Abstract:Objective: This systematic review was conducted to evaluate the efficacy and safety of continuous nursing care for the recovery of joint function in older adults with total hip or knee arthroplasty. Methods: Randomized controlled trials and cohort studies of continuous nursing in older patients after joint replacement were searched from the database of Cochrane Library, Web of Science, PubMed, and Embase from their establishment to October 25, 2023. After literature screening, two researchers completed data extraction, and the risk of bias was assessed using the Cochrane risk-of-bias tool. The risk analysis included in cohort studies was based on the Newcastle-Ottawa Scale (NOS). Results: The study included a total of 15 articles, comprising 34,186 knee and hip replacement patients. In this review, the effects of continuous nursing on the recovery of joint function of knee replacement and hip replacement in older adults were classified and discussed. Continuous nursing interventions targeted for total hip replacement could greatly increase the range of joint mobility, enhance muscle strength during hip movements like flexion, extension, and abduction, maintain joint stability, relieve pain, improve daily activities, and lower the risk of complications. For older patients with knee arthroplasty, continuous nursing programs could markedly improve knee motion range, joint flexion, joint stability, daily activities, and pain management. Despite the implementation of interventions, the incidence of complications caused by total knee replacement did not decrease. Out of all the studies reviewed, only one used a theoretical framework for interventions provided to patients during the postoperative period of hip arthroplasty. The overall quality of the included studies was very high. Conclusion: Continuous nursing can effectively improve the joint function of older patients after joint replacement. However, its effectiveness in terms of clinical outcomes, patient satisfaction, and medical cost of associated continuous nursing needs to be further clarified. In addition, continuous nursing has no significant advantage in the safety of postoperative complications and readmission rates in older adults after knee joint replacement. To enhance the efficacy and safety of continuous nursing effectively, it is crucial to refine the continuous nursing program in the future, thereby elevating the quality of nursing services.

Abstract:BACKGROUND Joint replacement is a common treatment for older patients with high incidences of hip joint diseases. However, postoperative recovery is slow and complications are common, which reduces surgical effectiveness. Therefore, patients require long-term, high-quality, and effective nursing interventions to promote rehabilitation. Continuity of care has been used successfully in other diseases; however, little research has been conducted on older patients who have undergone hip replacement. AIM To explore the clinical effect of continuous nursing on rehabilitation after discharge of older individuals who have undergone joint replacement. METHODS A retrospective analysis was performed on the clinical data of 113 elderly patients. Patients receiving routine nursing were included in the convention group (n = 60), and those receiving continuous nursing, according to various methods, were included in the continuation group (n = 53). Harris score, short form 36 (SF-36) score, complication rate, and readmission rate were compared between the convention and continuation groups. RESULTS After discharge, Harris and SF-36 scores of the continuation group were higher than those of the convention group. The Harris and SF-36 scores of the two groups showed an increasing trend with time, and there was an interaction effect between group and time (Harris score: F intergroup effect = 376.500, F time effect = 20.090, F interaction effect = 4.824; SF-36 score: F intergroup effect = 236.200, F time effect = 16.710, F interaction effect = 5.584; all P < 0.05). Furthermore, the total complication and readmission rates in the continuation group were lower (P < 0.05). CONCLUSION Continuous nursing could significantly improve hip function and quality of life in older patients after joint replacement and reduce the incidence of complications and readmission rates.

Abstract:Objective: To investigate the effect of continuous nursing on the functional recovery after arthroscopic reconstruction of anterior cruciate ligament. Methods: Randomly divided 82 patients who had Arthroscopic reconstruction of anterior cruciate ligament into two groups, with 44 in observation group and 38 cases in control group. The observation group were provided with continuous nursing care including health education, telephone follow-ups and home visits. Control group did not have any care. Compared two groups of patients after discharge for the following: compliance behavior of functional rehabilitation exercise and follow up visits, knee extension activities and lfexion degree of knee joint function rehabilitation exercise using lysholm score. Results: There are statistically signiifcant differences between the two groups in the compliance of rehabilitation exercises at 6 and 24 weeks (P<0.05). The lysholm score at 24 weeks of knee rehabilitation exercise in the observation group is better than the control group (P<0.001). Conclusion: The implementation of continuous nursing care for patients with arthroscopic anterior cruciate ligament reconstruction has a signiifcant role in promoting the recovery of knee joint function, improve their quality of life.
Internet-based outpatient telerehabilitation for patients following total knee arthroplasty: a randomized controlled trial

