Abstract:Significance The recent emergence and spread of zoonotic viruses, including Ebola virus and severe acute respiratory syndrome coronavirus 2, demonstrate that animal-sourced viruses are a very real threat to global public health. Virus discovery efforts have detected hundreds of new animal viruses with unknown zoonotic risk. We developed an open-source risk assessment to systematically evaluate novel wildlife-origin viruses in terms of their zoonotic spillover and spread potential. Our tool will help scientists and governments assess and communicate risk, informing national disease prioritization, prevention, and control actions. The resulting watchlist of potential pathogens will identify targets for new virus countermeasure initiatives, which can reduce the economic and health impacts of emerging diseases.

Abstract:Ebola disease (EBOD) remains a significant and ongoing threat to African countries, characterized by a mortality rate of 25% to 90% in patients with high viral load and significant transmissibility. The most recent outbreak, reported in Uganda in September 2022, was declared officially over in January 2023. However, it was caused by the Sudan Ebola virus (SUDV), a culprit species not previously reported for a decade. Since its discovery in 1976, the management of EBOD has primarily relied on supportive care. Following the devastating outbreak in West Africa from 2014 to 2016 secondary to the Zaire Ebola virus (EBOV), where over 28,000 lives were lost, dedicated efforts to find effective therapeutic agents have resulted in considerable progress in treating and preventing disease secondary to EBOV. Notably, 2 monoclonal antibodies—Ebanga and a cocktail of monoclonal antibodies, called Inmazeb—received Food and Drug Administration (FDA) approval in 2020. Additionally, multiple vaccines have been approved for EBOD prevention by various regulatory bodies, with Ervebo, a recombinant vesicular stomatitis virus-vectored vaccine against EBOV being the first vaccine to receive approval by the FDA in 2019. This review covers the key signs and symptoms of EBOD, its modes of transmission, and the principles guiding supportive care. Furthermore, it explores recent advancements in treating and preventing EBOD, highlighting the unique properties of each therapeutic agent and the ongoing progress in discovering new treatments.

Abstract:The wildlife reservoir and spillover mechanisms of the Ebola virus remain elusive despite extensive research efforts in endemic areas. This study employed a One Health approach to examine the virus’ circulation in wildlife and the associated human exposure risks in the Tshuapa province of the Democratic Republic of the Congo. In 2021, we collected 1015 samples from 888 animals, predominantly small mammals, and 380 inhabitants of Inkanamongo village, the site of an Ebola virus disease outbreak in 2014. These samples were screened for evidence of current (RNA) or past (IgG antibodies) Ebola virus infections. We also conducted interviews with 167 individuals in the surrounding districts to assess their interactions with wildlife. While no Ebola virus RNA was detected in the wildlife samples, anti-orthoebolavirus IgG antibodies were found in 13 bats and 38 rodents. Among the human participants, 120 individuals had IgG antibodies against at least one orthoebolavirus antigen, with 12 showing seropositivity for two antigens of the same orthoebolavirus, despite not having a prior Ebola disease diagnosis. Furthermore, over 50% of respondents reported frequent visits to the forest to hunt a variety of wild animals, particularly ungulates and rodents, which could account for occasional viral spillovers. The absence of active Ebola virus circulation in wildlife may reflect seasonal patterns in reservoir ecology, like those observed in bats. Similarly, seasonal human activities, like hunting and foraging, may result in periodic exposure risks. These findings highlight the importance of continuous, multidisciplinary surveillance to monitor changes in seasonal spillover risks.

