Abstract:Abstract Introduction Behavioural interventions may be effective for the prevention of occupational musculoskeletal disorders as an addition to technical and organisational measures. This open label randomised controlled trial evaluates the effect of an ergonomic patient handling training program on the musculoskeletal load of care staff in a nursing home. Methods The program consisted of 15 training sessions right in the nursing home, combined with additional meetings on the ward. The primary outcome was the weighted cumulative musculoskeletal load of the lower back, knees and shoulders, assessed with a motion-capturing system and video for each body region. Results We were able to perform the study with some protocol changes, despite of massive delays and high drop-out rates during the COVID-19 pandemic. For six remaining participants of the intervention group the median cumulative load on the lower back increased from 12,632°s at baseline to 30,735°s at follow-up. The median cumulative load on the shoulders increased from 17,274°s to 63,884°s. Simultaneously the time to perform the task increased more than fourfold from baseline to follow-up. The relative load per minute decreased. For the knees, not only the cumulative load increased from 8,418°s to 48,694°s, but also the relative load. Discussion/Conclusion The training did not clearly reduce the musculoskeletal load. Although extreme deviations from the neutral position (such as leaning forward and twisting) have been reduced, the time spent in postures that are still unfavorable, such as slightly leaning forwards and twisting, has multiplied, since the execution of the processes required much more time than before.
