Abstract:Metallic materials are among the most crucial engineering materials widely utilized as biomaterials owing to their significant thermal conductivity, mechanical characteristics, and biocompatibility. Although these metallic biomedical implants, such as stainless steel, gold, silver, dental amalgams, Co-Cr, and Ti alloys, are generally used for bone tissue regeneration and repairing bodily tissue, the need for innovative technologies is required owing to the sensitivity of medical applications and to avoid any potential harmful reactions, thereby improving the implant to bone integration and prohibiting infection lea by corrosion and excessive stress. Taking this into consideration, several research and developments in biomaterial surface modification are geared toward resolving these issues in bone-related medical therapies/implants offering a substantial influence on cell adherence, increasing the longevity of the implant and rejuvenation along with the expansion in cell and molecular biology expertise. The primary objective of this review is to reaffirm the significance of surface modification of biomedical implants by enlightening numerous significant physical surface modifications, including ultrasonic nanocrystal surface modification, thermal spraying, ion implantation, glow discharge plasma, electrophoretic deposition, and physical vapor deposition. Furthermore, we also focused on the characteristics of some commonly used biomedical alloys, such as stainless steel, Co-Cr, and Ti alloys.

Abstract:Surface modification is the science of manipulating surface morphology and interfacial properties and also plays a vital role in biomedical implantation. A few of the interfacial properties are biocompatibility, protein adsorption, wettability, cell proliferation, collagen, etc. These properties depend on surface modification strategies and significantly impact the implant response within the host body. Generally, the corrosion, surface wear, and degradation in the physiological environment limit the application of different biomaterials and can address through various surface modification strategies. These surface modifications developed over the years to improve the morphology and interfacial properties to meet the specific functional surface application in biomedical implantation. It can be done through surface roughening, patterning/texturing, coating with different materials, and hybrid modification. Further, the process development for bio-medical application, process capabilities, limitations, challenges, and characterization aspects are correlated to identify the effectiveness of different surface modification strategies. Finally, various innovative biomedical applications and surface characteristics are also present with future scope in the direction of surface modification for biomedical implantation.

Abstract:With the increasing demand for bone implant therapy, titanium alloy has been widely used in the biomedical field. However, various potential applications of titanium alloy implants are easily hampered by their biological inertia. In fact, the interaction of the implant with tissue is critical to the success of the implant. Thus, the implant surface is modified before implantation frequently, which can not only improve the mechanical properties of the implant, but also polish up bioactivity and osseoconductivity on a cellular level. This paper aims at reviewing titanium surface modification techniques for biomedical applications. Additionally, several other significant aspects are described in detail in this article, for example, micromorphology, microstructure evolution that determines mechanical properties, as well as a number of issues concerning about practical application of biomedical implants.

Abstract:Ti is widely used as a material for orthopedic implants. As rapid and effective osseointegration is a key factor for the successful application of implants, biologically inert Ti materials start to show inherent limitations, such as poor surface cell adhesion, bioactivity, and bone-growth-inducing capabilities. Surface modification can be an efficient and effective approach to addressing the biocompatibility, mechanical, and functionality issues of the various Ti implant materials. In this study, we have overviewed more than 140 papers to summarize the recent progress in the surface modification of Ti implants by physical and/or chemical modification approaches, aiming at optimizing their wear resistance, biocompatibility, and antimicrobial properties. As an advanced manufacturing technology for Ti and Ti alloys, additive manufacturing was particularly addressed in this review. We also provide an outlook for future research directions in this field as a contribution to the development of advanced Ti implants for biomedical applications.

Abstract:Implant surface modification can improve osseointegration and reduce peri-implant inflammation. Implant surfaces are modified with metals because of their excellent mechanical properties and significant functions. Metal surface modification is divided into metal ions and nanoparticle surface modification. These two methods function by adding a finishing metal to the surface of the implant, and both play a role in promoting osteogenic, angiogenic, and antibacterial properties. Based on this, the nanostructural surface changes confer stronger antibacterial and cellular affinity to the implant surface. The current paper reviews the forms, mechanisms, and applications of nanoparticles and metal ion modifications to provide a foundation for the surface modification of implants.

