Abstract:Natural joints have excellent low-friction properties that positively inspire the frictional wear properties of artificial joints. Replacing damaged biological joints with artificial ones can restore bio-function and reduce the pain of arthritic patients. Improvements in the frictional wear properties of artificial joints will effectively increase the life of the prosthesis, especially for young arthritic patients. This paper reviews the lubrication mechanism of natural joints before systematically introducing the development of lubrication and wear simulation techniques for artificial joints, offering references and insights for further studies of artificial joints and the optimization of tribological properties of commercial artificial joint prostheses.

Abstract:A healthy natural synovial joint is very important for painless active movement of the natural musculoskeletal system. The right functioning of natural synovial joints ensures well lubricated contact surfaces with a very low friction coefficient and wear of cartilage tissue. The present paper deals with a new method for visualization of lubricating film with simultaneous measurements of the friction coefficient. This can contribute to better understanding of lubricating film formation in a natural synovial joint. A newly developed device, a reciprocating tribometer, is used to allow for simultaneous measurement of friction forces with contact visualization by fluorescence microscopy. The software allowing for snaps processing and subsequent evaluation of fluorescence records is developed. The evaluation software and the follow-up evaluation procedure are also described. The experiments with cartilage samples and model synovial fluid are carried out, and the new software is applied to provide their evaluation. The primary results explaining a connection between lubrication and friction are presented. The results show a more significant impact of albumin proteins on the lubrication process, whereas its clusters create a more stable lubrication layer. A decreasing trend of protein cluster count, which corresponds to a decrease in the thickness of the lubrication film, is found in all experiments. The results highlight a deeper connection between the cartilage friction and the lubrication film formation, which allows for better understanding of the cartilage lubrication mechanism.

A medical economist and author living in Denver, Kleinke advocates a true partnership between hospitals and physicians--a marriage with both parties contributing equally to the relationship. He believes that "physicians and people who are running the administrative infrastructures of hospitals and other facilities need to recognize that they are equal partners in a death struggle against the insurers for ultimate control of the premium and the consumer." Though physicians are sure to balk at the suggestion that they become "captive" to the hospital, Kleinke explains that, "captivity is a necessary condition before they can work functionally together and take on managed care and contract directly with consumers, employers, and the government." Abstract:Healthy articular cartilage, covering the ends of bones in major joints such as hips and knees, presents the most efficiently-lubricated surface known in nature, with friction coefficients as low as 0.001 up to physiologically high pressures. Such low friction is indeed essential for its well being. It minimizes wear-and-tear and hence the cartilage degradation associated with osteoarthritis, the most common joint disease, and, by reducing shear stress on the mechanotransductive, cartilage-embedded chondrocytes (the only cell type in the cartilage), it regulates their function to maintain homeostasis. Understanding the origins of such low friction of the articular cartilage, therefore, is of major importance in order to alleviate disease symptoms, and slow or even reverse its breakdown. This progress report considers the relation between frictional behavior and the cellular mechanical environment in the cartilage, then reviews the mechanism of lubrication in the joints, in particular focusing on boundary lubrication. Following recent advances based on hydration lubrication, a proposed synergy between different molecular components of the synovial joints, acting together in enabling the low friction, has been proposed. Additionally, recent development of natural and of bio-inspired lubricants is reviewed.

Abstract:In this study, the complementary lubricating function of the superficial area of the articular cartilage and synovial fluid (SF) constituents was examined. The cartilage specimens underwent two different degenerative treatments: gentle washing with detergent to remove lipids and proteins absorbed onto the cartilage surface and incubation in a NaCl solution to remove lubricin from the surface. Sliding experiments with a glass probe and cartilage specimens were conducted at various speeds and low contact loads using lubricants containing SF constituents, such as phospholipids, proteins, and hyaluronic acid (HA). The treated cartilage surface and protein adsorption were observed using a fluorescence microscope and water immersion objectives to explore the underlying mechanisms of the difference in friction. The results showed that fresh SF exhibited low friction even after degenerative treatment. HA and phospholipids had no boundary lubrication effect, whereas the lubricant containing albumin and γ-globulin maintained a consistently low coefficient of friction, even after degenerative treatment. The significance of the interaction between albumin and γ-globulin should be emphasized.

