Abstract:Traditional health systems mostly rely on rules created by experts to offer adaptive interventions to patients. However, with recent advances in artificial intelligence (AI) and machine learning (ML) techniques, health-related systems are becoming more sophisticated with higher accuracy in providing more personalized interventions or treatments to individual patients. In this paper, we present an extensive literature review to explore the current trends in ML-based adaptive systems for health and well-being. We conduct a systematic search for articles published between January 2011 and April 2022 and selected 87 articles that met our inclusion criteria for review. The selected articles target 18 health and wellness domains including disease management, assistive healthcare, medical diagnosis, mental health, physical activity, dietary management, health monitoring, substance use, smoking cessation, homeopathy remedy finding, patient privacy, mobile health (mHealth) apps finder, clinician knowledge representation for neonatal emergency care, dental and oral health, medication management, disease surveillance, medical specialty recommendation, and health awareness. Our review focuses on five key areas across the target domains: data collection strategies, model development process, ML techniques utilized, model evaluation techniques, as well as adaptive or personalization strategies for health and wellness interventions. We also identified various technical and methodological challenges including data volume constraints, data quality issues, data diversity or variability issues, infrastructure-related issues, and suitability of interventions which offer directions for future work in this area. Finally, we offer recommendations for tackling these challenges, leveraging on technological advances such as multimodality, Cloud technology, online learning, edge computing, automatic re-calibration, Bluetooth auto-reconnection, feedback pipeline, federated learning, explainable AI, and co-creation of health and wellness interventions.

Abstract:Abstract Artificial intelligence (AI) refers to the performance of tasks by machines ordinarily associated with human intelligence. Machine learning (ML) is a subtype of AI; it refers to the ability of computers to draw conclusions (ie, learn) from data without being directly programmed. ML builds from traditional statistical methods and has drawn significant interest in healthcare epidemiology due to its potential for improving disease prediction and patient care. This review provides an overview of ML in healthcare epidemiology and practical examples of ML tools used to support healthcare decision making at 4 stages of hospital-based care: triage, diagnosis, treatment, and discharge. Examples include model-building efforts to assist emergency department triage, predicting time before septic shock onset, detecting community-acquired pneumonia, and classifying COVID-19 disposition risk level. Increasing availability and quality of electronic health record (EHR) data as well as computing power provides opportunities for ML to increase patient safety, improve the efficiency of clinical management, and reduce healthcare costs.

ML is an essential element of the rapidly growing area of information science. Abstract:The use of machine learning (ML) approaches to target clinical problems is called to revolutionize clinical decision-making in cardiology. The success of these tools is dependent on the understanding of the intrinsic processes being used during the conventional pathway by which clinicians make decisions. In a parallelism with this pathway, ML can have an impact at four levels: for data acquisition, predominantly by extracting standardized, high-quality information with the smallest possible learning curve; for feature extraction, by discharging healthcare practitioners from performing tedious measurements on raw data; for interpretation, by digesting complex, heterogeneous data in order to augment the understanding of the patient status; and for decision support, by leveraging the previous steps to predict clinical outcomes, response to treatment or to recommend a specific intervention. This paper discusses the state-of-the-art, as well as the current clinical status and challenges associated with the two later tasks of interpretation and decision support, together with the challenges related to the learning process, the auditability/traceability, the system infrastructure and the integration within clinical processes in cardiovascular imaging.

Abstract:Big data and artificial intelligence are currently two of the most important and trending pieces for innovation and predictive analytics in healthcare, leading the digital healthcare transformation. Keralty organization is already working on developing an intelligent big data analytic platform based on machine learning and data integration principles. We discuss how this platform is the new pillar for the organization to improve population health management, value-based care, and new upcoming challenges in healthcare. The benefits of using this new data platform for community and population health include better healthcare outcomes, improvement of clinical operations, reducing costs of care, and generation of accurate medical information. Several machine learning algorithms implemented by the authors can use the large standardized datasets integrated into the platform to improve the effectiveness of public health interventions, improving diagnosis, and clinical decision support. The data integrated into the platform come from Electronic Health Records (EHR), Hospital Information Systems (HIS), Radiology Information Systems (RIS), and Laboratory Information Systems (LIS), as well as data generated by public health platforms, mobile data, social media, and clinical web portals. This massive volume of data is integrated using big data techniques for storage, retrieval, processing, and transformation. This paper presents the design of a digital health platform in a healthcare organization in Colombia to integrate operational, clinical, and business data repositories with advanced analytics to improve the decision-making process for population health management.

