Abstract:"Red" is indistinguishable from "red." Pooling of blood in the colon is often encountered during urgent colonoscopy and is reportedly useful for detecting the lesion responsible for acute lower gastrointestinal bleeding [<a href="#JR2091-1">1</a>]. However, discerning active or recent bleeding is cumbersome in an environment containing blood that has accumulated over various time points. The novel image-enhanced endoscopic technique, red dichromatic imaging (RDI), may overcome this problem. It clearly demarcates active bleeding from the surrounding residual blood based on the color-contrast principle (i. e. the difference in the target's hemoglobin concentration and thickness) [<a href="#JR2091-2">2</a>]. A 66-year-old man was admitted to our hospital with hematochezia. Contrast-enhanced computed tomography revealed extravasation in the ascending colon ([<a href="#FI2091-1">Fig. 1</a>]). Urgent colonoscopy was performed using the CF-HQ290I enteroscope (equipped with a waterjet) and EVIS X1 device (Olympus Co., Tokyo, Japan) after bowel preparation with polyethylene glycol (PEG). Massive blood pooling was observed in the ascending colon ([<a href="#FI2091-2">Fig. 2 a</a>]), which hindered endoscopic vision and obscured the bleeding points. Upon switching from white-light endoscopy (WLE) to the RDI mode, an amber stream, clearly demarcated from the translucent surroundings, was identified at the medial aspect of the ascending colon ([<a href="#FI2091-2">Fig. 2 b</a>]). The amber stream was found to be fresh blood or active bleeding, while the surrounding environment contained blood diluted with PEG. The causative diverticulum with active bleeding from the dome was successfully identified on RDI, having been obscured by residual blood on WLE ([<a href="#FI2091-3">Fig. 3</a>], [<a href="#ME2091-1">Video 1</a>]). Hemostasis was successfully achieved using endoscopic band ligation. The patient was discharged 5 days after treatment without any rebleeding or adverse events.<span class="b">Video 1</span> Pooled blood hindered endoscopic vision and identification of the exact bleeding point. Red dichromatic imaging revealed active or recent bleeding by providing a vivid color contrast from the surrounding residual blood.This case demonstrated the clinical utility of the RDI mode, which enhances only fresh blood that is otherwise missed on WLE and may help to identify the lesion responsible for bleeding, especially in the presence of residual blood in the colon. Abstract:BackgroundBleeding and hematoma formation during submucosal injection in esophageal endoscopic submucosal dissection (ESD) reduce the visibility of the submucosa. Red dichromatic imaging (RDI) is an endoscopic technique that provides better visualization of the deep submucosal blood vessels. We speculated that blood vessel injury could be avoided with RDI. This pilot study evaluated the role of RDI in preventing bleeding and hematoma formation during esophageal ESD.MethodsThis was a single-center retrospective observational study. We examined 60 patients who underwent ESD with white light imaging (WLI) and RDI. A single endoscopist reviewed all of the surgical videos to document the incidence and severity of bleeding episodes. Eighteen videos provided adequate quality and detail, and the number of blood vessels traversing the mucosal incision lines of the lesions in these videos was evaluated under WLI and RDI.ResultsThe WLI group had a significantly higher incidence of hematomas per unit area compared to the RDI group (0.18/cm2 [range 0–0.38] vs. 0 [0–0.18]/cm2, p = 0.024). The WLI group also had a significantly higher incidence of total bleeding episodes compared to the RDI group (42.9% [range 21.7–60.4] vs 16.7% [range 13.8–22.9], p < 0.001). Significantly more blood vessels were visible under RDI compared to WLI (5 [range 4–8] vs. 2 [range 1–5], p = 0.0020).ConclusionRDI reduced the incidence of bleeding and hematoma formation during submucosal injection in esophageal ESD. It was assumed that the improvement of blood vessel visibility by RDI might have contributed to the result.

