Abstract:Background: This study aimed to analyse the 3D patterns of the mandibular and mental canals (MDC and MC) referring to the surrounding prominent surgical landmarks such as teeth and mental foramen by cone beam computed tomography (CBCT). Methods: CBCT scans of 354 patients aged 18-67 years with mandibular first premolar to second molar were included and reconstructed 3-dimensionally (3D) by mimics. The parameters of MDC and MC were measured referring to teeth and mental foramen. Results: From the first premolars to the second molars, the mandibular canals showed a trend of gradually closer to the cementoenamel junction (CEJ) of the adjacent teeth and farther away from the buccal cortical plate. The distance of the MDC with the root apexes (RA) was relatively constant from the first premolar to the first molar, but became much closer to the second molar. About 10.8% of the second molars had MDC-RA distances of shorter than 2 mm, and 1.34% even had the MDC superior to the RA. Moreover, the Type III of MC presented in 66.0% of the subjects and had a relatively longer length. Besides, the existence of Type I MC may be related to the MDC featuring with close distances to the RA and CEJ of the adjacent teeth. Conclusion: Dentists and surgeons should know the patterns of mandibular and mental canals. A better understanding of the MDC and MC and their relationship to local anatomical landmarks may facilitate the planning of surgeries and alert potential nerve injuries in the operative procedures.
Use of CBCT to Investigate the Root Canal Morphology of Mandibular Incisors.

Abstract:PURPOSE:To investigate the root canal morphology of mandibular incisors using cone-beam computed tomography (CBCT).MATERIALS AND METHODS:A total of 353 patients with 1,412 healthy, well-developed mandibular incisors were enrolled. Radiographic examination by CBCT was conducted as part of their routine examination, diagnosis and treatment planning. The following observations were made using CBCT: (1) the number of roots; (2) the number of canals; (3) canal configuration according to Vertucci's classification; (4) the position of root canal bifurcations.RESULTS:Two canals were found in 10.9 % of mandibular central incisors, 25.5 % of lateral incisors and in 18.2 % of all the 1,412 mandibular incisors. Significantly, more lateral incisors possessed two canals than central incisors (p < 0.05). Of the teeth with two canals, type III incisors were the most prevalent, followed by types II, IV and V. Furthermore, 37.7 % of teeth were found to have root canal bifurcations that were at or near to the cortical-middle third junction regions of the roots.CONCLUSION:Routine mode CBCT imaging was clinically useful for detection of two canals and determines the position of root canal bifurcations in mandibular incisors.
The study of mandibular alveolar nerve canal by cone-beam computed tomography

Abstract:Objective To analysis the structure of the inferior alveolar nerve canal(IANC) in the mandible using cone-beam computed tomography(CBCT). Methods Sixty patients with the CBCT data of the mandible posterior part were enrolled in this study. IANC images were measured with KaVo eXam Vision software in different regions. The independent sample t-tests were used to analyze the variable differences. Results IANC is close to the mandibular lower border and goes from the lingual side to the buccal side of the mandible in the area between mandibular second molar to mental foramen. There is no significant difference between the left side and the right side, neither between females and males(P > 0.05). Conclusions CBCT could be used in the observation of the structure of IANC. The characteristics of IANC should be taken into consideration in wisdom tooth extraction and dental implant placement.

Abstract:OBJECTIVE:To investigate the root and canal morphology of maxillary first and second molars by using Cone-beam computed tomography (CBCT).METHODS:The data of patients who underwent the CBCT scan for dental or periodontal diseases between May and September 2012 were analyzed retrospectively. In the study, 630 maxillary first molars, 519 maxillary second molars which were well-developed and without treatment were included. The number of roots, the number of root canals, and the canal configuration according to Vertucci's classification were determined. The vertucci type I was regarded as the conventional form. Other types were the variation forms and the mutation rate was calculated.RESULTS:2.38% of the maxillary first molars and 15.22% of the maxillary second molars had 2 separated roots; 97.14% of the maxillary first molars and 73.60% of the maxillary second molars had 3 roots; 10.41% of the maxillary second molars had one single root; 0.77% of the maxillary second molars had 4 roots. In 612 3-rooted maxillary first molars, additional canals were found in 30.88% of the mesiobuccal (MB) roots. In 519 3-rooted maxillary second molars, additional canals were found in 13.87% of the MB roots.CONCLUSION:CBCT scans show the root and canal anatomy accurately, with the potential of improving the outcome of endodontic treatment.
[A Study of Trabecular Bone Structure in the Mandibular Condyle of Healthy Young People by Cone Beam Computed Tomography].

