Regulation of engineered nanomaterials: current challenges, insights and future directions

Abstract:Substantial production and wide applications of engineered nanomaterials (ENMs) have raised concerns over their potential influences on the environment and humans. However, regulations of products containing ENMs are scarce, even in countries with the greatest volume of ENMs produced, such as the United States and China. After a comprehensive review of life cycles of ENMs, five major challenges to regulators posed by ENMs are proposed in this review: (a) ENMs exhibit variable physicochemical characteristics, which makes them difficult for regulators to establish regulatory definition; (b) Due to diverse sources and transport pathways for ENMs, it is difficult to monitor or predict their fates in the environment; (c) There is a lack of reliable techniques for quantifying exposures to ENMs; (d) Because of diverse intrinsic properties of ENMs and dynamic environmental conditions, it is difficult to predict bioavailability of ENMs on wildlife and the environment; and (e) There are knowledge gaps in toxicity and toxic mechanisms of ENMs from which to predict their hazards. These challenges are all related to issues in conventional assessments of risks that regulators rely on. To address the fast-growing nanotechnology market with limited resources, four ENMs (nanoparticles of Ag, TiO2, ZnO and Fe2O3) have been prioritized for research. Compulsory reporting schemes (registration and labelling) for commercial products containing ENMs should be adopted. Moreover, to accommodate their potential risks in time, an integrative use of quantitative structure-activity relationship and adverse outcome pathway (QSAR-AOP), together with qualitative alternatives to conventional risk assessment are proposed as tools for decision making of regulators.

Abstract:The concept of nanomedicine has evolved significantly in recent decades, leveraging the unique phenomenon known as the enhanced permeability and retention (EPR) effect. This has facilitated major advancements in targeted drug delivery, imaging, and individualized therapy through the integration of nanotechnology principles into medicine. Numerous nanomedicines have been developed and applied for disease treatment, with a particular focus on cancer therapy. Recently, nanomedicine has been utilized in various advanced fields, including diagnosis, vaccines, immunotherapy, gene delivery, and tissue engineering. Multifunctional nanomedicines facilitate concurrent medication delivery, therapeutic monitoring, and imaging, allowing for immediate responses and personalized treatment plans. This review concerns the major advancement of nanomaterials and their potential applications in the biological and medical fields. Along with this, we also mention the various clinical translations of nanomedicine and the major challenges that nanomedicine is currently facing to overcome the clinical translation barrier.

Abstract:Nanomedicine has the potential to transform healthcare by offering targeted therapies, precise diagnostics, and enhanced drug delivery systems. The National Institutes of Health has coined the term "nanomedicine" to describe the use of nanotechnology in biological system monitoring, control, diagnosis, and treatment. Nanomedicine continues to receive increasing interest for the rationalized delivery of therapeutics and pharmaceutical agents to achieve the required response while reducing its side effects. However, as nanotechnology continues to advance, concerns about its potential toxicological effects have also grown. This review explores the current state of nanomedicine, focusing on the types of nanoparticles used and their associated properties that contribute to nanotoxicity. It examines the mechanisms through which nanoparticles exert toxicity, encompassing various cellular and molecular interactions. Furthermore, it discusses the assessment methods employed to evaluate nanotoxicity, encompassing in-vitro and in-vivo models, as well as emerging techniques. The review also addresses the regulatory issues surrounding nanotoxicology, highlighting the challenges in developing standardized guidelines and ensuring the secure translation of nanomedicine into clinical settings. It also explores into the challenges and ethical issues associated with nanotoxicology, as understanding the safety profile of nanoparticles is essential for their effective translation into therapeutic applications.

