Abstract:Obesity has become a pressing global public health crisis, particularly acute among children and adolescents. In 2022, the World Health Organization (WHO) identified approximately 390 million children aged 5–19 as overweight or obese, marking a significant increase over the last decade. 1 In China, the crisis is stark, with the overweight and obesity rate among children and adolescents aged 6–17 reaching 19%, reflecting broader global trends. 2 These statistics are not just numbers; they represent a growing cohort at increased risk for non-communicable diseases such as diabetes and cardiovascular disorders—conditions that could affect their health well into adulthood. Moreover, obesity profoundly impacts the mental health and overall quality of life, turning the effective prevention and control of this issue into an urgent priority for public health systems worldwide. 3 Addressing this challenge requires a concerted policy effort aimed at both prevention and management. The WHO defines obesity as "abnormal or excessive fat accumulation that presents a risk to health". 4 This condition significantly exacerbates various health issues through complex biological mechanisms. Obesity induces insulin resistance, increasing type 2 diabetes risk by disrupting insulin receptor signaling through pro-inflammatory cytokines and free fatty acids from visceral fat. 5 It also elevates cholesterol levels and blood viscosity, promoting thrombosis and atherosclerosis, thus heightening cardiovascular disease risk. 6 Obesity adversely affects the respiratory system as well, such as by causing obstructive sleep apnea syndrome. This occurs because fat accumulates in the throat area, narrowing the upper airway, and increased abdominal and thoracic fat reduces thoracic volume and lung compliance. Additionally, adipose tissue produces pro-inflammatory cytokines, leading to localized and systemic inflammation, which increases the risk of airway collapse. These mechanisms collectively exacerbate the occurrence and progression of obstructive sleep apnea. 7 The increased mechanical load on joints and inflammatory responses in obese individuals lead to musculoskeletal problems such as osteoarthritis. 8 Research also indicates that adolescent obesity creates a pro-carcinogenic environment through mechanisms such as chronic low-grade inflammation, dysfunction of natural killer cells, increased pro-inflammatory cytokines and adipokines, as well as growth factors and oxidative stress. These metabolic and immune system alterations significantly increase the risk of cancer in adulthood, including leukemia, Hodgkin's disease, colorectal cancer, and breast cancer. 9
