Abstract:Depression severely impairs psychosocial functioning and quality of life, which places a huge burden on patients and their families. However, the physiological mechanism of depression remains unknown. Treatment with existing antidepressant medications is effective in around 50% of patients according to various studies, but is associated with severe side effects including nausea and headaches. Chinese herbal medicine (CHM) has been approved and widely used for depression as an alternative medicine in Chinese culture for decades. It has certain advantages and potential in the prevention and treatment of depression. In this review, we summarize the currently available evidence for the efficacy of CHM for the treatment of depression and physiological diseases comorbid with depression. We further discuss the possible mechanisms of action of CHM and the relationships to our current understanding of depression. The majority of current evidence has suggested that the combined treatment with CHM and mainstream antidepressants improves the response rate and reduces the side effects, while CHM alone could be more effective than placebo. However, the results should be carefully interpreted due to the shortcomings of existing clinical trials and a high risk of bias in meta-analyses. Our review provides a summary of the current applications and understanding of widely used CHMs for depression.

Abstract:Currently,the research or publications related to the clinical comprehensive evaluation of Chinese patent medicine are increasing,which attracts the broad attention of all circles. According to the completed clinical evaluation report on Chinese patent medicine,there are still practical problems and technical difficulties such as unclear responsibility of the evaluation organization,unclear evaluation subject,miscellaneous evaluation objects,and incomplete and nonstandard evaluation process. In terms of evaluation standards and specifications,there are different types of specifications or guidelines with different emphases issued by different academic groups or relevant institutions. The professional guideline is required to guide the standardized and efficient clinical comprehensive evaluation of Chinese patent medicine and further improve the authority and quality of evaluation. In combination with the characteristics of Chinese patent medicine and the latest research achievement at home and abroad,the detailed specifications were formulated from six aspects including design,theme selection,content and index,outcome,application and appraisal,and quality control. The guideline was developed based on the guideline development requirements of China Assoication of Chinese medicine. After several rounds of expert consensus and public consultation,the current version of the guideline has been developed.

Abstract:The clinical comprehensive evaluation of Chinese patent medicine is an important direction in the evaluation of traditional Chinese medicine(TCM), which positively promotes the development of TCM industry. The evaluation system of Chinese patent medicine is helpful to comprehensively evaluate the clinical value of different Chinese patent medicine in the same category, different dosage forms, and specifications, from different manufacturers on the basis of evidence and value. The establishment of a scientific and reasonable comprehensive evaluation index system for Chinese patent medicine is an important prerequisite to ensure clinical value. However, there has been neither a recognized systematic review on the clinical comprehensive evaluation of Chinese patent medicine nor a methodological system used for reference. The evidence and value: impact on decision-making(EVIDEM), developed by the international research team, is used to evaluate the comprehensive value of medical interventions. EVIDEM provides a methodological tool for scientific decision-making to evaluate evidence and value for health technologies on the basis of the multi-criteria decision analysis(MCDA) model and health technology assessment(HTA). Based on the ongoing EVIDEM research, the present study put forward that EVIDEM-based clinical comprehensive evaluation of Chinese patent medicine consisted of four aspects, seven modules, and ten steps, which is expected to references and practical experience for the follow-up comprehensive evaluation of Chinese patent medicine in the TCM field.
Critical Quality Evaluation and Application Value of Network Meta-analyses in Traditional Chinese Medicine

