Abstract:A case of drug hypersensitivity syndrome caused by allopurinol was reported.A 49-year-old man suffering from gout presented with skin rash over many parts of the body a few days after stopping oral allopurinol which was intaken for 15 days.Skin examination:clustering red maculopapule and papulovesicle,some of which formed plaques,were found over face,neck,trunk and limbs.Purpura-like skin rash were seen over both lower extremities.Histological examination of the skin lesion showed that serious edema in dermal papillary layer,and some had formed subepidermal blister.Perivascular lymphocytes infiltration was seen in superficial layer of dermis.

Abstract:A case of acute generalized exanthematous pustulosis(AGEP) mimicking toxic epidermal necrolysis(TEN) is reported. A 36-year-old man presented with widespread erythema and pustules that appeared 5 days after starting the therapy with paracetamol tablets and intravenous cefradine. Most of pustules coalesced to form lakes of pus, and widespread skin detachment could be seen, mimicking the clinical picture of TEN. Histopathology showed the characteristics of acute generalized exanthematous pustulosis(AGEP). The patient was cured by systemic corticosteroid and vancomycin treatment.

Abstract:Background: Acute generalized exanthematous pustulosis (AGEP) is a severe cutaneous adverse reaction characterized by sterile pustules on erythematous skin associated with fever and leukocytosis. The annual incidence of AGEP is estimated to be 1-5 cases per million. Cases of AGEP with oral mucosa involvement have been reported. However, reports of AGEP involving airway mucosa are limited. Case summary: We report a 42-year-old woman with serious AGEP involving the airway mucosa. The patient initially developed fever and a small rash on her forehead and face. Over the next 2 d, she developed a diffuse, pustular rash over her trunk and legs. In addition, she complained of a cough with white foam-like sputum, chest tightness and dyspnea. Four days later, due to dyspnea, her mental status started to gradually deteriorate. She became more and more drowsy. Biopsies of the skin and airway mucosa suggested the diagnosis of AGEP. According to the European study of severe cutaneous adverse reactions group's scoring system, the patient scored +6 indicating a probable diagnosis of AGEP. She received intravenous methylprednisolone 120 mg/12 h for 3 d, and was eventually discharged in good condition. This patient had already experienced respiratory failure and airway mucosa involvement on admission; however, the clinicians had an insufficient understanding of AGEP. Glucocorticoid was administered for more than 10 d following onset of the disease, and her overall prognosis was satisfactory. Conclusion: This case represents a rare clinical feature of AGEP and an important finding for clinicians.
A case report of Paracetamol related pyroglutamic acidosis: mind the gap in a malnourished patient

Abstract:Background: Pyroglutamic acidosis is a rare cause of high anion gap metabolic acidosis. Most cases of paracetamol related pyroglutamic acidosis are described in malnourished women and patients with kidney/liver failure, alcohol use or severe sepsis. In this report, we describe how pyroglutamic acidosis could be related to the use of chronic therapeutic paracetamol with only malnutrition as an associated risk factor. Case presentation: We report a case of a 67-year-old male patient developing a pyroglutamic acidosis. The patient was initially admitted to hospital for infectious osteoarthritis and developed a metabolic acidosis during his hospital stay. Analgesics included daily therapeutic doses of paracetamol. What makes our case unusual is that our malnourished male patient did not have renal or hepatic failure. The diagnosis of paracetamol related pyroglutamic acidosis was made after ruling out the main causes of metabolic acidosis. It was further confirmed by urine organic acids measurement showing a markedly elevated level of pyroglutamic aciduria. Paracetamol was discontinued allowing a prompt correction of the anion gap. Conclusion: This case is a representative of pyroglutamic acidosis related to chronic therapeutic paracetamol with only malnutrition as an associated risk factor. Physicians should be aware of such unusual cause of metabolic acidosis, which may be more common than expected in hospitalized patients. A high clinical suspicion is needed when urine organic acids analysis is not available.

