Abstract:Diet quality, assessed by the Healthy Eating Index-2015 (HEI-2015), the Alternative Healthy Eating Index-2010 (AHEI-2010), the alternate Mediterranean Diet (aMED) score, the Dietary Approaches to Stop Hypertension (DASH) score, and the Dietary Inflammatory Index (DII®), was examined in relation to risk of lung cancer in the Multiethnic Cohort Study. The analysis included 179,318 African Americans, Native Hawaiians, Japanese Americans, Latinos, and Whites aged 45-75 years, with 5350 incident lung cancer cases during an average follow-up of 17.5 ± 5.4 years. In multivariable Cox models comprehensively adjusted for cigarette smoking, the hazard ratios (95% confidence intervals) for the highest vs. lowest quality group based on quintiles were as follows: 0.85 (0.77-0.93) for HEI-2015; 0.84 (0.77-0.92) for AHEI-2010; 0.83 (0.76-0.91) for aMED; 0.83 (0.73-0.91) for DASH; and 0.90 (0.82-0.99) for DII. In histological cell type-specific analyses, the inverse association was stronger for squamous cell carcinoma than for adeno-, small cell, and large cell carcinomas for all indexes. There was no indication of differences in associations by sex, race/ethnicity, and smoking status. These findings support that high-quality diets are associated with lower risk of lung cancer, especially squamous cell carcinomas, in a multiethnic population.

Abstract:A case-control study involving interviews with 1,249 patients with lung cancer and 1,345 population-based controls was conducted in Shenyang, an industrial city in northeastern China, where mortality rates are high among men and women. Cigarette smoking was found to be the principal cause of lung cancer in this population, accounting for 55% of the lung cancers in males and 37% in females. The attributable risk percentage among females is high compared to elsewhere in China, largely because of a higher prevalence of smoking among women. After adjustment for smoking, there were also significant increases in lung cancer risk associated with several measures of exposure to air pollutants. Risks were twice as high among those who reported smoky outdoor environments, and increased in proportion to years of sleeping on beds heated by coal-burning stoves (kang), and to an overall index of indoor air pollution. Threefold increases in lung cancer risk were found among men who worked in the nonferrous smelting industry, where heavy exposures to inorganic arsenic have been reported. The associations with both smoking and indoor air pollution were stronger for squamous cell and small cell carcinomas than for adenocarcinoma of the lung. Risks due to smoking or air pollution were not greatly altered by adjustment for consumption of fresh vegetables or sources of beta carotene or retinol, prior chronic lung diseases, or education level. The findings suggest that smoking and environmental pollution combine to account for the elevated rates of lung cancer mortality in Shenyang.
Diet Quality and Risk of Bladder Cancer in the Multiethnic Cohort Study

Abstract:This study analyzed the overall quality of the diet using predefined indices, including the Healthy Eating Index-2015 (HEI-2015), the Alternative Healthy Eating Index-2010 (AHEI-2010), the alternate Mediterranean Diet (aMED) score, the Dietary Approaches to Stop Hypertension (DASH) score, and the Dietary Inflammatory Index (DII®), to explore their association with the risk of bladder cancer in the Multiethnic Cohort Study. Data were taken from 186,979 African American, Japanese American, Latino, Native Hawaiian, and non-Hispanic White participants aged 45–75 years, with 1152 incident cases of invasive bladder cancer during a mean follow-up period of 19.2 ± 6.6 years. Cox models were used to calculate hazard ratios (HRs) and 95% confidence intervals (CIs) with comprehensive adjustment for smoking. Comparing the highest vs. lowest diet quality score quintile, HRs (95% CIs) in men was 1.08 (0.86–1.36) for HEI-2015, 1.05 (0.84–1.30) for AHEI-2010, 1.01 (0.80–1.27) for aMED, 1.13 (0.90–1.41) for DASH, and 0.96 (0.76–1.21) for DII®, whereas the corresponding HRs for women were 0.75 (0.53–1.07), 0.64 (0.45–0.92), 0.60 (0.40–0.88), 0.66 (0.46–0.95), and 0.63 (0.43–0.90) with all p values for trend <0.05. The inverse association found in women did not vary by smoking status or race and ethnicity. Our findings suggest that adopting high-quality diets may reduce the risk of invasive bladder cancer among women in a multiethnic population.

