzed. Results: A novel model-defined continuous severity marker was developed and used to estimate breath phenotypes of FL-VD breaths. The phenotypes had a predictive accuracy of over 97% with respect to the previously developed ML-VD identification algorithm. To understand the incidence of FL-VD breaths and their association with the patient state, these phenotypes were further successfully correlated with ventilator-measured parameters and electronic health records. Conclusion: This work provides a computational pipeline to identify and quantify the severity of FL-VD breaths and paves the way for a large-scale study of VD causes and effects. This approach has direct application to clinical practice and in meaningful knowledge extraction from the ventilator waveform data.
Early Prediction of Ventilator-Associated Pneumonia in ICU Patients Using An Interpretable Machine Learning Algorithm

Abstract:(1) Background: Ventilator-associated pneumonia (VAP) causes high mortality among patients with respiratory disease and imposes major burdens on healthcare infrastructure. Models that use electronic health record data to predict the onset of VAP may spur earlier treatment and improve patient outcomes. We developed and studied the performance of interpretable machine learning (ML) models that predict the onset of VAP from electronic health records (EHRs); (2) Methods: We trained Logistic Regression (LR), full feature Explainable Boosting Machine (fEBM), and eXtreme Gradient Boosting (XGBoost) ML models on data from the MIMIC- III (v1.3) database. Model performance was measured by area under the receiver operating characteristic curves (AUCs). We trained a minimal-feature EBM model (mEBM) with features derived from white blood cell (WBC) counts, duration of ventilation, and Glasgow Coma Scale (GCS). Finally, model robustness was evaluated on randomly sparsified EHR datasets; (3) Results: The fEBM model outperformed the XGBoost and LR models at 24 hours post-intubation. The mEBM model maintained an AUC of 0.893. The fEBM model performance remained robust on sparsified datasets; (4) Conclusions: Our novel interpretable ML algorithm reliably predicts the onset of VAP in intubated patients. Integration of this EBM-based model into clinical practice may enable clinicians to better anticipate and prevent VAP.

Abstract:Ventilator-associated pneumonia (VAP) is the most common and fatal nosocomial infection in intensive care units (ICUs). Existing methods for identifying VAP display low accuracy, and their use may delay antimicrobial therapy. VAP diagnostics derived from machine learning (ML) methods that utilize electronic health record (EHR) data have not yet been explored. The objective of this study is to compare the performance of a variety of ML models trained to predict whether VAP will be diagnosed during the patient stay. A retrospective study examined data from 6126 adult ICU encounters lasting at least 48 hours following the initiation of mechanical ventilation. The gold standard was the presence of a diagnostic code for VAP. Five different ML models were trained to predict VAP 48 hours after initiation of mechanical ventilation. Model performance was evaluated with regard to the area under the receiver operating characteristic (AUROC) curve on a 20% hold-out test set. Feature importance was measured in terms of Shapley values. The highest performing model achieved an AUROC value of 0.854. The most important features for the best-performing model were the length of time on mechanical ventilation, the presence of antibiotics, sputum test frequency, and the most recent Glasgow Coma Scale assessment. Supervised ML using patient EHR data is promising for VAP diagnosis and warrants further validation. This tool has the potential to aid the timely diagnosis of VAP.

Abstract:Lung ultrasound imaging is reaching growing interest from the scientific community. On one side, thanks to its harmlessness and high descriptive power, this kind of diagnostic imaging has been largely adopted in sensitive applications, like the diagnosis and follow-up of preterm newborns in neonatal intensive care units. On the other side, state-of-the-art image analysis and pattern recognition approaches have recently proven their ability to fully exploit the rich information contained in these data, making them attractive for the research community. In this work, we present a thorough analysis of recent deep learning networks and training strategies carried out on a vast and challenging multicenter dataset comprising 87 patients with different diseases and gestational ages. These approaches are employed to assess the lung respiratory status from ultrasound images and are evaluated against a reference marker. The conducted analysis sheds some light on this problem by showing the critical points that can mislead the training procedure and proposes some adaptations to the specific data and task. The achieved results sensibly outperform those obtained by a previous work, which is based on textural features, and narrow the gap with the visual score predicted by the human experts.
Detecting central apneas using multichannel signals in premature infants