Abstract:Cutaneous adverse drug reactions vary in severity, from mild erythematous skin lesions to life-threatening toxic epidermal necrolysis. Antiepileptics, antipsychotics, antimicrobials, and diuretics are associated with these adverse reactions. We report a case of cefditoren pivoxil-induced toxic epidermal necrolysis in a 46-year-old Indian woman who presented initially with a maculopapular rash that eventually progressed to steroid-refractory toxic epidermal necrolysis. The patient succumbed to her illness on day 33 of the index rash following septic shock.

As the disease is often associated with conditions such as inflammatory bowel disease (IBD), rheumatoid arthritis, haematological disorders and other diseases, diagnosis and treatment often require cooperation with other medical departments. Accordingly, dissemination of information about the disease to doctors in departments other than dermatologists, especially paediatricians, can help in its early detection. Case Presentation: The 11-year-old pediatric patient in the case initially diagnosed with acute febrile neutrophilic dermatosis was eventually confirmed as pustular PG through histopathological examinations of skin and other relevant examinations. The medical condition is lessened after treatment with a combination of glucocorticoids and adalimumab. Conclusion: PG is relatively rare in clinical settings, particularly among pediatric patients exhibiting persistent high fever and signs of pustular pyoderma gangrenosum. This case underscores the importance of considering the potential diagnosis of pediatric pustular PG when confronted with a child presenting persistent high fever and pustules after trauma. Additionally, the proactive initiation of adalimumab emerges as a promising treatment option for pediatric IBD -associated pustular PG. Keywords: pustular pyoderma gangrenosum, acute febrile neutrophilic dermatosis, misdiagnosis, adalimumab Pustular PG represents a rare etiology of skin ulcers in pediatric population, accounting for approximately 4% of global cases. 1 The ulcerative (classic) subtypepredominates, typically addressed as the first-line treatment with systemic corticosteroids. However, the occurrence of the blistering variant of PG, especially in children, is relatively uncommon. The absence of well-defined diagnostic criteria, encompassing specific clinical, histopathological, and laboratory findings, contributes to the potential for misdiagnosis. 2 The most common underlying etiologies in pediatric PG are inflammatory bowel diseases, hematologic malignancies, immune deficiencies and vasculitis. Thorough clinical examination is necessary to clarify the diagnosis and select appropriate treatment methods. 3 Here, we present the case of an 11-year-old pediatric patient who was initially misdiagnosed with acute febrile neutrophilic dermatosis. Subsequent skin histopathology and relevant examinations confirmed the diagnosis of pustular PG. The timely initiation of a treatment combining glucocorticoids and Tumour necrosis factor antagonists (adalimumab) yielded notable improvement in systemic symptoms. An 11-year-old patient suffered a knee injury on May 6, 2023, followed by the onset of fever on May 9. Subsequently, on May 10, the patient developed pustules on the upper lip and sought medical attention at an external institution. The initial diagnosis was "bullous impetigo and ulcerative stomatitis", and the patient received treatment involving intravenous drip administration of metronidazole and levofloxacin, along with the use of chlorhexidine mouthwash. However, the condition progressively deteriorated, leading to ulceration of the oral mucosa and difficulties in eating. On May 13, erythematous patches appeared on the patient's back, accompanied by the development of pustules. The rash extended progressively to involve the face, limbs, anterior chest and buttocks. Persistent fever persisted throughout this period, reaching a maximum temperature of 39.8°C. Empirical treatment attempts with azithromycin, penicillin, and linezolid did not result in any improvement and the condition continued to worsen. On May 21, the patient was admitted to our department with the provisional diagnoses of "unexplained fever" and "acute febrile neutrophilic dermatosis". The patient also presented with ongoing diarrhea and had recently undergone an 8kg weight loss. There was no significant medical history, allergies, or pertinent family medical history in the patient's background. An Atypical Presentation of Acute Generalized Exanthematous Pustulosis Induced by Use of Meropenem: A Case Report and Literature Review