Abstract:Ebola virus (EBOV) disease is a zoonotic disease that is caused by four species of genus Ebolavirus. The EBOV disease has outbreaks (1976-2022) in West and Central African countries, high mortality rate has made it a public health concern. EBOV is a filamentous virus having negative stranded and non-segmented RNA. The EBOV genome has the unique capability of forming soluble glycoprotein. Since its first emergence in human history, several effective therapeutics and vaccines are developed. These vaccines include replicative or non-replicative vectored vaccines, DNA vaccines and monovalent or polyepitopic vaccines. This article reviewed the structure of EBOV and pathophysiology as well as the immune response of EBOV infection. In addition to diagnosis and vaccinations available to cure EBOV at acute and chronic stages. Abstract:Over the past 30 years, Zaire and Sudan ebolaviruses have been responsible for large hemorrhagic fever (HF) outbreaks with case fatalities ranging from 53% to 90%, while a third species, Côte d'Ivoire ebolavirus, caused a single non-fatal HF case. In November 2007, HF cases were reported in Bundibugyo District, Western Uganda. Laboratory investigation of the initial 29 suspect-case blood specimens by classic methods (antigen capture, IgM and IgG ELISA) and a recently developed random-primed pyrosequencing approach quickly identified this to be an Ebola HF outbreak associated with a newly discovered ebolavirus species (Bundibugyo ebolavirus) distantly related to the Côte d'Ivoire ebolavirus found in western Africa. Due to the sequence divergence of this new virus relative to all previously recognized ebolaviruses, these findings have important implications for design of future diagnostic assays to monitor Ebola HF disease in humans and animals, and ongoing efforts to develop effective antivirals and vaccines.

Abstract:As we all know, the Ebola virus is one of the most dangerous pathogens in the world. The outbreak of the Ebola virus diseases, began in December 2013, Guinea, West Africa, was the most serious one ever recorded in the history of mankind. Due to underestimation of the epidemic and the lack of effective drug, the effect of the prevention and control of this epidemic was not optimistic. ZMapp and TKM-Ebola were emergently approved by FDA for the treatment of the Ebola virus disease. However, the signiifcance of the use of two drugs was limited because of the unknown efifcacy, safety and the shortage of production capacity. In this paper, the existing therapies and the latest research progress of new drugs for Ebola virus diseases are reviewed so as to help the relevant persons get this information.

Abstract:Ebolavirus (EBOV) has caused disease outbreaks taking thousands of lives, costing billions of dollars in control efforts and threatening great ape populations. EBOV ecology is not fully understood but infected wildlife and consumption of animal carcasses have been linked to human outbreaks, especially in the Congo Basin. Partnering with the Congolese Ministry of Health, we conducted wildlife mortality surveillance and educational outreach in the northern Republic of Congo (RoC). Designed for EBOV detection and to alert public health authorities, we established a low-cost wildlife mortality reporting network covering 50 000 km2. Simultaneously, we delivered educational outreach promoting behavioural change to over 6600 people in rural northern RoC. We achieved specimen collection by training project staff on a safe sampling protocol and equipping geographically distributed bases with sampling kits. We established in-country diagnostics for EBOV testing, reducing diagnostic turnaround time to 3 days and demonstrated the absence of EBOV in 58 carcasses. Central Africa remains a high-risk EBOV region, but RoC, home to the largest remaining populations of great apes, has not had an epidemic since 2005. This effort continues to function as an untested early warning system in RoC, where people and great apes have died from past Ebola virus disease outbreaks. Abstract:The crisis generated by the emergence and pandemic spread of COVID-19 has thrown into the global spotlight the dangers associated with novel diseases, as well as the key role of animals, especially wild animals, as potential sources of pathogens to humans. There is a widespread demand for a new relationship with wild and domestic animals, including suggested bans on hunting, wildlife trade, wet markets or consumption of wild animals. However, such policies risk ignoring essential elements of the problem as well as alienating and increasing hardship for local communities across the world, and might be unachievable at scale. There is thus a need for a more complex package of policy and practical responses. We undertook a solution scan to identify and collate 161 possible options for reducing the risks of further epidemic disease transmission from animals to humans, including potential further SARS-CoV-2 transmission (original or variants). We include all categories of animals in our responses (i.e. wildlife, captive, unmanaged/feral and domestic livestock and pets) and focus on pathogens (especially viruses) that, once transmitted from animals to humans, could acquire epidemic potential through high rates of human-to-human transmission. This excludes measures to prevent well-known zoonotic diseases, such as rabies, that cannot readily transmit between humans. We focused solutions on societal measures, excluding the development of vaccines and other preventive therapeutic medicine and veterinary medicine options that are discussed elsewhere. We derived our solutions through reading the scientific literature, NGO position papers, and industry guidelines, collating our own experiences, and consulting experts in different fields. Herein, we review the major zoonotic transmission pathways and present an extensive list of options. The potential solutions are organised according to the key stages of the trade chain and encompass solutions that can be applied at the local, regional and international scales. This is a set of options targeted at practitioners and policy makers to encourage careful examination of possible courses of action, validating their impact and documenting outcomes.

