Abstract:Atrial fibrillation (AF) is the most common sustained arrhythmia. Renin-angiotensin system (RAS) inhibitors were reported to modify the arrhythmia substrate and reverse atrial remodeling. However, the role of RAS inhibitors on AF recurrence after catheter ablation remains much more controversial. In this study, a meta analysis was performed to explore the effect of RAS inhibitors on AF recurrence after catheter ablation. We searched PubMed, Cochrane Library, EMBASE, and Web of Science for all articles published up to July 2019 on the effect of RAS inhibitors on AF recurrence rate after ablation. We used the random-effects model to estimate the odds ratios (ORs) and confidence intervals (CI). The I2 statistic was used to evaluate statistical heterogeneity. A two-tailed P value of <0.05 was considered statistically significant. Results were further analyzed by subgroup according to the type of study design. We included 13 studies, including 3661 patients with AF, in this analysis, of which 4 were randomized controlled trials (RCTs) and the others were cohort studies. Overall, treatment with RAS inhibitors showed a significant reduction of AF recurrence after catheter ablation (OR, 0.61; 95% CI, 0.45-0.82). Additionally, both the RCT (OR, 0.35; 95% CI, 0.24-0.49) and non-RCT (OR, 0.76; 95% CI, 0.57-1.00) groups demonstrated that RAS inhibitors could reduce the AF recurrence rate after catheter ablation in the subgroup analysis. Our meta-analysis suggests that RAS inhibitors had significant benefit in reducing the recurrence rate of AF after catheter ablation.
Oral Amiodarone In Blank Period Affects Mmp-9 And Timp-1 Level In Post-Ablation Paroxymal Atrial Fibrillation Patients

Abstract:Objective: This study is to investigate whether post-ablation oral amiodarone in blank period could affect levels of MMP-9/TIMP-1 and long-term recurrence in patients with paroxymal atrial fibrillation (PAF). Methods: A total of 100 patients with PAF undertaken pulmonary vein isolation (PVI) and were divided into two groups (50 in each group): group A received oral amiodarone therapy for three months after catheter ablation, and group B were not given amiodarone post-ablation. Atrial arrhythmic data including atrial premature, atrial tachycardia, atrial flutter, and atrial fibrillation (AF) were collected by routine electrocardiogram (ECG) and 24 h-Holter every month during 1-year post-operation follow-ups. MMP-9 and TIMP-1 levels were measured before ablation and at 1, 3, 6, 9, and 12 months post-ablation. Results: During the blank period, the recurrence rate of AF and atrial premature was statistically lower in group A than in group B (both P < 0.05), but no difference was found in the recurrence rate of atrial flutter and atrial tachycardia (both P > 0.05). One-year AF ablation success rate of group A and B were at the same level (P > 0.05). MMP-9 level significantly decreased in both successful ablation sub-groups, while TIMP-1 level increased simultaneously. PVI was prone to affect MMP-9 and TIMP-1 levels, however, there were no statistically differences between two groups. Conclusion: Oral amiodarone could reduce the recurrence rate of AF in blank period. MMP-9 in both successful ablation sub-groups was significantly lower than that of recurrence group during post-ablation follow-up, while TIMP-1 in successful ablation sub-groups was significantly higher than that in the recurrence group.

Perindopril for the Prevention of Atrial Fibrillation Recurrence after Radiofrequency Catheter Ablation: One-year Experience

