Abstract:Purpose. Single dose of epidural hydromorphone has been introduced to serve as an alternative method for postcesarean section analgesia. However, optimal dose of epidural hydromorphone remains unknown. Hence, we evaluated and compared the analgesic and adverse effects of postoperative different doses of epidural hydromorphone coadministered with ropivacaine after cesarean section. Methods. Eighty term parturients with elective cesarean section under epidural anesthesia were allocated into four groups. Epidural analgesia was administered with an epidural bolus of either 0mg (group H0), or 0.2mg (group H1), or 0.4mg (group H2), or 0.6mg (group H3) hydromorphone coadministered with ropivacaine. The primary outcome was the visual analogue pain scores (VAPSs) and rescue opioid consumption (PCIA with sulfentanil) in 24hours. Adverse effects such as respiratory depression, pruritus, nausea, and vomiting were recorded. Results. The VAPSs of group H1 at 2, 4, 6, 12h and 24h after surgery was similar to group H0. The VAPSs of group H2 at 4 and 6h postoperatively were significantly decreased when compared to group H0. But, the VAPSs of group H2 at 2, 12, and 24h postoperatively were similar to those of group H0. The VAPSs of group H3 at 4, 6, 12h, and 24h after surgery were significantly decreased when compared to those of group H0. The total sulfentanil consumption in 24hours was 90 +/- 26g in group H0, 75 +/- 29g in group H1, 54 +/- 32g in group H2, and 15 +/- 16g in group H0. Adverse effects were comparable in the four groups. Conclusions. Epidural administration of 0.6mg hydromorphone coadministered with ropivacaine after cesarean section provided satisfactory pain relief with less sulfentanil consumption. This trial is registered with ChiCTR-IPR-16010026.

Abstract:Objective To access the application of Ropivacaine for combined spinal\|edidural anesthesia in labor analgesia by investigate the influence on labor progress, degree of analgesia and the times of analgesia.Methods 100 healthy nulliparous parturients in spontaneous labor with ASA Ⅰ-Ⅱ, singleton, full\|term fetuses were enrolled and received combined spinal\|epidural labor analgesia then their cervical dilation was in 3～4cm. The patients were randomly assigned in Group B(bupivacaine) and Group R(ropivacaine). The spinal drugs of Group R were 3.75mg ropivacaine +25μg fentanyl and the spinal drugs of Group B were 2.5mg bupivacaine +25μg fentanyl, the epidural drug of Group R is 0.1% ropivacaine include 1μg/ml fentanyl and Group B is 0.0625% bupivacaine include 1μg/ml fentanyl. Data were collected on the time of onset and perfect of analgesia, the degree of motor nerve block, the degree of analgesia, degree of comfort,labor progress and mode of delivery.Results The whole time of analgesia of Group B(\$P\$<0.05), the perfect time of analgesia of Group R is later than Group B(\$P\$<0.05), the degree of comfort of Group R is higher than Group B(\$P\$<0.01).Conclusion Ropivacaine may be applied for combined spinal\|epidural anesthesia in labor analgesia and is superior to bupivacaine.

Abstract:(Reg Anesth Pain Med. 2018;43(8):885–889) Spinal-induced hypotension is a common problem resulting from the sympathectomy associated with the administration of intrathecal local anesthetic for cesarean delivery, and is associated with nausea, vomiting, loss of consciousness, impaired fetal oxygenation, and fetal acidosis. Prophylactic phenylephrine infusion is often used to decrease the risk of spinal-induced hypotension; however, its use has been associated with decreased rostral spread of intrathecal local anesthetic in pregnancy. The purpose of this study was to determine the median effective dose (ED50) and 95% effective dose (ED95) of spinal hyperbaric bupivacaine for cesarean delivery with and without a phenylephrine infusion.
ED50 and ED95 of Intrathecal Bupivacaine Coadministered with Sufentanil for Cesarean Delivery under Combined Spinal-epidural in Severely Preeclamptic Patients