Abstract:Machine learning (ML) models have proven their potential in acquiring and analyzing large amounts of data to help solve real-world, complex problems. Their use in healthcare is expected to help physicians make diagnoses, prognoses, treatment decisions, and disease outcome predictions. However, ML solutions are not currently deployed in most healthcare systems. One of the main reasons for this is the provenance, transparency, and clinical utility of the training data. Physicians reject ML solutions if they are not at least based on accurate data and do not clearly include the decision-making process used in clinical practice. In this paper, we present a hybrid human–machine intelligence method to create predictive models driven by clinical practice. We promote the use of quality-approved data and the inclusion of physician reasoning in the ML process. Instead of training the ML algorithms on the given data to create predictive models (conventional method), we propose to pre-categorize the data according to the expert physicians' knowledge and experience. Comparing the results of the conventional method of ML learning versus the hybrid physician–algorithm method showed that the models based on the latter can perform better. Physicians' engagement is the most promising condition for the safe and innovative use of ML in healthcare.

This is not only detrimental to individual patients, but also to the healthcare system as health outcomes are affected and healthcare expenditure inevitably increases. There are many potential solutions to limit the number of missed opportunities for diagnosis and management, one of which is through the use of artificial intelligence. Artificial intelligence and machine learning research and capabilities have exponentially grown in the past decades, with their applications in medicine showing great promise. Abstract:<p>Decision-making related to health is complex. Machine learning (ML) and patient generated data can identify patterns and insights at the individual level, where human cognition falls short, but not all ML-generated information is of equal utility for making health-related decisions. We develop and apply attributable components analysis (ACA), a method inspired by optimal transport theory, to type 2 diabetes self-monitoring data to identify patterns of association between nutrition and blood glucose control. In comparison with linear regression, we found that ACA offers a number of characteristics that make it promising for use in decision support applications. For example, ACA was able to identify non-linear relationships, was more robust to outliers, and offered broader and more expressive uncertainty estimates. In addition, our results highlight a tradeoff between model accuracy and interpretability, and we discuss implications for ML-driven decision support systems.</p>

Abstract:Precision medicine is one of the recent and powerful developments in medical care, which has the potential to improve the traditional symptom-driven practice of medicine, allowing earlier interventions using advanced diagnostics and tailoring better and economically personalized treatments. Identifying the best pathway to personalized and population medicine involves the ability to analyze comprehensive patient information together with broader aspects to monitor and distinguish between sick and relatively healthy people, which will lead to a better understanding of biological indicators that can signal shifts in health. While the complexities of disease at the individual level have made it difficult to utilize healthcare information in clinical decision-making, some of the existing constraints have been greatly minimized by technological advancements. To implement effective precision medicine with enhanced ability to positively impact patient outcomes and provide real-time decision support, it is important to harness the power of electronic health records by integrating disparate data sources and discovering patient-specific patterns of disease progression. Useful analytic tools, technologies, databases, and approaches are required to augment networking and interoperability of clinical, laboratory and public health systems, as well as addressing ethical and social issues related to the privacy and protection of healthcare data with effective balance. Developing multifunctional machine learning platforms for clinical data extraction, aggregation, management and analysis can support clinicians by efficiently stratifying subjects to understand specific scenarios and optimize decision-making. Implementation of artificial intelligence in healthcare is a compelling vision that has the potential in leading to the significant improvements for achieving the goals of providing real-time, better personalized and population medicine at lower costs. In this study, we focused on analyzing and discussing various published artificial intelligence and machine learning solutions, approaches and perspectives, aiming to advance academic solutions in paving the way for a new data-centric era of discovery in healthcare.

