Abstract:Object: To explore the post-hemispherotomy seizure outcome and its prognostic predictors in children with refractory epilepsy.Methods: We reviewed 83 consecutive child patients with refractory epilepsy who underwent a hemispherectomy from June 2014 to January 2017 at our Pediatric Epilepsy Center. Demographic, clinical, EEG, neuroimaging, and surgical data were collected. Seizure outcome data were collected via outpatient clinics as well as telephone visits and were graded according to Engel criteria. Logistic regression model and Cox proportional hazard regression model were, respectively, applied to explore the related factors predicting the seizure outcomes of children after a hemispherotomy.Results: Of the 83 patients, 55 (63.2%) were male. The mean seizure onset age was 1.9 years (0-8.7 years), and the mean surgery age was 5 years (0.8-14 years). At a mean follow-up of 3 years, 69 children (83.1%) were seizure free, and 14 (16.9%) exhibited seizure recurrence. In a univariate analysis, whether or not considering follow-up time, a non-lateralized interictal EEG pattern, bilateral PET abnormalities and acute postoperative seizures (APOS) could all predict poor seizure outcomes post-hemispherotomy. Bilateral PET abnormalities were independently correlated with unfavorable seizure outcomes in the multivariate Logistic regression analysis (Odds ratio(OR) = 13.05, 95%CI = 1.52-112.29, P = 0.019) and in the multivariate Cox proportional hazard analysis(OR = 13.99, 95%CI = 2.75-71.17, P = 0.001).Conclusions: Child epileptic patients with bilateral PET abnormalities may have poor seizure outcomes after a hemispherotomy procedure. This study will facilitate better candidate selection for hemispherotomies and early identification of unfavorable seizure outcomes.

Abstract:Congenital heart disease (CHD) is one of the main supportive diseases of extracorporeal membrane oxygenation in children. The management of extracorporeal membrane oxygenation (ECMO) for pediatric CHD faces more severe challenges due to the complex anatomical structure of the heart, special pathophysiology, perioperative complications and various concomitant malformations. The survival rate of ECMO for CHD was significantly lower than other classifications of diseases according to the Extracorporeal Life Support Organization database. This expert consensus aims to improve the survival rate and reduce the morbidity of this patient population by standardizing the clinical strategy. The editing group of this consensus gathered 11 well-known experts in pediatric cardiac surgery and ECMO field in China to develop clinical recommendations formulated on the basis of existing evidences and expert opinions. The primary concern of ECMO management in the perioperative period of CHD are patient selection, cannulation strategy, pump flow/ventilator parameters/vasoactive drug dosage setting, anticoagulation management, residual lesion screening, fluid and wound management and weaning or transition strategy. Prevention and treatment of complications of bleeding, thromboembolism and brain injury are emphatically discussed here. Special conditions of ECMO management related to the cardiovascular anatomy, haemodynamics and the surgical procedures of common complex CHD should be considered. The consensus could provide a reference for patient selection, management and risk identification of perioperative ECMO in children with CHD.

Abstract:BACKGROUND:This study aimed to discribe the experience in supporting children with refractory cardiopulmonary failure with extracorporeal membrane oxygenation (ECMO).METHODS:We retrospectively reviewed 12 children with refractory cardiopulmonary failure supported with ECMO from February 2009 to August 2015 in the Pediatric Intensive Care Unit (PICU), Children's Hospital, Zhejiang University School of Medicine.RESULTS:Seven of the 12 patients were weaned successfully from ECMO and dischaged from the hospital, with a survival rate of 58.3% (7/12). Among them, five patients had acute fulminant myocarditis (AFM). Complications during ECMO included hemorrhage, hemolysis, thrombosis, acute kidney injury, and secondary hematogenous infection. During 1-24 month follow-up, the seven surviving patients recovered with normal cardiopulmonary function.CONCLUSIONS:ECMO is useful for supporting children with refractory cardiopulmonary failure, especially for treatment of AFM.