Abstract:Background: Spinal anesthesia was considered as a reasonable anesthetic option in severe preeclampsia when cesarean delivery is indicated, and there is no indwelling epidural catheter or contraindication to spinal anesthesia. However, the ideal dose of intrathecal bupivacaine has not been quantified for cesarean delivery for severe preeclamptic patients. This study aimed to determine the ED50 and ED95 of intrathecal bupivacaine for severely preeclamptic patients undergoing elective cesarean delivery. Methods: Two hundred severely preeclamptic patients are undergoing elective cesarean delivery under combined spinal-epidural anesthesia enrolled in this randomized, double-blinded, dose-ranging study. Patients received 4 mg, 6 mg, 8 mg, or 10 mg intrathecal hyperbaric bupivacaine with 2.5 &mgr;g sufentanil. Successful spinal anesthesia was defined as a T6 sensory level achieved within 10 minutes after intrathecal drug administration and/or no epidural supplement was required during the cesarean section. The ED50 and ED95 were calculated with a logistic regression model. Results: ED50 and ED95 of intrathecal bupivacaine for successful spinal anesthesia were 5.67 mg (95% confidence interval [CI]: 5.20–6.10 mg) and 8.82 mg (95% CI: 8.14–9.87 mg) respectively. The incidence of hypotension in Group 8 mg and Group 10 mg was higher than that in Group 4 mg and Group 6 mg (P < 0.05). The sensory block was significantly different among groups 10 minutes after intrathecal injection (P < 0.05). The use of lidocaine in Group 4 mg was higher than that in other groups (P < 0.05). The use of phenylephrine in Group 8 mg and Group 10 mg was higher than that in the other two groups (P < 0.05). The lowest systolic blood pressure before the infant delivery of Group 8 mg and Group 10 mg was lower than the other two groups (P < 0.05). The satisfaction of muscle relaxation in Group 4 mg was lower than other groups (P < 0.05). There was no significant difference in patients’ satisfaction and the newborns’ Apgar score and the blood gas analysis of umbilical artery serum (P > 0.05). Conclusion: Our study showed that the ED50 and ED95 of intrathecal bupivacaine for severely preeclamptic patients undergoing elective cesarean delivery were 5.67 mg and 8.82 mg, respectively. In addition, decreasing the dose of intrathecal bupivacaine could reduce the incidence of maternal hypotension.
Comparison of Dural Puncture Epidural, Epidural and Combined Spinal-Epidural Anesthesia for Cesarean Delivery: A Randomized Controlled Trial.

Abstract:Purpose: Catheter-based techniques such as combined spinal-epidural (CSE) anesthesia which are sometimes indicated for obstetric anesthesia have a complex mechanism of action. The application of the dural puncture epidural (DPE) anesthesia for cesarean section (CS) has not been well investigated. The present study compared the relatively novel DPE technique with epidural (EA) and CSE anesthesia.Patients and Methods: We randomly assigned 150 parturients who underwent elective CS to receive DPE, EA or CSE anesthesia. The primary outcome was the onset of sensory anesthesia to the T5 dermatome assessed using the Cox proportional hazards model. Secondary outcomes included median time to sensory block, quality of block, patient and surgeon satisfaction, APGAR scores and other side effects.Results: For DPE anesthesia versus EA anesthesia, the onset of anesthesia was faster (hazard ratio 2.47 [95% CI 1.56 to 3.90], adjusted P < 0.001) and the median time to surgical level was shorter (16 [IQR 14-18] min versus 19 [15.5-21] min, adjusted P < 0.001); the incidence of intraoperative pain was lower (7/48 versus 17/47, adjusted P = 0.046) and the median patient satisfaction score was higher (9 [IQR 9-10] versus 8 [8-9.5], adjusted P = 0.004). In the CSE group, the onset of anesthesia was faster than in the other two but the incidence of hypotension was higher (P < 0.001) and the phenylephrine requirement was greater (P < 0.001).Conclusion: DPE anesthesia had a faster onset and better quality of block than EA anesthesia and provided less influence to maternal hemodynamic parameters than CSE anesthesia for CS. These results suggest that the dural puncture plays a significant role in enhancing the effectiveness of epidural top-ups during CSE anesthesia and indicates enlightenment that contributes to the satisfaction of anesthetic effect in DPE technique labor analgesia transferred to CS.
Impact of ropivacaine combined spinal epidural anesthesia on hemodynamic and anesthesia quality in cesarean section pregnant women