Abstract:Acute generalized exanthematous pustulosis (AGEP) is a rare dermatological disorder characterized by the development of sterile pustular lesions on erythematous and edematous skin, followed by desquamation of the affected regions. The majority of cases occur following the administration of antimicrobials and antihypertensives. Our case report illustrates the atypical presentation of AGEP in an elderly female following treatment with meropenem for an infected pressure ulcer. Only a limited number of meropenem-associated AGEP have been documented. Our patient's presentation also highlights the difficulty of diagnosing AGEP and its similarity to other rare, drug-associated rashes. By illustrating this atypical clinical presentation of AGEP and highlighting its association with meropenem, we hope to provide insight to clinicians dealing with similar presentations.
Terbinafine-induced acute generalized exanthematous pustulosis

Abstract:Terbinafine is an allylamine antifungal agent, effective in the treatment of dermatomycoses. Many cutaneous adverse reactions have been reported (in about 3% of treated patients). Furthermore terbinafine has been associated with pustular eruptions, as well as the induction and exacerbation of pre-existing psoriasis and acute generalized exanthematous pustulosis (AGEP). AGEP is an uncommon aseptic pustular eruption, classified for many years as a pustular psoriasis, that usually follows recent administration of oral or parenteral drugs. The disease is most frequently triggered by antibiotics, most of all aminopenicillins and macrolides. Characteristic AGEP features include the sudden onset of fever above 38 C with widespread erythematous eruption, rapidly progressing to a fine, non-follicular, micropustular rash. Leucocytosis is generally present, sometimes associated with eosinophilia. The illness usually resolves spontaneously with the fever and the pustulation clearing within 15 days, sometimes followed by desquamation. Hystopathology shows non-follicular spongiotic pustules in the epidermis filled with neutrophils, a mixed perivascular infiltrate of neutrophils and occasional eosinophils with papillary dermal oedema. On this subject, Sideroff et al. recently elaborated a validation score based on morphology, histological criteria, and disease course. The pathogenetic mechanism which leads to the induction of AGEP by some medicines has still not been clarified, but T cells seem to play a crucial role. The authors report a case of a patient with terbinafine-induced AGEP and a review of the literature about this topic. The case illustrates once again the role of terbinafine in AGEP and reminds us that early diagnosis of AGEP is important to avoid unnecessary investigations and/or the administration of antibiotics.

Abstract:Acute generalized exanthematous pustulosis (AGEP) is an uncommon eruption characterized by sterile pustules on an erythematous background, which is usually associated with drugs. AGEP is described as a self-limiting disease with favorable prognosis. We reported a case of Kawasaki Disease (KD) following AGEP. A 3-year-old male, who was admitted with pustules and five days of fever at our hospital, was diagnosed with AGEP. Despite the skin lesions and fever improving drastically after prednisolone therapy, the fever recurred on hospitalization day 5. The following symptoms suggestive of KD also appeared: bulbar conjunctival hyperemia, cervical lymphadenopathy, erythema of the lips, eruption on his trunk, and erythema and edema of the hands and feet. He was diagnosed with KD and treated with intravenous immunoglobulin. He was discharged on the thirteenth day of hospitalization without cardiac complications. Drug-induced lymphocyte stimulation test revealed carbocysteine as the suspected cause of AGEP, which consequently triggered KD. Because a mucosal lesion is uncommon in AGEP, bulbar conjunctival hyperemia suggested that KD sequentially occurred after AGEP. Since AGEP is benign and self-limited in most cases, it is necessary to differentiate other diseases, including KD, when recurrent fever or rash occurs in the course of AGEP.

[Evaluation of quality assurance program in high dose rate remote after-loading brachytherapy].

Abstract:In the past 9 years, we have routinely used high-dose-rate remote afterloading systems, a Toshiba RAL-302-2 remote afterloading system and a self-fabricated Ir-192 system, for radiation therapy of various cancers, particularly cancers of the uterine cervix and nasopharynx. The frequency of using these two systems is more than 30 times per week. These machines can deliver high-dose radiation (300-1000 cGy) to a specifically planned target on the patient in a few minutes, therefore, the performance of the systems requires well planned dosimetry and quality control assurance in order to achieve effective therapeutic results and ensure safety of the patient under treatment. Any malfunction of the machines may cause over-radiation that exceeds a planned dose to the patient, and any distortion in the distribution of a planned dose may give an unexpected high dose of radiation to the therapist operating the machines. Hence, a quality assurance program must be enforced regularly to avoid or minimize any possible accidents. Based on information and recommendations reported in the literature, as well as from our own experience, we set up a program of routine checks to correct and adjust the machinery conditions and to ensure the optimum operational status of the equipment on a weekly, monthly, semiannual and annual basis. In checking the Co-60 machine, we found that the rate of occurrence for abnormal source position was 3.9% and that for the timer was 2.8%. In the Ir-192 machine checks, abnormal conditions were found only in the source position at the rate of 7.8%.