Abstract:Background: total knee arthroplasty is an effective means for relieving the symptoms associated with degenerative arthritis of the knee. Rehabilitation is a necessary adjunct to surgery and is important in regaining optimum function. Access to high-quality rehabilitation services is not always possible, especially for those who live in rural or remote areas. The aim of this study was to evaluate the equivalence of an Internet-based telerehabilitation program compared with conventional outpatient physical therapy for patients who have had a total knee arthroplasty. Methods: this investigation was a single-blinded, prospective, randomized, controlled noninferiority trial. Sixty-five participants were randomized to receive a six-week program of outpatient physical therapy either in the conventional manner or by means of an Internet-based telerehabilitation program. The primary outcome measure was the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) measured at baseline and six weeks by a blinded independent assessor. Secondary outcomes included the Patient-Specific Functional Scale, the timed up-and-go test, pain intensity, knee flexion and extension, quadriceps muscle strength, limb girth measurements, and an assessment of gait. Noninferiority was assessed through the comparison of group differences with a noninferiority margin and with linear mixed model statistics. Results: baseline characteristics between groups were similar, and all participants had significant improvement on all outcome measures with the intervention (p < 0.01 for all). After the six-week intervention, participants in the telerehabilitation group achieved outcomes comparable to those of the conventional rehabilitation group with regard to flexion and extension range of motion, muscle strength, limb girth, pain, timed up-and-go test, quality of life, and clinical gait and WOMAC scores. Better outcomes for the Patient-Specific Functional Scale and the stiffness subscale of the WOMAC were found in the telerehabilitation group (p < 0.05). The telerehabilitation intervention was well received by participants, who reported a high level of satisfaction with this novel technology. Conclusions: the outcomes achieved via telerehabilitation at six weeks following total knee arthroplasty were comparable with those after conventional rehabilitation. Effect of Internet-Based Rehabilitation Programs on Improvement of Pain and Physical Function in Patients with Knee Osteoarthritis: Systematic Review and Meta-analysis of Randomized Controlled Trials

Abstract:Background Osteoarthritis (OA) is a chronic, debilitating, and degenerative joint disease. However, it is difficult for patients with knee OA to access conventional rehabilitation when discharging from the hospital. Internet-based rehabilitation is one of the promising telemedicine strategies to provide a means combining monitoring, guidance, and treatment for patients with knee OA. Objective The aim of this study was to conduct a systematic review and meta-analysis for assessing the effect of internet-based rehabilitation programs on pain and physical function in patients with knee OA. Methods Keywords related to knee OA and internet-based rehabilitation were systematically searched in the Web of Science, MEDLINE, EMBASE, CENTRAL, Scopus, PEDro (Physiotherapy Evidence Database), CNKI, SinoMed, and WANFANG databases from January 2000 to April 2020. Only randomized controlled trials were included. The authors independently screened the literature. The main outcome measures were focused on pain and physical function. A meta-analysis was performed on the collected data. Review Manager (RevMan, version 5.3) was used for all analyses. Results The systematic review identified 6 randomized controlled trials, 4 of which were included in the meta-analysis, comprising a total of 791 patients with knee OA. The meta-analysis with the fixed-effects model showed that the internet-based rehabilitation programs could significantly alleviate the osteoarthritic pain for patients compared with conventional rehabilitation (standardized mean difference [SMD] –0.21, 95% CI −0.4 to –0.01, P=.04). No significant difference was found in the improvement of physical function in patients with knee OA compared with conventional rehabilitation within 2 to 12 months (SMD –0.08, 95% CI −0.27 to 0.12, P=.43). Conclusions This systematic review shows that internet-based rehabilitation programs could improve the pain but not physical function for patients with knee OA. However, there was a very small number of studies that could be included in the review and meta-analysis. Thus, further studies with large sample sizes are warranted to promote the effectiveness of internet-based rehabilitation and to develop its personalized design.