Abstract:Lung cancer is the leading cause of cancer-related death worldwide. In China, the incidence of lung cancer has grown rapidly, resulting in a large social and economic burden. Several researchers have devoted their studies to lung cancer and have demonstrated that there are many risk factors for lung cancer in China, including tobacco use, environmental pollution, food, genetics, and chronic obstructive pulmonary disease. However, the lung cancer incidence is still growing rapidly in China, and there is an even higher incidence among the younger generation. One explanation may be the triple-neglect situation, in which medical policies that neglect prevention, diagnosis, and supportive care have increased patients' mortality and reduced their quality of life. Therefore, it is necessary to enhance the efficiency of prevention and early diagnosis not only by focusing more attention on treatment but also by drawing more attention to supportive care for patients with lung cancer. Abstract:Background: Few studies have been conducted in China to investigate the association between diet and the risk of head-and-neck cancer (HNC). The aim of this study was to determine the relationship between diet and HNC risk in the Chinese population and to examine whether smoking status has any effect on the risk.Methods: Our multicenter case-control study included 921 HNC cases and 806 controls. We obtained information on the frequency of both animal-and plant-based food consumption. Unconditional logistic regression was used to estimate the odds ratios (ORs) and 95% confidence intervals (95% CIs).Results: The risk of HNC increased with more frequent consumption of processed meat and fermented foods but decreased with frequent consumption of fruits and vegetables. There was a significant increasing P for trend of 0.006 among smokers who consumed meat and an increased OR among smokers who consumed processed meat (OR 2.95, 95%CI 1.12-7.75). Protective odds ratios for vegetable consumption were observed among smokers only. We also observed protective odds ratios for higher egg consumption among never-smokers (P for trend = 0.0.003).Conclusions: Reduced HNC risks were observed for high fruit and vegetable intake, a finding consistent with the results of previous studies. Processed meat intake was associated with an increased risk. The role of dietary factors in HNC in the East Asian population is similar to that in European populations. (C) 2016 Elsevier Ltd. Abstract:The aims of our study were to estimate the contribution of known lung cancer risk factors, and provide evidence to support the National Cancer Prevention and Control Program in China. We calculated the proportion of lung cancer attributable to specific risk factors. Data on exposure prevalence were from meta-analyses and large-scale national surveys of representative samples of the Chinese population. Data on relative risks were from meta-analyses and large-scale prospective studies. Lung cancer mortality and incidence were taken from the Third National Death Survey and from cancer registries in China. We estimated that in China 285 304 lung cancer deaths and 327 465 cases were attributable to smoking, involuntary smoking (women only), occupational exposure, indoor radon, and low fruit intake in 2005. The proportion of lung cancer deaths attributable to smoking, involuntary smoking among non-smoking women, occupational exposure, indoor radon and low fruit intake was 56.9%, 11.1%, 9.5%, 0.2% and 12.4%, respectively. About 41% of lung cancer mortality and incidence in women was caused by risk factors in our study. However, over half of lung cancer deaths and cases among women were not attributable to known risk factors. It is necessary to conduct large-scale studies to identify additional risk factors of lung cancer in non-smoking women.
Environmental and dietary factors and lung cancer risk among Chinese women: a case-control study in southeast China

Abstract:After decades of increase, lung cancer has become the leading cause of cancer death among women. The mortality rate from lung cancer is ascending at a relatively steady rate, which has greatly affected the health of the female population and become a serious issue. A case-control study of 226 female lung cancer cases and 269 controls was conducted from 2006 to 2010 in Fujian Province. A structured questionnaire was used to gather information on demographic characteristics as well as dietary and environmental factors. Odds ratios and 95% confidence intervals were estimated by using univariate logistic regression. Multivariate unconditional logistic regression analysis was applied to evaluate the potential interactions of variables or confounders. The consumption of fruit, eggs, and tea was inversely associated with the risk of lung cancer. As expected, cooking oil fumes and environmental tobacco exposure were positively associated with elevated risk. In addition, frequent physical activity and late age at menarche were identified as protective factors of female lung cancer. The results demonstrate that some environmental and dietary factors are related to the risk of lung cancer among the female population in southeast China.