Abstract:Objective To study the effect of small doses ropivacaine combined spinal epidural anesthesia on hemodynamic and anesthesia quality in cesarean section pregnant women.Methods A total of 132 cases pregnant women with cesarean section were divided into experimen-tal group and control group, all patients given L2, L3 lumbar line com-bined with spinal epidural anesthesia. Experimental group were given 0.75% ropivacaine 1 mL +2 mL water injection; control group given 0.75%ropivacaine 2 mL +1 mL water injection.Heart rate ( HR ) , mean artery pressure ( MAP ) , oxygen saturation ( SpO2 ) , anesthesia plane fixed time, anesthesia time, muscle relaxant satisfaction, adverse reactions were compared between the two groups.Results Before anes-thesia, 15 min after anesthesia and operation completed, there were no statistical significance of HR, MAP, SpO2 between the two groups (P>0.05).After fetal childbirth, HR (89.3 ±9.7) times? min-1 in control group was significantly faster, MAP (97.0 ±10.3) mmHg and SpO2(95.1 ±0.7)%significantly decreased (P<0.05).There were no statistical significance of anesthesia plane, anesthesia time, muscle relaxant between the two groups(P>0.05);mater-nal anesthesia awakening time (19.27 ±3.19) min and automatic micturition time (18.42 ±2.12) h in experimental group were shorter than those of control group ( P <0.05 ) .There was no statistical significance of neonatal 1 min Apgar score between the two groups ( P >0.05 ) .The incidence rate of adverse reactions in experimental group (7.58%) was significantly lower than that in control group(28.79%, P<0.05).Conclusion Small doses ropiva-caine combined with spinal epidural anesthesia helped hemodynamic of pregnant women to be relative stability, and could improve the quality of anesthesia.
Effect of Ephedrine Combined with Bupivacaine on Maternal Hemodynamic and Spinal Nerve Block in Cesarean Delivery

Abstract:The aim of the study was to investigate the effects of ephedrine intrathecal injection on maternal thermodynamics and spinal nerve block in cesarean section. A total of 107 patients undergoing elective cesarean section under combined spinal epidural anesthesia were randomly divided into two groups: group E: Bupivacaine (6 mg) combined with ephedrine (15 mg) and group C: Bupivacaine (6 mg). Systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), the incidence of hypotension, hypertension, tachycardia, nausea and vomiting were all recorded. The umbilical vein blood gases and noradrenaline, as well as maternal peripheral venous blood were analyzed. The time of high-quality spinal block was also recorded. Postoperatively, 1 day, 2 days bedside follow-up and 7, 21, 35 days telephone follow-up were carried out to determine any neurological deficit maternal performance. SBP and DBP decreased in group C but not in group E at 3 min after spinal anesthesia and 1 min after delivery (P<0.05). From 3 min post-anesthesia to the end of surgery, the HR levels in the C group were significantly higher than those of the E group (P<0.05). The incidence of hypotension, tachycardia, nausea and vomiting and neonate acidosis was low in group E compared with group C (P<0.05). The time of high-quality spinal block in group E was longer than that of group C (P<0.05). After spinal anesthesia, the concentration of noradrenaline from maternal peripheral venous blood was low in group C but high in group E compared with before spinal anesthesia (P<0.05). Ephedrine combined with low-dose bupivacaine intrathecal injection effectively maintained maternal hemodynamics and prolonged the time of sensory spinal block.

Abstract:Objective: To investigate the safety and effect of ropivacaine,bupivacaine and teracaine in caesarean section in combined spinal and epidural anesthesia.Methods: Ninety parturients were randomly divided into three groups.Made high density mixture from 0.89% ropivacaine(group A),0.75% bupivacaine(group B) and 1% teracaine(group C) 1 ml,respectively puls,10% glucose 1 ml and 3% ephedrine 1 ml respectively.In all parturients,a Tuohy needle(B.D) was introduced into the L2-3 epidural space,and 2~3 ml medicine was injected into subarachnoid space in 0.1 ml/s.Parturients received extradural injection 0.89% ropivacaine or 2% lidocaine 5~10 ml to maintain block level.To compare difference in sensory block,motor block and recovery time in three groups.Side-effects and effect on neonate score were observed.Results: The onset time of maximum sensory block and motor block in group A was prolonged compared with group B and group C(P0.05);There were no significantly difference in maximum sensory block level in three groups(P0.05);Effect time in group A was shorter than group B and group C(P0.05); Onset time of motor block in group A was prolonged compared with group B and group C(P0.05),maximum Bromage score was 0~1 in group A,significantly smaller(P0.05),time of maximum motor block was prolonged(P0.05),time of motor motor block disappear was shorter(P0.05);The incidence of hypotension,nausea vomiting in group A was lower(P0.05);Neonatal Apgar score and incidence of postoperation headache had no significantly difference(P0.05).Conclusion: Ropivacaine is safety and high effective in caesarean section in combined spinal and epidural anesthesia for parturient and neonatal.