Abstract:BACKGROUND Recurrence of atrial fibrillation (AF) after ablation is still high. Perindopril plays an essential role in AF induction and maintenance.OBJECTIVE We aimed to prove that perindopril (8 mg) could prevent recurrence after pulmonary vein isolation.METHODS Patients with paroxysmal AF who received radio frequency ablation were randomized to a 3 -month course of perindopril 8 mg once daily (perindopril group) or placebo (placebo group). Angiotensin-II (Ang-II) therapy and standard transthoracic echocardiography were performed. All 256 patients with paroxysmal AF who received radiofrequency ablation were randomized. And we followed them for complete 1 year. The 3 -month recurrence and the 1 -year recurrence were compared between the 2 groups.RESULTS The 3 -month recurrence of AF was observed in 33 (26.19%) of 126 patients in the placebo group vs 19 (14.62%) 130 patients who received perindopril 8 mg once daily (x2, P =.021). One-year recurrence of AF was observed in 36 (28.5%) of 126 patients in the placebo group as compared with 21 (16.2%) of 130 patients who received perindopril after 1 year (P =.017). The kappa value was 0.94 in the control group (P <.001) and 0.96 in the perindopril group (P <.001) between 3 -month and 1 -year recurrence. The Ang-II level was related to the left atrial distance with the reduction in AF recurrence (r = 0.17, P =.005 at 3 months; r = 0.25, P <.001 at 1 year).CONCLUSION Perindopril is an effective and safe treatment for the prevention of AF recurrence after radiofrequency catheter ablation. This effect seems to be strongly associated with a significant decrease in Ang-II level and left atrial distance.

Abstract:Background: The efficacy of antiarrhythmic drugs in reducing the risk of recurrence of atrial fibrillation (AF) after ablation is still uncertain. Therefore, we conducted a systematic evaluation on post ablation antiarrhythmic drugs (AADs) to reduce the risk of recurrent atrial fibrillation. Methods: The databases of PubMed, Embase, Web of Science (WOS), China Science and Technology Journal (CSTJ) Database, Wanfang Database, China National Knowledge Infrastructure (CNKI), and China Biology Medicine (CBM) were searched from inception to 31 December 2023. Randomized controlled trials (RCTs) investigating the efficacy of AADs in preventing AF recurrence were included. Statistical analysis was performed using Review Manager 5.3 (Copenhagen: The Nordic Cochrane Centre, The Cochrane collaboration, 2014) and Stata18.0 (Stata, College Station, TX, USA). Results: A total of 16 studies, with 3834 patients were included in the final analysis. The use of AADs was found to reduce early risk of recurrence (≤3 months) by 28% (risk ratio (RR) = 0.72, 95% confidence interval (CI): 0.53–0.99, p = 0.04), intermediate risk of recurrence (3–12 months) by 22% (RR = 0.78, 95% CI: 0.67–0.91, p = 0.001), and late risk of recurrence (≥12 months) by 29% (RR = 0.71, 95% CI: 0.47–1.07, p = 0.01). No published bias was detected. In sensitivity analyses, the result is consistent and stable after removal of either study. Conclusions: The use of AADs after ablation can reduce the recurrence of AF, and the effect can last for at least 6 months in the overall population. In subgroup analysis, this protective effect can even last for 12 months in the Asian region. In addition, AADs should be used for at least 3 months after ablation to achieve this protective effect.

Prevention and Treatment of Atrial Fibrillation via Risk Factor Modification

Abstract:Atrial fibrillation (AF) is the most common clinically significant arrhythmia, and it increases stroke risk. A preventive approach to AF is needed because virtually all treatments such as cardioversion, antiarrhythmic drugs, ablation, and anticoagulation are associated with high cost and carry significant risk. A systematic review was performed to identify effective lifestyle-based strategies for reducing primary and secondary AF. A PubMed search was performed using articles up to March 1, 2021. Search terms included atrial fibrillation, atrial flutter, exercise, diet, metabolic syndrome, type 2 diabetes mellitus, obesity, hypertension, stress, tobacco smoking, alcohol, Mediterranean diet, sodium, and omega-3 fatty acids. Additional articles were identified from the bibliographies of retrieved articles. The control of hypertension, ideally with a renin-angiotensin-aldosterone system inhibitor, is effective for preventing primary AF and recurrence. Obstructive sleep apnea is a common cause of AF, and treating it effectively reduces AF episodes. Alcohol increases the risk of AF in a dose-dependent manner, and abstinence reduces risk of recurrence. Sedentary behavior and chronic high-intensity endurance exercise are both risk factors for AF; however, moderate physical activity is associated with lower risk of AF. Recently, sodium-glucose cotransporter-2 inhibitors and glucagon-like peptide-1 agonists have been associated with reduced risk of AF. Among overweight/obese patients, weight loss of ≥10% is associated with reduced AF risk. Lifestyle changes and risk factor modification are highly effective for preventing AF.

