Abstract:Background: Previous studies addressing the question of whether continuous positive airway pressure (CPAP) could improve the insulin resistance and glucose control in patients with obstructive sleep apnea (OSA) have led to conflicting results. Therefore, we conducted the meta-analysis to evaluate the effects of CPAP on glycemic control and insulin resistance in OSA patients. Methods: We searched PubMed, HighWire Press, Ovid Medline (R), Cochrane library, and EMBASE before December 2011 on original English language studies. The meta-analysis was conducted using Review Manager Version 5. Results: The summary estimate for mean difference of homeostasis model assessment insulin resistance (HOMA) from 12 non-diabetic studies was -0.55 (95 % CI, -0.91 to -0.20; P = 0.002). When compared with fasting blood glucose at baseline, 3 to 24 weeks of CPAP treatment did not improve glycemic control in non-diabetic subjects (-0.12; 95 % CI, -0.3 to 0.06; P = 0.20), as well as in diabetic subjects (-0.71; 95 % CI, -2.24 to 0.83; P = 0.37). There were no intervention-related changes in body mass index. Conclusions: Our analysis showed that CPAP significantly improved insulin resistance in non-diabetic patients with moderate to severe OSA, while no significant change in body mass index was detected. Compared with fasting blood glucose at baseline, there was no change in glycemic control with CPAP. Further large-scale, randomized, and controlled studies are needed to evaluate the longer treatment and its possible effects on weight loss and glycemic homeostasis.

Abstract:Abstract Purpose This study aimed to investigate the effect of continuous positive airway pressure (CPAP) on blood glucose fluctuation in patients with type 2 diabetes mellitus (T2DM) and obstructive sleep apnea (OSA). Methods Patients with T2DM and OSA were divided into an intervention group and a control group. All patients were treatment naïve. The intervention group was given CPAP therapy. The subjects were monitored using a continuous glucose monitoring system (CGMS) for 2 weeks. Results Of 60 patients, 30 were selected to receive CPAP intervention while 30 without CPAP served as controls. The CPAP tolerance of the intervention group was good, with average time on CPAP therapy of 55.2 ± 4.3 days, and average daily time on CPAP therapy of 8.3 ± 2.8 h. The postprandial blood glucose (PBG), fasting blood glucose (FBG), and HbA1c levels in the intervention group decreased significantly ( P < 0.05). Significant variations in 24-h mean blood glucose and night-time mean blood glucose were significantly lower with CPAP therapy than without therapy ( P < 0.05, respectively). The mean of daily differences and mean ambulatory glucose excursions were both considerably lower with treatment than without ( P < 0.05, respectively). There was also a significant difference in time in range and time above range ( P < 0.05, respectively). Conclusion CPAP treatment may significantly improve the blood glucose level and blood glucose stability in patients with T2DM and OSA. CPAP is an effective treatment method beyond lifestyle intervention and drug therapy.

Abstract:Study objectives: Obstructive sleep apnea (OSA) is characterized by disordered breathing during sleep and is associated with major cardiovascular complications. Glucagon-like peptide 1 receptor agonists (GLP-1RA) as an important treatment for obesity and diabetes mellitus show promising therapeutic prospect in OSA. We conducted a meta-analysis to evaluate the effect of GLP-1RA intervention in OSA individuals. Methods: We searched the PubMed and Web of Science databases (published until July 1, 2024), The included studies evaluated the GLP-1RA in OSA individuals and the efficacy outcomes measured by apnea-hypopnea index (AHI). Results: Six studies with a total of 1067 participants enrolled. GLP-1RA significantly decreased AHI with an estimated treatment difference of-9.48 events per hour (95% CI, -12.56 to -6.40, I2=92%). The change in weight was -10.99kg and BMI was -1.60kg/m2. The mean difference in SBP was -4.81mmHg and in DBP was -0.32 mmHg. Tirzepatide significantly reduced AHI more than liraglutide with an estimated treatment difference of -21.86 events per hour (95% CI, -25.93 to-17.79) vs -5.10 events per hour (95% CI, -6.95 to-3.26). Obese individuals experienced a more significant decrease in AHI with an estimated treatment difference of-12.93 events per hour vs -4.31 events per hour. The application of CPAP and the duration of follow-up did not affect the therapeutic effect. Conclusion: GLP-1RA could significantly reduce the severity of OSA, and also lead to weight loss and lower blood pressure. Further high-quality RCTs are needed to explore different GLP-1RA treatments and durations in OSA, and identify patient subgroups that may benefit the most.