Abstract:Abstract Lung cancer incidence has been on the rise in less-developed countries. Indoor air pollution caused by poor ventilation, burning of solid fuels, and cooking oils has been associated with lung cancer risk in developing poor and rural countries in many retrospective case-control studies. However, few studies have been conducted in more urban and more economically developed populations, even less in prospective studies. To assess the relationship of indoor air quality and risk of lung cancer in a modern population, a prospective cohort study was conducted in the Shanghai Women's Health Study. 73,442 women were followed through December 2009. A total of 469 lung cancer patients were diagnosed during the follow-up period, but after excluding ever smokers, 71,320 women in the cohort and 429 women with lung cancer were included in this analysis. Standardized and structured questionnaires were used to collect information on all household living and cooking practices: past three residences lived, cooking fuel utilized, cooking oil utilized, ventilation conditions, smoking patterns of family at home, and coworker smoking at work. Effect estimates for household ventilation conditions, cooking fuels, and cooking oils use for the risk of lung cancer are presented as hazard ratios (HR) with 95% confidence intervals (95% CI) from Cox proportional hazards regression. Ever having poor household ventilation was associated with a 61% increase in lung cancer risk (HR: 1.61; 95% CI: 1.19-2.18), and more than 20 years was associated with a 100% increase in risk (HR: 2.00; 95% CI: 1.32-3.01) compared to always having good ventilation. Ever use of coal with poor ventilation was associated with a 71% increased risk of lung cancer (HR: 1.71; 95% CI: 1.19-2.46), and more than 20 years of using coal with poor ventilation was associated with an 83% increase (HR: 1.83; 95% CI: 1.16-2.89) compared to gas users with good ventilation. There was some suggestion of lung cancer risk with vegetable oil use compared to soybean oil use, although the number of lung cancer cases was few and not statistically stable. In summary, this prospective cohort study of non-smoking Chinese women suggests that exposure to poor ventilation and poorly vented cooking coal increases the risk of lung cancer. These findings were consistent with past retrospective case-control studies and suggesting that past exposure to coal use with poorly ventilated conditions pose a health risk. These results suggest that current modern day cooking conditions, particularly in poorly ventilated homes, is still a public health issue. Dietary Fat Intake and Lung Cancer Risk: A Pooled Analysis.

Abstract:Purpose Dietary fat may play a role in lung carcinogenesis. Findings from epidemiologic studies, however, remain inconsistent. In this pooled analysis of 10 prospective cohort studies from the United States, Europe, and Asia, we evaluated the associations of total and specific types of dietary fat with lung cancer risk. Methods Cox regression was used to estimate hazard ratios (HRs) and 95% CIs in each cohort. Study-specific risk estimates were pooled by random- or fixed-effects meta-analysis. The first 2 years of follow-up were excluded to address potential influence of preclinical dietary changes. Results Among 1,445,850 participants, 18,822 incident cases were identified (mean follow-up, 9.4 years). High intakes of total and saturated fat were associated with an increased risk of lung cancer (for highest v lowest quintile: HR, 1.07 and 1.14, respectively; 95% CI, 1.00 to 1.15 and 1.07 to 1.22, respectively; P for trend for both < .001). The positive association of saturated fat was more evident among current smokers (HR, 1.23; 95% CI, 1.13 to 1.35; P for trend < .001) than former/never smokers ( P for interaction = .004), and for squamous cell and small cell carcinoma (HR, 1.61 and 1.40, respectively; 95% CI, 1.38 to 1.88 and 1.17 to 1.67, respectively; P for trend for both < .001) than other histologic types ( P for heterogeneity < .001). In contrast, a high intake of polyunsaturated fat was associated with a decreased risk of lung cancer (HR, 0.92; 95% CI, 0.87 to 0.98 for highest v lowest quintile; P for trend = .02). A 5% energy substitution of saturated fat with polyunsaturated fat was associated with a 16% to 17% lower risk of small cell and squamous cell carcinoma. No associations were found for monounsaturated fat. Conclusion Findings from this large, international cohort consortium suggest that modifying dietary fat intake (ie, replacing saturated fat with polyunsaturated fat) may reduce lung cancer risk, particularly among smokers and for squamous cell and small cell carcinoma.
Low-fat dairy consumption and the risk of lung cancer: A large prospective cohort study