Abstract:Background: Accurate diagnosis and early resection of colorectal polyps are important to prevent the occurrence of colorectal cancer. However, technical factors and morphological factors of polyps itself can lead to missed diagnoses. Image-enhanced endoscopy and chromoendoscopy (CE) have been developed to facilitate an accurate diagnosis. There have been no reports on visibility using a combination of texture and color enhancement imaging (TXI) and CE for colorectal tumors. Aim: To investigate the visibility of margins and surfaces with the combination of TXI and CE for colorectal lesions. Methods: This retrospective study included patients who underwent lower gastrointestinal endoscopy at the Toyoshima Endoscopy Clinic. We extracted polyps that were resected and diagnosed as adenomas or serrated polyps (hyperplastic polyps and sessile serrated lesions) from our endoscopic database. An expert endoscopist performed the lower gastrointestinal endoscopies and observed the lesion using white light imaging (WLI), TXI, CE, and TXI + CE modalities. Indigo carmine dye was used for CE. Three expert endoscopists rated the visibility of the margin and surface patterns in four ranks, from 1 to 4. The primary outcomes were the average visibility scores for the margin and surface patterns based on the WLI, TXI, CE, and TXI + CE observations. Visibility scores between the four modalities were compared by the Kruskal-Wallis and Dunn tests. Results: A total of 48 patients with 81 polyps were assessed. The histological subtypes included 50 tubular adenomas, 16 hyperplastic polyps, and 15 sessile serrated lesions. The visibility scores for the margins based on WLI, TXI, CE, and TXI + CE were 2.44 ± 0.93, 2.90 ± 0.93, 3.37 ± 0.74, and 3.75 ± 0.49, respectively. The visibility scores for the surface based on WLI, TXI, CE, and TXI + CE were 2.25 ± 0.80, 2.84 ± 0.84, 3.12 ± 0.72, and 3.51 ± 0.60, respectively. The visibility scores for the detection and surface on TXI were significantly lower than that on CE but higher than that on WLI (P < 0.001). The visibility scores for the margin and surface on TXI + CE were significantly higher than those on CE (P < 0.001). In the sub-analysis of adenomas, the visibility for the margin and surface on TXI + CE was significantly better than that on WLI, TXI, and CE (P < 0.001). In the sub-analysis of serrated polyps, the visibility for the margin and surface on TXI + CE was also significantly better than that on WLI, TXI, and CE (P < 0.001). Conclusion: TXI + CE enhanced the visibility of the margin and surface compared to WLI, TXI, and CE for colorectal lesions.