Abstract:Background: Evidence suggests that 15–30% of individuals with obstructive sleep apnoea (OSA) have type 2 diabetes mellitus (T2DM), and that OSA is an independent risk factor for T2DM. There is considerable interest in ascertaining whether OSA treatment improves glycaemic control and insulin sensitivity in patients with OSA and T2DM. Aims: To assess the effects of continuous positive airway pressure (CPAP) therapy on glycosylated haemoglobin (HbA1c) level, insulin sensitivity and body mass index (BMI) in patients with OSA and T2DM. Methods: MEDLINE, EMBASE and the Cochrane Library were searched to identify prospective studies involving patients with OSA and T2DM who had received CPAP, and data on primary outcome (change in HbA1c) and/or secondary outcomes (changes in insulin sensitivity and BMI) were reported. All relevant studies published before 31 January 2014 were included. Results: Six studies were included in the systematic review and meta-analysis. The numbers of patients ranged from 9 to 44 (total=128), and mean age ranged from 50.7 to 66.1 years. For the change in HbA1c (six studies, 128 patients), the combined standardised paired difference revealed no significant effect of CPAP (−0.071, 95% confidence interval (CI)=−0.245, 0.103; P =0.421). Similarly, there was no significant effect of CPAP on the change in BMI (−0.102, 95% CI=−0.296, 0.092; P =0.302; five studies, 103 patients). In contrast, there was a significant effect of CPAP on the change (improvement) in insulin sensitivity (0.330, 95% CI=0.001, 0.658; P =0.049; three studies, 39 patients). Conclusion: The limited available evidence from randomised controlled trials and prospective observational studies suggests that CPAP does not decrease HbA1c level or BMI in patients with OSA and T2DM but may improve insulin sensitivity.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) is a common condition being increasingly recognized and is a known risk factor for cardiovascular and metabolic disease. Impaired sleep architecture and (or) frequent nocturnal hypoxia are characteristic features in patients with OSA. However, little is known about the sleep parameters linking OSA with glucose metabolism. The objective of this study was to evaluate the association between glycated hemoglobin (HbA1c) and sleep traits from polysomnography and subjective sleep diary in a Chinese population with proven OSA (Huashan OSA Registry Study, Hs-OSARS). Methods 535 participants were recruited in Huashan OSA Registry Study, from January 2018 to August 2021. Objective sleep characteristics were measured via one-night laboratory PSG and biochemical parameters was tested the next morning. Associations were conducted using Pearson’s correlation with adjustments for age, gender, and body mass index (BMI). Regression was used to investigate predictive power of sleep subscales for glucose levels. Results Among the participants, 133 subjects underwent HbA1c and lipid profile (triglyceride, total cholesterol, LDL- and HDL-cholesterol, etc). Age, BMI, ApoE, TC and non-HDL-C (rage=0.281, p=0.001; rBMI =0.249, p=0.004; rApoE=0.250, p=0.004; rTC=0.264, p=0.005; rnon-HDL-C=0.281, p=0.002) had positive associations with HbA1c. Among sleep traits, only stage 3 sleep had a negative association with HbA1c (r=-0.219, p=0.011). After controlling for gender, age and BMI, these associations remained and were statistically significant. Stepwise regression analysis found that non-HDL-C, stage 3 sleep and age were significant predictors retained in the model. Upon fixing the covariates (including gender, age and BMI), TC and stage 3 sleep were significant for predicting HbA1c outcomes. Conclusion Insufficient slow wave sleep is associated with higher blood glucose levels. Our study shows that other PSG indices, such as total sleep time, sleep efficiency, arousal index, apnea-hypopnea index and hypoxemic burden, are not associated with HbA1c. These findings highlight the importance for clinicians to evaluate and improve the slow wave sleep as part of preventing glucose metabolism impairment in OSA patients. Support (if any)