Abstract:Background: Despite the possible contribution of dairy products to the development or prevention of cancers, there is a lack of epidemiological evidence linking low-fat dairy consumption to the risk of developing lung cancer. This research was conducted to fill this knowledge gap. Methods: The data for this research were collected from the Prostate, Lung, Colorectal, and Ovarian (PLCO) Cancer Screening Trial. The Cox proportional risk model was employed to evaluate the link between low-fat dairy consumption and the risk of developing lung cancer. Hazard ratios (HRs) and 95% confidence intervals (CIs) were measured in both unadjusted and adjusted models. A series of predefined subgroup analyses were performed to identify potential effect modifiers, and several sensitivity analyses were conducted to assess the stability of the findings. Results: The study included data from 98,459 individuals. During a total of 869,807.9 follow-up person-years, 1642 cases of lung cancer were observed, with an incidence of 0.189 cases for every 100 person-years. In the fully adjusted model, participants in the highest quartile of low-fat dairy consumption had a significantly decreased risk of lung cancer compared to the ones in the lowest quartile (HRquartile 4 vs. 1 : 0.769, 95% CI: 0.664, 0.891, ptrend = 0.005). The restricted cubic spline plot revealed an inverse nonlinear dose-response relationship between low-fat dairy consumption and lung cancer risk (pnonlinearity = 0.008). Subgroup analyses demonstrated that the inverse association was stronger among participants with higher daily caloric intake (pinteraction = 0.031). Various sensitivity analyses produced consistent results. Conclusion: Consuming more low-fat dairy products is significantly linked to a reduced risk of developing lung cancer, indicating that an appropriate increase in the use of low-fat dairy products may help prevent lung cancer.
A Case-Control Study of Dietary Factors in Patients with Lung Cancer

Abstract:A case-control study was designed to investigate association of dietary factors with the risk of lung cancer in Sichuan, China. The cases consisted of 135 patients with preinvasive lung cancer which had been confirmed with histopathology, fiber bronchoscope, CT and X-ray film in three provincial hospitals in the recent one year. Controls were healthy subjects who went to one of these hospitals for health check-up; patients with pulmonary diseases was excluded. Controls were matched to cases for sex and age with a ratio of 1:1. Nutrient intakes, the eating habit and other relevant factors were investigated. The data analyzed with the conditional logistic regression model indicated that dietary beta-carotene intakes had a significantly inverse association with the risk of lung cancer. Vitamin C had a less significantly inverse association with the risk. Association of protein, fat, energy, retinol intakes or diet-balance index with the risk was not significant. Association of tea, alcohol, garlic or mushroom, respectively, with the risk was also not observed. Consumption of more processed foods and deep-fried foods were found to be risk factors. Smoking and air pollution from coal burning stoves were also observed as independent risk factors of lung cancer in the present study. The mental stress incidence in the case was significantly higher than that in the control.

Abstract:Abstract Importance: Reducing lung cancer risk by modifying diet is highly desirable. However, the association of different U.S. dietary patterns with lung cancer risk is poorly understood, especially for regional dietary patterns. Objective: We investigated whether different U.S. dietary patterns were associated with lung cancer risk and whether the associations could be modified by genome-wide association study identified susceptibility loci. Design, Setting, and Participants: Study participants were accrued from a large ongoing case-control study of lung cancer initiated in 1995. Cases were newly-diagnosed and histologically confirmed non-small cell lung cancer patients from The University of Texas MD Anderson Cancer Center, and healthy controls without a history of cancer were from the Kelsey-Seybold Clinics. We limited the analysis to non-Hispanic whites, and after exclusion, 2,139 cases and 2,163 frequency-matched controls on age (±5 years), sex, and smoking status (current, former, never) were included in the final analysis. Exposures: Dietary intake was assessed with a validated food frequency questionnaire. Three dietary patterns (i.e., “Tex-Mex”, “fruits and vegetables”, and “American/Western”) were derived using exploratory factor analysis, and factor scores of derived dietary patterns were categorized into quintiles. Main Outcome and Measure: The association between dietary patterns and lung cancer risk. Results: The adjusted odds ratios comparing the highest to the lowest quintile of the factor scores of “Tex-Mex”, “fruits and vegetables”, and “American/Western” patterns were 0.45 (95% CI = 0.37-0.56), 0.68 (95% CI = 0.55-0.85), and 1.45 (95% CI = 1.18-1.78), respectively. The effects were stronger for squamous cell carcinoma and among former or current smokers for the “fruits and vegetables” pattern, and stronger for other non-small cell lung cancer and among never smokers for the “American/Western” pattern. Additionally, a variant (rs2808630) of the C-reactive protein gene significantly modified the associations of the “fruits and vegetables” (P for interaction = 0.03) and “American/Western” (P for interaction = 0.02) patterns with lung cancer risk. Conclusions and Relevance: Our study provides the first evidence that the “Tex-Mex” dietary pattern is associated with reduced lung cancer risk. Also, the “fruits and vegetables” and “American/Western” patterns affects lung cancer risk, and the effects are further modified by host genetic background. Lung function, COPD and Alternative Healthy Eating Index in US adults

