Abstract:Abstract Background Loneliness is more common in older adults and those who face structural vulnerabilities, including homelessness. The homeless population is aging in the United States; now, 48% of single homeless adults are 50 and older. We know little about loneliness among older adults who have experienced homelessness. We aimed to describe the loneliness experience among homeless-experienced older adults with cognitive and functional impairments and the individual, social, and structural conditions that shaped these loneliness experiences. Methods We purposively sampled 22 older adults from the HOPE HOME study, a longitudinal cohort study among adults aged 50 years or older experiencing homelessness in Oakland, California. We conducted in-depth interviews about participants perceived social support and social isolation. We conducted qualitative content analysis. Results Twenty participants discussed loneliness experience, who had a median age of 57 and were mostly Black (80%) and men (65%). We developed a typology of participants’ loneliness experience and explored the individual, social, and structural conditions under which each loneliness experience occurred. We categorized the loneliness experience into four groups: (1) “lonely– distressed”, characterized by physical impairment and severe isolation; (2) “lonely– rather be isolated”, reflecting deliberate social isolation as a result of trauma, marginalization and aging-related resignation; (3) “lonely– transient”, as a result of aging, acceptance and grieving; and (4) “not lonely”– characterized by stability and connection despite having experienced homelessness. Conclusions Loneliness is a complex and heterogenous social phenomenon, with homeless-experienced older adults with cognitive or functional impairments exhibiting diverse loneliness experiences based on their individual life circumstances and needs. While the most distressing loneliness experience occurred among those with physical impairment and mobility challenges, social and structural factors such as interpersonal and structural violence during homelessness shaped these experiences.

Abstract:Abstract Solitude, defined as physical aloneness or the absence of social interactions, has been associated with both positive and negative outcomes. Older adults spend a significant amount of their waking time by themselves (exacerbated during the pandemic), but they may be better equipped to regulate the negative emotions that are often linked with being alone. Additionally, solitude may provide a balance between social connectedness and autonomous needs. Using pandemic, end-of-day daily diary data across 10 days from 136 older Canadian adults and a close other of their choice (59% spouses, M = 66.49 years, SD = 13.26 range: 18-87 years, 88% White, 62% women), this project aims to examine associations of actor and partner solitude with relationship quality and daily affect. Multilevel models revealed that participants who reported more voluntary solitude experienced more daily positive affect and those who reported more negative solitude experienced both more negative affect and less positive affect at both daily and overall levels. We also found daily partner effects such that more voluntary solitude of the partner was associated with less actor negative affect and negative solitude of the partner being associated with less actor positive affect and more negative affect over and above actor effects. Finally, when partners reported more voluntary solitude, participants reported higher relationship support. Findings shed light on the benefits and drawbacks of how solitude is experienced in older adulthood, highlighting the influence social partners affect quality and relationship satisfaction.
Loneliness in nursing homes: A qualitative meta-synthesis of older people's experiences