Abstract:Background: Obstructive sleep apnea (OSA) is an independent and modifiable risk factor in the initiation and maintenance of atrial fibrillation (AF). However, the effective of the continuous positive airway pressure (CPAP) on AF patients with OSA after ablation is elusive. Methods: Cochrane Library, PubMed, Embase, and Web of Science were systematically searched up to February 1, 2023. Studies comprising the AF recurrence rate between the CPAP therapy group and non-CPAP therapy group for the AF patients with OSA were included. Meanwhile, trial sequential analysis (TSA) was conducted to adjust the lower statistical power and random error in this study. Subgroup analysis identified the potential determinants for the AF recurrence rate with CPAP therapy. Results: A total of eight studies including 1,231 AF patients with OSA were eligible. Compared with non-CPAP treatment group, CPAP treatment group was statistically associated with a lower AF recurrence rate (risk ratio [RR], 0.58; p = 0.000). TSA indicated the firm evidence favoring CPAP group for AF recurrence risk. Three significant intervention-covariate interactions for AF recurrence was identified, including study design, non-paroxysmal AF (PAF) proportion, and CPAP treatment strategy. Conclusion: Our study suggests that CPAP therapy might be an effective strategy on reducing AF recurrence post-ablation for AF patients with OSA. The CPAP treatment strategy and the non-PAF proportion might be the possible determinants on AF recurrence for AF patients with OSA after ablation. Effects of Oral Inflammatory Diseases and Oral Hygiene on Atrial Fibrillation: A Systematic Review

Abstract:Objective. Research evidence suggests a link between periodontitis (PD) and atrial fibrillation, but the nature of this link is unclear. This study aimed to systematically review and evaluate the association between PD, other oral diseases, and atrial fibrillation and the role of oral hygiene in preventing atrial fibrillation. Methods. We searched the Medline, Embase, Cochrane Library, and Web of Science databases for the clinical study of oral health and atrial fibrillation from inception to November 2022. Oral health conditions included PD and other oral inflammatory diseases, regular oral hygiene, and tooth brushing. The primary outcomes were the risk of new-onset atrial fibrillation in patients with oral disease, the effect of regular oral care on preventing atrial fibrillation, the effect of frequent tooth brushing on preventing atrial fibrillation, and the incidence of atrial fibrillation in PD patients. Results. Eight clinical trials with a total of 4,328,355 patients were included. The result of the research showed that PD and other impaired oral health may be associated with new-onset atrial fibrillation, and its severity was dose-responsive to the risk of atrial fibrillation. The incidence of atrial fibrillation in patients with severe PD was about 16.3%. Moreover, PD may increase the risk of long-term arrhythmia in patients with atrial fibrillation. Regular oral care and frequent tooth brushing can reduce the incidence of atrial fibrillation. Conclusion. Regular and moderate oral hygiene, frequent tooth brushing, and prevention of PD and other oral inflammatory diseases could reduce the occurrence of atrial fibrillation. It is recommended to strengthen the popularization of oral health knowledge in the publicity related to atrial fibrillation.

Lowering serum homocysteine in H-type hypertensive patients with atrial fibrillation after radiofrequency catheter ablation to prevent atrial fibrillation recurrence

Abstract:Background: Prior investigation revealed that elevated serum total homocysteine (tHcy) are strongly correlated with atrial fibrillation (AF) recurrence. Herein, the goal of this study was to elucidate whether folic acid (FA) treatment reduced AF recurrence following radiofrequency catheter ablation (RFCA). Methods: To conduct this retrospective research, we included consecutive H-type hypertensive AF patients, who were treated with first RFCA, between January 2010 and January 2022. We assessed the AF recurrence risk between patients who were taking 10 mg enalapril and 0.8 mg FA in a single-pill combination (enalapril-FA) daily and those who were taking a pill of 10 mg enalapril only. Outcomes were compared using the propensity-score matched analysis. Cox regression model was employed for the evaluation of AF recurrence events. Results: Out of 2,714 patients, 645 patients receiving enalapril and 282 patients receiving enalapril-FA were included for analysis. Following propensity score matching, 239 patients remained in each group. These patients were followed-up for a median of 379 (137-596) days, and revealed that the enalapril-FA patients had drastically reduced AF recurrence, compared to the enalapril patients [adjusted hazard ratio (HR), 0.68; 95% confidence interval (CI), 0.48-0.97; P = 0.029]. Apart from this, no interactions were detected in the subgroup analysis. Conclusion: In H-type hypertensive AF patients who were treated with first RFCA, FA supplementation was correlated with a reduced AF recurrence risk.
Efficacy of Catheter Ablation and Concomitant Antiarrhythmic Drugs on the Reduction of the Arrhythmia Burden in Patients with Long-Standing Persistent Atrial Fibrillation.