Abstract:Background This article summarizes the use of extracorporeal membrane oxygenation (ECMO) for the treatment of children with severe hemodynamic alteration in perioperative cardiovascular surgery. Methods Four children with congenital heart disease (CHD) (3 boys and 1 girl, aged 6 days to 4 years and weighing 2.8–15 kg) associated with severe heart failure and/or hypoxemia were treated with ECMO cardiopulmonary support in perioperative cardiovascular surgery between July 2007 and July 2008. We retrospectively analyzed the medical records of the 4 children. Results Of the 4 children, 2 survived and 2 died. The survivors were treated with venoarterial (VA) ECMO due to severe low output syndrome after arterial switch operation. They were weaned successfully from 22-hour and 87-hour ECMO support, and discharged 20 days and 58 days after ECMO explantation, respectively. The other boy treated with venovenous ECMO died of severe hypoxemia and metabolic acidosis. The other girl with VSD, treated with VA ECMO because of failure to wean from cardiopulmonary bypass, died from irreversible heart failure 11 hours after ECMO explantation. The main complications in this series included pulmonary hemorrhage, blood tamponade, surgical site bleeding, hemolysis and hyperbilirubinemia. Conclusions ECMO is an effective therapy for patients with severe heart failure in the perioperative cardiovascular surgery. The keys to successful ECMO are selection of indications, time to set up ECMO, and good management of complications during ECMO.

Abstract:This is a reply to the letter titled "Understanding lactate and its clearance during extracorporeal membrane oxygenation for supporting refractory cardiogenic shock patients" by Eva Rully Kurniawati et al. In response to the concerns raised about our paper published in BMC Cardiovascular Disorders, titled "Association between serum lactate levels and mortality in patients with cardiogenic shock receiving mechanical circulatory support: a multicenter retrospective cohort study," we have addressed the confounding bias on the population included and the use of VA-ECMO and Impella CP. Furthermore, we have provided new data on the correlation of oxygen supply and lactate levels at admission of cardiogenic shock.
[Emergency Use of Extracorporeal Membrane Oxygenation in Pediatric Critically Ill Patients].

Abstract:OBJECTIVE:The history of clinical application of extracorporeal membrane oxygenation (ECMO) has been more than 30 years. But in China, there were only a few ECMO centers with limited successful cases reported by the end of twentieth century. The high morbidities and mortalities in current pediatric ECMO practice are noted in China. Therefore, it is necessary to review the experience on rescue use of ECMO in critically ill pediatric patients.METHOD:A retrospective analysis was done for patients who had been receiving ECMO treatment to rescue refractory cardiorespiratory failure from different causes in a hospital between July 2007 and May 2011.RESULT:A total of 12 patients were treated with ECMO; 7 of them were male and 5 female, they aged 6 days to 11 years, weighed 2.8 - 35 (17.21 ± 11.64) kg. The underlying causes of cardiorespiratory failure were as follows: two cases with acute respiratory distress syndrome (ARDS) leading to respiratory failure, 4 with failure of weaning from cardiopulmonary bypass, 3 with fulminant myocarditis, 1 with right ventricular cardiomyopathy leading to repeated cardiac arrest, 1 with preoperative severe hypoxemia, and 1 with anaphylactic shock complicated with massive pulmonary hemorrhage and severe hypoxemia. Of the 12 cases, 3 were established ECMO (E-CPR) while underwent chest compression cardiopulmonary resuscitation (CPR). The mean ECMO support time was 151.75 (15 - 572) h. Seven patients (58.33%) were weaned from ECMO, 6 patients (50.00%) were successfully discharged. Six cases had bleeding from sutures, 2 cases with severe bleeding underwent thoracotomy hemostasis, 2 presented with acute renal failure. Infection was documented in 3 cases, hyperbilirubinemia in 2 cases, lower limb ischemia in 1 case, hyperglycemia in 3 cases, disseminated intravascular coagulation in 1 case, membrane lung leakage in 2 cases, systemic hemolysis in 3 cases, oxygenator failure in 2 cases and oxygenator thrombosis in one case. During the follow-up between 6 months and 4.5 years, 5 patients survived with good quality of life, without any documented central nervous system disorders. One case survived with the right lower extremity disorder from ischemic damage. His motor function has been improved following orthopedic operation at one year after discharge.CONCLUSION:ECMO is a justifiable alternative treatment for reversible severe cardiopulmonary failure in critically ill children.
Local Experience With Extracorporeal Membrane Oxygentaion In Children With Acute Fulminant Myocarditis