Abstract:Background: There is a large burden of COPD in the US. The purpose of this study was to investigate the association between diet quality with lung function, spirometric restriction and spirometrically defined COPD in a nationally representative sample of US adults. Methods: Adults (19-70 years of age) from the National Health and Nutrition Examination Survey 2007-2012 cycles were included (n=10 428). Diet quality was determined using the Alternative Healthy Eating Index (AHEI-2010). Pre-bronchodilator measurements of forced expiratory volume in 1 s (FEV1), forced vital capacity (FVC) and the FEV1/FVC were described. Calibrated lower limit of normal (LLN) estimates were derived to determine prevalence of spirometric restriction (FVC<LLN) and COPD (FEV1/FVC ratio<LLN). Population-weighted linear and logistic regression models were used to investigate the association of AHEI-2010 and respiratory outcomes. Results: The mean±SD AHEI was 45.3±12.2, equivalent to meeting 41% of the daily recommendations for optimal diet quality. Those in the highest quartile of AHEI had better FEV1 (adjusted (a)β 47.92, 95% CI 2.27-93.57) and FVC (aβ 80.23, 95% CI 34.03-126.42; p-value interaction (*) of AHEI and smoking >0.05) compared to those in quartile 1. Higher AHEI was also associated with lower odds of spirometric restriction (OR 0.23, 95% CI 0.08-0.67; p-value AHEI*ethnicity >0.05). Conclusions: Diet quality was independently associated with better FEV1 and FVC and with lower odds of spirometric restriction. These findings highlight the need for research to further elucidate the possible beneficial role of diet in the preservation of lung function.

Abstract:Introduction: The incidence of lung cancer in women living in China is among the highest in the world but it does not appear that tobacco smoking is a major risk factor for lung cancer. As tobacco smoking is highly prevalent in Chinese men, exposure to environmental tobacco smoke (ETS) may play an important role in the development of lung cancer in Chinese women who never smoked. We conducted the present investigation because previous studies did not account for dietary habits or indoor air pollution from Chinese-style cooking and they did not assess the effect of occupational exposure to ETS.

Abstract:China is faced with heavy burdens caused by lung cancer, which has climbed to the top of both cancer incidence and mortality spectrums. The age-standardized rates of incidence and mortality have shown a trend of gradual up-trends in the last decades, while the crude rates rise much quickly due to the aging of population. Although the improvement in health care has contributed to better survival of lung cancer, its prognosis is still challenging. Apart from the common risk factors such as tobacco use, air pollution, and occupational hazards, some specific factors like Chinese-style cooking also have posed great threats to human health. In light of such national conditions, specific interventions should be conducted to curb the burden of lung cancer including smoking cessation, improvement of air quality, early detection and effective treatment of lung cancer.