Abstract:Nanomedicine, under the spotlight of biomedical research, has embraced rapid evolution and considerable accomplishments in the fundamental discovery and drug development for potential clinical translation. For the past decades, many novel concepts have been proposed by using engineered nanoparticles or nanomaterials for drug delivery, disease prevention, intervention and monitoring. Given the unique physicochemical properties, nanoparticles protect the payload (therapeutic or imaging agents, nucleic acids, etc.) from degradation, fine-tune the in vivo transport for targeting and clearance, modulate the controllable payload release. 1 From the very first FDAapproved nano-drug in 1995 -Doxil (in the form of PEGylated liposomal doxorubicin) 2 to now broadly applied lipid nanoparticle-based Covid-19 mRNA vaccines (under emergency use authorization in 2020), 3,4 nanomedicines have not only improved the life quality of patients bearing cancers, rare genetic disorders 5 and other diseases, but also protected healthy people effectively from viral infection and resulting complications. Over a hundred other candidates are being investigated in clinical trials as nanoparticle vaccines, therapies and diagnostics. 6 Meanwhile, the overall success rate falls short in the clinical translation of nanomedicines. And the clinically approved nanomedicines are mostly in the context of liposomes or lipid-based nanocarriers. In many research studies, the strategies in nanomedicine design, material synthesis and analytics often lack the consideration of translation potential or clinically relevant solutions. 7 Notably, the practice of transforming an interesting concept into a scalable, applicable and marketable medical product is quite challenging, as its development may be hampered or terminated at any time due to the limitations towards preclinical proof of concept, chemistry, manufacturing and controls (CMC), clinical safety and efficacy, regulatory compliance and even post-approval. So, nanomedicine researchers have recently concluded a series of developmental barriers to overcome, 8 including the formulation, toxicological, physiological, immunological, and translational barriers. More importantly, the successful clinical translation involves extensive collaborations with pharmaceutical, clinical and regulatory groups in the assessment of drug's critical quality attributes, pharmacokinetics, dosimetry, therapeutic index, cost-effectiveness as well as consideration of the complexity of a disease.We opened the Research Topic on Nanomedicine Development and Clinical Translation last year, aiming to better bridge the basic science research and clinical translation for current nanomedicine development. Herein, our hope is to receive the input from innovative researchers pinpointing the underlying challenges and opportunities of nanomedicine, so that clinical translation of nano-products may be accelerated become successful broadly.

Abstract:Nanotherapeutics exhibit unique advantages in clinical outcomes compared with conventional small molecular drugs. The past three decades have seen intensive research in the field of nanomedicine, leading to a large number of nanomedical products commercialized globally as well as a pipeline of candidates being translated. However, the development of nanomedicine still faces poor efficacy in many applications. The key to advance the field is a deep understanding of the current barriers, addressable challenges, and future demands. Herein, we first present an overview of the more conventional nanotherapeutics that have been widely investigated so far. This is accompanied by a discussion of the next-generation precision and high-efficient nanotherapeutics, exemplified by functional nanodrugs with enhanced passive targeting, active targeting, and stimuli responsiveness. Next, we emphasize the importance of translating nanomedicine research from bench to bed, detailing issues of safety evaluation, biological fate, manufacturing, cost, and regulatory hurdles. Finally, we provide our perspectives on the challenges and opportunities awaiting scientists to advance this exciting field.

Abstract:The design and use of materials in the nanoscale size range for addressing medical and health-related issues continues to receive increasing interest. Research in nanomedicine spans a multitude of areas, including drug delivery, vaccine development, antibacterial, diagnosis and imaging tools, wearable devices, implants, high-throughput screening platforms, etc. using biological, nonbiological, biomimetic, or hybrid materials. Many of these developments are starting to be translated into viable clinical products. Abstract:Burgeoning applications of nanotechnology are altering practices in traditional medicine. Promoted by the National Institutes of Health, nanomedicinal research is advancing technologies and revolutionizing strategies in clinical science by providing easy access to innovative nanodevices and nanosystems based on the rational design and precise integration of functional nanomaterials. Many long-standing challenges in clinical science could be met through advancement and revolutionization. Nanomedicinal diagnostics could acquire critical information regarding the status of diseased tissues and organs quickly and inexpensively with minimal sampling size and invasion. New strategies in therapeutic and regenerative nanomedicines will enable clinicians to take actions in a timely fashion and patient-friendly manner.