Abstract:Acute generalized exanthematous pustulosis, an infrequent adverse drug reaction, mainly results from drugs. Clinically, acute generalized exanthematous pustulosis manifests as a high fever, with skin lesions of small monomorphic subcorneal sterile pustules on an erythematous that presents at 1-4 days after medication exposure. The incidence of acute generalized exanthematous pustulosis varies from 3/1, 000, 000 to 5/1, 000, 000, while the mortality rate is typically around 5%. We present a case of a 69-year-old female who developed a diffuse, erythematous, pustular rash over the entire body and exhibited a fever of 38.3°C after 4 days of icotinib therapy. Considering her medication history and the appearance of the lesions, she was diagnosed with acute generalized exanthematous pustulosis and received appropriate treatment. We also conducted a literature review through PubMed to compare similarities and differences between our case and those reported in the literature.

Abstract:Acute generalized exanthematous pustulosis (AGEP) is a rare, usually drug‐induced, acute pustular rash. Despite the lack of strong data supporting the effectiveness of topical or systemic corticosteroids in this drug reaction, they are widely used. More generally, there is no consensus on the diagnostic modalities and the management of patients with AGEP. We aimed to provide European expert recommendations for the diagnosis and management or patients with AGEP. Members of the ToxiTEN group of the European Reference Network (ERN)‐skin, all dermatologists and/or allergologists with expertise in drug reactions, elaborated these recommendations based on their own experience and on a review of the literature. Recommendations were separated into the following categories: professionals involved, assessment of the diagnosis of AGEP, management of the patient and allergological work‐up after the acute phase. Consensus was obtained among experts for the list of professionals involved for the diagnosis and management of AGEP, including the minimum diagnostic work‐up, the setting of management, the treatments, the modalities and the timing of allergological work‐up and follow‐up. European experts in drug allergies propose herein consensus on the diagnosis and management of patients with AGEP. A multidisciplinary approach is warranted, including dermatologists, allergologists and pharmacovigilance services.

Drug-induced Toxic Epidermal Necrolysis with Secondary Aspergillus Fumigatus Infection: a Case Report

Abstract:Among the various drug induced dermatological entities toxic epidermalnecrolysis (TEN) and Stevens-Johnson syndrome (SJS) occupy a primary place in terms of mortality. Toxic epidermal necrolysis also known as Lyell's syndrome was first described by Lyell in 1956. Drugs are by far the most common cause of toxic epidermal necrolysis, in which large sheets of skin are lost from the body surface making redundant the barrier function of the skin, with its resultant complications. Drug-induced toxic epidermal necrolysis are severe adverse cutaneous drug reactions to various precipitating agents that predominantly involve the skin and mucous membranes. Toxic epidermal necrolysis is rare but considered medical emergencies as they are potentially fatal. Drugs are the most common cause accounting for about 65%-80% of the cases. The most common offending agents are sulfonamides, NSAIDs, butazones and hydrantoins. An immune mechanism is implicated in the pathogenesis, but its nature is still unclear. There is a prodormal phase in which there is burning sensation all over the skin and conjunctivae, along with skin tenderness, fever, malaise and arthralgias. Early sites of cutaneous involvement are the presternal region of the trunk and the face, but also the palms and soles, rapidly spread to their maximum extent, the oral mucosa and conjunctiva being affected. Initial lesions are macular, followed by desquamateion, or may be from atypical targets with purpuriccenters that coalesce, from bullae, then slough. The earlier a causative agent is withdrawn the better is the prognosis. Several treatment modalities given in addition to supportive care are reported in the literature, such as systemicsteroids, high-dose intravenous immunoglobulins, ciclosporin, TNF antagonists. Recovery is slow over a period of 14-28 days and relapses are frequent. Mortality is 25%-50% and half the deaths occur due to secondary infection. Here we report a 50-year-old female of drug-induced toxic epidermal necrolysis. She was admitted to the dermatology ward with extensive peeling of skin over the trunk and limbs. She had taken alamotrigine for epilepsy. A week after taking the tablets, the patient developed a severe burning sensation all over the body and followed by a polymorphic erythematous dermatitis and widespread peeling of skin. We treated this patient with high dose corticosteroids, high-dose intravenous immunoglobulins and etanercept, but eventually she died of secondary aspergillus fumigatus infection.