Abstract:To introduce the application status of network Meta-analysis( NMA) in the field of traditional Chinese medicine,and to discuss the application value of NMA in the field of traditional Chinese medicine,this study comprehensively reviewed the systematic reviews with application of NMA in the field of traditional Chinese medicine. CNKI,Wan Fang,Sino Med,VIP,Embase,PubMed and Cochrane Library and the reference list of previous studies were searched. The AMSTAR scale was used to evaluate the quality of literature methodology,and PRISMA-NMA checklist was used to measure the degree of report specification. Overall,122 articles were included,including 80 in Chinese and 42 in English. The included studies centered on cancer,bone and joint disease,cardiovascular disease,respiratory disease,mental disease and digestive disease. Additionally,the intervention can be categorized into three groups,traditional Chinese medicine injection,oral Chinese medicine or prescription,and traditional physical therapy including acupuncture.Nearly one-third of the researches' intervention program is aimed at comparing the effect of Chinese and Western combined therapy and monotherapy. The overall methodology quality grade is medium and the report quality is average,with methodology reporting and result reporting especially need to be improved. The subgroup analysis shows that the methodology quality of the English literatures is evidently higher than Chinese literatures,and the quality of the literatures published after 2015 is higher than those published in or before 2015.This study indicates that the NMA can compare multiple treatments simultaneously,which accords with characteristics of the clinical practice in traditional Chinese medicine that is complex and individual. NMA in the field of traditional Chinese medicine is still in the process of development. With higher level of quality control and reporting as well as the improvement of the statistical methodology and the accumulation of original researches,NMA application in the field of traditional Chinese medicine will be promising.
The Challenges of Ethical Review in Clinical Research of Traditional Chinese Medicine

Abstract:The ethical review of TCM (traditional Chinese medicine) clinical research in China is highly consistent with that of Western medicine, but it lacks the characteristics and culture of TCM. Compared with modern medicine, TCM has its own characteristics, such as the theory of Yin-Yang and the five elements, the treatment of syndrome differentiation, and the compatibility of prescriptions. These characteristics determine the ethical particularity of TCM clinical research. This article discusses the challenges in the ethical review of TCM clinical research, such as scientific design, efficacy evaluation, risk assessment, informed consent, and placebo use. We propose opportunities and difficulties in the ethical review of TCM clinical research and provide some relevant suggestions.

Abstract:Traditional Chinese medicine (TCM) is currently considered a complementary or alternative medical system in most Western countries and has been increasingly accepted worldwide. More and more clinical trials on TCM have been conducted internationally, and scientists worldwide are becoming increasingly interested in the evaluation of clinical efficacy of TCM based on clinical trials. This paper reviews the situation of clinical trials on TCM in the past decade, including systematic reviews about clinical trials either focusing on the treatment of disease with TCM approaches or focusing on one herbal product, conduction of clinical trials on TCM either with randomization and controlled methods or general observation. Some general issues on the conduct of clinical trials on TCM, such as randomization, control, quality of life (QOL), patient reported outcomes (PROs) and biomarkers, quality control, safety evaluation and case studies, are discussed, and accordingly some suggestions are proposed.

Abstract:To assess the methodology and report quality of systematic evaluation and Meta-analysis of acupuncture and moxibustion in China.Retrieve CBM, CNKI, WF and VIP database, collect data from the information system established by Epidata 2.1, assess the methodology and report quality by using the QQAQ and QUOROM, calculate the percentage of adequate rate.Thirty-eight reviews, including twenty six systematic evaluation and twelve Meta-analyses, met the enrolled criteria. Twenty-two kinds of diseases and six diseases systems were included. The methodology quality scores were generally low (3.34 +/- 1.44). The causes of the problems were insufficient literature resource, bias in data collections and inaccurate merging methods. The report quality was relatively low in abstracts, methods, trial flow, introduction and data merging.The amount of literatures on systematic evaluation and Meta-analysis of acupuncture is gradually increasing from 2002. However, the quality control is not ideal. It is important to improve the methodology and report quality.

