Rare Cardiac Myxoma with A Hard Texture: A Case Report

Abstract:Background: Primary tumors of the heart are uncommon but not rare. Myxoma comprises 50% of all benign cardiac tumors in adults and 15% of such tumors in children. Cardiac myxoma is usually soft in texture and has a friable or villous surface, which tends to be associated with embolic events. It is also rare for myxoma to be full of calcium and metastatic bone deposits.Case presentation: We herein present a 58-year-old female patient with cardiac space-occupying lesion within the left atrium, and the lesion is lubricous and hard. Her only symptoms were hypertension, dizziness, and chest stuffiness. After surgical resection, we confirmed that the lesion was atrial myxoma by histopathology examination. The patient recovered uneventfully and was discharged. A 2-year follow-up exam showed no evidence of tumor recurrence.Conclusions: Cardiac myxoma is rare and has various clinical manifestations. It is usually friable and has an irregular surface, making embolic manifestations one of the most common complications. Consequently, immediate surgical excision is suggested upon diagnosis. In our case, the patient had a rare type of cardiac myxoma, which had a hard texture and lubricous surface. In our opinion, this case has a low risk of embolism. We prefer elective surgery rather than emergency surgery to perfect the preoperative examination for this kind of patient.
Giant Left Atrial Myxoma Misdiagnosed As Schizophrenia: A Case Report

Abstract:Background: Primary cardiac tumors are rare. Myxoma is the most common cardiac tumor and presents in the left atrium. Improvements tend to occur with early diagnosis, and with the help of high-resolution imaging technology, such as transesophageal echocardiography (TEE). Patients typically present with tightness of the chest, fatigue and lassitude. There are also cases of misdiagnosis and missed diagnosis. Case presentation: A 59-year-old Chinese woman complained of tightness in her chest, fatigue and lassitude for eight years. She had been misdiagnosed with schizophrenia by the local rural hospital. Computed tomography (CT) confirmed a giant mass in the left atrium, and transthoracic echocardiography (TTE) revealed left atrial enlargement with an occupying lesion. For this patient, we performed tumor resection surgery with a cardiopulmonary bypass. The giant tumor was partially detached during the operation, and the thrombus was removed successfully. After the operation, the mental health department of our hospital thought that the patient's diagnosis of schizophrenia was misdiagnosed. The patient recovered well and appeared rejuvenated after the operation. A two-year follow-up found no adverse events since the operation. Conclusions: When making a diagnosis for a patient who presented with tightness of the chest, we need to administer a cardiac physical examination and echocardiography. The physician diagnosing schizophrenia needs to first rule out any organic diseases. A careful and complete resection of the cardiac myxoma is a vital operation, due to the risk of thromboembolism.
Diagnostic mystery—a rare right ventricular cardiac hemangioma: a case report

Abstract:Background Cardiac hemangiomas are rare in all kinds of benign cardiac tumors. Although cardiac hemangiomas affect all ages and may occur anywhere within the heart, right ventricular hemangiomas are extremely uncommon. Case presentation We report a 56-year-old woman presented with chest tightness and breath shortness for 3 months. Transthoracic echocardiography and coronary computed tomography angiography showed a mass located adjacent to the apex of the right ventricle but both failed to figure out where the mass originated from, remaining a diagnostic mystery preoperatively. The mass was removed successfully and the histopathological examination confirmed it was hemangioma. Conclusions Cardiac magnetic resonance should be the ultimate diagnostic tool of cardiac tumors. Surgical removal, associated with a low recurrence rate and long-term survival benefits, should be the first choice of therapy for cardiac hemangiomas.