Abstract:The outstanding lubrication of articular cartilage in the major synovial joints such as hips and knees, essential for the joint well-being, has been attributed to boundary layers of lipids at the outer cartilage surfaces, which have very low friction mediated by the hydration lubrication mechanism at their highly hydrated exposed headgroups. However, the role of spontaneously present lipid splays, lipids with an acyl tail in each of the opposing bilayers, in modulating the frictional force between lipid bilayers has not, to date, been considered. In this study, we perform all-atom molecular dynamics simulations to quantitatively assess the significance of splayed molecules within the framework of lubricating lipid bilayers. We demonstrate that, although transient, splayed molecules significantly increase the inter-membrane friction until their retraction back into the lamellar phase, with this effect more steadily occurring at lower sliding velocities that are comparable to the physiological velocities of sliding articular cartilage.

Multi-lipid synergy in synovial lubrication: natural redundancy vs. natural selection

Abstract:The very low sliding friction of articular cartilage in the major synovial joints such as hips and knees is crucial for their well-being, and has been attributed to lubrication by phospholipid boundary layers. While single-component lipid layers have demonstrated efficient lubricity in model studies, in living joints there is a large number of different lipids, raising the question of whether this is natural redundancy, or whether this multiplicity confers any benefits. Here we examine lubrication by progressively more complex mixtures of lipids representative of those in joints, using a surface forces balance at physiologically-relevant salt concentrations and pressures. We find that different lipid combinations differ very significantly in their lubricating ability, as manifested by their robustness to hemifusion under physiological loads, pointing to a clear lubrication synergy arising from multiple lipid types in the lubricating layers. Insight into the origins of this synergy is provided by molecular dynamics (MD) simulations of the different lipid mixtures used in the experiments, which directly reveal how hemifusion - associated with greatly increased friction - depends on the detailed lipid composition. Our results provide insight into the role of lipid type proliferation in healthy synovial joints, and point to new treatment modalities for osteoarthritis.

Abstract:Despite continuous progress toward tissue engineering of functional articular cartilage, significant challenges still remain. Advances in morphogens, stem cells, and scaffolds have resulted in enhancement of the bulk mechanical properties of engineered constructs, but little attention has been paid to the surface mechanical properties. In the near future, engineered tissues will be able to withstand and support the physiological compressive and tensile forces in weight-bearing synovial joints such as the knee. However, there is an increasing realization that these tissue-engineered cartilage constructs will fail without the optimal frictional and wear properties present in native articular cartilage. These characteristics are critical to smooth, pain-free joint articulation and a long-lasting, durable cartilage surface. To achieve optimal tribological properties, engineered cartilage therapies will need to incorporate approaches and methods for functional lubrication. Steady progress in cartilage lubrication in native tissues has pushed the pendulum and warranted a shift in the articular cartilage tissue-engineering paradigm. Engineered tissues should be designed and developed to possess both tribological and mechanical properties mirroring natural cartilage. In this article, an overview of the biology and engineering of articular cartilage structure and cartilage lubrication will be presented. Salient progress in lubrication treatments such as tribosupplementation, pharmacological, and cell-based therapies will be covered. Finally, frictional assays such as the pin-on-disk tribometer will be addressed. Knowledge related to the elements of cartilage lubrication has progressed and, thus, an opportune moment is provided to leverage these advances at a critical step in the development of mechanically and tribologically robust, biomimetic tissue-engineered cartilage. This article is intended to serve as the first stepping stone toward future studies in functional tissue engineering of articular cartilage that begins to explore and incorporate methods of lubrication.