Abstract:Background: Older people in the nursing home environment are much less mobile and capable of taking care of themselves as they age, and most of them face the plight of loneliness, which seriously affects the quality of life of older people in their later years. Aims: A systematic review and synthesis of older people's experiences of loneliness in nursing homes. Design: Following ENTREQ, do a systematic evaluation and synthesis of qualitative investigations. Methods: A search of PubMed, Cochrane Library, Web of Science, Embase, the Chinese biomedical literature service system, the China National Knowledge Infrastructure, the Wanfang Database and the Wipu Database for qualitative studies of older people's experiences of loneliness in nursing homes was conducted with a search time frame of March 2023. Evaluation of the quality of the literature using the Joanna Briggs Institute's Australian Centre for Evidence-Based Health Care Quality Assessment Criteria for Qualitative Research, And the data were synthesised using Thomas and Harden's method of thematic and content analysis. Results: A total of 13 papers were included, and 36 research findings were distilled and integrated into three themes: causes of loneliness; feelings of loneliness; coping with loneliness; and seven sub-themes: aging and loss; environmental transformation; loneliness is a pain; loneliness is a choice; participation; strengthening social ties; and diverting attention. Conclusions: Older people in nursing homes face varying degrees of loneliness, which is a subjective feeling influenced by the interplay between personal awareness and the external environment, so future care interventions should be developed in a comprehensive manner, taking into account the characteristics of the older people themselves and their external environment. Abstract:Abstract Loneliness has become a big issue in the time of the COVID pandemic. The attention to the topic also has to do with the increase in people living alone in Europe, although this also has to do with prosperity. Living alone does not yet mean being lonely. In the scientific discussion and especially in the measures, a differentiation must be made between loneliness, solitude and social disintegration. Poor social integration is easily measurable and has a lot to do with the physical health (mobility, vision and hearing) of the people concerned. However, the extent to which participation in social and cultural opportunities is possible, for example through the expansion of public transport, also plays an important role. Loneliness, on the other hand, is by definition subjective and strongly linked to mental health. It describes the subjective suffering of missing or unsatisfactory social relationships, lack of integration and security. Loneliness is often found in two peaks, among the young and the old. Political and public health campaigns often focus on social integration measures. However, measures to combat loneliness mean first and foremost recognising mental illness in old age, especially depression. Abstract:Background and objectives: While research on loneliness in later life has increased during the coronavirus disease 2019 (COVID-19) pandemic, we still lack diverse qualitative approaches capturing lived experiences of lonely older adults. Approaches to research with older adults instead of on them are also scant in gerontological literature on loneliness. Through solicited diaries, this study aimed to explore how older Australians who were already lonely before the pandemic managed lockdowns (stay-at-home orders) in Victoria, Australia, which lived through one of the longest lockdowns in the world. Research design and methods: This article is based on qualitative diaries completed by 32 older adults (aged 65 and older). Diaries provide a 'live' document where participants become active research partners, recording and sharing their perceptions and experiences. This method is useful to capture sensitive issues, but to the best of our knowledge has not been employed in loneliness studies. Data were thematically analyzed through the identification of themes within and across diaries. Results: Loneliness was defined by participants as a detrimental absence of companionship and meaningful social interactions. For most, lockdowns exacerbated loneliness, presented new triggers, and upended coping strategies. The disruption of the 'emotion work' involved with managing loneliness led to a reconfiguration of response strategies, including through digital technology, which brought both challenges (e.g., digital ageism) and opportunities (e.g., novel communication forms). Discussion and implications: Understanding how older people living alone define and respond to loneliness in diverse contexts-for example, before and during a pandemic that restricts social interaction-provides critical insights to inform interventions to tackle loneliness.

Abstract:Abstract The number of older adults identifying themselves as lonely has increased during the last two years as they have struggled with the impact of the pandemic on their perceptions of relationships with others. Recent research has shown that the negative health implications of loneliness are similar to those found with smoking or obesity. Most current interventions continue with past practice by adding people to engage with the older adult, although research suggests that while this intervention addresses social isolation, it does not address loneliness directly. Information regarding the development and implementation of a loneliness intervention with older adults based on the Health Beliefs Model in one rural mid-west county will be presented. Participants were recipients of home-delivered meals and had volunteered for a project that provided peer-to-peer instruction to improve digital literacy. In addition to technology training sessions, participants received a weekly contact from a separate volunteer as a friendly ‘check-in.’ Beginning in the fourth week, brief 5-10 minute coaching sessions about loneliness were included in these weekly contacts. The presentation will include details of the loneliness content, and describe training and support provided to the volunteers. Interviews were conducted with participants at baseline, approximately three months, and at the end of contact with each participant. Results showed no statistically significant change in loneliness, but participants reported they saw potential usefulness for the content with family and friends and potentially themselves in the future. They also provided helpful suggestions for revision. Plans for revision will be presented.

Abstract:<span class="a-plus-plus abstract-section id-a-sec1"><h3 class="a-plus-plus">Purpose</h3><p class="a-plus-plus">Loneliness is increasingly recognised as the next critical public health issue. A plausible reason for this concern may be related to emerging societal trends affecting the way we relate, communicate, and function in our social environment. In 2006, a prominent review of the clinical significance of loneliness was published. However, there has not been a comprehensive update on known and emerging risk factors and correlates of loneliness since then. Furthermore, there is no conceptual model that has been developed to better account for the complexity of loneliness and to inform the development of evidence-based solutions as we challenge the issues of the twenty-first century.</p></span><span class="a-plus-plus abstract-section id-a-sec2"><h3 class="a-plus-plus">Methods</h3><p class="a-plus-plus">We reviewed the current literature to identify either known or emerging risk factors and correlates of loneliness since 2006. This includes new or known evidence on: (1) demography; (2) health, including physical health; mental health; cognitive health; brain, biology, and genetics; and (3) socio-environmental factors including digital communication and the workplace.</p></span><span class="a-plus-plus abstract-section id-a-sec3"><h3 class="a-plus-plus">Results</h3><p class="a-plus-plus">We synthesized the literature according to a new proposed conceptual model of loneliness which showed the interplay between known and emerging correlates and risk factors from demography, health, to socio-environmental factors. In the conceptual model of loneliness, we illustrated how solutions can be delivered and tailored to an individual based on their life circumstances and preferences.</p></span><span class="a-plus-plus abstract-section id-a-sec4"><h3 class="a-plus-plus">Conclusion</h3><p class="a-plus-plus">We concluded by making specific recommendations in advancing our scientific understanding of loneliness. Our knowledge can only be deepened if we increase scientific rigour via accounting for confounding variables and using longitudinal, multi-disciplinary, and multiple methodologies in research. We also call for the rigorous evaluation of programs targeting loneliness.</p></span>