Abstract:Bleeding is the most common and time consuming complication during ESD. Usually, nearly 30% of procedure time is spend just for hemostasis. Therefore, we developed a new endoscopic imaging technology, Dual Red Imaging, which has the potential to drastically improve the visualization of bleeding points by use of two wavelength lights in the red band. Abstract:Dual Red Imaging has the potential to drastically improve the visualization of not only thick blood vessels in deeper tissue but also bleeding points by use of two wavelength lights in the red band. Contrast enhancement of thick blood vessels and bleeding points would be very useful for the reduction of bleeding risk and average time required for hemostasis e.g. during endoscopic submucosal dissection. Abstract:Endoscopic sphincterotomy (ES) is a crucial procedure in the treatment of biliary disease, but post-ES bleeding is an occasional adverse event [1]. Massive bleeding makes it difficult to continue the procedure, including hemostasis, because of poor visibility due to coagulum, the narrowness of the duodenum, and technical difficulty of using a side-viewing endoscope. Red dichromatic imaging, a recently developed novel image-enhanced endoscopy technology with good visualization of bleeding points and deep vessels, has been reported to be useful in various situations during upper and lower gastrointestinal endoscopy [2] [3] [4] [5]. Herein we present a case in which red dichromatic imaging was useful for hemostasis of massive post-ES bleeding.A 75-year-old-man developed cholangitis due to bile duct stones. Endoscopic retrograde cholangiography was performed with a TJF-Q290V scope with the EVIS X1 system (Olympus Medial Systems, Tokyo, Japan). After biliary cannulation, ES was conducted with a standard sphincterotome. However, massive persistent bleeding occurred immediately. The endoscopic views of the duodenal papilla and the surroundings became red with blood, and it was impossible to identify the bleeding point clearly. The white-light imaging was therefore switched to red dichromatic imaging. The accumulated residual blood became inconspicuous, and the active blood stream and bleeding point could be visualized in darker yellow ([Fig. 1], [Video 1]). Hemostasis was achieved with a local epinephrine injection near the identified bleeding point on red dichromatic imaging. No adverse events and no re-bleeding were observed after the procedure.Video 1 Successful hemostasis of massive post-endoscopic sphincterotomy bleeding with red dichromatic imaging. Red dichromatic imaging confers better visualization of bleeding points and active fresh bleeding, which appears as darker yellow in color. Moreover, pooled residual blood surrounding the active bleeding is less noticeable in red dichromatic imaging; thus, a clear field of view can be obtained compared to white-light imaging. Abstract:BACKGROUND AND AIMS:Hemostasis during endoscopic submucosal dissection (ESD) can sometimes be challenging and stressful for the endoscopist. Therefore, we aimed to assess the usefulness of dual red imaging (DRI), a new image enhancement technique that uses 3 wavelengths (540, 600, and 630 nm) to visualize bleeding points and to examine the efficacy of DRI in shortening the time required to achieve hemostasis (hemostasis time) during ESD.METHODS:DRI and white-light imaging (WLI) were used alternately for managing 378 bleeding events in 97 patients undergoing ESD. Hemostasis time for each hemostasis event was measured. Using portable eye-tracking glasses, 4 experienced endoscopists were shown random videos of intraoperative bleeding during ESD (20 cases each on WLI and DRI) and identified the bleeding point in each video. The mean distances of eye movement per unit of time until the bleeding point were identified in each video and compared between the WLI and DRI groups.RESULTS:Average hemostasis time was significantly shorter in the DRI group. The mean distance of eye movement was significantly shorter in the DRI group than in the WLI group for all endoscopists.CONCLUSIONS:DRI can offer useful images to help in clearly detecting bleeding points and in facilitating hemostasis during ESD. It is feasible and may help in successfully performing ESD that is safer and faster than WLI. (Clinical trial registration number: UMIN000018309.).

Abstract:Image-enhanced endoscopy (IEE) allows real-time high-contrast imaging of a targeted lesion without any special equipment. Among various IEE technologies, narrow-band imaging, in which a light of shorter wavelength is used, emphasizes the surface and blood vessel patterns on the mucosal surface. This technology has been widely used in endoscopic diagnosis in the gastrointestinal tract. Red dichromatic imaging (RDI) was recently developed; it utilizes lights of longer wavelengths (520–550, 595–610, and 620–640 nm), which have weak light scattering characteristics in contrast to narrow-band imaging. RDI was designed to enhance the visibility of deep-lying blood vessels and areas of bleeding, and it has been installed in the latest Olympus endoscopy system, EVIS X1, as an advanced version of the optical-digital method that was originally developed. Improving the visibility of deep blood vessels allows more accurate evaluation of esophageal varices and the degree of inflammation in ulcerative colitis. Easier identification of a bleeding source makes hemostasis quicker and easier to accomplish during endoscopic resection procedures such as endoscopic submucosal dissection and peroral endoscopic myotomy as well as during treatment of gastrointestinal bleeding from a peptide ulcer or colon diverticulum. The authors herein review the technological development and principles, review the existing literature on RDI, and discuss the utility and effectiveness of this unique IEE technology in gastrointestinal endoscopy.

Analysis of Texture and Color Enhancement Imaging for Improving the Visibility of Non-polypoid Colorectal Lesions