Abstract:Objective To study the influence of obstructive sleep apnea syndrome (OSAS)to the development of abnormal glucose metabolism and insulin insistence. Methods Fifty - eight OSAHS patient and 57 normal volunteers were measured by polysomnograpby during sleep and 75 g oral glucose tolerance test, then compared the homeostasis model assessment(HOMA) index and abnormal glucose metabolism frequency. Results ①HOMA - IR of OSAHS group were significantly higher than that in the control group( P < 0. 05 ), HOMA index were positively correlated with AHI and negatively correlated with the nocturnal lowest SpO2 respectively with statistical significance.②The frequency of abnormal glucose metabolism in OSAHS group were significantly higher than that in the control group( P <0. 01 ).Conclusions The extent of respiratory disturbance is closely related with insulin resistance and abnormal glucose metabolism. Insulin resistance inclined and the frequency of abnormal glucose metabolismn more higher accompany with the respiratory disturbance increased in OSAHS.
Dynamic changes in nocturnal blood glucose levels are associated with sleep-related features in patients with obstructive sleep apnea

Abstract:Abstract Obstructive sleep apnea (OSA) has a bidirectional relationship with insulin resistance conditions; however, the mechanism remains unclear. This study aimed to compare dynamic nocturnal glucose changes among patients with OSA of varying levels of severity and evaluate temporal changes associated with the cardinal features of OSA (sympathetic hyperactivation, intermittent hypoxemia, and sleep fragmentation) in nondiabetic subjects. Nocturnal glucose was measured with a continuous glucose monitoring device every 5 min during polysomnography (PSG). The OSA features were evaluated using heart rate variability (HRV), minimum saturation, and electroencephalography. Eleven subjects with moderate to severe OSA and 12 subjects with no or mild OSA were evaluated. Those with moderate to severe OSA showed an increasing trend in blood glucose levels after sleep onset, whereas those without or with mild OSA showed a decreasing trend (F = 8.933, p < 0.001). Delta band power also showed different trends during sleep between the two groups (F = 2.991, p = 0.009), and minimum saturation remained lower in the moderate to severe OSA group than in the no or mild OSA group. High degrees of coupling between nocturnal glucose levels and each OSA feature were observed. Altered trends in nocturnal glucose in moderate to severe OSA may reflect glucose intolerance and result in metabolic consequences. Managing the features of sleep-related OSA may have implications for metabolic management in the future.

Abstract:Objective Analysis on glucose metabolism changes in patients with obstructive sleep apnea syndrome (OSAS) and normal glucose tolerance (NGT). Methods Thirty male patients with OSAS and NGT, matched with body mass index (BMI) were selected. The fasting blood glucose, fasting insulin, HOMA2-β cell function, HOMA2-insulin resistance index, HOMA2-insulin sensitivity index were measured and analyzed. Results Compared with control group, the AHI＞15 times/h group observes a significantly higher AHI( P ＜0.05), lower lowest SpO2 ( P ＜0.05), higher fasting insulin ( P ＜0.05), lower HOMA-insulin sensitivity index ( P ＜0.05), and higher HOMA-insulin resistance index ( P ＜0.05). BMI, age, waist to hip ratio,neck, FBG and HOMA2-β cell function were similar in both group( P ＞0.05). Conclusions The data showed that the patients with OSAS and NGT exhibit hyper insulinemia, but not a deficit with pancraetic β cell.