Abstract:Background Little is known about the long-term outcome and recurrent form recurrence after catheter ablation of atrial fibrillation (AF) in patients with long-standing persistent AF. Methods Two hundred thirty-six patients with persistent AF (193 men; age, 61.5±10.7 years) were enrolled, and were classified according to the duration of AF: AF duration of <1 year (group A, n=99), between 1 to 5 years (group B, n=101), and ≥5 years (group C, n=36). The long-term recurrence rate and recurrent form were compared among the groups. Results During a median follow-up of 3.7 years, the recurrence rate was significantly worse in group C after the index and multiple procedures (Log-Rank, both for a P<0.001 in comparison to group A). In the multivariate analysis, the duration was an independent predictor of an arrhythmia recurrence (HR, 1.206; 95%CI, 1.053 to 1.381; P=0.007). Recurrent AF became permanent in 8 patients (3.4%), which was not associated with a difference in the groups (Log-rank, P=0.055), while antiarrhythmic drugs (AADs) were continued in 70% of the recurrent patients. Conclusion Despite a high AF recurrence rate in the patients with an AF duration of ≥5 years, the majority of the patients with recurrence who continued on AADs had a paroxysmal form of AF. Catheter ablation and concomitant AADs may be effective in reducing the AF burden in such patients with an advanced AF disease stage.

Effects of Glucagon-Like Peptide 1 Receptor Agonists on Atrial Fibrillation Recurrence After Catheter Ablation: A Systematic Review and Meta-analysis

Abstract:Introduction: Despite the technological advancements in catheter ablation strategies, the recurrence of atrial fibrillation (AF) post-ablation remains a concern that requires further investigation. Glucagon-like peptide 1 (GLP-1) receptor agonists have shown a significant effect on weight reduction, which in turn is associated with freedom from AF recurrence in both patients who are obese and not obese undergoing ablation. Therefore, we aimed to summarize the available evidence on the efficacy of GLP-1 receptor agonists in maintaining sinus rhythm post-ablation. Methods: Medline, Cochrane Library, and Scopus were searched until June 9, 2024. Double-independent study selection, data extraction, and quality assessment were performed. Evidence was pooled using DerSimonian-Laird random effects meta-analysis. Results: Three propensity score-matched studies (n = 6031 participants) were analyzed. Over a 12-months follow-up, the use of GLP-1 receptor agonists was associated with a significant reduction in AF recurrence compared to controls, hazard ratio (HR) = 0.549, 95% confidence interval (CI) = [0.315, 0.956], P = 0.034; I2 = 57%. No significant heterogeneity was observed (Q statistic = 4.6, heterogeneity P = 0.1). Conclusion: The use of GLP-1 receptor agonists is associated with a lower risk of AF recurrence in patients receiving AF ablation therapy. Further large-scale randomized trials are necessary to explore the efficacy of GLP-1 receptor agonists in maintaining ablation outcomes over the long term.
Efficacy of dapagliflozin in improving arrhythmia-related outcomes after ablation for atrial fibrillation: a retrospective single-center study