Abstract:Abstract Nanomedicines have evolved into various forms including dendrimers, nanocrystals, emulsions, liposomes, solid lipid nanoparticles, micelles, and polymeric nanoparticles since their first launch in the market. Widely highlighted benefits of nanomedicines over conventional medicines include superior efficacy, safety, physicochemical properties, and pharmacokinetic/pharmacodynamic profiles of pharmaceutical ingredients. Especially, various kinetic characteristics of nanomedicines in body are further influenced by their formulations. This review provides an updated understanding of nanomedicines with respect to delivery and pharmacokinetics. It describes the process and advantages of the nanomedicines approved by FDA and EMA. New FDA and EMA guidelines will also be discussed. Based on the analysis of recent guidelines and approved nanomedicines, key issues in the future development of nanomedicines will be addressed.

Abstract:This review summarizes recent progress performed in the design and application of analytical tools and methodologies using nanomaterials for pharmaceutical analysis, and specifically new nanomedicines at distinct phases of development and translation from preclinical to clinical stages. Over the last 10-15 years, a growing number of studies have utilized various nanomaterials, including carbon-based, metallic nanoparticles, polymeric nanomaterials, materials based on biological molecules, and composite nanomaterials as tools for improving the analysis of pharmaceutical products. New and more complex nanomaterials are currently being explored to influence different stages of the analytical process. These materials provide unique properties to support the extraction of analytes in complex samples, increase the selectivity and efficiency of chromatographic separations, and improve the analytical properties of many sensor applications. Indeed, nanomaterials, including electrochemical detection approaches and biosensing, are expanding at a remarkable rate. Furthermore, the analytical performance of numerous approaches to determine drugs in different matrices can be significantly improved in terms of precision, detection limits, selectivity, and time of analysis. However, the quality control and metrological characterization of the currently synthesized nanomaterials still depend on the development of new and improved analytical methodologies, and the application of specific and improved instrumentation. Therefore, there is still much to explore about the properties of nanomaterials which need to be determined even more precisely and accurately.

Abstract:The field of nanotechnology has been a significant research focus in the last thirty years. This emphasis is due to the unique optical, electrical, magnetic, chemical and biological properties of materials approximately ten thousand times smaller than the diameter of a hair strand. Researchers have developed methods to synthesize and characterize large libraries of nanomaterials and have demonstrated their preclinical utility. We have entered a new phase of nanomedicine development, where the focus is to translate these technologies to benefit patients. This review article provides an overview of nanomedicine's unique properties, the current state of the field, and discusses the challenge of clinical translation. Finally, we discuss the need to build and strengthen partnerships between engineers and clinicians to create a feedback loop between the bench and bedside. This partnership will guide fundamental studies on the nanoparticle‐biological interactions, address clinical challenges, and change the development and evaluation of new drug delivery systems, sensors, imaging agents, and therapeutic systems.

Abstract:The development and application of nanotechnology-related knowledge and tools in modern medicine has been showing great potentials in elevating human's living standards and improving mankind's healthcare conditions via the prevention, early detection, diagnosis, treatment and follow-up of various diseases. The significant progress of nanomedicine has been achieved by the exploration of nanoparticulate formulations for theranostic purposes in a living organism. Among abundant nanoparticles, inorganic nanoparticles feature distinctive physiochemical properties and biological effects, with which conventional organic counterparts are typically not endowed. In the past thirty years, to be true, very few inorganic nanomaterials have been made from bench to bedside translation. It is thus more vital than ever for the scientific community to make ever-greater efforts to realize their clinical translations. Therefore, this review highlights and discusses such a compelling topic on the clinical trials and translation of inorganic nanoparticles. We will provide an update of the current progresses in clinic-relevant applications in terms of inorganic nanoparticles being used as detecting tools, contrast agents as well as therapeutic vehicles. Especially, the crucial challenges and future prospects in translating inorganic nanoparticles into clinical practices are highlighted and outlooked. This review aims to bring the issues of clinical relevance and translatability of inorganic nanoparticles to the forefront, which is the primary impetus, and also critically important, for the development of inorganic nanoparticles-based nanobiotechnology and nanomedicine.

