Abstract:Restricted exposure and inconsistencies in sinus pneumatization place the optic nerve at risk during operations on the sphenoid sinus and posterior ethmoid cells. In this study, computed tomography was used to examine these relationships. We reviewed 80 patients who underwent high-resolution computed tomographic scanning for ophthalmologic complaints in which the scan was negative. Forty-eight percent of posterior ethmoid cells are separated from the optic nerve by the thin bony lamina of the optic canal. Nearly 90% of sphenoid sinuses contact the ipsilateral optic nerve and 10% contact both nerves. Eight percent of posterior ethmoid cells override the ipsilateral sphenoid sinus and contact the optic nerve on that side. Paraxial reformatted displays allowed estimation of the degree of projection of the optic nerve into adjacent sinus cavities. Three percent of optic nerves have significant projection into the posterior ethmoid cell, and 23% project significantly into the sphenoid sinus. The width of the bony plate that separates the optic nerve from the sinus cavity was the same for sphenoid and ethmoid sinuses. Although sinus pneumatization varies among individuals, right and left sides are generally similar within one person.
Analyzing Taiwan Bridge Management System for decision making in bridge maintenance: A big data approach

Abstract:The Taiwan Bridge Management System (TBMS) has been online for 15 years and has an inventory of 33,275 bridges, including all kinds of bridges and culverts in Taiwan. Currently, the number of fields in all tables in the databases of TBMS is around 6,500 with more than 3 million data records in its databases. Meanwhile, bridge inspection data and maintenance data are increasing at a speed of 15,000 records annually. Thus, the TBMS databases are deemed as “Big Data.” There are more than 9,500 bridges that are over 20 years old with another 7,200 bridge having unknown built years in the TBMS. The bridges in Taiwan have stepped into the stage where maintenance is crucial and frequently required. Therefore, this research aims at analysing the database in the TBMS using “Big Data” approach for determining maintenance strategies for these bridges. This paper describes results of the first year's research efforts. Relevant literature in bridge maintenance, prioritization, and life-cycle bridge management were firstly reviewed. Concepts, theories, techniques, and available software for analysing “Big Data” were also intensively examined and summarized. In next year, functions will be programmed and applied to the TBMS databases using appropriate “Big Data” software to obtain useful information in bridge deterioration, repair methods, and maintenance costs.
Protrusion of the optic nerve into the ethmoid and sphenoid sinus: prospective study of 150 CT studies

Abstract:Damage to the optic nerve is a serious complication of intranasal sinus surgery. Protrusion of the optic canal into the sphenoid sinus is a major risk factor. In this prospective study of 150 CT studies, we found this anatomical variation in 8% of cases. It was consistently associated with pneumatisation of the ipsilateral anterior clinoid process. Protrusion of the optic nerve into the posterior ethmoid cells was never observed. CT is a useful part of the preoperative investigation of patients undergoing intranasal sinus surgery. Axial and coronal images obtained or by direct acquisition by reconstructions are necessary to show the position of the optic nerve.

Abstract:INTRODUCTION: Posterior paranasal sinuses consisting of posterior ethmoid and sphenoid sinuses show varying pneumatization and relationship to Optic Nerve Canal (ONC). Most commonly ONC is superolateral to sphenoid sinus, however varied protrusion of ONC into the sinuses may occur. Rarely it may pass completely through the sinus. Optic Nerve (ON) is covered by a thin bony layer or by periosteum and sinus mucosa without bone. Extensive pneumatization of sphenoid sinus and bony dehiscence predisposes to ON injury. The posterior ethmoid cell namely the Onodi cell is of great surgical importance when the ON is along its lateral wall and surrounds the nerve. These varied relations imply that a detailed knowledge of the intimate relationship of ONC with the posterior paranasal sinuses is necessary to avoid unintentional complications during the surgeries involving the ONC region.AIM: To assess relationship of ONC to the posterior paranasal sinuses in Indian Ethnic study group.MATERIALS AND METHODS: Retrospective review of 100 paranasal sinus Computed Tomography (CT) scans were done using 64 row Multi Detector Computed Tomography (MDCT). Multiplanar reformation images were assessed for ONC relation to posterior ethmoid and sphenoid sinuses, wall dehiscence, and Pneumatization of Anterior Clinoid Process (PACP).RESULTS: 200 optic nerve canals were assessed and grouped into four types based on the modification of Delano et al., classification. The most common OCN was Type-1 (60%), followed by Type-2 (15%), Type-3 (14%) and Type-4 (11%). Dehiscence was seen in 35(17.5%) mostly in Type-3 canals. PACP was seen in 30 (15%).CONCLUSION: Knowledge of ON relationship to posterior paranasal sinuses will reduce the devastating complications during sinus surgeries. Our modified ONC classification is based on the bony canal morphology and the degree of protrusion into sinuses. As with other reported studies worldwide, Type-1 ONC is the commonest among the Indians also. Type-2 and 3 ONC with sinus wall dehiscence and PACP are the most important anatomical variations which may potentially predispose to risk of surgical injury to the optic nerves.