Abstract:Cardiac tumors are very rare and myxoma is the most common primary cardiac tumor. Cardiac myxomas frequently originate from the left atrium and predominantly affect women. We present a case of a 65-year-old male who was admitted for hernia surgery and underwent preoperative evaluation. On evaluation, he was found to have a diastolic murmur on auscultation and had a history of exercise intolerance, and a transthoracic echocardiogram was done. It revealed a large mass in the left atrium attached to the interatrial septum suggestive of left atrial myxoma. He then underwent surgical resection of the tumor. The lack of recognition of this condition by appropriate history and physical exam can lead to a delay in diagnosis and treatment and the risk of cardiovascular complications.
The MRI Manifestations of Cardiac Myxoma

Abstract:Objective To explore the MRI manifestations of cardiac myxoma.Methods 18 cases of myxoma which were confirmed through pathology were respectively analysed,and the patients were all examined by enhanced cardiac MRI before surgical resection.The sequences of cine,SE (T1 WI,T2 WI) or IR (Double IR,Tripple IR),first pass and delayed gadolinium-enhanced were performed.The amount,location,size,margin,MRI signal manifestation,base,mobility,enhancement manifestation of myxoma,cardiac structure and haemodynamics were analysed.Results 14 cases of cardiac myxoma were in the left atrium.10 cases originated from the artial septum.6 cases were attached to the free wall of the atrium.9 ca ses were pedunculated,and the other half exhibited a wide base.10 cases demonstrated a round or roundlike shape and 8 cases demonstrated a lobulated shape.The signal intensity was low in cine which was lower than the heart muscle,high or moderately high in T2WI or Tripple IR,higher than the heart muscle.The signal of 3 cases were not homogeneous with mixed punctiform low signal intensity.Filling defects were shown in all cases.Early and late gadolinium enhancement was seen in 8 cases and no enhancement was displayed in 6 cases,and the other 4 cases were mildly enhanced.The haemodynamic changes were seen in 3 cases,in which two tumors were close to atrioventricular valve,the mass fluttered between atrium and ventricle during the heart cycle.The right inferior pulmonary vein was dilated mildly for the mass was close to the ostium of the right inferior pulmonary vein.Cardiac remodeling was seen in 4 cases,the atrium or ventricle was dilated.Conclusion The MRI manifestations of cardiac myxoma were characteristic to a certain extent,so cardiac MRI is an effective,important examination in the diagnosis of cardiac myxoma.

Abstract:Abstract Background Cardiac myxomas are the most common benign primary cardiac tumours. The natural history of left cardiac myxomas is thought to be of slowly growing tumours. Cardiac myxomas are a heterogeneous group with a variable growth rate. They present usually with stroke, valve obstruction, or non-specific symptoms. Surgical resection is the effective treatment. Case summary This case report describes a 56-year-old hypertensive and dyslipidaemic female, when she was admitted in January 1990, complaining of loss of appetite, aches, pains, and palpitations. Her workup included a transthoracic echocardiography and transoesophageal echocardiography, which showed a left atrial mass attached to the inter-atrial septum, highly suggestive of left atrial myxoma. She was referred for surgical removal of the left atrial mass. However, she was reluctant to undergo surgery as she felt better. The patient was followed-up for almost 30 years with the left atrial mass confirmed as left atrial myxoma by cardiac magnetic resonance imaging. The left atrial mass became smaller in size and more calcified. Discussion Cardiac myxomas are a group of heterogeneous tumours, thought to be slowly growing. The growth rate of cardiac myxomas prior to diagnosis is not well known, as the vast majority is treated with surgical resection immediately after diagnosis. Our case showed the natural progression of an unoperated smooth-surfaced left atrial myxoma followed-up for almost 30 years, which slowly became smaller and more calcified.

Abstract:Abstract Background Primary cardiac tumors are rare, and approximately 90% of them are benign. Myxoma is the most common type of these tumors occurring in the left atrium in 75–85% of cases. The tumor can cause the left atrio-ventricular valve obstruction and embolization phenomenon. Case presentation We reported a case of 54-year-old man with complaints of dyspenea and amnesia. In our patient, transthoracic echocardiography revealed a mass of 28*63 mm attached to the upper intra-atrial septum, which was prolapsing through the mitral valve into the left ventricle during diastole, being indicative of the left atrial myxoma. On examination, he was alert and conversant, and no pathological abnormality was observed in the examination of cardiovascular, gastrointestinal, respiratory, hepatic, renal and nervous systems. After myxoma diagnosis, the tumor was removed under cardiac surgery and discharged under good conditions. In the telephone follow-up after discharge, the patient recovered and did not report the disease and surgery complications. Conclusions Patients with cardiac myxoma are usually asymptomatic, but they may have manifestations related to the embolism phenomenon or intracardiac obstruction. Therefore, myxoma may represent an emergency. Surgery should be performed as soon as possible. If surgery is delayed, the patient may suffer from serious and irreversible complications, such as stroke and cardiac arrest.