Abstract:The concept of nanomedicine is not new. For instance, some nanocrystals and colloidal drug molecules are marketed that improve pharmacokinetic characteristics of single-agent therapeutics. For the past two decades, the number of research publications on single-agent nanoformulations has grown exponentially. However, formulations advancing to pre-clinical and clinical evaluations that lead to therapeutic products has been limited. Chronic diseases such as cancer and HIV/AIDS require drug combinations, not single agents, for durable therapeutic responses. Therefore, development and clinical translation of drug combination nanoformulations could play a significant role in improving human health. Successful translation of promising concepts into pre-clinical and clinical studies requires early considerations of the physical compatibility, pharmacological synergy, as well as pharmaceutical characteristics (e.g. stability, scalability and pharmacokinetics). With this approach and robust manufacturing processes in place, some drug-combination nanoparticles have progressed to non-human primate and human studies. In this article, we discuss the rationale and role of drug-combination nanoparticles, the pre-clinical and clinical research progress made to date and the key challenges for successful clinical translation. Finally, we offer insight to accelerate clinical translation through leveraging robust nanoplatform technologies to enable implementation of personalised and precision medicine.

Abstract:Compared with traditional drug therapy, nanomedicines exhibit intriguing biological features to increase therapeutic efficiency, reduce toxicity and achieve targeting delivery. This review provides a snapshot of nanomedicines that have been currently launched or in the clinical trials, which manifests a diversified trend in carrier types, applied indications and mechanisms of action. From the perspective of indications, this article presents an overview of the applications of nanomedicines involving the prevention, diagnosis and treatment of various diseases, which include cancer, infections, blood disorders, cardiovascular diseases, immuno-associated diseases and nervous system diseases, etc. Moreover, the review provides some considerations and perspectives in the research and development of nanomedicines to facilitate their translations in clinic.

Abstract:Abstract New nanomedicine formulations and novel applications of nanomedicinal drugs are reported on an almost daily basis. While academic progress and societal promise continue to shoot for the stars, industrial acceptance and clinical translation are being looked at increasingly critically. We here discuss five key challenges that need to be considered when aiming to promote the clinical translation of nanomedicines. We take the perspective of the end-stage users and consequently address the developmental path in a top-down manner. We start off by addressing central and more general issues related to practical and clinical feasibility, followed by more specific preclinical, clinical, and pharmaceutical aspects that nanomedicinal product development entails. We believe that being more aware of the end user’s perspective already early on in the nanomedicine development path will help to better oversee the efforts and investments needed, and to take optimally informed decisions with regard to market opportunities, target disease indication, clinical trial design, therapeutic endpoints, preclinical models, and formulation specifications. Critical reflections on and careful route planning in nanomedicine translation will help to promote the success of nanomedicinal drugs.

Abstract:Nanomaterials (NMs) are having a huge impact in several domains, including the fabrication of medical devices (MDs). Hence, nanostructured MDs are becoming quite common; nevertheless, the associated risks must be carefully considered in order to demonstrate safety prior to their immission on the market. The biological effect of NMs requires the consideration of methodological issues since already established methods for, e.g., cytotoxicity can be subject to a loss of accuracy in the presence of certain NMs. The need for oversight of MDs containing NMs is reflected by the European Regulation 2017/745 on MDs, which states that MDs incorporating or consisting of NMs are in class III, at highest risk, unless the NM is encapsulated or bound in such a manner that the potential for its internal exposure is low or negligible (Rule 19). This study addresses the role of NMs in medical devices, highlighting the current applications and considering the regulatory requirements of such products.

