Abstract:This study was conducted to determine the relationship between preparedness for caregiving and spiritual well-being in the carers of stroke patients. This descriptive and cross-sectional study was conducted with 130 primary carers of patients hospitalized with the diagnosis of stroke at the neurology clinic of a hospital. It was determined that the participants had moderate levels of preparedness for caregiving, they had high levels of spiritual well-being, and there was a positive significant relationship between these two variables. According to the regression analysis results, the spiritual well-being level of the participants was a predictor of their preparedness for caregiving ( B : 0.144, p < 0.001). The results of this study demonstrated that the spiritual well-being of the carers of stroke patients is important in increasing preparedness for caregiving. In this context, to ensure that the carers of all stroke patients feel prepared for the caregiving process, it is recommended to evaluate their preparedness levels, support them in their preparation for their caregiving roles, and identify their spiritual needs.

Abstract:Background Stroke is a disease that affects the arteries leading to and within the brain. Stroke usually creates a range of disabilities such as physical deficits and difficulty in performing daily activities. These long‐term needs and the continuous caregiving sequence affect the care burden and spiritual well‐being of caregivers. Method The correlational study population consisted of caregivers of stroke older patients (N = 100). Burden Interview Scale, Spiritual Well‐Being Scale and Religious Coping Scale were used. Descriptive analyses, independent sample t‐test, one‐way analysis of variance, correlation, regression analyses were used to evaluate the data. Results The mean age of caregivers was 48.38 ± 13.52 years. The score of Burden Interview Scale was 31.41 ± 15.13 (light/moderate), Negative Religious Coping was 7.39 ± 3.17 (moderate), Positive Religious Coping was 24.30 ± 4.34 (high), and Spiritual Well‐Being was 21.65 ± 6.73 (low). There is a significant relationship between Burden Interview and Negative Religious Coping (r = 0.274; P = 0.006), Burden Interview and spiritual well‐being (r = −0.563; P = 0.000), and spiritual well‐being and positive religious coping (r = 0.228; P = 0.026). The Lived Experience of Caregivers in the Older Stroke Survivors' Care Pathway during the Transitional Home Program—A Qualitative Study

Abstract:Background: stroke is a major cause of disability and death, globally. Many stroke survivors live with disabilities, requiring significant caregiving support. Caregivers often feel unprepared and burdened, experiencing isolation and health declines. Their well-being and involvement in discharge planning impact post-discharge care quality. Purpose: to investigate the experiences of caregivers of older stroke survivors regarding their expectations in the care pathway during the transitional home program, as this phenomenon is currently understudied in the present context. Methods: by employing a qualitative design, this study utilized a phenomenological approach developed by Cohen. Eighteen caregivers of stroke survivors participated. Findings: four main themes emerged from the analysis: (1) the sense of loss experienced by stroke survivors and perceived by caregivers; (2) the importance of social support in the recovery and well-being of both stroke survivors and caregivers; (3) the increased workload of caregivers; and (4) the lack of awareness of the needs during the transition, leading to resignation when facing complications. Conclusion: the findings highlight the significant impact of stroke on caregivers, affecting both the individual characteristics and social relationships of stroke survivors and their caregivers. There is a need for a nuanced understanding of caregivers' roles and responsibilities.

Abstract:Background: Globally, stroke is a leading cause of death and disability, with most care undertaken by caregivers who are generally family and friends without prior experience of care. The lack of experience or unpreparedness results in feelings of uncertainty, burnout, anxiety, burden, etc. Hence, it is necessary to identify the needs of caregivers to better support them in their caregiving journey and improve the quality of care delivered. Methods: The study employed a grounded theory methodology that utilizes information gathered from literature reviews and social media to represent the needs and create a storyline visually. The storyline is further refined and evaluated using an online survey of 72 participants recruited through online stroke caregiving communities. Results: The study identified four core categories of needs: (i) Information: sufficient information delivered in layman's terms based on the individual situation of the caregiver and survivor through oral and hands-on demonstrations, (ii) Involvement: inclusion in the decision-making processes at different stages of recovery through face-to-face communication at the hospital, (iii) Self-care: ability to engage in work and leisure activities, (iv) Support: receive support in the form of resources, services and finances from different other stakeholders. Conclusions: There is a need to create a caregiver-centered approach in stroke recovery to ensure limited obstruction to care and reduced uncertainty in stroke recovery. Moreover, through the inclusion of caregivers in stroke recovery, it may be possible to reduce the burden of care to the caregiver and ensure the satisfaction of the healthcare system throughout stroke recovery.
BURDEN OF FAMILY CAREGIVER IN CARING FOR STROKE PATIENTS: A MIXED METHOD RESEARCH