Abstract:To date, the relationship between obesity and the incidence of lung cancer remains unclear and inconclusive. Thus, we conducted a meta-analysis of published studies to provide a quantitative evaluation of this association. Relevant studies were identified through PubMed and EMBASE databases from 1966 to December 2011, as well as through the reference lists of retrieved articles. A total of 31 articles were included in this meta-analysis. Overall, excess body weight (body mass index, BMI >= 25 kg/m(2)) was inversely associated with lung cancer incidence (relative risk, RR = 0.79; 95% confidence interval, CI: 0.73-0.85) compared with normal weight (BMI = 18.5-24.9 kg/m(2)). The association did not change with stratification by sex, study population, study design, and BMI measurement method. However, when stratified by smoking status, the inverse association between excess body weight and lung cancer incidence in current (RR = 0.63, 95% CI: 0.57-0.70) and former (RR = 0.73, 95% CI: 0.58-0.91) smokers was strengthened. In non-smokers, the association was also statistically significant (RR = 0.83, 95% CI: 0.70-0.98), although the link was weakened to some extent. The stratified analyses also showed that excess body weight was inversely associated with squamous cell carcinoma (RR = 0.68, 95% CI: 0.58-0.80) and adenocarcinoma (RR = 0.79, 95% CI: 0.65-0.96). No statistically significant link was found between excess body weight and small cell carcinoma (RR = 0.99, 95% CI: 0.66-1.48). The results of this meta-analysis indicate that overweight and obesity are protective factors against lung cancer, especially in current and former smokers.
Adherence to the low-fat diet pattern reduces the risk of lung cancer in American adults aged 55 years and above: a prospective cohort study

Abstract:Objectives: There is little evidence on the association between low-fat dietary patterns and lung cancer risk among middle-aged and older adults. To fill this gap, we comprehensively investigated the association of adherence to a low-fat diet (LFD) and intake of different fat components including saturated, monounsaturated, and polyunsaturated fatty acids with incidence of lung cancer and its subtypes [non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC)] among adults aged 55 years and older. Design: A prospective cohort study with a mean follow-up time of 8.8 years. Setting and participants: This study used data from the Prostate, Lung, Colorectal, and Ovarian (PLCO) Cancer Screening Trial. The study population included 98,459 PLCO participants age 55 and over at baseline who completed food frequency questionnaires providing detailed dietary information and had no history of cancer. Methods: Dietary intake was assessed using a validated food frequency questionnaire at baseline. A LFD score was calculated based on fat, protein, and carbohydrate intake as a percentage of total calories. Cox proportional hazards regression models were used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) for the association between LFD score and intake of fat components (in quartiles) and incident lung cancer and its subtypes over follow-up. Restricted cubic spline analyses were conducted to examine possible nonlinear relationships. Subgroup analyses were performed to evaluate potential effect modifiers, and several sensitivity analyses were conducted to assess the stability of the findings. Results: During a follow-up of 869,807.9 person-years, 1,642 cases of lung cancer were observed, consisting of 1,408 (85.75%) cases of NSCLC and 234 (14.25%) cases of SCLC. The highest versus the lowest quartiles of the LFD score were found to be associated with a reduced risk of lung cancer (HR, 0.76; 95% CI, 0.66-0.89), NSCLC (HR, 0.79; 95% CI, 0.67-0.93), and SCLC (HR, 0.59; 95% CI, 0.38-0.92). The restricted cubic spline plots demonstrated a linear dose-response relationship between the LFD score and the risk of lung cancer as well as its subtypes. This risk reduction association for overall lung cancer was more pronounced in smokers (HR, 0.71; 95% CI, 0.60-0.84; P for interaction = 0.003). For fat components, high consumption of saturated fatty acids was associated with an increased lung cancer risk (HR, 1.35; 95% CI, 1.10-1.66), especially for SCLC (HR, 2.05; 95% CI, 1.20-3.53). No significant association was found between consumption of monounsaturated or polyunsaturated fatty acids and incident lung cancer and its subtypes. Conclusions: Our findings suggest that adherence to LFD may reduce the lung cancer risk, particularly in smokers; while high saturated fatty acids consumption may increase lung cancer risk, especially for SCLC, among middle-aged and older adults in the US population.