Abstract:Objective To investigate the effects of ropivacaine and bupivacaine on hemodynamic in patients undergoing cesarean section with spinal-epidural anesthesia.Methods A total of 98 pregnant women undergoing elective cesarean section were randomly divided into treatment group (49 cases) and control group (49 cases).The treatment group was received 0.75% ropivacaine 1.5 mL.Control group was received 0.75%bupivacaine 1.5 mL.The hemodynamic level, the effect of anes-thesia and the incidence of adverse drug reactions were compared between the two groups.Results Mean arterial pressure and systolic blood pres-sure in the treatment group were significantly higher than those in control group at 5 and 10 min after anesthesia ( P <0.05 ).In the treatment group, the sensory level and motor block were significantly longer than those of control group ( P<0.05).The time of sensory block and motor block in the treatment group were significantly less than those in control group ( P<0.05 ).There was no significant difference in the incidence of adverse reactions between the two groups ( P >0.05 ) .Conclusion Ropivacaine and bupivacaine for caesarean section are safe and effective, but ropivacaine has less hemodynamic disturbances and short duration of sensory level block and motor block, and therefore more contribute to recovery.

Abstract:Abstract Background To compare the application and efficacy of ropivacaine combined with sufentanil for continuous epidural anesthesia (CEA) and combined spinal-epidural anesthesia (CSEA) in labor analgesia. Methods Three hundred sixty pregnant women requesting labor analgesia from October 2017 to August 2018 were selected retrospectively. According to the anesthetic method, subjects were divided into CSEA group and CEA group. Ropivacaine combined with sufentanil were used in all subjects. The labor time, visual analogue scale (VAS), Apgar score of newborn, adverse pregnancy outcomes and adverse drug reactions were observed. Results There was no significant difference in pre-analgesia (T 0 ) VAS scores between the two groups ( P > 0.05). VAS scores of first stage of labor (T 1 ), second stage of labor (T 2 ) and third stage of labor (T 3 ) in CSEA group were significantly lower than CEA group ( P < 0.01). The onset time, T 1 and total labor time in CSEA group were significantly shorter than CEA group ( P < 0.01). There were no significant differences between T2 and T3 ( P > 0.05). There were no significant differences in adverse pregnancy outcomes and Apgar scores at 1, 5 and 10 min after birth between the two groups ( P > 0.05). The incidence of adverse drug outcomes in CSEA group was significantly lower than CEA group ( P < 0.01). Maternal satisfaction in CSEA group was significantly higher than CEA group ( P < 0.01). Conclusion Considering ropivacaine combined with sufentanil for CSEA achieved a shorter onset time and labor period, significant analgesic effect, lower adverse drug reactions rates and higher subject satisfaction than CEA, it may be worthy of clinical promotion and application.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Study objective</h3><p>It is controversial whether local anesthetic dose requirement for spinal anesthesia for cesarean delivery differs between patients with singleton and patients with multiple gestation pregnancies. The aim of this study was to determine and compare the ED<sub>50</sub> and ED<sub>90</sub> for hyperbaric ropivacaine used for spinal anesthesia for cesarean delivery in patients with singleton pregnancies versus patients with twin pregnancies.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>Prospective, randomized, comparative dose-finding study.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Setting</h3><p>Operating room, Women's Hospital, Zhejiang University School of Medicine.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Patients</h3><p>100 patients with singleton pregnancies (Group S) and 100 patients with twin pregnancies (Group T) presenting for scheduled cesarean delivery under combined spinal-epidural anesthesia were enrolled in the study.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Interventions</h3><p>Patients in Group S or Group T were randomly allocated to receive 9.5, 11, 12.5, 14 or 15.5 mg of hyperbaric ropivacaine intrathecally. A dose was considered effective when it achieved a bilateral sensory block level at the T6 dermatome or above within 10 min after intrathecal injection, there was no numerical rating scale (NRS) pain score ≥ 3 intraoperatively, and there was no requirement for epidural supplementation at any time during anesthesia and operation. Values for ED<sub>50</sub> and ED<sub>90</sub> for ropivacaine were determined using probit regression. The difference in ropivacaine dose requirement between patients with singleton pregnancies and patients with twin pregnancies was assessed by calculating relative median potency.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Measurements</h3><p>Success rates for different intrathecal doses of ropivacaine, side effects and neonatal outcomes were recorded.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Main results</h3><p>The estimated (95% confidence interval) values for ED<sub>50</sub> and ED<sub>90</sub> of intrathecal ropivacaine in patients with singleton pregnancies were 11.2 (10.2 to 12.0) mg and 15.7 (14.4 to 18.3) mg, respectively. The values for ED<sub>50</sub> and ED<sub>90</sub> in patients with twin pregnancies were 10.5 (9.5 to 11.3) mg and 14.8 mg (13.6 to 17.0) mg, respectively. The estimate of relative median potency for ropivacaine between patients with singleton and twin pregnancies was 0.94 (95% confidence interval 0.83 to 1.04).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Patients with singleton and twin pregnancies have similar dose requirement for hyperbaric ropivacaine used for spinal anesthesia for cesarean delivery in the setting of combined spinal-epidural (CSE) anesthesia, no opioids, low weight cohort, insertion with the patients in the right lateral position, and norepinephrine boluses.</p>