Abstract:Background: Stroke is the fifth leading cause of death in the world, stroke does not only have a negative impact on patients but also has an impact on family caregivers. This study aims to describe the burden experienced by family caregivers in caring for stroke patients. Methods: This research uses a mixed method with sequential explanatory approach. The research was conducted from August to October 2019 involved 106 family caregivers using a purposive sampling technique. Burden experienced was measured with the Zarit Burden Interview (ZBI) questionnaire. Quantitative data analysis was performed by SPSS and qualitative data analysis with Opencode. Results: Burden experienced was at a mild level, there was a significant relationship to education variables (p = 0.007, r = -0.297), family relationships (p = 0.001, r = 0.001), number of caregivers (p = 0.001, r = 0.364), and the patient's duration of stroke (p = 0.006, r = -0.322). There are three themes in this study, that is difficulties in caring, motivation for caring and sources of support. Conclusion: Although quantitatively the family caregiver experiences a mild burden, qualitatively the family caregiver reports complaints in caring for almost all aspects, so treatment is needed to resolve this burden.
Resilience and caregiving ability among caregivers of people with stroke: The mediating role of uncertainty in illness

Abstract:Background: In China, stroke survivors are usually cared for by their family members. However, the caregiving ability of these informal caregivers remain inadequate during the hospitalization of their family members following a sudden onset of a stroke, and this sudden need for care overwhelms caregivers even after the hospital discharge. Therefore, research is required to identify predictors of caregiving ability that could be targeted in future interventions aimed at improving caregiving skills and reducing the burden on caregivers who care for stroke survivors. Materials and methods: From August 2019 to February 2020, stroke survivors were hospitalized for the first time, and their family caregivers were registered via convenience sampling. Caregiver demographic information, resilience status, uncertainty in illness, caregiving ability, and patients' severity of stroke were measured using standardized questionnaires. Structural equation modeling was used to test the proposed model, where caregiver resilience and stroke severity predicted caregiving ability directly, and uncertainty in illness mediated the association between caregiver resilience and caregiving ability. Results: A total of 306 dyads were included in the study. The tested model fit the data well (χ 2 = 118.2, df = 64, RMSEA = 0.053, CFI = 0.946, TLI = 0.923). Statistically significant pathways linked caregivers' resilience status to uncertainty in illness (β = −0.558, S.E. = 0.022, P < 0.01), caregivers' resilience to the status of caregiving ability (β = −0.269, S.E. = 0.013, P < 0.01) and caregivers' uncertainty about the illness to caregiving ability (β = 0.687, S.E. = 0.051, P < 0.01). We also found that caregivers' uncertainty in illness mediated the association between caregivers' resilience and caregiving ability (β = −0.384, S.E. = 0.061, P < 0.01). Conclusions: Our structural equation modeling result identified resilience and uncertainty about the illness as predictors of the caregiving ability of informal family caregivers who suffered from care burdens. Supporting family caregivers to build their resilience and reduce illness uncertainty may improve caregiving for stroke survivors.