Abstract:Reducing lung cancer risk by modifying diet is highly desirable. We investigated whether different U.S. dietary patterns were associated with lung cancer risk. Dietary patterns were derived using exploratory factor analysis for 2139 non-small cell lung cancer (NSCLC) cases and 2163 frequency-matched controls. Logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals (95% CIs). Highest adherence (highest vs . lowest quintile) to the “Tex-Mex”, “fruits and vegetables” and “American/Western” patterns was associated with a 55% reduced (OR = 0.45; 95% CI = 0.37–0.56; P < 0.001), 32% reduced (OR = 0.68; 95% CI = 0.55–0.85; P = 0.001) and 45% increased (OR = 1.45; 95% CI = 1.18–1.78; P < 0.001) risk of lung cancer, respectively. The effects were stronger for squamous cell carcinoma and ever smokers for the “fruits and vegetables” pattern and stronger for other non-small cell lung cancer and never smokers for the “American/Western” pattern. Among six genome-wide association (GWA) studies-identified lung cancer susceptibility loci assessed, a variant (rs2808630) of the C-reactive protein gene modified the associations for the “fruits and vegetables” ( P for interaction = 0.03) and “American/Western” ( P for interaction = 0.02) patterns. Our study first showed that the “Tex-Mex” dietary pattern was associated with a reduced lung cancer risk. Also, the “fruits and vegetables” and “American/Western” patterns affected lung cancer risk and the effects were further modified by host genetic background.

Abstract:Background In rural Xuanwei County, China, the high incidence of lung cancer is attributable largely to burning smoky coal indoors for heating and cooking without adequate ventilation. Such burning generates very high levels of indoor air pollutants, including carcinogenic polycyclic aromatic hydrocarbons, which could contaminate foodstuffs in the home. Thus, residents could be exposed to carcinogenic coal emissions not only via inhalation but also via ingestion of these foodstuffs. Methods A population-based case–control study of 498 lung cancer patients and 498 controls was conducted from 1985 through 1990 in Xuanwei. The interviewer-administered study questionnaire queried the frequency of food items commonly consumed in this region. Overall and sex-specific multiple logistic regression models were constructed to estimate Odds ratios (OR) and 95% confidence intervals (CI) for consumption of these foods. Results Intake of rice, green vegetables, mushrooms and fresh meat was associated with an increased risk of lung cancer. In contrast, intake of corn, buckwheat, radishes, peppers, melons, pickled vegetables, and salt-preserved meats was associated with reduced risk. The detrimental effect of ingesting green vegetables (OR, 2.39; 95% CI, 1.28–4.48) is consistent with previous reports. Conclusions These findings suggest that in Xuanwei, food contamination by environmental polycyclic aromatic hydrocarbons may be an important risk factor for lung cancer, and that differential contamination of foods by polycyclic aromatic hydrocarbons possibly explained the different associations with lung cancer risk.
Indoor Air Pollution and Lung Cancer in China

Abstract:Although indoor air pollution has been suggested as a risk factor for lung cancer, it has not been fully studied in Chinese population. This study aims to investigate associations between cooking, solid fuel usage, environmental tobacco smoking exposure, housing characteristics and lung cancer in an urban Chinese population, with a focus on nonsmoking women. We conducted a case-control study in Taiyuan, China, consisting of 399 lung cancer cases and 466 healthy controls, of which 164 cases and 218 controls were female non-smokers. Indoor PM concentrations, including PM1, PM2.5, PM7, PM10 and TSP, were measured using a particle mass monitor. Unconditional logistic regression models were used to calculate odds ratios (ORs) and 95% confidence intervals after adjusting for age, education level, annual personal income and packyears of smoking (when the analyses included smokers). Among non-smoking women, lung cancer was strongly associated with multiple sources of indoor air pollution, including heavy exposure to ETS at work (aOR=3.65), high frequency of cooking (aOR=3.30), and solid fuel usage for cooking (aOR=4.97) and heating (aORcoal stove=2.00, aORheatable brick bed =8.1). Housing characteristics related to poor ventilation, including single-story houses, less window area, no separate kitchen, no kitchen ventilator and rarely having windows open, are associated with lung cancer, with aORs varying between 1.8-3.0. Indoor mean PM levels in the study population are much higher than WHO guidelines for ambient PM. Mean PM2.5 concentration was 74ug/m , and PM10 was 232ug/m . PM levels in winter are strongly correlated with solid fuel usage for cooking and heating, as well as use of ventilators. PM1 levels in cases’ houses are more than 3 times higher than that in controls’ houses. Our results suggest that exposure to indoor air pollution may play an important role in the development of lung cancer among Chinese female non-smokers. Both reducing pollutant emission and improving ventilation may be preventive measures in control of PM levels inside the house, in order to reduce the development of lung cancer.