Abstract:The clinical comprehensive evaluation of drugs is an important basis for the return of clinical value, decision-making of medical and health authorities, and allocation of medical resources. In July 2021, the National Health Commission issued the Guidelines for the Management of Clinical Comprehensive Evaluation of Drugs(trial version 2021), which required the evaluation to be implemented from the six dimensions(safety, effectiveness, economy, innovation, suitability, and accessibility), and made detailed arrangements for the clinical comprehensive evaluation of drugs. As Chinese patent medicine differs from chemical medicines in terms of effective components and action modes, the clinical comprehensive evaluation of Chinese patent medicine should highlight the characteristics and advantages of traditional Chinese medicine(TCM) on the basis of general requirements of comprehensive clinical evaluation of drugs. At present, in the clinical comprehensive evaluation of Chinese patent medicine, unified report standards have not yet been generated, resulting in the uneven quality of existing reports. To standardize the clinical comprehensive evaluation report of Chinese patent medicine and improve its quality, the editorial team, based on the relevant policy documents of clinical comprehensive evaluation of drugs, formulated the clinical comprehensive evaluation report standards for Chinese patent medicine in combination with the previous practice and expert opinions. The report standards, containing seven sections with 15 items determined, focus on data source, evaluation content, evidence synthesis, quality control, and evaluation results supported with detailed interpretations to help researchers better understand and apply the report standards for clinical comprehensive evaluation of Chinese patent medicine, improve the report quality, and provide references for the decision-making by the national medical management authorities.

Abstract:Coronary heart disease (CHD) has severely impacted the lives and health of patients for ages. Although the modern methods used for the prevention and treatment of CHD have been increasingly perfected, clinical problems remain that require solutions. In the prevention and treatment of CHD with traditional Chinese medicine (TCM), great therapeutic advantages have been demonstrated. However, the expression of its advantages relies on scientific evidence-based research and evaluation, which require further optimization and improvement. Therefore, this review aims to comprehensively emphasize our understanding of the current deficiencies in TCM evidence-based studies and the necessity of adopting scientific evidence-based optimization methods for use in research. In this process, evidence should be produced in a standard manner, and the advantages of TCM patterns should be accurately stated because these factors contribute to efficient transformation and proper verification of the obtained evidence. Additionally, focusing on patient-oriented medicine and doctor–patient relationships is also essential. These methods are of great significance to prove the clinical efficacy of TCM and promote its development.

Abstract:Since the evidence-based medicine (EBM) being evoked widely, to seek out the best evidence is becoming the pivotal step for the development of Chinese medicine (CM), and randomized controlled trial (RCT, at most times, it means explanatory clinical trial--ECT) has been accepted as the general golden standard for the evaluation of clinical intervention. However, it is noted that the traditional RCT (ECT) is unsuitable for the special characteristics of individual treatment in CM. The formation and development of CM theory is found on large amount of clinical experiences, and to evaluate its clinical efficacy and safety is the most primary task. Because the CM intervention measures are complex and individualized, the clinical effectiveness of CM is embodied as a complex system, which impacted greatly by the environment factors. It is deemed that the pragmatic clinical trial (PCT) has the character just fitting for the evaluation. The feasibility of PCT in CM clinical evaluation is discussed in this paper in order to raise a new issue for the evaluation of CM effectiveness.

Abstract:Application experience in humans, a summary of the clinical practice of traditional Chinese medicine(TCM), serves as an important data source for evaluating the safety, effectiveness, and clinical value of drugs in the development of new Chinese medicine. The collected data serving as the evaluation evidence through statistical analysis are critical to the research on the application experience in humans. This article summarized and analyzed the data characteristics and statistical methodology of application experience of Chinese medicine in humans, and concluded the data types, outcome evaluation, bias evaluation, confounding factors, and missing values. Furthermore, the article emphasized the importance of data analysis of application experience of Chinese medicine in humans for TCM evidence and put forward the current difficulties, such as low data quality and large internal bias, lack of individualized data processing methods, and lack of methods for "disease-syndrome combination" data. We believe that with the development of methodology, the application experience of Chinese medicine in humans can strongly support the development of new drugs in TCM.