Enhanced walking function and quality of life in unicondylar knee replacement patients through continuous nursing intervention: A clinical study

Abstract:Background: The knee joint anatomical structure is susceptible to external forces, which can lead to functional break down, profoundly affecting the quality of life and daily functioning of individuals. Objective: To investigate the effects of continuous nursing intervention on walking function, quality of life, and treatment satisfaction in patients undergoing unicondylar knee replacement. Methods: This prospective study included 90 patients who underwent unicondylar knee arthroplasty due to unicondylar osteoarthritis. Participants were divided into two groups based on their nursing methods: the control group (n= 45) and the observation group (n= 45). Results: Three months post-intervention, the observation group demonstrated significant improvements in knee joint range of motion, Hospital for Special Surgery Knee Score score, and Lysholm knee joint score compared to the control group (P< 0.05). The observation group also had a shorter completion time for the Timed Up and Go test and significantly higher SF-36 scores (P< 0.05 for both). Additionally, Knee Injury and Osteoarthritis Outcome Score scores in the observation group were significantly higher after three months of intervention (P< 0.05). And the observation group reported higher satisfaction rates and lower dissatisfaction rates compared to the control group (P< 0.05). Conclusion: Early postoperative rehabilitation guidance, regular reviews, and rehabilitation exercise guidance result in better rehabilitation outcomes, enhanced knee joint function, improved walking ability, and overall quality of life for patients undergoing unicondylar knee replacement.
Effects of Internet Plus-Based Continuous Nursing on Hemodialysis Adherence in Patients with Chronic Renal Failure: A Retrospective Study

Abstract:Aims/Background Chronic renal failure (CRF) is the eventual outcome shared by various progressive renal diseases, posing a serious threat to the physical health of patients. CRF patients are required to undergo hemodialysis (HD), which imposes heavy psychological and mental burdens for most individuals. This study explores the effects of Internet Plus-based continuous nursing on the compliance of CRF patients with HD. Methods This study retrospectively analyzed the clinical data of 160 CRF patients undergoing HD in the Yantai Yuhuangding Hospital from March 2021 to April 2023, after excluding eight cases from an originally selected cohort of 168 cases. These patients were divided into two groups: 79 cases who received the routine nursing from March 2021 to March 2022 were categorized as the routine group, whereas 81 cases who were given Internet Plus-based continuous nursing from April 2022 to April 2023 were assigned into the observation group. The treatment adherence, self-management behaviors, quality of life and incidence of HD complications were compared in both groups. Results Both groups demonstrated no significant difference in the baseline information (p > 0.05). The scores of adherence in terms of HD attendance, medications, fluid restrictions and diet recommendations in the observation group were significantly higher than those in the routine group (p 0.05). After 6 months of follow-up, the observation group showed significantly higher scores of partnership, problem solving, self-management execution and emotional processing than the routine group (p 0.05). After 6 months of follow-up, the scores of KDTA and SF-36 in the observation group were significantly higher than those in the routine group (p < 0.001). The incidence of HD complications in the observation group (7.41%) was significantly lower than that in the routine group (21.52%) (p < 0.05). Conclusion Internet Plus-based continuous nursing can effectively improve treatment adherence, self-management behaviors as well as quality of life in patients, and reduce the incidence of HD complications.