Abstract:SUMMARY The aim of this study was to determine the effect of home care provided for stroke patients and education of caregivers on the caregiver burden and quality of life. The study was conducted by using a true experimental method with pre-test and post-test control group. The study included 43 experimental and 43 control patients and their caregivers. The Patient and Caregiver Description Form, Patient Problem Identifying Form, Caregiving Burden Scale and SF-36 Quality of Life Scale were used to collect data in the study. Nursing care was provided to the experimental group patients according to the model of daily living activities, while their caregivers received training and consultancy. In the control group, there was no such intervention. Percentage distribution, χ2, independent sample t-test, paired t-test, Mann-Whitney U test, and Wilcoxon signed-rank tests were used on statistical analyses. There was no significant difference in pre-test score means between experimental and control group caregivers. However, a statistically significant difference was found in all sub-dimensions of the quality of life scale between pre-test and post-test score means in the experimental group caregivers. This study results indicated that home care provided for stroke patients and education of caregivers decreased the caregiver burden and increased their quality of life.

Abstract:Background, Stroke is a scientific emergency, for people experiencing a stroke, the difference between recovery and disability or loss of life is measured in hours, rehabilitation clinician to be familiar with stroke, information a way to understand and manage the acute stroke further to having the cap ability to provide evaluation and personal own family members. The aim was to assess care provided by family caregivers for post stroke patient at home. Design: Descriptive research design was used. Setting: This study was conducted at the stroke neurological departments and the neurological outpatient clinic at Minia University Hospital. Sample: A purposive sample of 180 cases within six months (from first July to the end December 2020), it was completed with care givers at home visit evaluation patients and their family's caregivers. Tools: first tool included part 1: Socio-demographic characteristics and medical health history about patient, Part 2: The Barthel Index to assess degree of independence of the patient ,it is consist of three items .second tool included part 1: post stroke care checklists to assess (exercise, medications, preventing another stroke, pressure ulcer, mobility and rehabilitation) part 2: post stroke complications checklists to assess (pneumonia, urinary tract infection and Deep venous thrombosis) (DVT). Results: the most of study sample are married while two third of sample has insufficient incomes to their family, two third of post stroke patients of study are affected by pneumonia, and half of study sample are complained from hypertension. Less than half of study sample are independent for total Barthel Index. Conclusion: The current study concluded that less than half of post stroke patients were complained from hypertension, while less half of post stroke patient in the present study sample were independent for feeding, also more than half of post-stroke caregivers gave patient medications on time. The most of stroke patients was suffering from depression and two third have urinary tract infection, the half of study sample were independent for total Barthel Index in spite of there were two third of studied sample were satisfied for total post stroke care . Recommendation: the study recommended that; health education with the preventive information especially at the primary level about the stroke, develop a team of discharge plan to be responsible about providing the patients and their family caregivers with the knowledge and practice about post stroke care.

Abstract:Taking care of a family member(s) who suffers from stroke at home especially during the covid-19 pandemic time is unquestionably not easy. This study aimed at exploring the experience of family-caregivers in caring for post-stroke patients at home during the covid-19 pandemic. This qualitative research employed a phenomenological approach with participants consisting of 14 family members who have been caring for post-stroke patients at home during the Covid-19 pandemic in the Pontianak City area. This research began with observation for 3 months which was then followed by interviews and FGDs. The data obtained from the results of in-depth interviews and FGDs were processed using content analysis. The result of his study generated 5 themes regarding the experience of family caregivers in caring for post-stroke patients at home during the Covid-19 pandemic including 1) Changes in caregiver roles, 2) engagement values, 3) spiritual values, 4) Isolation in the surrounding community, 5) The experience of situational anxiety. Considering that both stroke survivors and caregivers of post-stroke patients do not only face several problems and conditions related to stroke and stroke recovery as well as community stigma, but several live scenarios are also provided for them especially during the COVID-19 pandemic. Therefore, further community reintegration ought to exist in the rehabilitation where it can prepare someone who used to suffer from stroke to have ability to do self-reorganization psychologically, physically and socially.
Understanding Experiences of Caregivers of Spouses with Dementia During Caregiver Healthcare Emergencies