Abstract:The boundary lubrication mechanism at the articulating surface of natural synovial joints has been the subject of much discussion in tribology. In this study, to elucidate the lubricating function of the superficial area of articular cartilage and synovial fluid (SF), cartilage specimens were processed with four different treatments: gentle and severe washing with detergent, incubation in NaCl solution, and trypsin digestion to selectively remove certain constituents from the cartilage surface. Subsequently, the frictional characteristics were examined in phosphate-buffered saline (PBS) and SF against glass. Angularly reciprocating sliding tests with a spherical glass probe and square articular cartilage specimens were performed at low contact loads in the mN range to extract the frictional behavior in the superficial area of the cartilage specimens. Meanwhile, the cartilage surface was observed to confirm the effects of treatments on the morphology of the cartilage surface using a fluorescence microscope and water-immersion methods. The coefficient of friction (COF) of the prepared cartilage specimens was varied from 0.05 to over 0.3 in PBS. However, a certain group of cartilage specimens exhibited a low COF of less than 0.1 with limited variation. For the low COF group of specimens, all four treatments increased the COF in PBS to different extents, and fluorescence microscopy revealed that the integrity of the cartilage surface was deteriorated by treatments. This means that the intact cartilage surface had lubricating constituents to maintain low friction, and the removal of such constituents resulted in the loss of the intrinsic boundary lubricity of the cartilage surface. The variation in the COF of the cartilage specimens was suppressed in SF because it had a clear boundary lubrication effect on the cartilage surface. The lubricating effect of SF could be confirmed even after degenerative treatment.

Abstract:Semi‐solid lubricant in the insect legs joints is represented by cylindrical fragments with a diameter of ≈1 μm. The results of numerical simulation show that the fragments tend to adhesion and the formation of clumps of different shapes and sizes. Such a system allows maintaining clearance between surfaces while reducing friction and elimination of contaminant particles. Semi‐solid lubricant in the leg joints of insects is represented by tiny cylindrical fragments. The dynamics of lubricant behavior between contact surfaces at the joint scale are investigated using numerical simulation. Results of modeling show that lubricant fragments tend to stick together and form clots of varying shapes and sizes under the influence of shear motion. Formation of such clots, accompanied by constant loss and reproduction of the lubricant, allows maintaining of fluctuating clearance between contact surfaces and helps to reduce friction. Foreign contaminant particles penetrating an open insect joint are removed by the lubricant, to which they stick, reducing abrasive wear.

Abstract:The synovial joint of the knee can be affected by diseases, such as arthritis or osteoarthritis, being a knee replacement the selected clinical solution in irreversible cases. This work is aimed at investigating lubricated contact in healthy and pathological knees, with the aim of achieving knowledge that can be used to delay knee replacement. The equivalent geometric model of a rigid ellipsoid on a deformable plane is adopted, where the synovial fluid is considered to be Newtonian and the articular cartilage is a linear elastic material. Studies were carried out varying the modulus of elasticity E of the cartilage, its thickness e and the viscosity of the synovial fluid , adopting values corresponding to healthy and osteoarthritic natural joints, in order to evaluate their influence on the functioning of the joint considering squeeze film lubrication. Compared to what was obtained for a healthy joint, the decrease in the elastic modulus E would not have a significant influence on the minimum lubricant film thickness, while the decrease in cartilage thickness would improve said thickness. The decrease in viscosity decreases the minimum thickness of the lubricating film by 65 %, so viscosupplementation would increase the viscosity, which would also increase said thickness and prevent wear of the articular surfaces. A decrease in cartilage thickness or in cartilage module would produce a decrease of the cartilage tensions. On the other hand, a decrease of viscosity produces an increase of these tensions. In a pathological joint (where the fluid viscosity and the cartilage thickness and elastic module are diminished), and considering a load of 800 N, the minimum film height, the maximum pressure and the maximum Von Mises tension obtained are 66 %, 22 % and 28 % respectively lower than those observed for a healthy articulation.