Abstract:Introduction Osteoarthritis (OA) is one of the main causes of mobility impairment in the elderly worldwide. Therefore, total knee arthroplasty (TKA) is often performed and is one of the most successful surgery and has resulted in substantial quality-of-life gains for people with end-stage arthritis. There is still room for improvement in the standard treatment process in the preoperative, intraoperative and postoperative period of TKA. Telerehabilitation has the potential to become a positive alternative to face-to-face rehabilitation nowadays. But it remains unclear how well telemedicine interventions cover the entire surgical pathway (preoperation, intraoperation, postoperation). This study aims to explore the effectiveness of Joint Cloud (JC, an online management platform) compared with existing standard process in regulating functional recovery, pain management, muscle strength changes and other health-related outcomes in patients undergoing total knee arthroplasty preoperation, intraoperation and postoperation. Methods and analysis A randomised controlled trial was designed to compare the online management platform (JC) with standard process (SP) in patients undergoing TKA. A total of 186 TKA patients will be randomly assigned to the intervention (n=93) or control (n=93) group. Patients in the intervention group will receive access to the 'JC' mini-program. This mini-program provides popular science information (eg, information about OA and TKA), functional exercise information and communication channels. Patients evaluate their condition and functional level through standardised digital questionnaires. The control group of patients will not accept any functions of this mini-program. The primary outcome is knee functional recovery, and the secondary outcomes are pain management, isometric knee extensor muscle strength, patient satisfaction and cost-benefit analysis. Assessments will be performed 1 month and 3 days before surgery (T0) and 1 month and 3 months after surgery. Data analysis will be performed according to the intent-to-treat (ITT) principle. Repeated measures of linear mixed models and parametric and non-parametric testing will be used for statistical analysis. Ethics and dissemination The study was reviewed and approved by the Tianjin Hospital Medical Ethics Review Committee on 10 February 2023 (2022YLS155). Test data are considered highly sensitive but are available upon request. The findings will be disseminated in peer-reviewed publications. Trial registration number ChiCTR2300068486.

Abstract:BackgroundContinuous nursing care (CNC) is an extended service based on meeting the needs of discharged patients for post-discharge treatment and rehabilitation. This research aimed to investigate the effects of CNC on older patients with total hip arthroplasty and to offer a scientific basis for improving the prognosis.MethodsA total of 134 patients with total hip arthroplasty were randomly divided into the control group (n = 67) and the intervention group (n = 67). The control group was treated by conventional nursing care and the intervention group was treated by CNC. Harris hip score, Barthel index, the activities of daily living (ADL) scale, self-rating depression scale (SDS) and self-rating anxiety scale (SAS) in these two groups were evaluated. Demographic characteristics between groups were analyzed by unpaired t test. The observation indexes between groups were assessed by two-way ANOVA test followed by Tukey's multiple comparisons test.ResultsThe scores of Harris hip score, Barthel index, ADL, SDS and SAS in the intervention group after intervention and after follow-up were better than the intervention group before intervention (all p < 0.01). Meanwhile, the scores of Harris hip score, Barthel index, ADL, SDS and SAS in the intervention group were better than the control group both after intervention and after follow-up (all p < 0.01).ConclusionIn conclusion, CNC showed better efficacy than conventional nursing care in promoting hip joint function recovery, improving quality of life and alleviating anxiety and depression for older patients with total hip arthroplasty.

Application of Comprehensive Rehabilitation Nursing Based on 5E Concept in Patients after SuperPath Minimally Invasive Total Hip Replacement

Abstract:Objective? To explore the effect of rehabilitation nursing based on 5E concept in patients after SuperPath minimally invasive total hip replacemen(t THR). Methods? From February 2016 to February 2018, 194 patients who underwent THR through SuperPath approach in Huzhou Central Hospital of Zhejiang Province were selected as the study subjects. The patients were randomly divided into observation group (n=97) and control group by coin flipping method. The control group was given routine nursing care while the observation group was given the rehabilitation nursing based on 5E concept on the basis of that in the control group. The degree of pain, hip function score, incidence of complications and compliance of functional exercise were compared between the two groups. Results? The Harris score of the patients in the observation group was (65.25±5.29) at 1 month, (87.69±6.47) at 3 months and (93.85±8.66) at 6 months after operation, while that of the control group was (55.84±4.14), (77.48±5.89), (82.45±7.69). There were significant differences between the two groups (t=13.767, 11.511, 9.465;P＜ 0.05). The incidence of postoperative complications was 4.12% (4/97) in the observation group and 27.84% (27/97) in the control group (χ2=20.310, P＜ 0.05). The total compliance rate of functional exercise was 95.88% (93/97) in the observation group and 87.63% (85/97) in the control group, the difference was statistically significant (χ2=4.359,P＜ 0.05). The nursing satisfaction of patients in the observation group was 96.91% (94/97) and 81.44% (79/97) in the control group. The difference was statistically significant (χ2=12.015,P＜0.05). Conclusions? The application of comprehensive rehabilitation nursing based on 5E concept in SuperPath minimally invasive THR can effectively promote the recovery of hip function, reduce the occurrence of complications after operation, enhance the compliance of patients with functional exercise, and significantly improve the satisfaction of patients with nursing.
Effect of Internet + nursing intervention based on pre-rehabilitation strategy on quality of life and cognitive function of postoperative glioma patients