Evaluation of variations of optic nerve course in relation to posterior paranasal sinuses in MDCT in a tertiary care center of Nepal: a retrospective cross-section study

Abstract:Background: The proximity of optic nerves to the posterior paranasal sinuses (PNS) is a critical consideration in preventing optic nerve injuries during functional endoscopic sinus surgery. Methods: A retrospective cross-sectional study was conducted on 367 patients aged 13 years and above. Four radiologists, each with 2-6 years of experience, evaluated computed tomography scans of the PNS and the head of these patients. The optic nerves were classified into four types based on DeLano's classification, and their respective prevalence rates were determined. Additionally, the prevalence of optic nerve dehiscence and pneumatization of the anterior clinoid process was assessed. Results: A total of 734 optic nerves were evaluated and categorized into four groups as per DeLano's classification. The most common type was classified as type 1 optic nerve, representing 65.4% of all optic nerves. Types 2, 3, and 4 optic nerves accounted for 16.9, 8.6, and 9.1%, respectively. The prevalence rates of type 1, type 2, type 3, and type 4 optic nerves were 76.6, 24.5, 12.3, and 14.4%, respectively. Optic nerve dehiscence was observed in 14.3% of cases, with a prevalence of 21.2% in the study. Type 3 optic nerves were most commonly associated with dehiscence, with 49.2% of them showing dehiscence in their course through the ethmoid or sphenoid sinus. The prevalence of pneumatization of the anterior clinoid process in the study population was 28.3%, with type 3 optic nerves being the most frequently associated. Conclusion: An understanding of the relationship between optic nerves and posterior PNS, as well as awareness of optic nerve dehiscence, is essential in preventing injuries during sinus surgeries. Type 1 optic nerve predominates among Nepalese patients visiting our hospital.
A Computed Tomography (CT)-Based Observational Study of Anatomical Variations in the Sphenoid Sinus: Implications for Surgical Planning and Patient Outcomes

Abstract:Introduction: The sphenoid sinus (SS), a paired paranasal sinus located within the sphenoid bone, is crucial in various physiological and pathological processes. Its anatomical variations are of significant interest in clinical practice, particularly in otolaryngology, neurosurgery, and radiology. This study aims to determine the anatomical variations of the SS and related structures using computed tomography (CT). Materials and methods: An observational study was conducted at a tertiary care center. The study included 300 patients aged 7-70 who underwent CT brain scans. Exclusions included prior sinonasal surgeries, tumors, nasal polyposis, recurrent pituitary lesions, head trauma, and past orbital or cranial surgeries. Three-dimensional reconstructions assessed SS dimensions, pneumatization types, and variations in the internal carotid artery and optic nerve. Results: A study on the types of SSs revealed that the reseller type is the most common, accounting for 45% of cases (135 instances). The sellar type accounts for 36% (110 instances), while the conchal type is the least common, observed in 18.33% of cases (55 instances). A significant association between the SS type and variations between neurovascular structures was seen, which was confirmed using chi-square tests. There was a statistically significant relationship between carotid artery variations and SS, with the normal course being predominant at 200 individuals (73.33%). Approximately 40 cases (13.33%) present with dehiscence through the sinus, while 30 individuals (10%) show close proximity. Other, less common variations are observed in 10 patients (3.33%). Optic nerve variations displayed distinct frequencies, with the normal course prevailing in 250 cases (83.33%). Approximately 30 cases (10%) exhibit close proximity to surrounding structures, while 15 cases (5%) present with dehiscence through the sinus. Other less common optic nerve variations are observed in five patients (1.67%). Conclusion: Comprehensive knowledge of SS anatomy through CT scans is essential for enhancing surgical outcomes and ensuring patient safety.
[Relationship of Onodi cell to optic neuritis--radiological anatomy on coronal CT scanning]