Abstract:Introduction: An endoscopic system using 5-color light-emitting diodes (LEDs) (EVIS X1; Olympus Co., Tokyo, Japan), which includes texture and color enhancement imaging (TXI), has been released. In this study, we analyzed the effects of TXI on the visibility of non-polypoid colorectal lesions and its diagnostic accuracy. Methods: We reviewed 101 non-polypoid lesions from 26 patients observed with white light imaging (WLI), narrow band imaging (NBI), and TXI. One representative image of each mode was evaluated by 6 endoscopists using a polyp visibility score of 4 (excellent) to 1 (poor). We calculated the color difference (CD) values for each lesion in the three modes. For tumor characteristics, one representative image of TXI and NBI magnification was evaluated by 3 experts according to a NBI classification. Results: The least squares means [95% confidence interval] of polyp visibility score of TXI (3.42 [3.06-3.77]) was significantly higher than that of WLI (2.85 [2.49-3.20], p < 0.001) but not that of NBI (3.33 [2.98-3.69], p = 0.258). The CD value of TXI (13.3 ± 6.3) was higher than that of WLI (9.7 ± 6.0, p < 0.001) but not that of NBI (13.1 ± 6.8, p = 0.81). For sessile serrated lesions, the CD value of TXI (11.1 ± 4.4) tended to be lower than that of NBI (12.6 ± 6.0, p = 0.07). The diagnostic accuracy and confidence level of magnification for NBI were significantly better than those for TXI (87.1 vs. 80.5%, p = 0.027, 87.5 vs. 62.7%, p < 0.001, respectively). Conclusion: TXI showed better visibility than WLI in terms of the endoscopist's score and CD value and may improve polyp detection.

Abstract:Objectives: Although color information is important in gastrointestinal endoscopy, there are limited studies on how endoscopic images are viewed by people with color vision deficiency. We aimed to investigate the differences in the visibility of blood vessels during endoscopic submucosal dissection (ESD) among people with different color vision characteristics and to examine the effect of red dichromatic imaging (RDI) on blood vessel visibility. Methods: Seventy-seven pairs of endoscopic images of white light imaging (WLI) and RDI of the same site were obtained during colorectal ESD. The original images were set as type C (WLI-C and RDI-C), a common color vision. These images were computationally converted to simulate images perceived by people with color vision deficiency protanope (Type P) or deutanope (Type D) and denoted as WLI-P and RDI-P or WLI-D and RDI-D. Blood vessels and background submucosa that needed to be identified during ESD were selected in each image, and the color differences between these two objects were measured using the color difference (ΔE 00) to assess the visibility of blood vessels. Results: ΔE 00 between a blood vessel and the submucosa was greater under RDI (RDI-C/P/D: 24.05 ± 0.64/22.85 ± 0.66/22.61 ± 0.64) than under WLI (WLI-C/P/D: 22.26 ± 0.60/5.19 ± 0.30/8.62 ± 0.42), regardless of color vision characteristics. This improvement was more pronounced in Type P and Type D and approached Type C in RDI. Conclusions: Color vision characteristics affect the visibility of blood vessels during ESD, and RDI improves blood vessel visibility regardless of color vision characteristics.
Green-colored areas in laterally spreading tumors on narrow-band imaging: a future target for artificial-intelligence-assisted detection of malignancies?

Abstract:Accurate real-time characterization of colorectal lesions during endoscopy is crucial for histological prediction, allowing the choice of the most appropriate treatment [<a href="#JR2446-1">1</a>]. Polyps are characterized on the basis of an evaluation of their macroscopic appearance, vascular pattern, and pit pattern with magnification, under both white-light and virtual chromoendoscopy such as narrow-band imaging (NBI) [<a href="#JR2446-2">2</a>]. However, colorectal lesions are not homogeneous: a malignant component can sometimes occupy only a small area of the whole lesion and be relatively difficult to detect, especially for inexperienced endoscopists.We report on four colorectal lesions with focal malignancies ([<a href="#ME2446-1">Video 1</a>]). The first lesion was a 4-cm granular laterally spreading tumor (LST-G) in the sigmoid with a 5-mm area classified as Kudo Vn, Sano IIIa. The second lesion was a 3-cm pseudo-depressed nongranular LST in the sigmoid with a 5-mm area classified as Kudo Vn. The third lesion was a pseudo-depressed polyp in the right colon with a central area classified as Kudo Vi. The fourth lesion was a LST-G of the cecum with a 15-mm nodule classified as Kudo Vi, Sano IIIa. The four lesions were resected by endoscopic submucosal dissection and the suspect areas were identified as adenocarcinoma invading the submucosa to depths of, respectively, 3500 μm, 900 μm, and more than 1500 μm. In all four lesions, we detected with NBI a green-colored area (also visualized as a reddish area in TXI mode; Olympus, Tokyo, Japan) that corresponded to the area where invasive cancer was found ([<a href="#FI2446-1">Fig. 1</a>], [<a href="#FI2446-2">Fig. 2</a>], [<a href="#FI2446-3">Fig. 3</a>], [<a href="#FI2446-4">Fig. 4</a>]).<span class="b">Video 1</span> Endoscopic examination of the four colorectal lesions with focal malignancies.These cases show that the malignant component within a large colorectal lesion can have a green-colored area that is easily detectable on a distant view of the lesion, without the need to analyze the entire surface under magnification. Efficacy of using red dichromatic imaging throughout endoscopic submucosal dissection procedure