Abstract:Osteoarthritis (OA) is a multi-factor disorder of sinovial joints, which characterized by escalated degeneration and loss of articular cartilage. Treatment of OA is a critical unmet need in medicine for regeneration of damaged articular cartilage in elderly. On the other hand, lubricin, a glycoprotein specifically synthesized by chondrocytes located at the surface of articular cartilage, has been shown to provide boundary lubrication of congruent articular surfaces under conditions of high contact pressure and near zero sliding speed. Lubrication of these surfaces is critical to normal joint function, while different gene expressions of lubricin had been found in the synovium of rheumatoid arthritis (RA) and OA. Moreover, mutations or lacking of lubricin gene have been shown to link to the joint disease such as camptodactyly-arthropathy-coxa vara-pericarditis syndrome (CACP), synovial hyperplasia and failure of joint function, suggesting an important role of lubricin in the pathogenesis of these joint disease. Recent studies demonstrate that administration with recombinant lubricin in the joint cavity would be effective in the prevention of cartilage degeneration in animal OA models. Therefore, a treatment with lubricin which would protect cartilage in vivo would be desirable. This article reviews recent findings with regard to the possible role of lubricin in the progression of OA, and further discusses lubricin as a novel potential biotherapeutic approaches for the treatment of OA.

Abstract:The aim of the conducted research was the evaluation of the topography and the structure of the superficial layer of meniscus and articular cartilage. These are surfaces that optimise the friction and lubrication process in the knee joint. The animal samples of the menisci and the articular cartilage were examined. The research was performed using scanning electron microscopes and an atomic force microscope. The structure of the surface of meniscus and articular cartilage is very regular. The collagenous fibres, which are embedded in the ground substance, are parallel to the surface. The undulation of the surface was observed. In the area of the anterior horn on tibia side of both menisci as well as in the anterior area of tibial plateau, the concavity and convexity pattern is evident. The observed cavities enable the accumulation of the synovial fluid. The synovial fluid plays the role of the lubricant in the knee joint, and its presence is highly desired during the load transmission.

Abstract:Why is friction in healthy hips and knees so low? Hydration lubrication, according to which hydration shells surrounding charges act as lubricating elements in boundary layers (including those coating cartilage in joints), has been invoked to account for the extremely low sliding friction between surfaces in aqueous media, but not well understood. Here we report the direct determination of energy dissipation within such sheared hydration shells. By trapping hydrated ions in a 0.4-1 nm gap between atomically smooth charged surfaces as they slide past each other, we are able to separate the dissipation modes of the friction and, in particular, identify the viscous losses in the subnanometre hydration shells. Our results shed light on the origins of hydration lubrication, with potential implications both for aqueous boundary lubricants and for biolubrication.

Abstract:A charged polymer brush grafted surface, inspired by natural articular cartilage, achieves an excellent lubricating performance with a low coefficient of friction due to the mechanism of hydration lubrication. The surface charged brush forms a hydration shell which can undergo large normal load without deformation. Additionally, the rapid exchange with the water molecules nearby results in a fluidlike manner of the hydrated shells when being sheared and correspondingly the significantly reduced friction force at the sliding interface. Effect of tibial plateau fracture on lubrication function and composition of synovial fluid