Abstract:The relationship of paranasal sinusitis to optic neuritis remains controversial. One of the major sources of this controversy is that there are some reports of rhinogenic optic neuritis (RON) in patients with mild paranasal sinusitis or with almost normal paranasal sinuses. The Onodi cell is a posterior ethmoid cell which pneumatized far laterally and to some degree superiorly to the sphenoid sinus and is intimately associated with the optic nerve. Coronal CT scanning is requisite to detect the Onodi cell; when it is present, an image of the sphenoid sinus just as if it were divided into top and bottom is characteristic. In our material from 200 patients (direct coronal CT scans 10 mm in width), the Onodi cell was observed in 7%. A case of RON whose pathogenesis was considered to be a direct spread of inflammation from the localized infection of the Onodi cell is reported. Ethmoiditis localized to the Onodi cell seems to play an important role in the pathogenesis of RON. Continued careful documentation of the localized posterior paranasal sinus lesion around the optic canal by detailed diagnostic imaging and endoscopic sinus surgery is necessary to resolve the disease entity of RON.
The Optic Nerve in the Posterior Ethmoid in Asians

Abstract:Anatomical dissections employing the endoscopic sphenoethmoidectomy technique were performed in 51 fresh Asian cadaveric heads. Specific data concerning the anatomical variations of the posterior ethmoid sinus and its relationships to the optic nerve were recorded. Approximately two-thirds (65%) of the specimens showed a direct relationship between the optic nerve and posterior ethmoid sinus. Posterior ethmoid cells with an optic canal bulge were found in 50.98% of the 102 sides dissected. In 14.7% no optic canal bulge was present, yet the optic nerve was intimately related to the posterior ethmoid sinus. In this latter group, the average thickness of bone separating the optic nerve from the posterior ethmoid sinus was 0.25 mm. The authors consider that in endoscopic sinus surgery, this type of relationship is potentially the most hazardous to the optic nerve.

Abstract:The relationship of paranasal sinusitis to optic neuritis remains an intriguing curiosity to both the otolaryngologist and the ophthalmologist. The literature is replete with anecdotal case reports of patients whose sinusitis appears to have caused optic neuritis. There is much speculation about the pathophysiological mechanisms which relate these two distinct disease entities. Five new cases are described which highlight distinct pathophysiologic routes through which paranasal sinus disease has caused optic neuritis. These include compressive optic neuropathy secondary to mucoceles and/or pyoceles; direct extension of sinus infection to the optic nerve from suppurative paranasal sinusitis; and, in one case, from osteomyelitis of the ethmoid and sphenoid sinuses. The usefulness of computerized axial tomography of the orbits and paranasal sinuses to evaluate optic neuritis and to elucidate in detail the pathophysiology of its relationship to disease of the paranasal sinuses is emphasized. Currently, optic neuritis is felt to be a rare complication of paranasal sinusitis. Paranasal sinus surgery is advocated in those cases where sinus suppuration is suspected, or when a compressive optic neuropathy is caused by a sinus mucocele or pyocele. Since in most cases, however, optic neuritis is self-limited, it is difficult to evaluate the results of surgery in circumstances other than those mentioned already. Continued careful evaluation, management, and documentation of this group of patients is necessary to help better define the relationship between these two disease entities.

Optic nerve changes in chronic sinusitis patients: Correlation with disease severity and relevant sinus location