Abstract:The response of host organism in macroscopic, cellular and protein levels to biomaterials is, in most cases, closely associated with the materials’ surface properties. In tissue engineering, regenerative medicine and many other biomedical fields, surface engineering of the bio-inert synthetic polymers is often required to introduce bioactive species that can promote cell adhesion, proliferation, viability and enhanced ECM-secretion functions. Up to present, a large number of surface engineering techniques for improving biocompatibility have been well established, the work of which generally contains three main steps: (1) surface modification of the polymeric materials; (2) chemical and physical characterizations; and (3) biocompatibility assessment through cell culture. This review focuses on the principles and practices of surface engineering of biomedical polymers with regards to particular aspects depending on the authors’ research background and opinions. The review starts with an introduction of principles in designing polymeric biomaterial surfaces, followed by introduction of surface modification techniques to improve hydrophilicity, to introduce reactive functional groups and to immobilize functional protein molecules. The chemical and physical characterizations of the modified biomaterials are then discussed with emphasis on several important issues such as surface functional group density, functional layer thickness, protein surface density and bioactivity. Three most commonly used surface composition characterization techniques, i.e. ATR-FTIR, XPS, SIMS, are compared in terms of their penetration depth. Ellipsometry, CD, EPR, SPR and QCM's principles and applications in analyzing surface proteins are introduced. Finally discussed are frequently applied methods and their principles to evaluate biocompatibility of biomaterials via cell culture. In this section, current techniques and their developments to measure cell adhesion, proliferation, morphology, viability, migration and gene expression are reviewed.

Abstract:At the time of implanting bone-related implants into human body, a variety of biological responses to the material surface occur with respect to surface chemistry and physical state. The commonly used biomaterials (e.g. titanium and its alloy, Co-Cr alloy, stainless steel, polyetheretherketone, ultra-high molecular weight polyethylene and various calcium phosphates) have many drawbacks such as lack of biocompatibility and improper mechanical properties. As surface modification is very promising technology to overcome such problems, a variety of surface modification techniques have been being investigated. This review paper covers recent advances in surface modification techniques of bone-related materials including physicochemical coating, radiation grafting, plasma surface engineering, ion beam processing and surface patterning techniques. The contents are organized with different types of techniques to applicable materials, and typical examples are also described.

Abstract:Dental implants are widely used in the field of oral restoration, but there are still problems leading to implant failures in clinical application, such as failed osseointegration, marginal bone resorption, and peri-implantitis, which restrict the success rate of dental implants and patient satisfaction. Poor osseointegration and bacterial infection are the most essential reasons resulting in implant failure. To improve the clinical outcomes of implants, many scholars devoted to modifying the surface of implants, especially to preparing different physical and chemical modifications to improve the osseointegration between alveolar bone and implant surface. Besides, the bioactive-coatings to promote the adhesion and colonization of ossteointegration-related proteins and cells also aim to improve the osseointegration. Meanwhile, improving the anti-bacterial performance of the implant surface can obstruct the adhesion and activity of bacteria, avoiding the occurrence of inflammation related to implants. Therefore, this review comprehensively investigates and summarizes the modifying or coating methods of implant surfaces, and analyzes the ossteointegration ability and anti-bacterial characteristics of emerging functional coatings in published references.

Abstract:Titanium and its alloys are widely used in biomedical fields as metallic biomaterials, specifically for implant applications. There are many kinds of titanium alloys capable to be used in clinical treatment, Ti–6Al–4V alloys are among the most widely used biomedical implant materials due to their favorable strength, high corrosion and wear resistance along with acceptable biocompatibility. Based on the complex physiological environment in the human body and the different needs for the treatment of various diseases, the performance requirements of Ti–6Al–4V alloy are getting higher and higher. Unfortunately, Ti–6Al–4V alloy has some disadvantages like high friction coefficient, mismatch of elastic modulus, and release of cytotoxic elements, hence its original performance is not satisfactorily sufficient to meet the growing clinical requirements. In consequence, further multilateral improvement of the overall performance of Ti–6Al–4V alloy by surface modification leads to achieving more and more compatible materials with the human body environment with stable and long-term effective use. Surface modification alone is not enough for improvement of implants' efficiency and the structural influence and topological features of cellular structures should also be considered. This paper is focused on the surface modification methods for improving wear resistance, corrosion resistance, biocompatibility, antibacterial property, wettability, and elastic modulus of Ti–6Al–4V alloy with the aim to achieve high throughput nascent Ti–6Al–4V alloys for biomedical applications. It mainly focuses on the new generation of medical implants especially additively manufactured ones and it summarizes the latest articles related to Ti–6Al–4V surface modification methods and elucidates how biocompatibility improvement will be a key research direction in the field of modern Ti–6Al–4V implants in the future.