Abstract:Abstract Social connectivity is a crucial human experience. The objective experience of being disconnected (i.e., social isolation) and the subjective feeling of being disconnected (i.e., loneliness) are related but distinct experiences that have unique impacts on physical, mental, and cognitive health. The COVID-19 pandemic forced much of the global population into a prolonged state of isolation, and many older adults found themselves abruptly cut-off from important sources of social interaction. The lasting impacts of the pandemic are still unknown. This symposium brings together several projects addressing key questions about longitudinal processes of loneliness and social isolation among middle aged and older adults, both prior to and during the COVID-19 pandemic. Jackson will present results from a coordinated data analysis (CDA) showing the extent to which loneliness, social isolation, and social asymmetry are related to MCI, dementia, mortality, and life expectancy. Rule will discuss the impact of stroke on trajectories of loneliness in a CDA. Zhang and Fingerman will present findings that show how loneliness and social isolation vary among individuals with functional limitations. Zavala will present results from a CDA that explored daily patterns of loneliness during the early months of the COVID-19 pandemic. Lastly, Beck will discuss a CDA that examined trajectories of social asymmetry prior to and during the COVID-19 pandemic. Antonucci will synthesize and discuss each project within the context of broader implications for empowering older adults. In sum, this symposium presents novel evidence showing that loneliness and social isolation in older adulthood are impactful predictors of health.
Association of Loneliness With Change in Physical and Emotional Health of Older Adults During the COVID-19 Shutdown

Abstract:Objectives: To examine factors influencing loneliness and the effect of loneliness on physical and emotional health, in the context of the COVID-19 pandemic. Design: Prospective, observational cohort. Setting: Community-dwelling participants. Participants: Older adults (n = 238) enrolled in a longitudinal study. Measurements: Interviews were completed July-December 2020. Loneliness was measured with the UCLA 3-item loneliness scale. Data including age, marriage, education, cognitive functioning, functional impairment, vision or hearing impairment, depression, anxiety, medical comorbidity, social network size, technology use, and activity engagement were collected. Health outcomes included self-rated health, and physical and mental composites from the 12-item Short Form Survey. Physical function was measured by a PROMIS-scaled composite score. Results: Thirty-nine (16.4%) participants reported loneliness. Vulnerability factors for loneliness included age (RR = 1.08, 95% CI 1.02-1.14); impairment with instrumental activities of daily living (RR = 2.08, 95% CI 1.14-3.80); vision impairment (RR = 2.09, 95% CI 1.10-3.97); depression (RR = 1.34, 95% CI 1.25-1.43); and anxiety (RR = 1.92, 95% CI 1.55-2.39). Significant resilience factors included high cognitive functioning (RR = 0.88, 95% CI 0.83-0.94); large social network size (RR = 0.92, 95% CI 0.88-0.96); technology use (RR = 0.81, 95% CI 0.73-0.90); and social and physical activity engagement (RR = 0.91, 95% CI 0.85-0.98). Interaction analyses showed that larger social network size moderated the effect of loneliness on physical function (protective interaction effect, RR = 0.64, 95% CI 0.15-1.13, p <.01), and activity engagement moderated the effect of loneliness on mental health (protective interaction effect, RR = 0.65, 95% CI 0.25-1.05, p <.001). Conclusions: Resilience factors may mitigate the adverse health outcomes associated with loneliness. Interventions to enhance resilience may help to diminish the detrimental effects of loneliness and hold great importance for vulnerable older adults.