Abstract:Abstract Background and Objectives Caregivers of persons with dementia are frequently spouses. Caregiver hospitalization causes disruption to caregiving. The goal of this research was to understand the preparedness and stress trajectory peri-caregiver hospitalization. Research Design and Methods Mixed methods were used. Caregivers of spouses with dementia (n=1000) were surveyed to determine their perceived preparedness for their own hospitalization. Journey mapping interviews (n=18) were used to map caregivers’ experiences during five phases: 1) their spouse with dementia (SWD)’s dementia diagnosis, 2) their SWD’s dementia progression, 3) their own health event, 4) their own hospitalization, and 5) their own return home from hospital. Results Among the 452 (45%) eligible caregiver survey respondents, 75 (17%) had experienced a hospitalization in the previous 12 months and 51 (68%) hospitalizations were unexpected. Twenty-three (31%) of hospitalized caregivers indicated they did not have prior plans in place for the care of the SWD. When asked about an unexpected hospitalization in the future, 233 (52%) felt somewhat prepared and 133 (29%) felt not at all prepared. Journey mapping revealed three groups of caregivers: Group 1 (n=7) rated their stress lower during their hospitalization, Group 2 (n=7) rated their stress highest during their hospitalization, and Group 3 (n=4) were at a sustained high-stress level. Discussion and Implications Many caregivers are not prepared for their own hospitalization. The stress trajectory through important phases of dementia caregiving and a caregiver’s own hospitalization is not universal. Meeting the needs of caregivers peri-hospitalization should be tailored to the individual caregiver.

Abstract:BACKGROUND: Stroke is a life-changing event for both stroke survivors and their family caregivers. After receiving acute care at the hospital, family members are expected to take care of stroke survivors at home and to continue treatment and rehabilitation. The new role of "informal caregiver" is a challenge that creates many difficulties for family caregivers that are not explicit in the Sri Lankan context.OBJECTIVES: This study aimed at exploring family caregivers' experiences of providing informal care for dependent stroke survivors.METHODS: The sample was chosen by purposive sampling with a maximum variation by age, ethnicity, religion, educational level, relationship, and monthly income. Ten informal family caregivers to stroke survivors with hemiplegia who had been treated at the National Hospital of Sri Lanka participated in in-depth interviews analyzed using conventional content analysis.RESULTS: Qualitative content analysis of data resulted in an overriding theme, "Caring with love, against all odds," along with four categories, "Life alterations," "Lack of resources," "Compassionate care," and "Coping strategies." Although the increased workload, restricted social life, physical problems, and knowledge and financial deficits were challenging for the family caregivers, self-strength and supportive social networks helped them to compassionately care for their stroke survivor.CONCLUSIONS: The phenomenon of family caregivers providing informal care for stroke survivors was explicated as compassionate care, notwithstanding numerous difficulties. The findings motivate further research and strategies to minimize family caregivers' burden and facilitate the positive aspects of caregiving to promote the health and well-being of both stroke survivors and their families.

Theoretically Based Factors Associated With Stroke Family Caregiver Health

Abstract:Purpose: Most persons who have had strokes are cared for at home by family members-many of whom experience depressive symptoms and quality of life changes as a result of providing care. The objective of this study is to determine theoretically based factors associated with unhealthy days in stroke family caregivers. Research design and methods: Secondary data analysis was conducted using baseline data from a large randomized controlled clinical trial testing the Telephone Assessment and Skill-Building Kit program with 254 family caregivers of persons who have had strokes. Guided by a conceptual model derived from Lazarus' transactional approach to stress, data were analyzed using multiple regression with unhealthy days as the dependent variable and theoretically based factors as independent variables. Results: Caregivers were mostly female (78%), White (71%), spouses (47%), or adult children (29%). Caregivers reported nine unhealthy days on average within the past month. A total of 37.8% of the variance in unhealthy days was explained by caregiver task difficulty, level of optimism, threat appraisal, depressive symptoms, and life changes with depressive symptoms being the strongest individual predictor because of shared variance. Clinical relevance: Unhealthy days is an important part of stroke family caregiver health. Factors associated with unhealthy days in this study provide areas to consider in future intervention development.
Nursing care for patients with rare diseases on the example of patients with Alternating Hemiplegia of Childhood