Abstract:To analyze the clinical effect of extracorporeal membrane oxygenation (ECMO) in children with acute fulminant myocarditis, we retrospectively analyzed the data of five children with acute fulminant myocarditis in the intensive care unit (ICU) at the Affiliated Children's Hospital, Zhejiang University from February 2009 to November 2012. The study group included two boys and three girls ranging in age from 9 to 13 years (median 10 years). Body weight ranged from 25 to 33 kg (mean 29.6 kg). They underwent extracorporeal membrane oxygenation (ECMO) through a venous-arterial ECMO model with an average ECMO supporting time of 89.8 h (40-142 h). Extracorporeal circulation was established in all five children. After treatment with ECMO, the heart rate, blood pressure, and oxygen saturation were greatly improved in the four children who survived. These four children were successfully weaned from ECMO and discharged from hospital machine-free, for a survival rate of 80% (4/5). One child died still dependent on the machine. Cause of death was irrecoverable cardiac function and multiple organ failure. Complications during ECMO included three cases of suture bleeding, one case of acute hemolytic renal failure and suture bleeding, and one case of hyperglycemia. During the follow-up period of 4-50 months, the four surviving children recovered with normal cardiac function and no abnormal functions of other organs. The application of ECMO in acute fulminant myocarditis, even in local centers that experience low incidence of this disease, remains an effective approach. Larger studies to determine optimal timing of placement on ECMO to guide local centers are warranted.
Successful Rescue of a Child with Severe Anaphylactic Shock by Extracorporeal Membrane Oxygenation

Abstract:A 9-year and 8-month old boy suffered from anaphylactic shock induced by cefotaxime. The patient's condition deteriorated after the emergent therapy with epinephrine, high-dose of methylprednisolone and mechanical ventilation. The extracorporeal membrane oxygenation (ECMO) with V-A model was carried out successfully after 4 hours of admission to paediatric intensive care unit. The patient's conditions were getting better after ECMO application with gradual disappearance of the pulmonary oedema and haemorrhage. He was weaned from ECMO after 64 hours and extubated successfully 84 hours later.
Distinct profiles of cerebral oxygenation in focal vs. secondarily generalized EEG seizures in children undergoing cardiac surgery

Abstract:Objectives: Seizures are common in children undergoing cardiopulmonary bypass (CPB). Cerebral oxygen saturation (ScO 2 ) by near-infrared spectroscopy is routinely monitored in many centers, but the relations between the levels and changes of ScO 2 and brain injuries remain incompletely understood. We aimed to analyze the postoperative profiles of ScO 2 and cerebral blood flow velocity in different types of EEG seizures in relation to brain injuries on MRI. Methods: We monitored continuous EEG and ScO 2 in 337 children during the first 48 h after CPB, which were analyzed in 3 h periods. Cerebral blood flow peak systolic velocity (PSV) in the middle cerebral artery was measured daily by transcranial Doppler. Postoperative cerebral MRI was performed before hospital discharge. Results: Based on the occurrence and spreading types of seizures, patients were divided into three groups as patients without seizures (Group N; n = 309), those with focal seizures (Group F; n = 13), or with secondarily generalized seizures (Group G; n = 15). There were no significant differences in the onset time and duration of seizures and incidence of status epilepticus between the two seizures groups ( Ps ≥ 0.27). ScO 2 increased significantly faster across Group N, Group G, and Group F during the 48 h ( p < 0.0001) but its overall levels were not significantly different among the three groups ( p = 0.30). PSV was significantly lower ( p = 0.003) but increased significantly faster ( p = 0.0003) across Group N, Group G, and Group F. Group F had the most severe brain injuries and the highest incidence of white matter injuries on MRI among the three groups ( Ps ≤ 0.002). Conclusion: Postoperative cerebral oxygenation showed distinct profiles in secondarily generalized and particularly focal types of EEG seizures in children after CPB. A state of 'overshooting' ScO 2 with persistently low PSV was more frequently seen in those with focal seizures and more severe brain injury. Information from this study may have important clinical implications in detecting brain injuries when monitoring cerebral oxygenation in this vulnerable group of children after CPB.