Abstract:Abstract Background Clinical Practice Guidelines (CPGs) play an important role in clinical practice, and they require appropriate evaluation, especially in application. This study explores the application evaluation method of CPGs for Traditional Chinese Medicines (TCM). It uses the Analytic Hierarchy Process (AHP) and clinical cases to evaluate the consistency between CPGs of TCM and clinical practice. Methods To evaluate the consistency between CPGs of TCM and clinical cases, a 3-level AHP construction was built. Weightings were calculated by collecting questionnaires according to AHP theory. To test the evaluation system, a retrospective study was performed. The study evaluated the China Association of Chinese Medicine’s Guidelines for Diagnosis and Treatment of Common Internal Diseases in Chinese Medicine Diseases of Modern Medicine (CPGs of DTCID) (ZYYXH/T50–135-2008). A total of 150 cases were involved. The evaluation system was used to assess the consistency between CPGs of DTCID and clinical cases of angina pectoris. Results The results showed that the overall consistency between CPGs of DTCID and the 150 cases was 42.32 ± 6.94%, ranging from 35.21 to 63.37%. The overall consistency was not affected by age, gender, type of angina pectoris, condition of percutaneous coronary intervention (PCI), or angina classification as determined by the Canadian Cardiovascular Society. The consistencies of each index were as follows: Diagnosis of TCM, 100%; Diagnosis of Western medicine, 100%; Syndrome classification, 38.25 ± 4.40%; Syndrome key point, 34.17 ± 8.15%; TCM Decoction, 31.08 ± 23.64%; TCM particular treatment, 7.92 ± 19.13%; and Recuperation and prevention, 0. The most frequent syndromes were qi -deficiency, phlegm and blood stasis ( n = 124) (82.7%). The overall consistency of qi -deficiency, turbid phlegm and blood stasis was lower than the overall consistency of the group without that syndrome. The difference was statistically significant ( P < 0.05). 42 cases (28%) applied the TCM decoction recommended by CPGs of DTCID. Of these, Gualouxiebaibanxia decoction was applied in 34 cases. Wendan decoction, the most frequently used, was applied in 64 cases (42.7%). Conclusion This study indicates that the AHP system can perform quantitative evaluation of consistency between TCM CPG and clinical practice. It also found the factors affecting the application of TCM CPGs and might indicate the need for revisions of CPGs.

Abstract:With the advent of modern medical science,the effective compounds of traditional Chinese medicine has become the sure way into the world and get some recognition in the international area.A review,including the major domestic and foreign research methods in this field,was made,based on the summary of many reports,and some of the details in the research process,such as the selection of subjects,as well as effects of the experimental method for testing the sample processing,and ultimately to determine the effect of composition and other issues are analyzed and explained.As science and technology advances,the effective compounds of Chinese herbal formula research will have more new methods,new technologies inevitably,to resolve the current problem which is not yet solved.Through a multi-disciplinary,collaborative and exchanges,which will bring new development of the effective compounds of Chinese herbal formula.

Abstract:This article systematically reviews the clinical researches on properties of traditional Chinese medicine(TCM) since the building up of our country.There are a lot of problems in this subject although preliminary conclusions have been obtained from the researches on properties of TCM in the elds of central nervous system,endocrine system,energy metabolism,immune system and hemorheology.Through deep thoughts of the ideas and methods on clinical researches,it is suggested by the author that the clinical researches on properties of TCM should be integrated with the researches on essence of coldness and hotness of syndrome and integrated with experiments.We also need establish the system of objective evaluation methods and indexes,and proceed dynamic researches on properties of TCM according to the dynamic changes of diseases.

Abstract:The difficulty in defining the ambiguous “natural language” within the theoretical system of traditional Chinese medicine (TCM), the dynamic evaluation and individual differences of treatment based on syndrome differentiation, as well as “hard-to-quantify” characteristics of subjectiveness in the evaluation of treatment efficacy during the treatment, has resulted in the questioning and limited use of various TCM therapies with extensive application in clinical practice, which also arises from the lack of recognized and scientific evaluation methods. The standards evaluating the clinical efficacy of TCM remain the key bottleneck restricting the modernization and internationalization of TCM. Although the concept of evidence-based medicine was introduced into the field of TCM and applied to the evaluation and exploration of the clinical efficacy of TCM, there is still a lack of systematic methodological tools for its applicability evaluation. The concept of “digital-intelligent TCM” provides a new direction for the exploration of TCM based on the scientific and technological background of the times. It is not antagonistic to evidence-based TCM; instead, it complements and prompts evidence-based TCM to jointly accelerate the standardization of the evaluation of the clinical efficacy of TCM.

