Role of Hormonal Change after Start and Discontinuation of the Pill in Women’s Functional Brain Architecture and Mental Health

Abstract:Background Oral contraceptive (OC) intake has been linked to alterations in functional brain architecture and socio- affective processes, which may influence mental health. In a longitudinal study, we aimed to investigate the relation between endogenous and synthetic hormone profile changes with functional brain architecture and socio-affective functions important for mental health, such as mood and emotion recognition. Methods To this end, we measured 88 young healthy women twice, three to eight months apart: 26 natural cycling women, 26 long-term OC users, 25 OC discontinuers and 11 OC starters before and after discontinuation or start, respectively. In addition to mean-based analyses, we used inter-subject representational similarity analyses (IS-RSA) to assess relationships between interindividual variability of the different measurement levels (i.e., hormone concentrations, resting-state functional connectivity (RSFC), and socio-affective functions). Results Changes in progestogen levels were reflected by RSFC changes in frontal, subcortical and cerebellar regions. Most notably, for the superior orbitofrontal gyrus, RSFC change patterns were additionally paralleled by positive mood. While we found no association in change patterns of depressive symptoms nor emotion recognition, the depressive symptoms mean was reduced after OC discontinuation. Conclusions Overall, changes in progestogen profiles rather than estrogen concentrations tend to be associated with functional brain architecture and positive mood. Our data support the notion that women suffering from adverse mood effects are more likely to discontinue OC intake and may be therefore less likely to be represented in scientific studies possibly leading to an underestimation of the proportion of women suffering from OC-related mental health problems.