Abstract:Background: Intra-articular fractures may hasten posttraumatic arthritis in patients who are typically too active and too young for joint replacement. Current orthopaedic treatment principles, including recreating anatomic alignment and establishing articular congruity, have not eliminated posttraumatic arthritis. Additional biomechanical and biological factors may contribute to the development of arthritis. The objective of the present study was to evaluate human synovial fluid for friction-lowering function and the concentrations of putative lubricant molecules following tibial plateau fractures. Methods: Synovial fluid specimens were obtained from the knees of eight patients (twenty-five to fifty-seven years old) with a tibial plateau fracture, with five specimens from the injured knee as plateau fracture synovial fluid and six specimens from the contralateral knee as control synovial fluid. Each specimen was centrifuged to obtain a fluid sample, separated from a cell pellet, for further analysis. For each fluid sample, the start-up (static) and steady-state (kinetic) friction coefficients in the boundary mode of lubrication were determined from a cartilage-on-cartilage biomechanical test of friction. Also, concentrations of the putative lubricants, hyaluronan and proteoglycan-4, as well as total protein, were determined for fluid samples. Results: The group of experimental samples were obtained at a mean (and standard deviation) of 11 ± 9 days after injury from patients with a mean age of 45 ± 13 years. Start-up and kinetic friction coefficients demonstrated similar trends and dependencies. The kinetic friction coefficients for human plateau fracture synovial fluid were approximately 100% higher than those for control human synovial fluid. Hyaluronan concentrations were ninefold lower for plateau fracture synovial fluid compared with the control synovial fluid, whereas proteoglycan-4 concentrations were more than twofold higher in plateau fracture synovial fluid compared with the control synovial fluid. Univariate and multivariate regression analysis indicated that kinetic friction coefficient increased as hyaluronan concentration decreased. Conclusions: Knees afflicted with a tibial plateau fracture have synovial fluid with decreased lubrication properties in association with a decreased concentration of hyaluronan.

Abstract:The remarkable lubrication properties of normal articular cartilage play an essential role in daily life, providing almost frictionless movements of joints. Alterations of cartilage surface or degradation of biomacromolecules within synovial fluid increase the wear and tear of the cartilage and hence determining the onset of the most common joint disease, osteoarthritis (OA). The irreversible and progressive degradation of articular cartilage is the hallmark of OA. Considering the absence of effective options to treat OA, the mechanosensitivity of chondrocytes has captured attention. As the only embedded cells in cartilage, the metabolism of chondrocytes is essential in maintaining homeostasis of cartilage, which triggers motivations to understand what is behind the low friction of cartilage and develop biolubrication-based strategies to postpone or even possibly heal OA. This review firstly focuses on the mechanism of cartilage lubrication, particularly on boundary lubrication. Then the mechanotransduction (especially shear stress) of chondrocytes is discussed. The following summarizes the recent development of cartilage-inspired biolubricants to highlight the correlation between cartilage lubrication and OA. One might expect that the restoration of cartilage lubrication at the early stage of OA could potentially promote the regeneration of cartilage and reverse its pathology to cure OA.

Interactions Between Bilayers of Phospholipids Mixture Extracted from Human Osteoarthritic Synovial Fluid

Abstract:Duncan Dowson, whom this issue commemorates, was a world leader in the field of biotribology, with prolific contributions both in fluid-based and boundary lubrication of biological tissues, in particular articular cartilage, a central issue in biotribology due to its importance for joint homeostasis. Here we explore further the issue of cartilage boundary lubrication, which has been attributed to phospholipid (PL)-exposing layers at the cartilage surface in part. A surface force balance (SFB) with unique sensitivity is used to investigate the normal and frictional interactions of the boundary layers formed by PLs extracted from osteoarthritic (OA) human synovial fluid (hSF). Our results reveal that vesicles of the OA-hSF lipids rupture spontaneously to form bilayers on the mica substrate (which, like the in-vivo articular cartilage surface in synovial joints, is negatively-charged) which then undergo hemifusion at quite low pressures in the SFB, attributed to the large heterogeneity of the hSF lipids. Nanometric friction measurements reveal friction coefficients mu = ca. 0.03 across the hemi-fused bilayer of these lipids, indicating residual hydration lubrication at the lipid-headgroup vs. substrate interface. Addition of calcium ions causes an increase in friction to mu = ca. 0.2, attributed either to calcium-bridging attraction of lipid headgroups to the negatively-charged substrate, or a shift of the slip plane to the more dissipative hydrophobic-tail vs. hydrophobic-tail interface. Our results suggest that the heterogeneity and composition of the OA-hSF lipids may be associated with higher friction at the cartilage boundary layers, and thus a connection with greater wear and degradation, due to hemifusion of the exposed lipid bilayers.