Abstract:Often health systems functions are identified with service provision within a personal healthcare dimension or a public health framework. This constitutes the healthcare system which is a part of a health system, which views health in its inter-sectoral scope. It is well established that factors which determine health status range much broader than those which are within the realm of the health sector. Many socio-economic and environmental conditions are known to affect health status; liberalization of international trade, global pandemics, natural disasters and humanitarian crises can be detrimental to public health outcomes as can be changes in international cooperation and geopolitical situations, which may have implications for the manner in which health is resourced in a country such as Pakistan. As a roadmap for health systems reform in Pakistan, the Gateway Paper factors these considerations into health policy and planning underscoring the need to develop alternative policy approaches to health within its inter-sectoral scope, redefining targets within the health sector in order to garner support from across various sectors; creating inter-sectoral agencies that concentrate on prevention and health promotion at multiple levels and developing overarching policy and legislation for health promotion.

Abstract:Myxoma is the most commonly found cardiac primary tumor. The left atrium is the most common localization of myxoma, followed by the right atrium. However, it is rare in the left and right ventricles. Myxoma originating from cardiac valves is extremely rare. This article presents a case of a 17-year-old male who was admitted due to heart murmur for one year. Transthoracic echocardiography indicated a 1.9 cm round solid mass in the left ventricular outflow tract. Excision surgery and aortic valve replacement were performed in this patient. Histopathology revealed the mass as a myxoma. The aortic valve remains a very rare myxoma localization position. Echocardiography can provide a precise method for myxoma diagnosis. Early excision associated with valve replacement can provide good curative effects.

Abstract:We report a rare case of coincidental left atrial and right ventricular myxomas manifesting as masses with different echodensities on transthoracic echocardiography. This patient had a history of left atrial myxoma, left intra-left internal carotid artery myxoma, and facial cutaneous myxoma 3 years prior to admission. A Carney complex was suspected, and the patient subsequently tested positive for PRKAR1A mutations. The patient was followed up regularly by a biannual echocardiography, which was free from abnormalities until the date of admission. A repeat transthoracic echocardiography revealed a massive left atrial mass of solid echodensity, and a minute hypoechoic entity in the right ventricular outflow tract. Both masses were confirmed for existence by an enhanced cardiac CT. Chest CT also revealed multiple pulmonary emboli. Successful surgical repair was performed revealing that both masses were hemorrhagic nipple-like lesions and that the pulmonary emboli were myxomatous in nature. Postoperative recovery was uneventful. Postoperative echocardiography showed a clear heart chamber, and the 1-year follow-up showed no abnormalities. Further research is needed to clarify the echocardiographic characteristics of multiple myxomas when they occurred simultaneously in different chambers.

Abstract:An 80-year-old female was referred to our institution due to transient right upper limb weakness. Transthoracic and transesophageal echocardiography revealed a tumor in the left atrium. The tumor was attached to the posterior wall of the left atrium near the atrioventricular node. Intraoperative pathological examination revealed that the tumor was a myxoma, and complete resection was successfully performed. However, she experienced persistent complete atrioventricular block postoperatively and required pacemaker implantation.

Abstract:Left atrial appendage mass can occasionally pose a serious challenge to physicians to identify the nature of the mass with the aid of imaging techniques. We present a case of 67-year-old man, who was evaluated for suspected left atria myxoma. Transesophageal echocardiography revealed a heterogeneous density originating from left atrial appendage, thought to be most consistent with a myxoma. Cardiac magnetic resonance imaging, uncharacteristically, gave an equivocal picture, suggesting the mass to be a myxoma on initial imaging and a thrombus with evidence of liquefaction necrosis following postcontrast enhancement. Surprisingly, histopathology of the mass following its surgical excision yielded a rare diagnosis of myxofibrosarcoma.