Abstract:Introduction: Rare diseases are those which occur in 1 person in 2000. Usually they are genetically conditioned. They also comprise diseases which appear as a result of metabolic, immunological defects, infections and uncommon cancers. Development of science, especially genetics prompted discovery of new gene’s mutations. One of them is a mutation of ATP1A3 which alternating hemiplegia of childhood. It is a disease whose manifestations appear in early childhood and are unpredictable. Strokes can be hemiplegic or tetraplegic. Sometimes they are accompanied by problems with swallowing and breathing. The aim of the thesis was to compare 2 different cases of patients suffering from AHC, to show problems connected with taking care of them in bio-psycho-socio-spiritual dimensions and to present a model of care on the example of Dorothey Orem’s theory. Material and methods: The comparative method used in the work compares two cases of patients. It is a quality work. Techniques of interview, observation, measurement and documentation analysis were also applied. An interview questionnaire containing 13 questions and an observation conducted in everyday circumstances made it possible to show care problems of patients with AHC. The research was carried out from March 2018 to May 2018. Two cases were juxtaposed: a 10-year-old girl and a 34-year-old man. Results and conclusions: Patients who suffer from rare diseases have restricted access to diagnosis, cure, rehabilitation and social assistance. They often grapple with lack of understanding on the part of society and medical staff who do not have enough knowledge about this issue. By comparing 2 cases it was proved that care problems of these patients are very complicated and vary depending on age, type of stroke, intensity, duration of the disease and family’s and society’s support. The care system of Dorothey Orem seems to be the best one.

Abstract:Background: Nursing care based on caring can improve the outcomes of post-stroke patients. However, many nurses tend to prioritize technical tasks over caring behaviors. The factors causing the lack of caring attitudes among nurses in providing care to stroke patients in hospitals are not yet known. Objective: To determine nurses' perceptions of caring-based nursing care for stroke patients with thrombolysis and the influencing factors. Methods: A quantitative research design using descriptive analysis with a cross-sectional approach was employed. Inclusion criteria included practicing nurses aged 20-50 with at least one year of work experience. Exclusion criteria were nurses who were sick, on leave, or engaged in educational assignments. Sample selection was done using proportional stratified random sampling over two weeks in September 2022. Results: A total of 158 respondents from three hospitals completed the questionnaire. Most nurses had good knowledge about assessment, nursing diagnosis, intervention, implementation, and evaluation (80.4%, 69.6%, 63.3%, 58.9%, and 69.6%, respectively). The ability to assess the NIHSS was also good (72.2%), but the ability to measure mRS was less satisfactory (52.5%). The results of the Coefficient of Determination (R2) test showed that the contribution of the factors of nurses, information, organization, and job to the nursing care of stroke patients was 50.3%. The direct hypothesis results also showed a significant influence of nurses, information, and job factors on caring-based nursing care for stroke patients (t-statistic value > 1.96 and p-value < 0.05). The better the nurse, information, and job factors, the more likely it is to improve nurses' caring attitudes (positive coefficient values). Conclusion: Nurses' knowledge about caring-based nursing care for stroke patients still needs improvement. Nurse, information, and job factors significantly influence caring-based nursing care for stroke patients with thrombolysis.

Abstract:Background Nurses' critical input and firsthand disaster management experience are vital to healthcare quality. Aim This study aims to comprehend nurses' experiences during and after the earthquakes, their coping mechanisms for posttraumatic stress disorder (PTSD), and the consequent effects on their profession, given the nurses' crucial role in disaster response. Methods A descriptive qualitative study using semistructured interviews was conducted between April and May 2024. Fifteen nurses were selected using criterion and snowball sampling methods from those working as earthquake survivor nurses in the affected areas. The COREQ guidelines for reporting qualitative research were followed. Findings Five main themes were: emotional reactions/physical difficulties, career and motivation, PTSD and well‐being, suggestions, and work performance. The results highlighted the significant psychological and professional challenges that nurses encounter, including the effects of severe PTSD symptoms, coping strategies, and the influence on their capacity to deliver care. Conclusions Nurses dealing with earthquakes face unique challenges that test their resilience on all levels. Learning from their experiences and coping mechanisms can help healthcare systems support these frontline workers better. Investing in disaster preparedness and mental health resources for nurses is crucial for their well‐being and the overall strength of the healthcare system during crises. Implications for nursing and health policy Effectively managing workloads and rest periods can reduce physical fatigue and prevent burnout. Shift rotations and adequate staffing levels are necessary. Access to psychological mental health support is vital for nurses to recover from the psychological effects of disasters. Regular disaster preparedness training is imperative for physical safety and psychological resilience. Robust support systems, including safe accommodation and peer support groups, are crucial for nurses in disaster response.

