Abstract:STRESZCZENIE Cel pracy. Zbadanie i ocena skuteczności klinicznej i bezpieczenstwa przeskornego plastra z klonidyną w leczeniu zespolu Tourette’a (TS). Material i metody. Grupa 130 dzieci w wieku od 2 do 15 lat z zespolem Tourette’a zostala zrandomizowana do dwoch grup: 71 dzieci z badanej grupy bylo leczonych plastrem przezskornym z klonidyną, zaś 59 z grupy kontrolnej – haloperidolem. Po czterech tygodniach leczenia skutecznośc kliniczna zostala oceniona za pomocą skali Yale Globar Tic Severity Scale (YGTSS). Skutecznośc krotkoterminowa, objawy uboczne leczenia zostaly ocenione pod koniec leczenia, zaś skutecznośc dlugoterminową, jak rowniez czestośc nawrotow oceniono po pol roku od zakonczenia leczenia. Wyniki. Punktacja YGTSS w obu grupach obnizyla sie po czterech tygodniach leczenia, ale grupa chorych leczonych klonidyną wykazala wiekszą redukcje tikow w ogolnej punktacji tikowej (61,7 ± 7,3%), niz grupa leczonych haloperidolem (40,8 ±6,5%; P<0,05). Średni wspolczynnik skuteczności przezskornego plastra z klonidyną wynosil 81,7% (skutecznośc u 58 chorych), podczas gdy w przypadku doustnego haloperidolu skutecznośc wynosila 66,1% (skutecznośc u 39 chorych) (p<0,05). Nie obserwowano powaznych objawow ubocznych w zadnej z badanych grup. Niewielkie objawy uboczne pod postacią spadku ciśnienia krwi i zawrotow glowy obserwowano u jednego pacjenta leczonego przezskornie klonidyną. W grupie leczonych haloperidolem obserwowano lagodną miotonie, nudności lub sennośc u sześciu chorych. To examine and evaluate the clinical efficacy and safety of clonidine transdermal patch in treating Tourette syndrome. (TS). Material and Methods. A total of 130 children aged from 2 to 15 years with TS were randomized into two groups: 71 patients in the treated group were treated with clonidine transdermal patch and 59 in the control group were treated with haloperidol. After 4 weeks of treatment, the clinical efficacy was assessed by the Yale Global Tic Severity Scale(YGTSS). The short-term efficacy and adverse reaction of treatment were assessed at the end of treatment, the longterm efficacy as well as the recurrent rate were evaluated half a year after the end of treatment. Results. The YGTSS score in both groups decreased after 4 weeks of treatment, but the clonidine transdermal patch group showed a higher reduction in the overall tic symptom scores (61.7 ± 7.3%) than that in the haloperidol group. (40.8 ±6.5%; P<0.05). The overall effective rate in Clonidine transdermal patch treatment was 81.7% (effective in 58 patients) and 66.1% to oral haloperidol (effective in 39 patients) (p<0.05). There were no severe adverse events in both groups, but mild side effects (decreased blood pressure and dizziness) were observed in 1 patient in the clonidine transdermal patch group. Mild hypermyotonia, drowsiness or lassitude as side effects occurred in 6 patients in the haloperidol group. Conclusion. Clonidine transdermal patch treatment in TS Chinese children was more effective with lower incidence of adverse reactions and lower recurrent rate as compared with haloperidol group. Clonidine represents a new therapeutic option for the treatment of tic disorder.