Abstract:BackgroundManagement of bleeding during endoscopic submucosal dissection (ESD) is critical. Red Dichromatic Imaging (RDI), a novel image-enhanced endoscopy technology, has been reported to improve the visibility of deep vessels and bleeding source compared to white light imaging (WLI). We hypothesized that using RDI during the entire cutting process (full time RDI ESD: FTR-ESD), higher R0 resection rate, shorter procedure time and fewer complications could be achieved. Therefore, the aims of the present study were to investigate the efficacy and safety of FTR-ESD.MethodsThis retrospective observational study included a total of 82 consecutive patients who underwent ESD by a single expert endoscopist for 40 esophageal, 17 gastric and 25 colorectal cancers at our institution from January 2018 to March 2021. The clinicopathological data were collected from patients’ medical records and the treatment outcomes were analyzed according to the treatment phase (early; 57 WLI-ESD and late; 25 FTR-ESD).ResultsThe median of the greatest diameter of resected specimen was 40.0 mm. The median procedure time was relatively shorter in the FTR-ESD group (35 min) than in the WLI-ESD group (40 min), but the difference was not statistically significant (p = 0.34). The median dissection speed in the FTR-ESD group (27.23 mm2/min) was significantly faster than that in the WLI-ESD group (20.94 mm2/min) (p = 0.025). The dissection speed was not different among different organs. A multivariate analysis revealed that tumor size (more than 30 mm) and FTR-ESD were significant independent factors contributing to faster dissection speed (p < 0.05). There were no significant differences in the rates of en bloc resection, HM0, VM0 or occurrence of adverse events between WLI-ESD and FTR-ESD.ConclusionsFTR-ESD significantly increases the dissection speed compared to WLI-ESD. FTR-ESD can be performed safely and therapeutic outcomes in FTR-ESD are comparable with WLI-ESD. A further multicenter prospective study is warranted to confirm our results.

Abstract:Aim: In this pilot animal study we examined whether it is possible to visualize the embryonal resection layers by using intraarterial indocyanine green (ICG) staining when performing total mesorectal excision (TME) for carcinoma of the rectum. Material and methods: We injected ICG into the inferior mesenteric artery (AMI) of four swines to see whether the watershed area of the arterial supply zone can be sufficiently visualized by fluorescence imaging in order to mark the right dissection area along the fascia parietalis before and during resection. Results: We observed a fluorescence signal in all the supplied areas of AMI but not in other parts of the abdominal cavity or other organs. Additionally, the mesorectum also showed a sharp border between colored and non-colored tissue. Conclusion: In this study we present that resection borders may be determined before resection based on ICG-perfusion and we showed that intraoperative exclusive coloring of the rectum including the mesorectum is possible. Visualizing resection borders based on ICG-perfusion before settling the first cut may be a new approach in oncological surgery.

Abstract:A 67-year-old woman complaining of intermittent lower abdominal discomfort and hematochezia was referred to our endoscopy center. Colonoscopy revealed a 3 × 3-cm sessile polyp located in the sigmoid colon (about 15 cm from the anal verge) ([Fig. 1]). The polyp was hidden behind colonic folds and was difficult to access with a colonoscope. After submucosal injection, the polyp was snared using electrocautery (endocut mode) in a piecemeal fashion ([Fig. 2]).