Clinical Application and Choice of Mechanical Circulatory Support Devices in Children

Abstract:Objective To explore the clinical application of centrifugal ventricular assist device (VAD) and extracorporeal membrane oxygenation (ECMO) at Shanghai Children's Medical Center and summarize the experiences of selecting an ideal method of mechanical circulatory support (MCS) in pediatric cases. Methods From January 2005 to December 2015, 60 cases were support by MCS devices. There were congenital heart defects (n=56) and acute myocarditis (n=4). The mechanical circulatory support devices were used for supporting circulatory functions because of heart failure or arrhythmia. Thirty-two cases were supported with centrifugal VAD (VAD group) and 26 with ECMO (ECMO group) and other 2 cases were started by VAD then transferred to ECMO (VAD-ECMO group). Results The duration for VAD was from 3 to 458 hours. Twenty-two cases were weaned and 20(62.50%) were successfully discharged. The duration of ECMO was from 26 to 498 hours. Sixteen cases were weaned and 14 (53.85%) successfully discharged. None survived in VAD-ECMO group. Bleeding and neurological complications were two major mortal causes. ALCAPA (anomalous origin of coronary artery from pulmonary artery) cases had the biggest ratio in VAD group and the survival rate was 81.82%. Conclusions Pediatric patients with cardiac failure are suitable for VAD and ECMO supports. Cardiac function, respiratory function, age, complications and costs should be considered for selecting from these two methods. Treatments of Intracranial Hemorrhage after Extracorporeal Membrane Oxygenation: Case Report and Literature Review

Abstract:Objective To explore the diagnosis,treatment and prognosis of intracranial hemorrhage after extracorporeal membrane oxygenation (ECMO) in children.Methods Retrospective analysis was performed for 89 ECMO cases with intracranial hemorrhage from 2011 to 2018.Their clinical data were collected and analyzed.Six children (6.7％) had intracranial hemorrhage during ECMO treatment.Among 6 cases of intracranial hemorrhage,there were 4 boys and 2 girls with an age range from 1 day to 11 years.The causes were myocarditis (n =1) and severe pneumonia (n =5).Two cases required ECMO out-hospital catheterization.The average ECMO duration was 126.8(28.0-229.5) hours.Results Computed tomography was examined for unequal pupils (n =2),magnetic resonance imaging (MRI) for hemiplegia (n =1),B-mode ultrasound for routine uses (n =2) and MRI for intracranial hemorrhage (n =1).The locations of lesion were subdural (n =1),deep brain tissue (n =1) and brain lobe (n =4).The interventions were surgery (n =2),conservative measures (n =2) and death after giving up treatments (n =2).GOS score of surgical children was 5 points (epidural hemorrhage) and 4 points (hemiparesis).Conclusions Intracranial hemorrhage is a severe hemorrhagic complication after ECMO.Early diagnosis and surgical treatment may improve its prognosis.