Abstract:Background Atrial fibrillation (AF) is a widespread type of sustained arrhythmia that poses significant health risks. Catheter ablation is the preferred treatment; however, arrhythmia recurrence remains challenging. Sodium-glucose co-transporter 2 inhibitors, particularly dapagliflozin (DAPA), have exhibited cardiovascular benefits. However, to date, the influence of these inhibitors on AF post-ablation remains unclear. Methods We analyzed the records of 272 patients who underwent catheter ablation for AF from January 2018 to December 2022. Patients were divided into the control ( n = 199) and DAPA ( n = 73) groups based on DAPA prescription post-ablation. The primary outcome was total atrial arrhythmia recurrence after a 3-month blanking period. Results The mean age was 72.19 ± 5.45 years; 86.8% of the patients were men. At 18 months post-ablation, 36.2% and 9.5% of the patients in the control and DAPA groups, respectively, reported atrial arrhythmia. Multivariate analysis revealed that DAPA use was associated with a significantly reduced risk of arrhythmia recurrence (adjusted hazard ratio [aHR]: 0.15, 95% confidence interval [CI]: 0.07–0.32, p < 0.001). After propensity score-matching (PSM) in 65 pairs, arrhythmia recurrence was lower in the DAPA group compared with the control (8.3% versus 30.8%, aHR: 0.17, 95% CI: 0.06–0.51, p = 0.002). Freedom from total arrhythmia recurrence was significantly higher in the DAPA group compared with the control group in both the overall and PSM population (log-rank test p < 0.01). Conclusion DAPA administration post-ablation was associated with significantly reduced atrial arrhythmia recurrence rates, indicating its potential as an adjunct therapy for enhancing the success of AF ablation.

Abstract:BACKGROUND: Inflammation is implicated in the pathophysiology of atrial fibrillation (AF). Periodontitis causes a general inflammatory response. Whether periodontitis is related to AF is unknown.OBJECTIVE: The aim of the study was to test the hypothesis that inflammation facilitates AF.METHODS: Twenty-two adult mongrel canines of either sex were used for this study. Periodontitis was induced in 12 dogs (periodontitis group) by tying 2-0 silk ligatures at the second premolar of mandibula. Ten healthy dogs were used as controls. Before the ligation procedure and on the day 30, 60, and 90 after ligation, an electrophysiologic evaluation was performed to measure atrial refractoriness and AF inducibility by delivering a single atrial extrastimuli in the high right atrium, atrial septum (AS), and coronary sinus (CS), respectively. Before each electrophysiologic study, blood samples were taken for determining the levels of C-reactive protein (CRP) and tumor necrosis factor-alpha (TNF-alpha). Animals were killed after 90 days. The hearts and mandibulae were harvested for morphological study, and the periodontal disease severity was quanti fi ed.RESULTS: Atrial effective refractory period (AERP) shortened, and AF inducibility increased progressively in the periodontitis group. At a drive length of 300 milliseconds, AERP in the CS was 126.7 +/- 13.0 milliseconds and 107.5 +/- 9.7 milliseconds after 60 and 90 days of ligation, respectively (vs 165.8 +/- 10.8 milliseconds at baseline; P < .001). By CS pacing, AF was induced in 5 and 10 of 12 dogs on day 60 and 90 after ligation, respectively (vs 1/12 at baseline; P < .05 and P < .01, respectively). Elevation of CRP and TNF-alpha occurred after 60 days of ligation (CRP, 13.42 +/- 2.21 mg/L vs control, 1.92 +/- 0.38 mg/L; P < .001; TNF-alpha, 9.85 +/- 1.72 mg/L vs control, 3.36 +/- 0.75 mg/L; P < .001) and reached the peak at the end of the study (CRP, 31.38 +/- 2.69 mg/L vs control, 1.99 +/- 0.40 mg/L; P < .001; TNF-alpha, 12.32 +/- 1.07 mg/L vs control, 3.24 +/- 0.53 mg/L; P < .001). There was a negative correlation between the levels of serum inflammatory factors and AERP values (P < .05). Alveolar bone level decreased in the periodontitis group (P < .001). The long axis (P < .001) of atrial cardiomyocytes including the right atrial appendage (25.50 +/- 3.58 microm vs 18.14 +/- 3.32 microm), AS (24.78 +/- 3.45 microm vs 17.47 +/- 2.57 microm), and left atrial appendage (31.90 +/- 4.80 microm vs 18.78 +/- 2.42 microm) from the periodontitis group was larger than the control group. The short axis of atrial cardiomyocytes was larger than the control group, too (P < .001). Inflammatory cells were more generally found in the atria of the periodontitis group (P < .001). Myolysis affected some atrial cardiomyocytes of the dogs with periodontitis.CONCLUSION: Periodontitis led to inflammatory responses in the atrial myocardium, which disturbed the structural and electrophysiologic properties of the atrium and facilitated AF.