Abstract:Background Incorporating a pre-rehabilitation strategy, internet-based nursing intervention allows patients to engage with nurses through online platforms. Objective To explore the effect of Internet + nursing intervention based on pre-rehabilitation strategy on postoperative quality of life and cognitive function of patients with glioma. Methods 100 patients with gliomas treated in our hospital from Jan 2021 to Sep 2023 were selected. The patients were randomly divided into two groups ( n = 50). The control group was treated with routine nursing intervention. The observation group took Internet + nursing intervention based on pre-rehabilitation strategy on the basis of the control group. The general data, the score of self-management ability before and after intervention, the cognitive function before and after operation and the quality of life one month after discharge were compared between the two groups. The influencing factors of postoperative quality of life and cognitive function were analyzed by Logistics regression. Results There was no significant difference in the general data and the score of self-management ability between the two groups before intervention, but there was significant difference in the score of self-management ability after intervention. The results of binary Logistics regression analysis showed that the scores of self-management ability were independent factors affecting the outcome of patients with glioma. The scores of cognitive function and quality of life one month after discharge in the observation group were significantly better than those in the other ( P < 0.05). Conlclusions Internet + nursing intervention based on pre-rehabilitation strategy can improve the quality of life of glioma patients after operation and reduce the effect of operation on cognitive function of glioma patients.

Clinical Effectiveness of Home-Based Telerehabilitation Program for Geriatric Hip Fracture Following Total Hip Replacement

Abstract:Objective: To compare the effectiveness of a six-month home-based telerehabilitation based on the Internet-based rehabilitation management system coupled with conventional outpatient care in elderly patients with hip fractures following total hip replacement (THR). Methods: Elderly patients (aged over 65 years) with first hip fractures who underwent THR between March 2018 and September 2018 in Tianjin Hospital were enrolled in this study. Patients were divided into two groups: telerehabilitation group (n = 43) and telephone group (n = 42). A Internet-based telerehabilitation management system was established and applied on patients in the telerehabilitation group. For patients in the telephone group, the rehabilitation intervention was administered through conventional outpatient care (telephone along with outpatient follow-up). Data from the Harris hip scale (HHS), functional independence measure (FIM), self-rating anxiety scale (SAS), and postoperative complications at 1, 3, and 6 months after surgery were collected and compared between the two groups. Results: A total of 85 elderly patients completed the 6-month follow-up assessment. Results showed that the HHS score was significantly higher in the telerehabilitation group than in the telephone group at 1 month (66.35 ± 4.63 vs 63.48 ± 4.49), 3 months (76.33 ± 4.52 vs 71.81 ± 3.84), and 6 months (84.23 ± 3.13 vs 77.29 ± 4.95) after surgery (P < 0.001). The FIM score was significantly higher in the telerehabilitation group than in the telephone group at 1 month (89.00 ± 5.63 vs 73.35 ± 8.70), 3 months (100.16 ± 4.56 vs 92.81 ± 5.17), and 6 months (111.70 ± 3.13 vs 98.64 ± 5.12) after surgery (P < 0.001). The SAS score was significantly lower in the telerehabilitation group than in the telephone group at 1 month (42.40 ± 3.07 vs 46.21 ± 3.53), 3 months (36.77 ± 2.26 vs 40.24 ± 1.66), and 6 months (29.26 ± 1.63 vs 33.81 ± 2.62) after surgery (P < 0.001). The overall complication rate was significantly lower in the telerehabilitation group than in the telephone group (14% vs 40.5%) (P < 0.05). Conclusion: Internet-based rehabilitation management system can not only promote the physical rehabilitation of patients, but also play a positive role in psychological rehabilitation and the prevention of complications, which provides new ideas and methods for clinical rehabilitation.