Legal aspects of the risks raised by nanotechnologies in the field of medicine

Abstract:The major breakthroughs achieved in nanotechnologies open new avenues in the field of healthcare--aid to diagnosis, upgrading medical treatment efficacy, development of regenerative medicine--but they are also associated with risks, hence the increasing need of legislation. So far very little research work has been conducted on this technology whose applications are still limited and whose potential hazards are not yet clearly apprehended. The more and more frequent uses of nanoparticles in medical imaging and in current research projects dealing with tissue engineering or RFID raises the following question: is the current legislative framework relevant in light of the specificities of nano-objects? The challenge is twofold: the legal approach must encompass the nanometric element itself as a "legal object" but it must include the use of nanotechnologies and their final aims. There is still some degree of uncertainty concerning the innocuity of nanoparticles so that the use of nanoelements in aid to medical diagnosis and in clinical trials must take into account and anticipate the potential harmful effects on patients and on biomedical research teams. But due to the fact that a clear understanding of nanoparticles as specific objects with new features is still missing, the existing regulations on chemical substances, medicinal products, medical devices or cosmetics do not seem to be appropriate. So considering nanoparticles as "singular" legal objects is a prerequisite requiring an approach based on the precautionary principle. Misusing nanotechnologies in the medical field is also a cause for great concern. Threats on individual freedom and on private life as well as on human identity are real and they raise recurring questions. The possible deviations in the use of these techniques, the temptations to "trespass the limits" are also common to info technologies and to biotechnologies but the threats triggered by the nanotechnologies are enhanced by the possibilities offered by the nanometric size and the expected convergence of these different technologies. One should refer to leading guiding principles in order to solve the future conflicts between the different sets of values, especially in the medical field by always remembering the Hippocratic oath "primum non nocere, deinde curare"

Abstract:Nanotoxicology and nanomedicine are two sub-disciplines of nanotechnology focusing on the phenomena, mechanisms, and engineering at the nano-bio interface. For the better part of the past three decades, these two disciplines have been largely developing independently of each other. Yet recent breakthroughs in microbiome research and the current COVID-19 pandemic demonstrate that holistic approaches are crucial for solving grand challenges in global health. Here we show the Yin and Yang relationship between the two fields by highlighting their shared goals of making safer nanomaterials, improved cellular and organism models, as well as advanced methodologies. We focus on the transferable knowledge between the two fields as nanotoxicological research is moving from pristine to functional nanomaterials, while inorganic nanomaterials – the main subjects of nanotoxicology – have become an emerging source for the development of nanomedicines. We call for a close partnership between the two fields in the new decade, to harness the full potential of nanotechnology for benefiting human health and environmental safety.

Feeding Next-Generation Nanomedicines to Europe: Regulatory and Quality Challenges

Abstract:New and innovative nanomedicines have been developed and marketed over the past half-century, revolutionizing the prognosis of many human diseases. Although a univocal regulatory definition is not yet available worldwide, the term "nanomedicines" generally identifies medicinal products that use nanotechnology in their design or production. Due to the intrinsic high structural complexity of these products, the scientific and regulatory communities are reflecting on how to revise the regulatory framework to provide a more appropriate benefit/risk balance to authorize them on the market, considering the impact of their peculiar physicochemical features in the evaluation of efficacy and safety patterns. Herein, a critical perspective is provided on the current open issues regarding regulatory qualification and physicochemical characterization of nanosystems considering the current European regulatory framework on nanomedicine products. Practicable paths for improving their quality assurance and predicting their fate in vivo are also argued. Strengthening the multilevel alliance among academic institutions, industrial stakeholders, and regulatory authorities seems strategic to support innovation by standard approaches (e.g., qualification, characterization, risk assessment), and to expand current knowledge, also benefiting from the new opportunities offered by artificial intelligence and digitization in predictive modelling of the impact of nanomedicine characteristics on their fate in vivo.