Abstract:Obesity is a complex chronic disease resulting from the interaction of multiple behavioral factors. This paper presentsthe application of Machine Learning to identify the primary groups of behaviors contributing to the development of obesity.Supervised machine learning emphasizes decision trees and deep artificial neural networks from datasets. The study also references related work that utilizes predictive methods to estimate obesity levels based on physical activity and dietary habits. Furthermore,it compares the performance of classification algorithms such as J48, Naive Bayes, Multiclass Classification, Multilayer Perceptron, KNN, and decision trees when predicting diabetes cases. The objective is to analyze different tools in the assessment based on physical activity and dietary habits, contributing to the improvement of obesity risk diagnosis. In addition, MLP and J48 demonstrated strong performance among all the algorithms, but BPTT achieved the highest overall performance.

Abstract:Healthcare professionals decide wisely about personalized medicine, treatment plans, and resource allocation by utilizing big data analytics and machine learning. To guarantee that algorithmic recommendations are impartial and fair, however, ethical issues relating to prejudice and data privacy must be taken into account. Big data analytics and machine learning have a great potential to disrupt healthcare, and as these technologies continue to evolve, new opportunities to reform healthcare and enhance patient outcomes may arise. In order to investigate the patient’s outcomes with empirical evidence, this research was conducted using an online survey to incorporate healthcare professionals, patient’s reviews, and clinical staff. The data were analyzed using SmartPLS 4.0 to predict the structural model. The findings revealed a direct impact as positive influence of using machine learning on healthcare performance and patient outcomes through big data analytics. Moreover, it is evident that this can lead to personalized treatment plans, early interventions, and improved patient outcomes. Additionally, big data analytics can help healthcare providers optimize resource allocation, improve operational efficiency, and reduce costs. The impact of big data analytics on patient outcome and healthcare performance is expected to continue to grow, making it an important area for investment and research

Abstract:As machine learning and artificial intelligence are more frequently being leveraged to tackle problems in the health sector, there has been increased interest in utilizing them in clinical decision-support. This has historically been the case in single modal data such as electronic health record data. Attempts to improve prediction and resemble the multimodal nature of clinical expert decision-making this has been met in the computational field of machine learning by a fusion of disparate data. This review was conducted to summarize this field and identify topics ripe for future research. We conducted this review in accordance with the PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) extension for Scoping Reviews to characterize multi-modal data fusion in health. We used a combination of content analysis and literature searches to establish search strings and databases of PubMed, Google Scholar, and IEEEXplore from 2011 to 2021. A final set of 125 articles were included in the analysis. The most common health areas utilizing multi-modal methods were neurology and oncology. However, there exist a wide breadth of current applications. The most common form of information fusion was early fusion. Notably, there was an improvement in predictive performance performing heterogeneous data fusion. Lacking from the papers were clear clinical deployment strategies and pursuit of FDA-approved tools. These findings provide a map of the current literature on multimodal data fusion as applied to health diagnosis/prognosis problems. Multi-modal machine learning, while more robust in its estimations over unimodal methods, has drawbacks in its scalability and the time-consuming nature of information concatenation.
Predictive models for clinical decision making: Deep dives in practical machine learning

Abstract:The deployment of machine learning for tasks relevant to complementing standard of care and advancing tools for precision health has gained much attention in the clinical community, thus meriting further investigations into its broader use. In an introduction to predictive modelling using machine learning, we conducted a review of the recent literature that explains standard taxonomies, terminology and central concepts to a broad clinical readership. Articles aimed at readers with little or no prior experience of commonly used methods or typical workflows were summarised and key references are highlighted.Continual interdisciplinary developments in data science, biostatistics and epidemiology also motivated us to further discuss emerging topics in predictive and data‐driven (hypothesis‐less) analytics with machine learning. Through two methodological deep dives using examples from precision psychiatry and outcome prediction after lymphoma, we highlight how the use of, for example, natural language processing can outperform established clinical risk scores and aid dynamic prediction and adaptive care strategies. Such realistic and detailed examples allow for critical analysis of the importance of new technological advances in artificial intelligence for clinical decision‐making. Abstract:A consequence of the fragmented and siloed healthcare landscape is that patient care (and data) is split along multitude of different facilities and computer systems and enabling interoperability between these systems is hard. The lack interoperability not only hinders continuity of care and burdens providers, but also hinders effective application of Machine Learning (ML) algorithms. Thus, most current ML algorithms, designed to understand patient care and facilitate clinical decision-support, are trained on limited datasets. This approach is analogous to the Newtonian paradigm of Reductionism in which a system is broken down into elementary components and a description of the whole is formed by understanding those components individually. A key limitation of the reductionist approach is that it ignores the component-component interactions and dynamics within the system which are often of prime significance in understanding the overall behaviour of complex adaptive systems (CAS). Healthcare is a CAS. Though the application of ML on health data have shown incremental improvements for clinical decision support, ML has a much a broader potential to restructure care delivery as a whole and maximize care value. However, this ML potential remains largely untapped: primarily due to functional limitations of Electronic Health Records (EHR) and the inability to see the healthcare system as a whole. This viewpoint (i) articulates the healthcare as a complex system which has a biological and an organizational perspective, (ii) motivates with examples, the need of a system's approach when addressing healthcare challenges via ML and, (iii) emphasizes to unleash EHR functionality - while duly respecting all ethical and legal concerns - to reap full benefits of ML.