Abstract:Objective: Monitoring of apnea of prematurity, performed in neonatal intensive care units by detecting central apneas (CAs) in the respiratory traces, is characterized by a high number of false alarms. A two-step approach consisting of a threshold-based apneic event detection algorithm followed by a machine learning model was recently presented in literature aiming to improve CA detection. However, since this is characterized by high complexity and low precision, we developed a new direct approach that only consists of a detection model based on machine learning directly working with multichannel signals. Approach: The dataset used in this study consisted of 48 hours of ECG, chest impedance and peripheral oxygen saturation extracted from 10 premature infants. CAs were labeled by two clinical experts. 47 features were extracted from time series using 30-s moving windows with an overlap of 5 s and evaluated in sets of 4 consecutive moving windows, in a similar way to what was indicated for the two-step approach. An undersampling method was used to reduce imbalance in the training set while aiming at increasing precision. A detection model using logistic regression with elastic net penalty and leave-one-patient-out cross-validation was then tested on the full dataset. Main results: This detection model returned a mean AUROC value equal to 0.86 and, after the selection of a FPR equal to 0.1 and the use of smoothing, an increased precision (0.50 vs. 0.42) at the expense of a decrease in recall (0.70 vs. 0.78) compared to the two-step approach around suspected apneic events. Significance: The new direct approach guaranteed correct detections for more than 81% of CAs with length L ≥ 20 s, which are considered among the most threatening apneic events for premature infants. These results require additional verifications using more extensive datasets but could lead to promising applications in clinical practice.

Abstract:Depth cameras can provide an effective, noncontact, and privacy-preserving means to monitor patients in the Neonatal Intensive Care Unit (NICU). Clinical interventions and routine care events can disrupt video-based patient monitoring. Automatically detecting these periods can decrease the time required for hand-annotating recordings, which is needed for system development. Moreover, the automatic detection can be used in the future for real-time or retrospective intervention event classification. An intervention detection method based solely on depth data was developed using a vision transformer (ViT) model utilizing real-world data from patients in the NICU. Multiple design parameters were investigated, including encoding of depth data and perspective transform to account for nonoptimal camera placement. The best-performing model utilized ∼85 M trainable parameters, leveraged both perspective transform and HHA (Horizontal disparity, Height above ground, and Angle with gravity) encoding, and achieved a sensitivity of 85.6%, a precision of 89.8%, and an F1-Score of 87.6%.

Abstract:During mechanical ventilation, patient-ventilator disharmony is frequently observed and may result in increased breathing effort, compromising the patient's comfort and recovery. This circumstance requires clinical intervention and becomes challenging when verbal communication is difficult. In this work, we propose a brain computer interface (BCI) to automatically and non-invasively detect patient-ventilator disharmony from electroencephalographic (EEG) signals: a brain-ventilator interface (BVI). Our framework exploits the cortical activation provoked by the inspiratory compensation when the subject and the ventilator are desynchronized. Use of a one-class approach and Riemannian geometry of EEG covariance matrices allows effective classification of respiratory states. The BVI is validated on nine healthy subjects that performed different respiratory tasks that mimic a patient-ventilator disharmony. Classification performances, in terms of areas under ROC curves, are significantly improved using EEG signals compared to detection based on air flow. Reduction in the number of electrodes that can achieve discrimination can often be desirable (e.g. for portable BCI systems). By using an iterative channel selection technique, the Common Highest Order Ranking (CHOrRa), we find that a reduced set of electrodes (n=6) can slightly improve for an intra-subject configuration, and it still provides fairly good performances for a general inter-subject setting. Results support the discriminant capacity of our approach to identify anomalous respiratory states, by learning from a training set containing only normal respiratory epochs. The proposed framework opens the door to brain-ventilator interfaces for monitoring patient's breathing comfort and adapting ventilator parameters to patient respiratory needs.