Abstract:In recent years,there has been great progress in the field of analysis of Traditional Chinese Medicine(TCM),with the following features:the research object of TCM analysis has been beyond the common analysis technology and focused on the key scientific issues of TCM;it has been beyond the tool of chemical analysis and tried to set up new systems to solve the key issues of TCM by integrating the methods and technologies of various subjects;the analysis has been beyond the identification of the composition of TCM and paid more attention to the relationship between composition and function.Therefore,the progress and trend of modern TCM analysis is discussed and prospected in the present review paper,focusing on four aspects,namely the changes of information expression patterns,the changes of research strategies on TCM chemistry,the changes of the research objects and the changes of analysis model of pharmaceutical activity.
Traditional Chinese Medicine: from Experience Towards Evidence-Based Medicine

Abstract:Traditional Chinese medicine (TCM) has over 2000 years of history and played important role in the peoples' health and social development in Chinese history. In the past 50 years, Chinese scientists have made great effort to modernize TCM to make it a evidence-based medicine from the experence-based origin. The current review has summerized the recent advances in the modern research on the various respects of TCM including the resouces investigation, GAP cultivation, phytochemistry, quality assessment, safety issues, metabolic investigation, pharmacology, systems biology etc. It pointed out that GAP for Chinese herbs is the first and key step in the quality control cycle of traditional Chinese medicine. In addition, the research on TCM active principles and quality control methods were also summarized. Over 12,000 chemical constituents have been isolated from over 600 species of traditional Chinese medicines, among which over 2000 new compounds were discovered, which layed a solid foundation for clarifying the material basis of action and providing reference substances fro the quality control of TCM. Systems biology approach is now being actively practiced for the action mechanism studies of TCMs and their active principles, which provided a valuable approach for complex TCM systems. Currently, genomics, preotenomics and metabolomics have obtained good application in TCM studies. Finally, the progress of TCM new drug research and development has been outlined. Several famous new drugs from TCM or based on TCM have been sucessfully marketed. Examples are artemisinin, artemether, hupzine A, etc. Other types of new drugs include Fufang Danshen Dripping Pill, Diao Xinxuekang, and so on. Abstract:Traditional Chinese Medicine (TCM) has been extensively used as a mainstay for treating various pathologies. Combing the pharmacology and systems biology approaches, the network pharmacology (NP) approach was developed to predict the probable mechanism underlying the therapeutic effect of TCM. However, approaches solely based on NP cannot effectively elucidate the curative mechanism in a holistic and reliable manner due to limitations in NP-based methods and complexity of TCM components. Thus, integration strategies combining NP with other approaches are increasingly being used. Since the interdisciplinary research in TCM has received much attention in the advent of the big data era of which the NP-based integration strategy is broadly used, the strategy is clearly elaborated in the present review. We summarized several NP-based integration strategies and their applications in TCM studies, including multi-omics approach, gut microbiota study, chemical information analysis, data-mining, and network toxicology study.

Abstract:Traditional Chinese medicine (TCM) is one of the important components of Chinese civilization and has played an important role in the life of the Chinese nation since ancient times. In ancient times, TCM was the main means of treating diseases and an important method of daily health care for people. Ancient Chinese medicine is a part of Chinese Traditional medicine and a precious medical heritage accumulated over thousands of years. It focuses on the human body, emphasizes the overall balance of the individual, focuses on treating the root causes of diseases, and emphasizes the comprehensive and long-term nature of prevention and treatment. TCM has been widely used in ancient Chinese medical practice, and through long-term practical experience and theoretical summary, many clinically effective treatment plans have been formed. This review aims to review and explore the clinical application of TCM in ancient times, in order to provide reference and inspiration for the inheritance and development of TCM.