Abstract:OBJECTIVE:To evaluate the feasibility of cone beam computed tomography (CBCT) for the evaluation of trabecular bone structure in mandibular condyle and to investigate the distribution of the trabecular bone structure within mandibular condyle. METHODS:Eighty condyles from 40 healthy young volunteers (aged 20-32) were scanned by CBCT. A coronoid image was acquired of each condyle and divided into 8 regions where regions of interest were specified. After CBCT images were binarized, four morphological parameters including bone volume fraction, trabecular thickness, trabecular number and trabecular separation were computed. RESULTS:All parameters were significantly different between the superior zone and middle/inferior zone of the condyle (P < 0.05). Superior zone showed the largest bone volume fraction (52.2%), the highest trabecular number (1.33 mm(-1)), the thinnest trabecular thickness (393.48 microm), and the smallest trabecular separation (361.59 microm). Inferior zone showed the smallest bone volume fraction (49.64%). These results were not significantly different between bilateral sides of the condyles (P > 0.05). CONCLUSIONS:Trabecular bone structure was inhomogeneous within the condyle, but symmetrical between bilateral sides of the condyles. CBCT combined with image processing is a feasible tool in evaluating trabecular bone structure of human mandibular condyle.

Abstract:Objective The purpose of this systematic review was to evaluate the mandibular canal (MC) variations and their anatomical incidences using cone-beam computed tomography (CBCT). Designs Related articles on the anatomical variations of the MC using CBCT were searched through PubMed, Cochrane, and China National Knowledge Infrastructure (CNKI) to identify all relevant articles eligible for inclusion. Data extracted included incidences of the anatomical variations of the MC including the accessory mental foramen (AMF), retromolar foramen (RMF), mandibular lingual foramina (MLF), and the bifid mandibular canal (BMC). Results In total, 16 descriptive cross-sectional studies (one low quality and fifteen high quality) were included in the meta-analysis analyzing 8862 MC. The meta-analysis performed with a random-effects model showed that the incidence of AMF was 9.54% (95% CI 6.39–12.69%), the incidence of RMF was 23.64% (95% CI 14.44–32.84%). MLF was found in almost all adults studied, and the incidence of BMC was about 38.0%. Conclusions The foramina and canals branched from the MC presented significant anatomical variations among individuals. CBCT-based study on the anatomical variations of the MC could provide guidance for clinical practice.

Abstract:Objectives. The aim of this study was to evaluate and quantify variations of bifid mandibular canals (BMCs) in a population of Northern China by using cone beam computed tomography (CBCT). Study Design. CBCT images of 1000 consecutive patients were analyzed by using the NewTom proprietary software. BMCs were identified and classified on the basis of the Naitoh classification. Linear and angular measurements of BMCs were performed. Statistical analyses were conducted by using chi(2) and Wilcoxon tests. Results. BMCs were observed in 13.2% of 1000 patients and 8.4% of 2000 sides. The prevalence of BMCs was significantly lower in patients in the first 2 decades and in cases with a class II molar relationship. The retromolar canal (68.4%) was the most common type of BMC observed. No buccolingual canals were identified; however, 2 special canals were detected. A classification system of 3 subtypes of retromolar canals was suggested. On average, the beginning site of the branches from the opening of the main canal was at a distance of 8.1 mm. The mean diameter and length of BMCs were 2.1 mm and 12.6 mm, respectively. Conclusions. This study underlined the prevalence and characteristics of BMCs in a population of Northern China. Preoperative identification of BMCs with CBCT may help prevent postoperative complications.

Comparative Study of Root Canal Morphology of Mandibular Incisors by Cone-Beam CT and Canal Staining and Clearing Technique

Abstract:PURPOSE:To compare the root canal morphology of mandibular incisors by using cone-beam CT (CBCT) and canal staining and clearing technique.METHODS:Sixty-one extracted mandibular incisors with complete dental root and apex which received no endodontic treatment and no post crown restoration were selected. Each tooth was radiographed with CBCT, and the root canal system was stained by canal staining and clearing technique. The consistency of the number of root canal, root canal Vertucci type of mandibular permanent incisors between the two methods were compared, and the differences of the detection rate on root canal branch structure between the two methods were analyzed. The results were statistically analyzed with SPSS 17.0 software package.RESULTS:The Kappa value of single and double root canal types between CBCT and canal staining and clearing technique was 0.847 (P<0.001). The Kappa value of Vertucci root canal types between CBCT and canal staining and clearing technique was 0.861 (P<0.001). The detection rates of root canal branch structure were 8.19% and 22.95%, respectively, with significant difference between the two methods (P=0.025). The canal staining and clearing technique was significantly better than CBCT in detection of root canal branch structure.CONCLUSIONS:CBCT can reflect the root canal types nearly perfectly, but inferior to canal staining and clearing technique in detection of root canal branch structure, CBCT is a relatively accurate clinical diagnosis tool of root canal morphology.