Abstract:Abstract Introduction In patients with Type 2 diabetes (T2D), obstructive sleep apnea (OSA) is associated with worse cardiometabolic risk factors. Whether the same occur in Type 1 Diabetes (T1DM) has not been well established. Methods Retrospective cross-sectional study, we assessed T1D patients from our endocrinology clinic between 2015-2022, who had a sleep study done in the Sleep Center. We compared the groups for diabetes duration, glycemic control, complications, and metabolic profile. An unpaired t-test was performed for comparison of means, chi-square test for nominal data, and a p-value of P< 0.05 was considered statistically significant. Results We identified 55 patients with T1D and a sleep study, 33 had OSA based on sleep report and 22 did not. Both groups had similar diabetes duration, BMI, micro and macrovascular complications. Hypertension and dyslipidemia were more frequent in the OSA group (P< 0.05). A1C values were similar between OSA and non-OSA groups (7.5% vs 7.9%). Total daily insulin dose per kilogram was similar too (0.42 vs 0.30). Of the 14 patients with a follow up visit after initiation of PAP therapy, A1c levels remained unchanged before and 3-6 months after PAP initiation (7.7% vs. 7.0%, P=0.25), however, 57% of them improved their A1c level, and only 21% worsened. The OSA group had higher triglycerides (mean 142mg/dl vs 81.7mg/dl, P=0.003) and lower HDL (mean 44.5mg/dl vs 62.7mg/dl, P=0.06) despite similar A1c levels. In terms of liver fibrosis, an FIB-4 score < 1.3 was considered low risk based on ADA guidelines, and we found 14 out of 33 patients in the OSA group (42%) and 4 out of 22 patients in the non-OSA group (18%) had >1.3, (P=0.06). Conclusion In patients with T1DM, OSA is associated with metabolic features consistent with insulin resistance: trends towards higher BMI, insulin needs and worse lipid profile, despite similar A1c levels. The glycemic impact of OSA treatment in T1DM requires further investigation. Abstract:The obstructive sleep apnea syndrome and diabetes are common diseases. Epidemiologic and experimental evidence linking sleep apnea and disorders of glucose metabolism. Although the cause-and-effect relationship remains to be determined, the available data suggest that sleep apnea is independently associated with altered glucose metabolism and may predispose to the eventual development of type 2 diabetes mellitus. Abstract:Obstructive sleep apnea (OSA) is a prevalent condition associated with increased cardiovascular risk, particularly in individuals with comorbid obesity and type 2 diabetes (T2D). Despite the widespread use of continuous positive airway pressure (CPAP) for OSA management, adherence remains suboptimal, and CPAP has not consistently demonstrated reductions in surrogate cardiovascular events. Recently, attention has focused on glucagon-like peptide-1 receptor agonists (GLP-1RAs) and sodium-glucose cotransporter-2 (SGLT2) inhibitors as potential therapeutic agents for mitigating cardiovascular risk in OSA patients. These agents, originally developed for T2D management, have demonstrated pleiotropic effects, including significant weight loss, blood pressure reduction, and amelioration of endothelial dysfunction and arterial stiffness, along with anti-inflammatory benefits, which may be particularly beneficial in OSA. Emerging clinical evidence suggests that GLP-1RAs and SGLT2 inhibitors can reduce OSA severity and improve daytime sleepiness, potentially reversing the adverse cardiovascular effects observed in OSA. This review explores the pathophysiological mechanisms linking OSA with cardiovascular disease and evaluates the potential therapeutic roles of GLP-1RAs and SGLT2 inhibitors in addressing cardiovascular risk in OSA patients. Further research, including long-term clinical trials, is necessary to establish the effectiveness of these therapies in reducing cardiovascular events and improving patients' reported outcomes in this population.