Abstract:Zoonotic diseases spread through pathogens-infected animal carriers. In the case of Ebola Virus Disease (EVD), evidence supports that the main carriers are fruit bats and non-human primates. Further, EVD spread is a multi-factorial problem that depends on sociodemographic and economic (SDE) factors. Here we inquire into this phenomenon and aim at determining, quantitatively, the Ebola spillover infection exposure map and try to link it to SDE factors. To that end, we designed and conducted a survey in Sierra Leone and implement a pipeline to analyze data using regression and machine learning techniques. Our methodology is able (1) to identify the features that are best predictors of an individual's tendency to partake in behaviors that can expose them to Ebola infection, (2) to develop a predictive model about the spillover risk statistics that can be calibrated for different regions and future times, and (3) to compute a spillover exposure map for Sierra Leone. Our results and conclusions are relevant to identify the regions in Sierra Leone at risk of EVD spillover and, consequently, to design and implement policies for an effective deployment of resources (e.g., drug supplies) and other preventative measures (e.g., educational campaigns).

Abstract:Mpox is a zoonotic illness caused by the Mpox virus (MPXV), a member of the Orthopoxvirus family. Although a few cases have been reported outside Africa, it was originally regarded as an endemic disease limited to African countries. However, the Mpox outbreak of 2022 was remarkable in that the infection spread to more than 123 countries worldwide, causing thousands of infections and deaths. The ongoing Mpox outbreak has been declared as a public health emergency of international concern by the World Health Organization. For a better management and control of the epidemic, this review summarizes the research advances and important scientific findings on MPXV by reviewing the current literature on epidemiology, clinical characteristics, diagnostic methods, prevention and treatment measures, and animal models of MPXV. This review provides useful information to raise awareness about the transmission, symptoms, and protective measures of MPXV, serving as a theoretical guide for relevant institutions to control MPXV.

Forty years of the war against Ebola

Abstract:Humans have been fighting against the Ebola virus disease (EVD) since its first outbreak in 1976 in southern Sudan and Yambuku in Zaire which lies on the Ebola River. According to the data from the World Health Organization (WHO, 2014b), the first outbreak claimed 431 lives in 1976, and the disease awoke transiently in Sudan three years later and then disappeared for 15 years afterwards. Following that, large outbreaks appeared in 1995 in Zaire with 250 deaths of people, 2001-2002 in Uganda with 224 deaths, 2002-2003 in Congo with 128 deaths, and 2007 in Congo with 187 deaths. In 2014, the most severe and complicated outbreak swept through the West African countries having already taken 1069 lives, with the situation seeming to be out of control. To date, there have been 15 outbreaks in Africa, which have caused 4362 infected cases and claimed 2659 lives. The pandemics of Ebola show obvious independence from any season. Humans are generally susceptible to the Ebola virus without gender or age variation. The natural reservoir of the Ebola virus still remains unclear. During the past 40 years or so, the EVD disappeared after an outbreak in one region and erupted in another region without any warning. The difficulty in understanding the spreading pattern of Ebola was compared to that of the wave-particle duality of light.
Knowledge, perceptions and effects of Ebola virus disease outbreak on the pig value chain in the agro-pastoralist district of Luwero, Central Uganda