Abstract:Objectives: To evaluate the accuracy of CBCT in assessing trabecular structures.Methods: Two human mandibles were scanned by micro-CT (Skyscan 1173 high-energy spiral scan micro-CT; Skyscan NV, Kontich, Belgium) and CBCT (3D Accuitomo 170; Morita, Japan). The CBCT images were reconstructed with 0.5 and 1mm thicknesses. The condylar images were selected for registration. A parallel algorithm for histogram computation was introduced to perform the registration. A mutual information (MI) value was used to evaluate the match between the images obtained from micro-CT and CBCT.Results: In comparison with the micro-CT image for the two samples, the CBCT image with 0.5mm thickness has a MI value of 0.873 and 0.903 while that with 1.0mm thickness has a MI value of 0.741 and 0.752. The CBCT images with 0.5mm thickness were better matched with micro-CT images.Conclusions: CBCT shows comparable accuracy with high-resolution micro-CT in assessing trabecular structures. CBCT can be a feasible tool to evaluate osseous changes of jaw bones.

Abstract:Background Recently, using cone-beam computed tomography (CBCT) to assess root canal morphology has become popular; however, few studies have examined its efficiency to assess the entire root canals, including the tiny lateral and accessory canals (LACs). This study aimed to assess the ability of CBCT to evaluate the root canal of mandibular incisors at three different scanning settings, compared with the canal staining and tooth clearing (CS) technique as the gold standard. Methods CBCT images of 70 extracted mandibular incisors were taken using NewTom VG CBCT at high-resolution scan mode (HZ), zoom scan mode (ZS), and full scan mode (FS), with different scanning settings. A radiologist, a postgraduate student, and an endodontist assessed the root canal morphology in a blinded manner. The number of root canals (NC), canal configuration according to Vertucci’s classification (VC), and LACs were evaluated twice by each evaluator using the CBCT images, in comparison with CS. Comparisons of the differences were used the chi-square test, and the intra-evaluator and inter-evaluator agreement were used the Kappa statistics; the significance level was set at 0.05. Results The voxel dimension of HZ, ZS and FS modes were 0.125 mm, 0.20 mm and 0.25 mm respectively, and the HZ mode had significant increased scanning doses. For NC, the diagnostic accuracy was >90% in all three modes, with no significant difference among the evaluators and modes. VC and LAC could only be evaluated in HZ mode. For VC, the accuracies were 97.1%, 94.3%, and 92.9% respectively, with no significant differences among the three evaluators. For LAC, the accuracies were 80.0%, 13.3%, and 33.3% respectively, and there were significant differences among the three evaluators. Intra-evaluator agreement was excellent, with the kappa values indicating “perfect” to “substantial” agreement. Inter-evaluator agreement was excellent for NC and VC; however, Kappa values could not be analyzed due to LACs detected were so variable. Conclusions As far as possible, the HZ mode should be chosen to demonstrate the root canal system, and partial LACs could be detected using this mode; however, the potential benefit of the diagnostic information must be weighed against the increased radiation dose.

Abstract:To propose a method of establishing the reference mandibular plane (MP), which could be reestablished according to the coordinates of the reference points, and then facilitate the assessment of anterior alveolar morphology using cone beam computed tomography (CBCT), sixty patients with bimaxillary protrusion were randomly selected and CBCT scans were taken. The CBCT scans were transferred to Materialism’s interactive medical image control system 10.01 (MIMICS 10.01), and three dimensional models of the entire jaws were constructed. Reference points determining the reference MP were positioned in the coronal, axial, sagittal windows, and the points were exactly located by recording their coordinates in the interfaces of software. The reference MP provided high intra-observer reliability (Pearson’s r 0.992 to 0.999), and inter-observer reliability (intra-class correlation coefficients (ICCs) 0.996 to 0.999).

Abstract:BACKGROUND/PURPOSE: Minimally invasive endodontic approach become a research hotspot and may prevent the fracture of endodontically-treated teeth. This research aims to measure the coronal root canal morphology of permanent mandibular first molars in 3D and propose a new minimally invasive endodontic approach based on this measurement.MATERIALS AND METHODS: Data of 136 permanent mandibular first molars were involved and reconstructed in 3D models with canals. Then, the morphology characteristics of the coronal root canal were measured.RESULTS: Overall, the distribution of root canal orifices was more centralized than other landmarks. The landmarks were located more mesiobuccally to the center of the occlusal plane of molars. Specifically, the measurements of the maximum curvature of coronal root canals in the axial direction were: in 3-canals 2-rooted teeth, the average angles of curvatures were 23°,25°,11° for mesiobuccal (MB), mesiolingual (ML) and distobuccal (DB) canals, respectively; in 4-canals 2-rooted teeth were 23°,25°,12°,16°for MB, ML, DB, and distolingual (DL) canals, respectively; in 4-canals 3-rooted teeth were 25°,27°,17°,39° for MB, ML, DB, and DL canals, respectively. The degrees of coronal root canal curvatures in the horizontal direction were: in 3-canals teeth, the average angles of curvatures were -1°,47°,-2° for MB, ML and DB canals, respectively; in 4-canals 2-rooted teeth were -4°,41°,-25°,48° for MB, ML, DB, and DL canals, respectively; in 4-canals 3-rooted teeth were -3°,33°,-43°,79° for MB, ML, DB, and DL canals, respectively.CONCLUSION: The results of this study are similar to those previously obtained using CBCT and can help us design endodontic approaches.