Abstract:BACKGROUND: A large number of studies have demonstrated that orthopedic implant infection is related to bacteria biomembrane on the surface of materials. As the effect of general antibiotic is not favorable and the adverse effect is great, anti-infection method with opposing mechanism of bacteria biomembrane attachment release should be used. OBJECTIVE: To explore predisposing factor and drug resistance factor following implanting and investigate effect of various antibiosis coating. METHODS: A computer-based online search of Scopus database and Vip Chinese Periodical Database was performed for articled published between January 1990 and January 2010 related to infection following implant implantation, bacteria biomembrane and surface modification of implants. RESUTLS AND CONCLUSION: The studies on the biocompatibility, wear-resisting and anti-corrosivity on the surface modification of implants are conducted, as well as the antibiosis. The idea of anti-bacterial surface modification of implants is widely accepted. It can be said that the "zero adhesion" of the bacteria is the ultimate goal of antibacterial modification. Although there are numerous studies of various anti-bacterial coating, the majority of them have focused on the pilot phase in vitro, related in vivo animal experiments are few. The immune defense mechanism of the body around the implant has been considered to play a major role in the implant infections. Therefore, the imminent problem requires further investigation to enhance the study on antibiotic modification with little effect on the bone fusion and the surrounding structures.

Abstract:Titanium and its alloys have become key materials for biomedical applications, mainly owing to their compatibility with human tissues and their mechanical strength. Effects of surface topography on cell and tissue response have been investigated extensively in the past, while (bio)chemical surface modification and its combination with designed topographies have remained largely unexplored. The following report describes some of the strategies used or intended to modify titanium surfaces, based on biological principles, with a focus on ultrathin biomimetic adlayers. One of the visions behind such approaches is to achieve improved healing and integration responses after implantation for patients, especially for those suffering from deficiencies, for example, diabetes or osteoporosis, two diseases that have increased drastically in our society during the last century.

Abstract:The surface design of biomedical materials is becoming more and more important in the fields of tissue engineering, biomedical devices, biosensing and detection, and biochips. In-depth understanding of the properties of materials, especially the surface properties, has a certain guiding role in the development of biomedical materials. Among various methods of surface modification based on biomaterials, photochemical modification is very simple and efficient, and has the advantages of spatiotemporal controllability and non-invasiveness. Based on this, in recent years, photochemical modification has become one of the hot research fields in the interface modification of biomaterials. This review first simply introduces the recent development of photochemical modification methods on biochemical surface, then mainly focuses on tissue regeneration materials (extracellular matrix biomimetic surfaces, stiffness-controllable hydrogel films, patterning and gradient surfaces, photo-responsive smart surfaces), 2D droplet microarrays surfaces, high-throughput biochip and other applications. Finally, this article points out the current shortcomings of photochemical surface modification and the future development trend of this field is also proposed.

Abstract:Tremendous advances in quality, reliability, performance, and versatility of surgical instrumentation and devices have been achieved over the past 50 years using biomaterials. The global orthopaedic implant industry is expected to grow to $41.8 billion by 2016, driven primarily by advancements in implant designs, including materials that provide improved biocompatibility, durability, and expanded clinical applications. Biomaterials have evolved through 3 clinical "generations": (1) "bio-inert materials," (2) materials with intrinsic bioactivity and degradability, and (3) biomaterials that stimulate specific biological host responses. In all cases, surface modifications, including coatings, represent a key strategy for improvements in tissue-contacting properties. Surfaces continue to be a focus for many device improvements and for tissue interfacing, especially for many orthopaedic structural implants comprising metal and metal alloys. Progress in implant materials processing, coating technologies, and coating combinations with therapeutic agents provide new properties and functionalities to improve device-tissue integration and reduce foreign body reactions and infections. Performance criteria for these surface modifications success in clinical practice are daunting, and translation of several technologies from in vitro proof-of-concept to in vivo applications has proven challenging.

Abstract:Biomedical alloys have an important share in orthopedic applications. Among them, titanium and its titanium alloys are widely used as implant materials because of their excellent mechanical properties and non-cytotoxicity. However, its disadvantages such as its biological inertness and poor antibacterial properties inhibit its further development. Therefore, the surface properties of titanium are crucial in the implantation process and determine the success of the implant. The main purpose of this review is to provide a comprehensive and detailed description of the modification techniques used for the surface modification of titanium implants. In this paper, the corresponding technical methods are introduced systematically from four aspects: mechanical method, physical surface modification, chemical surface modification and electrochemical technique to understand the experimental mechanism of each modification technique, and the above methods can indeed improve the various properties of titanium and its alloys. With the increasing demand for implants in the future, the requirements for surface properties will also increase. Therefore, the development of new coating materials with higher performance by combining various advantages of existing modification technologies is the main trend of future research on surface modification of titanium alloys.