Abstract:Background: Ebola Virus Disease (EVD) outbreaks have a significant impact on the health and wellbeing, and livelihoods of communities. EVD response interventions particularly affect the food value chain, and income security of pig farmers in agro-pastoral communities. Despite the enormous effort of EVD response interventions, there is paucity of information towards EVD among those involved in the pig value chain, as well as the effect of EVD outbreaks on the pig value chain. This study therefore, assessed the knowledge, perceptions on the occurrence of Ebola and its effects on the pig value chain in the agro-pastoral district of Luweero, Central Uganda. Methods: A cross sectional study was conducted in two parishes of Ssambwe and Ngalonkulu, Luwero district. A total of 229 respondents were included in the study. Structured questionnaires, key informant interviews and focus group discussions were conducted to collect data. Quantitative data was analysed using SPSS version 22 while qualitative data was analysed using thematic content analysis. Results: Of the 229 respondents, 95.6% could recall the occurrence of the last EVD outbreak in their locality. About 24.5% associated EVD with touching pigs or eating pork. Regarding knowledge, 194 (84.7%) correctly associated EVD with handling Ebola infected persons, 191 (83.4%) with migration of people from endemic areas, 148 (64.9%) eating monkey meat, 127 (55.5%) with eating bats, and 198 (64.9%) with conducting public meetings where there is an Ebola infected person. Out of 142 farmers, 55 (38.7%) believed that Ebola outbreaks affected demand and sale of pigs. The EVD outbreak significantly led to a reduction in the average number of pigs sold (P = 0.001), the average number of pigs bought by traders (P = 0.04), and the number of pigs sold/ slaughtered by butcher men at pork eating places (P = 0.03). Conclusion: This study showed that EVD outbreak negatively affected the pig value chain i.e., the demand and supply of pigs and pork. Therefore, there is need to sensitize the stakeholders in the pig value chain on EVD in order to minimize the negative economic impacts associated with EVD outbreaks.

Abstract:The devastating impact of Coronavirus Disease 2019 (COVID-19) on human health globally has prompted extensive discussions on how to better prepare for and safeguard against the next pandemic. Zoonotic spillover of pathogens from animals to humans is recognized as the predominant cause of emerging infectious diseases and as the primary cause of recent pandemics [1]. This spillover risk is increased by a range of factors (called drivers) that impact the nature, frequency, and intensity of contact between humans and wild animals. Many of these drivers are related to human impact, for example, deforestation and changes in land use and agricultural practices. While it is clear that the triad of prevention-preparedness-response (P-P-R) is highly relevant, there is much discussion on which of these 3 strategic activities in the field of emerging infectious disease should be prioritized and how to optimally target resources. For this, it is important to understand the scope of the respective activity and the consequences of prioritization. Already, the World Bank Pandemic Fund and forthcoming global Pandemic instrument [2] negotiated by the World Health Organization (WHO) [3] appear primarily focused on the early detection, and reaction to the appearance of human illnesses, often with explicit focus only on action to be taken once pathogen spillover and spread have occurred. Strategies to reduce the probability of spillover events are under-prioritized and underutilized, as highlighted by recent infectious disease crises such as Ebola and Mpox epidemics, and have been lost in overall preparedness discussions and recovery financing. This "more of the same" focus suggests that it is politically more expedient to allocate financial resources to deal with a problem once it has arisen, rather than taking the steps necessary to reduce the risk of it occurring in the first place. It is often claimed that allocating resources to prevent something from happening is politically difficult as the value of prevention is largely "invisible" (prevention paradox) or it will take a long time to show effects. However, there are now several communications highlighting the economic benefits of prevention of spillover [1,4,5]. If taken, actions to prevent spillover are estimated at 4 billion per year in social benefits from reduced greenhouse gas emissions [4]. COVID-19 has demonstrated the immense burden of a pandemic, including significant mortality resulting in economic recession, with the global economy contracting by 4.4 percent in 2020. The expected economic losses from this pandemic are estimated at nearly 52 billion; the Ebola virus disease outbreak in West Africa in 2014 to 2016 with a GDP loss of 53.19 billion [8]; and the 2015 to 2016 Zika virus disease outbreak with an estimated loss in the United States, Caribbean, and Latin America of $20 billion [9,10]. If invested in, prevention strategies would reduce the likelihood of another pandemic substantially and likely generate sufficient return on investment over time while also having the potential to generate substantial co-benefits [1,10]. Prevention is already valued in other sectors: policymakers and industries have led on prevention in other areas, such as expenditure on counter-terrorism, driving laws and insurance incentives to reduce the frequency of traffic accidents, on the nuclear deterrent, and in some cases on flood prevention and other water management measures, exemplifying a political willingness to spend vast sums of money to preempt a harmful event in certain areas or circumstances, but not on pandemic prevention.