Abstract:BACKGROUND We hypothesized that adenovirus-mediated transfer of the vascular endothelial growth factor (VEGF121) complementary DNA (cDNA) to murine laparotomy fascial wounds would enhance vascularity and bursting strength. METHODS Microfibrillar collagen sponges saturated with adenovirus (Ad) vectors encoding for the human VEGF121 cDNA (Ad(CU)VEGF121.1), a control marker gene (Ad beta gal, AdLuc) or no transgene (AdNull) were sutured to fascial edges during laparotomy closure in normal mice and mice treated with dexamethasone. Endpoints addressed included transgene expression in the fascia and surrounding tissue, the number of blood vessels in the healing wound determined using immunostaining, and wound bursting strength using a calibrated tensinometer. RESULTS Transgene expression was detected readily in the fascial edges, but only marginally detectable in neighboring tissues. In normal mice and mice treated with dexamethasone, no differences were observed at 7 days. Strikingly, however, 21 days after wound closure/therapy, significantly more blood vessels were present in the wounds that received the VEGF121 vector compared with controls (normal: AdNull: 4.2 +/- 1.8; Ad(CU)VEGF121.1: 11.2 +/- 1.2; P <.05; dexamethasone: AdNull: 1.4 +/- 0.8; Ad(CU)VEGF121.1: 5.4 +/- 1.2; P <.05), and bursting strength was significantly higher in VEGF121-treated wounds (normal: AdNull: 665 +/- 68 mN/mm; Ad(CU)VEGF121.1: 956 +/- 82 mN/mm; P <.0005; dexamethasone: AdNull: 234 +/- 111 mN/mm; Ad(CU)VEGF121.1: 592 +/- 121 mN/mm; P <.03). CONCLUSIONS Adenovirus-mediated gene transfer to healing fascial wounds is achieved readily using a microfibrillar collagen sponge, with transfer of the human VEGF121 cDNA significantly enhancing wound vascularity and bursting strength in normal mice, as well as in mice treated with dexamethasone.