Abstract:BACKGROUND: Titanium and titanium alloy are commonly used for medical biological materials, they have good biocompatibility, but lack of the ability of bone induction, the contact between titanium and bone tissue is a mechanical contact. OBJECTIVE: To review the methods and efficacy of surface bioactivity modification for titanium and titanium alloy, and to provide references for further clinical application. METHODS: A computer-based online search of literatures related to methods of surface bioactivity modification for titanium and titanium alloy was performed in PubMed database and CNKI database using the key words of "titanium, titanium alloy, coating, bioactivity, bone" in English and in Chinese respectively. RESULTS AND CONCLUSION: The surface modification methods include plasma spraying, micro-arc oxidation, sol-gel and biomimetic methods. The surface coating of titanium and titanium alloy has been developed from a single coating to the composite coatings, gradient coatings and nano-gradient coatings. The joint use of two or three modification methods enhances the properties of the coating. After the surface modifications, the efficacy is investigated by simulating body fluid soaking, the culture of the osteoblasts, bone affinity fluorescein staining and radiology detection. Surface bioactivity modification for titanium and titanium alloy is a complex engineering, which requires to consider the bioactivity of surface coating and its bonding strength between the substrate, as well as the ability of bone induction. With the development of surface modification technology, the bioactivity and binding of surface coating for titanium and titanium alloy will be enhanced.

However, as bioinert materials, they often face challenges such as implant failure caused by bacterial infections and inadequate osseointegration post-implantation. To address these issues, researchers have developed various surface modification techniques to enhance the surface properties and bioactivity of titanium-based implants. This review aims to outline several key surface modification methods for titanium-based implants, including acid etching, sol-gel method, chemical vapor deposition, electrochemical techniques, layer-by-layer self-assembly, and chemical grafting. Abstract:Decades of intense scientific research investigations clearly suggest that only a subset of a large number of metals, ceramics, polymers, composites, and nanomaterials are suitable as biomaterials for a growing number of biomedical devices and biomedical uses. However, biomaterials are prone to microbial infection due to Escherichia coli (E. coli), Staphylococcus aureus (S. aureus), Staphylococcus epidermidis (S. epidermidis), hepatitis, tuberculosis, human immunodeficiency virus (HIV), and many more. Hence, a range of surface engineering strategies are devised in order to achieve desired biocompatibility and antimicrobial performance in situ. Surface engineering strategies are a group of techniques that alter or modify the surface properties of the material in order to obtain a product with desired functionalities. There are two categories of surface engineering methods: conventional surface engineering methods (such as coating, bioactive coating, plasma spray coating, hydrothermal, lithography, shot peening, and electrophoretic deposition) and emerging surface engineering methods (laser treatment, robot laser treatment, electrospinning, electrospray, additive manufacturing, and radio frequency magnetron sputtering technique). Atomic-scale engineering, such as chemical vapor deposition, atomic layer etching, plasma immersion ion deposition, and atomic layer deposition, is a subsection of emerging technology that has demonstrated improved control and flexibility at finer length scales than compared to the conventional methods. With the advancements in technologies and the demand for even better control of biomaterial surfaces, research efforts in recent years are aimed at the atomic scale and molecular scale while incorporating functional agents in order to elicit optimal in situ performance. The functional agents include synthetic materials (monolithic ZnO, quaternary ammonium salts, silver nano-clusters, titanium dioxide, and graphene) and natural materials (chitosan, totarol, botanical extracts, and nisin). This review highlights the various strategies of surface engineering of biomaterial including their functional mechanism, applications, and shortcomings. Additionally, this review article emphasizes atomic scale engineering of biomaterials for fabricating antimicrobial biomaterials and explores their challenges.

Abstract:Adaptation (or incorporation) of nanostructured materials into biomedical devices and systems has been of great interest in recent years. Through the modification of existing nanostructured materials one can control and tailor the properties of such materials in a predictable manner, and impart them with biological properties and functionalities to better suit their integration with biomedical systems. These modified nanostructured materials can bring new and unique capabilities to a variety of biomedical applications ranging from implant engineering and modulated drug delivery, to clinical biosensors and diagnostics. This review describes recent advances of nanostructured materials for biomedical applications. Abstract:Due to the good biocompatibility and ideal mechanical property, titanium implants have been widely used in dental clinic and orthopedic surgery. However, bacteria induced infection can cause per-implant inflammation and decrease the success rate of implant surgery. Therefore, developing antimicrobial techniques is essential to successful application of titanium implants. Many surface antimicrobial techniques, including antimicrobial coating and surface modifications, have been explored and they always exert antimicrobial effect by reducing bacterial adhesion, inhibiting their metabolism, or destructing cell structure. In this paper, different surface antimicrobial techniques and their recent research progress are reviewed to provide a brief insight on this area.