Abstract:Cardiac myxomas are the most common primary cardiac tumours in adults. Clinical presentation is variable, with few cases of infected myxomas reported in the literature. We describe a rare case of a 63-year-old patient who presented with splenic abscesses and a left atrial mass suggestive of emboligen myxoma. The patient underwent a successful emergency sur- gical excision of the atrial mass followed by splenectomy. Blood cultures were positive for Fusobacterium nucleatum, whereas the histopathological examination of the excised mass confirmed the presence of a myxoma with a marked inflammatory infiltrate. All these findings allowed us to confirm the diagnosis of definite infected myxoma. Some aspects related to the aetiology, diagnosis and management of this entity are discussed.

Abstract:A hypertensive man in his 80s presented to the emergency department with a fall preceded by a 'funny turn' and syncope. Examination findings were consistent with fracture of the left neck of femur. Laboratory investigations, including ECG, were normal. He was discharged after total left hip arthroplasty. During outpatient evaluation for syncope and dizziness, his 24-hour Holter examination showed predominantly sinus rhythm with episodes of first-degree atrioventricular block, ventricular ectopy <1%. Transthoracic echocardiogram showed a large, elongated, mobile left atrial mass with no clear attachment, measuring 4.8 cm × 1.6 cm and a dilated left atrium. (figure 1). Transoesophageal echocardiogram revealed a 4.7 cm × 3.5 cm × 2.6 cm multilobulated, gelatinous, mobile left atrial mass attached to the area around the fossa ovalis of the interatrial septum not prolapsing through or obstructing the mitral valve (figure 2, video 1). CT of thorax, abdomen and the pelvis confirmed intracardiac left atrial mass. There were no signs of extracardiac primary malignancy, metastatic disease or lymphadenopathy. Preoperative workup for surgery was carried out. Coronary angiogram revealed severe proximal left anterior descending artery stenosis and diffuse mild disease in the left circumflex and right coronary artery. He was started on secondary prevention for coronary artery disease. He underwent surgery for myxoma resection and coronary artery bypass grafting with left internal mammary artery graft to the left anterior descending artery. Electrical cardioversion was done for postoperative atrial fibrillation. The patient was discharged home following restoration of sinus rhythm. His biopsy report confirmed left atrial myxoma.
Disruption of Disulfide Bonds Is Responsible for Impaired Secretion in Human Complement Factor H Deficiency*

Abstract:Factor H, a secretory glycoprotein composed of 20 short consensus repeat modules, is an inhibitor of the complement system. Previous studies of inherited factor H deficiency revealed single amino acid substitutions at conserved cysteine residues, on one allele arginine for cysteine 518 (C518R) and on the other tyrosine for cysteine 941 (C941Y) (Ault, B. H., Schmidt, B. Z., Fowler, N. L., Kashtan, C. E., Ahmed, A. E., Vogt, B. A., and Colten, H. R. (1997) J. Biol. Chem. 272, 25168–25175). To ascertain if the phenotype, impaired secretion of factor H, is due to the C518R substitution or the C941Y substitution and to ascertain the mechanism by which secretion is impaired, we studied COS-1 and HepG2 cells transfected with wild type and several mutant factor H molecules. The results showed markedly impaired secretion of both C518R and C941Y factor H as well as that of factor H molecules bearing alanine or arginine substitutions at the Cys518–Cys546 disulfide bond (C518A, C546A, C546R, C518A-C546A). In each case, mutant factor H was retained in the endoplasmic reticulum and degraded relatively slowly as compared with most other mutant secretory and membrane proteins that are retained in the endoplasmic reticulum. These data indicate that impaired secretion of the naturally occurring C518R and C941Y mutant factor H proteins is due to disruption of framework-specific disulfide bonds in factor H short consensus repeat modules.
An atypical case of right ventricular myxoma, a paradox of pulmonary valve endocarditis with right ventricular outflow tract obstruction. From a surgeon's perspective

Abstract:Intracardiac tumour is rare, and intracardiac myxoma is the most common intracardiac tumour of the heart. The majority of these tumours arise at the left and right atrium, and a presentation of such a tumour other than the mentioned is atypical and very rare. Due to the rare occurrence, clinicians often misdiagnose it as infective endocarditis especially if the tumour is located near the valves and causing outflow gradient stenosis on echocardiography. A multi-modal cardiac imaging and a multi-disciplinary approach are paramount to make a correct diagnosis and treatment strategies. We would like to report a rare case of a right ventricular myxoma of a young girl, who was initially treated with infective endocarditis, which turned out to be a rare atypical Right ventricular myxoma, which was then surgically excised. The patient was successfully discharged after 3 years of follow up echocardiography showed free of tumour recurrence.
An exceptionally giant left atrial myxoma: a case report and literature review