Impact of Pericoronary Adipose Tissue Attenuation on Recurrence after Radiofrequency Catheter Ablation for Atrial Fibrillation

Abstract:BackgroundInflammation plays a vital role in the occurrence and progression of atrial fibrillation (AF). The association between pericoronary adipose tissue attenuation (PCATA) and AF recurrence following ablation has not been fully clarified. HypothesisWe aimed to evaluate the association between PCATA and AF recurrence after radiofrequency catheter ablation (RFCA). MethodsPatients who underwent the first RFCA for AF and performed coronary computed tomography angiography before ablation between 2018 and 2021 were enrolled. The predictive values of PCATA for AF recurrence after ablation were investigated. The area under curve (AUC), relative integrated discrimination improvement (IDI), and categorical free net reclassification improvement (NRI) were used to assess the discrimination ability of different models for AF recurrence. ResultsDuring 1-year follow-up, 34.1% patients experienced AF recurrence. The multivariable analysis model revealed that PCATA of the right coronary artery (RCA) was an independent risk factor for AF recurrence. Patients with a high level of RCA-PCATA had a high risk of recurrence, after adjusting for other risk factors by restricted cubic splines. The performance in predicting AF recurrence was significantly improved by adding the marker of RCA-PCATA to the clinical model (AUC: 0.724 vs. 0.686, p = .024), with a relative IDI of 0.043 (p = .006) and continuous NRI of 0.521 (p < .001). ConclusionsPCATA of RCA was independently associated with AF recurrence after ablation. PCATA may be helpful for risk classification for AF ablation patients.

Abstract:The management of atrial fibrillation (AF) has focused on anticoagulation, rhythm control and ventricular rate control. Recently, a fourth pillar of AF management has been incorporated recognising the importance of risk factor management (RFM). There are several risk factors that contribute to the development and progression of AF, these include traditional risk factors such as age, hypertension, heart failure, diabetes and valvular heart disease. However, increasingly it is recognised that obesity, sleep apnoea, hyperlipidaemia, smoking, alcohol, physical inactivity, genetics, aortic stiffness are associated with the development of AF. Importantly, several of these risk factors are modifiable. We have seen the evolution of RFM programmes which have demonstrated promising results. Indeed, the evidence is now so compelling that major clinical guidelines strongly advocate that aggressive treatment of these risk factors as a key component of AF management. Patients with AF who comprehensively managed their risk factors demonstrate greater reduction in symptoms, AF burden, more successful ablations and improved outcomes with greater AF freedom. In this article, we will review the evidence for the association between cardiac risk factors and AF and assess the burgeoning evidence for improved AF outcomes associated with aggressive cardiac RFM.

Abstract:Introduction: Dyslipidemia is a modifiable cardiovascular risk factor. However, it is unclear if baseline lipid levels prior to atrial fibrillation (AF) ablation are associated with AF recurrence. Hypothesis: This study aimed to examine if there was an association between baseline lipid profiles and AF recurrence after AF ablation. Methods: We retrospectively studied patients who underwent AF ablation from January 2016 to September 2021 at two high-volume international centers. Patients with a lipid profile in the previous 24 months prior to AF ablation and a one-year post-ablation follow-up with electrocardiogram were included. AF recurrence was defined as having a 12-lead electrocardiographic evidence of AF or atrial tachycardia within one year after a 3-month blanking period. Logistic regression analysis was performed to examine association of lipid profiles (low-density lipoprotein (LDL), high-density lipoprotein (HDL), total cholesterol (TC), and triglycerides) with AF recurrence. Lipid profiles were evaluated both as continuous and categorical variables. Well-established cut-points were used for LDL, and tertiles were used for HDL, TC, and triglycerides. Results: A total of 287 patients were included in the study (mean age (SD), 63.6 (10.5), 36% female). Patients with LDL<70 mg/dL or TC < 141 mg/dL had a decreased risk of AF recurrence in unadjusted analysis. These associations remained significant after adjustment by traditional AF risk factors and statin use (odds ratios (ORs) (95% confidence interval): 0.27 (0.09 - 0.79) and 0.28 (0.09 - 0.84), respectively. TC analyzed as a continuous variable was associated with increased risk of AF recurrence (1.012 (1.002 - 1.022) per 1 mg/dL increase). (Table). Conclusions: In our international registry, patients with LDL <70 mg/dL or TC <141 mg/dL were associated with a reduced risk of AF recurrence, suggesting a beneficial effect of low LDL and TC in patients with AF undergoing catheter ablation.