Abstract:Objective: To explore the clinical effect and safety of different concentration of ropivacaine in sufentanil combined with ropivacaine in postoperative analgesia after cesarean section.Methods: 1460 patients for cesarean section were randomly divided into three groups including Group A(n=489),Group B(n=501) and Group C(n=470).All the patients were performed by combined spinal-epidural anesthesia(CSEA),and given 0.1% ropivacaine combined with 0.3μg/mL,0.4μg/mL,0.5μg/mL sufentanil separately.Results: The VAS scores after cesarean section for 2h,8h,12h in Group B and Group C were significantly lower than that in Group A(P<0.05),while there was no significantly difference between B group and C group(P>0.05).The VAS scores after cesarean section for 24 h among three groups were no statistically different(P<0.05).Compared with Group A,the effective rate of PECA bump was significantly increased in Group B and Group C(P<0.05),while there was no significantly difference between B group and C group(P>0.05).The rate of side effect in Group A,Group B(8.0%,10.2%) was significantly lower than that in C group(20.2%)(P<0.05).Conclusions: The concentration of 0.4μg/mL sufentanil +0.1% ropivacaine in postoperative analgesia after cesarean section can contribute to relief early pain after operation and can reduce the side effect.

Abstract:Objective To explore the clinical effect of three concentrations of ropivacaine combined with butophanol on epidural anesthesia for cesarean section,and identify the optimal concentration.Methods 1620 patients treated with cesarean section were randomly divided into three groups,Group A(n=542),Group B(n=530) and Group C(n=548).All the patients were performed by epidural anesthesia,and given 0.004% butophanol combined with 0.10%,0.15%,0.20% ropivacaine respectively.Results The onset time of sense block,pain disappeared time in Group B and Group C were significantly shorter than that of Group A(P<0.05),while there was not significantly difference between B group and C group(P>0.05).Compared with Group A and Group B,the onset time,duration and degree of motor block in Group C was significantly longer,while there was not significantly difference between Group A and Group B(P>0.05).The rate of side effect including nausea and vomiting,sleepiness and itchy skin in Group A and Group B was significantly lower than that of Group C(P<0.05).Conclusion The concentration of 0.15% ropivacaine+0.004% butophanol in epidural anesthesia for cesarean section can provide good effect of anesthesia and analgesia,and reduce the side effect.
Association Between Plain Ropivacaine Dose and Spinal Hypotension for Cesarean Delivery: a Retrospective Study