Abstract:Abstract Background Cardiac myxomas are considered the most common benign heart tumours. The clinical manifestations mainly depend on the size of the tumour. They usually vary from asymptomatic, mild non-specific symptoms, to severe obstructive cardiac and systemic findings. We describe herein a significantly large left atrial myxoma in a patient misdiagnosed with respiratory asthma. Case summary A 54-year-old lady, was diagnosed previously with asthma, presented with a history of dyspnoea on exertion, palpitations, and mild peripheral oedema. Chest X-ray suggested pulmonary congestion. Due to high suspicion of cardiac issues, transthoracic echocardiography was done revealing giant left atrial mass. Consequently, the mass was approached and excised surgically through the inverted T biatrial incision. Grossly, the mass measured 10 × 8 × 6 cm, and it had a smooth surface and was filled with gelatinous material. The histopathology confirmed benign myxoma without malignant features. Discussion Our article mainly focuses on the diagnostic challenges of a patient with atrial myxoma. The major discrepancy between the tumour size and the severity of the patient’s symptoms should draw physicians’ attention to consider atrial myxoma over a long list of differentials, in order to take immediate action to reduce the mortality and improve the overall prognosis.
A Silent Left Atrial Myxoma: A Rare Benign Cardiac Tumor

Abstract:Primary cardiac tumors are extremely rare. In one study, incidence was reported being less than 0.1%. The purpose of this case report is to review different presentations of cardiac myxoma. A 34-year-old female with past medical history of drug abuse was brought into the emergency department (ED) after a motor vehicle collision. She was found to have multiple fractures with a hypodense mass in the left atrium. Further evaluation showed a left atrial myxoma. The patient underwent myxoma resection. The clinical appearance of myxoma varies from non-specific to life-threatening complications, such as stroke, acute heart failure, or even sudden death. A surgical resection is the treatment of choice for cardiac myxomas.

Abstract:Background: Myxomas are the most common primary cardiac tumor and typically originate in the left atrium. Atrial myxomas may present following complications of obstruction and emboli. If an atrial myxoma goes untreated, complications such as congestive heart failure, embolic stroke, and sudden death can occur. Case presentation: A 58-year-old Caucasian male presented following a cardiac arrest. He was taken emergently to the cardiac catheterization lab and received two drug eluting stents. Following the procedure, he was found to have a left atrial mass that was intermittently obstructing the mitral valve on echocardiography. After leaving the cardiac catheterization lab, he was hypotensive and placed on multiple intravenous medications for hemodynamic support as well as an Impella device. Following medical optimization, he underwent one vessel coronary artery bypass graft as well as surgical excision of the left atrial mass, which pathology had shown to be an atrial myxoma. Conclusion: This patient's case of cardiogenic shock following revascularization was complicated by the identification of an atrial myxoma, which, when large enough, can obstruct blood flow through the mitral valve leading to acute mitral dynamic stenosis. This condition results in circulatory collapse due to obstruction of the left ventricle in diastole as the myxoma occludes the mitral valve.
Yearlong Near-Syncope: Presentation of a Rare Right Atrial Myxoma

Abstract:Atrial myxomas are the most common form of primary benign cardiac tumors. The left atrium is typically the most common location while right atrial myxomas are much rarer and only occur in about 15%-25% of all myxoma patients. Typically, left atrial myxomas have the ability to cause symptoms such as syncope. We report a case of a 67-year-old female who presented with complaints of palpitations, dizziness, and near-syncope that had been ongoing for about a year. Other causes of syncope were investigated and ruled out. A transthoracic echocardiogram (TTE) found a large 4.3 x 4.0 cm spherical mass in the right atrium which was confirmed by surgical resection and immunohistochemistry to be a myxoma. The patient's condition of syncope-like symptoms warrants elevating atrial myxomas to a higher position in the diagnostic differential.