Abstract:Background/aim: Intraoperative identification of the cancer location is often difficult during robot-assisted surgery, especially in early stage cancers. This study aimed to investigate the feasibility and accuracy of a novel endoscopic clip emitting near-infrared (NIR) fluorescence during robot-assisted surgery for gastrointestinal cancer. Patients and methods: Preoperative placement of endoscopic marking clips equipped with NIR fluorescent resin was performed to determine the resection margins in six patients with gastrointestinal cancer. During robot-assisted surgery, a NIR fluorescence imaging system was used to detect the fluorescence. The evaluation examined whether fluorescence from the clips was visualized during robot-assisted surgery. Results: The NIR fluorescent signals emitted from the clips were successfully detected in all six patients from the serosal surfaces, resulting in the quick and accurate identification of the resection line. There were no significant differences in age, sex, or body mass index between the patients in whom we could detect NIR fluorescence. Conclusion: This novel NIR fluorescent clip is a promising diagnostic tool for accurately detecting tumor locations during robot-assisted surgery for gastrointestinal cancer.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Red dichromatic imaging (RDI) is a novel image‐enhanced endoscopy released in 2020, which increases the visibility of deeper vessels. In this study, we retrospectively investigated whether RDI can shorten treatment times of endoscopic injection sclerotherapy (EIS), and which operational procedure times are affected compared with white light imaging (WLI).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>A total of 155 patients (RDI: 70; WLI: 85) with risky esophageal varices (EV), who were treated with EIS were analyzed. Treatment times were compared, and predictors associated with treatment time were analyzed by multivariate analysis. For 24 cases (RDI: 12; WLI: 12) in which treatment videos were recorded, the procedure times of each step (observation of EV, needle flush, positioning, puncture, observation of bleeding, hemostasis, observation after hemostasis) were measured. Regarding the 7 patients with EV bleeding, color differences were calculated between the bleeding point and the blood pool using the CIE (L*a*b*) color‐measurement method, and results were compared between using RDI and WLI.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Treatment times were shorter in the RDI group (RDI vs. WLI = 35.1 vs. 42.2 min, <i>p</i> < 0.01). 'RDI function' and 'amount of sclerosant' were extracted as independent predictors of treatment time. Times for 'observation of EV' and 'observation of bleeding' were shorter in the RDI group (<i>p</i> = 0.01 and < 0.01, respectively). Regarding the color difference, RDI significantly increased bleeding point visibility (RDI vs. WLI = 31.4 ± 11.8 vs. 8.6 ± 6.2, <i>p</i> < 0.001). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>RDI can shorten the treatment time of EIS by increasing bleeding point visibility.</p></section>

Abstract:Real-time visualization of red blood cell flow inside subepithelial microvessels is performed with magnifying endoscopy. However, microvascular blood flow velocity in the colorectum has not been investigated. Here, we aimed to evaluate the blood flow velocity of microvessels of colonic polyps and to compare it with that of surrounding mucosa. We examined 50 lesions, including 30 adenomas (ADs) and 20 hyperplastic polyps (HPs). Blood flow velocities of lesions and their surrounding mucosa were evaluated using magnifying blue laser imaging (BLI) prior to endoscopic resection. Calculation of mean blood flow velocities was based on mean movement distance of one tagged red blood cell using split video images of magnifying BLI. Mean microvascular blood flow velocity was significantly lower in ADs (1.65±0.66 mm/sec; range 0.46-2.90) than in HPs (2.83±1.10 mm/sec; 1.07-4.50) or the surrounding mucosa (3.73±1.11 mm/sec; 1.80-6.20; P <0.001). The blood flow velocity rate compared with the surrounding mucosa was significantly lower in ADs (0.41±0.16; 0.10-0.82) than in HPs (0.89±0.25; 0.46-1.51; P <0.001). We found that mean microvascular blood flow velocity was significantly lower in ADs than in HPs and the surrounding non-neoplastic mucosa. These findings indicate that a novel dynamic approach with microvascular blood flow velocity using magnifying endoscopy may be useful in assessing physiological differences between ADs and HPs.