Abstract:The increasing frequency of zoonotic spillover events and viral mutations in low and middle-income countries presents a critical global health challenge. Contributing factors encompass cultural practices like bushmeat consumption, wildlife trade for traditional medicine, habitat disruption, and the encroachment of impoverished settlements onto natural habitats. The existing "vaccine gap" in many developing countries exacerbates the situation by allowing unchecked viral replication and the emergence of novel mutant viruses. Despite global health policies addressing the root causes of zoonotic disease emergence, there is a significant absence of concrete prevention-oriented initiatives, posing a potential risk to vulnerable populations. This article is targeted at policymakers, public health professionals, researchers, and global health stakeholders, particularly those engaged in zoonotic disease prevention and control in low and middle-income countries. The article underscores the importance of assessing potential zoonotic diseases at the animal-human interface and comprehending historical factors contributing to spillover events. To bridge policy gaps, comprehensive strategies are proposed that include education, collaborations, specialized task forces, environmental sampling, and the establishment of integrated diagnostic laboratories. These strategies advocate simplicity and unity, breaking down barriers, and placing humanity at the forefront of addressing global health challenges. Such a strategic and mental shift is crucial for constructing a more resilient and equitable world in the face of emerging zoonotic threats.

Abstract:Proposed mechanisms of zoonotic virus spillover often posit that wildlife transmission and amplification precede human outbreaks. Between 2006 and 2012, the palm Raphia farinifera , a rich source of dietary minerals for wildlife, was nearly extirpated from Budongo Forest, Uganda. Since then, chimpanzees, black-and-white colobus, and red duiker were observed feeding on bat guano, a behavior not previously observed. Here we show that guano consumption may be a response to dietary mineral scarcity and may expose wildlife to bat-borne viruses. Videos from 2017–2019 recorded 839 instances of guano consumption by the aforementioned species. Nutritional analysis of the guano revealed high concentrations of sodium, potassium, magnesium and phosphorus. Metagenomic analyses of the guano identified 27 eukaryotic viruses, including a novel betacoronavirus. Our findings illustrate how "upstream" drivers such as socioeconomics and resource extraction can initiate elaborate chains of causation, ultimately increasing virus spillover risk.

Abstract:Abstract Background On 20 September 2022, the Ugandan Ministry of Health declared an outbreak of Ebola disease caused by Sudan ebolavirus. Methods From 6 October 2022 to 10 January 2023, Centers for Disease Control and Prevention (CDC) staff conducted public health assessments at five US ports of entry for travellers identified as having been in Uganda in the past 21 days. CDC also recommended that state, local and territorial health departments (‘health departments’) conduct post-arrival monitoring of these travellers. CDC provided traveller contact information, daily to 58 health departments, and collected health department data regarding monitoring outcomes. Results Among 11 583 travellers screened, 132 (1%) required additional assessment due to potential exposures or symptoms of concern. Fifty-three (91%) health departments reported receiving traveller data from CDC for 10 114 (87%) travellers, of whom 8499 (84%) were contacted for monitoring, 1547 (15%) could not be contacted and 68 (1%) had no reported outcomes. No travellers with high-risk exposures or Ebola disease were identified. Conclusion Entry risk assessment and post-arrival monitoring of travellers are resource-intensive activities that had low demonstrated yield during this and previous outbreaks. The efficiency of future responses could be improved by incorporating an assessment of risk of importation of disease, accounting for individual travellers’ potential for exposure, and expanded use of methods that reduce burden to federal agencies, health departments, and travellers.

Abstract:Ebola virus causes a severe haemorrhagic fever in humans with high case fatality and significant epidemic potential. The 2013–2016 outbreak in West Africa was unprecedented in scale, being larger than all previous outbreaks combined, with 28 646 reported cases and 11 323 reported deaths. It was also unique in its geographical distribution and multicountry spread. It is vital that the lessons learned from the world's largest Ebola outbreak are not lost. This article aims to provide a detailed description of the evolution of the outbreak. We contextualize this outbreak in relation to previous Ebola outbreaks and outline the theories regarding its origins and emergence. The outbreak is described by country, in chronological order, including epidemiological parameters and implementation of outbreak containment strategies. We then summarize the factors that led to rapid and extensive propagation, as well as highlight the key successes, failures and lessons learned from this outbreak and the response.