Abstract:Machine learning technologies are increasingly developed for use in healthcare. While research communities have focused on creating state-of-the-art models, there has been less focus on real world implementation and the associated challenges to accuracy, fairness, accountability, and transparency that come from actual, situated use. Serious questions remain under examined regarding how to ethically build models, interpret and explain model output, recognize and account for biases, and minimize disruptions to professional expertise and work cultures. We address this gap in the literature and provide a detailed case study covering the development, implementation, and evaluation of Sepsis Watch, a machine learning-driven tool that assists hospital clinicians in the early diagnosis and treatment of sepsis. We, the team that developed and evaluated the tool, discuss our conceptualization of the tool not as a model deployed in the world but instead as a socio-technical system requiring integration into existing social and professional contexts. Rather than focusing on model interpretability to ensure a fair and accountable machine learning, we point toward four key values and practices that should be considered when developing machine learning to support clinical decision-making: rigorously define the problem in context, build relationships with stakeholders, respect professional discretion, and create ongoing feedback loops with stakeholders. Our work has significant implications for future research regarding mechanisms of institutional accountability and considerations for designing machine learning systems. Our work underscores the limits of model interpretability as a solution to ensure transparency, accuracy, and accountability in practice. Instead, our work demonstrates other means and goals to achieve FATML values in design and in practice.

Abstract:In the present day, there are many diseases which need to be identified at their early stages to start relevant treatments. If not, they could be uncurable and deadly. Due to this reason, there is a need of analysing complex medical data, medical reports, and medical images at a lesser time but with greater accuracy. There are even some instances where certain abnormalities cannot be directly recognized by humans. In healthcare for computational decision making, machine learning approaches are being used in these types of situations where a crucial data analysis needs to be performed on medical data to reveal hidden relationships or abnormalities which are not visible to humans. Implementing algorithms to perform such tasks itself is difficult, but what makes it even more challenging is to increase the accuracy of the algorithm while decreasing the required time for the algorithm to execute. In the early days, processing of large amount of medical data was an important task which resulted in machine learning being adapted in the biological domain. Since this happened, the biology and biomedical fields have been reaching higher levels by exploring more knowledge and identifying relationships which were never observed before. Reaching to its peak now the concern is being diverted towards treating patients not only based on the type of disease but also their genetics, which is known as precision medicine. Modifications in machine learning algorithms are being performed and tested daily to improve the performance of the algorithms in analysing and presenting more accurate information. In the healthcare field, starting from information extraction from medical documents until the prediction or diagnosis of a disease, machine learning has been involved. Medical imaging is a section that was greatly improved with the integration of machine learning algorithms to the field of computational biology. Nowadays, many disease diagnoses are being performed by medical image processing using machine learning algorithms. In addition, patient care, resource allocation, and research on treatments for various diseases are also being performed using machine learning-based computational decision making. Throughout this paper, various machine learning algorithms and approaches that are being used for decision making in the healthcare sector will be discussed along with the involvement of machine learning in healthcare applications in the current context. With the explored knowledge, it was evident that neural network-based deep learning methods have performed extremely well in the field of computational biology with the support of the high processing power of modern sophisticated computers and are being extensively applied because of their high predicting accuracy and reliability. When giving concern towards the big picture by combining the observations, it is noticeable that computational biology and biomedicine-based decision making in healthcare have now become dependent on machine learning algorithms, and thus they cannot be separated from the field of artificial intelligence.