Abstract:This qualitative study aims to investigate the cultural understandings of loneliness, identify the contexts of loneliness, and to examine its effect on the health and well-being of U.S. Chinese older adults. Despite loneliness is one of the main indicators of well-being, little attention has been paid to understanding loneliness among immigrant older adults. This study utilizes both survey questionnaires and semi-structured focus group methods to investigate the feelings of loneliness among U.S. Chinese older adults. Based on interviews with 78 community-dwelling Chinese older adults in Chicago Chinatown, this community-based participatory research study (CBPR) shows loneliness is common among U.S. Chinese older adults. It was frequently identified through emotional isolation and social isolation. Social, psychological and physical health factors could contribute to the experience of loneliness. In addition, the health of older adults with loneliness may be associated with worsening overall health, elder mistreatment, health behavior changes and increased healthcare utilizations. This study has implications for healthcare professionals, social services agencies and policy makers. Our findings point to the needs for healthcare professionals to be more alert of the association of loneliness and adverse health outcomes. Communities and social services agencies should collectively take a lead in reducing social isolation, improving intergenerational relationships, and increasing social networks and companionship for this group of vulnerable older adults.

Abstract:In this paper, through a thematic literature review of recent studies, our objective was to explore the impact of loneliness and its effects on health. We emphasize the human need for a sense of belonging and societal inclusion. We further discuss the link of loneliness to physiological alterations, to the development and exacerbation of mental health conditions, the influence it has on cognitive processes, and the role it plays in contributing to maladaptive coping strategies. Contrary to the feelings of loneliness, solitude and its benefits for self-reflection and personal growth are explored. The prevalence of loneliness and its cultural similarities and differences are discussed at a global level. Moreover, this review explores the intricate relationship between loneliness and various health outcomes, including chronic illness and mortality risks. It further emphasizes the importance of acknowledging loneliness as a public health concern and the need for interventions at the individual and societal level to mitigate its adverse effects.

Abstract:Objectives: We performed a systematic review and meta-analysis to examine the prevalence and antecedents/consequences of chronic loneliness and social isolation (i.e. enduring or persistent experience that extends over a certain period of time) among older adults. Moreover, we conducted a meta-regression to explore sources of heterogeneity. Method: A search was conducted in four electronic databases. We included observational studies that reported prevalence and, where available, antecedents/consequences of chronic loneliness or chronic social isolation amongst older adults. Key characteristics of the studies were extracted. Results: Across 17 studies included in the meta-analysis, the estimated prevalence of chronic loneliness was 20.8% (95% CI: 16.1-25.5%), including 21.7% among women (95% CI: 16.1-27.4%) and 16.3% among men (95% CI: 10.6-21.9%). One study reported chronic social isolation (13.4%) and found that chronic social isolation predicted higher depression scores. Meta-regressions indicated that loneliness was less prevalent when assessed with single-item measures. Regarding antecedents/consequences, spousal loss can contribute to chronic loneliness which in turn may contribute to adverse health-related outcomes. Conclusion: About one in five older adults experiences chronic loneliness reflecting the need to address chronic loneliness. More longitudinal research is needed on chronic loneliness and social isolation, particularly from low and middle-income countries.
Loneliness and Coping Strategies among Elderly

Abstract:Present study aim to assess the loneliness (emotional and social loneliness) and coping strategies and their relationship among elderly living in institutions and non-institutions. The study compares the loneliness including factors such as emotional loneliness, social loneliness, emotional isolation and social isolation of old age people in institutions and non-institutions and says about the relationship between loneliness and coping strategies among elderly. The sample size is 60 includes 30 institutionalized elderly people and 30-non institutionalized elderly above age 65. To compare the factors the tool used is Emotional/Social Loneliness Inventory (ESLI) by Harry Vincenzi and Fran Grabosky which is a 15 items scale presented in a paired format. To understand the coping strategies, the statistical tool Ways of Coping Questionnaire (WCQ) by Folkman and Lazarus which is self-report inventory composed of 8 subscales is used. The study shows a significant difference in social loneliness and emotional loneliness between old age people in institutions and non-institutions. It shows that instituonalized elderly people have higher social and emotional loneliness compared to other group, also there is a significant difference in emotional isolation, but no significant difference in social isolation between old age people in institutions and non-institutions. Then the study says about the relationship between loneliness and coping strategies among institutionalized people and non-institutionalized people and a positive correlation is found through the study.