Abstract:Background Data on the association between the plain ropivacaine dose and maternal hypotension during cesarean delivery are limited. Thus, this study aimed to explore this association. Methods This retrospective study included patients undergoing cesarean sections under spinal or combined spinal-epidural anesthesia with plain ropivacaine at The First Hospital of Fuyang, Hangzhou, China, between 2018 and 2022. Data were obtained from the anesthesia information management system. Liner trend tests were used to distinguish the linear relationship between spinal hypotension and the plain ropivacaine dose, and receiver operating characteristic curves were used to calculate the dose threshold. Logistic regression was used to adjust for confounders. Sensitivity analyses were performed to evaluate the stability of the results. The secondary outcome was vasopressor use (metaraminol and ephedrine). Results In total, 1,219 women were included. The incidence of hypotension linearly correlated with the plain ropivacaine dose (adjusted P-value for trend, P < 0.001). Thus, we used a dose threshold of 17.5 mg to compare the dose as a binary variable (≥17.5 mg vs. < 17.5 mg). Plain ropivacaine doses of ≥17.5 mg were associated with a higher incidence of spinal hypotension (adjusted odds ratio: 2.71; 95% confidence interval [1.85–3.95]; P < 0.001). The sensitivity analyses yielded similar results. The plain ropivacaine dose also correlated with metaraminol use but not ephedrine use. Conclusions The incidence of spinal-induced hypotension in women undergoing cesarean section linearly correlated with the plain ropivacaine dose. The dose threshold for hypotension risk was 17.5 mg.

Abstract:Objective: The optimal dose of epidural morphine after cesarean section (CS) still remains unknown when combined with low-concentration ropivacaine based on a continuous basal infusion (CBI) mode. The aim of this study was to assess the impact of different dose of epidural morphine plus ropivacaine on maternal outcomes.Materials and methods: Data of parturients who received epidural analgesia for CS at a teaching hospital from March 2021 to June 2022 were retrospectively collected. Parturients were divided into two groups (RM3 group and RM6 group) according to different medication regimens of morphine. The implementation of epidural analgesia was performed with 3 mg morphine in RM3 group and 6 mg morphine in RM6 group in combination with 0.1% ropivacaine via a CBI pump. The primary outcomes included pain intensity at rest and movement and the incidence of urinary retention and pruritus within postoperative 48 h. The secondary outcomes included the incidence and severity of postoperative nausea and vomiting (PONV) and pruritus, the rate of rescue analgesia and grading of motor Block.Results: Totally, 531 parturients were eligible for the final analysis, with 428 and 103 parturients in the RM3 group and RM6 group, respectively. There were no statistically significant differences in the visual analogue scores (VAS) at rest and movement within postoperative 48 h between the two groups (all p > 0.05). Compared with the RM6 group, the incidence of urinary retention was lower in the RM3 group within 48 h after CS (4.0% vs. 8.7%, p = 0.044). No significant difference was found in the incidence and severity of PONV and pruritus, the rate of rescue analgesia and grading of motor block between RM3 and RM6 groups.Conclusion: Epidural 3 mg morphine plus 0.1% ropivacaine in a CBI mode can provide equal efficacy and have lower incidence of urinary retention compared with 6 mg morphine after CS.
The application of spinal anesthesia via 24G needle combined with single injection of epidural morphine analgesia for cesarean section

Abstract:Objective To investigate the effects and safety of combining spinal anesthesia with single injection of epidural morphine analgesia via 24G spinal needle for cesarean section. Methods Sixty puerperae scheduled for cesarean section were included in this investigation. They were randomly assigned to the combined block group (CBG) and the single block group (SBG), 30 puerperae in each. The routine intrathecal puncture point was at L3—L4 interval via 24G spinal needle in two groups. Firstly 0.5% hyperbaric bupivacaine (8 mg) was injected cephalad when the needle tip reached the subarachnoid space and the cerebrospinal fluid outflew clearly, then the needle was drawn back slowly into the epidural space carefully without cerebrospinal fluid or blood; secondly 10 mL normal saline with 1.5 mg morphine was injected into the epidural space in CBG while 10 mL normal saline was injected into SBG. The routine monitoring including noninvasive blood pressure, heart rate and oxygen saturation (SpO2) was available. The indices were recorded and compared, including intraoperative hypotension, nausea and vomiting, pruritus, hypoxemia, the application of vasopressor and intravenous anesthetics. The postoperative adverse reactions, including headache, nausea and vomiting, pruritus, urinary retention and low back pain, and the visual analog scale (VAS) score of postoperative pain 6, 12, 24 h after procedure, were recorded and compared. The VAS score of the satisfaction with the effect of anesthesia was recorded. The 5 min Apgar score of neonates was recorded. Results No puerperae of both groups sufferred hypoxemia or pruritus during the procedure without application of the intravenous anesthetic. There were no difference in the prevalence of intraoperative hypotension, vasopressor application, nausea and vomiting between groups (all P>0.05). Both the constituent ratio of VAS score ≤3 of postoperative pain 6, 12, 24 h after procedure and the constituent ratio of VAS score ≥8 of puerperae’ satisfaction in CBG were higher than that in SBG (all P<0.05). The prevalence of the postoperative complications had no significant difference between groups (all P>0.05). All neonates′ 5 min Apgar score were higher than 8, showing no difference between groups (P<0.05). Conclusion Combination of the spinal anesthesia with single injection of epidural morphine analgesia via 24G spinal needle is safe and effective for the puerperae undergoing cesarean section.