Abstract:OBJECTIVE:To investigate the effects of early rehabilitation training after total knee arthroplasty surgery by continuous femoral nerve block (CFNB) with or without periarticular local infiltration analgesia (PLIA).METHODS:In this randomized, double-blind, controlled study, 100 patients under-going primary unilateral total knee arthroplasty in patients with knee osteoarthritis were enrolled. All the patients received CFNB for postoperative analgesia before combined spinal epidural anesthesia. They were randomly divided into 2 groups (n=50 each): CFNB group, CFNB combined with PLIA group (PLIA group). Group PLIA received periarticular local infiltration analgesia with 20 mL ropivacaine (5 g/L), while the equal volume of normal saline was used instead of ropivacaine in group CFNB. Postoperative pain during rest and passive exercises including front and rear portions of knees, the time of ability to perform an active straight leg raise, the time of ability to reach 90° knee flexion, and preoperative and postoperative hospital for special surgery knee score (HSS) were evaluated.RESULTS:Compared with group CFNB, the visual analogue scores (VAS) of front of knees at rest time in group PLIA had no significant difference (P>0.05); there were significant differences at 4, 8, 12, 24 h postoperation in portions of knees at rest time (P<0.05); the VAS had significant differences at 24 h in passive exercises of knees (P<0.05); the VAS had significant differences at 12, 24 h in portions of knees at passive exercises of the knees (P<0.05); the time of ability to perform an active straight leg raise had significant differences in the two groups (P<0.05).CONCLUSION:Compared with CFNB postoperative analgesia alone, CFNB with PLIA could relieve rest pain and pain during passive movement after total knee arthroplasty. CFNB with PLIA could shorten the time to perform an active straight leg raise and the time of ability to reach 90° knee flexion. And so some patients could improve postoperative rehabilitation training.
Internet+Continuing Nursing (ICN) Program Promotes Motor Function Rehabilitation of Patients With Ischemic Stroke

Abstract:Background and purpose: Ischemic stroke is a severe disorder with high rates of death and recurrence that causes disability in patients and for which there is currently no effective treatment. Internet-based rehabilitation helps patients with disability recover at home with the help of their household or family members in a nonclinical setting. However, the effects of the internet+continuing nursing (ICN) program on the recovery of patients after ischemic stroke remains unknown. Methods: In the present study, patients were treated with an ICN-based rehabilitation training program; subsequently, the Self-efficacy Scale for Chronic Disease, Questionnaire of Exercise Adherence, Motor Assessment Scale, Activities of Daily Living, and Stroke-specific Quality of Life were performed to evaluate the effects of the ICN program on patient self-confidence to persist with rehabilitation, functional exercise compliance, motor function, ability to live independently and quality of life following ischemic stroke. Results: We observed that, after the ICN intervention for 6 weeks and 3 months, the scores of Self-efficacy Scale for Chronic Disease, Questionnaire of Exercise Adherence, Motor Assessment Scale, Activities of Daily Living, and Stroke-specific Quality of Life in the ICN-treated group were significantly higher compared with those in the control group. Conclusion: These results suggested that the ICN program may promote the recovery of patients after ischemic stroke.
Research on the Application of "Internet Plus" Rehabilitation Nursing in Children with Cerebral Palsy

Abstract:Abstract: Currently, cerebral palsy has emerged as a severe disease deserving significant attention. Amidst the thriving development of internet technology, the "Internet+" platform has brought new support to the medical field. This study aims to explore the application effects of "Internet+"-based rehabilitation nursing for children with cerebral palsy. A total of 60 children with cerebral palsy were randomly divided into a control group and an experimental group. The control group received conventional rehabilitation nursing, while the experimental group underwent "Internet+"-based rehabilitation nursing intervention. Comparisons were made between the two groups in terms of overall disease recovery, fine motor function, gross motor function, activities of daily living (ADL), nursing satisfaction, and quality of life. The results showed that the experimental group surpassed the control group in motor function, ADL, disease recovery efficiency, nursing satisfaction, and quality of life assessments, with statistically significant differences (P<0.05). The application of "Internet+"-based rehabilitation nursing for children with cerebral palsy can lead to more desirable overall nursing outcomes.
Effectiveness and safety of outpatient rehabilitation versus home-based rehabilitation after knee arthroplasty: a systematic review and meta-analysis