Abstract:Background: Gastroesophageal reflux disease (GERD) has been associated with increased incidence/recurrence of atrial fibrillation (AF). However, the impact of GERD and proton pump inhibitor (PPI) therapy on outcomes of AF catheter ablation remains unclear. We aimed to assess the association between the presence of GERD and risk of repeat AF ablation, stratified by PPI therapy. Methods: A retrospective cohort study was conducted on paroxysmal/persistent AF patients undergoing initial ablation in 1/2011-9/2015. GERD was defined by endoscopic findings, objective reflux testing, or clinical symptoms. The association between GERD/PPI use and time to repeat ablation was evaluated by time-to-event analysis with censoring at last clinic follow-up within one year. Results: 381 subjects were included. GERD patients (n=80) had a higher one-year repeat ablation rate compared to no GERD (25% vs 11.3%, p=0.0034). Stratifying by PPI use, untreated GERD patients (37.5%) more likely needed repeat ablation compared to reflux-free (11.3%, p=0.0003) and treated GERD (16.7%, p=0.035) subjects. On multivariable Cox regression analyses, GERD was an independent risk factor for repeat ablation (HR 3.30, CI:1.79-6.08, p=0.0001). Specifically, untreated GERD was associated with earlier repeat ablation compared to those with no GERD (HR 4.02, CI:1.62-12.05, p=0.0013). However, no significant difference in repeat ablation risk was noted between reflux-free and PPI-treated GERD groups. Conclusion: GERD was an independent predictor for risk of repeat AF ablation within one year, even after controlling for major cardiovascular comorbidities and confounders. PPI therapy modulated this risk, as repeat ablation-free survival for PPI-treated GERD was non-inferior to reflux-free patients.

Abstract:BackgroundThe association between physical activity (PA) and atrial tachyarrhythmia (AT) recurrence after ablation for atrial fibrillation (AF) remains unclear. MethodsWe consecutively enrolled 496 patients treated with AF ablation therapy in Beijing Anzhen Hospital. After excluding 6 patients with valvular heart disease, 7 patients with congenital heart disease, 33 patients lost to follow‐up, and 14 patients who did not provide PA level during follow‐ups, 436 patients had their PA level assessed by the International Physical Activity Questionnaire‐Short Form before ablation and each time of follow‐up. The association between PA level (measured at the time closest to AT recurrence, or the end of 12‐month follow‐up if no AT recurrence), as well as active PA during follow‐up, and post‐ablation AT recurrence was tested by multivariate logistic regression. Results: Of the enrolled patients, 134 (30.7%) patients experienced AT recurrence in the first 12 months post‐ablation. Compared to patients with low PA, patients with moderate or high PA had a lower risk of AT recurrence [odds ratio (OR) 0.44, 95% confidence interval [CI] 0.25‐0.80, P = 0.01 for patients with moderate PA; and OR 0.43 (0.21‐0.85), P = 0.02 for patients with high PA]. Compared to patients without active PA, patients with active PA had a lower risk of AT recurrence [OR 0.44 (95% CI 0.27‐0.70), P<0.01]. ConclusionsModerate and high PA are associated with a lower risk of AT recurrence after AF ablation. Active PA during follow‐up is also associated with a significantly lower risk of AT recurrence in the post‐ablation AF population.This article is protected by copyright. All rights reserved