Abstract:PURPOSE: This study was to evaluate whether optic nerve damage occurs in eyes with adjacent chronic sinusitis.METHODS: Data were collected from eighty-eight eyes of 46 chronic sinusitis patients and 93 eyes of 57 normal controls. Visual sensitivity using standard automated perimetry (SAP) and inner retinal thickness using optical coherence tomography (OCT) were measured. The Lund-Mackay system was used to quantify radiographic findings on the ostiomeatal unit CT scan with a numerical score representing the severity of sinusitis.RESULTS: There was a significant positive correlation between the pattern standard deviation (dB) and Lund-Mackay score (P = 0.031). Nasal retinal nerve fiber layer (RNFL) thickness, average, minimum, superotemporal, superior, superonasal, and inferonasal ganglion cell-inner plexiform layer (GCIPL) thickness were negatively correlated significantly with Lund-Mackay score (all, P < 0.05). Eyes with grade 2 opacification of the posterior ethmoid sinus showed a significantly lower mean deviation (dB) and higher pattern standard deviation (dB) than those with clear respective sinuses (P = 0.007 and <0.001, respectively). Eyes with grades 1,2 and 3 opacification of the sphenoid sinus had a significantly less average RNFL thickness (P = 0.004, <0.001, and <0.001, respectively) and a significantly less average GCIPL thickness (P = 0.004, 0.003, and 0.003, respectively) than those with a clear sphenoid sinus.CONCLUSIONS: Structural and functional optic nerve changes were correlated with the severity of chronic sinusitis. Inflammation of the posterior ethmoid and sphenoid sinuses was associated with optic nerve changes to a greater extent than that of the other paranasal sinuses.

Abstract:Objective: This study aims to determine the prevalence and types of Onodi cells through computed tomography and investigate the relationship between Onodi cell and the surrounding structures, paying particular attention to the risky proximity to the optic nerve canal.Methods: In this study, 430 computed tomography scans of paranasal sinuses were analysed to establish the prevalence and different types of Onodi cells. Furthermore, the relationship between Onodi cell and different patterns of sphenoid sinus pneumatisation and surrounding structures were investigated. Special attention was paid to the relationship between Onodi cell and the optic nerve canal, particularly in cases when the optic nerve canal was bulging by more than 50% into the Onodi cell (Type IV).Results: The Onodi cell was detected in 21.6% of cases, with the most common being Type I (48.5% right, 54.3% left). Type IV bulging of the optic nerve canal into the Onodi cell was observed in 47.1% of cases on the right side, 41.2% on the left side and bilateral in 11.7% of cases.Conclusions: In our series, we observed a high prevalence of Type IV optic nerve bulging into the Onodi cell. For this reason, we suggest that clinicians should always try to identify it in a pre-operative setting with computed tomography to avoid catastrophic consequences during endoscopic sinus surgery approaching the sphenoid area.

Abstract:Of the various conditions of the eye due to disease of the sinuses none are so well recognized or established as the diseases of the optic nerve. It has been several years since Onodi (1904) and others demonstrated the close relationship of the optic nerve to the sphenoid and posterior ethmoid cells. Since then, a large accumulation of clinical cases has shown that the causal relationship exists beyond question. There are today few ophthalmologists of experience who on seeing an inflammation of the optic nerve do not send the patient to a rhinologist for a report as to the presence of ethmoiditis or sphenoiditis. Many times a positive report is returned, but on other occasions nothing suggestive of disease is found in the nose. This happens so frequently that evidently there is a large class of cases in which no intranasal symptoms occur, or else they are peculiarly difficult

Unravelling the Complex Relationship Between Frontal Sinus Pneumatization and Paranasal Sinus Anatomical Variants: Insights from CT Imaging

Abstract:The aim of this study was to analyse the relationship between frontal sinus pneumatization and its effect on various variations of paranasal sinuses on paranasal sinus CT scan. A total number of 100 patients (51 males, 49 females) were included in this study. Patients were divided into three groups on the basis of their frontal sinus pneumatization extent on paranasal sinus CT as Type 1 pneumatization (aplasia and hypoplasia), Type 2 pneumatization (medium) and Type 3 pneumatization (hyperplasia). The prevalence of various variations like pneumatization of upper and middle turbinate, presence of agger nasi, Haller and Onodi cell, maxillary sinus hypoplasia, extent of sphenoid sinus pneumatization, pneumatization of anterior clinoid process, pterygoid process and greater wing of sphenoid sinus, dehiscence and protrusion of internal carotid artery and optic nerve types were ascertained and compared within each group. The prevalence of Onodi cell, anterior clinoid process and greater wing of sphenoid pneumatization and optic nerves type 3 and 4 were higher in Type 3 frontal sinus pneumatization group as compared with other groups. Excessive pneumatization of frontal sinus has various effects on paranasal sinus variations. Surgeons dealing with endoscopic sinus surgery, maxillofacial and skull base surgery must be vigilant of association between these variations preoperatively for avoiding complications.