Abstract:Abstract Background Rehabilitation post-knee arthroplasty is integral to regaining knee function and ensuring patients’ overall well-being. The debate over the relative effectiveness and safety of outpatient versus home-based rehabilitation persists. Methods A thorough literature review was conducted adhering to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines across four databases. Two researchers independently identified eligible studies centering on knee arthroplasty patients undergoing either outpatient or home-based rehabilitation. Study quality was assessed using the Cochrane Collaboration’s risk of bias tool, while continuous outcomes were subject to meta-analyses using Stata 17 software. Results Our analysis identified no significant differences in primary outcomes, including Range of Motion, Western Ontario and McMaster Universities Arthritis Index, Knee Injury and Osteoarthritis Outcome Score, Oxford Knee Score, and the Knee Society Score, between home-based and outpatient rehabilitation across different follow-up points. Adverse reactions, readmission rates, the need for manipulation under anesthesia, reoperation rate, and post-surgery complications were also similar between both groups. Home-based rehabilitation demonstrated cost-effectiveness, resulting in substantial annual savings. Furthermore, quality of life and patient satisfaction were found to be comparable in both rehabilitation methods. Conclusions Home-based rehabilitation post-knee arthroplasty appears as an effective, safe, and cost-efficient alternative to outpatient rehabilitation. Despite these findings, further multicenter, long-term randomized controlled trials are required to validate these findings and provide robust evidence to inform early rehabilitation choices post-knee arthroplasty.

The impact of internet multidisciplinary continuity care on the living quality of home-based patients with malignant tumors undergoing PICC catheterization

Abstract:Objective: To evaluate the impact of Internet-based multidisciplinary continuity care on the quality of life of home-based patients with PICC catheters for malignant tumors. Method: This study is a retrospective research analysis. A total of 131 patients with malignant tumors who received PICC catheterization at Xijing Hospital from Jan. 2023 to Dec. 2023 were enrolled in this study. Patients were divided into two groups: 61 in the control group who received conventional nursing care, and 70 in the observation group who received multidisciplinary continuous nursing care via an Internet platform in addition to conventional nursing. The Strategies Used by Patients to Promote Health (C-SUPPH), the Quality-of-Life Core Questionnaire (QLQ-C30), the Exercise of Self-care Agency Scale (ESCA), Self-Rating Depression Scale (SDS) and Self-Rating Anxiety Scale (SAS) were adopted for investigation and analysis. Six months after intervention, the two groups' ego-efficacy, living quality, self-care ability and psychological status, as well as treatment compliance and satisfaction with care were compared. Results: Post-intervention, both groups showed significant improvements in ego-efficacy and self-care ability scores (all P<0.05). The observation group scored higher in all dimensions of ego-efficacy and self-care ability compared to the control group (all P<0.05). Furthermore, the observation group demonstrated significant improvements across all dimensions of quality of life (P<0.05), whereas the control group only showed improvement in social function (P<0.05). The observation group also exhibited lower scores for anxiety and depression post-intervention compared to the control group (all P<0.05). Additionally, the observation group had a significantly lower incidence of complications and higher post-intervention treatment compliance and satisfaction with nursing (P<0.05). Conclusion: Multidisciplinary continuing nursing care delivered via an Internet platform significantly enhances self-management efficiency, quality of life, and self-care ability of home-based patients with PICC catheters for malignant tumors. This care model also effectively improves patients' mental health and increases their compliance with treatment and satisfaction with nursing.