Abstract:Objective: To survey the conduction and evaluate the effectiveness of extracorporeal membrane oxygenation (ECMO) therapy in pediatric intensive care unit (PICU) in China mainland. Methods: In a questionnaire-based survey, we retrospectively reviewed the application of ECMO in children's hospital and general hospital in China mainland to summarize and analyze the categories of diseases and prognosis of children treated with ECMO therapy. Results: By December 31, 2017, a total of 23 hospitals using ECMO, including 22 tertiary referral hospitals and 1 secondary hospital, among which 16 were children's hospitals and 7 were general hospitals. Thirty-seven ECMO equipment was available. A total of 518 patients treated with ECMO, within whom 323 (62.4%) successfully weaned from ECMO and 262 (50.6%) survived to discharge. Among 375 pediatric patients, 233 (62.1%) were successfully weaned from ECMO and 186 (49.6%) survived to discharge. Among 143 newborn patients, 90 (62.9%) successfully weaned from ECMO, 76 (53.1%) survived to discharge. ECMO was applied in veno-arterial (VA) mode to 501 (96.7%) patients, veno-venous (VV) mode to 14 (2.7%) patients, and VV-VA conversion mode to 3 (0.6%) patients. Sixty-nine patients required extracorporeal cardiopulmonary resuscitation (ECPR), including 20 newborn patients (29.0%) and 38 pediatric patients (71.0%), who were all with cardiovascular disease. Neonatal respiratory distress syndrome (26/61), persistent pulmonary hypertension of the newborn (PPHN) (12/61), and meconium aspiration syndrome (MAS) (11/61) are the most common pulmonary diseases in newborn patients; among whom, infants with PPHN had highest survival rate (10/12), followed by MAS (9/11). Among newborn patients with cardiovascular diseases, those who admitted were after surgery for congenital cardiac disease were the most common (54/82), while those with septic shock had the highest survival rate (2/3). In pediatric pulmonary diseases, acute respiratory distress syndrome was the most common (42/93), while plastic bronchitis was with the highest survival rate (4/4), followed by viral pneumonia (13/16). Among pediatric cardiovascular diseases, congenital cardiac defect was the most common (124/282), while fulminant myocarditis had the highest survival rate (54/77). Conclusion: The application of ECMO as a rescue therapy for children with severe cardiopulmonary failure has dramatically developed in China mainland.

Abstract:Objective To summarize extracorporeal life support ( ECLS) cases in the perioperative period of pediatric cardiac surgery in mainland China (excluding Hong Kong, Macao, Taiwan). Methods A questionnaire survey was conducted to investigate pediatric extracorporeal life support in China before June 30, 2017. Results A total of 43 hospitals in China that carried out children open-heart surgery participated in this questionnaire survey and it covered all hospitals that performed more than 300 open-heart surgeries per year. Among them, ECLS has been undertaken in children who received cardiac surgery in 30 hospitals, most of which located in the more developed regions. The higher the number of operations, the higher the proportion of ECLS employed. A total of 457 cases of extracorporeal membrane oxygenation ( ECMO) were used in these hospitals during the children's heart perioperative period. This survey included 375 children (＞28 days to≤18 years) with 229 (61% ) cases successfully weaned and 166 ( 44%) cases survved, and 77 neonates (≤28 days) with 37 (48%) cases successfully weaned and 25 (32%) cases discharged. The neonatal survival rate was significantly lower than that in older children. Five patients were supported with ECMO after heart transplantation with 4 (80% ) cases successfully weaned and discharged. A total of 39 cases were supported with ventricular assist device (VAD) ， with left ventricular assist device (LVAD) in 38 cases and right ventricular assist device (RVAD) in 1 case. Conclusions ECLS has been used for decades in China, but the development is uneven and gaps among regions are still obvious. Although the survival rate is acceptable ，differences among centers are still considerable and the results are under the international standard.

Abstract:To the Editor: I have been deeply engaged in contemplating the recent publication in the Journal of Trauma and Acute Care Surgery , titled "The Application of ECMO in Trauma Care: What You Need to Know." 1 This article provides a comprehensive review of extracorporeal membrane oxygenation (ECMO) utilization in trauma patients, offering valuable insights for clinical practice. However, when discussing ECMO's application in trauma care, I believe that it is imperative to delve further into the necessity of rigorous indications assessment. After all, ECMO, as a specialized treatment modality, while rescuing patients, may also incite a range of societal, ethical, and medical resource allocation dilemmas.

Respiratory Support of Extracorporeal Membrane Oxygenation in Children

Abstract:Patients with respiratory crisis occupied a large numbers in extracorporeal membrane oxy-genation(ECMO) group. ECMO is able to give a plenty amount of gas exchange for body essential metabolic needs by ECMO oxygenator whoever the lungs are of function or not. The key indications for respiratory ECMO are that the patient had no response to all medical treatment except ECMO,and the origin cause of the disease is potentially recoverable. Besides the contraindications and accessibility, ECMO has no special settings among the different ages or cause of diseases. The VV-ECMO is preferred for respiratory support. VA-ECMO should be considered if cardiovascular failure exists. ECMO may offer a powerful respiratory support to patient with critically ill conditions until recovery. The outcomes are influenced by some factors including the reversibility and the cause of diseases.