Ethical Dilemmas and Coping Experiences of Nurses Caring for Patients With Hematologic Cancer: A Phenomenological Study

Abstract:The study aimed to evaluate the ethical dilemmas faced by nurses who provide care to patients with hematologic cancer and determine their coping mechanisms using a phenomenological approach. This qualitative study was conducted with 35 nurses employed in clinics specializing in hematologic cancer patient care, living in diverse regions of Turkey, using semistructured, in-depth interviews through the WhatsApp mobile application. The snowball sampling technique was utilized to identify participants, and interviews continued until data saturation was achieved. All interviews were recorded and subsequently transcribed. Data analysis was conducted using Colaizzi's phenomenological analysis method, with the study adhering to the COREQ (Consolidated Criteria for Reporting Qualitative Research) checklist for reporting. In the analysis of the data, 2 categories (ethical dilemma situations and coping experiences) and 6 subthemes (treatment, care, professional values and beliefs, emotions, struggling with emotional burdens, and psychosocial approach) emerged. According to the results, nurses responsible for hematologic cancer patients struggle with ethical dilemmas in healthcare delivery and face challenges in effectively addressing them, which could potentially impact the quality of healthcare provided by nurses.

Abstract:Disease uncertainty widely exists among family caregivers of patients with moderate and severe craniocerebral injury. This negative emotional reaction will reduce the ability of family caregivers to make decisions during the critical stage of the patient, causing serious effects on the rescue and prognosis of patients with moderate and severe craniocerebral injury. Therefore, this article aims to understand the state of the uncertainty of the disease of family caregivers of patients with moderate and severe craniocerebral injury in China, to analyze the influencing factors, and to explore the specific resource of the uncertainty of the disease combined with qualitative study. The outcomes will provide a theoretical basis for formulating an accurate clinical nursing intervention strategy.

Abstract:Introduction: Stroke occurs suddenly causing significant residual impairment for stroke survivors requiring long-term assistance by family caregivers. Becoming a caregiver can be an overwhelming, difficult process and stroke caregivers often experience poorer health and higher mortality rates than non-caregivers. Methods: In this grounded theory study, we explored stroke caregivers’ needs and assessed their capacity and readiness to assume the caregiving role as the stroke survivor transitioned home from inpatient rehabilitation. Our sample included a comparison group (9 patient/family units) and an intervention group (10 patient/family units) for a total of 19 stroke survivors and 22 family caregivers. Participants were interviewed during inpatient rehabilitation, and at 1 and 3 months post-discharge. In the first interview, comparison group questions focused on understanding family members’ perceptions about how prepared they were to assume the caregiving role. In the intervention group we conducted 1 hour interviews to assess specific domains of caregiver capacity and readiness to assume the role and reported the findings to the stroke rehabilitation team prior to discharge. In the 2 post-discharge interviews in both groups, we explored caregivers’ perceptions about how prepared they were to manage the stroke survivors’ post-discharge needs. Interviews were analyzed using dimensional analysis and coded in NVivo data management software. Findings: Family members in both groups indicated they were not “ready” to assume the caregiving role and there was not enough time for them to prepare pre-discharge. Dimensions of readiness included having the commitment, resources, and ability to sustain the role. Pre-existing conditions, such as physical and mental health concerns, other roles/responsibilities, and home inaccessibility limited their caregiving capacity. Conclusions: In order to adequately prepare family caregivers, we must assess their caregiving capacity pre-discharge and develop plans to address gaps so that they will be ready to assume the role long-term. Rehabilitation lengths of stay are often insufficient to address these gaps requiring interventions to help caregivers bridge the gap as they transition home.