Abstract:Abstract: The patient with hip replacement becomes a chronic patient who require a successful recovery post surgery. Successful recovery requires not only proper surgical technique but also effective pre and postoperative nursing care to improve their activity of daily livings and enhance their quality of life. Purpose: To determine the effect of pre and postoperative nursing intervention on activities of daily livings and quality of life among patients undergoing hip replacement. Setting: The study was conducted at orthopedic departments at Menoufia University and Shebin El-Kom teaching hospitals. Sampling: A consecutive sample of 80 adult patients who were planned for hip replacement surgery was assigned randomly and alternatively into two equal groups, 40 patients for each group. Instruments: Two instruments were used for data collection: Structured interview questionnaire and hip disability and osteoarthritis outcome score. Results: Pre intervention about half of both study and control groups had sever hip disability related function and activity of daily living scores (55.0 % and 57.5% % respectively) that were highly significantly improved among study group to none complains at follow up period (60.0% of study group compared to 5.0% of control group for function and activity of daily living score respectively). Also pre intervention about half of both study and control groups had sever hip disability related quality of life score (47.5 % and 52.5% respectively) that were highly significantly improved among study group to none complains at follow up period (72.5% compared to 7.5% for quality of life score respectively) .Conclusions: Pre and postoperative nursing intervention has a positive impact on improving activities of daily living and quality of life among patients undergoing hip replacement. Recommendations: Supervised health teaching regarding pre and postoperative care should be given for hip replacement patients to improve their activities of daily livings and quality of life.

Abstract:BACKGROUND: The traditional method of post-hospital intervention and guidance of stroke patients has some limitations. OBJECTIVE: To investigate the effects of Internet + wearable device training on limb function recovery and the levels of serum neurocytokines (BDNF, NT-3, and NGF) in stroke patients. METHODS: 80 stroke patients with hemiplegia were randomly selected from the Department of Neurorehabilitation, Affiliated Rehabilitation Hospital of Chongqing Medical University. They were divided into a control group and an observation group, with 40 patients in each group. The control group received routine post-hospital follow-up guidance, while the observation group received Internet remote home rehabilitation guidance combined with wearable device training. The interventions were compared between the two groups. RESULTS: At 4 weeks and 12 weeks after discharge, the observation group showed higher scores on the Fugl-Meyer scale (FMA), Berg Balance Scale (BBS), modified Barthel Index (MBI), stride length, gait speed, gait frequency, and higher levels of BDNF, NT-3, and NGF. Additionally, the observation group had lower scores on the Hamilton Anxiety and Depression Scale (HADS) (P < 0.05). CONCLUSIONS: The application of Internet remote home rehabilitation guidance combined with wearable device training in stroke patients with hemiplegia can improve limb function recovery. It effectively increases the levels of BDNF, NT-3, and NGF, promoting the nutritional repair of damaged nerves. These findings hold clinical significance.

A Nurse-Led App-Based Home Exercise Program After Total Knee Arthroplasty: A Quasi-Experimental Study

Abstract:Purpose: Reduced activity because of pain is a major health issue associated with total knee arthroplasty. This study evaluated the effectiveness of a nurse-led app-based home exercise program for patients who underwent total knee arthroplasty. Methods: A nonequivalent control group of pretest-posttest design was used. Data from 45 patients were collected. The control group received individualized face-to-face exercise education, whereas the experimental group received training on the use of app-based home exercise education, including exercise videos and app push notifications to encourage exercise after discharge. Pain, range of motion, exercise self-efficacy, and quality of life were measured at baseline and 3 and 12 weeks after surgery. Nursing care satisfaction was evaluated 12 weeks after surgery. Results: There were significant overall reductions in pain intensity ( p = .001), improvements in limited range of motion ( p < .001), and increases in exercise self-efficacy ( p = .034) and quality of life ( p = .033) in the experimental group ( n = 22) compared with those in the control group ( n = 23). Nursing care satisfaction was significantly higher in the experimental group than in the control group ( p < .001). Conclusions: Rehabilitation nurses can offer app-based home exercise education with push notifications to alleviate pain, enhance range of motion, improve exercise self-efficacy, and increase nursing care satisfaction for patients who have had a knee arthroplasty.