Abstract:Aims/Introduction To estimate the prevalence, and patient clinical and demographic profile, as well as risk factors associated with obstructive sleep apnea syndrome (OSAS) in hospitalized patients with type 2 diabetes mellitus in Beijing, China. Materials and Methods Hospitalized adult patients with type 2 diabetes mellitus were consecutively screened and invited for an overnight polysomnography from four hospitals in Beijing, China, from May 2016 to February 2017. We used the American Academy of Sleep Medicine 2012 polysomnography recording techniques and scoring criteria to identify the type of apnea and the severity of OSAS. The chi(2)-test was used to evaluate differences between groups regarding the prevalence, and demographic and other clinical parameters. Results A total of 735 patients were found eligible for the study, of whom 309 patients completed the overnight polysomnography. The mean age of the patients was 58.2 +/- 10.9 years, and most (67.3%) were men. The prevalence of overall (apnea hypopnea index >= 5/h), moderate-to-severe (apnea hypopnea index >= 15/h) and severe (apnea hypopnea index >= 30/h) OSAS was 66.3% (95% confidence interval 60.8-71.6%), 35.6% (95% confidence interval 30.3-41.2%) and 16.5% (95% confidence interval 12.5-21.1%), respectively. Central and mixed apnea contributed 12% to all sleep-disordered breathing. With the aggravation of OSAS, the combined prevalence for central, mixed and obstructive apnea increased from 57% to 70%. We found OSAS to be associated with older age, obesity, self-reported snoring and apnea, and diabetes complications. Conclusions Guidelines on screening and treatment of OSAS among hospitalized patients with diabetes are needed to direct the routine practice for diabetes endocrinologists for optimal clinical care of such patients.

Abstract:OBJECTIVE:To assess the effect of short-term continuous positive airway pressure (CPAP) upon 24 h glucose control via a continuous glucose monitoring system (CGMS) in patients with obstructive sleep apnea hypopnea syndrome and type 2 diabetes (OWD). METHODS:Eleven cases of hospitalized OWD with age 43 - 70 (56 +/- 10) years old, body mass index 22.3 - 38.3 (28.5 +/- 5.5) kg/m(2) and apnea hypopnea index (AHI) 12 - 68 (45 +/- 23) times/h was recruited. CGMS was applied 2 days before and 4 days during CPAP treatment. The 24 h, treatment hours (6 h, 0:00-6:00) and non- treatment hours (18 h) glucose level and glucose variability were analyzed. Insulin resistance was assessed with fasting plasma blood glucose (FBG), plasma insulin (FINS) and homeostatic model assessment of insulin resistance index (HOMA-IR). RESULTS:Short-term CPAP treatment corrected sleep disordered breathing, and induced significant decreases of 24 h and treatment hours blood glucose level [(7.97 +/- 1.31) vs (7.52 +/- 0.94) mmol/L, (7.24 +/- 1.51) vs (6.77 +/- 1.65) mmol/L, both P < 0.05). Glucose variability of 24 h and treatment hours and non-treatment hours significantly decreased after CPAP treatment (1.22 +/- 0.34 vs 0.89 +/- 0.28, 0.43 +/- 0.24 vs 0.31 +/- 0.18, 1.23 +/- 0.89 vs 0.49 +/- 0.26, all P < 0.05). Short-term treatment also induced increase of insulin sensitivity, as indicated by a significant decrease of HOMA-IR (3.65 +/- 1.93 vs 2.79 +/- 1.68, P < 0.05). CONCLUSION:Short-term CPAP treatment in OWD may have an improving effect not only upon insulin resistance but also upon whole-day blood glucose and glucose variability.

Abstract:Certain studies have indicated a link between obstructive sleep apnea and insulin resistance in specific populations. To gain more clarity, extensive research involving a broad sample of the overall population is essential. Association of Diabetic Microvascular Complications and Parameters of Obstructive Sleep Apnea in Patients with Type 2 Diabetes