Abstract:Introduction: The Mandibular Lingual Canals (MLC), which carry the sublingual artery, are present in the mandible near the genial tubercle. Identification of these canals is important before any dental surgical procedures. Aim: To identify and evaluate the mandibular lingual canal in Cone Beam Computed Tomography (CBCT) of the Western Indian Population. Materials and Methods: The current cross-sectional study was conducted at the Department of Oral Medicine and Radiology, Sumandeep Vidyapeeth, Vadodara, Gujarat, India between October 2022 and June 2023. A total of 100 CBCT volumes showing the mandibular anterior region were evaluated for the presence or absence of a mandibular lingual canal. The canals were further evaluated for their number, diameter, length, thickness of the buccal cortical plate, and the remaining bone above and below the canal. The evaluation was done by two separate investigators. The data were entered into Microsoft Excel and subjected to statistical analysis. Data collected were tested statistically using the Chi-square test. Results: A total of 59 males and 41 females’ mandibular CBCT volumes were evaluated, out of which 93 CBCT volumes showed the mandibular lingual canal. The maximum number (n=57) of individuals showed three canals, followed by two canals (n=26) and one canal (n=10). There was no statistical significance between genders regarding canal number, diameter, length, thickness of the buccal cortical plate, and the remaining bone above and below the canal. Conclusion: Three-canal morphology was the most common anatomical form of MLC found in the Western Indian population.

Abstract:OBJECTIVETo investigate the root canal configuration of mandibular anterior teeth with cone-beam computed tomography (CBCT).METHODSThe CBCT imaging data of 866 patients who visited Peking University School of Stomatology from October 2012 to July 2013 were inspected by an endodontist anda radiologists together. A total of 4 674 mandibular anterior teeth were involved. The number of root, root canals and root canal configuration were observed and analyzed statistically (Chi-squared test).RESULTSAll the mandibular central incisors and lateral incisors were single root, and 0.7% (11/1 542) of canines were double roots. 6.7% (105/1 566) of central incisors, 17.4% (273/1 566) of lateral incisors and 3.0% (46/1 542) of canines had double root canals. The frequency of symmetry of double root canal was 58.7% (37/63) in central incisors, 76.1% (108/142) in lateral incisors and 29.6% (8/27) in canines. The highest frequency of double root canals in different ages was 9.8% (28/287, 31-40 years) in central incisors, 21.5% (61/284, 31-40 years) in lateral incisors and 9.2% (19/207, ≥51 years) in canines. Vertucci type III canal configurations were the most prevalent in mandibular anterior teeth.CONCLUSIONThe double root canals are most common in mandibular lateral incisors. The highest incidence of symmetry of double root canal is also observed in mandibular lateral incisors. Vertucci type III canal configurations are the most frequent type in mandibular anterior teeth.
Analysis of mandibular molar anatomy in Taiwanese individuals using cone beam computed tomography

Abstract:Background/purpose Before periapical surgery in the mandibular posterior teeth is performed, the thicknesses of the buccal alveolar bone wall and buccolingual root might be a critical issue. This study aimed to assess the anatomical structure of the posterior region of the mandible in Taiwanese individuals using cone-beam computed tomography (CBCT). Materials and methods The CBCT images of 96 Taiwanese individuals (51 male and 45 female), which included 192 mandibular first molars and 192 mandibular second molars, were imported into medical imaging software to measure the buccal alveolar bone thickness and buccolingual root thickness at 3 mm above the root apex. Statistical analysis was conducted to examine the impact of tooth position, gender, and age on the anatomical position of mandibular molars. Results The buccal alveolar bone thickness at 3 mm above the root apex of the mandibular second molar demonstrates a significantly higher value when compared to that of the first molar. Nonetheless, concerning the buccolingual root thickness, no significant differences were observed between these two teeth. In addition, the buccal alveolar bone thickness and buccolingual root thickness at 3 mm above the root apex may not be influenced by gender and age. Conclusion The anatomical structures of the posterior region of the mandible in Taiwanese individuals exhibited variations between the mandibular first and second molars. However, these differences were not influenced by gender or age.