Abstract:Objective. The objective is to evaluate the analgesic, labor, and prognostic effects of patient-controlled epidural analgesia (PCEA) versus sufentanil in conjunction with ropivacaine in normal labor. Methods. Sixty pregnant women who had a normal delivery at our hospital between February 2019 and April 2021 were included. Pregnant women were arbitrarily assigned to a control group and a research group. Pregnant women in the control group received lidocaine analgesia and PCEA with sufentanil combined with ropivacaine in the research group. Satisfaction with care, fetal umbilical artery blood flow, VAS score, labor and bleeding, neonatal Apgar score and incidence of adverse events were analyzed. Results. First, we made a comparison of satisfactory performance of nursing care. The satisfaction rate of the research group was 100.00%, compared to 83.33% for the control group. Nursing satisfaction was higher in the research group, and the difference was statistically significant ( P < 0.05 ). Following analgesia, PI, RI, and S/D values of umbilical artery blood flow were lower in the research group than those in the control group, but the difference was not statistically significant ( P > 0.05 ). The VAS scores at 10 min, 20 min, and 30 min were found to be lower in the research group than in the control group after analgesia, and the difference was statistically significant ( P < 0.05 ). Bleeding was significantly lower in the research group for all stages of labor, and the difference was statistically significant ( P < 0.05 ). Apgar scores at 1 minute, 5 minutes, and 10 minutes postpartum were greater in the research group than in the control group, and the difference was statistically significant ( P < 0.05 ). As a final note, the incidence of pruritus, hypotension, respiratory depression, nausea, and vomiting was found to be lower in the research group than in the control group, and the difference was statistically significant ( P < 0.05 ). Conclusion. PCEA with sufentanil coupled with ropivacaine was used to perform labor analgesia. With significant reduction in maternal pain and assurance of labor, ropivacaine combined with sufentanil epidural labor analgesia did not reduce fetal umbilical artery blood flow without extended labor. It could not affect the labor process or the safety of the fetus, which is safe for the mother and fetus.

Abstract:<span><b>Background:</b> Spinal anesthesia is the most common technique used for cesarean delivery due to some advantages compared to regional anesthesia. It is easily performed and provides a rapid onset of block. Though bupivacaine is a generally used long-acting amide type local anesthetic drug for spinal anesthesia, ropivacaine may sometimes be selected. <b>Objectives:</b> This prospective, randomized, double-blinded study was aimed at comparing clinical efficacy and safety between ropivacaine and bupivacaine during cesarean section. <b>Methods:</b> After getting ethical committee approval and written informed consent, 65 women who referred to Imam Khomeini Hospital of Ahvaz, Iran in 2018 were chosen for elective cesarean delivery under spinal anesthesia. They were randomly allocated to receive either ropivacaine 1% (n = 33) or bupivacaine 0.5% (n = 32). Afterwards, the differences in the anesthetic efficacy, vital signs, and hemodynamics of participants between the two groups were recorded. <b>Results:</b> Duration of sensory block was shorter in the ropivacaine group than bupivacaine group (132.5 ± 21.6 min vs. 175.8 ± 26.2 min; P < 0.001). Ropivacaine also produced a shorter duration of motor blockade than bupivacaine (124.8 ± 20.2 min vs. 168.2 ± 21.7 min; P < 0.001). There is no difference between the two groups in terms of systolic and diastolic blood pressure, but the heart rate of patients in the bupivacaine group is significantly higher than the ropivacaine group. <b>Conclusions:</b> The results suggest that ropivacaine and bupivacaine are two efficient drugs in anesthesia in the cesarean section, ropivacaine is a better choice due to little influence on the hemodynamics and shorter duration of sensory block and motor block which are useful for the recovery and also safe to the patients.