Abstract:Abstract The relationship of sinusitis and paranasal sinus mucoceles to optic neuritis (ON) remains a challenge for both the otolaryngologist and the ophthalmologist. Currently, ON is assumed to be a rare complication of paranasal sinus diseases. Three new cases of ON associated with paranasal sinus diseases are described. We postulate different pathophysiologic mechanisms of ON in our three patients: compression of optic nerve due to sphenoid mucocele in the first patient; extension of bacterial infection to the optic nerve in the second patient; and invasion of aspergillosis to the orbital apex in the third patient. We assume that the complete recovery of the patient’s vision in the second case resulted from surgery, which had been performed immediately. A timely operation could possibly have prevented permanent blindness in cases 1 and 3 as well. Optic neuritis can occasionally be a complication of bacterial and mycotic sinusitis or paranasal sinus mucoceles, especially if located in the sphenoid sinus and posterior ethmoids. The evaluation of paranasal sinuses using imaging techniques is necessary to diagnose the problem, and immediate surgery can prevent permanent sequelae in indicated cases.

Abstract:PurposeTo describe the clinical presentation, imaging characteristics and management of a rare case of Onodi cell sinusitis associated with optic neuropathy.ObservationsA 46-year-old male presented to the emergency department with progressive left eye vision loss over the course of ten days. The constellation of findings from this patient's history, physical, and fundoscopic exam, as well as CT and MR imaging led to the diagnosis of Onodi air cell sinusitis complicated by optic neuropathy. The patient's symptoms resolved fully and vision returned to baseline with oral antibiotics.Conclusionsand Importance: The sphenoethmoidal air cell, also known as the Onodi cell, is an anatomic variant of the paranasal sinuses whose spatial relationship with important neurovascular structures carries significant clinical implications when it becomes inflamed or infected. Our case of Onodi cell sinusitis complicated by optic neuropathy demonstrates how vision loss secondary to sinusitis may resolve with oral antibiotic treatment. We additionally review the relevant anatomy, clinical course and treatment of Onodi cell pathologies.

Intra-optic nerve abscess: A rare complication of acute sinusitis

Abstract:IntroductionIntracranial complications from acute sinusitis are rare. In particular, intra-optic nerve abscess has not been described. A case report and review of orbital complications from sinusitis is presented here to emphasize the extreme rarity of this clinical entity.MethodsCase report and literature review. The patient's pertinent history, clinical findings and radiologic studies are examined.ResultsThe case of a 33 year old male who was transferred to a tertiary care academic medical center with acute vision loss after developing signs and symptoms consistent with acute sinusitis is presented and reviewed. A computed tomography scan demonstrated pansinusitis with worse severity on the left side. Magnetic resonance imaging revealed a right-sided abscess along the roof of the right sphenoid sinus and the right optic nerve towards the optic chiasm. The patient's clinical exam was consistent with right-sided optic neuropathy. He underwent an emergent functional endoscopic sinus surgery and subsequent right-sided open craniotomy for drainage of an epidural and intracranial abscess involving the right optic nerve, chiasm, and optic tract. The sinusitis was odontogenic in origin. The patient was treated for six weeks with IV ceftriaxone, penicillin and metronidazole and transitioned to five months of PO amoxicillin. At 3-month follow-up, the patient had no evidence of sinusitis and stable ophthalmologic findings.ConclusionsThe occurrence of an intra-optic nerve abscess is a rare but possible sequela of acute sinusitis. A multidisciplinary approach involving otolaryngology, neurosurgery, ophthalmology, neuroradiology and infectious diseases is paramount. Surgery and antibiotic therapy is the standard of care.

Abstract:Expression of the iap gene of Listeria monocytogenes in the L. monocytogenes rough mutant RIII and in Bacillus subtilis DB104 caused the disruption of the cell chains which these two strains normally form under exponential growth conditions. The p60 protein produced by L. monocytogenes and B. subtilis DB104 also exhibited bacteriolytic activity detected in denaturing polyacrylamide gels containing heat-killed Micrococcus lysodeikticus. Purification of the p60 protein led to aggregation of p60 and loss of the cell chain disruption and bacteriolytic activities. A cysteine residue in the C-terminal part of p60 which is conserved in all p60-like proteins from the other Listeria species seems to be essential for both activities. The iap gene could not be inactivated without a loss of cell viability, indicating that p60 is an essential housekeeping protein for L. monocytogenes and probably also for other Listeria species. These data suggest that p60 possesses a murein hydrolase activity required for a late step in cell division.