Abstract:Background: Obstructive sleep apnea (OSA) is prevalent in patients with type 2 diabetes, but the influence of OSA on diabetes complications is not clear. We aimed to investigate the association of OSA with chronic diabetes complications in Chinese patients with type 2 diabetes.Subjects and Methods: In total, 880 hospitalized patients were enrolled in a multicenter, cross-sectional study that involved 12 hospitals from six cities in the People's Republic of China. Overnight sleep monitoring with a portable monitor was used to record respiratory parameters, including the apnea-hypopnea index (AHI), the oxygen desaturation index (ODI), the oxygen saturation (SPO2), and the cumulative time of SPO2 below 90% or 85% (CT90% and CT85%, respectively). Chronic diabetes complications were recorded from medical charts.Results: CT90% was independently associated with diabetic nephropathy (DN) after adjusting for age, sex, diabetes duration, glycosylated hemoglobin, body mass index, hypertension, and the use of angiotensin converting enzyme inhibitor/angiotensin receptor blocker drugs within 1 week. The associated parameters increased from two (the average SPO2 and CT90%) to three (ODI, the lowest SPO2, and CT85%) when the severity of DN increased from microalbuminuria to renal insufficiency. The estimated glomerular filtration rate was independently correlated with ODI (beta = -0.172, P = 0.029) and the lowest SPO2 (beta = 0.354, P = 0.004) after adjustments. The lowest SPO2 was associated with proliferative diabetic retinopathy by univariate logistic regression but was not significant in multivariate regression after adjustment.Conclusions: Parameters of nocturnal hypoxemia are associated with DN and renal function of patients with type 2 diabetes. The parameters of hypoxemia may more sensitively reflect the association of OSA and diabetic microvascular complications than AHI.

Abstract:Objective To systematically review the evidence for the link between obstructive sleep aponea syndrome(OSAS),glucose intolerance,and insulin resistance(IR).Methods Searches of MEDLINE,OVID,ELSEVIER,CNKI and WANFANG data were performed for OSAS and IR.Results Sixty-one articles met the inclusion criteria(40 with English and 21 with Chinese).25 of 35 studies which have used objective measures of OSAS provide support for an independent link between OSAS,glucose intolerance,and IR.14 of 26 studies that used continuous positive airway pressure(CPAP)to treat IR indicated that CPAP was effective,but most of them did not have control group.Conclusions OSAS is independently associated with glucose intolerance and IR.However,randomized controlled studies of large sample are needed to identified the improvement of metabolic disturbance after treatment.