The main causes are drug and viral infections. Unfortunately, no expert consensus on treating this disease exists, and a standard therapy is absent. Up to now, glucocorticoids combined with gamma globulin are commonly used in clinical practice, but their efficacy is highly controversial. This study reports on a 7-year-old girl with TEN who did not respond to traditional therapy, such as methylprednisolone combined with gamma globulin, but was finally cured with an additional low-dose etanercept. The results showed that etanercept therapy in paediatric TEN is safe, reliable and worth recommending. Keywords: child toxic epidermal necrolysis, etanercept, glucocorticoids, gamma globulin Toxic epidermal necrolysis (TEN) is a potentially life-threatening delayed hypersensitivity reaction primarily triggered by medications. It exhibits rapid onset and severe progression, with a 30–35% mortality rate. 1 Toxic epidermal necrolysis can affect individuals of all ages and is linked to various medications and infections. Causative medications for TEN include antibiotics, anticonvulsants, sulphonamides and allopurinol. The typical clinical manifestations of TEN are widespread erythema, blistering and severe epidermal necrosis and detachment, sometimes leading to multi-organ involvement. Despite the growing understanding of the condition, consensus regarding the optimal treatment regimen for TEN remains elusive. Clinically, conventional treatments such as cyclophosphamide, cyclosporine and glucocorticoid (GC) therapy are often employed but are accompanied by significant side effects and an increased risk of infection. 2,3 Although previous literature has discussed using biological agents in treating adult patients with TEN, scant information exists regarding paediatric patients. 4 To date, only 4 children with toxic epidermal necrolysis have been treated with infliximab and 2 with etanercept. Andrea et al 5 reported an 8-year-old boy who was diagnosed with TEN induced by levetiracetam. He was first treated with methylprednisolone 1.5mg/kg/d and intravenous immunoglobulin 1g/kg/d for 5 consecutive days. However, significant progression and mucosal lesions were observed 2 days later, followed by etanercept at a dose of 0.8mg/kg (25mg), which stopped blistering within 24 hours after the use of etanercept, and the mucosal lesions were alleviated. Gavigan et al 6 reported an 11-year-old girl whose symptoms were significantly relieved after receiving methylprednisolone for 4 days 30mg/kg/d, cyclosporine 5mg/kg/d for 3 days, and 2 doses of etanercept 25mg every 24 hours. Although it has been reported that etanercept is effective in the treatment of TEN, there are still few reports about etanercept in the treatment of TEN in children. Therefore, it is necessary to explore the treatment of etanercept in children with TEN and summarize the clinical experience. In our department, we encountered a paediatric patient with TEN and effectively treated her using a combination therapy of etanercept (Yisaipu), GCs, and intravenous immunoglobulin (IVIG). Given the favourable outcomes, a detailed account of this case is provided in this report. A 7-year-old girl was admitted with the chief complaints of fever, rash and lip swelling for 2 days. Two days before admission, the patient developed an unprovoked fever, with the highest recorded temperature reaching 39.6°C. Initial fever management with ibuprofen failed to alleviate the symptoms. The patient did not experience chills or seizures. Subsequently, erythematous patchy skin rashes appeared on her face and ears, gradually spreading throughout her body. Concurrently, the patient developed blisters on her limbs and trunk and complained of itching on her hands and feet. Pronounced lip swelling was also noted (Figure 1A). Abstract:Toxic epidermal necrolysis (TEN) is a life-threatening, blistering dermatitis. It is characterized by fever and the development of mucocutaneous lesions, which lead to necrosis and sloughing of the epidermis. It is commonly triggered by medications and infections. We present the case of a 75-year-old male who presented to the hospital with a fever and widespread exfoliating skin rash involving 41% of his body surface area (BSA). He has a past medical history of gout, hypertension, asthma, and depression. He was recently started on allopurinol by his general practitioner (GP) for hyperuricemia. The condition also involved oral, eye, and pharynx mucosae. He was diagnosed with toxic epidermal necrolysis and was managed with intravenous (IV) hydrocortisone, steroid and antibiotic eye drops, and steroid and antibiotic topical creams. Due to the weak available evidence supporting the use of ciclosporin and intravenous immunoglobulins, this patient was managed with steroid use only. His rash initially worsened, but ultimately, he made a full recovery without any sequelae. The patient was reviewed in the dermatology clinic four weeks post-discharge, and he did not have any residual disease.

Abstract:Abstract We report a case of a patient admitted to intensive care with a pustulating skin eruption and haemodynamic compromise with near immediate response to infliximab. A 16-year-old male with a six-month history of mild plaque psoriasis presented to acute dermatology services with widespread annular pustular eruption, which developed days after a throat infection. Histopathology was consistent with pustular psoriasis. After an initial partial response, the pustules spread and persisted despite 2-month treatment with topical steroids and ciclosporin (dosed to 4 mg/kg). With raised inflammatory markers and repeated growth of Staphylococcus aureus, high dose intravenous flucloxacillin was administered. Four hours post infusion the patient became erythrodermic and haemodynamically unstable, requiring management in ICU. Repeat skin biopsy showed features of psoriasis without clear drug reaction features. Switching classes of antimicrobials and administration of intravenous immunoglobulins led to infection resolution but no significant clinical status change. A multi-disciplinary decision was made to trial intravenous infliximab. This resulted in near-immediate 90% improvement of skin and haemodynamic status. His excellent clinical response has been maintained on adalimumab thus far. Genetic screening for possible mutations associated with pustular psoriasis, including IL36RN and CARD14, are in process.