Abstract:<span>Loneliness and social isolation are described similarly yet are distinct constructs. Numerous studies have examined each construct separately; however, less effort has been dedicated to exploring the impacts in combination. This study sought to describe the cumulative effects on late-life health outcomes.Survey data collected in 2018–2019 of a randomly sampled population of US older adults, age 65+, were utilized (<i>N</i> = 6,994). Survey measures included loneliness and social isolation using the UCLA-3 Loneliness Scale and Social Network Index. Participants were grouped into four categories based on overlap. Groups were lonely only, socially isolated only, both lonely and socially isolated, or neither. Bivariate and adjusted associations were examined.Among participants (mean age = 76.5 years), 9.8% (<i>n</i> = 684) were considered lonely only, 20.6% (<i>n</i> = 1,439) socially isolated only, 9.1% (<i>n</i> = 639) both lonely and socially isolated, and 60.5% (<i>n</i> = 4,232) neither. Those considered both lonely and socially isolated were more likely to be older, female, less healthy, depressed, with lower quality of life and greater medical costs in bivariate analyses. In adjusted results, participants who were both lonely and socially isolated had significantly higher rates of ER visits and marginally higher medical costs.Results demonstrate cumulative effects of these constructs among older adults. Findings not only fill a gap in research exploring the impacts of loneliness and social isolation later in life, but also confirm the need for approaches targeting older adults who are both lonely and socially isolated. As the COVID-19 pandemic continues, this priority will continue to be urgent for older adults.</span>

Older persons making a life closure: experiences of loneliness in an academic nursing home - a phenomenological qualitative interview study

Abstract:Background: Loneliness is a public health concern and more than half of the residents in nursing homes experience lonliness. Risk factors are age and loss of close relatives. Purpose: This study aimed to describe experiences of loneliness among older people living in an academic nursing home. Methods: Qualitative semi-structured interviews were conducted with ten older people and data analysed with systematic text condensation inspired by a phenomenological approach. Results: Three themes were identified: "Relatives and health care professionals matter"; "Acceptance and meaningful existence alleviate loneliness"; and "Challenges affecting the experience of loneliness". The older persons described themselves as lonely, but their experience of loneliness differed. They managed loneliness by adapting to it or getting used to it; some also chose to be alone. To add meaningfulness to their daily life, talking about memories and their past were appriciated. Personality traits and variations in functional ability were identified as barriers to social interactions. Conclusions: Health care professionals can reduce negative experiences of loneliness by listening to nursing home residents, creating a meaningful daily life with individualized activities, and by encouraging contacts with close relatives. This can be a way of maintaining older persons' dignity and coping with the longing for what has been.

Abstract:Background: The intense focus on major psychiatric disorders in both contemporary psychiatric research and clinical practice has resulted in the relative neglect of less definable constructs such as loneliness and how such entities might impact on health outcomes. The purpose of this review is to raise awareness among physicians and psychiatrists of the medical impact and biological effects of loneliness as well as making the argument that loneliness should be a legitimate therapeutic target. Methods: Using Pubmed we searched the literature for research and review papers looking at loneliness as a construct, how it is measured and its health effects. We reviewed the relevant papers and have summarized their main findings. Results: Loneliness has strong associations with depression and may in fact be an independent risk factor for depression. Furthermore loneliness appears to have a significant impact on physical health being linked detrimentally to higher blood pressure, worse sleep, immune stress responses and worse cognition over time in the elderly. There is a relative deficiency in adequate evidence based treatments for loneliness. Conclusion: Loneliness is common in older people an is associated with adverse health consequences both from a mental and physical health point of view. There needs to be an increased focus on initiating intervention strategies targeting loneliness to determine if decreasing loneliness can improve quality of life and functioning in the elderly.

Abstract:Loneliness is increasingly recognized as a significant health risk. This review provides an overview of current research on the impact of loneliness on mental and physical health. The findings indicate that loneliness is a significant risk factor of both mental and physical health. Previous reviews focusing on mental health effects reveal that loneliness is associated with the onset of depression, social anxiety, and cognitive decline. Furthermore, loneliness can impede recovery from mental disorders in general, which can be partly attributed to the bidirectional interplay between loneliness and mental health. Regarding physical health, consequences of loneliness have been mostly studied in relation to cardiovascular diseases. Loneliness predicts the onset of cardiovascular diseases and impedes recovery from them. Reviews also highlight that loneliness enhances the likelihood of suicidality and mortality. However, this article also points out the shortage of longitudinal studies, complicating the investigation into how loneliness causally affects future health problems. The health implications of loneliness outlined in this review, some of which are irreversible and can induce suffering and impairment, underscore the importance of interventions aimed at alleviating loneliness as a preventive strategy against both mental and physical illnesses.