Materials and Methods: The cross-sectional analysis included 422 subjects (261 males/161 females) with the mean age of 27.77 ± 7.51 years and average body mass index (BMI) of 34.84 ± 5.69 kg/m 2 . All subjects underwent polysomnography (PSG), oral glucose tolerance-insulin releasing test (OGTT-IRT) and serum glycosylated hemoglobin A1 (HbA1c) measurement. The glucose metabolism and pancreatic β-cell function in relation to measures of OSA were determined adjustment for important confounders such as age and sex. Results: OSA subjects accounted for 54.91% in the normal glucose tolerance (NGT) group and 72.11% in the prediabetes (preDM) group ( P =0.001). HbA1c was the highest in the preDM subjects with severe OSA. In the NGT subjects, the 1-h glucose level significantly elevated with the OSA severity, and the homeostasis model assessment-β (HOMA-β) was negatively related to nocturnal mean SpO 2 ( P < 0.05). In the preDM subjects, HOMA-β, early phase insulinogenic index (∆I 30 /∆G 30 ), total area under the curve of insulin in 180 min (AUC-I 180 ), and the oral disposition index (DI O ) were the lowest in the severe OSA group. DI O was associated with higher oxygen desaturation index (ODI) and lower nocturnal mean SpO 2 , and AUC-I 180 was negatively related to TS90 ( P < 0.05). Conclusion: Our study indicated higher prevalence of OSA in overweight and obese nondiabetic young adults, especially preDM subjects. The impaired glucose tolerance was observed early after glucose intake in the NGT subjects. OSA induces compensatory increase in the pancreatic β-cell function in the NGT subjects, while pancreatic β-cell dysfunction is present in the preDM subjects with severe OSA. Abstract:STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is associated with an increased risk of diabetes-related complications. Hence, it is plausible that Continuous Positive Airway Pressure (CPAP) could have a favorable impact on these complications. To assess the feasibility of conducting a randomized control trial (RCT) in patients with type 2 diabetes (T2D) and OSA over 2 years.METHODS: An open-label multicenter feasibility RCT of CPAP vs no CPAP in patients with T2D and OSA. Patients with resting oxygen saturation <90%, central apnea index >15/hour or Epworth Sleepiness Scale (ESS) ≥11 were excluded. OSA was diagnosed using a multichannel portable device (ApneaLink Air, ResMed). The primary outcome measures were related to feasibility, and the secondary outcomes were changes in various clinical and biochemical parameters related to diabetes outcomes.RESULTS: Eighty-three (40 CPAP vs 43 no CPAP) patients were randomized, with a median (IQR) follow-up of 645 [545, 861] days. CPAP compliance was inadequate, with a median usage of approximately 3.5 hours/night. Early CPAP use predicted longer-term compliance. The adjusted analysis showed a possible favorable association between being randomized to CPAP and several diabetes-related endpoints (chronic kidney disease (CKD), neuropathy, and quality of life (QoL)).CONCLUSIONS: It was feasible to recruit, randomize, and achieve a high follow-up rate over 2 years in patients with OSA and T2D. CPAP compliance might improve by a run-in period before randomization. Abstract:Background: Obstructive Sleep Apnea (OSA) is a prevalent disorder characterized by repetitive upper airway obstructions during sleep, leading to intermittent hypoxia and sleep fragmentation. Current treatments, particularly Continuous Positive Airway Pressure (CPAP), face adherence challenges, necessitating novel therapeutic approaches. Methods: This review explores the potential of Glucagon-like Peptide-1 receptor agonists (GLP-1RA), commonly used for type 2 diabetes and obesity, in managing OSA. GLP-1RA promotes weight loss, enhances insulin sensitivity, and exhibits anti-inflammatory and neuroprotective properties, potentially addressing key pathophysiological aspects of OSA. Results: Emerging evidence suggests that these agents may reduce OSA severity by decreasing upper airway fat deposition and improving respiratory control. Clinical trials have demonstrated significant reductions in the Apnea-Hypopnea Index (AHI) and improvements in sleep quality with GLP-1 therapy. Conclusions: Future research should focus on elucidating the mechanisms underlying GLP-1 effects on OSAS, optimizing combination therapies, and identifying patient subgroups that may benefit the most. Integrating GLP-1RA into OSAS management could revolutionize treatment by addressing both the metabolic and respiratory components of the disorder, ultimately enhancing patient outcomes.

Abstract:OBJECTIVE:To assess the effects of short-term continuous positive airway pressure (CPAP) upon un-treatment hour glucose control via a continuous glucose monitoring system (CGMS) in patients with obstructive sleep apnea hypopnea syndrome and type 2 diabetes (OWD).METHODS:A total of 23 case of hospitalized OWD were recruited. CGMS was applied for 2 days before and 4 days during CPAP treatment. The treatment hour (6 h, 0:00 - 6:00) and un-treatment hour (14 h, 6:00 - 22:00) glucose level and glucose variability were analyzed. Insulin resistance was assessed with fasting plasma blood glucose (FPG), plasma insulin (FINS) and homeostatic model assessment of insulin resistance index (HOMA-IR).RESULTS:The short-term CPAP treatment corrected sleep-disordered breathing and induced significant decreases of treatment and un-treatment hour glucose level ((7.07 ± 2.02) mmol/L vs (6.34 ± 1.57) mmol/L, (8.04 ± 1.99) mmol/L vs (7.64 ± 1.81) mmol/L, both P < 0.05). Glucose variability of treatment and un-treatment hours significantly decreased after CPAP treatment (0.41 ± 0.24 vs 0.29 ± 0.18, 1.02 ± 0.50 vs 0.78 ± 0.45, all P < 0.05). Short-term treatment also induced an increase of insulin sensitivity, as indicated by a significant decrease of HOMA-IR (4.02 ± 2.07 vs 3.08 ± 1.58, P < 0.05).CONCLUSION:Short-term CPAP treatment in OWD may improve not only insulin resistance but also blood glucose and glucose variability during treatment and un-treatment hours.