In this work, a systematic review of studies applying a hybrid simulation models and artificial intelligence approach to address healthcare management challenges was carried out. Scopus, Web of Science, and PubMed databases were screened by independent reviewers. The main strategies to combine simulation and artificial intelligence as well as the major healthcare application scenarios were identified and discussed. Moreover, tools and algorithms to implement the proposed approaches were described. Results showed that machine learning appears to be the most employed artificial intelligence strategy in combination with simulation models, which mainly rely on agent-based and discrete-event systems. The scarcity and heterogeneity of the included studies suggested that a standardized framework to implement hybrid machine learning-simulation approaches in healthcare management is yet to be defined. Future efforts should aim to use these approaches to design novel intelligent in-silico models of healthcare processes and to provide effective translation to the clinics.
The Intersection of Health Literacy and Public Health: A Machine Learning-Enhanced Bibliometric Investigation

Abstract:In recent decades, health literacy has garnered increasing attention alongside a variety of public health topics. This study aims to explore trends in this area through a bibliometric analysis. A Random Forest Model was utilized to identify keywords and other metadata that predict average citations in the field. To supplement this machine learning analysis, we have also implemented a bibliometric review of the corpus. Our findings reveal significant positive coefficients for the keywords “COVID-19” and “Male”, underscoring the influence of the pandemic and potential gender-related factors in the literature. On the other hand, the keyword “Female” showed a negative coefficient, hinting at possible disparities that warrant further investigation. Additionally, evolving themes such as COVID-19, mental health, and social media were discovered. A significant change was observed in the main publishing journals, while the major contributing authors remained the same. The results hint at the influence of the COVID-19 pandemic and a significant association between gender-related keywords on citation likelihood, as well as changing publication strategies, despite the fact that the main researchers remain those who have been studying health literacy since its creation.

Abstract:The integration of machine learning (ML) and big data analytics within smart healthcare systems represents a transformative advancement in medical services, emphasizing efficiency, accuracy, and patient-centered care. This paper investigates the application of these advanced technologies in real-time disease detection, showcasing their potential to revolutionize healthcare delivery. Smart healthcare systems leverage a multitude of technological components, including Internet of Things (IoT) devices, sensors, and artificial intelligence (AI), to enable continuous monitoring and diagnostics. This real-time monitoring facilitates prompt interventions and treatment adjustments, which is particularly advantageous for managing chronic conditions and acute illnesses where timely responses are critical to improving patient outcomes. Despite the evident benefits, traditional healthcare infrastructures face significant challenges such as delays in diagnosis due to manual processes, inefficient data handling resulting in data silos, and limited interoperability between different healthcare providers, leading to worsened health outcomes and increased healthcare costs. The integration of ML and big data analytics offers promising solutions to these challenges. ML algorithms can process vast amounts of healthcare data to identify patterns and predict outcomes with high accuracy, such as recognizing early signs of diseases like cancer or diabetes from medical images or electronic health records (EHRs). Big data analytics complements ML by providing the necessary infrastructure to handle and process large volumes of health data, enabling the collection, storage, and analysis of structured data from EHRs, unstructured data from clinical notes, and real-time data from wearable devices. By integrating these technologies, healthcare providers can gain deeper insights into patient health trends and outcomes, leading to more informed decision-making and better patient management. This study employs a qualitative research design, focusing on five genuine case studies: the Mayo Clinic's predictive analytics for heart disease, Cleveland Clinic's use of ML for cancer diagnosis, Kaiser Permanente's diabetes management program, Johns Hopkins Hospital's sepsis detection system, and Mount Sinai Health System's genomic data analysis. Each case study is chosen for its relevance and comprehensive data, detailing the specific healthcare environment and context. This paper interprets these findings in the broader context of smart healthcare systems and existing literature, emphasizing the importance of these technologies in modernizing healthcare and addressing inefficiencies. The challenges encountered during integration, such as data privacy concerns and interoperability issues, are examined along with implemented solutions.
